Histopathological evaluation of polymethylmethacrylate in rats for one year

ABSTRACT

Introduction: PMMA is used as a permanent filler in the form of microspheres. There are several commercial applications; many of them are used indiscriminately. Objective: Histological evaluation (of the applied place and organs) after PMMA filling in rats. Methods: 60 Wistar rats were used; each received an application of 1 ml of PMMA (Meta-Crill®) in deep frame (near the occipital region). The euthanasia occurred in groups of 15 rats at 1, 3, 6 and 12 months after application. Results: Presence of inflammatory infiltrate and fibrosis in the injected site, in addition to the reduced amount of PMMA in all samples. Foreign body reaction (FBR) granuloma type occurred in 3.3% of samples (2 mice). Statistical analysis was shown significant by Kruskal-Wallis. Conclusion: The PMMA used had a higher FBR and evident fibrosis in late samples, occurring FBR granuloma type. Although the PMMA seems safe, several complications can occur. Its use should be judicious, knowing that granulomas occur regardless of the technique.

RESUMO

Introdução: O PMMA é utilizado como preenchedor permanente na forma de microesferas. Existem diversas apresentações comerciais, muitas são utilizadas indiscriminadamente. Objetivo: Avaliação histológica (do local aplicado e órgãos) após preenchimento com PMMA em ratos. Método: Utilizaram-se 60 ratos Wistar, todos receberam aplicação de 1 ml de PMMA (Meta-Crill®) em plano profundo, próximo à região occipital. A eutanásia ocorreu em grupos de 15 ratos no 1º, 3º, 6º e 12º mês após aplicação. Resultados: Identificada presença de infiltrado inflamatório e fibrose no local injetado, além da diminuição da quantidade de PMMA em todas as amostras. Reação de corpo estranho (RCE) tipo granuloma ocorreu em 3,3% das amostras (2 ratos). A análise estatística mostrou-se significante pelo teste de Kruskal-Wallis. Conclusão: O PMMA utilizado apresentou maior RCE e evidente fibrose em amostras tardias, podendo ocorrer RCE tipo granuloma. Embora o PMMA pareça seguro, diversas complicações podem ocorrer. Sua utilização deve ser criteriosa, sabendo que granulomas ocorrem independente da técnica.

Rhytidoplasty practices and histological notes for the facial skin of post-bariatric patients

ABSTRACT

Introduction: The number of patients classified as overweight or obese has resulted in an increased number of post-bariatric patients, generating a demand for rhytidoplasty in this public profile. expose the routine and surgical tactics in a teaching hospital, patient profiles, subcutaneous cellular tissue, and the superficial musculoaponeurotic system are compared between post-bariatric and non-post-bariatric patients, as well as histological superficial comparison of the skin.
Methods: A review of the data of 32 cases treated in 2012-2016 was conducted in a public hospital in Goiânia-GO.
Results: The tactics of rhytidoplasty used in post-bariatric patients do not differ significantly from those used in nonpost- bariatric patients. A histopathological analysis of the collected samples showed marked differences between post-bariatric and non-post-bariatric patients.
Conclusion: The surgical technique of rhytidoplasty of post-bariatric patients sustained specific details with satisfactory postoperative surgical results, skin with worse histological quality, perioperative complications within the expected range, and the need for further studies to assess the procedure's durability

RESUMO

Introdução: O número de pacientes classificados com sobrepeso ou obesos e, com isso, dos pacientes pós-bariátricas vem aumentando, gerando uma demanda por ritidoplastias neste perfil de público. O objetivo é expor a rotina e táticas cirúrgicas em um hospital de ensino, o perfil destes pacientes, além de um comparativo superficial histológico da pele, tecido celular subcutâneo e SMAS, entre os pacientes pós e não pós-bariátrica.
Métodos: Foi realizado revisão de dados do centro cirúrgico e de prontuário entre os anos de 2012 e 2016 em um hospital público na cidade de Goiânia-GO, com levantamento de 32 casos.
Resultados: A tática de ritidoplastia utilizada nos pacientes pós-bariátrica não apresenta grandes diferenças entre a utilizada nos pacientes não pós-bariátrica. A análise histopatológica das amostras colhidas evidenciou diferenças marcantes entre os pacientes pós e não pós-bariátrica.
Conclusão: Percebeu-se que a técnica operatória na ritidoplastia do paciente pós-bariátrica sustentou detalhes específicos, com resultado cirúrgico pós-operatório satisfatório, uma pele de pior qualidade histológica, com complicações perioperatórias dentro do esperado e com necessidade de maiores estudos para avaliar durabilidade do procedimento.

Anatomic Study of the Communicating Venous System of the Anteromedial Region of the Leg

ABSTRACT

This study demonstrates that drainage, important for the anteromedial fasciocutaneous flap of the leg, the communicating veins that unite the superficial to the deep sysrem of the oa:ompanying veins of the posterior tibial artery, continually poss between the soleus and flexor longus muscles and Ihat there are two communicating veins for each artery. The major number of communicating vessels are found in the lower third ofthe leg up to ten cm above the medial maleolus. In addition, the more proximal the communicating vessels are to the anterior wberosity of the tibia, the greater their diameter will be. No significant differences were encountered regarding the researched parameters, based on right or left side.

