Refinement of the Mustardé technique for the treatment of prominent ears

ABSTRACT

INTRODUCTION: Traditionally, multiple abnormalities can cause prominent ears, and specific techniques have been developed for the treatment of each abnormality. In this study, we tested a single approach aimed at achieving satisfactory outcomes. METHOD: Patients submitted to bilateral otoplasty, between June 2010 and December 2012, were retrospectively evaluated and divided into two groups: group 1 - patients submitted to the conventional Mustardé technique and group 2 - those who underwent the modified Mustardé technique. For comparison, we collected data relating to early and late complications, requirement for a new surgery, and degree of patient satisfaction. Statistical analysis was performed with Fisher's exact test. RESULTS: Group 1 included nine patients treated with the conventional Mustardé technique. A case of suture extrusion (11.11%) was observed. Seven patients reported to be very satisfied, one satisfied, and one dissatisfied because of insufficient correction and a subsequent need for a second surgery. Group 2 included 19 patients submitted to the modified Mustardé technique, which involved three sutures in the middle third of the ear and the posterior rotation of the vertex of the antihelix. One case of bilateral infection (5.26%) was observed. Seventeen patients reported to be very satisfied, one satisfied, and another dissatisfied because of insufficient correction, thus needing to undergo a second surgery. No statistical difference was observed between the two groups. CONCLUSION: The identification of the basic abnormality associated with prominent ears allows performing a single approach that is easy to learn and implement, in addition to providing good outcome (i.e., natural appearance) and a high degree of patient satisfaction.

RESUMO

INTRODUÇÃO: Tradicionalmente, múltiplas anomalias podem causar as orelhas proeminentes e foram desenvolvidas técnicas específicas para o tratamento de cada deformidade. Neste trabalho, testou-se uma abordagem única para alcançar resultados favoráveis. MÉTODO: Foram avaliados, retrospectivamente, os pacientes submetidos a otoplastia bilateral entre junho de 2010 e Dezembro de 2012, divididos em dois grupos: Grupo 1 - técnica convencional de Mustardé e Grupo 2 - Refinamento da técnica de Mustardé. Para comparação foram coletados os dados referentes às complicações precoces, tardias, à necessidade de reoperação e ao grau de satisfação do paciente. A análise estatística foi realizada através do teste exato de Fisher. RESULTADOS: No Grupo 1 foram incluídos 9 pacientes tratados com a técnica tradicional de Mustardé. Houve um caso de extrusão de pontos (11,11%). Sete pacientes ficaram muito satisfeitos, um satisfeito e um insatisfeito por correção insuficiente, sendo reoperado. No Grupo 2 foram incluídos 19 pacientes submetidos a uma variação da técnica de Mustardé com 3 suturas no terço médio da orelha, rotacionando posteriormente o vértice da antélice. Houve um caso de infecção bilateral (5,26%). Dezessete pacientes ficaram muito satisfeitos, um satisfeito e outro insatisfeito por correção insuficiente, sendo reoperado. Não houve diferença estatística entre os grupos. CONCLUSÃO: A identificação da alteração básica relacionada com orelhas proeminentes permite uma abordagem única, de fácil aprendizado e execução, com resultados de aparência natural e elevado grau de satisfação do paciente.

ABSTRACT

Introduction: Fanned ear is the most common of all congenital defects of the ear. As the ears in abyss occur because of lack of anti-helix safety, a woman may be absent or even by excessive excess of the concha.
Objective: To demonstrate a surgical approach, the degree of satisfaction and the results obtained in the application of a technique for the surgery of joints and lesions in the memory of the plastic surgery for correction of fanned ears.
Methods: Retrospective cross-sectional study of patients who underwent correction of deformity of ears in the period from 01/07/2014 to 07/01/2017, in the practice of Plastic Surgery and Reconstructive Microsurgery at Walter Cantídio University Hospital (HUWC). Federal University of Ceará (UFC).
Results: A total of 38 patients with ears in patients with otoplasty, ranging in age from 15 to 53 years, with a mean of 27.2 years, 24 males and 14 females. Regarding satisfaction, only 1 (2.6%) patient had a regular result, 2 (5.3%) reported a satisfactory result and 35 (92.1%) reported satisfactory results. 16 patients had concha hypertrophy, 9 had anti-helix malformation and 13 had concha hypertrophy and anti-helix malformation at the same time.
Conclusion: The technique based on results compared and compared with the literature is recommended for universal use, fewer complications and larger scale for most patients.

RESUMO

Introdução: Orelha em abano é o mais comum de todos os defeitos congênitos da orelha. Pode ser notado ao nascimento da criança e geralmente torna-se pior com o passar do tempo. As orelhas em abano ocorrem por desenvolvimento insuficiente da dobra anti-hélice, a qual pode estar ausente ou insuficiente e ainda por crescimento excessivo da concha.
Objetivo: Demonstrar a abordagem cirúrgica, o grau de satisfação e os resultados obtidos da aplicação de uma técnica formada pela união de diversas técnicas cirúrgicas consagradas na história da cirurgia plástica para correção de orelhas em abano.
Método: Estudo de coorte transversal retrospectivo de pacientes que foram submetidos à correção de deformidade orelhas em abano, no período de 01/07/2014 a 01/07/2017, realizados no Serviço de Cirurgia Plástica e Microcirurgia Reconstrutiva do Hospital Universitário Walter Cantídio (HUWC) da Universidade Federal do Ceará (UFC).
Resultados: Um total de 38 pacientes portadores de orelhas em abano foram submetidos à otoplastia. A idade variou de 15 a 53 anos, com média de 27,2 anos, sendo 24 do sexo masculino e 14 do sexo feminino. Em relação à satisfação, apenas 1 (2,6%) paciente declarou regular resultado, 2 (5,3%) declararam bom resultado e 35 (92,1%) afirmaram ótimo resultado satisfatório. 16 pacientes eram portadores hipertrofia da concha, 9 apresentavam mal formação da anti-hélice e 13 possuíam ao mesmo tempo hipertrofia da concha e mal formação da anti-hélice.
Conclusão: A técnica apresentada fundamentada nos resultados encontrados e comparados com a literatura é recomendada para uso universal, fornecendo baixo número de complicações e ótima satisfação pela maioria dos pacientes.

