Breast augmentation after bariatric surgery by using complementary prosthetic support for glandular containment

ABSTRACT

INTRODUCTION: The skin presents structural and proteomic changes after bariatric surgery and cannot withstand the weight of the breasts, which leads to early residual flaccidity, requiring auxiliary support. The goal is to apply a nonabsorbable polypropylene-poliglecaprone absorbable auxiliary mesh support or Bustos silicone sheet, in the form of an "internal brassiere," to support the skin in order to maintain the shape and flaccidity of the breasts comparable with those of normal breasts. METHODS: Nine patients underwent mammoplasty in inverted T with modeling of tissue implants, reduction of breast volume, and wrapping with an auxiliary support mesh. RESULTS: Flaccidity was not observed at an early stage and the condition was acceptable for up to 4 years of follow-up. One of the patients had a seroma, which was drained with good resolution. Another 24 patients will be operated according to the protocol and observed for 2 years by using mammography, ultrasonography, and magnetic resonance imaging. CONCLUSION: Reduction mammoplasty after bariatric surgery requires additional support for the mammary gland. The structurally altered skin could not support the weight of the breasts, resulting in early flaccidity.

RESUMO

INTRODUÇÃO: A pele de pacientes emagrecidas pós-cirurgia bariátrica apresenta alterações estruturais e proteômicas e não resiste ao peso das mamas, levando-as à flacidez residual precoce, exigindo suporte auxiliar. O objetivo é aplicar suporte auxiliar de lâmina de polipropileno não absorvível mais poliglecaprone absorvível ou lâmina de silicone de bustos, em forma de "soutien interno" como auxiliar de sustentação da pele, para manutenção da forma das mamas, e flacidez no mínimo comparável a outras pacientes sem emagrecimento pela cirurgia bariátrica. MÉTODOS: Nove pacientes foram submetidas à mamoplastia em T invertido com modelagem dos tecidos mamários, com redução do volume e envolvidos com lâmina de sustentação auxiliar. RESULTADOS: Não houve flacidez precoce e em observação de até 4 anos ela era aceitável. Uma delas apresentou seroma, drenado com boa resolução. Serão operadas outras 24 pacientes dentro do protocolo, acompanhadas e observadas por 2 anos com controle mamográfico, ultrassônico e ressonância magnética. CONCLUSÃO: A mamoplastia redutora pós-cirurgia bariátrica requer suporte extra à glândula mamária. A pele estruturalmente alterada não suporta o seu peso e haverá flacidez precoce.

Profession-related postural changes in surgeons

ABSTRACT

BACKGROUND: Any deviation from the standard posture is considered a postural alteration; although this is not necessarily considered a disease, it may substantially affect the quality of life of surgeons. The aim of this study was to evaluate backbone alignment and posture in surgeons and physicians in order to identify and quantify the positioning of body segments and investigate possible postural alterations. METHODS: Thirty subjects participated in the study group (CI) and corresponded to the following inclusion criteria: physician, aged 30-60 years, with at least 2 years of surgical practice and performing 4 weekly surgeries lasting 2 h each. The comparative group (CL) included 32 clinical physicians, in the same age range as the CI group. The control group (CO) included 33 non-physicians in the same age range as the CL and CI groups, who satisfied the following exclusion criteria: chronic muscle-skeletal pain or acute intense pain, diagnosis of or sequelae from orthopedic, rheumatologic, or respiratory disease, or prosthetic use. RESULTS: We observed a high incidence of postural changes in the CI group, including varus thrust, hyperextension, lumbar hyperlordosis, and more foot support in the heel, and other misalignments. The results indicate that the activities of surgeons may be considered as a risk for the musculoskeletal system. Because the specialty of plastic surgery is particularly surgical, the risk is even higher in these surgeons. CONCLUSIONS: Surgical activity provokes progressive postural changes in surgeons and may reduce useful time in the profession and hinder the quality of life.

