ABSTRACT

BACKGROUND: Scalping is a trauma caused by partial or total avulsion of the scalp. It is primarily due to accidental contact of long hair with the rotary shaft of a motor. Rotary shafts suck in the victim's hair, causing traction and abruptly pulling the scalp off. METHODS: A longitudinal, observational, retrospective, and descriptive study of 62 scalping victim-patients treated at Fundação Santa Casa de Misericórdia do Pará was conducted. RESULTS: The most commonly affected patients were children between 7 and 16 years of age (56.4%) and women (93.5%); 59.7% were students and 43.5% were Catholics. CONCLUSIONS: Prophylactic campaigns about the risks of traveling in and maintaining a vessel without motor shaft protection are essential because scalping is primarily caused by hair twisting around the motor shaft.

RESUMO

INTRODUÇÃO: Escalpelamento é trauma causado por avulsão parcial ou total do couro cabeludo, decorrente, principalmente, de contato acidental dos cabelos longos com motor de eixo rotativo. A alta rotação desses motores gera uma força que suga os cabelos da vítima, tracionando e arrancando o couro cabeludo de forma abrupta. MÉTODO: Foi realizado um estudo observacional transversal, retrospectivo, descritivo, incluindo 62 pacientes vítimas de escalpelamento tratados na Fundação Santa Casa de Misericórdia do Pará. RESULTADOS: Os pacientes mais acometidos foram crianças de 7 anos a 16 anos (56,4%), do gênero feminino (93,5%), sendo 59,7% estudantes e que, predominantemente, praticavam a religião católica (43,5%). CONCLUSÕES: Campanhas profiláticas sobre os riscos de se viajar e manter uma embarcação sem proteção no eixo do motor são fundamentais, uma vez que a tragédia do escalpelamento é causada principalmente pelos cabelos que se enrolam no eixo do motor.

Distant flap transfer with the vascular pedicle section in second stage

ABSTRACT

Introduction: Occasionally, there is no possibility of covering a wound with local grafts or flaps. This study aimed to evaluate the capacity of the distant flap transfer with the vascular pedicle section in second stage to cover the region that suffered substance loss.
Methods: Five patients with substance loss, with no reconstruction option using grafts or local flaps, had their wounds covered by distant flap transfer with the vascular pedicle section in second stage.
Results: The transferred flaps were useful in covering the wounds.
Conclusion: distant flap transfer with the vascular pedicle section in second stage is a simple and effective procedure. Every plastic surgeon must master that.

RESUMO

Introdução: Ocasionalmente não há possibilidade de se cobrir uma ferida com enxertos ou retalhos locais. O objetivo deste trabalho foi avaliar a resolutividade da transferência de retalho à distância com secção do pedículo vascular em segundo tempo para cobertura de região que sofreu perda de substância.
Métodos: Cinco pacientes com perda de substância, sem opção de reconstrução com enxertos ou retalhos locais, tiveram suas feridas cobertas por retalhos transferidos à distância, com secção dos pedículos vasculares em segundo tempo.
Resultados: Os retalhos transferidos foram eficazes na cobertura das feridas.
Conclusão: A transferência de retalho à distância com secção do pedículo vascular em segundo tempo é procedimento simples e eficaz, que deve ser dominado por todo cirurgião plástico.

ABSTRACT

Pyoderma gangrenosum is a chronic and recurrent disease which presents cutaneous necrosis and ulcerations. Its occurrence at skin graft donor sites is unknown and there are only few mentions about that. As the authors did not find any association of pyoderma gangrenosum and skin graft donor sites in literature, they decided to describe this case. The clinical evolution of a 45-year-old white woman, who presented difficult cicatrization ulcers at skin grafts donor sites is related, emphasizing that the delayed time the diagnosis was suspected led to retarded specific treatment. Concerning that, it is important the knowledge about the pathergy phenomenon, that describes the development of a new inflammatory lesion at the site of trauma. In pathergy condition, a minor trauma can be followed by progressive destruction of healthy skin. Recognition of postsurgical pyoderma gangrenosum has generally been delayed because consideration is given to the most likely diagnosis of infection or wound breakdown. The destructive nature of ulcers appears with the progression of illness, which contributes to the correct diagnosis with the negative culture and compatible biopsy. In this case, some lesions would be avoided if precocious treatment had been applied. Therefore, a high suspecting guidance must always be done in these situations.

