Forehead reduction in patients with precapillary incision: 10 years of experience

ABSTRACT

INTRODUCTION: The long forehead gives a less harmonious appearance, seems disproportionate and characterizes the aging face. Surgical procedures for frontal reduction with precapillary incision present insightful information and provide harmonious and pleasant restoration of the frontal region. This study evaluated results obtained in patients who underwent frontal reduction using precapillary incision at Daher Hospital plastic surgery service within the last ten years. METHODS: This was a retrospective longitudinal study. The surgical technique entailed a careful dissection of the bilateral supraorbital ridge. The scalp was dissected in the parietoccipital region in order to easily move forward the flap. The vigorous hemostasis and resection of excessive flap were performed. In the end, synthesis of plans were performed. RESULTS: The age of patients ranged from 48 to 76 years, the mean age was 58 years. Mean surgical time was 3 hours and 38 minutes. No complications were seen such as hematoma, flap necrosis, supratrochlear nerve injury, alopecia, infections, deep vein thrombosis, pulmonary embolism and/or dehiscence. Four patients (7%) had seroma that was drained. The symmetric of eyebrows and scars positioning was considered satisfactory in both assessment by patients and the technical evaluator. CONCLUSION: The aesthetic results in patients who underwent frontal reduction by precapillary incision were satisfactory. The choice of the ideal patient was crucial for success of the surgery.

RESUMO

INTRODUÇÃO: A testa longa confere uma aparência menos harmônica, desproporcional e caracteriza o envelhecimento. Procedimentos cirúrgicos para redução frontal com incisão pré-capilar apresentam indicações criteriosas e proporcionam a restauração harmônica da região frontal. O estudo avalia os resultados obtidos em pacientes submetidos à redução frontal por incisão pré-capilar no serviço de cirurgia plástica do Hospital Daher nos últimos dez anos. MÉTODOS: Trata-se de um estudo observacional longitudinal retrospectivo. A técnica cirúrgica consiste em uma dissecção cuidadosa até o rebordo supraorbital bilateral. O couro cabeludo é dissecado até a região parietoccipital de forma a avançar com facilidade o retalho. Procede-se à hemostasia vigorosa e ressecção do retalho excedente. Por fim, realiza-se a síntese por planos. RESULTADOS: A faixa etária das pacientes variou de 48 a 76 anos, com média de 58 anos. O tempo médio operatório foi de 3 horas e 38 minutos. Não foram observadas complicações como hematoma, necrose do retalho, lesão do nervo supratroclear, alopécia, infecções, trombose venosa profunda, embolia pulmonar e/ou deiscências. Quatro pacientes (7%) apresentaram seroma, os quais foram todos drenados. A simetrização das sobrancelhas e a posição das cicatrizes foram consideradas satisfatórias tanto pela avaliação feita pelos pacientes quanto pela avaliação técnica. CONCLUSÃO: Os resultados estéticos obtidos em pacientes submetidos à redução frontal pela incisão pré-capilar foram satisfatórios. A escolha do paciente ideal foi fundamental para o bom sucesso operatório.

Endoscopic forehead and midface surgery: eight years experience

ABSTRACT

In the early 90's, plastic surgery videoendoscopic technique was introduced and accepted as a effective method of forehead rejuvenation, mainly for brow and frontal area elevation. This procedure presents multiple indications and is performed to correct brow ptosis and frown lines caused by the depressor muscles. From January 1996 to December 2004, 207 patients were submitted to videoendoscopic forehead surgery in association or not with midface surgery with the some technique. Results were satisfactory with maintenance of brow elevation, decrease of glabelar wrinkles, superiorlateral rotation of oculo-palpebral elements and elevation of midface.

RESUMO

Desde o início dos anos 90, a técnica videoendoscópica em cirurgia plástica foi introduzida e aceita como método eficaz para o rejuvenescimento do terço superior da face, principalmente para elevação dos supercílios e região frontal. Este procedimento apresenta múltiplas indicações, sendo realizado para corrigir a ptose dos supercílios e rugas glabelares causadas pelos músculos depressores. No período de janeiro de 1996 a dezembro de 2004, 207 pacientes foram submetidas à cirurgia endoscópico da região frontal, associando ou não o tratamento do terço médio da face pela mesma técnica. Os resultados obtidos foram satisfatórios com manutenção da elevação dos supercílios, redução das rugas de expressão de glabela, rotação dos elementos óculo-palpebrais no sentido látero-superior e elevação do terço médio da face.

Results in surgical rehabilitation for facial palsy

ABSTRACT

Objectives: To describe results in surgical rehabilitation in a series of cases with facial nerve injury, evaluated over a five year period at Hospital de Reabilitação SARAH. Methods: Retrospective study of a series of cases admitted during the years 2001 at 2006 period, with assessment of the surgical rehabilitation using the House Brackmann scale in 29 (10%), 16 (55%) and 13 (45%) female, were submitted to surgical rehabilitation. The following surgical procedures adopted are direct nerve suturing in 2 (7%), end-to-end neurorraphy, facial-hypoglossal in 4 (14%), temporal muscle transposition in 7 (24%), canthoplasty in 12 (41%) and complementary procedures in 4 (14%) cases. The follow up was three years in average. Results: The improvement in all case was at least one degree in HB score. Conclusion: All patients evolved with a functional improvement with a surgical rehabilitation and can be considered a highly relevant for the paralyzed face rehabilitation.