RESUMO

Os achados do presente estudo vêm demonstrar que, em termos de drenagem venosa com importância ao retalho fasciocutâneo ântero-medial da perna, as veias comunicantes que unem o sistema superficial ao profundo das veias acompanhantes da artéria tibial posterior passam constantemente por entre os músculos sóleo e flexor longo dos dedos e que há duas veias comunicantes para cada artéria. Verificou-se que o maior número de comunicantes se encontra no terço inferior da perna, até 10 cm acima do maléolo medial, e que quanto mais proximais da tuberosidade anterior da tíbia estiverem as comunicantes, maior será seu calibre. Também não encontramos diferenças importantes quanto aos parâmetros pesquisados segundo a lateralidade.

Effect of Autonomization Surgical Techniques on Transverse Rectus Abdominis Myocutaneous Flaps - Experimental Study in Female Rats

ABSTRACT

A study was conducted on the effect of autonomization surgical techniques on monopediculated transverse rectus abdominis mycutaneous flaps in female rats. Animals in Group A (control) were submitted to the flap procedure proposed for the study, consisting of an incision, removal of the fragment in area 3 and detachment of the flap up to the lower abdomen, followed by on site suture. Animals in groups B, C, D, E, F, G, and H were submitted to different autonomization surnical techniques, seven days before the flap procedure proposed for the study. The area of necrosis in relation to the total flap healing area was assessed by using a sheet of standard graph paper, a fine point Pilot® pen and a transparent cast. A light microscopy histological study was performed on the fragments removed from the contra lateral end of the vascular pedicle of the flap. The autonomization surgical technique that divided the cranial portion of rectus abdominis muscles bilaterally and cauterized both upper deep epigastric vessels used on group C animals was more effective in reducing the area of necrosis of the flap studied, and did not result in histological changes in the fragments examined.

RESUMO

Estudou-se o efeito das técnicas operatórias de autonomização utilizadas no retalho musculocutâneo transverso do músculo reto do abdome monopediculado de ratas. Os animais do grupo A (controle) foram submetidos à operação do retalho proposto para o estudo, que consistiu na incisão, remoção do fragmento na área 3 e descolamento do retalho até o abdome inferior, seguido de sutura no próprio leito. Os animais dos grupos B, C, D, E, F, G e H foram submetidos a diferentes técnicas operatórias de autonomização, sete dias antes da operação do retalho proposto para o estudo. Avaliou-se a porcentagem de área de necrose em relação à área total do retalho em cicatrização, utilizando-se uma folha milimetrada padronizada, uma caneta Pilot® de ponta fina e um molde transparente. Um estudo histológico, por microscopia de luz, foi realizado nos fragmentos removidos da extremidade contralateral ao pedículo vascular do retalho. A técnica operatória de autonomização que realizou divisão bilateral cranial dos músculos retos do abdome e cauterização de ambos os vasos epigástricos superiores profundos utilizada nos animais do grupo C foi a mais eficaz na redução da área de necrose do retalho estudado, não determinando alterações histológicas nos fragmentos examinados.

Histological study of the human dermis: interesting aspects for liposuction

ABSTRACT

Objective: The authors proposed to study the distribution of collagen fibers in the papillary and reticular layers of the human dermis. Method: Twenty fresh cadavers were utilized, from which skin samples were removed. The studied regions were: the anterior cervical (1); anterior brachial (2); vertebral interscapular (3); and abdomen (4) regions, observing the morphometric reference points. The specimens were analysed and the results were submitted to statistical calculations. Results: The average concentration of collagen fibers was 62 % in the papillary layer of the dermis and 88.5% in the reticular layer. The distribution of collagen fibers in the papillary layer of the dermis was of a similar dense homogenous type in all the regions studied. However, in the reticular layer the distribution varied between regions and individuals. Matted types were found more frequently in region 1 (45%), parallel in regions 2 and 4 (45%) and chaotic in region 3 (50%). Conclusion: The collagen fibers presented different disposition in the human dermis. The authors believe that this layer might have an influence on skin repositioning after liposuction.

RESUMO

Objetivo: Os autores pesquisaram a distribuição das fibras colágenas nas camadas papilar e reticular da derme humana. Método: Vinte cadáveres frescos foram utilizados para a retirada de amostras de pele de diferentes regiões usualmente submetidas à lipoaspiração. As regiões estudadas foram: cervical anterior (1); braquial anterior (2); vertebral interescapular (3); e abdominal (4), observando pontos de referência morfométricos. Os espécimes foram analisados e os resultados submetidos a cálculos estatísticos. Resultados: A concentração média de fibras colágenas foi 62% na camada papilar e 88,5% na camada reticular. A distribuição das fibras colágenas na camada papilar teve densidade similar e tipo homogêneo em todas as regiões. Entretanto, a camada reticular teve distribuição de fibras colágenas variando de acordo com as regiões e os indivíduos. Disposição em emaranhado foi encontrada mais freqüentemente na região 1 (45%), disposição em paralelo foi encontrada de forma semelhante nas regiões 2 e 4 (45%) e, na região 3, houve predomínio do tipo anárquico (50%). Conclusão: A observação de diferentes variações na disposição de fibras colágenas na derme humana levou os autores a acreditar na sua provável relação com o reposicionamento da pele observado no pós-operatório da lipoaspiração.

