Profile of breast augmentation surgery in Brazil

ABSTRACT

Introduction: The latest research by the International Society of Aesthetic Plastic Surgery (ISAPS) showed that 2,524,115 plastic surgeries were performed in Brazil in 2017. Breast augmentation has been one of the most commonly-performed plastic procedures in Brazil, totaling more than 200,000 surgeries in 2016. The objective is to evaluate the current practices and trends in breast augmentation in Brazil.
Methods: A 31- item questionnaire was constructed and sent to 6,200 active members of the Brazilian Society of Plastic Surgery (SBCP). The research addressed some current trends and controversial practices in five areas: current controversies, new technologies, breast implants, technical considerations in secondary procedures, and demographic aspects. The questionnaire was designed to profile practices, procedures, and beliefs among plastic surgeons regarding the use of breast implants in Brazil.
Results: A total of 505 responses were collected, representing a response rate around 10%, which is well above the average response rate for Brazilian Society of Plastic Surgery (SBCP) questionnaires. The results were distributed in five tables by area of interest.
Conclusion: In this study, the most common practices included the use of microtexture round implants and polyurethane-coated silicone as the primary procedure, a subglandular pocket, inframammary incisions, preoperative sizing with round implant samples, intravenous and oral antibiotics, double irrigation antibiotics, an implant size that was generally less than 325 cc, and without drainage. There is no consensus on breast lip balancing and the management of capsular contracture and double bubble deformity.

RESUMO

Introdução: A última pesquisa da Sociedade Internacional de Cirurgia Estética (ISAPS) mostrou que 2.524.115 cirurgias plásticas foram realizadas no Brasil em 2017. O aumento de mama tem sido um dos procedimentos plásticos mais comumente realizados no Brasil, totalizando mais de 200.000 cirurgias no ano de 2016. O objetivo é avaliar as práticas e tendências atuais em aumento de mama no Brasil.
Métodos: Um questionário de 31 itens foi construído e enviado para 6.200 membros ativos da Sociedade Brasileira de Cirurgia Plástica (SBCP). A pesquisa abordou algumas tendências atuais e práticas controversas considerando cinco áreas: controvérsias atuais, novas tecnologias, implantes mamários, considerações técnicas em procedimentos secundários e aspectos demográficos. O questionário foi desenhado para delinear o perfil das práticas, procedimentos e crenças entre os cirurgiões plásticos sobre o uso dos implantes mamários no Brasil.
Resultados: Foram coletadas 505 respostas, representando uma taxa de resposta em torno de 10%, bem acima da taxa média de resposta de um questionário da SBCP. Os resultados foram distribuídos em 5 tabelas de acordo com as áreas de interesse.
Conclusão: Nesta pesquisa, as práticas mais comuns incluíram o uso de implantes redondos de microtextura e silicone revestido com poliuretano no procedimento primário, poket subglandular, incisões inframamárias, dimensionamento pré-operatório com amostras de implante redondas, antibióticos intravenosos e orais, irrigação com duplo antibiótico, implante faixa de tamanho geralmente menos 325cc e sem drenagem. Não há consenso sobre a lipotransferência mamária e o manejo da contratura capsular e da deformidade da bolha dupla.

Evaluation of the data contained in the requests for the anatomopathological examination of the breast in the surgery of exchange or explantation of silicone breast implants: analysis of 251 cases

ABSTRACT

Introduction: The description of the Anaplastic Large Cell Lymphoma and the explantation surgery resulted in an increase of histopathological exams in breast implant removing surgery.
Methods: 251 pathology requests were studied. The following data from the medical requests were analyzed: gender, age, type of surgery, number of specimens containers sent, laterality, anatomical and spatial location, clinical history, signs and symptoms, previous radiotherapy, previous chemotherapy, diagnostic hypothesis, previous surgeries, and reference to previous breast exams.
Results: The mean age was 43 years old. Laterality was not mentioned in 16 requests. The surgery performed was mentioned in 15.94% requests. The number of containers varies from 1 to 5, with a median of 2. The containers include capsules in 242 cases, 161 as isolated capsule, 27 mammary tissue, and capsule in the same specimen, 54 mammary tissues sent in a separate container, anatomical and spatial location was mentioned in 6.33% cases. The detailed clinical data was included in 19.12%, signs and symptoms 13.94%, contracture as the only item mention in 64 of them. In 27 requests, lymphoma evaluation was requested. 15 included seroma and from nine of those, liquid was sent with a request for immunohistochemical and cytology analysis. None of the requests had any data on implant type or brand.
Conclusion: The amount of information contained in the medical request forms is minimal. The authors recommend the need for a protocol to standardize the surgical removal of the capsule and the adjacent mammary tissue. Surgical specimens should be spatially oriented.

RESUMO

Introdução: A descrição do linfoma anaplásico de células T e o recente aumento das cirurgias de explante resultou na elevação do número de exames anatomopatológicos nas cirurgias de retirada de implantes mamários de silicone. O objetivo desta pesquisa é analisar a qualidade e quantidade de dados contidos na requisição do exame histopatológico.
Métodos: Foram estudados 251 casos. Os seguintes dados foram analisados: sexo, idade, localização anatômica e espacial, lateralidade, história clínica, sinais e sintomas, quimioterapia e radioterapia prévia, hipótese diagnóstica, cirurgias prévias, tipo e marca do implante, exames de imagem prévios e número e características dos espécimes enviados.
Resultados: A idade média foi de 43 anos. A lateralidade não foi mencionada em 16 (0,84%). A localização anatômica foi citada em 15 casos. O tipo de cirurgia foi mencionado por 40 (15,94%). O número de contêineres variou de 1 a 5, com mediana de 2. A cápsula foi enviada em 242 casos, em 161 de forma isolada, tecido mamário em conjunto com cápsula em 27, tecido mamário e cápsula em contêineres diferentes em 54 casos. A história clínica foi incluída em 19,12%, sinais e sintomas em 13,94%, em que a contratura foi o único termo inserido em 64. Em 27 requisições foi citado linfoma. Em 15 pacientes a presença de seroma foi referida e destes nove foram enviados. O tipo e marca do implante não foi citado.
Conclusão: Os dados são escassos. Recomenda-se a criação de protocolos na retirada da cápsula e tecido adjacente, incluindo a orientação anatômica e espacial.