RESUMO

INTRODUÇÃO: Qualquer desvio da postura padrão é considerado alteração postural, o que não é necessariamente classificado como doença, embora afete consideravelmente a qualidade de vida dos profissionais cirurgiões. O objetivo deste estudo foi avaliar o alinhamento e o controle posturais de médicos cirurgiões e não-cirurgiões, a fim de identificar e quantificar o posicionamento dos segmentos corporais e averiguar comparativamente possíveis alterações posturais. MÉTODO: Participaram do estudo 30 sujeitos no grupo teste (CI), que atenderam aos seguintes critérios de inclusão: ser médico, idade entre 30 anos e 60 anos, com, no mínimo, 2 anos de prática cirúrgica e 4 cirurgias semanais com duração de 2 horas cada uma. O grupo comparativo (CL) constituiu-se de 32 sujeitos, médicos clínicos, com idade similar ao do grupo CI. O grupo controle (CO) constituiu-se de 33 sujeitos não-médicos, com a mesma faixa etária dos grupos CL e CI, que atendiam aos seguintes critérios de exclusão: presença de dor musculoesquelética crônica ou dor aguda intensa; diagnóstico ou sequela de doença ortopédica, reumatológica ou respiratória; ou utilização de prótese. RESULTADOS: Observou-se alta incidência de alterações posturais no grupo CI, como joelho varo, hiperextensão, hiperlordose lombar e pé com maior apoio em calcâneo, além de outros desalinhamentos. Os resultados indicam que as atividades dos cirurgiões podem ser consideradas de risco para o sistema musculoesquelético. Por ser eminentemente cirúrgica, a especialidade de cirurgia plástica é de risco ainda maior. CONCLUSÕES: A atividade cirúrgica provoca alterações posturais progressivas nos cirurgiões, podendo reduzir a vida útil na profissão e prejudicar a qualidade de vida.

Indications and limits of mammoplasty with "L-scar": 30 years' experience

ABSTRACT

INTRODUCTION: Studies recommend reducing the size of the scar on mammoplasties to the smallest possible, using L-scars as one of the techniques. However, its use is limited based on the resections of excess skin because of mammary ptosis. The objective of this study is to determine the limitations of L-mammoplasty. METHODS: To describe the process of breast implantation, mammary lines are used to guide the mammoplasty to achieve anatomically perfect and beautiful breasts. This classifies as "aesthetic pathologies" with quantification of ptosis in centimeters and the limitations being considered in choosing the surgical technique. The proposed classification was used to reduce and model the hypertrophic and ptotic breasts. The suturing maneuvers involved a "semicolon" incision from the lateral side of the submammary groove to the medial and then upward toward point A, terminating in vertical suture, VY, or "areolar circular compensation bag," depending on the excess skin in the periareolar region. This maneuver reduces the extension of the horizontal scar to the maximum of AM plus 2 cm. RESULTS: When mammary ptosis was less than 7 cm, the results were of good quality, without significant residual ptosis and distortion of the areola, achieving a cone shape. CONCLUSION: L-mammoplasty is a good technique, but it should be limited to cases with AM flaccidness below 7 cm and good skin quality.

RESUMO

INTRODUÇÃO: O autor preconiza redução da dimensão das cicatrizes nas mamoplastias às menores possíveis, utilizando cicatrizes em L como uma das táticas. Estabelece limites no seu uso, baseando as ressecções dos excedentes de pele na medida da ptose mamária. O objetivo é determinar os limites da mamoplastia em L. MÉTODOS: Descreve a implantação das mamas no tórax, as linhas mamárias guias para qualquer mamoplastia, as mamas anatomicamente perfeitas e bonitas. Classifica o que denomina de "patologias estéticas" das mesmas, a maneira de quantificar a ptose em centímetros e escolhe a tática cirúrgica do L baseado nela, e suas limitações. Como reduzir e modelar as mamas hipertróficas e ptóticas conforme a classificação proposta. As manobras de suturar as incisões em "ponto e vírgula" partindo da lateral do sulco submamário para medial, depois para cima em direção ao ponto A, terminando em sutura vertical, V-Y ou "bolsa areolar de compensação circular", conforme o excedente de pele na região periareolar. Manobra que reduz a extensão da cicatriz horizontal, ao máximo de AM mais 2 cm. RESULTADOS: Quando a medida da ptose mamária foi menor que 7 cm, os resultados foram de boa qualidade, sem ptose residual importante, sem distorções da aréola e do cone obtido. CONCLUSÃO: A mamoplastia com cicatriz em L é boa tática, mas deve limitar-se a casos com flacidez de medida AM abaixo de 7 cm e pele de boa qualidade.