Total ear reconstruction on Clinical Hospital of the Federal University of Goiás

ABSTRACT

Introduction: Repair or rebuild partially or completely an ear is a challenge in medicine and requires technical ingenuity and surgical skill. The use of autogenous material (costal cartilage) is still considered the best surgical option. The success of the reconstruction of the ear depends crucially on the development of the cartilaginous framework. Methods: This is a cross-sectional study. We analyzed the records of 39 patients admitted to the clinic of the Plastic Surgery of the Hospital of the Federal University of Goiás, during the period from 1997 to 2007. All patients underwent total ear reconstruction by the same surgeon, using autologous costal cartilage. The reconstruction was performed in two surgical times. About the patients, was observed the predominance of children and adolescents (6 to 14 years), male and from capital cities (Goiânia). There was no predominance of right or left microtia. The average time of the first surgical time was 140 minutes and the second 120 minutes. Resuts: The pain at the site of costal cartilage removal was the main complication. There were no cases of extrusion of the cartilaginous framework, infection and perforation of the pleura. The findings are for a better understanding and characterization of patients. Conclusion: The standardization of the technique, in this case using costal cartilage to build the framework headset, along with experience of the surgeon can bring great benefits to patients, especially regarding to minimize the complications of surgeries performed.

RESUMO

Introdução: Reparar ou reconstruir parcial ou totalmente uma orelha constitui-se um desafio na Medicina e requer engenhosidade técnica e habilidade cirúrgica. A utilização de material autógeno (cartilagem costal) ainda é considerada a melhor opção cirúrgica. O sucesso da reconstrução da orelha depende sobremaneira da elaboração do arcabouço cartilaginoso. Método: O estudo é do tipo transversal, retrospectivo. Foram avaliados prontuários de 39 pacientes admitidos no ambulatório de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Goiás, durante o período compreendido entre 1997 a 2007. Todos foram submetidos à reconstrução total de orelha pelo mesmo cirurgião, sendo utilizada a cartilagem costal autóloga e realizada a reconstrução em dois tempos cirúrgicos. Observou-se predomínio de crianças e adolescentes (6 a 14 anos), sexo masculino e procedência da capital (Goiânia). Não houve predominância entre microtia direita e esquerda. O tempo médio do primeiro tempo cirúrgico foi de 140 minutos e, do segundo, 120 minutos. Resultados: A dor no local da retirada da cartilagem costal foi a principal complicação. Não houve nenhum caso de extrusão do arcabouço cartilaginoso, infecção e perfuração da pleura. Os resultados desse trabalho são válidos para um melhor conhecimento e caracterização dos pacientes. Conclusão: A padronização da técnica, nesse caso utilizando cartilagem costal para a construção do arcabouço auricular, aliada à experiência do cirurgião, pode trazer grandes benefícios aos pacientes, especialmente no que diz respeito à minimização das complicações advindas das cirurgias realizadas.

Otoplasty using cartilage abrasion and perichondrioplasty and conchamastoide suture

ABSTRACT

Introduction: Prominent ear has been treated with many different methods. The surgical correction of the conchoscaphal angle and of the distance between the antihelix rim and temporal scalp has been performed both by Brazilian and foreign surgeons. Methods: This paper describes our experience with the combination of different surgical techniques in the correction of prominent ears of 80 patients. The method includes the abrasion of the antihelix, so as to weaken it, and then its reshaping with perichondrioplasty. Concha was attached to the mastoid after removal of the posterior auricular muscle. Results: All patients were operated in the morning, with local anesthesia and sedation, and were discharged in the same day. The result shows a very natural look due to the weakening of the cartilage by abrasion, not breakage, and its reshaping with adequate sutures.

RESUMO

Introdução: A deformidade congênita conhecida como orelha proeminente já foi tratada com diferentes técnicas. Diversos métodos utilizados para a correção do ângulo céfalo-auricular e escafo-conchal são utilizados tanto por autores estrangeiros como brasileiros. Método: Este trabalho foi realizado com 80 pacientes, visando demonstrar a experiência do autor sênior na combinação de diversas técnicas. O tratamento da anti-hélix foi feito por meio da raspagem da cartilagem, buscando seu enfraquecimento, e posterior modelação, com a pericondrioplastia, enquanto o tratamento da concha foi feito pela sua fixação na mastóide após esvaziamento. Todos os pacientes foram operados no período da manhã sob anestesia local e sedação, com alta no mesmo dia. Resultados: A técnica resulta em aspecto visual bastante natural, evitando assim o "estigma de orelha operada", já que a cartilagem não é quebrada, sendo somente enfraquecida e posteriormente remodelada com pontos.

