Allograft use at extensive wound treatment - case report

ABSTRACT

Chronic wounds come from a retardation or wound healing inefficiency, traumatic or from basal diseases. The allograft, a possibility of treatment of major burns and extensive dermal wounds, allows a faster recovery of damaged areas favoring autograft. Studies point to a shorter hospitalization time, with less complications and death with allograft alone. In addition, the use of allograft reduces the epithelization time of injured areas, such as traumatic injuries and burns, up to 40%. This is due to the replacement of the allograft keratinocytes, the migrants from the edges or deep areas of the wound. Because of the high efficiency associated with easy use and storage, the customary use of allograft can be applied at a low cost and risk to the patient, even in outpatient treatment.

RESUMO

Feridas crônicas surgem de um retardo ou ineficiência cicatricial, seja trauma ou doenças de base. O aloenxerto, uma possibilidade de tratamento de grandes queimados e lesões dérmicas de grande extensão, viabiliza uma recuperação mais rápida de áreas lesadas favorecendo a autoenxertia. Estudos apontam para um menor tempo de internação, complicações e morte com uso da aloenxertia, isoladamente. Além disso, o uso de aloenxertia reduz em até 40% o tempo de epitelização de áreas lesadas, como ferimentos traumaticos traumáticos e queimaduras. Isso se dá pela substituição dos queratinócitos do aloenxerto, pelos migrantes dos bordos ou áreas profundas da ferida. Devido a à alta eficácia associado associada a à facilidade de uso e estocagem, o uso rotineiro da aloenxertia pode ser empregado a um baixo custo e risco ao paciente, mesmo em tratamento ambulatorial.

Epidemiological characterization of burn patients treated at the Burn Unit of the University Mackenzie Evangelical Hospital in Curitiba in the year 2017

ABSTRACT

The objective of this study was to delineate epidemiological aspects related to patients treated in the burn sector of the Evangelical Mackenzie University Hospital, admitted in 2017.
Method: Data was obtained from the registry of patients entering the burn sector with 4.398 patients. Patients were analyzed by gender, age, causes of the burns and months of greater occurrence.
Results: A prevalence of care was observed in female patients. Of all visits, 2.361 (53.8%) were female and 2.031 (46.2%) were male patients that entered the service. A total of 391 (8.89%) hospitalizations were necessary among children and adults. Regarding the etiological factors, it was possible to observe a greater number of burns caused by scalds.
Conclusions: The study showed a correspondence of the profile of burns when compared to other services, emphasizing the importance of greater population education for the prevention of burns, mainly in order to combat negligence with children.

RESUMO

O objetivo desse estudo foi delinear aspectos epidemiológicos relativos aos pacientes atendidos no setor de queimados do Hospital Universitário Evangélico Mackenzie, admitidos em 2017.
Método: Foram avaliados dados obtidos do registro de entrada de pacientes no setor de queimados, perfazendo um total de 4.398 pacientes. Os pacientes foram analisados quanto ao sexo, idade, causas de queimadura e meses de maior ocorrência.
Resultados: Foi observada uma prevalência de atendimento em pacientes do sexo feminino. Deram entrada no serviço 2.361 (53,8%) pacientes do sexo feminino e 2.031 (46,2%) do sexo masculino. Foram realizados 391 (8,89%) internamentos contemplando crianças e adultos. No que diz respeito aos fatores etiológicos, foi possível observar maior número de queimaduras por escaldaduras.
Conclusões: O estudo demonstrou correspondência do perfil das queimaduras em relação a alguns serviços, ressaltando-se a importância de maior educação populacional para prevenção de queimaduras, principalmente visando combater a negligência infantil.

RESUMO

A amputação traumática do pavilhão auricular, seja total ou subtotal, acarreta sérios problemas estético-funcionais, de difícil manejo para o cirurgião plástico, e apresenta elevada incidência na última década. Relatamos um caso em que, após sofrer traumatismo com amputação por mordedura humana, é realizada reconstrução total do polo superior da orelha, com enxerto de cartilagem costal, em 3 tempos. Paciente evoluiu bem, com enxerto apresentando completa viabilidade no pós-operatório.