RESUMO

Objetivo: Descrever os resultados da reabilitação cirúrgica de uma série de casos com lesão de nervo facial, avaliados durante um período de cinco anos no Hospital de Reabilitação SARAH-Brasília. Método: Estudo retrospectivo de série de casos admitidos no período de 2001 a 2006, avaliando-se os resultados da reabilitação cirúrgica conforme a escala de House Brackmann (HB), em 29 (10%) pacientes, sendo 16 (55%) homens e 13 (45%) mulheres. Os procedimentos realizados foram microneurorrafia término-terminal em 2 (7%) pacientes, hipoglossofacial em 4 (14%), transposição do músculo temporal em 7 (24%), cantoplastia em 12 (41%) e procedimentos complementares em 4 (14%). O tempo de seguimento pós-operatório foi, em média, de 3 anos. Resultados: Todos os casos evoluíram com a recuperação em no mínimo um grau na escala de HB. Conclusão: Todos os pacientes apresentaram melhora funcional com a reabilitação cirúrgica e esta pode ser considerada um importante componente na reabilitação da face paralisada.

Surgical treatment of cervical region associated with polytetrafluoroethylene platysmal suspension sutures

ABSTRACT

Introduction: Since the 1970s, great importance has been atributed to superficial musculoaponeurotic system (SMAS) and platysma dissection, considering the proper approach of these structures the key point in order to achieve better results in facial rejuvenation. Difficult necks, as referred as grade IV by McKinney, are those with significant flaccidity of the platysma muscle and skin, pronounced and separated medial bands, large protrusion of the submandibulary glands, low hyoid position, great amount of fat and poorly defined jawline. Method: A group of 89 patients with these characteristics and submitted to surgical treatment was retrospectively studied. The surgical treatment included face-neck lifting, with SMAS-platysma dissection and traction, Feldman's corset platysmaplasty and cross-crossed polytetrafluoroethylene (PTFE) suture on platysmal edges and anchored to the mastoid. It is recommended that the suspension suture of the SMAS-platysma complex is done only after the conclusion of the neck region once the platysma movement towards the midline will cause inferior displacement and tissue accumulation at the mandibular edge. Results: The results achieved show low complication rates (3.2%) and high post-operative satisfaction. Conclusion: The results also point to the efficacy of this surgical routine and the safety of the PTFE suture that secondarily contributes to favorable results in such difficult condition.

RESUMO

Introdução: A partir da década de 70, grande importância tem sido atribuída ao sistema músculo-aponeurótico superficial (SMAS) e ao platisma, sendo o manejo adequado destas estruturas considerado ponto-chave na obtenção de melhores resultados nas cirurgias de rejuvenescimento facial. Consideram- se casos de difícil resolução, atribuídos por McKinney como grau IV, pacientes com significativa flacidez de pele e músculos, bandas platismais pronunciadas e separadas, protrusão das glândulas submandibulares, baixa posição do osso hióide, grande quantidade de gordura e uma linha mandibular pouco definida. Método: Um grupo de 89 pacientes com estas características e submetido a tratamento cirúrgico foi avaliado, retrospectivamente. O tratamento cirúrgico constou de ritidoplastias cérvico-faciais associadas a dissecção e tração do complexo SMAS-platisma, platismoplastia medial segundo Feldman e sutura entrecruzada com fio de politetrafluoroetileno (PTFE), realizada nos bordos platismais e fixa à mastóide. Preconiza- se que a sutura de suspensão do complexo SMAS-platisma seja efetuada somente após a conclusão da região cervical, caso contrário, o avanço do platisma à linha média promoveria tração inferior e novo acúmulo tecidual na margem mandibular. Resultados: Os resultados apurados demonstram baixos índices de complicações relevantes (3,2%), associados a maior satisfação pós-operatória. Conclusão: Conclui-se pela eficácia da rotina cirúrgica adotada e pela segurança da sutura de PTFE, possibilitando resultados favoráveis nesta difícil condição.

Surgical anatomy of the spinal accessory nerve: how to avoid injuries to the posterior cervical triangle during surgical procedures

ABSTRACT

Background: The resulting syndrome from iatrogenic injury to the spinal accessory nerve (SAN) is characterized by pain, paralysis and winging of the scapula are often the source of considerable morbidity. This study analyses the degree of ramification and variations of the SAN at the posterior triangle (PT). Method: The necks of 8 adult cadavers were dissected bilaterally to expose the SAN. The ramifications and variations of the SAN at the PT were recorded. Measurements related to anatomical landmarks of the Sternocleidomastoid Muscle (SM) and the Trapezius Muscle (TM) were analyzed. Results: The sixteen dissections performed showed the ramification pattern of the SAN as: no ramus in 25%, one ramus in 37.5% and 2 ramus in 37.5% of cases. Considerable variation was seen in regional anatomy of the SAN at the PT. Measurements revealed that the SAN has a relatively constant relationship with the great auricular nerve (GAN) and this is a important landmark for the identification of the SAN at the PT. Conclusions: The GAN is a useful landmark for identification of the SAN at the PT. This approach suggests a possibility to decrease the incidence of iatrogenic injury of the SAN, that is frequently related to dissection of surrounding anatomical structures and reduced direct exposure of the nerve.