Firmin's surgical classification for ear deformities: critical analisys of a series operated in Brazil

ABSTRACT

Introduction: The reconstruction to correct microtias and acquired ear deformities is a defying surgery due to clinical variations and many forms of treatment. Rogers and Tanzer described the clinical classification that is the most utilized in the literature and Firmin described a surgical classification to assist the plastic surgeon in the treatment. Objective: Demonstrate that there is no strict indications between the clinical types and types of skin incision and to present the best way to use Firmin's surgical classification. Methods: 12 patients with congenital (microtias) or acquired (burn sequela) ear deformities were evaluated. The patients were categorized clinically and surgically by the senior author. All patients underwent reconstructive surgery and were followed up for one year. It was also evaluated the complication index. Results: The clinical classification was: 3 patients as small anomaly, 4 as lobular type, 3 as conchal type and 2 patients with total defect / burn sequel. The surgical classification of skin incision was: 3 patients as type II, 2 as type IIIa and 4 as type IIIb. For the framework was: 5 patients type I and 4 patients type III. The complication index was low. Conclusion: The Firmin's surgical classification for ear reconstruction demonstrated to be an excellent tool to guide the plastic surgeon in the therapeutic treatment. The Firmin's types of incision utilized in the corrective surgery have a relationship with the size and location of the auricular remanent and are independent of the clinical classification.

RESUMO

Introdução: A reconstrução para corrigir as deformidades auriculares, congênitas ou adquiridas, é uma cirurgia desafiadora devido à grande variabilidade clínica e dos tipos de tratamento. Firmin descreveu uma classificação cirúrgica com a finalidade de auxiliar o cirurgião plástico na realização do tratamento. Objetivo: Demonstrar que não existe uma regra única entre os tipos clínicos e os possíveis tipos de incisão na pele e apresentar a melhor maneira de utilizar a classificação cirúrgica de Firmin. Método: Foram avaliados 12 pacientes, todos portadores de deformidades congênitas ou adquiridas. Os pacientes foram classificados clínica e cirurgicamente pela autora sênior. Foram excluídos do estudo os pacientes submetidos à reconstrução de orelha sem a necessidade de cartilagem costal, utilizando cartilagem conchal da orelha contralateral. Todos os pacientes foram submetidos à reconstrução de orelha e acompanhados por um ano. Foi avaliado também o índice de complicações. Resultados: As classificações cirúrgicas de incisão na pele foram: 3 pacientes tipo II, 2 pacientes tipo IIIa e 4 pacientes tipo IIIb. Os tipos de maquete foram: 5 pacientes tipo I e 4 pacientes tipo III. Não houve mudanças das indicações cirúrgicas. O índice de complicações foi considerado baixo. Conclusão: A classificação cirúrgica de Firmin para reconstrução auricular demonstrou ser uma excelente ferramenta para direcionar o cirurgião plástico no planejamento terapêutico. O tipo de incisão, proposto por Firmin, a ser utilizado na correção cirúrgica tem relação com o tamanho e a localidade do remanescente auricular ou com sua ausência, e é independente da classificação clínica.

Ear shut and Dentistry: ethical and legal approach

ABSTRACT

Introduction: As a health science, dentistry seeks to raise self-esteem and improve patients' quality of life. However, the emergence of the aesthetic procedure "ear shut," which proposes correcting protruding ears without surgery, has raised ethical and legal doubts in the dental profession, especially concerning the limits of professional activity.
Objective: To conduct a survey of the laws, norms, and resolutions on the area of performance of dental surgeons, as well as to discuss the limits and consequences of their extrapolation from the perspective of the procedure propagated as "ear shut."
Method: A search was carried out for administrative norms on the Federal Council of Dentistry (CFO) websites and the Federal Council of Medicine and legal provisions on the Portal da Legislação website.
Results: In the civil sphere, the disclosure of procedures such as the "ear shut" can characterize a promise of result and lead to judicial accountability. Exceeding professional boundaries constitutes an illegal exercise and a crime under the Brazilian Penal Code. In addition to not covering the area of clinical practice of dentists, at the administrative level, CFO Resolutions No. 198/2019 and No. 230/2020 emphasize that ear procedures are not part of the scope of procedures relevant to Dentistry and may lead to ethical infractions and consequent administrative proceedings.
Conclusion: At present, based on the skills, prohibitions, rights, and duties of dentists, it can be stated that performing the "ear shut" by these professionals confronts civil, criminal, and administrative obligations.

RESUMO

Introdução: Como ciência da saúde, a Odontologia busca elevar a autoestima e melhorar a qualidade de vida dos pacientes. Entretanto, o surgimento do procedimento estético "ear shut", que propõe a correção da orelha em abano sem cirurgia, gerou dúvidas de ordem ética e legal na classe odontológica, sobretudo no que diz respeito aos limites de atuação profissional.
Objetivo: Realizar o levantamento das leis, normativas e resoluções sobre a área de atuação dos cirurgiões-dentistas, bem como discutir os limites e as consequências de sua extrapolação sob a perspectiva do procedimento propagado como "ear shut".
Método: Foi realizada uma busca de normas administrativas nos sites do Conselho Federal de Odontologia (CFO) e do Conselho Federal de Medicina, bem como de dispositivos legais no site Portal da Legislação.
Resultados: Em âmbito cível, a divulgação de procedimentos como o "ear shut" pode caracterizar promessa de resultado e levar à responsabilização judicial. Ultrapassar os limites profissionais constitui exercício ilícito e constitui crime segundo o Código Penal Brasileiro. Além de não abranger a área de atuação clínica dos cirurgiões-dentistas, no âmbito administrativo, as Resoluções do CFO N° 198/2019 e N° 230/2020 enfatizam que procedimentos na orelha não compõem o escopo de procedimentos pertinentes à Odontologia, podendo levar a infrações éticas e consequente processo administrativo.
Conclusão: No momento atual, com base nas competências, vedações, direitos e deveres dos cirurgiões-dentistas, pode-se afirmar que a realização do "ear shut" por estes profissionais confronta obrigações de ordem cível, penal e administrativa.