Applied anatomy of the branch of the facial nerve: study of the potential risk of injury during facial rejuvenation surgery

ABSTRACT

Background: The temporal branch of the facial nerve is one of the nerves more commonly injured due to the scarce subcutaneous tissue that protects it from its exit at the parotid gland. Method: Twenty five cadaveric hemifaces were dissected allowing a analysis of the temporal branch and: parotid gland, zygomatic arch, SMAS, superficial temporal artery and frontal muscle. Results: Twelve temporal branches exposed at a distance 1.7cm anterior to the trago. Two temporal branches crossing the zygomatic arc was the most frequent finding.This passage occurred between 3.2 to 3.9 cm posterior to the lateral border of the orbit. The trajectory of the temporal branch near the deep side of the SMAS and temporofacial fascia and above the superficial layer of the deep temporal fascia was constant. The frontal branch of the superficial temporal artery was superior and parallel to the nerve trajectory in 92% of the dissections Conclusion: The temporal branch follow a constant plane along the undersurface of the temporoparietal fascia and is quite superficial as it cross the zygomatic arch.

RESUMO

Introdução: O ramo temporal do nervo facial é um dos nervos mais comumente lesados, devido à pouca tela subcutânea que o protege a partir da sua saída da glândula parótida. Método: Vinte e cinco hemifaces de cadáveres foram dissecadas e analisadas as relações entre o ramo temporal e glândula parótida, arco zigomático, SMAS, artéria temporal superficial e músculo frontal. Resultados: Doze ramos temporais dissecados perderam a proteção da glândula parótida a uma distância de 1,7 cm anterior ao trago. O cruzamento do arco zigomático por dois ramos temporais foi o mais frequente. A passagem pelo arco zigomático ocorreu entre 3,2 e 3,9 cm posteriores à borda lateral da órbita. O curso do ramo temporal junto às faces profundas do SMAS e da fáscia temporoparietal, e acima da lâmina superficial da fáscia temporal profunda foi constante. O ramo frontal da artéria temporal superficial foi superior e sua trajetória paralela ao ramo temporal em 92% das dissecções. Conclusão: O ramo temporal do nervo facial segue um plano constante ao longo da face profunda da fáscia temporoparietal e está muito superficial quando cruza o arco zigomático.

Morphology and anatomical variations of the frontal bundles from the occipitofrontal muscle: anatomical study and classification

ABSTRACT

Introduction: The development of muscles, myocutaneous, osteomiocutaneous, and the variety of microsurgical flaps, added to the new aesthetic procedures on frontal bellies, became essential the well knowledge of this region. The studied in this paper covered the detailed morphology of the frontal and their anatomical variations, that can serve as useful reference points in reparative and cosmetic surgery. Methods: We studied 56 non-fixed cadavers, 35 men and 21 women, 37 white and 19 non-white, with age ranging from 19 to 85 years old. In this material, 112 muscles were dissected. Results: Data were available in tables followed by the parameters of sex, age, race, interventricular distances, heights and widths of the frontal bilaterally. The widths, thickness and interventricular distances were measured at 1.5, 3.0 and 4.5 cm from the glabella in both wombs. The frontal were presented in different ways, especially regarding the disposition of the medial and were grouped into four types. RESUMO

Introdução: Com o desenvolvimento dos retalhos musculares, miocutâneos, osteomiocutâneos, microcirúrgicos e os novos procedimentos estéticos sobre o músculo occipitofrontal, tornou-se essencial o conhecimento anatômico desta região. Estudamos neste trabalho a morfologia detalhada dos ventres frontais e suas variações anatômicas, para que possam servir como pontos de referência úteis em cirurgias reparadoras e estéticas. Método: Foram estudados 56 cadáveres não fixados, 35 homens e 21 mulheres, 37 de raça branca e 19 nãobranca, com idade variando de 19 a 85 anos. Neste material foram dissecados 112 ventres musculares. Resultados: Os dados obtidos foram colocados em tabelas, seguindo-se os parâmetros de sexo, idade, raça, distâncias interventrais, larguras, alturas e espessuras dos ventres frontais bilateralmente. As larguras, espessuras e distâncias interventrais foram medidas a 1,5, 3,0 e 4,5 cm da glabela, em ambos os ventres. Os ventres frontais apresentaram- se de maneiras diferentes, principalmente quanto à disposição das fibras mediais, sendo agrupados em 4 tipos. Conclusões: O amplo conhecimento anatômico nos permite um planejamento pré-operatório mais seguro.

ABSTRACT

SUMMARY: Introduction The authors refer to developments in the treatment of facial aging. OBJECTIVE: Through an incision marginal temporal intracapillary reduce sagging eyelids and raise the tail of the eyebrow, making economic blepharoplasty. METHOD: We propose performing rhytidectomy segment temporal incision intra capillary marginal curve, facilitating compensation skin, within 0.5 cm of the scalp, it desepidermizando 2 mm. Removing fat conclude the edge of the flap and performing suture tricofítica and camouflaging the best scar, which becomes pre capillary by growth of hair along it. Detachment in the subcutaneous plane to the edge of the lateral half of the orbicularis, traction suture and lateral-superior range with separate points of it. Other procedures for the remainder of the face and neck were performed when necessary. The lateral traction reduces sagging eyelid skin and its excesses, making blepharoplasty small economical resections with skin, and removal of bags for minimal dilatation individualized orbicularis muscle at the site of projection of the same. RESULTS: We obtained good results by reducing the stigma of aging orbital region. It is a simple procedure of low morbidity, leaving scar marginal temporal good quality for small skin traction and easily camuflável by the hair growing along her downward covering it. 485 cases were operated from 1997 to 2012. CONCLUSION: The method is easy to perform, low morbidity, scarring of good quality even in young people, reducing needs detachments of the eyelids.

