Mastopexy with superior pedicle flap and silicone implant

ABSTRACT

Introduction: Surgical correction of breast ptosis consist of increasing breast volume, reducing the skin envelope with breast lifting (mastopexy) or combination of both. A variety of procedures have been described with this in mind. Mastopexy plus augmentation is considered a more complex procedure than mastopexy isolated for the variable results, ptosis relapse and associated complications. Objective: To present and to analyse the superior pedicle flap mastopexy technique associated with silicone implants adapted for breast ptosis. Methods: Sixty-eight patients with breast ptosis were operated by superior pedicle flap mastopexy technique associated with silicone implants. Results: Sixty-three (92.64%) cases presented good results. Ptosis relapse happens in 5 (7.35%) patients. Conclusion: The superior pedicle flap mastopexy technique associated with silicone implants had good results for patients with hipomastia and glandular ptosis or true ptosis grades II or III. We had low rate of ptosis relapse and few complications in the short and medium term.

RESUMO

Introdução: A correção da ptose mamária é feita com aumento do volume mamário por meio de implantes de silicone, ou retirada do excesso de pele e levantamento (mastopexia), ou a associação destas duas. Vários procedimentos são descritos baseados nestes conceitos. A mastopexia associada com implante de silicone é considerada um procedimento mais complexo do que as mastopexias sem implante, pelos resultados variáveis, recidivas e complicações relacionadas. Objetivo: Apresentar e avaliar a adaptação da técnica de mastopexia com retalho de pedículo superior associada a implante mamário de silicone para correção de ptoses mamárias. Método: Foram operados 68 pacientes com ptose mamária pela técnica de mastopexia com retalho de pedículo superior associado a implante mamário. Resultados: Sessenta e três (92,64%) casos obtiveram bom resultado. Recidiva de ptose ocorreu em 5 (7,35%) pacientes. Conclusão: Em casos de ptose mamária, a mastopexia com retalho de pedículo superior associada a implante mamário produz bons resultados, com baixo índice de recidiva e poucas complicações a curto e médio prazo.

Evaluation of the data contained in the requests for the anatomopathological examination of the breast in the surgery of exchange or explantation of silicone breast implants: analysis of 251 cases

ABSTRACT

Introduction: The description of the Anaplastic Large Cell Lymphoma and the explantation surgery resulted in an increase of histopathological exams in breast implant removing surgery.
Methods: 251 pathology requests were studied. The following data from the medical requests were analyzed: gender, age, type of surgery, number of specimens containers sent, laterality, anatomical and spatial location, clinical history, signs and symptoms, previous radiotherapy, previous chemotherapy, diagnostic hypothesis, previous surgeries, and reference to previous breast exams.
Results: The mean age was 43 years old. Laterality was not mentioned in 16 requests. The surgery performed was mentioned in 15.94% requests. The number of containers varies from 1 to 5, with a median of 2. The containers include capsules in 242 cases, 161 as isolated capsule, 27 mammary tissue, and capsule in the same specimen, 54 mammary tissues sent in a separate container, anatomical and spatial location was mentioned in 6.33% cases. The detailed clinical data was included in 19.12%, signs and symptoms 13.94%, contracture as the only item mention in 64 of them. In 27 requests, lymphoma evaluation was requested. 15 included seroma and from nine of those, liquid was sent with a request for immunohistochemical and cytology analysis. None of the requests had any data on implant type or brand.
Conclusion: The amount of information contained in the medical request forms is minimal. The authors recommend the need for a protocol to standardize the surgical removal of the capsule and the adjacent mammary tissue. Surgical specimens should be spatially oriented.

RESUMO

Introdução: A descrição do linfoma anaplásico de células T e o recente aumento das cirurgias de explante resultou na elevação do número de exames anatomopatológicos nas cirurgias de retirada de implantes mamários de silicone. O objetivo desta pesquisa é analisar a qualidade e quantidade de dados contidos na requisição do exame histopatológico.
Métodos: Foram estudados 251 casos. Os seguintes dados foram analisados: sexo, idade, localização anatômica e espacial, lateralidade, história clínica, sinais e sintomas, quimioterapia e radioterapia prévia, hipótese diagnóstica, cirurgias prévias, tipo e marca do implante, exames de imagem prévios e número e características dos espécimes enviados.
Resultados: A idade média foi de 43 anos. A lateralidade não foi mencionada em 16 (0,84%). A localização anatômica foi citada em 15 casos. O tipo de cirurgia foi mencionado por 40 (15,94%). O número de contêineres variou de 1 a 5, com mediana de 2. A cápsula foi enviada em 242 casos, em 161 de forma isolada, tecido mamário em conjunto com cápsula em 27, tecido mamário e cápsula em contêineres diferentes em 54 casos. A história clínica foi incluída em 19,12%, sinais e sintomas em 13,94%, em que a contratura foi o único termo inserido em 64. Em 27 requisições foi citado linfoma. Em 15 pacientes a presença de seroma foi referida e destes nove foram enviados. O tipo e marca do implante não foi citado.
Conclusão: Os dados são escassos. Recomenda-se a criação de protocolos na retirada da cápsula e tecido adjacente, incluindo a orientação anatômica e espacial.