Development and implementation of an anthropometric protocol to evaluate results of otoplasty

INTRODUCTION: Prominence is the most common deformity of the ear, affecting about 5% of the population. Most reports on otoplasty describe subjective evaluations, and do not provide accurate postoperative assessment or a comparison between techniques. We propose the development and implementation of a specific protocol to evaluate results. METHOD: A prospective evaluation for a period of one year in patients who underwent bilateral otoplasty was performed, using a technique based on modeling of the cartilage with sutures, and helix-to-mastoid distance measurements at standardized points. RESULTS: A total of 23 patients with an average age of 17.8 years underwent surgery. Reoperation was performed in 21.7% of the patients or 10.7% of the ears. Nearly 45% of the correction obtained at the upper point and 35% at the middle and lower points were lost in patients who did not undergo reoperation. CONCLUSIONS: The protocol was easily used and allowed objective evaluation of the preoperative deformity and surgical results. This technique produced results considered adequate and comparable to the literature.

RESUMO

INTRODUÇÃO: A proeminência dos pavilhões auriculares é a forma mais comum de deformidade desta estrutura, afetando em torno de 5% da população. A maioria dos trabalhos acerca da cirurgia de otoplastia apresenta avaliações subjetivas, não permitindo apreciar acuradamente os resultados no pós-operatório, bem como dificultando a comparação entre técnicas. Propõe-se o desenvolvimento e aplicação de protocolo específico para avaliação dos resultados. MÉTODO: Avaliação prospectiva pelo período de um ano de pacientes submetidos à otoplastia bilateral utilizando técnica baseada em modelagem cartilaginosa com suturas, utilizando medidas da distância hélice-mastoide em pontos padronizados. RESULTADOS: Foram operados 23 pacientes com idade média de 17,8 anos. A incidência de reoperação foi de 21,7% dos pacientes ou 10,7% das orelhas. Houve perda de cerca de 45% da correção obtida no ponto superior e 35% nos pontos médio e inferior nos pacientes não reoperados. CONCLUSÕES: O protocolo foi de fácil utilização e permitiu a avaliação objetiva tanto da deformidade no pré-operatório quanto dos resultados cirúrgicos. A técnica utilizada produziu resultados considerados adequados e comparáveis aos da literatura.

ABSTRACT

Introduction: Constricted ears may present as different malformations that share the deformity upper part of ear. Deformity correction may range from a simple technique to a complex reconstruction of all segments, with costal cartilage grafts. Method: Eight patients were operated, with different deformity presentation, in a total of 10 constricted ears, between 2000 and 2004, in Plastic Surgery Division at University Hospital of Federal University of Rio de Janeiro. Patients had a follow-up range from 1 year to 3 years and 6 months. The aim of this study is to describe surgical techniques used according to auricular deformity severity. Results: Retrospective analysis has shown satisfactory results, without complication and relapse. Conclusion: The authors have concluded that surgical treatment of ear congenital malformation, as constricted ear, should be performed by surgeons experienced in several techniques adapting to the different auricular malformations.

Introdução: As orelhas constrictas apresentam-se sob formas variadas, tendo em comum o comprometimento predominante do terço superior. As deformidades podem ser de resolução técnica simples ou até necessitarem de reconstruções complexas de todo o segmento, com enxertia de cartilagem costal. Método: No período de 2000 a 2004, foram operados oito pacientes, totalizando 10 orelhas constrictas, com apresentações diferentes, no Serviço de Cirurgia Plástica do Hospital Universitário da Universidade Federal do Rio de Janeiro. Os pacientes foram acompanhados por um período que variou de 1 ano a 3 anos e 6 meses. Este trabalho tem como objetivo descrever as técnicas cirúrgicas utilizadas, de acordo com a gravidade da deformidade anatômica. Resultados: Os casos foram analisados de forma retrospectiva, evidenciando-se boa evolução, sem intercorrências ou recidivas. Conclusão: Concluímos que o tratamento das orelhas constritas necessita de um arsenal de técnicas adaptáveis às suas múltiplas formas. Cirurgias mal-sucedidas podem dificultar reconstruções futuras.

Question Mark Ear Treatment

ABSTRACT

Introduction: The congenital external ear deformity with weakening of the medium segment of conch and of scapha is usually denominated question mark ear because of the called minor development of that segment and by its characteristic shape. Some authors have published their experiences to correct this kind of deformity. Our experience is based on three patients using a sliding flap and a compensatory triangle of the conch to balance the reduction of the major axis of the ear and support its shape as natural as possible. Method: Three female patients were submitted to a surgical procedure. They were 17, 19 and 20 years old; the youngest has a unilateral deformity and the others, bilateral. The procedures were systematically done. The major axis and the size of the defect were measured before and after the surgery to be compared. Results: The surgical procedures were almost similar, however the deformities were not. They were different even in the same patient. Although the same technical principle of sliding flap and compensatory triangle of the conch has been used, the extension of the defect determined minor or major resection and suture lines different in position and extension. Conclusions: The described procedure is not technically difficult. It can be performed under local anesthesia, ambulatory condition and in only one operating time.