RESUMO

Introdução: A síndrome resultante da lesão iatrogênica do nervo acessório espinhal (NAE) se caracteriza por dor, paralisia e escápula alada, estando relacionada a morbidade considerável. O presente estudo investiga o grau de variação na ramificação e no curso do NAE no trígono cervical posterior (TCP). Método: A região cervical de oito cadáveres foi dissecada bilateralmente para expor o NAE. A ramificação e as variações no curso do NAE no TCP foram registradas. Medidas relacionadas a estruturas anatômicas do Músculo Esternocleidomastóideo (MEC) e do Músculo Trapézio (MTZ) foram analisadas. Resultados: As 16 dissecções demonstraram padrão de ramificação do NAE como: nenhum ramo em 25%, um ramo em 37,5% e dois ramos em 37,5% dos casos. Considerável variação foi observada na anatomia regional do NAE no TCP. Medidas revelaram que o NAE tem relação relativamente constante com o nervo auricular magno (NAM) e esta é uma importante referência anatômica para identificação do NAE no TCP. Conclusões: O NAM é uma referência anatômica útil para a identificação do NAE no TCP. Essa abordagem sugere uma possibilidade para diminuição de lesão iatrogênica do NAE, frequentemente relacionada à manipulação cervical de estruturas anatômicas adjacentes sem exposição direta do nervo.

Myectomy of the Orbicularis Oculi Muscle: A New Procedure Associated to Bleroplasty

ABSTRACT

The authors deseribe myectomy at the lateral border of the orbieularis oculi muscle during blepharoplasty to attenuate periorbital wrinkles ("crow's feet"). There were no surgieal, aesthetic or functional complications with this procedure. All patients that underwent surgery presented wrinkle attenuation. Most evident results could be seen in those patients with a lesser degree of cutaneous photoaging. The authors conclude that this is a safe, fast, and easy procedure, which presents good aesthetic and functional results.

RESUMO

Os autores descrevem a realização de uma miectomia na margem lateral do músculo orbicular dos olhos durante a blefaroplastia visando a atenuação das rugas periorbitais ("pés-de-galinha"). Não houve complicações cirúrgicas, estéticas ou funcionais com este procedimento. Todas as pacientes operadas apresentaram atenuação das rugas, sendo o resultado mais evidente naquelas com menor grau de fotoenvelhecimento cutâneo. Os autores concluem que este é um procedimento seguro, rápido e de fácil execução, apresentando bons resultados estéticos e funcionais.

Applied anatomy of the branch of the facial nerve: study of the potential risk of injury during facial rejuvenation surgery

ABSTRACT

Background: The temporal branch of the facial nerve is one of the nerves more commonly injured due to the scarce subcutaneous tissue that protects it from its exit at the parotid gland. Method: Twenty five cadaveric hemifaces were dissected allowing a analysis of the temporal branch and: parotid gland, zygomatic arch, SMAS, superficial temporal artery and frontal muscle. Results: Twelve temporal branches exposed at a distance 1.7cm anterior to the trago. Two temporal branches crossing the zygomatic arc was the most frequent finding.This passage occurred between 3.2 to 3.9 cm posterior to the lateral border of the orbit. The trajectory of the temporal branch near the deep side of the SMAS and temporofacial fascia and above the superficial layer of the deep temporal fascia was constant. The frontal branch of the superficial temporal artery was superior and parallel to the nerve trajectory in 92% of the dissections Conclusion: The temporal branch follow a constant plane along the undersurface of the temporoparietal fascia and is quite superficial as it cross the zygomatic arch.

RESUMO

Introdução: O ramo temporal do nervo facial é um dos nervos mais comumente lesados, devido à pouca tela subcutânea que o protege a partir da sua saída da glândula parótida. Método: Vinte e cinco hemifaces de cadáveres foram dissecadas e analisadas as relações entre o ramo temporal e glândula parótida, arco zigomático, SMAS, artéria temporal superficial e músculo frontal. Resultados: Doze ramos temporais dissecados perderam a proteção da glândula parótida a uma distância de 1,7 cm anterior ao trago. O cruzamento do arco zigomático por dois ramos temporais foi o mais frequente. A passagem pelo arco zigomático ocorreu entre 3,2 e 3,9 cm posteriores à borda lateral da órbita. O curso do ramo temporal junto às faces profundas do SMAS e da fáscia temporoparietal, e acima da lâmina superficial da fáscia temporal profunda foi constante. O ramo frontal da artéria temporal superficial foi superior e sua trajetória paralela ao ramo temporal em 92% das dissecções. Conclusão: O ramo temporal do nervo facial segue um plano constante ao longo da face profunda da fáscia temporoparietal e está muito superficial quando cruza o arco zigomático.

Facial paralysis: epidemiological analysis in a rehabilitation hospital

ABSTRACT

BACKGROUND: Facial paralysis is characterized by permanent or temporary loss of facial expression due to facial nerve injury. Several factors influence the development of facial nerve lesions. The purpose of this study was to evaluate the epidemiological aspects and incidence of sequelae after facial paralysis at a rehabilitation institution. METHODS: We performed a retrospective study of facial paralysis patients admitted to a rehabilitation hospital between January 2001 and January 2005. Sequelae were analyzed according to gender, age, etiology, functional status as measured by the House-Brackmann scale, evaluation time, follow-up, and surgical procedures. Statistical analyses were performed with Epi-info 3.2.2 software. RESULTS: A total of 285 facial paralysis patients, 157 male and 128 female, were admitted for a rehabilitation program. All subjects followed a rehabilitation program, and 29 (10.2%) underwent surgery; 80% were admitted during the 3rd week of the paralysis or later, and 121 (42.5%) showed gradual recovery after 3 months, either spontaneously or after clinical or physical therapies. Nevertheless, 119 (41.8%) sustained irreversible partial or complete facial paralysis. CONCLUSIONS: The prevalence of facial paralysis was greater among patients younger than 20 years. Among these patients, paralysis had different causes, and these patients were admitted with lower House-Brackmann grades. Most cases were associated with neurological deficits leading to facial paralysis of central or congenital origin.