Silicone breast implant: loops to facilitate its placement and to give better precision to the positioning

ABSTRACT

Background: Transaxillary breast augmentation has the disadvantages of greater difficulties on its implantation and the possibility of displacement and malpositioning (rotation), especially if anatomic ("tear drop") implants used. The polyurethane shell also increase he friction forces to its placement trough an axillary's incision. With the intent to avoid these problems, the authors designed four small Dacron loops on the implants undersurface, so that they could have been pulled through the skin with a 2-0 nylon. Methods: This operation was performed in six patients (ages 21 to 41) and the implants were placed transaxillary in the subfascial plane (under pectoralis major fascia), with fiber optic light source. The implants with Dacron loops had a polyurethane shell, around base and natural profile. The volumes varied from 120 to 205 ml. Two 2-0 nylon stitches were passed throu gh the skin at the medial and lateral aspects of inframammary fold. After implantation, the stitches were pulled out to determinate the adjustment of the implant to its ideal position. A third stitch in the upper lateral loop was exteriorized through the incision and helped the correct placement on the implant as well. Results: No inflammatory reaction or capsular contraction has been detected to the present date. The consistence of all breasts is normal and the axillary scars are all of good quality. The implants with loops can be used also if another incision is chosen (inframammary or periareolar). Conclusions: The authors concluded that the loops allowed an easy and precise transaxillary placement of natural profile breast implants with a polyurethane shell.

RESUMO

Introdução: A via axilar tem a desvantagem de maior dificuldade na colocação dos implantes e possibilidade do implante ficar rodado, mal posicionado, especialmente se for em forma de gota. O revestimento com poliuretano aumenta o atrito, dificultando também sua colocação via axilar. Tentando evitar estes percalços, os autores decidiram colocar quatro pequenas alças de Dacron sob a base dos implantes, de forma a se poder tracioná-los com fios passados pela pele. Método: Foram operadas seis pacientes, com idades variando de 21 a 41 anos. Os implantes mamários foram colocados no plano subfascial, via axilar, com visualização por fibra óptica. Foram passados dois fios de náilon 2-0 transcutâneos, nas extremidades medial e lateral do sulco inframamário. Após a introdução do implante pela incisão axilar, os fios foram tracionados, determinando o deslocamento do implante para a posição ideal. O terceiro fio, fixado na alça súpero-lateral, saía pela incisão e também auxiliou na determinação da adequada posição do implante. Resultados: Nenhuma paciente apresentou reação inflamatória local ou formação de cápsula até a presente data. A consistência em todas as mamas era normal. As cicatrizes axilares apresentaram excelente qualidade. Os implantes com alças podem ser utilizados também por outras vias, como a infra-axilar e transareolar. Conclusão: Os autores concluíram que as alças permitiram alocar os implantes mamários de perfil natural e superfície de poliuretano via axilar com facilidade e precisão.

ABSTRACT

Introduction: Throughout implants evolution, breast implants manufacturers have launched different formats of implants, which has led to a divergence of opinion among plastic surgeons about their indications.
Objective: To compare the aesthetic result between the inclusion of "Silimed natural®" (round base and anatomical front) or round implants in breast augmentation mammoplasty.
Methods: Double blind retrospective study, which evaluated 14 patients undergoing breast augmentation between 2011 and 2016. Patients selected were between 20 and 40 years of age, BMI 18 to 23 and breasts classified as moderate or severe hypomastia.
Results: Agreement rate among plastic surgeons on Silimed natural® implants evaluation was 76.2% (77 of 101) and for round implants it was 64.8% (116 of 179) (p<0.047).
Conclusion: The use of anatomical implants (Silimed natural®) can give a more natural shape when indicated to patients with moderate to severe hypomastia and if used in the subglandular plane.

RESUMO

Introdução: Ao longo da evolução dos implantes, fabricantes de implantes mamários lançaram diferentes formatos dos mesmos, o que trouxe uma divergência de opinião sobre suas indicações entre os cirurgiões plásticos.
Objetivo: Comparar o resultado estético entre a inclusão de implantes de formato "Silimed natural®" (base redonda e frente anatômica) ou redondo em mamoplastias de aumento.
Método: Estudo retrospectivo duplo cego, o qual avaliou 14 pacientes submetidas à mamoplastia de aumento entre 2011 e 2016. As pacientes selecionadas têm idades entre 20 a 40 anos, Índice de Massa Corporal de 18 a 23 e mamas classificadas como hipomastia moderada ou severa.
Resultados: A taxa de concordância entre cirurgiões plásticos na avaliação de implantes Silimed natural® foi de 76,2% (77 de 101) e para implantes redondos foi de 64,8% (116 de 179) (p<0,047).
Conclusão: A utilização de implantes anatômicos (Silimed natural®) pode conferir um formato mais natural quando indicados a pacientes com hipomastia moderada a severa e se utilizado o plano subglandular.

MAMASIZE: a new auxiliary instrument in the planning of enlargement mammaplasty

ABSTRACT

Breast volume is frequently discussed in mammaplasties. However, no reference about how to establish dimension the various breast sizes was found in the literature researched. The purpose of this work is to present 6 scales of different sizes, in the shape of female breast, transparent, empty, that measure the breasts by the volume and provide numbers that are similar to the numbers used by brassiere manufacturers. The scales were tested in 180 women, therefore, 360 breasts with a level of reliability over 95%. They can be helpful in the evaluations and surgery planning of the mammaplasties, besides helping to improve the relation doctor-patient.