RESUMO

Introdução: As deformidades congênitas uni ou bilaterais da orelha externa com atrofia do segmento médio da concha e da escafa são coloquialmente chamadas de orelha em ponto de interrogação, devido ao hipodesenvolvimento deste segmento e pelo formato característico. Diversos autores publicaram suas experiências para a correção deste tipo de deformidade congênita. Nossa experiência baseia-se na reconstrução de três casos, mediante a combinação de retalho de deslizamento e da ressecção de um triângulo de compensação na concha, ambos do tipo condro-cutâneo, na tentativa de evitar ao máximo a redução do eixo maior da orelha e manter o seu contorno o mais natural possível. Método: Foram operadas três pacientes do sexo feminino, com 17, 19 e 20 anos, sendo que na paciente de menor idade a deformidade era unilateral e nas outras duas, bilateral. Os procedimentos cirúrgicos obedeceram a mesma sistematização. Em todos os casos, o maior eixo da orelha foi medido antes e após a cirurgia, assim como o tamanho do defeito a ser corrigido, para avaliações comparativas. Resultados: Os detalhes cirúrgicos descritos foram basicamente similares, porém as deformidades encontradas não apresentaram um padrão idêntico, mesmo numa mesma pessoa com problema bilateralmente. A extensão sendo variável determinou ressecções maiores ou menores, linhas de sutura diversas na posição e na extensão, tendo como parâmetro o retalho de deslizamento e a ressecção de um triângulo de compensação. Conclusões: Os procedimentos descritos são de fácil execução, sob anestesia local, condições ambulatoriais e realizados num único tempo operatório.

Complications of otoplasty surgeries

ABSTRACT

INTRODUCTION: Protruding ears are caused by poor development of the antihelix, hyperplasia of the concha, abnormal cephaloauricular angle, a large and prominent lobule, or any combination of the above. METHODS: A prospective study was performed, in which 60 patients who underwent otoplasty were followed up from January 2014 to January 2015 at the Hospital Municipal Barata Ribeiro. RESULTS: Early complications included infection (0%), hematoma (1.6%), and skin necrosis (3.3%). Late complications included asymmetry (10%), recurrence (3.3%), extrusion of sutures (10%), granuloma formation (0%), hypertrophic scarring (0%), and keloid formation (0%). When patients were asked to rate the otoplasty, 3.3% stated that it was poor, 3.3% fair, 3.3% good, 3.3% very good, and 86.8% excellent. CONCLUSION: The rates of early and late complications are similar to those described in the literature.

RESUMO

INTRODUÇÃO: Orelha em abano é caracterizada pelo pobre desenvolvimento da anti-hélice, hiperplasia da concha, ângulo céfalo-auricular anormal, lóbulo grande e proeminente ou qualquer combinação dos itens citados. MÉTODOS: Foi realizado um estudo prospectivo, em que foram acompanhados 60 pacientes submetidos à otoplastia, no período de janeiro de 2014 a janeiro de 2015, no Hospital Municipal Barata Ribeiro. RESULTADOS: Entre as complicações precoces, citamos infecção (0%), hematoma (1,6%) e necrose de pele (3,3%). Entre as complicações tardias: assimetria (10%), recidiva (3,3%), extrusão de pontos (10%), granuloma, cicatriz hipertrófica (0%) e queloide (0%). Quando os pacientes foram questionados quanto aos resultados obtidos com a otoplastia, 3,3% responderam que o resultado foi ruim, 3,3% regular, 3,3% bom, 3,3% ótimo e 86,8% excelente. CONCLUSÃO: As taxas de complicações precoces e tardias são semelhantes às descritas na literatura.

ABSTRACT

BACKGROUND: Prominent ear is the most common congenital defect of the ear, with an incidence of 5% in Caucasians. Surgical treatment should correct the auriculocephalic and conchoscaphal angles as well as protrusion of the lobe when present. This paper aims to report the experience of our service in the treatment of prominent ears with a combination of several available techniques. METHODS: Forty-seven patients operated with a combination of previously described techniques were evaluated, and cartilaginous incision, Mustardé sutures for antihelix definition, and concha-mastoid fixation were performed. Patients less than 15 years of age were operated under general and local anesthesia, while the remaining patients underwent only local anesthesia. All patients were reassessed on the first postoperative day. RESULTS: The postoperative results were considered satisfactory by both patients and surgical staff, with no stigma development in the operated ear. CONCLUSIONS: The best treatment of prominent ears is achieved by a combination of techniques. The approach used on the studied patients has produced natural-looking results with low complication rates, satisfying the surgical staff and, most importantly, the patients.

INTRODUÇÃO: A orelha de abano é o mais comum de todos os defeitos congênitos da orelha, com incidência de 5% em caucasianos. O tratamento cirúrgico deve corrigir os ângulos auriculocefálico e escafoconchal, bem como a protrusão do lóbulo, quando presente. O objetivo deste trabalho é demonstrar a experiência de nosso serviço no tratamento da orelha de abano com a combinação de diversas técnicas disponíveis. MÉTODO: Foram avaliados 47 pacientes, operados com a associação de técnicas já descritas anteriormente, sendo utilizados incisão cartilaginosa, pontos de Mustardé para definição de antélice e fixação da concha na mastoide. Os pacientes com menos de 15 anos de idade foram operados sob anestesias geral e local, e os demais foram submetidos somente a anestesia local. Todos os pacientes foram reavaliados no primeiro dia de pós-operatório. RESULTADOS: Os resultados pós-operatórios foram considerados satisfatórios pelos pacientes e pela equipe cirúrgica, sem o aparecimento do estigma de orelha operada. CONCLUSÕES: O melhor tratamento de orelhas proeminentes é obtido com a associação de diversas técnicas. A abordagem empregada nos pacientes avaliados tem apresentado resultados naturais e com baixos índices de complicação, satisfazendo a equipe cirúrgica e, principalmente, os pacientes.

