Bichectomy and its contribution to facial harmony

ABSTRACT

Introduction: Buccal fat removal was described for the first time by Heister in 1732. In 1802, Bichat identified the adipose nature of this tissue. There is a close association between buccal fat and masticatory muscles, which helps infants suck owing to the sliding motion of the tissues. Its prominence diminishes with age, both through modest reduction in volume and facial growth. Bichectomy or bichatectomy is a surgical procedure that involves extraction of Bichat's fat pad for aesthetic and/or functional purposes. This study aimed to show that bichectomy is a low-risk surgical procedure with good results, provided it is appropriately indicated and is performed following technical standards according to the anatomy of the facial structures.
Methods: This prospective cohort study was conducted between January 2016 and April 2018 and included 59 patients with a mean age of 31 years. The follow-up time was 12-26 months.
Results: Of 59 patients, 46 were female, and 13 were male. All patients underwent same-day surgery. On average, 3.2 mL of fat was removed.
Conclusion: Removal of Bichat's fat according to anatomical principles is safe. This technique can improve facial harmony. Success depends on application of appropriate techniques.

RESUMO

Introdução: O tecido gorduroso bucal foi descrito pela primeira vez em 1732, por Heister. Em 1802, Bichat diagnosticou sua natureza adiposa. Há uma estreita relação entre a gordura bucal e músculos da mastigação, auxiliando os lactentes na sucção do leite por atuar com um tecido de deslizamento. Sua projeção diminui com o aumento da idade, tanto pela redução discreta de seu volume quanto pelo crescimento facial. A bichectomia ou bichatectomia é o procedimento cirúrgico que visa remover a "bola gordurosa de Bichat", com fins estéticos e/ ou funcionais. O objetivo é demonstrar que a bichectomia é um procedimento de baixo risco cirúrgico e com bons resultados, desde que seja bem indicada e siga os padrões técnicos, respeitando a anatomia das estruturas faciais.
Métodos: Estudo prospectivo de coorte, realizado entre janeiro de 2016 e abril de 2018, totalizando 59 pacientes, com média de idade de 31 anos. O tempo de seguimento foi de 12 a 26 meses.
Resultados: Foram avaliados 59 pacientes, com média de idade de 31 anos, sendo 46 do sexo feminino e 13 do sexo masculino. Todos os pacientes receberam alta no mesmo dia. Em média, foram retirados 3,2ml de volume da gordura de Bichat.
Conclusão: A retirada de porções das gorduras de Bichat é segura, desde que sejam observados os detalhes anatômicos. Pode oferecer maior harmonia entre os três terços da face. O sucesso dos resultados depende de uma indicação precisa para o procedimento.

Quality of life and aesthetic results after mastectomy and mammary reconstruction

ABSTRACT

Introduction: Due to the increased incidence of breast cancer, the demand for breast reconstruction has been increasing, along with concerns regarding the satisfaction and quality of life of the patients. Mastectomy can be a traumatic experience, especially when it is perceived as a mutilation, which can impact self-esteem and emotional stability. The BREAST-Q® questionnaire was internationally validated and formulated for the pre- and postoperative assessment of quality of life related to breast reconstruction. This study aimed to evaluate quality of life and aesthetic result satisfaction in patients who underwent breast reconstruction with implants by comparing the period after breast reconstruction with the period before.
Method: A retrospective longitudinal observational study was carried out by reviewing the charts of patients who underwent breast reconstruction using silicone or tissue expander implants from January 2014 to December 2016, in association with a cross-sectional study of the Breast-Q® questionnaire and an evaluation of aesthetic results based on photographic analysis before and after surgery.
Results: We selected 74 patients who underwent breast reconstruction with implants (79.7% with silicone prostheses and 20.3% with expanders); 95.94% of the reconstructions were immediate, and no particular laterality predominated. We obtained statistical significance in the domains of both breast satisfaction and physical well-being. Most cases were considered satisfactory by the external evaluator.
Conclusion: The patients' quality of life in the period after breast reconstruction with breast implants was superior to that in the period prior to the procedure.