RESUMO

Volume mamário com freqüência é discutido nas mamoplastias. Entretanto, nenhuma referência de como dimensionar os diversos tamanhos mamários foi encontrada na literatura pesquisada. O propósito deste trabalho é apresentar seis medidores de tamanhos diferentes, em formato da mama feminina, transparentes, ocos, que medem as mamas volumetricamente e fornecem numeração semelhante à utilizada pelos fabricantes de sutiã. Os medidores foram testados em 180 mulheres (360 mamas), com grau de confiabilidade acima de 95%. Podem ser úteis nas avaliações e planejamento cirúrgico das mamoplastias, além de facilitar a melhor relação médico-paciente.

Surgical treatment for breast ptosis with silicone prosthesis: evaluation of surgical results and patient satisfaction

ABSTRACT

Introduction: The correction of breast ptosis associated with skin flaccidity is done through mastopexy with the inclusion of an implant. This work's objective was to evaluate the surgical results and the satisfaction of patients who underwent breast ptosis surgery with silicone prosthesis placement.
Methods: We selected 22 patients who underwent mastopexy with implant placement, from February to September 2016, at the Plastic Surgery Service of Hospital Heliópolis. Interviews were conducted applying to the patients a questionnaire to verify the degree of satisfaction and changes in the daily routine after surgery. The surgical results evaluation was carried out by three surgeons, who attributed scores to different items.
Results: 100% of the interviewees feel satisfied with the surgery, and all reported an improvement in their self-esteem. In the evaluation of surgeries performed with surgeons, about 91% of the results are between regular and good.
Conclusion: The degree of patient's satisfaction who underwent mastopexy with insertion was excellent. There was a favorable impact on the quality of life and well-being of the patients evaluated, with the post-surgical result being classified as regular or good.

Introdução: A correção da ptose mamária associada à flacidez de pele é corrigida através da mastopexia com inclusão de implante. O objetivo deste trabalho foi avaliar os resultados cirúrgicos e a satisfação de pacientes submetidas à cirurgia de ptose mamária com inclusão de prótese de silicone.
Métodos: Foram selecionadas 22 pacientes submetidas à mastopexia com inclusão de implante, no período de fevereiro a setembro de 2016, no Serviço de Cirurgia Plástica do Hospital Heliópolis. Foi realizada entrevista com as pacientes, por meio de aplicação de questionário, com a finalidade de verificar o grau de satisfação e as alterações no cotidiano diário após a cirurgia. A avaliação dos resultados cirúrgicos foi realizada mediante avaliação de três cirurgiões, do qual atribuíram notas a diferentes itens.
Resultados: 100% das entrevistadas se sentem satisfeita com a cirurgia e todas relataram a melhora da autoestima delas. Na avaliação das cirurgias realizada com os cirurgiões, cerca de 91% dos resultados estão entre regular e bom.
Conclusão: O grau de satisfação das pacientes submetidas à mastopexia com inserção foi excelente e houve impacto favorável na qualidade de vida e bem-estar das pacientes avaliadas, sendo que o resultado pós-cirúrgico se enquadra como regular ou bom.

Breast silicone explant: a multicenter longitudinal study

ABSTRACT

Introduction: Following silicone breast implant placement, some patients present symptoms described as breast implant illness and seek explant surgery. This study aims to analyze the historical symptoms and ascertain breast explant patients' impressions at three different times: before breast implant placement while having the implants, and after the explant surgery.
Methods: This survey was designed as a multicenter longitudinal observational study using an online voluntary participation questionnaire sent by e-mail.
Results: 156 patients were analyzed, 84% had three or more symptoms, and 66.1% improved their symptoms after the explant (p<0.001). Before the placement of silicone, the median self-body satisfaction was 7, while with the implants, the median became 9, and after the explant surgery, the median remained up to 9 (p<0.001). Support groups on social networks helped in the decision to explant in 87.2% of the patients.
Conclusion: Patients presenting symptoms after silicone placement show improvement with breast implant removal. Body self-satisfaction increases with the placement of breast implants and remains increased after their removal. Patients who undergo the explant surgery usually regret having implanted silicone; they are very satisfied with the decision to remove them and equally satisfied with the result of the breast explant surgery. Support groups on social networks were important in the decision-making of these patients.

RESUMO

Introdução: Após a colocação de implantes mamários de silicone, algumas pacientes apresentam sintomas descritos como doença do implante mamário e buscam a cirurgia de explante. O objetivo deste estudo é analisar o histórico de sintomas e verificar as impressões dos pacientes submetidos ao explante mamário em três momentos distintos: antes de colocar os implantes mamários, enquanto estavam com os implantes e após a cirurgia de explante.
Métodos: Essa pesquisa foi delineada como um estudo observacional longitudinal multicêntrico utilizando um questionário on-line de participação voluntária enviado por e-mail.
Resultados: Foram analisados 156 pacientes, 84% apresentavam três ou mais sintomas e 66,1% destes obtiveram melhora de sua sintomatologia após o explante (p<0,001). Antes da colocação de silicone, a mediana de autossatisfação corporal era de 7, enquanto estavam com os implantes a mediana tornou-se 9 e após a cirurgia de explante a mediana se manteve em 9 (p<0,001). Grupos de apoio em redes sociais auxiliaram na decisão de fazer o explante em 87,2% das pacientes.
Conclusão: Pacientes que têm sintomas após colocarem silicone apresentam melhora com a retirada dos implantes mamários. A autossatisfação corporal aumenta com a colocação de implantes mamários e permanece elevada após a retirada destes. Pacientes que fazem a cirurgia do explante costumam estar arrependidas de terem colocado silicone, muito satisfeitas com a decisão de removêlos e igualmente satisfeitas com o resultado da cirurgia de explante mamário. Grupos de apoio em redes sociais foram importantes na tomada de decisão destas pacientes.