Surgical rejuvenating for the aged earlobe: a technical innovation

ABSTRACT

The authors present a new surgical approach to the aging earlobe. The technique is based on the classic marginal reduction technique of the earlobe contour that has been improved. In essence, it seeks to preserve the subdermal fat layer of the distal flap created for the purpose of raising the earlobe while being shortened, thickened and smooth, in a hidden scar. It is a tripartite resolution to this problem never seen in the relevant literature. Four 68-yearold women were treated: the first with the classic manner in 2009 and the others with this modification to improve the earlobe withered appearance in 2017-2019. The study shows a rejuvenating eutrophic reduction technique.

RESUMO

Os autores apresentam técnica corretiva conservadora para o lóbulo senil. A literatura pertinente é rica em técnicas redutoras de lóbulos longilíneos ou rasgados. Os autores creem que a cirurgia para os lóbulos deve poupar tecidos in situ, a despeito de usuais técnicas de preenchimento com material orgânico e biocompatíveis. A presente técnica se baseia na clássica redução lobular periférica da borda livre, que agrega ao componente do face lifting para reduzir o lóbulo e ao mesmo tempo poupar tecidos e encorpá-lo na cicatriz oculta retrolobular. Resolução tripartite até hoje não apresentada na literatura. Com oito casos operados: seis tipo pec-man no passado; um na forma clássica marginal e três recentes com esta modificação técnica.

ABSTRACT

Introduction: Protruding ear is the most common congenital deformity of the head and neck, with an autosomal dominant inheritance and no predilection for sex. Protruding ear or prominent ear occurs when there is concha excess or hypertrophy, erasure of the antihelix, a scapho-conchal angle greater than 90º, or a combination of these factors, occurring uni- or bilaterally. The objective is to present a conservative approach to correct protruding ear, with a combination of techniques.
Methods: The otoplasty surgical technique involved an anterior approach for resection of the auricular concha, which was associated with weakening of the antihelix, and partial incisions of the cartilage were performed through anterior access and of Mustardé sutures, through posterior access for better definition of the antihelix without fixation of the concha to the mastoid. Two hundred patients with a mean age of 17 years underwent operations between January 1987 and January 2015, 60% of whom were female.
Results: Of the 200 patients, only 24 patients needed discrete surgical revisions.
Conclusion: The surgical procedure is simple, easily reproducible, provides good results, and is associated with a high degree of satisfaction and a low rate of complications/morbidities.

Introdução: Orelha em abano é a deformidade congênita mais comum de cabeça e pescoço, cuja transmissão se dá por herança autossômica dominante, sem predileção por gênero. A orelha proeminente ou "em abano" ocorre quando há um excesso ou hipertrofia da concha auricular, apagamento da antélice, um ângulo escafoconchal maior que 90º ou uma combinação destes, ocorrendo uni ou bilateralmente. O objetivo é apresentar uma abordagem conservadora para correção de orelha em abano, com a associação de técnicas.
Métodos: Foi utilizada uma variação cirúrgica para realização de otoplastia com o auxílio de uma abordagem anterior para ressecção da concha auricular associada ao enfraquecimento da antélice com incisões parciais na cartilagem também por via anterior e a realização de pontos de Mustardé por via posterior para melhor definição da antélice, sem a fixação da concha à mastoide. Foram operados 200 pacientes com idade média de 17 anos, entre janeiro de 1987 e janeiro de 2015, sendo 60% do gênero feminino.
Resultados: Dos 200 pacientes, apenas 24 necessitaram revisões cirúrgicas discretas.
Conclusão: O procedimento cirúrgico é simples, facilmente reprodutível, proporcionando bons resultados, com alto grau de satisfação e baixo índice de complicações/morbidade.

ABSTRACT

Partial ear defects can be treated in several ways, including primary closure, healing by secondary intention, or flaps. Several surgical options have been described for reconstruction in order to maintain the natural contour of the ear, without sacrificing healthy tissues or changing the aesthetics and function. In this article, we present two cases of reconstruction of chondrocutaneous defects of the ear after resection of basal cell carcinoma in the central region of the ear, with the production of a retroauricular island flap transposed through a cartilaginous window with the de-epidermized pedicle. The donor area healed following a primary closure. This procedure can be performed in a single stage, yields satisfactory aesthetic and functional results, and is safe because the retroauricular region is richly vascularized.

RESUMO

Defeitos parciais de orelha podem ser tratados de diversas formas, dentre elas o fechamento primário, cicatrização por segunda intenção ou retalhos. Diversas opções técnicas foram descritas para a sua reconstrução de modo a manter o contorno natural da orelha, sem sacrificar tecido sadio ou alterar sua estética e função. Apresentamos neste artigo dois casos atendidos no Instituto do Câncer do Hospital de Base de São José do Rio Preto de reconstrução de defeitos condrocutâneos de orelha após ressecção de carcinoma basocelular em região central da orelha, com a confecção de retalho retroauricular ilhado transposto através de uma janela cartilaginosa e com o pedículo desepidermizado. Área doadora com fechamento primário. Tal procedimento constitui técnica segura, pois a região retroauricular é ricamente vascularizada, é de fácil execução, em único estágio e com resultado estético e funcional satisfatório.