INTRODUÇÃO: A paralisia facial é a perda temporária ou permanente dos movimentos da mímica facial em decorrência do acometimento do nervo facial. São vários os fatores que influenciam a evolução das lesões do nervo facial. Este estudo teve como objetivo avaliar os aspectos epidemiológicos e a frequência de sequelas após paralisia facial em um serviço de reabilitação. MÉTODO: Estudo retrospectivo dos pacientes com paralisia facial atendidos em hospital de reabilitação no período de janeiro de 2001 a janeiro de 2005. As sequelas foram avaliadas quanto a sexo, idade, etiologia, graduação funcional conforme a escala de House-Brackmann, tempo de evolução, seguimento e intervenções cirúrgicas. Para realização da análise estatística utilizou-se o programa Epi-Info versão 3.2.2. RESULTADOS: Foram admitidos para programa de reabilitação 285 pacientes portadores de paralisia facial, sendo 157 do sexo masculino e 128 do feminino. Todos os pacientes se submeteram a programa de reabilitação e 29 (10,2%), a cirurgia. Dentre os pacientes analisados, 80% foram admitidos a partir da terceira semana do surgimento da paralisia, e 121 (42,5%) tiveram recuperação gradual em 3 meses, espontaneamente, com tratamento clínico ou fisioterápico. Por outro lado, 119 (41,8%) pacientes permaneceram com paralisia facial parcial ou completa e irreversível. CONCLUSÕES: Os casos admitidos foram mais frequentes em pacientes com menos de 20 anos de idade, com causas diversas e quando admitidos em graus menores segundo a escala de House-Brackmann, pois muitos deles se associavam a déficits neurológicos consequentes a paralisia facial de origem central ou congênita.

Temporary hemostatic traction (THT) in rhytidoplasty

ABSTRACT

INTRODUCTION: To slow the aging process, facial contouring surgery was developed as a rejuvenating technique, and is constantly improving, as are the results. However, as with any surgical procedure, complications are possible, the main one being hematoma. Dissatisfied with the prospect of hematoma and the use of drains, and with a goal of lower costs, the authors felt the need to develop a new surgical procedure, temporary hemostatic traction (THT). METHODS: Sixty-four rhytidoplasties, all performed at the Valle Pereira Clinic (Florianópolis, SC, Brazil) from May 2012 to January 2013, were included in this study. All patients underwent the new THT procedure. RESULTS: Only two patients developed epidermolysis with optimal evolution following THT, and one developed generalized facial edema. There were no instances of hematoma at any of the postoperative stages. CONCLUSIONS: This new surgical technique avoids the use of drains and decreases dead space, thereby improving flap adhesion, and decreases hematoma, seroma, and postoperative edema; it also decreases tension on the incision, hence improving the results of rhytidoplasty.

RESUMO

INTRODUÇÃO: Na tentativa de desacelerar o processo de envelhecimento, as cirurgias de contorno facial foram estabelecendo-se como técnicas rejuvenescedoras, sendo constantemente aprimoradas, assim como os resultados. Porém, como qualquer procedimento cirúrgico, há a possibilidade de intercorrências e a principal complicação delas é o hematoma. Descontentes com a presença desta complicação, com o uso de drenos e visando menor custo, os autores viram a necessidade de desenvolver uma nova tática cirúrgica chamada tração hemostática temporária - THT. MÉTODOS: Foram incluídas neste estudo todas as ritidoplastias realizadas na Clínica Valle Pereira (Florianópolis, SC), no período compreendido entre maio de 2012 e janeiro de 2013, totalizando 64 pacientes. Todos foram submetidos à nova tática cirúrgica chamada tração hemostatica temporária. RESULTADOS: Apenas 2 pacientes apresentaram epidermólise com ótima evolução sob a THT, e 1 caso de edema facial generalizado. Não houve caso com hematoma, em qualquer período pós-operatório. CONCLUSÕES: esta nova tática cirúrgica evita o uso de drenos, diminui o espaço morto aumentando a adesão do retalho descolado e, consequentemente, diminui a presença de hematoma, seroma e edema pós-operatório, assim como diminui a tensão cicatricial na incisão da pele, visando aprimorar os resultados das ritidoplastias.

Modified minilifting results: prospective study

ABSTRACT

Introduction: The modified mini-facelift is based on a standardized tecnic consisting of a minimum undermining marked preoperatively. The plication of the muscle-aponeurotic superficial (SMAS) is done pulling all the tissues of the face, promoting facial rejuvenation with the maintenance of a flap with a lower risk of necrosis by maintaining smaller undermining area and minimum area for hematomas. Methods: We operated on 36 patients undergoing rhytidectomy technique of modified mini-lifting as they fulfilled the some criteria. Anthropometric data were collected, a questionnaire about the patient's history was performed. Results: From February 2008 to March 2009, 36 patients were consecutively operated by the modified mini-facelift technique. The age ranged from 44 to 67 years, with a mean 57 anos. The patients return to normal activities ranged from 7 to 14 days, a period that coincides with a reduction in more than 80% of postoperative edema. The vacuum drain was removed the next morning (1st day after surgery) in 34 patients (94.4%), the 2nd postoperative day in 1 (2.94%) case and in 1 (2.94%) case of postoperative hematoma. There were no cases of necrosis of the flap. There was 1 (2.94%) neuropraxia facial which was resolved spontaneously. Conclusion: The technique of modified mini-lifting is satisfactory for presenting good results, low complication rate and early return of patients to their activities.