Silicone-polyurethane breast prosthesis implantation

ABSTRACT

The authors describe the biomechanics of capsular contracture of silicone breast implants. This paper presents the authors' experience in 759 patients who received the silicone-polyurethane breast prosthesis, during ten years. In addiction, the techniques on application the polyurethane prosthesis, the personal method of medications used in the operatory act and in the post-operative period, as well as, the possible complications, are presented.

RESUMO

Os autores descrevem a biomecânica da contratura capsular de implantes de próteses de silicone. Este artigo apresenta a experiência e os resultados obtidos em 10 anos, com 759 pacientes que receberam próteses de silicone com revestimento de poliuretano. Também são apresentadas as táticas na aplicação dessas próteses, o método pessoal de utilização, de medicações durante o ato operatório e no pós-operatório, bem como possíveis complicações.

Late breast reconstruction with prior expansion of the mastectomy area and filling with a de-epidermized TRAM flap

ABSTRACT

Post-mastectomy late breast reconstruction was proposed by means of skin expansion in the first operative stage, substitution of the expander by a de-epidermized TRAM flap mounted in the shape of a cone with an adequate size and proportions in the second stage. In the third stage, the mammary groove and areola-papilla were reconstructed and symmetrization was achieved. Twenty-seven patients, with ages of between 27 and 66 years old and a mean age of 46.9 years, were operated on. In three cases, reconstruction was bilateral. In the long-term evolution, the symmetry and satisfaction of the patients were also assessed. There was no difference in the color of the skin by the transposition of the flap. The sensitivity of the thoracic skin that covered the breast was similar to the preexisting sensitivity after a period of two years. Patients assessed the results using a score of 0 to 10 giving a mean score of 9.28. In conclusion, the prior expansion of thoracic skin in mastectomized areas is a good option in selected patients, improving the sensitivity of the breast and solving the problem of skin color.

RESUMO

A reconstrução mamária tardia pós-mastectomia foi proposta por meio de expansão da pele em primeiro tempo operatório, substituição do expansor por retalho TRAM desepidermizado e montado em forma de cone com medidas e proporções adequadas em segundo tempo. No terceiro tempo, foram reconstruídos o sulco mamário, a aréola-papila e realizada a simetrização. Vinte e cinco pacientes, entre 27 e 66 anos, com idade média de 46,9 anos, foram operadas. Em 3 casos, a reconstrução foi bilateral. Evolução tardia, simetria e satisfação das pacientes também foram avaliadas. Não houve bicromia de pele pela transposição à distância do retalho. A sensibilidade da pele torácica que cobriu a mama foi semelhante à preexistente após dois anos. As pacientes avaliaram seus resultados com notas de 0 a 10. A média foi de 9,28. Conclui-se que a expansão prévia da pele torácica nas áreas mastectomizadas é boa opção em pacientes selecionadas, melhorando a sensibilidade mamária e fazendo desaparecer a bicromia de pele.

Rhytidectomy sector time - incision intracapilar marginal and drift of the orbicularis blepharoplasty

ABSTRACT

SUMMARY: Introduction The authors refer to developments in the treatment of facial aging. OBJECTIVE: Through an incision marginal temporal intracapillary reduce sagging eyelids and raise the tail of the eyebrow, making economic blepharoplasty. METHOD: We propose performing rhytidectomy segment temporal incision intra capillary marginal curve, facilitating compensation skin, within 0.5 cm of the scalp, it desepidermizando 2 mm. Removing fat conclude the edge of the flap and performing suture tricofítica and camouflaging the best scar, which becomes pre capillary by growth of hair along it. Detachment in the subcutaneous plane to the edge of the lateral half of the orbicularis, traction suture and lateral-superior range with separate points of it. Other procedures for the remainder of the face and neck were performed when necessary. The lateral traction reduces sagging eyelid skin and its excesses, making blepharoplasty small economical resections with skin, and removal of bags for minimal dilatation individualized orbicularis muscle at the site of projection of the same. RESULTS: We obtained good results by reducing the stigma of aging orbital region. It is a simple procedure of low morbidity, leaving scar marginal temporal good quality for small skin traction and easily camuflável by the hair growing along her downward covering it. 485 cases were operated from 1997 to 2012. CONCLUSION: The method is easy to perform, low morbidity, scarring of good quality even in young people, reducing needs detachments of the eyelids.