Use of retroauricular tubular flap for ear helix and ear lobe reconstruction

ABSTRACT

Ear defects are frequent and result of many etiologies, even though ear reconstruction remains a challenge in plastic reconstructive surgery due to anatomy and local tissue paucity. Despite being rarely used, the tubular retroauricular flap presents as an alternative for helix and lobule reconstruction. In this article, the authors describe the operative technique and report a case in which it was used. Also, plastic surgeons and laypeople rated the operative results of four cases of ear reconstructions using the tubular retroauricular flap. The final esthetic result was rated as good or excellent by 35% of laypeople and 50% of plastic surgeons, whereas the operative result was rated as good or excellent by 70% of laypeople and 80% of plastic surgeons. Thereby the findings and authors' experience, we can recommend the tubular retroauricular flap technique for reconstructing ear helix border defects.

RESUMO

Defeitos de orelha são frequentes e de etiologias diversas. Ainda assim, a reconstrução de orelha permanece um desafio dentro da cirurgia plástica reconstrutiva, principalmente devido a anatomia e a escassez de tecido local. Embora pouco utilizado, o retalho tubular retroauricular se apresenta como alternativa para reconstrução da hélice e lóbulo. No presente estudo os autores descrevem a técnica operatória, bem como relatam um caso em que foi utilizado. Realizou-se uma avaliação do resultado operatório de quatro casos de reconstrução de orelha utilizando o retalho tubular retroauricular, através de questionários encaminhados para avaliadores leigos e cirurgiões plásticos. O resultado estético final foi classificado como bom ou excelente por 35% dos avaliadores leigos e 50% dos cirurgiões plásticos. Já o resultado operatório foi avaliado como bom ou excelente por 70% dos leigos e 80% dos cirurgiões plásticos. Os resultados permitem concluir que a técnica do retalho tubular retroauricular para reconstrução de defeitos da borda de hélice pode ser indicada, com resultados satisfatórios.

Ear lobe reconstruction with transverse cartilage graft

ABSTRACT

Background: Due to the aesthetic concept of size, form and position of the ear, one of the most important point is the artistic reconstruction when we plan the surgery. Most of techniques demands more than one surgery time and can left scars in cheeks, neck or the posterior auricular region. The authors present a case of proeminent ear and anesthetic lobe that was treated in one stage. The lobe deformity is considered a rare congenital case of microtia. Case report: Female patient, 16 years old, that presented proeminent ear associated with a congenital defect of the lobe. The authors performed the correction of the proeminent ear with Converse technique and used structures that would be thrown away to rebuild the ear lobe. The reconstruction occurred in one time, using a cartilage graft and an auricular posterior cutanous covering with a great aesthetic result. Conclusion: The technique was considered easy, with excellent aestethic results and a great pacient satisfaction.

Introdução: Devido ao conceito estético relacionado com tamanho, forma e posição, a reconstrução dos defeitos da orelha é uma consideração importante no momento do planejamento cirúrgico. A maioria das técnicas exige mais de um tempo e pode deixar cicatrizes nas bochechas, pescoço ou região auricular posterior. Os autores apresentam relato de caso de paciente com hipodesenvolvimento congênito do lóbulo da orelha, bilateralmente, onde a reconstrução realizada foi feita em único tempo. Relato de caso: Paciente do sexo feminino, 16 anos, apresentando orelha em abano associada a defeito congênito de lóbulo de orelha bilateralmente. Os autores realizaram o tratamento da orelha em abano pela técnica de Converse e, ao abordar a deformidade congênita apresentada, aproveitaram estruturas que seriam desprezadas para a reconstrução do defeito lobular. A correção da deformidade ocorreu em tempo único, utilizando enxerto cartilaginoso e cobertura cutânea da região auricular posterior, com ótimo resultado estético. Conclusão: A técnica apresentada foi considerada de fácil execução; com correção de duas deformidades em tempo único e com ótimo resultado estético.

Aesthetic otoplasty using a crochet needle

ABSTRACT

In aesthetic otoplasty, it is widely known that only techniques that disassemble the resilient cartilaginous architecture in cases of protruding ears coupled with anchoring sutures can achieve good and stable results. All surgical otoplasty procedures aim to achieve a new, shapely, and aesthetic antihelix fold that shapes and beautifies the outer ear. In this article, we describe our 10-year experience with the use of a new instrument for auricular cartilage striation - the traditional crochet needle. Moreover, we describe the manner in which the new instrument can be prepared in order to have a lower bevel cutting edge as well as the technical management of this instrument in small buttonholes in the auricular helix. After molding the antihelix complex, the new form is fixed with absorbable and non-absorbable sutures. With this technique, there is a low complication rate due to narrow cutaneous detachment tunnels in the anterior surface of the ear. In the article, we also describe the successful clinical use of the new instrument.

Um consenso atual em otoplastia estética apregoa que somente as técnicas que desmontem a resiliente arquitetura cartilaginosa das orelhas protrusas, aliadas à fixação da nova forma com suturas, podem alcançar resultados bons e estáveis. O alvo cirúrgico de toda otoplastia é a nova dobra da antélice torneada e estética: uma roliça dobra afunilada que dá forma e beleza à orelha externa. Neste artigo é descrita a experiência de 10 anos com o uso de um novo instrumental no estriamento da cartilagem auricular: as tradicionais agulhas de crochê. Além disso, são demonstrados o modo de aparelhar o novo dispositivo, para ter uma borda inferior em bisel cortante, e a técnica de manejo desse dispositivo por pequenas botoeiras sob a hélice auricular. Após a moldagem do complexo antelical, é assegurada a nova forma com suturas, absorvível e não-absorvível. Com essa técnica, é observado baixo índice de complicações, em decorrência de estreitos túneis de descolamento cutâneo da face anterior do pavilhão auricular. No artigo, é também apresentado um caso clínico bem-sucedido com o uso do novo instrumental.