RESUMO

O pioderma gangrenoso é doença crônica e recorrente, apresentando necrose e ulcerações cutâneas. Sua ocorrência em áreas doadoras de enxertos cutâneos é desconhecida, vendo-se poucas menções a respeito. Como nenhuma associação entre pioderma gangrenoso e áreas doadoras de enxertos cutâneos foi encontrada na literatura, os autores decidiram descrever este caso. A evolução clínica de uma paciente de 45 anos, com úlceras de difícil cicatrização em áreas doadoras de enxertos de espessura parcial é relatada, enfatizando-se que a demora no diagnóstico de pioderma gangrenoso retardou o início do tratamento específico. Nesse contexto, é importante o conhecimento do fenômeno da patergia, que é o desenvolvimento de uma nova lesão inflamatória na área do trauma. Na condição de patergia, um mínimo trauma pode ser seguido por uma progressiva destruição da pele saudável. O reconhecimento de pioderma gangrenoso pós-cirúrgico, geralmente, é demorado, pois os diagnósticos mais comuns são primeiramente considerados, como infecção ou deiscência da ferida. A natureza destrutiva das úlceras aparece com a progressão da doença, o que contribui para o diagnóstico correto, com cultura negativa e biópsia compatível. No caso em questão, algumas lesões poderiam ter sido evitadas se a terapia precoce tivesse sido aplicada. Tal suspeita deve ser, portanto, sempre aventada nessas situações.

Treatment of leprosy-induced plantar ulcers

ABSTRACT

Introduction: Leprosy-induced plantar ulcers result from a lack of plantar sensitivity. Objective: This study aimed to describe the treatment provided to patients with leprosy-induced plantar ulcers.
Methods: We retrospectively reviewed the medical records of patients with plantar ulcers treated at Sarah Hospital in Brasilia from 2006 to 2016 and collected information about sex, age, etiology, location, and treatment.
Results: A total of 27 patients (17 [62.96%] men, 10 [37.04%] women; 40.74% were aged 41-60 years) were treated from Goiás and the Federal District. All required ≥1 surgical procedure.
Conclusion: A higher frequency of advanced grade was observed in men, primarily on the first toe. All needed surgical and non-surgical procedures and achieved complete wound healing. Transtibial amputation was required in 1 case and toe amputation in 7 cases; 90% patients developed ulcer recurrence after 1 year.

RESUMO

Introdução: A úlcera plantar por hanseníase é uma lesão no pé resultante da falta de sensibilidade plantar. O objetivo é descrever o tratamento realizado em portadores de úlceras plantares por hanseníase.
Métodos: Estudo de prontuários de portadores de úlcera plantar atendidos no Hospital Sarah em Brasília, de 2006 a 2016, quanto ao sexo, idade, etiologia, localização e tratamento.
Resultados: Foram atendidos 27 pacientes, 17(62,96%) homens e 10 (37,04%) mulheres, procedentes de Goiás e DF, na faixa etária de 41 a 60 anos (40,74%). Todos necessitaram de um ou mais procedimentos cirúrgicos.
Conclusão: Observou-se maior frequência no sexo masculino, grau avançado, localizadas no primeiro artelho. Todos necessitaram de procedimentos cirúrgicos e não cirúrgicos, evoluindo com cicatrização completa da ferida, amputação transtibial em um caso e de artelhos em sete casos, e 90% dos casos apresentaram recorrência da úlcera após um ano.

Comparison between surgical sutures and Prineo® in terms of esthetic result and scar formation

ABSTRACT

INTRODUCTION: Wound healing is a well-organized, directed process of tissue repair. The process can be expedited using topical glues and adhesives, which offer a non-invasive, easily removable alternative to suturing. Furthermore, such products have good tensile strength and involve lower application time. In particular, the Prineo® adhesive is applied to a polyester mesh that covers the wound. METHOD: We carried out a retrospective, cross-sectional study, with subsequent statistical analysis , involving 101 surgical procedures in which wound closure was performed using either nylon sutures or Prineo®. All the procedures were performed between 2012 and 2014. RESULTS: Six patients had contact dermatitis after Prineo® was used, with statistical significance (p = 0.042). Furthermore, Prineo® decreased the rate of scar enlargement (p < 0.05) . There was no statistical difference between the Prineo® and suture groups in terms of scar quality (p = 0.068); in both groups, the scar result was mostly excellent (87 %) to good (27%). CONCLUSION: Patients whose wounds were closed using Prineo® a system involving octyl-2-cyanoacrylate and an associated polyester mesh displayed lower rates of scar enlargement, which depended on whether the thickness of the dermis was satisfactory. However, the same patients had higher rates of contact dermatitis than those whose wounds were closed using surgical sutures. Both groups showed excellent to good scar quality, with no significant difference in terms of esthetic scar results.

RESUMO

INTRODUÇÃO: Cicatrização de ferida é um processo bem organizado que tem como finalidade a reparação tecidual completa. Colas e adesivos tópicos oferecem uma alternativa não invasiva, de retirada fácil e espontânea; boa força tênsil; menor tempo de aplicação, sendo o Prineo® uma cola adesiva associada a uma malha de poliéster aplicados sobre a ferida. MÉTODO: Trata-se de um estudo transversal retrospectivo com análise de 101 procedimentos cirúrgicos no período de 2012 a 2014, nos quais a síntese da ferida operatória ocorreu com fios cirúrgicos de náilon ou Prineo®, sendo aplicada análise estatística. RESULTADOS: Neste estudo, seis pacientes apresentaram dermatite de contato ao uso do Prineo® com significância estatística (p = 0,042). O uso desse sistema diminuiu a taxa de alargamento cicatricial (p < 0,05). O presente trabalho não apresentou diferença estatística (p = 0,068) na qualidade da cicatriz entre os pacientes que utilizaram Prineo® em relação ao grupo controle, demonstrando que em ambos os grupos o resultado cicatricial foi de excelente (87%) a bom (27%) na sua maioria. CONCLUSÃO: Conclui-se no estudo que os pacientes que utilizaram o sistema de octil-2-cianocrilato associado a malha, Prineo®, apresentaram menores índices de alargamento cicatricial, dependentes de uma espessura de derme satisfatória, e maiores taxas de dermatites por contato em relação àqueles em que a ferida foi encerrada com fios cirúrgicos. Os dois grupos demonstraram qualidades cicatriciais excelentes a bons, sem diferença estatística em tais resultados estéticos cicatriciais.