Increase in the volume of the breast implant by passing organic material into it

ABSTRACT

Introduction: The search for an anatomical substitute for the breast, for the purpose of correcting aesthetic problems has a long history of failure until the arrival of silicone. Since the beginning of its use, in the 1960s, many complications have appeared, such as infection, rejection, rupture, in addition to silicone leakage. We did not find in the literature, however, any report on the passage of materials from the human organism to the interior of implants. The objective is to identification by infrared spectrophotometry (FTIR) and clinical analysis, reporting the passage of organic substances into breast implants without any violation of their capsule.
Methods: 1500 pairs of breast implants were analyzed, surgically removed from 1998 to 2018. Of which six were included in the study.
Results: Three materials were analyzed showing macroscopic changes in their interior, without violating the capsule. A second sample was performed on a similar implant, but without use. The third material was a sample of fatty breast tissue removed from the patient during the surgery. Materials compatible with fat, animal protein and hemoglobin were found inside the implant.
Conclusions: The change evidenced in the material inside the two implants indicates the occurrence of the passage of organic materials through an intact capsule.

RESUMO

Introdução: A busca de um substituto anatômico para a mama, para fins de correção de problemas estéticos, tem longa história de insucessos até a chegada do silicone. Desde o início de seu uso, na década de 1960, muitas complicações surgiram, como infecção, rejeição, rotura, além do extravasamento de silicone. Não encontramos na literatura, porém, relato algum sobre a passagem de materiais do organismo humano para o interior de implantes. O objetivo é a identificação por espectrofotometria de infravermelho (FTIR) e análise clínica, relatando a passagem de substâncias orgânicas para o interior de implantes mamários sem que os mesmos apresentem violação qualquer de sua cápsula.
Método: Foram analisados 1500 pares de implantes mamários, removidos cirurgicamente no período de 1998 a 2018. Destes, seis foram encaminhados incluídos no estudo.
Resultados: Foram analisados três materiais apresentando alterações macroscópicas em seu interior, sem que houvesse violação da cápsula. Uma segunda amostra foi realizada em implante semelhante, porém sem uso. O terceiro material foi uma amostra de tecido gorduroso mamário removido da própria paciente durante o ato cirúrgico. Foram encontrados materiais compatíveis com gordura, proteína animal e hemoglobina no interior do implante.
Conclusões: A alteração evidenciada no material do interior dos dois implantes nos indica a ocorrência de passagem de materiais orgânicos através de cápsula intacta.

Mastopexy combined with augmentation: systematic use of Ribeiro's inferiorly-based flaps

ABSTRACT

INTRODUCTION: In the last few decades, mastopexy has been a much discussed topic among plastic surgery specialists. Mastopexy associated with augmentation improves breast projection and ptosis correction. However, several authors have reported a high incidence of complications; the most feared being the extrusion of the implants. The use of Ribeiro's inferiorly-based flaps can be a good alternative to increase the safety of the procedure. METHODS: Twenty-five consecutive patients who underwent augmentation-associated mastopexy using the inverted-T technique and Ribeiro's type I and type III inferiorly-based flaps between 2008 and 2011 were analyzed. The cases were assessed with regard to patients' age, type of surgery, purpose of the surgery, simple or associated procedure, follow-up period, positioning of the implants, type of implants, type of flaps, rate of complications and rate of revision surgeries. RESULTS: The mean age of the patients was 39.5 years; 92% of the surgeries were performed for cosmetic reasons; 72% of the surgeries were primary procedures and 80% were associated procedures. In 24% of the cases, the patients exhibited grade II ptosis and 76% exhibited grade III ptosis. The follow-up period ranged from 3 months to 48 months. Two patients (8%) exhibited slight distress at the junction of the inverted-T, which was resolved with conservative treatment. No complications, such as seroma, hematoma, necrosis of the nipple-areola complex, infection, or implant loss, were observed. Moreover, no revision surgeries due to unsatisfactory results were necessary. CONCLUSION: The systematic use of Ribeiro's inferiorly-based flaps can help increase the safety of the procedure and yield good results.

RESUMO

INTRODUÇÃO: A mastopexia tem sido muito discutida entre os especialistas nas últimas décadas. Mastopexia associada a implantes possibilita melhorar a projeção mamária e a correção da ptose. Vários autores relatam grande incidência de complicações, sendo a mais temida a extrusão dos implantes. O uso dos retalhos de base inferior de Ribeiro pode ser uma boa alternativa para aumentar a segurança do procedimento. MÉTODO: Foram analisados 25 casos consecutivos de mastopexia com implantes em T-invertido e uso de retalhos de base inferior de Ribeiro dos tipos I e III, operados entre 2008 e 2011. Os casos foram avaliados quanto a idade das pacientes, tipo de cirurgia, motivação, procedimento simples ou associado, tempo de seguimento, posição dos implantes, tipo de implante, tipo de retalho, e índice de complicações e de reoperações. RESULTADOS: A média de idade das pacientes foi de 39,5 anos, 92% das cirurgias foram por motivação estética, 72% das cirurgias foram primárias e 80%, associadas. Em 24% dos casos, as pacientes apresentavam ptose grau II e em 76%, grau III. O período de seguimento variou de 3 meses a 48 meses. Duas (8%) pacientes apresentaram pequenos sofrimentos na junção do T-invertido, tratados de maneira conservadora. Não foram obervadas complicações como seromas, hematomas, necroses do complexo areolopapilar, infecções, perda dos implantes, tampouco foram necessárias reoperações por resultados insatisfatórios. CONCLUSÕES: O uso sistemático dos retalhos de base inferior de Ribeiro pode auxiliar a aumentar a segurança do procedimento, permitindo a obtenção de bons resultados.