RESUMO

Introdução: Em decorrência do aumento na incidência de câncer de mama, a procura pela reconstrução mamária vem crescendo, juntamente com a preocupação em relação à satisfação e à qualidade de vida das pacientes. Mastectomia pode ser vivenciada de modo traumático, sendo considerada mutilação, afetando autoestima e estabilidade emocional. O questionário BREAST-Q® foi validado internacionalmente e formulado para avaliação pré e pós-operatória da qualidade de vida relacionada à reconstrução mamária. O objetivo do estudo é avaliar a qualidade de vida e satisfação com o resultado estético das pacientes submetidas à reconstrução mamária com implantes, comparando o período anterior com o período posterior à reconstrução mamária.
Métodos: Realizado estudo observacional longitudinal retrospectivo por meio da revisão de prontuários de pacientes submetidas à reconstrução mamária com uso de implantes de silicone ou de expansor de tecido no período de janeiro de 2014 a dezembro de 2016, associado a estudo transversal por meio da aplicação do questionário Breast-Q® e avaliação do resultado estético após análise fotográfica pré e pós-operatória.
Resultados: Foram selecionadas 74 pacientes que foram submetidas à reconstrução mamária com implantes (79,7% com prótese de silicone e 20,3% com expansor); 95,94% das reconstruções foram imediatas e não houve predomínio quanto à lateralidade. Obtivemos significância estatística tanto no domínio satisfação com a mama quanto no domínio bem-estar físico. A maioria dos casos foram considerados satisfatórios pelo avaliador externo.
Conclusão: A qualidade de vida das pacientes no período posterior à reconstrução mamária com implantes mamários é superior em relação ao período anterior ao procedimento cirúrgico.

Columellar rhinoplasty: a new vision using solid silicone

ABSTRACT

Background: The treatment of columello labial angle is an important point in rhinoplasty. We believe that this angle is depressed and acute, in function of a retracted position of the maxilla and nasal spine, that represent a "back" support to the nasal pyramid basis. Objective: We describe in this paper our technique, using a small compact silicone device, included through a vestibular approach, introduced into the columella and supported in front of the maxilla, functioning like an actual advance of the maxilla, redefining the columella shape and projecting the all basis of nasal pyramid and consequently, the nasal tip. Methods: This alloplastic implant was carved in a "medium" or "soft" silicone block (Silimed®), following a pre defined design, adapted to each patient. Results: We have excellent aesthetic with this procedure, replacing many times a classic rhinoplasty, more complex and invasive procedure, or joining additional help associated to it. In our 32 years practice using this technique and among 181 treated patients, we had late expositions of this implant in only two cases.

RESUMO

Introdução: Nas rinoplastias, o tratamento do ângulo columelo-labial é ponto de relevância. A observação levou-nos a crer que, muitas vezes, o ângulo nasal deprimido e agudo ou a falta de projeção da ponta nasal devem-se a uma hipotrofia ou posição recuada do maxilar e da espinha nasal, tornando-os um suporte "recuado" para a base da pirâmide. Isto, associado à falta de sustentação ou reduzidas dimensões das cruras medialis das cartilagens alares, leva a narizes achatados, aduncos, podendo produzir pseudogibas. Objetivos: O objetivo deste trabalho é descrever uma nova concepção de rinoplastia, por técnica em que usamos uma pequena peça de silicone compacta, colocada por via de acesso no vestíbulo bucal, que se encaixará na columela e se apoiará na maxila e espinha nasal, funcionando como um verdadeiro avanço frontal, redefinindo a forma da columela e recolocando e redimensionando toda a base nasal, cuja ponta é projetada no espaço para frente. Método: O implante aloplástico foi esculpido em silicone "medium" (Silimed®) ou "soft", com desenho prédefinido e que se ajustou a cada caso. O procedimento foi acompanhado (ou não) da fratura dos ossos nasais pelos processos convencionais. Resultados: Os resultados estéticos do procedimento são extremamente animadores, muitas vezes substituindo procedimentos de rinoplastias mais complexos e invasivos, ou complementando-os com eficiência. Em nossa prática de 32 anos de uso desta técnica, aplicamos o procedimento em 181 pacientes, com exposição do implante em apenas dois casos.