Reconstructive surgery in the context of Covid-19: complications in the treatment of an inguinal complex wound

ABSTRACT

Introduction: At the end of 2019, the world saw the emergence of a new respiratory syndrome called Covid-19, caused by a new type of coronavirus, Sars-CoV-2. Classified as a pandemic, it has caused impacts of considerable magnitude.
Case Report: A 57-year-old man developed a right inguinal wound after surgical exploration for infection of a prosthesis used in a femur-popliteal bypass. The Plastic Surgery team opted for treatment with surgical debridement associated with negative pressure therapy to prepare the wound bed. In the postoperative period, he had severe acute respiratory syndrome and suspected Covid-19, requiring intubation and intensive care. A sample for RT-PCR of Sars-CoV-2 was collected, and the medications chloroquine and azithromycin were associated with the treatment. Despite intensive treatment, the patient died. The result of the RT-PCR test for the new coronavirus was positive, being released two days after death.
Discussion: The analysis of this report allows us to suppose that the patient probably contracted the new coronavirus at the hospital, as he was hospitalized for 35 days before the evolution of respiratory failure. This fact, together with its unfavorable evolution, corroborates the orientation of minimizing hospitalizations and surgical procedures as much as possible to promote more safety for the patient and the health team.
Conclusion: Inpatients are susceptible to infection with the new coronavirus and can set up a group at higher risk since many of them are already weakened.

RESUMO

Introdução: No final de 2019, o mundo viu surgir uma nova síndrome respiratória denominada Covid-19, causada por um novo tipo de coronavírus, o Sars-CoV-2. Classificada como uma pandemia, ela tem causado impactos de magnitude ainda imensuráveis.
Relato de caso: Homem de 57 anos desenvolveu ferida inguinal direita, após exploração cirúrgica por infecção de prótese usada em bypass femoro-poplíteo. A equipe de cirurgia plástica optou pelo tratamento com desbridamento cirúrgico, associado com terapia por pressão negativa para preparo do leito da ferida. No pós-operatório, apresentou síndrome respiratória aguda grave e suspeita de Covid-19, com necessidade de intubação e de cuidados intensivos. Foi colhido amostra para RT-PCR do Sars-CoV-2 e associado ao tratamento as medicações cloroquina e azitromicina. Apesar do tratamento intensivo, o paciente foi a óbito. O resultado do exame RT-PCR para o novo coronavírus foi positivo, sendo liberado dois dias após a morte.
Discussão: A análise deste relato permite supor que o paciente provavelmente contraiu o novo coronavírus dentro do próprio hospital, pois o mesmo encontrava-se internado pelo período dos 35 dias anteriores à evolução para insuficiência respiratória. Esse fato, juntamente com sua evolução desfavorável, corrobora a orientação de minimizar ao máximo as internações e os procedimentos cirúrgicos a fim de promover maior segurança ao paciente e à equipe de saúde.
Conclusão: Pacientes internados estão susceptíveis à infecção pelo novo coronavírus e podem configurar grupo de maior de risco, uma vez que muitos deles já se encontram debilitados.

Successful treatment of pyoderma gangrenosum after reducing mammoplasty: a case report

ABSTRACT

Introduction: Pyoderma gangrenosum is a rare inflammatory disease characterized by the presence of ulcerated lesions. The etiology is unknown but apparently relates to immunological factors. The authors report a case of pyoderma gangrenosum in the postoperative period of a reduction mammoplasty, treatment, and the importance of early diagnosis to obtain a good aesthetic result.
Methods: This is an 18-year-old patient who developed pyoderma gangrenosum after a reduction mammoplasty. The initial symptoms were the formation of violet blisters and ulcerated lesions in both breasts. The disease was suspected, and the patient was promptly treated with prednisolone. Cultures, biopsies and blood tests were performed. Hyperbaric therapy and nutritional support were performed. Daily dressings with epidermal growth factors were performed. After 60 days of follow-up, it was possible to achieve adequate wound healing and good aesthetic results.
Results: Early diagnosis associated with systemic immunosuppressive therapy and rigorous local care were fundamental for disease stabilization. The association with hyperbaric therapy and nutritional support contributed to the lesion's epithelialization and the clinical picture control. The approximation of the wound edges with subdermal suture and the subsequent intradermal suture was possible due to the recovery of the dermis and stabilization of the disease.
Conclusion: Early diagnosis of pyoderma gangrenosum is essential to achieve stabilization of the disease. It is possible to achieve good aesthetic results based on four elements: early systemic corticosteroids, daily local dressings with epidermal growth factor, nutritional support, and hyperbaric therapy.