RESUMO

INTRODUÇÃO: Os autores referem à evolução no tratamento do envelhecimento facial. OBJETIVO Por meio de incisão marginal intracapilar temporal reduzir a flacidez palpebral e elevar a cauda do supercílio, tornar a blefaroplastia econômica. MÉTODO: Propõem realizar a ritidoplastia do segmento temporal com incisão intracapilar marginal curva, facilitando as compensações de pele, 0,5 cm dentro do couro cabeludo, desepidermizando 2 mm dele. Concluem o procedimento, desengordurando o bordo do retalho e realizando sutura tricofítica, camuflando melhor a cicatriz, que se torna pré-capilar, pelo crescimento de cabelos junto dela. Descolamento no plano subcutâneo até o bordo da metade lateral do músculo orbicular, tração e sutura latero-superior em leque com pontos separados do mesmo. Outros procedimentos no restante da face e pescoço quando necessários foram realizados. A tração lateral reduz a flacidez palpebral e seus excessos de pele, tornando a blefaroplastia econômica, com ressecções pequenas de pele, e remoção das bolsas por mínima divulsão individualizada do músculo orbicular, no local da projeção das mesmas. RESULTADOS: Obtiveram bons resultados, reduzindo o estigma do envelhecimento da região orbitaria. Trata-se de um procedimento simples, de morbidade baixa, deixando cicatriz marginal temporal de boa qualidade, pela pequena tração da pele, é facilmente camuflável pelos cabelos que crescem junto dela, em direção inferior, cobrindo-a. Foram operados 485 casos de 1997 a 2012. CONCLUSÃO: O método é de fácil execução, baixa morbidade, cicatrizes de boa qualidade mesmo em jovens, reduzindo necessidades de descolamentos das pálpebras.

Innervation of the nipple-areolar complex after reduction mammaplasty: a histological study

ABSTRACT

INTRODUCTION: The periareolar dermal release maneuver in mammoplasty promotes better mobility of the nipple-areola complex. However, there are doubts on possible nerve damages in this kind of topography. This quantitative analysis compared the nerve branches density from the medial, lateral and caudal side-flow of the nipple-areola complex (NAC). METHODS: This was a prospective study. The study included 26 women who have undergone a mammaplasty reduction using the Pitanguy's classic technique. The dermis fragments collected from the medial, lateral and caudal sides were properly prepared and subjected to a histological study in order to determine the nerve branches density in each studied sides. RESULTS: Of 26 studied patients, 42.3% had a higher nerve branches density in the lateral side; 38.5%, on the medial side and 19.2% on the caudal side. The statistical analysis used to evaluate whether there was a predominance of one side where the dermis has been sectioned showed that the proportion comparison test was not significant (p = 0.304). CONCLUSION: The comparative analysis has shown that there is no preponderance of nerve density in any periareolar dermis side.

RESUMO

INTRODUÇÃO: A manobra de liberação dérmica periareolar na mamoplastia promove uma melhor mobilidade do complexo mamilo-aréola (NAC). No entanto, existem dúvidas sobre possíveis danos nos nervos neste tipo de topografia. Este estudo objetiva uma análise quantitativa comparando a densidade dos nervos do fluxo lateral medial, lateral e caudal do NAC. MÉTODOS: Trata-se de um estudo prospectivo. O estudo incluiu 26 pacientes do sexo feminino que foram submetidas à mamoplastia redutora pela técnica clássica de Pitanguy. Os fragmentos da derme coletados dos lados medial, lateral e caudal foram devidamente preparados e submetidos a um estudo histológico para determinar a densidade dos nervos em cada um dos lados estudados. RESULTADOS: Dos 26 pacientes estudados, 42,3% apresentaram maior densidade de nervos no lado lateral; 38,5%, do lado medial e 19,2% do lado caudal. A análise estatística utilizada para avaliar se houve predominância de um lado onde a derme foi seccionada demonstrou que o teste de comparação de proporções não foi significativo (p = 0,304). CONCLUSÃO: A análise comparativa mostrou que não há preponderância de densidade de nervos em qualquer lado da derme periareolar.

Bichectomy: technical systematization applied in 27 consecutive cases

ABSTRACT

INTRODUCTION: Despite buccal fat removal having been performed for a long time, its popularity has increased only in recent years, leading to a rise in the demand for bichectomy in plastic surgery clinics. Buccal fat pad removal is still considered controversial, even when properly indicated, especially with the lack of a systematized surgical technique to make it safe and reproducible. METHODS: The intraoral bichectomy described herein was systematically applied in 27 consecutive patients from January 5, 2016 to December 15, 2016. RESULTS: The mean patient age was 32 years; 15% were men, and 85% were women. Isolated buccal fat pad removal was performed in six patients (22%) and in combination with other procedures in 21 patients (78%). The main procedure combined was neck liposuction (55%), and most patients were operated under general anesthesia (93%). No permanent or significant complications occurred, and there were only two minor complications, i.e., inferior mandibular neuropathy and significant swelling, which improved without treatment during the following weeks. CONCLUSIONS: Buccal fat removal can be performed in a predictable, fast, and safe manner, leading to volumetric reduction of the lower third of the face, enhancing facial shape. When applied in a safe surgical environment, following all surgical security standards and considering the complex anatomical boundaries of the cheek, our technique can yield secure and satisfactory outcomes both for surgeons and patients.

