Breast reduction with the lower pedicle technique: an observational study

ABSTRACT

INTRODUCTION: Reduction mammaplasty with the inferior pedicle nipple-areolar technique is widely used in North America but not in Brazil. Its main advantage lies in maintaining the sensitivity of the areola in large mammary ptoses. To describe the preoperative and postoperative characteristics of patients who underwent reduction mammaplasty with the inferior pedicle nipple-areolar technique at the Santa Casa de Campo Grande - MS in 2013. METHOD: We performed interview, physical examination, and review of medical records of patients operated by using this technique at this institution in 2013. RESULTS: Forty patients were evaluated. Their ages ranged from 21 to 68 years, with a mean of 40.62 years. The comorbidities reported were hypertension, diabetes, ulcerative colitis, disc herniation, and anxiety disorder. The average weight of resected tissue was 600.6 g from the right breast and 609.6 g from the left breast. The most common presurgery complaint was low back pain, followed by shoulder pain. The most frequent complications in the early postoperative phase were dehiscence of the mid-point of the strand (10%) and hematoma (5%). The predominant type of anesthesia was general anesthesia. All patients showed benign breast dysplasia in the postoperative histopathological examination. CONCLUSION: The inferior pedicle nipple-areolar technique was adequate for the treatment of major hypertrophies and mammary ptoses.

RESUMO

INTRODUÇÃO: A mamoplastia redutora utilizando pedículo inferior incluindo complexo aréolopapilar é muito utilizada na América do Norte, mas pouco difundida no Brasil. Sua principal vantagem é a utilização em grandes ptoses mamárias, mantendo a sensibilidade da aréola. OBJETIVO: O objetivo é descrever as características pré e pós-operatórias de pacientes submetidos a mamoplastia redutora pela técnica do pedículo inferior areolado na Santa Casa de Campo Grande - MS em 2013. MÉTODO: Entrevista, exame físico e dados de prontuário das pacientes operadas por esta técnica nessa instituição no ano de 2013. RESULTADOS: Foram avaliadas 40 pacientes, sendo que a idade variou de 21 a 68 anos, com média de 40,62 anos. As comorbidades relatadas foram hipertensão arterial sistêmica, diabetes, retocolite ulcerativa, hérnia de disco e distúrbio de ansiedade. O peso médio de tecido mamário ressecado foi 600,6g na mama direita e 609,6g na mama esquerda. Dentre as queixas pré-operatória, a mais comum foi a lombalgia, seguida por dor nos ombros. As mais frequentes complicações no pós-operatório recente foram a deiscência do ponto médio da vertente (10%) e o hematoma (5%).O tipo de Anestesia predominante foi a anestesia geral. Todas as pacientes apresentaram displasias benignas da mama nos histopatológicos de pós-operatórios. CONCLUSÃO: A técnica do pedículo inferior areolado mostrou-se adequada para o tratamento de grandes hipertrofias e ptoses mamárias.

Cost analysis in patients admitted to the Santa Casa Misericórdia de Campo Grande, treated with and without the aid of hyperbaric oxygen therapy

ABSTRACT

INTRODUCTION: Burns are among the leading external causes of death in Brazil, generating huge costs and having physical, psychological, and social consequences. The objective of this study was to compare two groups of burn patients, i.e., those with and those without adjuvant treatment with hyperbaric oxygen (HBO) therapy and to perform a cost analysis. METHODS: A retrospective study was carried out between 2011 and 2013, comparing 20 patients with moderate to severe burns who were hospitalized at the Santa Casa de Misericórdia de Campo Grande and treated with HBO (group I) and 22 patients treated without HBO (group II). RESULTS: Most of the patients, i.e., 24 (57.1%), were male. Flammable product was the most frequent etiologic agent in both groups, accounting for 56.6% of the burn cases. The average age of group I patients was of 26.6 years and that of group II was of 23.3 years, with a prevalence (50.4%) of those aged between 13 and 35 years. The average final cost per patient was R$17,292.00 for group I and R$25,360 for group II (p = 0.028). CONCLUSION: We conclude that, in our service, patients presenting moderate and severe burns and treated with HBO therapy underwent a lower number of surgical procedures, e.g., grafts and debridement that allowed reduced hospitalization time and led to significant savings in hospital costs.