Submuscular implant in double pocket for augmentation mastopexy

ABSTRACT

Introduction: The use of implants is steadily increasing. Reports have been published of implants in the subglandular, subfascial, total, and partial submuscular positions each with its indications, limitations, and complications.
Methods: This study presents a technique to cover the implant and support it using two muscle flaps. Between November 2009 and April 2012, 80 patients who submitted to inclusion of implants in the submuscular position in double pocket were analyzed, regardless of the access route, the degree of flaccidity, and ptosis.
Results: The versatility of this maneuver allows it to be used in a wide variety of breasts, from breast augmentation without flaccidity to mastopexy with implant, and may be carried out via the periareolar, inframammary, or open access routes. It was used in primary and secondary surgeries, with or without the use of breast tissue flap and removal of skin.
Conclusions: The coverage of the implant with the pectoral muscle by the double pocket technique displayed excellent results, with a low rate of complications and reinterventions. This technique is being studied in a larger sample with a longer follow-up to confirm the results obtained so far.

Introdução: A utilização de implantes mamários vem aumentando. O posicionamento do implante na literatura é relatado como subglandular, subfascial, submuscular total e parcial, cada qual com suas indicações, limitações e complicações.
Métodos: Este trabalho mostra uma manobra para cobertura do implante e sua sustentação utilizando dois retalhos musculares. Entre novembro de 2009 e abril de 2012, foram analisados 80 casos de pacientes submetidas à inclusão de implantes em posição submuscular em duplo bolso, independentemente da via de acesso, do grau de flacidez e ptose.
Resultados: A versatilidade desta manobra permite que seja utilizada em uma ampla variedade de mamas, desde aumento sem flacidez a mamopexia com implante, podendo ser realizada por via periareolar, inframamária ou aberta. Foi utilizada em cirurgias primárias e secundárias, com ou sem uso de retalho de tecido mamário e retirada de pele.
Conclusões: A cobertura do implante com o músculo peitoral pela técnica de duplo bolso demonstrou ótimos resultados, com baixo índice de complicações e reintervenções. Esta técnica está sendo utilizada em uma maior casuística e um seguimento mais longo tem sido feito para confirmar os resultados obtidos até o momento.

Secondary mammaplasty with monobloc resection and implant repositioning in the partial retropectoral pocket

ABSTRACT

Introduction: Breast augmentation with silicone implants is one of the most common plastic surgery procedures. The objective of this study was to evaluate patients with previous silicone implantations undergoing secondary mammaplasty, presenting an alternative approach with en block resection of breast tissue, fibrous capsule, and silicone implant, followed by implant repositioning in the partial retropectoral pocket.
Methods: This study included 24 cases of secondary mammaplasty with implant repositioning. It presents the indications for surgery and details the surgical approach for easier procedures and more satisfactory results.
Results: All the included cases presented breast changes on physical examination, such as breast ptosis, capsular contracture, improper implant position, and breast asymmetry. In the studied cases, the use of polyurethane-coated implants and their complete adherence to the fibrous capsule resulted in a more practical resection even with varying degrees of capsular contracture. Fibrous capsules of textured implants were thinner, and implant instability due to the presence of residual seroma or pockets bigger than necessary resulted in more difficult resection.
Conclusion: Secondary mammaplasty with en bloc resection, along with implant replacement and repositioning in the partial retropectoral pocket with sutures involving muscle and breast tissue is an alternative to optimize the procedure, providing greater muscle stability until complete healing and new fibrous capsule formation.

RESUMO

Introdução: As cirurgias de aumento de mamas com implantes de silicone estão entre os procedimentos de cirurgia plástica mais realizados. O objetivo desse estudo é avaliar os casos de pacientes com prótese de silicone, submetidas à mamoplastia secundária, apresentando uma alternativa de abordagem com ressecção em monobloco do tecido mamário, cápsula fibrosa e prótese de silicone; e o neoposicionamento do implante em loja retromuscular peitoral parcial.
Métodos: Foram analisados 24 casos de mamoplastia secundária com neoposicionamento da prótese, apresentando os motivos da indicação da cirurgia e o detalhamento da abordagem cirúrgica para maior facilidade na execução do procedimento e a obtenção de resultados mais satisfatórios.
Resultados: Todos os casos operados apresentavam alterações no exame físico das mamas, como: ptose mamária contratura capsular, posicionamento inadequado dos implantes e assimetrias mamárias. Nos casos estudados, as próteses com revestimento de poliuretano e sua completa adesão à cápsula fibrosa permitiram a ressecção mais prática, mesmo com variáveis graus de contratura capsular. As cápsulas fibrosas que envolviam implantes texturizados tinham características mais finas e a instabilidade das próteses, pela presença de seroma residual ou pelo excessivo tamanho da loja da prótese, desencadearam maior dificuldade na ressecção.
Conclusão: A realização de mamoplastia secundária com troca de prótese, empregando a ressecção em monobloco e neoposicionamento do implante no espaço retromuscular peitoral parcial com suturas do músculo ao tecido mamário, oferece uma alternativa para otimizar o procedimento, maior estabilidade do músculo até completa cicatrização e a formação de nova cápsula fibrosa.

