We would like to express our gratitude to the authors for their interest in the article titled “Dermatofibrosarcoma protuberans of the scalp: a case report”, published in Revista Brasileira de Cirurgia Plástica.1 Attention to the scalp as a site for malignant skin tumors-such as basal-cell carcinoma, squamous-cell carcinoma, melanoma, and less common subtypes, such as dermatofibrosarcoma-is of great impor-tance. There is often a tendency to focus exclusively on photoexposed areas, resulting in neglect of the scalp during dermatological examinations. As highlighted in the reported case, this neglect may lead to a delayed diagnosis, adversely affecting patient prognosis and requiring more complex reconstructive procedures. The occurrence of rare malignant tumors in the scalp reinforces the importance of careful surveillance of this anatomical site, often overlooked during the follow-up of patients with previous malignant lesions to other regions of the body. Dermatofibrosarcoma of the scalp accounts for approximately 5% of all cases of this tumor, and it presents particular challenges due to its potential to infiltrate the periosteum, cranial vault, or dura mater. In addition, malignant tumors of the scalp tend to have a worse prognosis compared with the same tumors in other body regions. Reconstruction of this area is also inherently complex and remains a challenge for plastic surgeons due to the limited distensibility of the scalp.