Introdução: O mini-lifting modificado se baseia em uma técnica padronizada, que consiste em um descolamento mínimo demarcado no pré-operatório. A plicatura do sistema músculo-aponeurótico superficial (SMAS) é realizada tracionando-se todos os tecidos da face, a fim de promover o seu rejuvenescimento com a manutenção de um retalho que apresenta menor risco de necrose por ter menor descolamento e, por isso, uma mínima área para formação de hematomas. Método: Foram operadas 36 pacientes submetidas a ritidoplastia pela técnica do mini-lifting modificado, que preencheram critérios pré-estabelecidos. Foram coletados os dados antropométricos e um interrogatório sobre a história clínica do paciente realizado. Resultados: No período de fevereiro de 2008 a março de 2009, foram operadas consecutivamente 36 pacientes pela técnica de mini-lifting modificado. A idade variou entre 44 e 67 anos, sendo a média de 57 anos. O retorno das pacientes às atividades habituais variou de 7 a 14 dias, período que coincide com a redução em mais de 80% do edema pós-operatório. O dreno a vácuo foi retirado na manhã seguinte em 34 (94,4%) pacientes, no 2º dia pós-operatório em 1 (2,94%) caso e houve 1 (2,94%) caso de hematoma pós-operatório. Não houve caso de necrose do retalho. Houve 1 (2,94%) caso de neuropraxia do facial, que foi resolvida espontaneamente. Conclusão: A técnica de mini-lifting modificado foi considerada satisfatória, por apresentar bons resultados, baixo índice de complicações e retorno precoce das pacientes às suas atividades.

ABSTRACT

Facial palsy is the most common cranial neuropathy. It is expressed clinically as a dysfunction of the facial expression. For many years, the surgical reanimation of facial movements and above all of the facial expression has been a challenge for plastic and microsurgeons. In this article, the authors present a review of the literature on surgical rehabilitation of facial paralysis. Many procedures have been used and still today the results are limited. Despite the need of a multidisciplinary team, the plastic surgeon has prominent role in facial surgical rehabilitation.

RESUMO

A paralisia do nervo facial é a neuropatia craniana mais comum. Clinicamente se observa a disfunção da expressão facial. Por muitos anos, a restauração cirúrgica dos movimentos faciais, principalmente da expressão facial, tem sido um grande desafio para os cirurgiões plásticos e microcirurgiões. Neste artigo, os autores apresentam uma revisão da literatura sobre a reabilitação cirúrgica da paralisia facial. Apesar da necessidade de uma equipe multidisciplinar, o cirurgião plástico tem papel fundamental na reabilitação cirúrgica facial. Vários procedimentos já foram empregados, e ainda hoje os resultados obtidos na recuperação da expressão facial têm se mostrado limitados.

Forehead reduction using precapillary incision: experience report and indications

ABSTRACT

BACKGROUND: A long forehead can give a less attractive and disproportionate appearance. It may also indicate aging. The objective of this study is to show the experience of the authors in forehead reduction using precapillary incision and to support the indications for this procedure. METHODS: Patients who underwent precapillary incision for forehead reduction between 2005 and 2011 were retrospectively analyzed. The indications for an anterior hairline incision were eyebrow ptosis in patients with thin and sparse frontal hair, congenital long forehead, or extensive forehead wrinkles. RESULTS: Precapillary incision was performed in 31 patients, who were followed up for an average of 1.5 years. No problems related to flap vascularization were detected. All patients reported temporary paresthesia, from which they recovered within 1 year. Three patients presented seromas during the postoperative period and were treated using puncturing. All patients reported that the benefits of altering the hairline were greater than the disadvantages of a possibly more visible scar. CONCLUSIONS: Frontal rhytidectomy using precapillary incision is recommended in patients who wish to correct thin and sparse frontal hair, extensive forehead wrinkles, or congenital/ senile long forehead.

RESUMO

INTRODUÇÃO: A testa longa pode dar uma aparência menos atraente, desproporcional e caracterizar o envelhecimento. O objetivo deste trabalho é demonstrar a experiência dos autores na redução da região frontal com incisão pré-capilar e reforçar as indicações desse procedimento. MÉTODO: Foram estudados, retrospectivamente, pacientes submetidos a redução da região frontal por incisão pré-capilar, no período de 2005 a 2011. As indicações para incisão anterior na linha do cabelo foram ptose da sobrancelha, em pacientes que possuíam cabelos frontais finos e escassos, testa longa congênita ou enrugamento amplo de testa. RESULTADOS: A incisão pré-capilar foi realizada em 31 pacientes, com acompanhamento médio de 1,5 ano. Não houve problemas relacionados à vascularização do retalho. Todos os pacientes relataram parestesia temporária, com recuperação em até 1 ano. Três pacientes apresentaram seromas no pós-operatório, tratados com punção. Todos os pacientes relataram que os benefícios da redução da linha do cabelo ultrapassaram as desvantagens de uma cicatriz possivelmente mais visível. CONCLUSÕES: O procedimento de ritidectomia frontal com incisão pré-capilar é indicado para pacientes com cabelos frontais finos e escassos, com enrugamento amplo da testa ou com testa longa congênita/senil e que desejem reduzi-la.

Segmented approach to the midface. Triangle on cheek area: dermal fat suture point

ABSTRACT

Introduction: The surgical treatment presented by the authors has tactical innovations and unique features that make the treatment safer. Objective: To show a different approach to cheek area and fix the skin with dermal fat points. Methods: Approach the cheek area with progressive retractors, leaving bands. Skin fixation with dermal fat points using nylon 4.0 favors tension reduction in the scar. Results: Thirty-nine pacients were operated using this tactic. The period of hospitalization was 24 hours and there was only one case of hematoma. Conclusion: It is an efficient and simple method where the approach to the cheek area using retractors show low bleeding. The adhesion left by this method encourage better skin irrigation. Dermal fat points keep the skin in place and reduce the tension in the scar.