Silicone breast prosthesis explant: breast reconstruction using the crossed flap technique

ABSTRACT

Introduction: In 1963 Cronin and Gerow introduced the use of the silicone implant and its use increased exponentially. However, complications related to implants emerged over time. The set of adverse situations to the use of silicone implants fueled by the growth of social media culminated in an increase in the permanent removal of the implant. Many cases of explants have the inferior pedicle compromised by injury to the perforating vessels, and the crossed flap technique is an alternative for the reconstruction of explanted breasts.
Methods: Silicone explants were performed with immediate breast reconstruction without the use of a new implant, motivated by medical indication or the patients own desire. The crossed flap technique was used in all cases. It uses the crossing of parenchymal patches of the superior pedicle, one medial and one lateral, as described by Sperli.
Results: 10 cases were operated from 2004 to 2021. The time of use of the prostheses ranged from 3 to 19 years and the main motivation for the explant was capsular contracture. No cases of necrosis were observed.
Conclusions: The crossed flap technique is a useful and safe alternative for breast reconstruction surgeries after definitive explantation.

Introdução: Em 1963 Cronin e Gerow introduziram o uso do implante de silicone e seu uso aumentou exponencialmente. Contudo, complicações relacionadas aos implantes surgiram ao longo do tempo. O conjunto de situações adversas ao uso dos implantes de silicone, alimentado pelo crescimento das mídias sociais, culminou em um aumento da retirada definitiva do implante. Muitos casos de explante têm o pedículo inferior comprometido pela lesão dos vasos perfurantes e a técnica dos retalhos cruzados é uma alternativa para a reconstrução das mamas explantadas.
Métodos: Foram realizados explantes de silicone com reconstrução imediata da mama sem o uso de um novo implante, motivados por indicação médica ou por desejo próprio do paciente. A técnica dos retalhos cruzados foi utilizada em todos os casos. Ela se vale do cruzamento de retalhos parenquimatosos de pedículo superior, um medial e outro lateral, conforme descrito por Sperli.
Resultados: Foram operados 10 casos de 2004 a 2021. O tempo de uso das próteses variou de 3 a 19 anos e a principal motivação para o explante foi contratura capsular. Nenhum caso de necrose foi observado.
Conclusões: A técnica dos retalhos cruzados é uma alternativa útil e segura para as cirurgias de reconstrução da mama após explante definitivo.

Modified double space technique for augmentation mastopexy

ABSTRACT

Introduction: Mastopexy with prosthesis remains a challenge for all plastic surgeons. Consistent results are still difficult to obtain for many reasons, and some authors advocate caution due to complications.
Methods: This retrospective study includes 92 consecutive single-stage augmentation mastopexy cases performed by the same surgeon between March 2012 and October 2019.
Results: The median follow-up was 14 months for the augmentation mastopexy group. Three patients (3.3%) had grade III capsular contracture. The revision rate was 25.3%: 7.6% with ptosis recurrence, scar revision in 6.5%, and 1.1% asymmetries. Three patients (3.3%) had ruptured muscle loop fixation, four patients (4.4%) had dynamic deformities.
Conclusion: The modified double-space technique in our experience shows consistent long-term results, and revision/complication rates were similar to some studies but higher than others. Prior breast surgery, smoking, breastfeeding, and previous bariatric surgery do not increase rates of complications and revisions. It can be one of the options for covering and supporting implants in augmentation mastopexy procedures.

RESUMO

Introdução: A mastopexia com prótese continua sendo um desafio para todos os cirurgiões plásticos. Resultados consistentes ainda são difíceis de se obter por muitas razões e alguns autores defendem a cautela devido a complicações.
Métodos: Este estudo retrospectivo inclui 92 casos consecutivos mastopexia de aumento em tempo único realizados pelo mesmo cirurgião, entre março de 2012 e outubro de 2019.
Resultados: O seguimento médio foi de 14 meses para o grupo de mastopexia de aumento. Três pacientes (3,3%) tiveram contratura capsular grau III. O índice de revisões foi de 25,3%: 7,6% com recidiva da ptose, revisão de cicatrizes em 6,5%, 1,1% de assimetrias. Três pacientes (3,3%) apresentaram ruptura da fixação da alça muscular, quatro pacientes (4,4%) tiveram deformidades dinâmicas.
Conclusão: A técnica modificada de duplo espaço apresenta em nossa experiência resultados consistentes a longo prazo, e as taxas de revisão/complicação foram semelhantes a alguns estudos, mas maiores do que outros. Cirurgia prévia da mama, tabagismo, amamentação e cirurgia bariátrica anterior não aumentam as taxas de complicações e revisões. Pode ser uma das opções para a cobertura e suporte dos implantes em procedimentos de mastopexia de aumento.

Intramuscular plane for breast augmentation with a silicone implant

ABSTRACT

Introduction: Breast augmentation is a frequent aesthetic procedure in plastic surgery; the ideal plane has not yet been found. Each pocket has its indication, its advantages, and disadvantages. The intramuscular pocket technique was initially described for use in buttock augmentation surgeries with silicone implants, and later its safety was reinforced with the use of the XYZ method. This study aims to present the use of the intramuscular technique in breast augmentation with a silicone implant.
Methods: This study describes the intramuscular pocket technique in breast augmentation with a silicone implant used in 50 female patients.
Results: The intramuscular technique provides a good definition of aesthetic contour in the medial quadrants of the breasts. We had a case of seroma in the postoperative period caused by a technical error in separating the fascicles.
Conclusion: The technique is safe and has a low incidence of complications.