RESUMO

Introdução: O pioderma gangrenoso é uma doença inflamatória rara caracterizada pela presença de lesões ulceradas. A etiologia é desconhecida, mas aparentemente relaciona-se com fatores imunológicos. Os autores relatam um caso de pioderma gangrenoso no pós-operatório de uma mamoplastia redutora, o tratamento e a importância do diagnóstico precoce para obtenção de um bom resultado estético.
Métodos: Trata-se de uma paciente de 18 anos que desenvolveu pioderma gangrenoso após uma mamoplastia redutora. Os sintomas iniciais foram a formação de bolhas de coloração violácea e lesões ulceradas em ambas mamas. Suspeitou-se a doença e a paciente foi tratada prontamente com prednisolona. Foram realizadas culturas, biópsias e exames sanguíneos. Realizou-se terapia hiperbárica e suporte nutricional. Foram realizados curativos diários com fator de crescimento epidérmico. Após 60 dias de acompanhamento foi possível conseguir uma cicatrização adequada da feridas e bons resultados estéticos.
Resultados: O diagnóstico precoce associado a uma terapia sistêmica imunossupressora e cuidados locais rigorosos foram fundamentais para a estabilização da doença. A associação com terapia hiperbárica e o suporte nutricional contribuíram para a epitelização das lesões e o controle do quadro clínico. A aproximação das bordas da ferida com sutura subdérmica e posterior sutura intradérmica foram possíveis devido à recuperação da derme e estabilização da doença.
Conclusão: O diagnóstico precoce de pioderma gangrenoso é fundamental para conseguir a estabilização da doença. É possível atingir bons resultados estéticos baseados em 4 elementos: corticoide sistêmico precoce, curativos locais diários com fator de crescimento epidérmico, suporte nutricional e terapia hiperbárica.

The medial plantar flap: a case series

ABSTRACT

INTRODUCTION: Reconstructive coverage of foot defects, especially those of the calcaneus region, is a unique technical challenge for the surgeon due to the high degree of specialization of the tissues involved and the relative immobility of the proximal tissues. METHODS: In the present study, we present our experience with the use of the flap based on the medial plantar artery to cover tissue defects in the foot, especially in the region of the calcaneal fat pad. Twelve medial plantar artery flaps constructed from 2001 to 2013 at the As a Norte Regional Hospital, Brasília, DF, were included. RESULTS: Of the 12 patients with the medial plantar artery flaps, 10 were men and 2 were women. The indications were traumatic tissue loss of the plantar fat pad in 10 patients and of the dorsum of the foot in two cases. All flaps were elevated as fasciocutaneous pedicle flaps based on the medial plantar artery. All the flaps healed without major complications, except one case that involved partial loss . The donor area was covered with a partial skin graft and there was one case of partial graft loss. The flap displayed a slightly lower protective sensitivity than the normal side. CONCLUSION: According to the results, the medial plantar flap is a good option for covering traumatic foot defects, especially those affecting the calcaneus. The versatility of the flap allows the coverage of defects of the calcaneus (e.g., the Achilles tendon and plantar fat pad regions), as well as those affecting the dorsum of the foot. The coverage of the plantar fat pad region with skin of similar texture and protective sensitivity confers to the medial plantar flap a large advantage over other flaps for reconstruction of this region.

RESUMO

INTRODUÇÃO: A cobertura do pé e especialmente da região do calcâneo são desafios técnicos para o cirurgião devido ao alto grau de especialização dos tecidos envolvidos e à relativa imobilidade dos tecidos próximos. MÉTODOS: No presente estudo, apresentamos nossa experiência com uso do retalho baseado na artéria plantar medial para cobertura de defeitos teciduais no pé, especialmente na região de apoio plantar no calcâneo. Doze retalhos da artéria plantar medial feitos de 2001 a 2013 no Hospital Regional da Asa Norte, Brasília, DF, foram incluídos. RESULTADOS: Dos 12 pacientes, 10 eram homens e dois eram mulheres. As indicações foram perda traumática do coxim do calcâneo em 10 pacientes e dorso do pé em dois casos. Todos os retalhos foram elevados como retalhos pediculados fasciocutâneos baseados na artéria plantar medial. Todos os retalhos cicatrizaram sem maiores complicações, exceto um caso com perda parcial. A área doadora foi coberta com enxerto de pele parcial e houve um caso de perda parcial do enxerto. Os retalhos apresentaram uma sensibilidade protetora levemente inferior ao lado normal. CONCLUSÃO: De acordo com os resultados, o retalho plantar medial é uma boa opção para cobertura do pé, especialmente do calcâneo. A versatilidade do retalho permite a cobertura de defeitos no calcâneo sobre o tendão de Aquiles e apoio plantar, assim como o dorso do pé. A cobertura da região de apoio plantar com pele de textura similar e sensibilidade protetora confere a esse retalho uma grande vantagem sobre outros retalhos para reconstrução dessa região.