Breast reconstruction with the transverse rectus abdominis flap: an alternative technique for the closure of abdominal defects

ABSTRACT

INTRODUCTION: The transverse rectus abdominis muscle (TRAM) flap, described by Drever in 1977, was subsequently modified and since then, has rapidly become an important alternative for breast reconstructions. However, bulges and abdominal hernias in the donor area are some of the complications arising from the TRAM flap. This study proposes an alternative technique for the repair of the abdominal defect that minimizes complications and provides better results. METHOD: The retrospective included 120 women who underwent breast reconstruction with TRAM with placement of an abdominal mesh between May 2003 and November 2012, with follow-up of at least six months. Patients were divided into two groups: Group 1 (53 cases) underwent reconstruction with polypropylene mesh positioned between the aponeurotic layers of the external and internal oblique muscles and Group 2 (67 cases) underwent closure of the arcuate Douglas' line and the remaining lower aponeurosis of the rectus abdominis muscle and reinforcement with mesh. RESULTS:  The patients were evaluated by the same surgeon, according to the same criteria. The rate of bulging was 18.9% in group 1 and 3% in group 2, a statistically significant difference. Bulges and hernias in the hypogastrium varied from 20% to 44% and 12.5% to 20% respectively, when closed primarily. When synthetic mesh was used, these percentages decreased to 1.5% to 6%. The technical improvement and comparison of results aims to minimize such complications. CONCLUSION: The technique described is advantageous in that, it significantly reduced the occurrence of the most common complication, and increased the aesthetic and functional satisfaction of the patients.

RESUMO

INTRODUÇÃO: O retalho do músculo reto abdominal, descrito por Drever em 1977, foi posteriormente modificado e, desde então, se tornou rapidamente uma importante alternativa para reconstrução da mama. Dentre as complicações do TRAM, citam-se os abaulamentos e hérnias abdominais na área doadora. Este estudo vem propor uma técnica alternativa de reparo do defeito abdominal, com o objetivo de minimizar tais inconvenientes e proporcionar melhores resultados. MÉTODO: O estudo é de caráter retrospectivo, totalizando 120 mulheres submetidas à reconstrução mamária com TRAM com colocação de tela abdominal, e seguimento mínimo de seis meses, entre Maio de 2003 e Novembro de 2012. As pacientes foram divididas em dois grupos: Grupo 1 (53 casos) reconstruídos com tela de polipropileno posicionada entre os folhetos aponeuróticos dos músculos oblíquo externo e interno, e Grupo 2 (67 casos) realizado fechamento da linha arqueada de Douglas e o remanescente inferior da aponeurose do músculo reto abdominal e reforço com tela. RESULTADOS: As pacientes foram avaliadas pelo mesmo cirurgião, obedecendo aos mesmos critérios. Os resultados obtidos foram 18,9% de abaulamentos no grupo 1 e 3% no grupo 2, estatisticamente significativo. Abaulamentos e hérnias no hipogástrio variam de 20% a 44% e 12,5% a 20% respectivamente, quando fechados primariamente. Quando associado a aplicações de telas sintéticas, esses percentuais diminuem para 1,5% a 6%. O aprimoramento técnico e a comparação de resultados têm a finalidade de minimizar tais complicações. CONCLUSÃO: A técnica descrita mostra-se vantajosa, pois permitiu significativa diminuição da complicação mais comum, permitindo satisfação estética e funcional das pacientes.