RESUMO

INTRODUÇÃO: Embora realizada há muito tempo, a remoção da bola de Bichat ganhou notoriedade nos últimos anos devido a sua grande procura nos consultórios do cirurgião plástico. Mesmo quando adequadamente indicada, ainda é considerada um procedimento controverso, uma vez que ainda não existe uma técnica cirúrgica sistematizada na literatura atual, visando torná-la segura e reprodutível. MÉTODOS: A técnica da bichectomia intraoral descrita no trabalho foi aplicada de maneira sistemática em uma série de 27 pacientes consecutivos, no período de 5 de janeiro de 2016 a 15 de dezembro de 2016. RESULTADOS: A idade média dos pacientes foi de 32 anos, sendo 15% do sexo masculino e 85% do sexo feminino. A bichectomia foi realizada isoladamente em 6 pacientes (22%) e em conjunto com outros procedimentos em 21 pacientes (78%). O procedimento mais comumente associado foi a lipoaspiração cervical, realizada em 55% dos pacientes. A imensa maioria dos casos foram operados com anestesia geral (93%). Nenhuma complicação permanente e importante foi verificada no pós-operatório, apenas um caso de neuropatia transitória do ramo bucal e um caso de edema mais pronunciado, que prontamente se resolveram nas semanas seguintes. CONCLUSÃO: A remoção de gordura bucal pode ser realizada de forma previsível, rápida e segura, proporcionando diminuição volumétrica do terço inferior facial, com maior realce dos contornos faciais. Quando aplicada em um ambiente cirúrgico seguro, seguindo todos os padrões de segurança da cirurgia e respeitando os complexos limites anatômicos da região, nossa técnica levará os cirurgiões e pacientes a um resultado seguro e satisfatório.

Histopathological findings in tissue specimens from reduction mammaplasties

ABSTRACT

Introduction: Reduction mammaplasty is among the 5 most commonly performed cosmetic surgeries in Brazil. There is no consensus in the literature regarding the need for routine examination of resected tissue obtained during the procedure. This study presents the statistics of specimens obtained from reduction mammaplasty during the last 5 years and discusses the relevance of the histological details of these specimens. Methods: The results of 96 surgical breast reduction specimens from 2006-2010 were obtained from a single institution and analyzed retrospectively. Results: Histopathological changes were observed in 83.3% of specimens. Non-proliferative benign lesions were the main findings; the most frequent were fibrosis, adenosis, and cysts. Proliferative changes were detected in a significant percentage of patients (13.54%). One patient (1.16%), aged 50 years, had lobular carcinoma in situ. Patients younger than 40 years showed no malignant or premalignant lesions. Conclusions: The routine practice of histopathological screening of reduction mammaplasty specimens in young patients seems questionable, although it has been beneficial in patients older than 40 years. Additional studies are needed to define groups for which preventive measures should be undertaken to ascertain the cost-effectiveness of this practice.

RESUMO

Introdução: A mamoplastia redutora está entre as cinco cirurgias estéticas mais realizadas no Brasil. Não há consenso na literatura sobre a necessidade do exame de rotina dos tecidos ressecados durante o procedimento. Esse trabalho apresenta a estatística dos espécimes obtidos de mamoplastias redutoras e discute a relevância do detalhamento histopatológico dessas peças. Método: Resultados de 96 peças cirúrgicas de reduções mamárias, realizadas durante o período de 2006 a 2010, foram obtidos de uma única instituição e analisados retrospectivamente. Resultados: Alterações histopatológicas foram observadas em 83,3% dos espécimes. As lesões não-proliferativas benignas foram os principais achados, sendo fibrose, adenose e cistos as mais frequentes. Alterações proliferativas foram encontradas em percentual significativo das pacientes (13,54%). Uma paciente (1,16%), com 50 anos de idade, apresentou carcinoma lobular in situ. Pacientes na faixa etária menor que 40 anos não apresentaram lesões malignas ou pré-malignas. Conclusões: A prática rotineira de rastreamento histopatológico de peças de mamoplastias redutoras em pacientes jovens parece discutível, embora tenha sido benéfica em uma paciente com idade superior a 40 anos. Trabalhos adicionais são necessários para definir um grupo em que medidas preventivas teriam melhor relação custo-benefício.

Anatomic study of the arcuate line and its importance on breast reconstruction

ABSTRACT

Objective: The knowledge and the importance of the location of the arcuate line (AL) and its relation to anatomic landmarks in breast reconstruction with a transverse rectus abdominis musculocutaneous (TRAM) flap. Methods: Human cadaveric abdominal walls of 17 formalin-phenol-alcohol solution fixed cadavers were dissected and the AL, inferior deep epigastric vessels and its intersection with the lateral rectus abdominis muscle sheath were identified. With the abdominal cavity opened, the AL was identified by transilumination. The distance between AL to the pubic symphysis, to the umbilicus and to the level of the anterior superior iliac spines (ASIS) were measured. Results: The AL was located at the upper third, between the umbilicus and pubic symphysis in 64% of the dissections. The distance of AL to umbilicus was found to be at a mean of 4.2 ± 1.6 cm and to pubic symphysis was 10.4 ± 1.5 cm. The AL was always located superior to the line draw between the ASIS at a mean of 2.7 ± 1.4 cm. Conclusion: The relationship between AL and surface anatomic landmarks such as ASIS, umbilicus and pubic symphysis is important to a better preoperative knowledge of the AL location and it helps preoperative planning.