ABSTRACT

Prototyping is a process of construction in order to obtain physical prototypes from 3D digital models. The introduction of prototyping in medicine is a recent event. Case 1: An 18-year-old patient with a 192 cm2 cranial defect secondary to a decompressive craniectomy. A cranioplasty was performed using a customized prosthesis manufactured by prototyping. Case 2: A 34-year-old patient with a panfacial fracture sequelae. This patient had a relevant defect in the zygomatico-orbital complex, with great zygoma dislocation. Surgical planning was performed using the prototype, which was taken to the surgery room to support the surgery. Case 3: A 29-year-old patient with a mandibular ameloblastoma from the subcondylar region to the right parasymphysis. Prototyping of the craniofacial skeleton and fibula were carried out in addition to a model surgery that was performed on the previous day. Discussion: Prototyping can be applied to craniofacial surgery in many ways, such as customized prosthesis manufacturing, surgical planning, and education of residents and patients. In case 1, a customized prosthesis had the advantage of not requiring a donor area and resulted in excellent esthetic results. In case 2, the presence of the prototype during surgery helped identify the zygoma more quickly in order to reposition it. In case 3, the model surgery helped decrease morbidity of the donor area, define fibular osteotomy and the resection margins, mold the plate, select the screws, ensure the condylar prosthesis fit in the temporomandibular joint (TMJ ), achieve the best occlusion possible, and reduce the surgical time, anesthesia, and hospital-associated costs.

RESUMO

A prototipagem é um processo de construção para obter protótipos físicos a partir de modelos 3D digitais. A introdução da prototipagem na medicina é recente. Caso1 - Paciente de 18 anos portador de defeito craniano de 192 cm2 secundário a uma craniotomia descompressiva. Foi feita uma cranioplastia com prótese customizada confeccionada a partir da prototipagem. Caso 2 - Paciente de 34 anos portador de sequela de fratura panfacial. Apresentava importante defeito do complexo zigomático-orbitário, com grande deslocamento do zigoma. O plano cirúrgico foi feito com o protótipo; e o protótipo foi levado à sala de cirurgia para auxiliar o ato operatório. Caso 3 - Paciente 29 anos portadora de um ameloblastoma de mandíbula da região subcondilar até parassínfise direita. Foi feita a prototipagem do esqueleto craniofacial e fíbula, e uma cirurgia de modelo 1 dia antes. Discussão: A prototipagem pode ter aplicação na cirurgia craniofacial de várias maneiras: confecção de próteses customizadas, planejamento cirúrgico e educação dos residentes e pacientes. No caso 1, a prótese customizada tem como vantagens a ausência de área doadora e o excelente resultado estético. No caso 2, a presença do protótipo na sala de cirurgia ajudou a identificar com mais rapidez o zigoma para resposicioná-lo. No caso 3, a cirurgia de modelo diminuiu a morbidade da área doadora; definiu a osteotomia da fíbula e as margens de ressecção; moldou a placa; escolheu os parafusos; encaixou a prótese de côndilo na ATM; alcançou a melhor oclusão possível; e diminuiu tempo cirúrgico, anestesia, e custo hospitalar.

Elastic suture as an alternative for the initial wound care in the emergency room

ABSTRACT

This is a case report of a 44-year-old male patient who was admitted to the emergency room on November 17, 2013, after an accident with a Makita Disc. The patient presented with an extensive wound throughout the dorsal surface of the left thumb, with tendon exposure. The use of elastic suture is demonstrated as the initial care in the emergency department. We present satisfactory resolution in a single surgical procedure of traumatic wounds in a patient presenting with loss of tissue. The chosen surgical approach presented adequate aesthetic result and functional preservation.

RESUMO

Relato do caso de um paciente do sexo masculino, 44 anos, que deu entrada pelo Serviço de Emergência, no dia 17 de novembro de 2013, vítima de acidente com disco de Makita, apresentando ferida extensa em toda face dorsal do primeiro quirodáctilo esquerdo e exposição tendínea. Demonstra a utilização de sutura elástica, em atendimento inicial na urgência, e a possibilidade de tratamento de ferida traumática com perda de substância, com resolução satisfatória em único tempo cirúrgico. A opção cirúrgica apresentou resultado funcional adequado e preservação estética.