Mammoplasty/mastopexy using implants: the Lockpocket technique

ABSTRACT

INTRODUCTION: Mammoplasty is the most commonly performed plastic surgery procedure in Brazil, which is second only to the United States in terms of the number of mammoplasties performed. Several techniques based on increasing volume using breast implants are used to correct breast ptosis. We aimed to describe a technical variation of implant mammoplasty: isolation of the implant in a closed pocket of fascioglandular tissue (Lockpocket). METHODS: This prospective study was carried out between June 2013 and June 2016. Forty-three patients underwent (1) augmentation mammoplasty using a silicone implant that was isolated from the external environment by a closed pocket of fascioglandular tissue, and (2) resection of excess dermoglandular tissue to correct mammary ptosis. Were subsequently analyzed statistically. RESULTS: Of the 43 patients, the majority (22 patients) presented grade II ptosis according to the Regnault classification. The volume of glandular tissue removed was similar to the volume of the prosthesis introduced. A total of seven complications were observed: partial dehiscence (n = 4), discrete asymmetry (n = 2), and residual ptosis (n = 1). CONCLUSION: Augmentation mammoplasty with correction of mammary ptosis (the Lockpocket technique) is a good option because it allows the surgeon to choose in advance the volume of the implant to be used. The method also isolates the implant in a totally closed plane of fascioglandular tissue, and involves the precise removal of breast tissue.

RESUMO

INTRODUÇÃO: A mamoplastia foi o procedimento em cirurgia plástica mais realizado no Brasil, ficando em segundo lugar no mundo, logo atrás dos Estados Unidos. Existem diversas técnicas para correção de ptose das mamas associado a aumento do volume com implantes mamários. O objetivo é descrever uma variação técnica de mamoplastia com prótese, isolando o implante em um bolsão fechado de tecido fascioglandular (Lockpocket). MÉTODOS: Realizado um estudo prospectivo entre junho de 2013 e junho de 2016 totalizando 43 pacientes que foram submetidos à mamoplastia de aumento com prótese de silicone, isolado do meio externo por um bolsão fechado de tecido fascioglandular, e ressecção de tecido dermoglandular excedente para correção de ptose mamária, sendo realizada análise estatística. RESULTADOS: Das 43 pacientes, a maioria (22 pacientes) apresentaram ptose grau II, segundo a classificação de Regnault. Os volumes de tecido glandular retirado e volume das prótese introduzidos foram semelhantes, sendo observado um total de sete complicações: deiscência parcial (n = 4), assimetria discreta (n = 2) e ptose residual em um caso. CONCLUSÃO: A mamoplastia de aumento com correção de ptose mamária - técnica Lockpocket - é uma boa opção, permitindo a escolha prévia do volume do implante utilizado, isolando-o em uma loja totalmente fechada de tecido fascioglandular, e exérese exata de tecido mamário.

Aesthetic and reconstructive application of mammoplasty with the inferior dermal pedicle technique

ABSTRACT

INTRODUCTION: Reduction mammaplasty is one of the most common surgeries performed in the field of plastic surgery, and several techniques have been described. The inferior dermal pedicle technique, which is relatively unknown in Brazil, may have applications in both aesthetic and reconstructive surgery. The aim of the current study was to analyze the results of mammaplasty performed with this technique. METHODS: Data were collected from a retrospective analysis of medical records. Study data also included data obtained through telephone contact with patients, photographic analysis, and patient profiles, as well as data on the purpose of the surgery, complications, satisfaction with results, and degree of sensitivity of the nipple-areolar complex (NAC). RESULTS: During the study period, 74 patients underwent mammaplasty with the inferior pedicle technique (42 aesthetic reduction mammaplasties, 13 mammaplasties for immediate breast reconstruction following segmental resections, and 19 post-reconstruction symmetrization mammaplasties with other techniques). Complications occurred in 20 patients. Regarding the assessment of satisfaction by the patients, the outcome was rated as excellent in 62.16% of cases, good in 25.67%, and fair in 12.17%. In terms of the sensitivity of the NAC, 72.97% of patients reported no loss of sensitivity, 20.27% reported a small loss, 6.76% reported significant loss, and no patients reported a total loss of sensitivity. CONCLUSIONS: The inferior pedicle reduction mammaplasty technique has good applicability both in aesthetic reconstructive surgeries and in immediate reconstructions (upper quadrantectomies) or symmetrizations, especially after the use of a transverse rectus abdominis myocutaneous flap. The technique offers a high degree of satisfaction with the results, a low rate of complications, a high degree of preservation of the sensitivity of the NAC, and good maintenance of the results.