RESUMO

INTRODUÇÃO: Os autores referem à evolução no tratamento do envelhecimento facial. OBJETIVO Por meio de incisão marginal intracapilar temporal reduzir a flacidez palpebral e elevar a cauda do supercílio, tornar a blefaroplastia econômica. MÉTODO: Propõem realizar a ritidoplastia do segmento temporal com incisão intracapilar marginal curva, facilitando as compensações de pele, 0,5 cm dentro do couro cabeludo, desepidermizando 2 mm dele. Concluem o procedimento, desengordurando o bordo do retalho e realizando sutura tricofítica, camuflando melhor a cicatriz, que se torna pré-capilar, pelo crescimento de cabelos junto dela. Descolamento no plano subcutâneo até o bordo da metade lateral do músculo orbicular, tração e sutura latero-superior em leque com pontos separados do mesmo. Outros procedimentos no restante da face e pescoço quando necessários foram realizados. A tração lateral reduz a flacidez palpebral e seus excessos de pele, tornando a blefaroplastia econômica, com ressecções pequenas de pele, e remoção das bolsas por mínima divulsão individualizada do músculo orbicular, no local da projeção das mesmas. RESULTADOS: Obtiveram bons resultados, reduzindo o estigma do envelhecimento da região orbitaria. Trata-se de um procedimento simples, de morbidade baixa, deixando cicatriz marginal temporal de boa qualidade, pela pequena tração da pele, é facilmente camuflável pelos cabelos que crescem junto dela, em direção inferior, cobrindo-a. Foram operados 485 casos de 1997 a 2012. CONCLUSÃO: O método é de fácil execução, baixa morbidade, cicatrizes de boa qualidade mesmo em jovens, reduzindo necessidades de descolamentos das pálpebras.

Correction of palatine fistulas with a musculo-mucosal buccinator flap: results of 6 cases after 27 years

ABSTRACT

Introduction: The buccal musculo-mucosal patch, described in 1989, can be used to correct palatine fistulas and fissures with stretching of the soft palate, or to cover bloody areas after tumor resection.
Methods: This is an analysis of the 27-year postoperative results for 6 patients who underwent operation at Base Hospital and Santa Casa de São José do Rio Preto between 1984 and 1989, and reassessed in 2016, when a myo-buccinator mucosa was used for cleft palate correction.
Results: Of the 36 operated cases, 6 were reevaluated after 27 years, of which 5 had primary correction and 1 had a secondary correction (fistula after cleft palate closure). All the cases had satisfactory results in terms of maxillary growth, correction of the palatine fistula, and speech function.
Conclusion: Although not statistically significant, the present study demonstrated that the buccal musculo-mucosal flap is an adequate procedure for correction and stretching of the palate, with normal or near-normal maxillary growth and practically normal speech even without adequate phono-audiological treatment.

RESUMO

Introdução: O retalho miomucoso de músculo bucinador, descrito em 1989, pode ser utilizado para corrigir fístulas palatinas, fissuras com alongamento do palato mole ou cobrir áreas cruentas após ressecções de tumores.
Métodos: Trata-se da análise do resultado após 27 anos de 6 casos de pacientes operados no Hospital de Base e na Santa Casa de São José do Rio Preto, no período de 1984 a 1989, e reavaliados em 2016, nos quais foram realizados retalhos miomucosos de bucinador para correção de fissura palatina.
Resultados: Dos 36 casos operados, 6 foram reavaliados após 27 anos, dos quais 5 trataram-se de correção primária e 1 de correção secundária (fístula após fechamento de fissura palatina). Todos os casos obtiveram resultados satisfatórios no crescimento maxilar, na correção da fistula palatina e na função da fala.
Conclusão: Apesar de estatisticamente não significativo, o presente estudo demonstrou que o retalho miomucoso de músculo bucinador para correção e alongamento do palato é um procedimento adequado, com resultados de crescimento maxilar normal ou próximo disso e fala praticamente normal, mesmo sem adequado tratamento fonoaudiológico.