Inclusion of the breast prosthesis in double space

ABSTRACT

The work presents a new location as far as the space for implantation of the silicone mammal prótese. It develops a new positioning of the prosthesis, not just sub-glandular or sub-muscular, which is named Inclusion of the Mammal Prosthesis in Double Space. With that, many of the unfavorable results are eliminated, not only in the subglandular, but also in the sub-muscular positioning. This new technique was applied efficiently by its author in 320 operated patients from March 1997 until March 2004, presenting as an additional benefit to the obtained results a better acceptance by the patients in relation to the prior techniques.

RESUMO

Este trabalho apresenta uma nova localização quanto ao espaço de implantação de próteses mamárias de silicone. Desenvolve um novo posicionamento da prótese, não sendo somente subglandular ou submuscular, ao qual se denomina de Inclusão de Prótese de Mama em Duplo Espaço. Com isso, elimina-se grande parte dos resultados desfavoráveis, tanto no posicionamento subglandular, quanto no submuscular. Esta nova técnica foi aplicada com eficiência pelo autor em 320 pacientes operadas, no período de março de 1997 a março de 2004, apresentando, como benefício adicional aos resultados obtidos, melhor aceitação pelas pacientes em relação às técnicas anteriores.

Avaliação da eficácia do retalho glandular na proteção da incisão no sulco mamário em cirurgias de implante de silicone

ABSTRACT

Silicone implants inclusion for augmentation mammoplasty is one of the most performed surgical procedures in Brazil and worldwide. Purpose of this surgery is to increase the volume so that the patient decreases their dissatisfaction with breast shape and appearance. Although it is a procedure with short surgical time and has low intraoperative risk, complications involving augmentation mammoplasty are dramatic and involve infection, long-term capsular contracture, surgery extrusion with surgery failure, and litigation risk because of patient dissatisfaction. At present, breast augmentation incisions have become smaller to improve aesthetic results, while at the same time breast implant volumes have increased. Incision location depends primarily on each surgeon individual preferences. It is known that incision position is a determining factor for tissue trauma degree, implant exposure to endogenous bacteria, potential sensory damage, visibility and hemostasis. This study aim is to evaluate inferior pole glandular flap functionality to protect exposure of silicone implant in patients who underwent mammoplasty surgeries via inframammary sulcus in a series of cases.

RESUMO

A inclusão de implantes de silicone para mamoplastia de aumento é um dos procedimentos cirúrgicos mais realizados no Brasil e no mundo. O propósito desta cirurgia é aumentar o volume para que o paciente diminua sua insatisfação quanto à forma e aparência da mama. Apesar de tratar-se de um procedimento com tempo cirúrgico rotineiramente pequeno e baixo risco intraoperatório, as complicações que envolvem a mamoplastia de aumento são dramáticas e envolvem infecção, contratura capsular a longo prazo, extrusão do implante com falência da cirurgia e o risco de litígio por insatisfação das pacientes. Na atualidade, as incisões para mamoplastia de aumento se tornaram menores para melhorar os resultados estéticos, enquanto que, ao mesmo tempo, os volumes dos implantes mamários aumentaram. A localização da incisão depende basicamente das preferências individuais de cada cirurgião. Sabe-se que a posição da incisão é um fator determinante do grau de trauma tissular, exposição do implante às bactérias endógenas, dano sensitivo potencial, visibilidade e hemostasia. O objetivo desse estudo é avaliar a funcionalidade do retalho glandular do polo inferior para proteção da exposição do implante de silicone nas pacientes que realizaram cirurgias de mamoplastia de aumento via sulco inframamário em uma série de casos.

Gluteal implants - XYZ method: retrospective study

ABSTRACT

Introduction: The concept of female beauty has changed through-out time, but the form and size of the breasts and gluteal region have remained constant as symbols of maximum femininity. The body expositions, especially on the gluteal area, has develop an increase of seek for buttocks surgery with silicone implants. Objective: The purpose of this study is to demonstrate the author's experience with the XYZ intramuscular method, emphasing the importance of the anatomical knowledge about gluteal area. Method: This paper was retrospective and consecutive of 11 gluteoplasty using the XYZ intramuscular method, between June 2006 and February 2008. All the patients were female. Results: All the implants used smooth and the sizes: 270 ml (45.45%); 300 ml (18.18%); 240 ml (18.18%); 350 ml (9.09%); 360 ml (9.09%). All the patients were necessary to perform liposuction of the flanks. In 2 (18.18%) cases were necessary to perform the lifting the dermo tuberal anchorage technique. Complications and their respective incidences include: seroma - 1 (9.09%) case; dehiscence of suture - 1 (9.09%) case; double-bubble - 1 (9.09%) case. Conclusion: The XYZ intramuscular method has proved to be a safe and reproduce to reshaping the buttocks, with very low rates of complication.