RESUMO

INTRODUÇÃO: As queimaduras estão entre as principais causas externas de morte no Brasil, geram enormes gastos financeiros e são responsáveis por sequelas físicas, psicológicas e sociais. O Objetivo deste estudo foi comparar os dois grupos de pacientes queimados com e sem tratamento coadjuvante com Oxigenoterapia Hiperbárica, assim como, realizar uma análise dos custos. MÉTODOS: Foi realizado estudo retrospectivo, período de 2011 a 2013, selecionando, 20 pacientes com queimaduras moderadas e graves, internados na Santa Casa de Misericórdia de Campo Grande, tratados com auxílio de OHB (grupo I) e 22 pacientes sem auxílio de OHB (grupo II). RESULTADOS: A maioria dos pacientes pertencia ao sexo masculino, 24 pacientes (57,1%). O agente etiológico mais frequente nos dois grupos foi produtos inflamáveis com 56,6% dos casos. A idade média do grupo I foi de 26,6 e a do grupo II 23,3, predominando a faixa etária entre 13 e 35 anos 50,4% dos pacientes. Custo médio final por paciente do Grupo I foi R$17.292,00, e grupo II R$25.360 (p=0,028). CONCLUSÃO: Concluímos que, no nosso serviço, o grupo de pacientes com queimaduras moderadas e graves tratados com auxílio da Oxigenioterapia Hiperbárica teve um menor número de procedimentos cirúrgicos, como enxertos e desbridamento, e menor tempo de internação, levando a uma economia importante dos gastos hospitalares.

Epidemiological profile of patients treated with the aid of hyperbaric oxygen in the State of Mato Grosso do Sul from May 2007 to October 2012

ABSTRACT

BACKGROUND: The Hyperbaric Oxygen Therapy-HOT, is a non-invasive therapeutic method in which patient breaths 100% oxygen (pure oxygen) through masks, while remain in a pressurized chamber to a pressure higher than atmospheric. Its act as an accelerator of recovery process, by increasing the oxygen saturation in the body that allows to speed up cicatrization and engagement to infection. The aim of this study was to determine the epidemiological profile of hyperbaric oxygen therapy patient of Mato Grosso do Sul state. OBJECTIVE: To demonstrate the Epidemiological Profile of patients treated with the aid of hyperbaric oxygen in the State of Mato Grosso do Sul May 2007 to October 2012. METHODS: We conducted a restrospective study of HOT patients admitted to Santa Casa de Campo Grande Hospital from May 2007 to October 2012. RESULTS: Among 600 patients who underwent HOT, there was a predominance for males (71% - 425 patients); Plastic surgery is the medical speciality which most request HOT associated with conventional treatment for their patients (71% of the total patients), and most of these are due to burn injury (82%). Most of the patients were aged between 31 and 60 years old. (the range with highest prevalence were 31 to 60 years). The mean number of hyperbaric sessions among the burn patients was between 6 and 10 sessions (66.19% of burn patients). CONCLUSIONS: The majority of our service to patients requiring hyperbaric oxygen therapy was male because of moderate to severe burns, and the Plastic Surgery specialty that most used this tool as adjuvant treatment.

RESUMO

INTRODUÇÃO: A oxigenoterapia hiperbárica - OHB é uma modalidade terapêutica não invasiva em que o paciente respira oxigênio a 100% (oxigênio puro), com o uso de máscaras, enquanto permanece em uma câmara pressurizada a uma pressão superior à atmosférica. Seu papel é atuar como um acelerador do processo de recuperação, por meio do aumento da saturação de oxigênio no organismo, que permite a aceleração nas cicatrizações e no combate a diversas infecções. OBJETIVO Demostrar o perfil epidemiológico dos pacientes tratados com auxílio da oxigenioterapia hiperbárica no Estado de Mato Grosso do Sul, de maio 2007 a outubro 2012. MÉTODO: Realizou-se estudo retrospectivo dos pacientes admitidos na Santa Casa de Campo Grande, que utilizaram oxigenioterapia hiperbárica, de maio de 2007 a outubro de 2012. RESULTADOS: Dos 600 pacientes que se submeteram à OHB, predominou o sexo masculino (71% - 425 pacientes); A cirurgia plástica foi a especialidade que mais solicitou OHB associada ao tratamento convencional de seus pacientes (71% do total de pacientes), sendo destes a maioria devido à lesão por queimadura (82%). O intervalo de idade de maior prevalência foi dos 31 aos 60 anos. A média do número de sessões de hiperbárica, dentre os pacientes com queimadura, foi entre 6 e 10 (66,19% do total de queimados). CONCLUSÕES: A maioria dos pacientes que necessitavam de oxigenioterapia hiperbárica era do sexo masculino devido a queimaduras moderadas a graves, sendo a Cirurgia Plástica a especialidade que mais utilizou esta ferramenta como tratamento adjuvante.
