Giant lipomas of the hand, case series

ABSTRACT

Introduction: Lipomas are sporadic in hand and represent less than 5% of soft tissue tumors in this topography. Giant lipomas, even less common, are defined as having a diameter greater than 5 cm.
Methods: We performed a retrospective analysis of the cases of patients with giant lipomas of the hand seen in the period 2017-2020, at Hospital Pasteur and Hospital de Clínicas, in Montevideo, Uruguay.
Results: We obtained four patients during the study period. The age of presentation was between 51 and 62 years (mean 57 years). Three lipomas were deep intramuscular, and one was superficial to the regional muscles. The largest had a longer axle of 70mm. All were symptomatic. Marginal resection was performed in all of them, and the postoperative period was uneventful or recurrent. Histopathology confirmed typical lipoma in three cases and lipoma with bone metaplasia in the other.
Conclusion: Giant lipomas of the hand present as painless, slow-growing tumors that may be symptomatic when compressing adjacent structures. Magnetic resonance imaging is the imaging study of choice for these cases. The applicable treatment is marginal surgical resection, and recurrence is uncommon.

RESUMO

Introdução: Os lipomas são esporádicos na mão e representam menos de 5% dos tumores de partes moles nesta topografia. Os lipomas gigantes, ainda menos comuns, são definidos por terem um diâmetro maior do que 5cm.
Métodos: Executamos uma análise retrospectiva dos casos de pacientes com lipomas gigantes de mão, atendidos no período de 2017-2020, no Hospital Pasteur e Hospital de Clínicas, em Montevidéu, Uruguai.
Resultados: Obtivemos quatro pacientes no período estudado. A idade de apresentação foi entre 51 e 62 anos (média de 57 anos). Três dos lipomas eram intramusculares profundos e um era superficial aos músculos regionais. O maior possuía eixo mais extenso de 70mm. Todos eram sintomáticos. A ressecção marginal foi realizada em todos eles, e o pós-operatório transcorreu sem intercorrências ou recorrências. A histopatologia confirmou lipoma típico em três casos, e lipoma com metaplasia óssea no outro.
Conclusão: Lipomas gigantes de mão se apresentam como tumores indolores e de crescimento lento, que podem ser sintomáticos ao comprimir estruturas adjacentes. A ressonância magnética é o estudo de imagem escolhido para estes casos. O tratamento aplicável é a ressecção cirúrgica marginal e a recorrência é pouco comum.

Complex dorsal dislocation of the metacarpophalangeal joint of the long fingers: case report and literature review

ABSTRACT

Dorsal dislocations of the metacarpophalangeal joint of the fingers are rare injuries that are seen more frequently in young patients secondary to trauma due to forced hyperextension of the finger on the extending hand. They are classified as simple when closed reduction is possible, or complex when reduction by closed methods is not possible given the interposition of peri-articular structures. It is important to distinguish between a simple and complex dislocation because their approach and treatment differ. The objective of this study is to update the clinical approach and the different surgical techniques used in the treatment of complex dislocations. We conducted a bibliographic review on metacarpophalangeal dorsal dislocation of the long fingers, excluding those of the thumb, including the Medline (PubMed interface), SciELO and academic google databases. All the articles reviewed conclude that attempts at closed reduction in these types of injuries are often unsuccessful and often lead to additional complications. Open surgical reduction is the method of choice, allowing joint anatomical recovery with the lowest risk of complications. Postoperative immobilization using a dorsal locking splint is recommended for two weeks followed by rehabilitation by occupational therapy, expecting a normal range of motion at six weeks. The low frequency added to the ignorance of the emergency physician when performing the reduction maneuver can often lead to transform a simple dislocation into a complex one, making it irreducible and injuring adjacent structures, which is why we believe it is essential to know the management of this injury.

RESUMO

As luxações dorsais da articulação metacarpofalangeana dos dedos são lesões raras, vistas com mais frequência em pacientes jovens, secundárias a trauma por hiperextensão forçada do dedo na mão que estende. São classificadas como simples, quando a redução fechada é possível; ou complexas, quando a redução por métodos fechados não é possível devido à interposição de estruturas periarticulares. É importante distinguir entre uma luxação simples e uma complexa porque sua abordagem e tratamento são diferentes. O objetivo deste estudo é atualizar a abordagem clínica e as diferentes técnicas cirúrgicas utilizadas no tratamento das luxações complexas. Foi realizada uma revisão bibliográfica sobre a luxação dorsal metacarpofalangeana dos dedos longos, excluindo os do polegar, incluindo o Medline (interface PubMed), SciELO e bancos de dados acadêmicos do Google. Todos os artigos revisados concluem que as tentativas de redução incruenta nesses tipos de lesões costumam ser malsucedidas e levar a complicações adicionais. A redução cirúrgica aberta é o método de escolha, permitindo a recuperação anatômica articular com o menor risco de complicações. A imobilização pós-operatória com uma tala de travamento dorsal é recomendada por duas semanas, seguida por reabilitação por terapia ocupacional, esperando-se uma amplitude de movimento normal em seis semanas. A baixa frequência somada ao desconhecimento do médico emergencista ao realizar a manobra de redução pode, muitas vezes, levar à transformação de um simples deslocamento em complexo, tornando-o irredutível e lesionando estruturas adjacentes, por isso, acreditamos ser fundamental conhecer o manejo desta lesão.

Cannulated compression screws without intramedullary heads for treatment of metacarpal fractures: experience at University Hospital

ABSTRACT

Introduction: Metacarpal fractures have a high incidence in young males. Most of them can be treated non-surgically; Recently, fixation with intramedullary headless compressive screw has been achieved, with good results.
Objective: Describing the experience in treatment of metacarpal fractures with intramedullary headless compressive screws. Descriptive, retrospective study, conducted at the University Hospital, Montevideo, Uruguay, from July 2017 to July 2018, including patients with metacarpal fractures, fixed by intramedullary headless compressive screws. Surgical technique, postoperative and follow-up are presented; as well as evaluating functional result, complications and return to work. Results were classified as good in five cases and regular in one.
Conclusions: This fixation seems to be a good alternative for short or transverse oblique fractures, providing adequate stability and allowing early mobilization.

RESUMO

Introdução: As fraturas do metacarpo apresentam alta incidência em jovens do sexo masculino. Na maior parte, elas podem ser tratadas não cirurgicamente. Recentemente surgiu a fixação com parafusos sem cabeça canulados intramedulares de compressão, com bons resultados.
Objetivo: Descrever a experiência no tratamento de fraturas do metacarpo com parafusos sem cabeça canulados intramedulares de compressão. Estudo descritivo, retrospectivo, realizado no Hospital Universitário de Montevidéu, Uruguai, de julho de 2017 a julho de 2018, incluindo pacientes com fraturas do metacarpo, fixados com parafusos canulados sem cabeça intramedulares de compressão. Apresenta-se técnica cirúrgica, pós-operatório e acompanhamento; avaliação do resultado funcional, complicações e tempo de reintegração ao trabalho. Os resultados foram classificados como bons em cinco casos e regulares em um.
Conclusão: Esse método de fixação parece ser uma boa alternativa para traços oblíquos curtos ou transversos, fornecendo estabilidade adequada e permitindo mobilização precoce.