INTRODUÇÃO: A mamoplastia redutora é uma das mais frequentes cirurgias realizadas no âmbito da cirurgia plástica, sendo que diversas técnicas são descritas. A técnica do pedículo inferior areolado é uma técnica pouco difundida no Brasil, que pode ter aplicação tanto em cirurgias estéticas quanto reconstrutoras. O objetivo deste estudo foi analisar os resultados de mamoplastias realizadas com esta técnica. MÉTODOS: Análise retrospectiva de prontuários, contato telefônico com as pacientes e análise fotográfica, identificando perfil das pacientes, finalidade da cirurgia, complicações, grau de satisfação e grau de sensibilidade do complexo aréolo-papilar (CAP). RESULTADOS: No período analisado, 74 pacientes foram submetidas à mamoplastia com a técnica do pedículo inferior areolado (42 mamoplastias redutoras estéticas, 13 mamoplastias para reconstrução imediata de mama pós-ressecções segmentares, e 19 mamoplastias de simetrização pós-reconstrução com outras técnicas). Complicações ocorreram em 20 pacientes. Em relação à avaliação da satisfação pelas pacientes, o resultado foi considerado ótimo em 62,16% dos casos, bom em 25,67%, e regular em 12,17%. Quanto à sensibilidade do CAP, 72,97% das pacientes referiram não haver perda da sensibilidade, 20,27% referiram pequena diminuição, 6,76% referiram diminuição significativa e nenhuma relatou perda total da sensibilidade. CONCLUSÕES: A técnica de mamoplastia redutora com pedículo inferior areolado apresenta boa aplicabilidade, tanto em cirurgias estéticas quanto reconstrutoras, seja em reconstruções imediatas (quadrantectomias superiores) ou em simetrizações, principalmente pós TRAM. Apresenta alto grau de satisfação com o resultado, baixa taxa de complicações, alta preservação da sensibilidade do CAP e boa manutenção do resultado.

Circumferential excisions in the trunk and associations in ex-obese patients

ABSTRACT

INTRODUCTION: Currently, obesity is a major problem. Changes in dietary habits, regular exercise and bariatric surgery promote better quality of life. Massive weight loss may cause deformities in different body segments, which are normally corrected in multiple surgeries. We present a systematic way of treating these patients, which enable surgical procedures in different regions of the body in a single step. METHODS: This was a retrospective observational longitudinal study. The surgical technique consisted of gull-wing type incision on the back with resection of fatty tissue associated with abdominoplasty that promoted improvements in the pubis, back, buttocks and lateral thighs via a single incision. The combination of satellite excision as the thigh lifting was associated in certain situations as well as treatment of the upper segment of the trunk in a second time. RESULTS: The age of patients ranged from 24 to 52 years, average of 34 years old. The mean surgical time in the circumferential excisions was 4 hours and 8 minutes. Surgical complications observed were hematoma, flap necrosis and/or infection. There were three cases of small dehiscence and seroma and one case of pulmonary thromboembolism. In the outpatient follow-up, 85.7% of patients showed to be very satisfied with the final result and no complaints about the size and position of scars. Technical analysis showed a satisfaction level of 92.8% of cases. CONCLUSION: Satisfactory results elucidated the safety and feasibility of the technique, and scars were acceptable due to significant improvement in body contouring.