Introdução: O aumento mamário é um procedimento estético frequente na cirurgia plástica, o plano ideal ainda não foi encontrado. Cada loja tem sua indicação, suas vantagens e desvantagens. A técnica da loja intramuscular foi inicialmente descrita para uso em cirurgias de aumento dos glúteos com implantes de silicone, e mais tarde sua segurança foi reforçada com a utilização do método XYZ. O estudo tem a finalidade de apresentar a utilização da técnica intramuscular na mamoplastia de aumento com implante de silicone.
Métodos: Este estudo descreve em detalhes a técnica da loja intramuscular na mamoplastia de aumento com implante de silicone, utilizada em 50 pacientes do sexo feminino.
Resultados: A técnica intramuscular propicia uma boa definição de contorno estético nos quadrantes mediais das mamas. Tivemos um caso de seroma no pós-operatório, causado por erro técnico na separação dos fascículos.
Conclusão: A técnica é segura e apresenta baixa incidência de complicações.

ABSTRACT

Introduction: Diastasis of the rectus muscles is a major complaint. In abdomens with smaller flaccidity the infraumbilical approach may even become unnecessary. We demonstrate the use of the hemostatic net of Auersvald for fixation cutaneous flap in the reverse approach for the correction of diastasis of the rectus muscles.
Case: ANF, 34 years old, without comorbidities, BMI 20.7, two pregnancies, complaining of bulging of the supraumbilical region and ptosis of the breasts. The proposed surgery was mastopexy with placement of the implants in the partial submuscular plane, associated with diastasis correction by reverse approach. We perform continuous suture with 3-0 transdermal Mononylon thread throughout the detachable flap extension. The transfixing suture was removed 48 hours after the procedure.
Discussion: We proposed a variation of the reverse approach and use of the transcutaneous fixation suture that allowed the fixation of the cutaneous flap to the region where the diastasis of the rectus muscles was corrected.

RESUMO

Introdução: A diástase dos músculos retos abdominais é motivo de grande queixa. Em abdomes com flacidez menor, a abordagem infraumbilical pode mesmo tornar-se desnecessária. Demonstramos a utilização da rede hemostática de Auersvald para fixação do retalho cutâneo na abordagem reversa para a correção da diástase dos retos abdominais.
Caso: ANF, 34 anos, sem comorbidades, IMC 20,7, duas gestações prévias, com queixa de abaulamento da região supraumbilical e ptose das mamas. A cirurgia proposta foi a mastopexia com colocação dos implantes no plano submuscular parcial, associada à correção da diástase dos músculos retos abdominais por abordagem reversa. Realizamos sutura contínua com fio Mononylon 3-0 transdérmico em toda a extensão descolada do retalho. A sutura transfixante foi removida 48 horas após o procedimento.
Discussão: Propusemos uma variação da abordagem reversa e utilização da sutura de fixação transcutânea que permitiu a fixação do retalho cutâneo à região da correção da diástase dos músculos retos.

ABSTRACT

INTRODUCTION: Mammoplasty is the most commonly performed plastic surgery procedure in Brazil, which is second only to the United States in terms of the number of mammoplasties performed. Several techniques based on increasing volume using breast implants are used to correct breast ptosis. We aimed to describe a technical variation of implant mammoplasty: isolation of the implant in a closed pocket of fascioglandular tissue (Lockpocket). METHODS: This prospective study was carried out between June 2013 and June 2016. Forty-three patients underwent (1) augmentation mammoplasty using a silicone implant that was isolated from the external environment by a closed pocket of fascioglandular tissue, and (2) resection of excess dermoglandular tissue to correct mammary ptosis. Were subsequently analyzed statistically. RESULTS: Of the 43 patients, the majority (22 patients) presented grade II ptosis according to the Regnault classification. The volume of glandular tissue removed was similar to the volume of the prosthesis introduced. A total of seven complications were observed: partial dehiscence (n = 4), discrete asymmetry (n = 2), and residual ptosis (n = 1). CONCLUSION: Augmentation mammoplasty with correction of mammary ptosis (the Lockpocket technique) is a good option because it allows the surgeon to choose in advance the volume of the implant to be used. The method also isolates the implant in a totally closed plane of fascioglandular tissue, and involves the precise removal of breast tissue.

RESUMO

INTRODUÇÃO: A mamoplastia foi o procedimento em cirurgia plástica mais realizado no Brasil, ficando em segundo lugar no mundo, logo atrás dos Estados Unidos. Existem diversas técnicas para correção de ptose das mamas associado a aumento do volume com implantes mamários. O objetivo é descrever uma variação técnica de mamoplastia com prótese, isolando o implante em um bolsão fechado de tecido fascioglandular (Lockpocket). MÉTODOS: Realizado um estudo prospectivo entre junho de 2013 e junho de 2016 totalizando 43 pacientes que foram submetidos à mamoplastia de aumento com prótese de silicone, isolado do meio externo por um bolsão fechado de tecido fascioglandular, e ressecção de tecido dermoglandular excedente para correção de ptose mamária, sendo realizada análise estatística. RESULTADOS: Das 43 pacientes, a maioria (22 pacientes) apresentaram ptose grau II, segundo a classificação de Regnault. Os volumes de tecido glandular retirado e volume das prótese introduzidos foram semelhantes, sendo observado um total de sete complicações: deiscência parcial (n = 4), assimetria discreta (n = 2) e ptose residual em um caso. CONCLUSÃO: A mamoplastia de aumento com correção de ptose mamária - técnica Lockpocket - é uma boa opção, permitindo a escolha prévia do volume do implante utilizado, isolando-o em uma loja totalmente fechada de tecido fascioglandular, e exérese exata de tecido mamário.