Introdução: O conceito de beleza feminina tem mudado ao longo dos anos, porém a forma e o tamanho das mamas e nádegas permanecem como símbolos de máxima feminilidade. A exposição corporal, especialmente da região glútea, tem proporcionado um aumento da procura pela cirurgia de gluteoplastia de aumento com utilização de implantes de silicone. Objetivo: O propósito do estudo é mostrar a experiência dos autores com a técnica XYZ de colocação de implantes glúteos intramusculares, enfatizando a importância do conhecimento anatômico da região glútea. Método: O trabalho foi um estudo retrospectivo e consecutivo de 11 gluteoplastias de aumento com a utilização da técnica XYZ, no período de junho de 2006 a fevereiro de 2008. Todos os pacientes eram do sexo feminino. Resultados: Os implantes utilizados foram todos lisos e os volumes: 270 ml (45,45%); 300 ml (18,18%); 240 ml (18,18%); 350 ml (9,09%) e 360 ml (9,09%). Em todas as pacientes, foi realizada lipoaspiração dos flancos. Em 2 (18,18%) casos, foi realizado o lifting glúteo inferior dermotuberal. As complicações e suas respectivas incidências foram: seroma - 1 (9,09%) caso; deiscência de sutura - 1 (9,09%) caso; double-bubble - 1 (9,09%) caso. Conclusão: A técnica intramuscular XYZ proporciona resultados seguros e reprodutíveis para o remodelamento das nádegas, com baixo índice de complicações.

Treatment of tuberous breasts by herniorrafia and inclusion of prosthesis

ABSTRACT

Introduction: The author describes a new procedure to repair a tuberous breast. Differently from most of the authors, this procedure prevents the destruction or the lesion of the periareolar constrictor ring. It avoids a frequent backsliding of glandular hernia process with the increase of the nipple areola complex. Methods: Since the projection of the gland through the constrictor ring is considered an hernia, the author makes an analogy with the other abdominal hernias by using a procedure which is similar to the repair of an incisional hernia through Mayo technique, that uses a superposition of the herniated layers besides the inclusion of prosthesis to repair the deficiency of the four quadrants of the breast in horizontal and vertical levels, and the glandular hipoplasy, without damaging the structure on the basis or the confection of glandular fragments, by associating muscle support on the basis, according to the technique described by Milton Daniel (2004 George Arié Prize). This technique will provide a bigger filling of the lower pole and support to the prosthesis by preventing the lower pole from turning out to be apparent and touchable.

Introdução: O autor descreve um novo procedimento para correção da mama tuberosa ou tubular, ao contrário da maioria dos autores, evitando a destruição ou lesão do anel constritor peri-areolar e, consequentemente, a frequente recidiva da herniação glandular, pelo aumento do complexo aréolo-mamilar. Método: Sendo a projeção da glândula através do anel constritor considerada uma hérnia, o autor faz uma analogia com outras hérnias abdominais, usando um procedimento semelhante ao da correção de uma hérnia incisional pela técnica de Mayo, que usa a sobreposição das camadas herniadas, além da inclusão de prótese para corrigir a deficiência dos quatro quadrantes da mama, nos níveis horizontal e vertical, e a hipoplasia glandular, sem desestruturar a base ou confecção de retalhos glandulares, associando suporte muscular na base, conforme técnica descrita por Milton Daniel (Prêmio George Arié 2004). Esta técnica proporcionará maior preenchimento do pólo inferior e suporte da prótese, evitando que o pólo inferior da mesma se torne aparente e palpável.

ABSTRACT

The retromuscular positioning of the mammary implants for the "Envelope Flap" technique demystifies definitely the possibility of the evolution for a "double breast" in the late augmentation mammaplasty follow-up, caused by the prosthesis imprisonment for mechanical action of the great pectoral muscle when it is not treated appropriately. The author proposes the treatment of the above-mentioned muscle through two oblique incisions of inferior convergence done in his inferior edge that allows a total freedom of the implant movement that accompanies the breast bascule movement along the time, maintaining a natural appearance in all the periods of this evolution.

RESUMO

O posicionamento retromuscular dos implantes mamários pela técnica em "Aba de Envelope" desmistifica definitivamente a possibilidade da evolução para uma "dupla mama" no seguimento tardio das mamaplastias de aumento, causada pelo encarceramento das próteses por ação mecânica do grande peitoral, quando não tratado adequadamente. O autor propõe o tratamento do referido músculo através de duas incisões oblíquas de convergência inferior, feitas em sua borda inferior, o que permite total liberdade de movimentação do implante que acompanha os movimentos de báscula das mamas ao longo do tempo, mantendo um resultado de total naturalidade em todos os períodos desta evolução.