RESUMO

INTRODUÇÃO: A obesidade é um problema importante na atualidade. Mudanças de hábitos dietéticos, exercícios físicos e a cirurgia bariátrica promovem melhor qualidade de vida. A redução ponderal excessiva ocasiona deformidades em diversos segmentos corporais que são corrigidas em múltiplos tempos operatórios. Apresentamos uma sistematização das maneiras de tratamento destes pacientes, que nos permitem atos operatórios envolvendo as várias regiões do corpo em uma mesma etapa. MÉTODOS: Estudo observacional longitudinal retrospectivo. A técnica cirúrgica consistiu em incisão do tipo asa de gaivota em dorso, com ressecção do excedente dermogorduroso associada à abdominoplastia, promovendo melhorias no púbis, dorso, nádegas e face lateral das coxas apenas por esta única incisão. A associação de excisões satélites como o lifting de coxas ocorreu em determinadas situações, bem como o tratamento do segmento superior do tronco em um segundo momento. RESULTADOS: A faixa etária dos pacientes variou de 24 a 52 anos, com média de 34 anos. O tempo médio do ato operatório nas excisões circunferenciais foi de 4 horas e 8 minutos. Não foram observadas complicações cirúrgicas como hematoma, necrose do retalho e/ou infecção. Três casos de pequenas deiscências e seroma, além de um caso de tromboembolismo pulmonar, foram observados. No seguimento ambulatorial, 85,7% dos pacientes mostraram-se muito satisfeitos quanto ao tamanho e posição das cicatrizes. A análise técnica apresentou um grau de satisfação de 92,8%. CONCLUSÃO: Os resultados satisfatórios elucidam a segurança e praticabilidade da técnica, sendo as cicatrizes aceitáveis devido à melhora do contorno corporal.

Alar cartilage flap for projection of the nasal tip

ABSTRACT

Introduction: The nasal tip is a frequent object of study by surgeons, since it is a common reason for pre- and postoperative complaints. Thus, the tip should be thoroughly evaluated to determine the best technique to be used. The objective is to demonstrate an original alar cartilage flap technique for projection of the nasal tip, performed alone or in combination with other procedures for the same purpose.
Method: A retrospective cross-sectional study of 22 patients submitted to open rhinoplasty with use of the alar flap to project the nasal tip.
Results: A total of 22 open rhinoplasties were performed on primary bulbous, fatty, and under-projected tips. Among the patients in the study, 90% were black and 2% were white; 72.7% reported a high degree of satisfaction with the operation, 18.1% considered the result very good, and 9.2% considered the result acceptable. There were no complications/reoperations.
Conclusion: The alar cartilage flap for projection of the nasal tip is simple to perform and can be considered an excellent supplemental method for use in treatment of the black nose.

Umbilicaplasty: experience with the skin mini-circle technique

ABSTRACT

Introduction: The umbilicaplasty is another important technical detail in the lower pubic transversal abdominoplasty. The described and published techniques have similar scope to preserve the navel pedicle but frequently leaving visible and even hypertrophic scars. From the stenotic umbilicus to the reverse open bally button bigger than the patterns, call the attention negatively base on the narrow safe limits to obtain the desired aesthetic aspects. Methods: Seventy two female patients submitted to abdominoplasty under the same umbilicaplasty procedures were followed in a period of 26 months. After the umbilical pedicle dissection from the skin flap a 5 mm mini skin circle is preserved on its free extremity. After the abdominoplasty steps be partially concluded the umbilicaplasty is performed. A small fat cone from the skin flap is resected below the umbilicus final position, a 7 mm transversal incision is done in the skin flap to extrude and suture with isolated non absorbable stitches the small skin circle of the umbilicus. Results: The obtained results were considered satisfactory by the surgical team and the patients, without post operative problems that had require special care. Conclusion: It has been an easy procedure without secondary aesthetic and technical problems and lack of visible scars.