RESUMO

Objetivo: Ressaltar a importância da localização da linha semicircular, e suas relações anatômicas, durante as reconstruções mamárias com retalho miocutâneo de reto-abdominal. Método: A parede abdominal de 17 cadáveres formolizados foi dissecada por planos, identificando-se a linha semicircular (LSC), os vasos epigástricos profundos inferiores e sua intersecção com a borda lateral do músculo reto-abdominal. A cavidade abdominal também foi aberta e visualizada a LSC sob transiluminação. Mediu-se a distância da LSC à cicatriz umbilical e ao púbis, além da distância de uma linha imaginária entre as espinhas ilíacas ântero-superiores. Resultados: Em 64% das dissecções, a LSC localizava-se no 1/3 superior da distância compreendida entre a cicatriz umbilical e o púbis. A média da distância da cicatriz umbilical à LSC foi de 4,2 ± 1,6 cm. A da LSC ao púbis foi de 10,4 ± 1,5 cm. A linha biespinha estava localizada sempre abaixo da LSC, estando disposta a cerca de 2,7 ± 1,4 cm. Conclusão: A projeção da LSC a partir da linha biespinha, assim como da cicatriz umbilical ou do púbis, nos oferece pontos externos de referência importantes para a localização da LSC e, portanto, para um planejamento pré-operatório mais seguro.

Histological evaluation of latissimus dorsi muscle subjected to tissue expansion after infiltration with botulinum toxin: an experimental study in rats

ABSTRACT

The authors describe histological changes in the latissimus dorsi muscle submitted to expansion after relaxation with botulinum toxin. The possible practical benefits include increased muscle compliance and better accommodation of a prosthesis. The experimental model involved 10 Wistar rats (Rattus norvegicus) of the same age, with average weight of 300 g. Muscle biopsies before and after expansion were performed in normal muscle, in a control group (with expanders alone), and in a group with expanders and botulinum toxin. Expanders measuring 3 cm3 were positioned below the muscle and expanded with 0.3 ml of saline weekly, for 10 weeks. Histological sections were stained using hematoxylin-eosin for general evaluation and Masson's trichrome for evaluation of connective tissue. The muscle fibers submitted to expansion under the action of botulinum toxin showed less fibrosis and less intense proliferation of blood vessels than in the group without botulinum toxin, and the atrophy and reduction in the number of muscle fibers were less prominent than in the group that did not receive botulinum toxin. The findings suggest that muscle expansion associated with botulinum toxin relaxation preserves skeletal muscle characteristics by providing better accommodation and protection for a prosthesis and facilitating expansion dynamics; this method may also reduce pain.

RESUMO

Os autores descrevem as alterações histológicas no músculo grande dorsal submetido à expansão após relaxamento com toxina botulínica e as possíveis correlações dos achados com os benefícios práticos como, por exemplo, aumento da complacência muscular e melhor acomodação da prótese. Foi empregado o modelo experimental, com dez ratas com peso médio de 300 g, mesma faixa etária, da cepa Wistar (Rattus norvegicus) e o músculo grande dorsal. Biópsias musculares foram feitas antes e após as expansões, no músculo normal, no grupo controle (apenas com expansores) e no grupo com expansores e toxina botulínica. Expansores de 3 centímetros cúbicos eram posicionados abaixo do músculo e expandidos com 0,3 mililitros de soro fisiológico semanalmente, por 10 semanas. Os cortes histológicos foram corados segundo as técnicas de Hematoxilina-eosina, para avaliação geral, e tricrômio de Masson para avaliação do tecido conjuntivo. As fibras musculares submetidas à expansão sob a ação da toxina botulínica apresentaram focos de fibrose e proliferação de vasos sanguíneos menos intensos que no grupo sem toxina botulínica e a diminuição do número de fibras musculares e a atrofia eram menores que no grupo que não utilizou a toxina. Os achados nos permitem presumir que a expansão muscular associada ao relaxamento com toxina botulínica preserva as características da musculatura esquelética, oferecendo melhor acomodação e proteção da prótese e facilitando a dinâmica da expansão, além de diminuir a dor.

Anatomic variations of the subcutaneous tissue after ponderal weight lost

ABSTRACT

Background: The two adipose stratus that compose the subcutaneous tissue present different thickness in the body regions, that suffer effects with the weight variations, age, genders and genetics. When submitted to extended body weight variations by bariatric surgeries or extreme diets, generalized macroscopic and histological changes are also observed. Methods: Twenty eight surgical skin human specimens obtained after surgeries, were selected from the epigastric, peri-umbilical, hypogastrium and flank regions to be evaluated in their thickness and structural variations, from patients after been submitted by bariatric surgery or diets with extensive ponderal weight lost. Results: Macroscopic analysis showed changes from the original patterns related to the fat color, at the superficial fascia and the architectural of the adipose lobules but similar in the selected regions.

RESUMO

Introdução: Os dois estratos de gordura que compõem o tecido celular subcutâneo variam de espessura nas diversas regiões do corpo, com o peso, idade, sexo e genética. Quando submetidos a amplas variações volumétricas decorrentes de cirurgia bariátrica ou por dietas extremas, alterações macroscópicas e histológicas são evidentes e generalizadas pelo corpo. Método: Foram estudadas 28 peças cutâneas cirúrgicas selecionadas nas regiões epigástrica, peri-umbilical, hipogástrio e lombar, para avaliações na diferença de espessura e das suas características estruturais macroscópicas e distribuídas em três grupos: o padrão normal para fins comparativos, os que foram submetidos a cirurgia bariátrica e os que somente tiveram grandes perdas ponderais por dietas alimentares. Resultados: Macroscopicamente foram encontradas alterações diversas das características padrões, na cor do tecido adiposo, na estrutura da fáscia superficial e na arquitetura dos lóbulos adiposos, porém similares nas regiões selecionadas.