Mastopexy and breast augmentation with protection and inferior support of the prosthesis with inferior pedicle flap

ABSTRACT

The authors mark the mammary meridian, the sub mammary fold, the intersection between these lines (point I), its projection on mamma (point A) and two others intermediaries points A1 and A2, quantifying the skin resection in two spindles between A1 and A2 and between A2 and I by bidigital impingement. They use a vertical mammary flap with inferior pedicle, slid downward in V-Y during the closure of the incisions, after the introduction of the prosthesis and increase of the projection of mamma, offering protection and higher support to the prosthesis, and avoiding the palpation feeling in the mammary groove. The technique still excludes the sliding of the prosthesis below the groove and the consequent emptying of the lap. The authors obtained good results in 56 patients operated.

RESUMO

Os autores marcam o meridiano mamário, o sulco mamário, a intersecção entre essas linhas (ponto I), a projeção dele sobre a mama (ponto A) e outros dois pontos intermediários A1 e A2, quantificando a ressecção da pele em dois fusos entre A1 e A2 e A2 e I determinados por pinçamento bidigital. Utilizam a tática do retalho mamário vertical de pedículo inferior deslizado para baixo em V-Y durante o fechamento das incisões, após a introdução da prótese e aumento da projeção da mama, a fim de oferecer proteção e maior suporte à prótese e eliminar a sensação de palpação da mesma no sulco mamário. A técnica ainda evita o deslizamento da prótese abaixo do sulco e o consequente esvaziamento do colo. Obtiveram bons resultados em 56 pacientes operadas.

Large volume or area vibroliposuction?

ABSTRACT

The authors present a method of evaluation of large scale liposuction, establishing a parallel to burn evaluation. The table of Lund and Browder was utilized as a parameter. The authors determined a maximum area to be treated, discuss the technique, medications and controls utilized to prevent severe complications. They also present results obtained from 487 surgeries, during the period of January 1998 and July 2004, with vibroliposuction performed in body areas which varied from 20 to 30%.

Os autores apresentam metodologia de avaliação para as grandes lipoaspirações, estabelecendo paralelo com queimaduras. Utilizam-se da tabela de Lund e Browder como parâmetro. Determinam área máxima de tratamento, discutem a técnica, as medicações e controles utilizados para prevenir complicações graves. Apresentam os resultados obtidos em 487 cirurgias com vibrolipoaspiração realizadas em áreas corporais que variam de 20 a 30%, no período de janeiro de 1998 a julho de 2004.

Composed gluteal-crural lift

ABSTRACT

Introduction: The authors propose a new technique of buttocks and thighs lift in massive weigh loss patients, overlapping the volume of the buttocks, elevating and projecting them. Methods: The trocantheric area and the internal aspect of the thighs, when necessary, are liposuctioned. They remove the skin excess through an incision that initiates in the intergluteal furrow and outlines the posterior and anterior iliac crest. It goes down for the pleat of the groins until the internal face of the thigh and, in "L", it goes down near the knees. The skin resections are always done above the fascia superficialis. The surgery begins in ventral decubitus for the suspension of the buttocks and it is completed in dorsal decubitus with the suspension of the thighs.

RESUMO

Introdução: Os autores propõem uma nova técnica de suspensão de nádegas e coxas em pacientes pós-grandes emagrecimentos, sobrepondo o volume das nádegas, elevando-as e projetando-as. Método: A região trocantérica e a face interna das coxas, quando necessário, são lipoaspiradas. Retira-se o excesso de pele por incisão que parte do sulco interglúteo, contorna a crista ilíaca posterior e anterior, desce pela prega inguinal até a face interna da coxa e, em "L", desce até acima dos joelhos. As ressecções de pele são feitas sempre acima da fascia superficialis. Inicia-se a cirurgia em decúbito ventral para a suspensão das nádegas e conclui-se em decúbito dorsal com a suspensão das coxas.