RESUMO

Introdução: O tratamento cirúrgico apresentado pelos autores tem inovações táticas e peculiares que tornam o tratamento seguro. Objetivo: Demonstrar uma maneira diferente de abordar a região geniana e de fixar a pele com pontos dermogordurosos. Método: Abordagem da região geniana com os descoladores progressivos, deixando bridas. A fixação da pele com pontos dermogordurosos usando fio de nylon 4.0 favorece a diminuição da tensão na cicatriz pré-auricular. Resultados: Foram operados trinta e nove pacientes usando essa tática. O período de internação foi de 24 horas, sendo observado apenas um caso de hematoma. Conclusão: O método proposto é eficiente e simples, onde a abordagem da região geniana com o uso dos descoladores é de pouco sangramento. As bridas deixadas por esse método favorecem melhor irrigação da pele. Os pontos dermogordurosos mantêm a pele no local e diminuem a tensão na cicatriz pré-auricular.

The peninsula in rhytidoplasty

ABSTRACT

INTRODUCTION: High implantation of the hairline by the ear is a frequent, unpleasant stigma in rhytidoplasties. To overcome this problem, we created the Peninsula technique in the mid-1999. Initially used in secondary face liftings, its indication has since been broadened, becoming indispensable in primary cases of high hair implantation and in patients requiring major skin resection. METHODS: The total number of rhytidoplasties performed was 7,356, with 2,512 patients operated using the Peninsula technique from 1999 to 2016. Mean age was 48, 98% were females and 2% males. The technique presents three variants. RESULTS: Patients were monitored on a monthly basis until discharge at the end of six months. Among the results, there were some complications, which are usual in any type of facial surgery. In the great majority of cases, with the use of the Peninsula, it was possible to obtain good skin traction, with the greater correction of the mid third, maintaining the capillary implantation line. CONCLUSION: We can conclude that the Peninsula technique is a unique resource, easily reproducible, capable of reducing the learning curve in rhytidoplasty, as it allows generous skin resection without changing capillary implantation, avoiding post-surgical stigma and optimizing the quality of the results and patient satisfaction, with a little scar burden.

RESUMO

INTRODUÇÃO: A ascensão do pé do cabelo é um estigma frequente e desagradável nas ritidoplastias. Para contornar esse problema, criamos a técnica da Península em meados de 1999. Inicialmente utilizada nas faces secundárias, sua indicação foi ampliada, tornando-se imprescindível nos casos primários de implantação alta do cabelo e naqueles pacientes que necessitariam de grande ressecção de pele. MÉTODOS: O total de ritidoplastias realizadas foi de 7356, tendo sido operados 2512 pacientes utilizando a técnica da Península, no período que cursa entre 1999 e 2016, com idade média de 48 anos, sendo 98% dos pacientes do sexo feminino e 2% do sexo masculino. A técnica apresenta três variantes. RESULTADOS: Os pacientes continuaram, posteriormente, em acompanhamento mensal até a alta ao final de 6 meses. Dentre os resultados, surgiram algumas complicações, comuns a qualquer tipo de cirurgia da face. Na grande maioria dos casos, com a utilização da Península, foi possível a realização de uma boa tração cutânea, com maior correção do terço médio, mantendo a linha de implantação capilar. CONCLUSÃO: Podemos concluir que a técnica da Península é um recurso único, de fácil reprodução, capaz de reduzir a curva de aprendizado em ritidoplastias, uma vez que permite generosa ressecção de pele sem alteração da implantação capilar, atuando de modo a evitar estigmas pós-cirúrgicos e otimizando a qualidade dos resultados e satisfação por parte dos pacientes, com pequeno ônus cicatricial.

ABSTRACT

The earlobe occupies a unique position among the structures of the face and is a very important region in aesthetic composition of the ear. The age alters the shape, width and length of the lobe due to sagging tissue and thus is at odds ratio with other aesthetic elements of the ear, requiring correction. Very little has been described to guide the surgeon in rejuvenating the ear. The correction aged lobe can be made alone or in combination with surgery of facial rejuvenation, as well as by secondary lobe rhytidoplasties inadequately secured to the face. The surgery is fairly simple, rapid recovery, with interesting results. The basic aim of the work is the description of the technique and its indications. The strategic positioning of the final scar in continuity with a depression anatomical ear that is escafa, and hidden by antitragus ensures a proper outcome, with little apparent scars. The technique described is a tactical option to reshape and rejuvenate the ear.

RESUMO

O lóbulo de orelha ocupa posição única entre as estruturas da face e é uma região importantíssima na composição estética da orelha. A idade altera o formato, a largura e o comprimento do lóbulo, em decorrência da flacidez dos tecidos e, assim, fica em desarmonia de proporção com outros elementos estéticos da orelha, exigindo correção. Muito pouco tem sido descrito para orientar o cirurgião no rejuvenescimento da orelha. A correção do lóbulo envelhecido pode ser feita isoladamente ou em combinação com uma cirurgia de rejuvenescimento facial, bem como em ritidoplastias secundárias por lóbulo inadequadamente preso à face. A cirurgia é bastante simples, de recuperação rápida, com resultados interessantes. O objetivo básico do trabalho consiste na descrição da técnica de rejuvenescimento do lóbulo da orelha e suas indicações. O posicionamento estratégico da cicatriz final em continuidade com uma depressão anatômica da orelha, que é a escafa, e ocultada pelo antitrágus garante resultado final adequado, com cicatrizes pouco aparentes. A técnica descrita representa uma opção tática para reformular e rejuvenescer a orelha.