Reconstruction of the abdominal wall in rats with hemicellulose

ABSTRACT

Introduction: Loss of abdominal wall substance (LAWS) is clinically and surgically important because of its high incidence, the distress it causes, and the high cost of its current treatments. Recently, new techniques have been developed to treat LAWS, including the creation of flaps and use of biocompatible synthetic materials. The aim of this randomized uncontrolled experimental study was to assess the suitability of hemicellulose film for LAWS repair in rats. Methods: Forty female Wistar EPM III rats were divided into 4 groups (I-IV) of 10 animals each. All the animals received general anesthesia with 40 mg/kg intraperitoneal thiopental sodium. A 2.0 × 3.0 cm area of muscle-aponeurotic tissue and peritoneal damage was created in the anterior abdomen of every rat. Fifteen 2.0 × 3.0 cm hemicellulose films soaked in 0.9% saline were affixed in overlapping layers to the adjacent muscle walls with 8 separate 5-0 nylon sutures, and the skin and panniculus carnosus were synthesized by placing continuous 5-0 nylon sutures. The animals in groups I, II, III, and IV were euthanized after 3, 28, 35, and 240 days, respectively. Conclusion: Hemicellulose film is effective for repairing LAWS in rats.

RESUMO

Introdução: As perdas de substância da parede abdominal (PSPA) revestem-se de importância clínica e cirúrgica em função da alta frequência com que ocorrem, do sofrimento que produzem e do alto custo dos tratamentos atualmente ministrados. Nos últimos anos, tem-se observado o desenvolvimento de novas técnicas para a resolução das perdas de substância da PSPA, entre as quais se incluem a realização de retalhos e o uso de materiais biocompatíveis e sintéticos. Método: Esse estudo experimental aleatório e não controlado avaliou o uso da hemicelulose para a reparação das PSPA em 40 ratos Wistar EPM III, femêos. Os animais foram divididos em quatro grupos experimentais de estudo: I (n=10), II (n=10), III = (n=10) e IV (n=10). Todos os animais foram submetidos a anestesia geral utilizando-se o tiopental sódico, via intraperitonial. Foi provocada uma lesão de 2,0 x 3,0cm no tecido músculo-aponeurótico e peritônio da região anterior do abdome. A aposição da película de hemicelulose sobre a lesão foi realizada superpondo-se 15 películas de 2,0 x 3,0cm, umedecidas em soro fisiológico a 0,9%. A película foi fixada à parede muscular adjacente e, em seguida, foi realizada a síntese da pele e panículo carnoso por meio de sutura contínua com fio de náilon de número cinco zeros. A eutanásia dos animais dos grupos I (n=10), II (n=10), III (n=10) e IV (n=10) foi realizada após 3, 28, 35 e 240 dias, respectivamente. Conclusão: Esse estudo concluiu que a hemicelulose foi eficiente em reparar as grandes perdas da parede abdominal em ratos.

ABSTRACT

Abdominoplasty is a common surgical procedure in plastic surgery. Consequently, several technical variations have been created with the aim of reducing complications and obtaining better results in the abdominal contour. The anatomy of the abdominal region plays an important role in this procedure, since arguments in favor of techniques, such as classic abdominoplasty and lipoabdominoplasty are linked to anatomical principles. This highlights the plastic surgeon's need to have a good knowledge of the anatomy of this region. Not only will this knowledge enable plastic surgeons to understand the reasons for adopting certain surgical procedures, but it will also enable them adapt these procedures to the anatomical characteristics of each patient. In addition, this knowledge will help determine the morphological changes caused by surgery. Therefore, this study aimed to identify studies in the literature focusing on the anatomical description of both classical abdominoplasty and other variants, such as lipoabdominoplasty. It also aimed to discover the anatomical and physiopathological explanations for the major inherent surgical complications.

A abdominoplastia está entre os procedimentos cirúrgicos mais realizados na cirurgia plástica, de forma que muitas variações técnicas têm sido criadas com o objetivo de se reduzirem as complicações e para se obter melhores resultados no contorno abdominal. Nesse contexto, a anatomia da parede abdominal ganha papel de destaque, uma vez que as justificavas em defesa de técnicas, como a abdominoplastia clássica e lipoabdominoplastia, estão atreladas a princípios anatômicos. Isso evidencia a necessidade do domínio da anatomia da região pelo cirurgião plástico, pois, além de entender os motivos de se adotar determinados procedimentos cirúrgicos, o torna mais capacitado de adaptá-los às características anatômicas de cada paciente, além de compreender as alterações morfofuncionais provocadas pela cirurgia. Diante disso, esse trabalho teve o objetivo de buscar na literatura estudos que enfocassem a descrição anatômica da abdominoplastia, tanto clássica quanto de outras variantes, como a lipoabdominoplastia, bem como as explicações anatomofisiopatológicas das principais complicações operatórias inerentes.

Earlobe hypertrophy correction

ABSTRACT

INTRODUCTION: The ear is a structure of the face with important aesthetic value. The dimensions of the ear and the appearance of its anatomical components change with age, disease and cultural practices which can result in an aesthetically unpleasant form over time. The earlobe is an important anatomical part of the ear and is considerably influenced by the factors above. It is made of fatty tissue and skin and can undergo changes in its proportion and flaccidity, which may be improved by surgical procedures to make it more aesthetically pleasing, providing a more proportional or younger appearance. This article reports the case of a 30-year-old patient who considered his earlobes disproportionate in relation to the rest of his ears and underwent surgery to have them reduced. METHODS: A simple and fast recovery surgical procedure was carried out delineating and resecting a part of each of the patient's earlobes. RESULTS: The results proved to be satisfactory as the patient had more proportional earlobes in relation to the rest of his ears achieving aesthetic improvement. CONCLUSION: The technique described in this article to reduce hypertrophied earlobes proved to be efficient.