Neocolpoplasty with excluded sigmoid segment

ABSTRACT

Introduction: The use of intestinal loops for treatment of vaginal agenesis remounts to the beginning of XX century; however it has been little popularized. Methods: The authors present seven cases wherein the neocolpoplasty has been realized with the use of excluded sigmoid loop, transposed trough the bottom of Douglas sack by peritoneal incision and accommodated in a dissected tunnel between the rectum and the bladder. The inferior extremity was sutured to the vaginal introit with incision in V, and the superior extremity closed in a blind bottom, or sutured around of the telescoped uterine cervix trough her, when the uterus was present. Results: The evolution is from three to ten years. The technique was executed in three cases with vagina absence associated to the uterus agenesis, one case of adrenogenital syndrome with the vagina atresia associated to an infantile uterus, and three cases of vaginal agenesis with the presence of functional uterus. The uterus was preserved with his lap inside of the neovagina, allowing normal menstruation in two of them. Discussion: Weren't necessary, not even indicated, dilations or use of molds in the postoperative period. The results were good and the index of complications was small. The vaginas allow easy penetration. They are ample, deep and elastic, with lubrication and normal visual aspect. Conclusion: The neocolpoplasty show it to be a viable technique with low complication index, mainly estenoses.

RESUMO

Introdução: O emprego de alças intestinais para tratamento de agenesia de vagina remonta ao início do século vinte, porém tem sido pouco popularizado. Método: Os autores apresentam sete casos em que a neocolpoplastia foi realizada com o emprego de alça exclusa de sigmóide, transposta através do fundo de saco de Douglas, por incisão peritoneal, e alojada em um túnel dissecado entre o reto e a bexiga. A extremidade inferior foi suturada ao intróito vaginal com incisão quebrada em V, e a extremidade superior fechada em fundo cego, ou suturada ao redor do colo uterino telescopado através dela, quando o útero estava presente. Resultados: A evolução é de três a dez anos. A técnica foi executada em três casos com ausência de vagina associada à agenesia de útero, um caso de síndrome adrenogenital com atresia de vagina associada a útero infantil, e três casos de agenesia de vagina com presença de útero funcional. O útero foi preservado, com seu colo dentro da neovagina, permitindo menstruações normais em dois deles. Discussão: Não foram necessários, nem indicados, dilatações ou uso de moldes no período pós-operatório. Os resultados foram bons e o índice de complicações pequeno. As vaginas permitem penetração fácil. São amplas, profundas e elásticas, com lubrificação e aspecto visual normais. Conclusão: A neocolplastia mostrou-se uma técnica viável com baixo índice de complicação, principalmente estenoses.

Classic abdominoplasty associated with sectorial vibroliposuction

ABSTRACT

Introduction: The authors propose a technique to treat flaccidity and excess of fat panicle in the upper and central area of the abdomen using the classic abdominoplasty associated with sectorial vibroliposuction non tumescent. The objective is to create a relief of saliencies and depressions in the abdominal area, reduce the projection of the epigastrium and to position the suprapubic transverse scar in a lower place. Method: Between September 2007 and January 2009, 16 female patients, with ages between 24 and 60 years old, with in abdominal group V, according to the classification of Bozola and Psillakis, and with an excess of fat tissue in the upper abdomen underwent classic abdominoplasty associated with sectorial vibroliposuction. Results: All the patients of the process were pleased with the results obtained. In 2 cases, were detected the presence of seromas which had been drained through ambulatory punctures. In another patient, there was hypertrofic scar, which was treated with infiltration of triamcinolone resulting in the complete regression of the hypertrophy. Nodulations in the central area of the abdomen were observed in 2 patients who had undergone superficial vibroliposuction. Conclusion: The procedure is effective in the aesthetic treatment of the abdominal region. When performed at the end of the abdominoplasty in the upper and central part of the abdomen the vibroliposuction improves abdominal contour. It maintains the naturalness of the results and the lower positioning of the transverse suprapubic scar that facilitates its coverage with small clothes. It is secure, fast and easy to implement.