Scalp reconstruction with large dorsal muscle-free flap after dog bite scalping

ABSTRACT

Introduction: Scalping is characterized by trauma in the scalp region, which can be classified as partial or total. Scalping trauma is extremely mutilating and stigmatizing, especially when exposing the bone without periosteum, leading to chronic osteomyelitis and external table erosion. When there are extensive scalp lesions above 200cm2, and with periosteum, the lesion is required a large amount of tissue with microsurgical flap, which is not available in all centers. This work aims to report a case of a 69-year-old female patient who suffered trauma due to total avulsion of a big scalp of 550cm2 with exposure of a skull cap without periosteum and total unviability of the scalp after a dog bite. Due to the total unviability of the avulsed scalp, we opted to transplant a large dorsal muscle free flap with microvascular anastomosis of the thoracodorsal pedicle with the superficial temporal vessels. The flap evolved with good perfusion, and partial grafting was performed in the bloody area.
Methods: Retrospective analysis of the medical records of the patient in question. This paper follows the Declaration of Helsinki's standards and the approval of the Ethics and Research Committee.
Conclusion: The free flap of the large dorsal muscle proved effective in this case of reconstruction of the scalp's extensive lesion (550cm2) with partial periosteum lesion due to scalping. The flap recovered the shape of the skull and the protective function of the skull cap.

RESUMO

Introdução: O escalpelamento é caracterizado pelo trauma em região do couro cabeludo, que pode ser classificado como parcial ou total. O trauma por escalpelamento é extremamente mutilante e estigmatizante, principalmente quando expõe o osso sem periósteo, que pode levar a quadro de osteomielite crônica e erosão de tábua externa. Quando há lesões extensas de couro cabeludo acima de 200cm² e com lesão de periósteo é necessária grande quantidade de tecido com retalho microcirúrgico, que não está disponível em todos os centros. O objetivo deste trabalho é relatar caso de paciente de 69 anos, feminina, que sofreu trauma por avulsão total de couro cabeludo de grande extensão de 550cm² com exposição de calota craniana sem periósteo e inviabilização total do escalpe após mordida de cachorro. Devido à inviabilização total do escalpe avulsionado, optou-se pelo transplante de retalho livre de músculo grande dorsal com anastomose microvascular do pedículo toracodorsal com os vasos temporais superficiais. O retalho evoluiu com boa perfusão e na área cruenta foi realizado enxertia parcial.
Métodos: Análise retrospectiva de prontuário da paciente em questão. O presente trabalho segue os padrões de declaração de Helsinque e aprovação do comitê de ética e pesquisa.
Conclusão: O retalho livre de músculo grande dorsal mostrou-se eficaz neste caso de reconstrução de lesão extensa do couro cabeludo (550cm²) com lesão parcial de periósteo devido ao escalpelamento. O retalho recuperou a forma do crânio e a função de proteção da calota craniana.

ABSTRACT

Pyoderma gangrenosum (PG) is an inflammatory disease of the skin that may develop spontaneously. It is associated with certain systemic and neoplastic diseases, including those of the breasts. PG is also associated with surgical trauma. It has been increasingly reported, along with the increase in the incidence of reduction mammoplasty procedures. The clinical manifestation of ulcers is characteristic of PG and it should be considered in cases of poor healing with intense inflammatory reaction, tissue loss, bloody and/ or purulent secretion, granular background, and lesions with high edges. We describe a patient who developed PG after reduction mammoplasty. She has since responded to systemic corticosteroids and has had no relapse to date.

RESUMO

O pioderma gangrenoso (PG) é doença inflamatória da pele, que pode se desenvolver espontaneamente, associado a certas doenças sistêmicas e neoplásicas, ou ao trauma cirúrgico, incluindo os das mamas. Há relatos cada vez mais frequentes, considerando o aumento desse procedimento nos dias atuais. A manifestação clínica das úlceras é característica e deve ser lembrada nas evoluções cicatriciais desfavoráveis com intensa reação inflamatória, perdas teciduais, secreção sanguinolenta e/ ou purulenta, fundo granuloso e bordas elevadas. Relatase o caso de paciente que teve pioderma gangrenoso após mamoplastia redutora. Respondeu ao corticosteroide sistêmico, e vem evoluindo sem recidivas até o momento.

ABSTRACT

Introduction: Cullen’s postoperative gangrene, also called pyoderma gangrenosum (PG) or sterile neutrophilic abscess, was first described in the medical literature by Cullen in 1924. Later Brusting et al., in 1930, described PG in more detail.
Objective: To report a rare case of pyoderma gangrenosum (PG) in an extraneous limb that was triggered by blunt trauma to the dorsum of the hand.
Discussion: A histopathological exam is not sufficient to diagnose PG, Therefore, a PG diagnosis is based on clinical evidence. The clinical presentation is variable and includes rare bullous, pustular and vegetative forms. Other rare forms of PG occur at sites of pathergy (20-30%), periostomal skin, the dorsum of the hand, the head and neck. PG can also be multisystemic and paraneoplastic.
Conclusion: The appearance of sudden-onset PG is rare in the dorsum of the hand. It is important to classify PG’s clinical forms, and to establish associations between PG and underlying pathologies. It is also very important to avoid surgery during early PG. The many drugs used to treat PG, and the multiple patient responses, demonstrate the difficulty of standardizing treatment. Physicians may have to use an empirical approach to select the appropriate drug for each patient.