RESUMO

Introdução: A umbilicoplastia compreende mais um detalhe técnico importante no decurso das abdominoplastias tipo incisão pubiana transversal baixa. As técnicas descritas e publicadas têm o mesmo princípio de preservar o umbigo original, deixando com frequência cicatrizes geralmente visíveis, retráteis e mesmo hipertróficas. Paralelamente, a estenose cicatricial ou o inverso, uma área umbilical maior do que a dos padrões estéticos, chama a atenção negativamente, decorrente de uma estreita margem de segurança para se obter efeitos estéticos desejados. Método: Foram revisados 72 pacientes do sexo feminino, operadas de abdominoplastia no período de 26 meses, com os mesmos detalhes técnicos de umbilicoplastia. Após da dissecação do pedículo umbilical, um minicírculo de pele com cerca de 5 mm de diâmetro é preservado na sua extremidade livre. Após as etapas da abdominoplastia, a reposição do umbigo é realizada mediante a exérese de um cone de gordura no retalho do abdome situado na projeção do mesmo e uma incisão cutânea transversal de 7 mm faz exteriorizar o círculo de pele que é suturado na pele mediantes pontos isolados de fio não absorvível. Resultados: Os resultados obtidos foram considerados satisfatórios pela equipe cirúrgica e pelas pacientes, sem presença de problemas pós-operatórios que exigissem cuidados especiais. Conclusão: A técnica de umbilicoplastia apresentada tem sido um procedimento de fácil execução, sem problemas secundários de caráter técnico ou estético e pela ausência de cicatrizes visíveis.

Inadvertent lesion due to surgical focus

ABSTRACT

Introduction: The objective of this study was to report two cases of patients who presented in the immediate postoperative period, tissue lesions that are difficult to explain, which progressed to located necrosis, its clinical evolution, the process of formulating etiological hypotheses and evidences, on which it is made a chance to burn caused by the heat of surgical focus. Case report: Two patients submitted to breast reconstruction developed in 13 years after surgery of radical mastectomy, with signs of radiothermitis in the chest wall in the irradiated areas. Both patients developed bullous lesions in the regions operated, atypical, leading to the investigation of such causes. Raise the possibility of the causative agent of the injuries being developed by the heat outbreaks in the surgical center of the area illuminated. Apart from a biopsy of the lesion of the patients, it was created and applied is a protocol for measuring the brightness and temperature developed under the lights of surgical focus. This was the biopsy with the result consistent with the diagnosis of burn. The analysis shows outbreaks of having achieved a temperature of 63.5°C (146.3ºF) in 135 minutes, overlaid with cloches. Conclusion: From the knowledge of the possibility, given the biopsy and the measurement of temperature indicating the possibility of burns, the doctor must be attentive to the proper use of medical equipment.

RESUMO

Introdução: Entenda-se por lesão inadvertida aquela ocorrida inesperadamente, por acaso ou evento imprevisível, acidental e involuntária. Objetivo: Relatar os casos de dois pacientes que apresentaram, nos pós-operatório imediato, lesões teciduais de difícil explicação, que evoluíram para necroses localizadas, sua evolução clínica, o processo da formulação de hipóteses etiológicas e comprovação, sobre os quais se formulou a hipótese de queimadura causada pelo calor do foco cirúrgico. Relato dos casos: Duas pacientes submetidas a reconstrução mamária desenvolveram lesões bolhosas nas regiões operadas, atípicas, levando à investigação de tais causas. Formulou-se a hipótese de o agente causal das lesões ser o calor desenvolvido pelos focos cirúrgicos no centro das superfícies iluminadas. Além da biópsia da lesão de uma das pacientes, criou-se e aplicou-se um protocolo para mensuração da luminosidade e da temperatura desenvolvida sob as lâmpadas dos focos cirúrgicos. Procedeu-se à biópsia com o resultado compatível com o diagnóstico de queimadura. A análise dos focos demonstra termos atingido a temperatura de 63,5°C em 135 minutos, com as campânulas superpostas, sendo que à temperatura de 44ºC já se pode ter lesão tecidual. Conclusão: Comprovada a possibilidade de queimadura pelo calor do foco cirúrgico, configura-se a necessidade do médico estar atento ao uso adequado dos equipamentos médico-hospitalares, intervir oportunamente junto à indústria para a produção de equipamentos seguros, bem como fazê-los acompanhar de adequadas instruções de uso.