ABSTRACT

The oculopalpebral traumas are very common and have great socioeconomic importance, bringing with them unhappiness, economic inefficiency and monetary loss. We describe in this article, the case of a severe facial trauma, resulting from a lacerating wound motivated by self-injury, in which the basic principles of eyelid reconstruction were used. The patient presented good evolution, and a satisfactory aesthetic result with adequate palpebral occlusion. We emphasize the importance of the plastic surgeon as an integral and necessary specialist of the trauma team in the emergency room, as well as their degree of resolubility, using basic and consecrated techniques for eyelid reconstruction.

RESUMO

Os traumas oculopalpebrais são muito comuns e têm grande importância socioeconômica, trazendo consigo infelicidade, ineficiência econômica e perda monetária. Descrevemos neste artigo o caso de um trauma facial grave, decorrente de lesão lacerante por arma branca, motivado por autoagressão, em que os princípios básicos da reconstrução palpebral foram utilizados. O paciente apresentou boa evolução, e resultado estético satisfatório com oclusão palpebral adequada. Ressaltamos a importância do cirurgião plástico como um especialista integrante e necessário da equipe de atendimento do trauma grave na sala de emergência, assim como seu grau de resolutibilidade, utilizandose de técnicas básicas e consagradas de reconstrução palpebral.

Bipedicled fasciocutaneous bilateral flap for treatment of meningomyelocele: case report

ABSTRACT

Myelomeningocele results from congenital defect of the neural tube. It is the most serious form of spinal dysraphism. Its treatment demands a multidisciplinary team, including the plastic surgeon for the repair of major defects with skin flaps, fasciocutaneous flaps, musculocutaneous flaps or skin grafts. We report a case of a full-term newborn with large ruptured myelomeningocele in thoracolumbar segment. The surgery was performed in the first hours of life by a multidisciplinary team. We chose the bilateral bipedicled fasciocutaneous flap to cover the defect and the patient had a good outcome, without postoperative complications.

RESUMO

A mielomeningocele decorre de defeito congênito do tubo neural. É a forma mais grave dos disrafismos espinhais. Seu tratamento é feito por equipe multidisciplinar, incluindo o cirurgião plástico para reparo de grandes defeitos com retalhos cutâneos, fasciocutâneos, musculocutâneos ou enxertos de pele. Relatamos caso referente a recém-nascido de termo com mielomeningocele rota de grandes dimensões em região toracolombar, cujo tratamento foi realizado nas primeiras horas de vida por equipe multidisciplinar. O retalho de cobertura do defeito foi o fasciocutâneo bipediculado bilateral, o qual apresentou evolução satisfatória no pós-operatório, sem complicações.

ABSTRACT

This study intends to report a case of combined use of allograft followed by autograft to cover extensive lesion in a patient with polytrauma evaluated at the Hospital Universitário Evangélico Mackenzie. The aim is to show that, as it has been done for many years in medium and large burned patients, the use of alloenxertia can be useful in evaluating and preparing the bed to receive the patient's own skin graft. This paper describes the treatment and evolution of an extensive and complex lesion of a polytraumatized patient, and what was the role of the allograft in the final outcome of the case.

RESUMO

Esse trabalho tem como intenção relatar um caso de uso combinado de aloenxerto seguido de autoenxerto para cobertura de lesão extensa, em um paciente vítima de politrauma atendido no Hospital Universitário Evangélico Mackenzie. O objetivo é mostrar que, da mesma forma como já vem sendo realizado há muitos anos nos pacientes com médio e grande queimados, o uso de aloenxertia pode ser útil, ao avaliar e preparar o leito para receber o enxerto de pele do próprio paciente. Esse trabalho descreve como foi o tratamento e evolução de uma lesão extensa e complexa, de um paciente politraumatizado, e qual foi o papel do aloenxerto no resultado final do caso.