Breast asymmetry: literature review and a new proposal for clinical classification

ABSTRACT

Breast asymmetry is a prevalent diagnosis that has several surgical modalities for its treatment. The correct diagnosis, taking into account the existing classification systems, is imperative for achieving the best results. The leading and most accepted proposals for the classification and treatment of breast asymmetries were raised through the literature review. These available classifications date from the 60s and 70s and need to be updated to the current clinical context. A more simplified and reproducible classification was proposed after a comprehensive literature review, considering the most frequent asymmetries in aesthetic plastic surgery offices, with their respective treatment guides. Five groups were created: 1 - hypotrophic breasts with volume asymmetry; 2 - hypotrophy with volume and contour asymmetry; 3 - normotrophic, ptotic breasts and with no desire to increase the volume; 4 - normotrophic, ptotic breasts and with a desire to increase the final volume; 5 - asymmetric and hypertrophic breasts. Based on the clinical findings, a treatment algorithm was created for each subtype of asymmetry, including in this arsenal, breast implants of different volumes, mastopexies, reduction mammoplasty, and fat grafting. It is important to emphasize that breast asymmetry is the rule and not the exception, therefore, it is a reason for patient dissatisfaction and a challenge for the plastic surgeon.

RESUMO

A assimetria mamária é um diagnóstico prevalente com diversas modalidades cirúrgicas para seu tratamento. O correto diagnóstico, levando-se em conta os sistemas de classificação existentes é imperativo para que os melhores resultados sejam alcançados. Através de revisão da literatura foram levantadas as principais e mais aceitas propostas de classificação e tratamento das assimetrias mamárias. Estas classificações disponíveis datam da década de 60 e 70 e carecem de atualização para o contexto clínico atual. Após ampla revisão da literatura foi proposta uma classificação mais simplificada e reprodutível, levando-se em conta as assimetrias mais frequentes nos consultórios de cirurgia plástica estética, com seus respectivos guias de tratamento. Cinco grupos foram criados: 1 - mamas hipotróficas com assimetria de volume; 2 - hipotrofia com assimetria de volume e contorno; 3 - mamas normotróficas, ptóticas e sem desejo de aumento do volume; 4 - mamas normotróficas, ptóticas e com desejo de aumento do volume final; 5 - mamas assimétricas e hipertróficas. Baseado nos achados clínicos, foi criado um algoritmo de tratamento para cada subtipo de assimetria, incluindo neste arsenal, próteses mamárias de volumes diferentes, mastopexias, mamoplastia redutoras, além da lipoenxertia. Importante ressaltar que a assimetria mamária é a regra e não a exceção, entretanto, é motivo de insatisfação das pacientes e um desafio para o cirurgião plástico.

ABSTRACT

Plastic surgery of the breast is one of the most popular in the world. These patients had breast enlargement, with large excess of loose skin. In such cases, the aesthetic result of breast reduction is unsatisfactory in most patients. It is reflected in the early return of these patients requesting the placement of breast implants. Based on, patient satisfaction, fewer cases of reduction mammoplasty revision, suggests the need to perform breast reduction with breast implant surgery, at the same time, in many selected cases.

RESUMO

A cirurgia plástica da mama é uma das mais realizadas no mundo. Em alguns casos de hipertrofia mamária, observa-se um grande excesso de pele e flacidez. O resultado estético da mamoplastia redutora é insatisfatório na maioria dos pacientes, ocasionando o retorno precoce destas pacientes, solicitando a colocação de implante mamário para a complementação do seu resultado. A observação de bons resultados, a satisfação das pacientes e a diminuição de casos complementares à mamaplastia redutora confirmam a necessidade de se realizar a mamoplastia redutora com colocação de implante mamário, num mesmo tempo cirúrgico, em casos selecionados.

Evaluation of nipple-areola complex position after breast augmentation surgery

ABSTRACT

INTRODUCTION: The augmentation mammoplasty commonly provokes in patients an inquiry about the possibility of breast falling. The plastic surgery usually answers yes, but don't find studies justifying this. In this research we focus to understand the position variation of papillary-areolar complex after breast implants observing principally the "ptosis tendency" and "bottoming out" phenomenon. METHODS: Medical records selection between patients operated in Instituto Ivo Pitanguy - Infirmary 38 of Santa Casa de Misericórdia do Rio de Janeiro since 2009 January until 2010 December based on inclusion and exclusion criterions using for this research medical records revision, photographer register and literature revision. RESULTS: Among the 20 breasts (10 patients) analyzed we observed "bottoming out" in 8 breasts (5 patients) 4 moderated and 4 mild. Occur "ptosis tendency" in 10 breasts (5 patients) 4 mild and 6 moderated. Only 1 breast didn't suffered APC variation. CONCLUSION: The "ptosis tendency" and "bottoming out" phenomenon are a reality that has to be largely studied to better elucidation for patients.