ABSTRACT

Breast reconstruction techniques are fundamental to plastic surgeon arsenal, since breast cancer accounts for approximately 28% of cancer new cases in Brazil. Treatment, in most cases, involves mastectomy and, when indicated, breast reconstruction. Among reconstructive techniques, we will report the double muscle flap of pectoralis major and anterior serratus, which provide a suitable store for a good number of reconstructions with prosthesis, in skin sparing mastectomies. Reproducible technique, of relatively fast learning curve and important to plastic surgeon arsenal. We will present a case and review bibliography.

RESUMO

As técnicas de reconstrução de mama são fundamentais ao arsenal do cirurgião plástico, haja vista o câncer de mama ser responsável por aproximadamente 28% dos novos casos de câncer no Brasil. O tratamento, na imensa maioria dos casos, envolve a mastectomia e, quando indicada, sua reconstrução. Dentre as técnicas reconstrutivas, relataremos o duplo retalho muscular de peitoral maior e serrátil anterior, que fornecem uma loja adequada a um bom número de reconstruções com próteses, em mastectomias poupadoras de pele. Técnica reprodutível, de curva de aprendizado relativamente rápida e importante ao arsenal do cirurgião plástico. Apresentaremos um caso e revisão da bibliografia.

ABSTRACT

Introduction: Macromastias with fatty substitution, great flaccidity, and severe ptosis, constitute a specific group of difficult treatment, many without long-term results. The authors present the technique called Chassaignac bursa flap, indicated for selected cases.
Methods: When applied to 41 patients (2013-2019), the tactic allowed the creation of two independent spaces, one for the removal of the mammary parenchyma and the other isolated, pre-muscular, a flap composed by pedicles semicircular, superior, medial and lateral, dome-shaped, which protect the implant in the Chassaignac retromammary area.
Results: The patients evolved without complications, with the maintenance of the upper pole, a symmetrical harmonic redistribution of the breasts, without recurrent ptosis.
Conclusion: The bag flap proved feasible, allowing for safety during the operation and a lasting implant support.

Introdução: As macromastias com substituição gordurosa, grande flacidez e ptose severa, constituem um grupo específico de difícil tratamento, muitos deles sem resultados perduráveis a longo prazo. Os autores apresentam a técnica denominada retalho bolsa de Chassaignac, indicada para casos selecionados.
Métodos: Aplicada em 41 pacientes (2013-2019), a tática permite a criação de 2 espaços independentes, um para a retirada do parênquima mamário e outro isolado, pré-muscular, que consiste em um retalho composto, de pedículo semicircular, superior, medial e lateral, em forma de cúpula, que protege o implante na área retromamária de Chassaignac.
Resultados: As pacientes evoluíram sem complicações, com manutenção do polo superior, uma redistribuição harmônica simétrica das mamas, sem ptoses recidivantes.
Conclusão: O retalho em bolsa se mostrou factível, permitindo segurança no transoperatório e sustentação perdurável do implante.

Late seroma after silicone breast implants: three different forms of presentation, evolution, and approach

ABSTRACT

BACKGROUND: Late seromas after augmentation mammaplasty are uncommon, can manifest without a defined cause, and can be treated by implant removal or replacement. This study aimed to analyze three cases of this complication that occurred 1-10 years postoperatively and were treated differently. METHODS: Data of three patients who developed late seroma after breast implant placement were analyzed. In this report, we present data on the indication for implant placement, time without complications, implant type, and the analytical results of the removed or drained material. RESULTS: Two patients underwent bilateral implant removal, although only one side was affected, and the implant was replaced with another of the same type and volume in the third patient. One case of sterile pus was diagnosed. CONCLUSIONS: Before undergoing breast implant surgery, patients should be informed of the implications of their decisions, such as the possible need to remove or replace them, resulting in more surgical procedures and/or new scars.

RESUMO

INTRODUÇÃO: Os seromas tardios após mamoplastia de aumento são ocorrências pouco comuns, que podem se manifestar sem causa definida e cujo tratamento implica a retirada ou a troca das próteses. Este trabalho objetiva analisar 3 casos dessa complicação, ocorrida entre 1 ano e 10 anos de pós-operatório, tratados de formas distintas. MÉTODO: Foram analisados os dados de 3 pacientes que apresentaram seroma no pós-operatório tardio de inclusão de próteses de mama. Neste artigo são apresentados dados relativos a indicação da inclusão, tempo de evolução sem complicações, tipo de prótese e resultado da análise do material retirado ou drenado. RESULTADOS: Em 2 pacientes, foi realizada retirada bilateral das próteses, apesar de somente um dos lados ter sido afetado; na terceira paciente, procedeu-se à troca da prótese por outra de mesmo volume e tipo. Um dos casos foi diagnosticado como pus estéril. CONCLUSÕES: Previamente à inclusão de próteses mamárias, a paciente deve ser alertada para o fato de que sua decisão poderá ter implicações futuras, sendo, eventualmente, necessário trocá-las ou retirá-las, o que resultará em novas cirurgias e/ou novas cicatrizes.

Choosing the augmentation mammaplasty technique: a method for preventing medical disputes

BACKGROUND: Plastic surgery is a medical specialty that is particularly at risk of unwarranted claims. Most complaints regarding plastic surgery are not a consequence of technical failures but rather of inadequate criteria of patient selection, failure in surgical indication, and lack of effective communication between the patient and the surgeon. Training, careful use of techniques, adopting precautions, compliance with safety regulations, and the informed consent document are fundamental principles of defensive medical practice. Moreover, documenting objective information about the surgery in the medical records validates the work of the plastic surgeon and ensures that procedures are productive and technical. The aim of this study was to develop a tool for preventing medical litigations in augmentation mammaplasty based on data expressed on the scale from I to IV. METHODS: Forty patients with an indication for augmentation mammaplasty were evaluated. All the patients were subjected to anatomical measurements, which were photographically documented, and the hereditary and congenital diseases of the patients presenting for breast augmentation were recorded. In addition, all the patients were informed about the indication and surgical procedure to be performed. They agreed with the surgical indication and provided informed consent. RESULTS: The follow-up period was 6 months. All the patients expressed a high degree of satisfaction with the results obtained after the procedure, and no cases of complaints or disputes were encountered. CONCLUSIONS: The authors propose the regular use of tables and classifications for the selection of implants, with the aim of establishing surgical indication as an objective parameter, facilitating patients' understanding of the procedure, and preventing litigation.