Facelift and Facial nerve injury: how to deal with?

ABSTRACT

Is largely known that the facial nerve virtually can be injured in every facial lift. Even though its importance, it is difficult to find in the literature articles related to this theme. Complete or incomplete facial paralysis after a facial procedures, may become a very uncomfortable situation between the patient and the surgeon, that is why we propose in this article a guide which intends to help avoid, identify and manage a facial nerve injury in the event of a face lift. Identifying Facial palsy before the surgery: Intending to make the pre-op facial assessment as simple as possible, we suggest a systematic approach examination. Avoiding facial nerve injury during face lifting: Several technical details are discussed along the text aiming to minimize the risk of nerve damage during the surgical procedure. What to in the event of a post-operative facial palsy: The authors propose a logical approach to the facial palsy, suggesting a planning for diagnosis, classification of the lesion and a rational treatment for the injury. Conclusion: This article presents a guide showing some safe technical options to avoid nerve damage during rythidoplasty, how to identify the lesion and treat it if necessary.

Não é novidade para os cirurgiões plásticos, especialmente aos que se dedicam à cirurgia de face, que o nervo facial pode ser lesado virtualmente em qualquer cirurgia de ritidoplastia. Porém, apesar da importância contida nesse tema, não é tarefa simples encontrar na literatura artigos que abordem de forma objetiva a correlação entre lesão do nervo facial e cirurgia facial. Paralisias faciais, sejam elas completas ou não, podem se tornar situações potencialmente conflituosas na relação médicopaciente. Este artigo, portanto, visa propor de forma clara e sucinta, baseada na experiência dos autores, um guia de como evitar, identificar e tratar uma potencial lesão do nervo facial no contexto de uma cirurgia de face, em especial a ritidoplastia.

Nerve alterations in rhytidoplasty: a systematic literature review

ABSTRACT

INTRODUCTION: Rhytidoplasty has become one of the most common aesthetic surgeries performed by plastic surgeons worldwide. Along with the increase in the number of surgeries performed, the number of procedure-related complications has also increased. In particular, nerve injuries are the major concern. By conducting a systematic review, the present study aimed to identify the main nerve structures injured during rhytidoplasty, by either the conventional or endoscopic technique. METHODS: A systematic literature review was performed in the main databases currently used. Articles that met the inclusion criteria were analyzed in their entirety, and their references were checked. Finally, 20 studies were included. RESULTS: In these 20 articles, 3,347 patients were evaluated and 142 nerve injuries found, of which 79 were of the facial nerve, 55 were of the trigeminal nerve, and eight were of the great auricular nerve. Of these, only two were definitive. The lesions were more prevalent (81%) with the video-assisted techniques than with the conventional techniques (19%). CONCLUSION: We found that the injuries of the temporal and buccal branches were more frequent during facelifts; and those of the great auricular nerve, during cervical rhytidoplasty. Although nerve injuries are infrequent in the literature, well-designed studies that aim to better understand these complications are lacking.

INTRODUÇÃO: A ritidoplastia se tornou uma das cirurgias estéticas mais realizadas por cirurgiões plásticos ao redor do mundo. Junto com o aumento do número de cirurgias, a quantidade de complicações associadas ao procedimento também aumentou, sendo que as alterações nervosas são uma das que despertam maiores preocupações. O presente estudo visa a identificar, por meio de uma revisão sistemática, as principais estruturas nervosas lesadas durante uma ritidoplastia, tanto por técnicas convencionais como endoscópicas. MÉTODOS: Uma revisão sistemática da literatura foi realizada nas principais bases de dados utilizadas atualmente. Artigos que preencheram os critérios de inclusão foram analisados na íntegra e suas referências, verificadas. Ao final, 20 estudos foram incluídos. RESULTADOS: Nestes 20 artigos, no total, foram avaliados 3.347 pacientes, sendo encontradas 142 lesões nervosas: 79 do nervo facial; 55 do nervo trigêmeo, e oito do nervo auricular magno. Destas, apenas duas foram definitivas. As lesões, proporcionalmente, foram mais comuns nas técnicas videoassistidas (81%), quando comparadas com as convencionais (19%). CONCLUSÃO: Encontramos que as lesões dos ramos temporal e bucal são mais frequentes no facelift e as do nervo auricular magno, na ritidoplastia cervical. Apesar de as lesões nervosas serem pouco frequentes na literatura, faltam estudos bem desenhados que busquem conhecer melhor estas complicações.

Closed circular suspension in the middle third of the face

ABSTRACT

The surgical treatment presented here involves technical innovations, making it a less-aggressive procedure. The goal is to demonstrate a different way of executing a suspension of the skin in the medial third of the face. The procedure is performed with semi-circular Stocchero needles with the passage of a 2-0 nylon thread through the subcutaneous plane at the anterior edge of the parotid gland and fixation to the temporal fascia. A total of 49 patients were operated on using this method with good aesthetic results. One patient required a preauricular incision to accommodate the skin. This method is safe, simple, does not require hospital inpatient admission, avoids a pre-auricular scar, and provides satisfactory results.

RESUMO

O tratamento cirúrgico apresentado neste artigo tem inovações táticas e torna o procedimento pouco agressivo. O objetivo é demonstrar uma maneira diferente de executar a suspensão do terço médio da face. O procedimento é realizado com agulhas semicirculares tipo Stocchero, com passagem de fio de náilon 2.0 no plano subcutâneo, na altura da borda anterior da glândula parótida, e fixação do fio na fáscia temporal. Foram operados com emprego desse método 49 pacientes, com obtenção de bons resultados estéticos. Em um paciente, houve necessidade de executar uma cicatriz pré-auricular, para acomodação da pele. O método é seguro, simples, não necessita de internação hospitalar, evita uma cicatriz pré-auricular e o resultado oferece boa satisfação aos pacientes.