INTRODUÇÃO: A mastoplastia de aumento, muito comumente, gera na paciente indagações acerca da possibilidade de queda das mamas. A resposta do cirurgião plástico é geralmente afirmativa, porém, este não encontra respaldo na literatura. Neste trabalho, objetivamos justamente entender a variação de posição do Complexo aréolo-papilar pós-inclusão de implantes mamários observando, principalmente, os fenômenos de "tendência à ptose" e "bottoming out". MÉTODOS: Seleção de prontuários dentre as pacientes operadas no Instituto Ivo Pitanguy - Enfermaria 38 da Santa Casa de Misericórdia do Rio de Janeiro de jan/2009 a dez/2010, com base em critérios de inclusão e exclusão, utilizando-se para esta pesquisa de revisão dos prontuários, registro fotográfico e revisão da literatura. RESULTADOS: : Dentre as 20 mamas ( 10 pacientes ) analisadas observamos "bottoming out" em 8 mamas ( 5 pacientes ) das quais 4 de graus moderados e 4 leves. Houve "tendência à ptose" em 10 mamas (5 pacientes) sendo 4 leves e 6 moderadas. . CONCLUSÃO: Os fenômenos "bottoming out" e tendência à ptose" são uma realidade que precisa ser amplamente estudada para melhor esclarecimento das pacientes.

Breast Implant Illness: where are we and where are we going?

ABSTRACT

Despite more than 60 years of history, numerous studies and large population samples, in recent years, several patients have returned to the plastic surgeons office. They have doubts about the silicone, some due to the recent reports of BIAALCL, but the vast majority due to the possibility of systemic symptoms related to the implants and which arouse the desire for its removal. Procedure known as explant. This phenomenon, whose doubts are numerous and the responses are minimal, is known in the world literature as Breast Implant Illness (BII). On the Internet and social networks, hundreds of signs and symptoms have been related to silicone implants, usually nonspecific. The most common symptoms reported by patients are chronic fatigue, arthralgia, mental confusion, myalgia, memory loss, difficulty concentrating and dry eyes. So far, there are no diagnostic tests for BII, no method based on scientific evidence to differentiate it from other conditions, and there is very little knowledge about its onset, course, risk factors, causes and proper management. The option for removing the implants has been growing dramatically in recent years, being one of the ten most performed surgeries in the United States last year. The literature shows variable improvement rates after the explantation and the patients are, as a rule, satisfied with their aesthetic result and have lower levels of anxiety and stress after the procedure. Prospective, well-designed randomized studies correlating different periods are necessary, from the preoperative period of the implant until after the explantation.

Apesar dos mais de 60 anos de história, dezenas de estudos e grandes amostras populacionais, nos últimos anos diversos pacientes têm retornado ao consultório do cirurgião plástico. Apresentam dúvidas a respeito do silicone, algumas devido aos relatos recentes de BIA-ALCL (Anaplastic Large Cell Lymphoma), mas a grande maioria pela possibilidade de sintomas sistêmicos relacionados aos implantes e que despertam o desejo da sua remoção. Procedimento conhecido como explante. Este fenômeno, cujas dúvidas são inúmeras e as respostas mínimas, é conhecido na literatura mundial como Breast Implant Illness (BII). Na Internet e redes sociais, centenas de sinais e sintomas têm sido relacionados às próteses de silicone, usualmente inespecíficos. Os sintomas mais comuns referidos pelas pacientes são fadiga crônica, artralgia, confusão mental, mialgia, perda de memória, dificuldade de concentração e olhos secos. Até o momento, não existem testes diagnósticos para BII, nenhum método baseado em evidência científica para diferenciá-la de outras condições e há muito pouco conhecimento a respeito do seu início, curso, fatores de risco, causas e manejo adequado. A opção pela retirada dos implantes vem crescendo vertiginosamente nos últimos anos, sendo uma das dez cirurgias mais realizadas nos Estados Unidos no ano passado. A literatura mostra taxas de melhora dos sintomas variáveis após o explante e as pacientes mostram-se, via de regra, satisfeitas com seu resultado estético e apresentam níveis de ansiedade e estresse menores após o procedimento. São necessários estudos prospectivos, randomizados bem desenhados correlacionando períodos distintos das pacientes, desde o pré-operatório do implante até o pós-explante.

ABSTRACT

Introduction: Breast surgery with silicone implants is gaining popularity and has become the most performed plastic surgery worldwide. However, there is increasing concern about the safety of silicone breast implants due to associated complications.
Objective: To review existing technologies, technological trends, and existing methods to minimize complications related to silicone breast implants. Methods: We conducted a literature review of articles describing new technologies and trends to reduce complications related to silicone breast implants, along with information on patents and manufacturers of silicone breast implants.
Results: We initially identified 78 articles, out of which 40 were shortlisted for publication . All articles had a common aim of obtaining better results and reducing complications related to silicone implants, either in aesthetic or reconstructive surgeries.
Conclusion: The search for a breast implant that reduces possible and frequent complications, especially biofilm formation, infectious processes, and abnormal immune response, was the focus of most articles studied. Acellular dermal matrix and fat grafting have been reported in the literature as promising alternatives.

Introdução: A cirurgia de mama, especialmente as associadas aos implantes de silicone, teve uma crescente popularização, tornando-se a cirurgia plástica mais realizada no mundo. Junto com esta proliferação, observa-se um aumento da preocupação com a segurança dos implantes mamários de silicone, pelas intercorrências relacionadas.
Objetivo: Revisar métodos existentes para minimizar as complicações relacionadas com implante mamário de silicone, bem como as tecnologias existentes e tendências tecnológicas para implantes mamários de silicone.
Métodos: Foi realizada revisão de artigos científicos relacionados com novas tecnologias e tendências para redução das complicações relacionadas com implantes mamários de silicone, bem como as patentes e fabricantes de implante de silicone mamário.
Resultados: Identificamos inicialmente 78 referências, sendo reduzido para 40 para publicação, todos com linhas de pesquisas que buscam melhores resultados e redução das complicações relacionadas com implantes de silicone, seja esta cirurgia com objetivo estético ou reconstrutivo.
Conclusão: A busca por um implante mamário que reduza as possíveis e frequentes complicações, principalmente a formação do biofilme, processos infecciosos e resposta imune, é o foco da maioria das pesquisas encontradas. Com o mesmo objetivo, porém surgindo mais recentemente como alternativas, existem as pesquisas para o uso de matriz dérmica acelular e a lipoenxertia, com boas expectativas.