RESUMO

INTRODUÇÃO: A cirurgia plástica é uma das especialidades médicas expostas ao risco de reivindicações abusivas. A maioria das queixas em cirurgia plástica não decorre de falhas técnicas, mas de inadequados critérios de seleção de pacientes, falha na indicação cirúrgica e falta de comunicação adequada entre o paciente e o cirurgião. O treinamento, a utilização cuidadosa de técnicas, a adoção de precauções, o cumprimento de normas de segurança e o formulário de consentimento informado são princípios fundamentais na prática da medicina defensiva. Ademais, ter nos registros dos pacientes informações objetivas sobre a cirurgia realizada avaliza o trabalho do cirurgião plástico, tornando os procedimentos profícuos e técnicos. O objetivo deste estudo é descrever uma ferramenta de prevenção de litígios médicos na mastoplastia de aumento baseada em dados de escala em graus I a IV. MÉTODO: Foram avaliadas 40 pacientes com indicação de mastoplastia de aumento. Todas as pacientes foram submetidas a mensurações anatômicas, documentação fotográfica e registro de doenças hereditárias e congênitas encontradas nas pacientes candidatas a aumento mamário. Além disso, todas as pacientes receberam explicação da indicação e do procedimento cirúrgico a ser realizado.As pacientes concordaram com a indicação cirúrgica e assinaram o termo de consentimento. RESULTADOS: O tempo de seguimento das pacientes foi de 6 meses. Todas as pacientes apresentaram alto grau de satisfação com o resultado obtido após o procedimento e não houve caso de queixa ou litígio. CONCLUSÕES: Os autores propõem o uso regular de tabelas e classificações para escolha prévia dos implantes, com o objetivo de tornar a indicação cirúrgica objetiva, facilitar o entendimento da paciente e prevenir o litígio.

Breast discomfort after augmentation mammoplasty: a case report

ABSTRACT

Mammoplasty with prostheses is one of the most performed plastic surgeries in the world. However, the healing process around the implant and the presence of a biofilm can lead to the development of pathologies such as capsular contracture and seroma. These pathologies seem to be physiologically related to the development of lymphoma associated with breast implants (BIA-ALCL), which is also a differential diagnosis. The purpose of this study is to report the case of a patient with breast discomfort who, after two previous surgeries for seroma drainage and prosthesis replacement, presented breast discomfort and alteration in imaging of the left breast. She was, submitted to a capsulectomy in a block of the left breast and complete on the right, having her prostheses replaced. Bia-ALCL investigation tests were negative and pathological findings were suggestive of left capsular contracture and double capsule formation on the right. The study emphasizes the importance of differential diagnosis in mammary pathologies, long-term follow-up, and prophylaxis measures in biofilm formation.

RESUMO

A mamoplastia com próteses é uma das cirurgias plásticas mais realizadas no mundo. O processo cicatricial ao redor do implante e a presença de um biofilme pode acarretar o desenvolvimento de patologias como contratura capsular e seroma. Essas patologias parecem estar relacionadas fisiopatologicamente com o desenvolvimento do linfoma associado aos implantes mamários (BIA-ALCL), sendo este também um diagnóstico diferencial. A proposta deste trabalho é relatar o caso de uma paciente com desconforto mamário, que após 2 cirurgias prévias para drenagem de seroma e troca de próteses, apresentava desconforto mamário e alteração em exames de imagens da mama esquerda. Sendo submetida a uma capsulectomia em bloco da mama esquerda e completa à direita, tendo suas próteses substituídas. Os exames para investigação de BIA-ALCL foram negativos e os achados patológicos foram sugestivos de contratura capsular à esquerda e formação de dupla cápsula à direita. O trabalho enfatiza a importância do diagnóstico diferencial em patologias mamárias, o acompanhamento a longo prazo e medidas de profilaxia na formação do biofilme.

ABSTRACT

INTRODUÇÃO: Capsular contracture is one of the most common causes of surgical revision in the mammary augmentation procedure. This phenomenon can happen with any kind of implant, or layer of the pocket. The literature shows that there is an increased prevalence in the implant in the retro mammary position, and for this reason the majority of patients whom operate for treating contracture are in this situation. In these patients we observe a significant thinning of the gland and the subcutaneous tissue, then, we create a rectropectoralis pocket to insert the new implant making use of a capsular flap, to involve it completely. This allows larger implants house with great security, nice cosmetic results and high satisfaction.

RESUMO

INTRODUÇÃO: Este trabalho propõe, nos casos de contratura capsular em mamoplastias de aumento retroglandulares, a inclusão de novos implantes no plano retropeitoral, empregando retalhos de cápsula para sua fixação. MÉTODO: Foram avaliadas, retrospectivamente, 27 pacientes portadoras de contratura capsular tratadas desta forma entre maio de 2007 e junho de 2012. RESULTADOS: A técnica utilizada permitiu abrigar implantes maiores com bom aspecto de contorno mamário, assegurados pela cobertura muscular, estabilizada pelo retalho capsular. CONCLUSÕES: A técnica apresentada é um método seguro, reprodutível, tecnicamente simples, de resultados satisfatórios e baixa prevalência de intercorrências.