Introdução: A gangrena pós-operatória de Cullen, também denominada de pioderma gangrenoso (PG) ou abscesso neutrofílico estéril, pelo fato das lesões cutâneas não conterem micro-organismos patogênicos teve sua primeira aparição na literatura médica no ano de 1924, pelo relato feito por Cullen, segundo Schofer e Baur. Mais tarde Brusting et al., em 1930, acrescentaram maiores detalhes a descrição inicial.
Objetivo: Relatar um caso raro de pioderma gangrenoso (PG) em extremidades de membros desencadeado por um trauma contuso em dorso de mão.
Discussão: O exame histopatológico do PG não é diagnóstico, portanto, a elucidação do quadro se baseia em evidências clínicas. A apresentação clínica é variável: a bolhosa, pustulosa e vegetante. Outras formas raras incluem o PG em locais de patergia (20-30%), periostomal, dorso da mão, PG maligno ou da cabeça e pescoço, multisistêmico e paraneoplásico.
Conclusão: O aparecimento do PG é de início súbito e raro em dorso de mão. É importante classificar sua forma clínica, estabelecer associações com patologias de base. A diversidade de drogas para a terapêutica demonstra a dificuldade de padronização de tratamento com variadas respostas, que pode ser empírico e evitar abordagem cirúrgica precoce.

ABSTRACT

Pyoderma gangrenosum is a disease characterized by painful ulcers, with imprecise borders of various sizes and depth. The diagnosis is essentially clinical and often exclusionary. The treatment is not specific. Surgical debridement is not indicated because of its potential pathergy. We report a case of pyoderma gangrenosum in a patient who underwent abdominoplasty.

Pioderma gangrenoso é uma doença caracterizada por úlceras dolorosas, com bordos imprecisos, de diversos tamanhos e profundidade. O diagnóstico é eminentemente clínico, sendo muitas vezes de exclusão. O tratamento não é específico. O desbridamento cirúrgico não é indicado devido a seu potencial de patergia. Neste trabalho, relatamos um caso de pioderma gangrenoso em uma paciente submetida à abdominoplastia.

Elastic suture: a treatment option for extensive skin loss

ABSTRACT

Extensive skin loss presents major challenges for plastic surgeons, making it necessary to develop different techniques to close extensive wounds. In this context, the elastic suture technique was performed in two patients at the Independência Hospital of the Divina Providência Group in Porto Alegre. This alternative technique was fast and effective and had a low cost. The technique is divided into two stages: the elastic suture itself, which consists of the approximation of the wound edges with the aid of an elastic band; and closure of the skin already approximated in the previous stage. The elastic suture is a highly safe and simple closure technique with the ability to approximate the edges of extensive wounds, avoiding the use of more complex techniques in some cases.

RESUMO

Em meio a tantos desafios aos quais os cirurgiões plásticos são impostos quando se trata de grandes perdas cutâneas, faz-se necessário o desenvolvimento de diferentes métodos de fechamento de grandes feridas. Assim sendo, foi realizada a técnica da sutura elástica em dois pacientes do Hospital Independência do grupo Divina Providência em Porto Alegre, uma alternativa que se demonstrou rápida, eficaz e de baixo custo. A técnica é dividida em duas etapas, a primeira com a sutura elástica propriamente dita - na qual consiste na maior aproximação dos bordos com auxílio de um elástico -, e a segunda com o fechamento da pele já aproximada pela fase anterior. Assim sendo, com o uso da técnica nesses dois pacientes, foi possível concluir que a sutura elástica é uma técnica de fechamento simples, com grande segurança e funcionalidade para aproximação de bordos de grandes feridas, evitando, em alguns casos, o uso de técnicas mais complexas.

Eponychium flap for nail lengthening: series of cases

ABSTRACT

Introduction: The eponychium flap for nail lengthening has been shown to improve the aesthetic result in partial fingertip amputations. The aim of this work is to describe our experience using a modification of the eponychium flap described by Bakhach, at the University Hospital.
Method: We included 6 patients from the Chair of Plastic Surgery of the University Hospital assisted in 2018, with fingertip injuries treated with eponychium flap for nail lengthening, with modified Bakhach technique.
Results: Good to very good results were achieved in all cases, with a nail lengthening of 3 to 5 mm. There was one complication (infection).
Conclusions: The eponychium flap achieves very good esthetic results in fingertip defects with partial compromise of the nail bed. It is an easily reproducible technique, which can be performed in the same act as the reconstruction of soft parts with local flaps.

RESUMO

Introdução: O retalho do eponíquio para alongamento ungueal tem provado melhorar notavelmente o resultado estético em amputações parciais da ponta do dedo. O objetivo deste estudo é descrever a nossa experiência utilizando uma modificação do retalho do eponíquio descrito por Bakhach, no Hospital Universitário.
Método: Foram incluídos 6 pacientes da Cátedra de Cirurgia Plástica do Hospital Universitário assistidos em 2018, com feridas da ponta do dedo tratadas com retalho do eponíquio para alongamento ungueal, com técnica de Bakhach modificada.
Resultados: Resultados bons a muito bons foram obtidos em todos os casos, com um alongamento ungueal de 3 a 5 mm. Surgiu uma complicação (infecção).
Conclusão: O retalho de eponíquio alcança resultados estéticos muito bons em defeitos na ponta do dedo, com compromisso parcial do leito ungueal. É uma técnica facilmente reprodutível, que pode ser realizada no mesmo ato que a reconstrução de partes moles com retalhos locais.