Transareolopapilar incision for breast augmentation: 10-year experience at the Ivo Pitanguy Institute

ABSTRACT

BACKGROUND: There are numerous access routes for inserting implants during breast augmentation surgery. In 1966, Pitanguy described the transareolopapilar route. The aim of this study was to assess the use of transareolopapilar incision during breast augmentation surgery at the Ivo Pitanguy Institute (Rio de Janeiro, RJ, Brazil), over the past 10 years. METHODS: Retrospective analyses of the size of the implants used, indications for transareolopapilar incision, postoperative complications, and postoperative scarring were performed. RESULTS: Fifty-three patients with a mean age of 33.54 years were included, and the mean follow-up period was 11.6 months. Most (60.4%) of the implants were <200 ml. Twelve patients required a second operation due to a breast lump (1 case); infection (1 case); capsular contracture (1 case); and dissatisfaction with breast shape (4 cases), volume (4 cases), and unilateral scarring (1 case). Sixteen (30.2%) patients developed some form of minor postoperative complication; 13 (24.5%) had one or more scarring issues, including hypochromia (18.9%), hypertrophy (1.9%), scar retraction (1.9%), and areola bifida (1.9%). Twenty (37.7%) patients underwent postoperative follow-up for more than one year and were satisfied with the postoperative scar. CONCLUSIONS: The transareolopapilar incision facilitates the insertion of small-to-moderate size implants with a low rate of postoperative complications and a low incidence of scarring, provided the correct surgical technique is used.

INTRODUÇÃO: Várias vias de acesso foram criadas para a inclusão de implantes na cirurgia de aumento das mamas. Em 1966, Pitanguy descreveu a via de acesso transareolopapilar. O objetivo do presente estudo é avaliar as mamoplastias de aumento realizadas no Instituto Ivo Pitanguy (Rio de Janeiro, RJ, Brasil), nas quais se utilizou a incisão transareolopapilar, nos últimos 10 anos. MÉTODO: Realizado estudo retrospectivo, analisando-se os seguintes parâmetros: tamanho dos implantes, indicação da incisão transareolopapilar e complicações pós-operatórias, como alterações cicatriciais. RESULTADOS: Foram incluídas no estudo 53 pacientes, com média de idade de 33,54 anos e tempo médio de seguimento de 11,6 meses. A maioria (60,4%) dos implantes possuía menos de 200 ml. Doze pacientes foram submetidas a reintervenções pelas seguintes razões: nódulo mamário (1 caso), infecção (1 caso), contratura capsular (1 caso), e insatisfação com a forma das mamas (4 casos), com o volume (4 casos) e com a cicatriz unilateral (1 caso). Dezesseis (30,2%) pacientes desenvolveram alguma complicação menor no pós-operatório e 13 (24,5%) apresentaram alguma alteração cicatricial no pós-operatório: hipocromia (18,9%), hipertrofia unilateral (1,9%), retração cicatricial unilateral (1,9%), e aréola bífida (1,9%). Vinte (37,7%) pacientes realizaram seguimento pós-operatório superior a um ano e relataram satisfação com a cicatriz. CONCLUSÕES: A incisão transareolopapilar permite a inclusão de implantes de tamanho pequeno a moderado, com baixo índice de complicações pós-operatórias e cicatriciais, desde que seguida a correta técnica cirúrgica.

Implant? Prostheses? Inclusion?

ABSTRACT

The author discusses the indiscriminate use of various terms to designate a single artifact and suggests the adoption of only one signifier. Although no publications deal specifically with the meaning of the terms "implant", "prosthesis" or "inclusion", apart from what is found in ordinary lexical and medical dictionaries, such terms are commonly and interchangeably used to designate a single artifact. According to the author, it is beneficial to select and utilize only the term that best conceptualizes the object designed for correcting lack of breast volume in specialty terminology. The use of a single term will facilitate scientific paper writing; therefore, the author proposes the use of the term "implant" to indicate the artifact placed inside the mammary gland.

RESUMO

O autor discute o uso indiscriminado de vários termos para designar um único artefato e sugere a adoção de apenas um significante. Embora não se encontrem publicações que tratem especificamente do significado dos termos implante, prótese e inclusão, a não ser o que se encontra nos dicionários léxicos e médicos comuns, esses termos têm sido usados rotineiramente para designar um único artefato. O autor considera ser possível incluir na nomenclatura da especialidade o termo que melhor conceitue o referido objeto destinado à correção da falta de volume mamário. O uso de um único termo facilitaria a redação de trabalhos científicos, e, por esse motivo, o autor propõe o uso do termo implante para o artefato colocado dentro da glândula mamária.