RESUMO

INTRODUÇÃO: A orelha é uma estrutura da face que tem valor estético. As dimensões da orelha e aparência dos seus componentes anatômicos são modificados com a idade, doenças e práticas culturais que podem resultar em uma estética desagradável com o tempo. O lóbulo é um componente anatômico importante da orelha que é bastante influenciado esteticamente por estes fatores. Ele é composto por tecido adiposo e pele e pode sofrer mudanças na sua proporção e flacidez, as quais podem ser melhoradas, por meio procedimentos cirúrgicos, para ficarem esteticamente mais agradáveis, com o intuito de oferecer uma aparência mais proporcional ou jovem. Este trabalho descreve um caso de um paciente de 30 anos que considerou os lóbulos das suas orelhas desproporcionais ao resto da orelha e fez cirurgia para reduzi-los. MÉTODOS: Foi realizado um procedimento cirúrgico simples, de recuperação rápida para reduzir o tamanho dos lóbulos do paciente por marcação e resseção de uma parte de cada lóbulo do paciente. RESULTADOS: Os resultados se mostraram satisfatórios com lóbulos mais proporcionais ao resto das orelhas e melhora da estética do paciente. CONCLUSÃO: A técnica descrita neste trabalho para a redução de lóbulos auriculares hipertrofiados se mostrou eficaz.

Reconstruction of the nasal dorsum: a case report

ABSTRACT

INTRODUCTION: The nasal dorsum is a common location for the development of cutaneous tumors, predominantly basal cell epithelioma. In 1964, Barsky recommended a quadrangular flap for coverage of the nasal dorsum, taken from the glabellar region, to most closely match the skin of the dorsal region. This was based on a simple procedure for covering raw areas up to the nasal tip, resulting in less noticeable scars. CASE REPORT: An 82-year-old woman presented to the Plastic Surgery Service of the University Hospital Maria Aparecida Pedrossian with a lesion on the nasal dorsum of approximately 3 x 2 cm, ulcerated with an erythematous base, and ill-defined limits. A fronto-glabellar sliding flap was used to correct the loss of substance. The patient progressed with flap viability, restoration of the nasal dorsum, and satisfactory esthetic results. CONCLUSION: Owing to the color and texture of the skin from this region, the fronto-glabellar flap is an excellent donor area for coverage of the nasal dorsum.

INTRODUÇÃO: O dorso nasal é um local comum para o desenvolvimento de neoplasias cutâneas, predominantemente o epitelioma basocelular. Em 1964, Barsky preconiza um retalho quadrangular do dorso nasal, com base na região glabelar, utilizando somente pele da região dorsal. Baseava-se em um procedimento simples recobrindo áreas cruentas até a ponta nasal, resultando em cicatrizes pouco perceptíveis. RELATO DE CASO: Paciente de 82 anos procurou o Serviço de Cirurgia Plástica do Hospital Universitário Maria Aparecida Pedrossian apresentando lesão em dorso nasal de aproximadamente 3 × 2 cm, ulcerada com base eritematosa e limites mal definidos. A técnica utilizada para correção da perda de substância foi o deslizamento de um retalho frontoglabelar com pedículo randomizado. A paciente evoluiu com viabilidade do retalho, apresentando restauração do dorso nasal e resultado estético satisfatório. CONCLUSÃO: Em decorrência da cor e textura adequadas da pele desta região, o retalho frontoglabelar é reconhecidamente uma excelente área doadora para cobertura do dorso do nariz.

Training in corpse surgery: transfer of tendon from the abductor of the minimal finger (Oppositionplasty)

ABSTRACT

Introduction: The minimum finger abductor (ADM) is one of the muscles used in surgery to restore the opposing function of the thumb. Training in corpse has been shown to be effective in anatomical learning and the surgical technique for acquiring surgical skills in controlled environments. The objective of this work is the surgical experimentation for the anatomical learning of hand structures.
Materials and Method: Top member of the UFCSPA Anatomy Laboratory was used.
Results and Conclusions: As a final result, we obtained an anatomical piece in which all the steps of the surgical procedure were faithfully reproduced. This kind of simulation provides realistic, risk-free experience in which we have the possibility of reviewing more relevant anatomical concepts at each stage of procedure, in addition to improving their skills in operative technique.

RESUMO

Introdução: O abdutor do dedo mínimo (ADM) é um dos músculos utilizados na cirurgia para restauração da função de oponência do polegar. Treinamento em cadáver vem demonstrando ser eficaz no aprendizado anatômico e da técnica operatória para aquisição de competências cirúrgicas em ambientes controlados. O objetivo do trabalho é a experimentação cirúrgica para o aprendizado anatômico de estruturas da mão.
Materiais e Método: Foi utilizado membro superior do Laboratório de Anatomia da UFCSPA.
Resultados e Conclusões: Como resultado final, obtivemos uma peça anatômica na qual foram reproduzidos, de maneira fiel, todos os passos do procedimento cirúrgico. Esse tipo de simulação proporciona experiência realista, livre de riscos, em que temos a possibilidade de revisar conceitos anatômicos mais relevantes em cada etapa do procedimento, além de aprimorar as suas habilidades em técnica operatória.