RESUMO

Introdução: Os autores propõem uma técnica para tratar a flacidez e o excesso de panículo adiposo nas regiões superior e central do abdome por meio da abdominoplastia clássica associada à vibrolipoaspiração setorial não tumescente. O objetivo é criar um relevo de saliências e depressões na região abdominal, reduzir a projeção do epigástrio e posicionar a cicatriz transversal suprapúbica em local baixo. Método: Entre setembro de 2007 e janeiro de 2009, 16 pacientes do sexo feminino, com idades entre 24 e 60 anos, com abdomes do grupo V na classificação de Bozola e Psillakis e excesso de tecido adiposo no abdome superior foram submetidos à abdominoplastia clássica associada à vibrolipoaspiração setorial. Resultados: Todas as pacientes da série encontram-se satisfeitas com os resultados obtidos. Em 2 pacientes, detectou-se a presença de seromas, que foram drenados por meio de punções ambulatoriais. Em outra paciente, houve cicatrização hipertrófica, tratada com infiltração de triancinolona e regressão completa da hipertrofia. Nodulações na área central do abdome foram observadas em 2 pacientes, nos quais se realizou vibrolipoaspiração superficial. Conclusão: A técnica descrita é eficaz no tratamento estético da região abdominal. A vibrolipoaspiração não tumescente, realizada no final da abdominoplastia nas regiões superior e central do abdome, melhora o contorno abdominal, mantendo a naturalidade dos resultados e o posicionamento mais baixo da cicatriz transversal suprapúbica, o que facilita sua cobertura com vestes menores. É de execução fácil, segura e rápida.

Geometric analysis of the shapes of beautiful breasts and breast implants based on the golden ratio (Phi): practical application

ABSTRACT

Introduction: The use of conventional prostheses for hypomastia often yields unsatisfactory and inconsistent results in the final form of the breasts. Considering the number of reported studies on breast and breast-prosthesis forms is negligible, the aim of this study was to optimize the shape results of breast implants on the basis of the Phi ratio, historically known as the golden or divine ratio. Methods: The Phi ratio (1/1.618...) is believed to be registered in the human limbic system as the value of beauty, harmony, and balance of proportions. The three-dimensional anatomy (front, lateral, and vertical views) of beautifully shaped breasts was analyzed geometrically on the basis of the Phi ratio. The various forms of hypomastia were classified. Further, two new forms of polyurethane-covered breast implants were fabricated on the basis of the Phi ratio. The prostheses were chosen by means of vertical measurement of the patient's chest in the frontal view and the hypomastia classification. Results: Ten pairs of prostheses were specially fabricated and implanted in 10 patients. Follow-up for 12 months showed good results for the breast shapes and volumes. Conclusion: Geometric and three-dimensional analysis of beautiful breast shapes and fabrication of breast prostheses based on the Phi ratio are possible to correct hypomastia. In addition to the simple choice of prostheses, the methods provide good and homogeneous results of breast shapes and volumes, reducing subjectivity.

RESUMO

Introdução: Os autores observaram resultados insatisfatórios e inconstantes na forma final de mamas após inclusão de próteses de formatos convencionais em casos de hipomastias, havendo poucos estudos a respeito de formas de mamas e de próteses. Método: Descrevem a proporção Phi (1/1.618...), historicamente conhecida como proporção áurea ou divina, e seus conceitos. Argumentam sua importância nas formas em geral, registrada no sistema límbico humano como valor de beleza, harmonia e equilíbrio de proporções. Propõem uma anatomia da beleza da forma da mama em visão tridimensional (visão frontal, lateral e vertical), geometrizada e baseada em Phi. Classificam as mamas quanto à forma incluindo as diferentes formas de hipomastias. Propõem nova forma de prótese mamária, contendo a proporção Phi e coberta com poliuretano. Escolhem as próteses a serem utilizadas por meio de uma medida vertical do tórax da paciente em visão frontal, e da classificação das hipomastias. Resultados: Dez pares de próteses foram especialmente confeccionados e implantados em dez pacientes que foram acompanhadas por 12 meses, apresentando bons resultados finais de forma e volume. Conclusão: É possível estabelecer visão geométrica e tridimensional das mamas bonitas quanto à forma, baseada na proporção Phi, e próteses com a forma também confeccionadas sob a mesma proporção para corrigir as hipomastias. Além de maneira simples de escolha das próteses, o método proporciona a obtenção de resultados bons e homogêneos de formas e volumes mamários, reduzindo subjetividade.