Bell's palsy subsequent to rhytidectomy

ABSTRACT

Introduction: The peripheral facial palsy is a disease that can leave significant aesthetical and functional sequelae in patients. According to medical literature, Bell's palsy (BP) is the most common cause, representing from 50 to 80% of the cases. However, it must be considered as an exclusion diagnosis. A slightly higher prevalence is estimated in women and its incidence is bimodal with peaks in the third and eighth life decades. Most patients recover within weeks, though a worsening degree may be common in the first 48 hours. Treatment must be performed to prevent complications and the medical procedures are not universally accepted. The following have been mentioned as BP risk factors: arterial hypertension, mellitus diabetes, pregnancy, puerperium and infection caused by type I herpes virus. Although there are several BP associated risk factors, regarding face-lifting surgery there is only one reported case in medical literature. Case Report: In the present study, the authors describe the second case of Bell's palsy after face-lifting surgery.

Introdução: A paralisia facial periférica é uma entidade que pode deixar sequelas estéticas e funcionais importantes nos pacientes. De acordo com a literatura, a paralisia de Bell (PB) é a causa mais comum, representando de 50 a 80% dos casos. Porém deve ser considerada como diagnóstico de exclusão. Estima-se uma prevalência ligeiramente maior entre as mulheres e sua incidência é bimodal, com picos na terceira e oitava décadas de vida. A maioria dos pacientes evolui para a recuperação dentro de algumas semanas, embora seja comum uma piora do quadro nas primeiras 48 horas. O tratamento deve ser realizado no sentido de prevenir complicações e as condutas medicamentosas não estão consagradas. Como fatores de risco para a PB têm sido relatados hipertensão arterial, diabete mellitus, gravidez, puerpério e infecção pelo vírus herpes tipo I. Embora existam vários fatores de risco associados à PB, em relação à cirurgia de face-lifting existe apenas um caso relatado na literatura. Relato do Caso: No presente estudo, os autores descrevem o segundo caso de paralisia de Bell após a cirurgia de face-lifting.

From Surface to Depth: Learning Curve in Extended Deep Facelift - My First 100 Cases.

ABSTRACT

Introduction: Facial aging is a gradual, complex, and multifactorial process. It is the result of changes in the quality, volume, and positioning of tissues. Plastic surgeons have modified their approach to facial rejuvenation surgery, opting for the subaponeurotic plane (SMAS). The objective of this study is to analyze 100 cases of patients operated on using the deep SMAS technique, evaluating its applicability and effectiveness.
Method: 100 patients were evaluated, undergoing facial plastic surgery using the deep SMAS technique - "Deep Smas", and followed up for 6 months. Patient satisfaction, number of complications, number of reoperations, risks, and advantages of the technique were observed.
Results: 100 patients were operated on over 3 years. Age ranged from 41 to 79 years, with 95% being female. The complications were 8 cases (8%) of injuries to branches of the facial nerve, of which 4 cases of zygomatic injury, 3 cases of mandibular injury, and 1 case of buccal injury; there was 1 case (1%) of post-auricular keloid; 1 case (1%) of hematoma. Regarding surgical revisions, there were 8 cases (8%) of surgical completion due to patient dissatisfaction. There were 15% of nerve injuries between the 1st and 40th surgery, 5% between the 41st and 80th, and no injuries between the 81st and 100th patient.
Conclusion: Deep facial lifting or subSMAS has proven to be effective, providing good aesthetic results. It has a low recurrence rate and low morbidity rate; however, it requires a long learning curve.

RESUMO

Introdução: O envelhecimento facial é um processo gradual, complexo e multifatorial. É o resultado de mudanças na qualidade, volume e posicionamento dos tecidos. Cirurgiões plásticos têm modificado sua abordagem na cirurgia do rejuvenescimento facial optando pelo plano subaponeurótico (SMAS). O objetivo deste estudo é analisar 100 casos de pacientes operados pela técnica de SMAS profundo, avaliando sua aplicabilidade e eficácia.
Método: Foram avaliados 100 pacientes, submetidos a cirurgia plástica facial pela técnica de SMAS profundo - "Deep Smas", e acompanhados por 6 meses. Observou-se a satisfação dos pacientes, número de complicações, número de reoperações, riscos e vantagens da técnica.
Resultados: Foram operados 100 pacientes, num período de 3 anos. A idade variou de 41 a 79 anos, sendo 95% sexo feminino. As complicações foram 8 casos (8%) de lesões de ramos do nervo facial, sendo: 4 casos lesão do zigomático, 3 casos de lesão do mandibular e 1 caso de lesão do bucal; houve 1 caso (1%) de queloide retroauricular; 1 caso (1%) de hematoma. Em relação às revisões cirúrgicas, houve 8 casos (8%) de complementação cirúrgica por insatisfação das pacientes. Houve 15% de lesões nervosas entre a 1ª e a 40ª cirurgia, 5% entre a 41ª e a 80ª, e nenhuma lesão entre o 81º e o 100º paciente.
Conclusão: O lifting facial profundo ou subSMAS mostrou ser efetivo, proporcionando bons resultados estéticos. Apresenta baixa taxa de recidiva e baixa taxa de morbidade, porém, necessita de uma longa curva de aprendizagem.