Reconstrução de concha auricular anterior com retalho auricular posterior ilhado: relato de caso

RESUMO

Aproximadamente 5-8% dos tumores cutâneos malignos da cabeça e pescoço localizam-se no pavilhão auricular, sendo a maioria classificados como carcinomas basocelulares (75%), seguidos dos carcinomas espinocelulares (25%). Diversas são as opções cirúrgicas para restaurar a integridade anatômica da orelha. Relatamos um caso de um paciente que apresentou carcinoma basocelular, localizado na concha do pavilhão auricular esquerdo e parte do conduto auditivo externo, de aproximadamente 3cm de diâmetro, que foi submetido à reconstrução imediata, em tempo único, através do uso do retalho auricular posterior ilhado, classicamente conhecido como Retalho de Masson, apresentando resultado funcional e estético satisfatório ao paciente.

ABSTRACT

Prominent ears are among the most frequent anomalies that affect the head and neck regions and present with conchal disproportionality and/or the lack of an antihelix curvature. A wide spectrum of techniques for the correction of these deformities has been published; the most commonly cited are the techniques from Pitanguy, Converse, Stenvers, Stenstroem, Mustardé, Furnas, and Horlock. All of these techniques seek to restore facial harmony with proportional scaling of the pinna, a suitable cephaloauricular angle, and natural curvature of the antihelix. After reviewing the literature, the present study aims to describe a new technique for correcting the auricular axis and ear upper pole with a suture in the portion of the helical crus known as the Sundfeld-Reis technique. According to Adamson et al., this technique provides better positioning of the ear's upper pole in relation to the temporal pole without the need for suturing the triangular fossa region in the periosteum of the temporal bone. Therefore, using the Sundfeld-Reis technique, the pinna is placed parallel to the nasal dorsum, providing a natural result after otoplasty. The proposed technique is the latest possibility in the world of otoplasty that attains more natural results with fewer surgical interventions.

RESUMO

Dentre as diferentes anomalias que acometem as regiões de cabeça e pescoço, as orelhas em abano se encontram entre as mais frequentes, apresentando desproporcionalidade conchal e/ou falta de curvatura da antélice. Existe na literatura um grande espectro de técnicas para a correção dessas deformidades, citando-se como as mais consagradas as técnicas de Pitanguy, Converse, Stenvers, Stenstroem, Mustardé, Furnas e Horlock. Todas essas técnicas buscam restaurar a harmonia da face com proporcional dimensionamento do pavilhão auricular, ângulo aurículo-cefálico adequado e curvatura de antélice natural. Após revisão da literatura, o presente estudo pretende descrever uma nova técnica para correção do eixo auricular e o polo superior da orelha com uma sutura na porção do ramo da hélice, descrita como técnica de Sundfeld-Reis. Tal técnica proporciona melhor posicionamento do polo superior da orelha em relação ao temporal, sem a necessidade da sutura realizada na região da fossa triangular no periósteo do osso temporal, como orientado por Adamson et al. Outrossim, com o ponto de Sundfeld-Reis há posicionamento do pavilhão auricular paralelo ao dorso nasal, proporcionando um resultado natural da otoplastia. A proposta técnica se fulgura como mais uma possibilidade arregimentada no universo da otoplastia, vislumbrando resultados mais naturais, com menores intervenções cirúrgicas.

Use of a composite auricular graft in nasal alar reconstruction

ABSTRACT

Since the nose is located in the center of the face, small asymmetries and imperfections are easily recognized. One subunit of the nose is the nasal ala, a region that, when reconstructed, requires the achievement of esthetic and functional results. This poses a challenge to the plastic surgeon. In this article, we describe 3 cases of composite auricular grafts used for nasal alar reconstruction. The grafts presented total integration and provided satisfactory esthetic and functional results. A literature review revealed no differences in the complication rates between grafts and local flaps. Moreover, the shape of the auricular cartilage was maintained over time, showing rare distortions and no or minimal absorption, contrary to costal cartilage and bone grafts. The use of composite auricular grafts is versatile and safe. This procedure ensures excellent results in nasal alar reconstruction and guarantees low morbidity within the donor areas. Therefore, the use of this technique efficiently enables the repair of nasal defects.

RESUMO

O nariz ocupa o centro da face, o que torna pequenas assimetrias e imperfeições evidentes. Uma de suas subunidades é a asa nasal, região que exige não apenas resultados estéticos, mas também funcionais, em sua reconstrução, tornando-se um desafio ao cirurgião plástico. Neste artigo são descritos 3 casos em que foi utilizado enxerto composto auricular para reconstrução da asa nasal. Os enxertos apresentaram integração total, com resultados estéticos e funcionais adequados. Segundo revisão da literatura, não há diferença nos índices de complicação comparando-se os enxertos com os retalhos locais e, a longo prazo, a cartilagem auricular tende a manter-se no formato moldado, sofrendo raras distorções e mínima ou nenhuma absorção, diferentemente das cartilagens costais e dos enxertos ósseos. O enxerto composto auricular é uma técnica versátil e segura, com excelentes resultados na reconstrução da asa nasal e com baixa morbidade das áreas doadoras, cumprindo com eficiência seu objetivo reparador.