Abdominoplasty: effectiveness of classification of Bozola and Psillakis - 27 years of note from the author

ABSTRACT

INTRODUCTION: The author recalls the classification proposed by Bozola and Psillakis in 1988, and "aesthetic pathologies" abdominal divided into 5 groups identical to initial treatment in this segment body as excess skin, subcutaneous fat trunk, diastasis of the rectus muscles, external obliques, and deployment vertical position of the navel. Proposed model abdomen aesthetically beautiful like "lira" as he had done with elevations and depressions, highlights and shadows, creating relief. Define beauty and possibility of its reproduction. OBJECTIVE: To assess the practicality of their diagnosis and treatments of the proposed classification and evolution of the same in the 27 subsequent years with different tactics from those described initially, keeping unchanged the classification. METHOD: Describes the five treatment groups and percentages of each of the last 200 operated cases. 614 cases were operated from 2001 to 2011, with possible outcomes to obtain. RESULTS: The classification was effective in 99% of the operated cases. CONCLUSION: The classification proposed by Bozola and Psillakis remains unchanged, the treatments included new tactics, now more extensive and intense liposuction (vibroliposuction) plications of the rectus and obliques through tunnels hipsters in areas without major perforating vessels that supply skin and subcutaneous tissue of the abdomen, it refixando aponeurotic wall, reducing seroma and hematoma. And reducing traction and skin resection, with better quality of scars, and results.

INTRODUÇÃO: O autor recorda a classificação proposta por Bozola & Psillakis em 1988, e as "patologias estéticas" abdominais divididas em cinco grupos idênticas à inicial, no tratamento desse segmento corporal, conforme excedente de pele, gorduras subcutâneas do tronco, diastasis dos músculos retos, oblíquos externos, e posição de implantação vertical do umbigo. Propõe modelo de abdome esteticamente bonito semelhante à "lira", como havia feito, com saliências e depressões, brilhos e sombras, criando o relevo. Define beleza e possibilidade de sua reprodução. OBJETIVO Analisar a praticidade de diagnósticos e respectivos tratamentos da classificação proposta, e evolução dos mesmos nos 27 anos subsequentes, com táticas diferentes das descritas inicialmente, mantendo inalterada a classificação. MÉTODO: Descreve o tratamento nos cinco grupos e porcentagens de cada um nos últimos 200 casos operados. Seiscentos e catorze casos foram operados de 2001 a 2011, com os resultados possíveis de se obter. RESULTADOS: A classificação foi efetiva em 99% dos casos operados. CONCLUSÃO: A classificação proposta por Bozola & Psillakis permanece inalterada, os tratamentos incluíram novas táticas, atualmente com mais extensa e intensa lipoaspiração (vibrolipoaspiração), plicaturas dos músculos retos e oblíquos através de túneis descolados em áreas sem vasos perfurantes importantes que irrigam pele e subcutâneo do abdome, refixando-o a parede aponeurótica, reduzindo seromas e hematomas. E redução da tração e ressecção de pele, com melhor qualidade das cicatrizes, e dos resultados.

Treatment of sindactylism and polydactylism in the same foot

ABSTRACT

The authors describe a case of a patient born with sindactylism and polydactylism - seven fingers left foot. The child had an important social-functional harm. The child had both extra fingers extirpated and subsequent fusion of first metatarsus epiphysis along with second metatarsus diaphysis.

RESUMO

Os autores descrevem o caso de uma paciente que nasceu com sindactilia e polidactilia em pé esquerdo com sete dedos, evidenciando prejuízo sociofuncional para a criança. Foi tratada com extirpação dos dedos extranumerários (dois) e fusão entre a epífise do primeiro e a diáfise do segundo metatarso.