Systematization of dressings for clinical treatment of wounds

ABSTRACT

The treatment of cutaneous wounds includes both medical and surgical methods; dressing is one of the most commonly used clinical treatments. An extensive therapeutic toolkit comprising passive dressings or dressings with active principles can help repair wounds in various situations. Dressings are used to improve the conditions of the wound bed and may occasionally be considered the definitive treatment, whereas in some cases, they may be considered an intermediate step to surgical treatment. Intelligent and biological wound dressings are currently classified as dermal substitutes and will not be discussed in this article. Dressings should be selected on the basis of knowledge of the pathophysiology of wound healing and tissue repair while keeping the systemic problems of the patient in mind.

RESUMO

O tratamento das feridas cutâneas inclui métodos clínicos e cirúrgicos, sendo o curativo um dos tratamentos clínicos mais frequentemente utilizados. Um vasto arsenal terapêutico composto por curativos passivos ou com princípios ativos é capaz de auxiliar na reparação do tegumento em diversas situações. Curativos visam a melhorar as condições do leito da ferida, podendo ser, em algumas ocasiões, o próprio tratamento definitivo, mas em muitas situações constituem apenas uma etapa intermediária para o tratamento cirúrgico. Curativos inteligentes e biológicos são hoje mais bem classificados como substitutos cutâneos e não serão considerados neste artigo. A escolha do curativo a ser utilizado deve ser baseada no conhecimento das bases fisiopatológicas da cicatrização e da reparação tecidual, sem nunca esquecer o quadro sistêmico do paciente.

ABSTRACT

Pyoderm gangrenosum is an unknown skin inflammatory and ulcerative non infected disease, painful that starts with vesicles formation like cutaneous rash that develop to skin necrosis. It may represent the first immunologic skin reaction during the surgery, similar to Crohn disease, Behçet disease, autoimmune diseases, leukemia, arthritis or even any type of small skin trauma days before the surgery. The immunologic reactions are still unknown.

RESUMO

Pioderma gangrenoso é uma doença rara de causa desconhecida, inflamatória, ulcerativa e dolorosa, com formação de bolhas e pústulas, tipo autoimune e não infecciosa. É caracterizado inicialmente por uma lesão cutânea avermelhada de caráter inflamatório do tipo rash cutâneo, que evolui para necrose de pele. Pode representar uma primeira resposta imunológica na pele durante o ato operatório, semelhante à doença de Crohn, doença de Behçet, doenças autoimunes, leucemias, artrites, ou mesmo ainda qualquer outro pequeno trauma ocorrido na pele em data anterior à cirurgia. Não se sabe a origem do processo que deu a esta reação imunológica.

Plexiform neurofibroma of the upper limb

ABSTRACT

The authors present an unusual case of plexiform neurofibroma affecting the upper limb in a patient diagnosed with type 1 neurofibromatosis. Tumor resection was performed on the median nerve. The patient showed maintenance of limb function and remission of symptoms of pain after four years of follow-up.

RESUMO

Os autores apresentam um caso incomum de neurofibroma plexiforme acometendo o membro superior, com diagnóstico de neurofibromatose do tipo 1. Realizou-se a ressecção do tumor no nervo mediano. A paciente evoluiu com manutenção da função do membro e remissão dos sintomas de dor após seguimento de quatro anos.

Partial autograft after allograft to cover extensive traumatic lesion: case report

ABSTRACT

This study intends to report a case of combined use of allograft followed by autograft to cover extensive lesion in a patient with polytrauma evaluated at the Hospital Universitário Evangélico Mackenzie. The aim is to show that, as it has been done for many years in medium and large burned patients, the use of alloenxertia can be useful in evaluating and preparing the bed to receive the patient's own skin graft. This paper describes the treatment and evolution of an extensive and complex lesion of a polytraumatized patient, and what was the role of the allograft in the final outcome of the case.

Esse trabalho tem como intenção relatar um caso de uso combinado de aloenxerto seguido de autoenxerto para cobertura de lesão extensa, em um paciente vítima de politrauma atendido no Hospital Universitário Evangélico Mackenzie. O objetivo é mostrar que, da mesma forma como já vem sendo realizado há muitos anos nos pacientes com médio e grande queimados, o uso de aloenxertia pode ser útil, ao avaliar e preparar o leito para receber o enxerto de pele do próprio paciente. Esse trabalho descreve como foi o tratamento e evolução de uma lesão extensa e complexa, de um paciente politraumatizado, e qual foi o papel do aloenxerto no resultado final do caso.