ABSTRACT

BACKGROUND: The correction of breast ptosis associated with skin sagging and low projection is still a subject of controversy in the literature. This study aims to describe the experience of the Plastic Surgery Department of Universidade Estadual de Campinas (Unicamp) with the technique of mastopexy with breast implants in a double plane and the pectoralis major muscle flap. METHODS: A retrospective study of 20 patients with grade II or III mammary ptosis, who underwent surgery between June 2008 and September 2010, was performed. RESULTS: A 9- and 12-month follow-up of patients showed neither breast or chest deformities nor recurrence of ptosis. All patients presented with good breast projection and adequate upper pole fill, with long-lasting and satisfactory results. CONCLUSIONS: Mastopexy with breast implants and the pectoralis major muscle flap technique is easy to perform, with a relatively short learning curve, good reproducibility, and satisfactory long-lasting results.

RESUMO

INTRODUÇÃO: A correção da ptose mamária associada à flacidez de pele e baixa projeção é ainda tema de discussões e controvérsias na literatura. O objetivo deste estudo é descrever a experiência da Disciplina de Cirurgia Plástica da Universidade Estadual de Campinas (Unicamp) com a técnica de mastopexia com implante mamário associado a retalho de sustentação do músculo peitoral maior. MÉTODO: Foi realizado estudo retrospectivo de 20 pacientes com ptose mamária graus II ou III, operadas no período de junho de 2008 a setembro de 2010. RESULTADOS: Após seguimento pós-operatório entre 9 meses e 12 meses, não foram observados casos de deformidades mamárias ou torácicas e nenhuma paciente evoluiu com recidiva da ptose. Foram observados, em todas as pacientes, manutenção de boa projeção da mama e adequado preenchimento do polo superior mamário, gerando resultados duradouros e satisfatórios. CONCLUSÕES: A técnica de mastopexia com uso de implantes associados a retalho de músculo peitoral maior é de fácil realização, com curva de aprendizado relativamente curta, boa reprodutibilidade, e resultados duradouros e satisfatórios.

Reoperações após mamoplastias redutoras e mastopexias associadas a implantes de silicone

RESUMO

Introdução: A mamoplastia redutora e a mastopexia associadas à colocação de implantes de silicone ganhou muito popularidade na última década. A expectativa das pacientes com o resultado destas operações é muito alta, este fator aliado a um maior índice de complicações leva a uma incidência alta de reoperações.
Método: Revisão de prontuários, entre outubro de 2016 e janeiro de 2018, de pacientes insatisfeitas com o resultado de uma mastopexia ou mamoplastia redutora associadas a implantes de silicone.
Resultados: Foram incluídos prontuários de 14 pacientes. A posição dos implantes foi subglandular em 12 casos e submuscular em 2 casos. O principal motivo de insatisfação, responsável por 78,5% das queixas, foi a perda de projeção do polo superior com abaulamento do polo inferior. A reoperação não foi indicada em 5 pacientes. Em 9 casos a reoperação foi indicada pelo autor. Destes, 5 pacientes foram reencaminhadas para o cirurgião que realizou a cirurgia primária e não retornaram ao consultório, e 4 pacientes foram reoperadas pelo autor.
Conclusão: O índice elevado de complicações e a alta exigência das pacientes com o resultado faz com que muitas pacientes fiquem insatisfeitas com o resultado e sejam candidatas a reoperação. A maior queixa é a perda de projeção no polo superior e o abaulamento do polo inferior, porém, cerca de um terço das pacientes insatisfeitas possuem resultados acima do padrão esperado. O cirurgião plástico deve orientar suas pacientes sobre os riscos das mamoplastias redutoras e mastopexias associadas aos implantes de silicone e sobre os resultados.

Risk factors resulting from complications in the postoperative period of primary breast augmentation with silicone implants: Retrospective study

ABSTRACT

Introduction: Breast augmentation is one of the most performed plastic surgeries in the world. Given its high applicability, it is essential to know the risk factors related to the incidence of postoperative complications to reduce them in this type of procedure.
Method: This retrospective study analyzed the medical records of 76 patients who underwent primary breast augmentation with silicone implants from January 2018 to December 2020.
Results: It was observed that the presence of comorbidities increased the incidence of early postoperative complications (p<0.001) but not of late postoperative complications (p=0.8). Longer surgical time also increased the incidence of postoperative complications (p=0.005).
Conclusion: The presence of comorbidities directly influences the incidence of postoperative complications after breast augmentation, and for this reason, adequate preoperative compensation must be carried out before submitting patients to this surgical procedure.

Introdução: A mamoplastia de aumento é uma das cirurgias plásticas mais realizadas no mundo. Tendo em vista sua alta aplicabilidade, é fundamental conhecer os fatores de risco relacionados à incidência de complicações pós-operatórias a fim de reduzi-las nesse tipo de procedimento.
Método: Este estudo é retrospectivo feito por meio da análise dos prontuários de 76 pacientes que realizaram mamoplastia de aumento primária com implantes de silicone durante os meses de janeiro de 2018 a dezembro de 2020.
Resultados: Observou-se que a presença de comorbidades aumentou a incidência de complicações pós-operatórias precoces (p<0,001), mas não de complicações pós-operatórias tardias (p=0,8). O maior tempo cirúrgico também aumentou a incidência de complicações pós-operatórias (p=0,005).
Conclusão: A presença de comorbidades influencia diretamente na incidência de complicações pós-operatórias da mastoplastia de aumento e por esta razão deve-se realizar uma adequada compensação pré-operatória antes de submeter as pacientes a este procedimento cirúrgico.
