Use of Bernard-Webster flap for lower lip reconstruction after excision of squamous cell carcinoma: analysis of functional results

ABSTRACT

INTRODUCTION Defects in the lower lips after the excision of squamous cell carcinoma (SCC) are usually repaired using several surgical techniques. However, the functional reconstruction of full-thickness defects in the lower lip remains a challenge. Therefore, the objectives of this study were to describe surgical strategies for the reconstruction of full-thickness defects of the lower lip after surgical excision of the SCC and evaluate the functional results. METHODS: This was a retrospective study of all patients with full-thickness defects in the lower lip after excisions of SCC that were greater than one third of the lip that were repaired using the Bernard-Webster flap between 2011 and 2014. The functional results (sphincteric, motor and sensory function) were evaluated according to previously used criteria. RESULTS: Six lip defects were reconstructed without complications using Bernard-Webster flaps. During the postoperative period, the patients reported liquid incontinence (16.67%), intermittent salivary incontinence (16.67%), and difficulty fully opening the mouth (33.33%). The tests revealed sensory deficits in the labiomental areas (100%) and lower lip (33.33%). There was full recovery of all functional changes an average of 3 months after surgery. The observed late functional results were considered satisfactory. CONCLUSIONS: The Bernard-Webster flap was an excellent alternative to repairing the defects that affect more than one third of the lower lip since it allowed the use of similar local tissues, a single surgery could be performed, and the result was functionally efficient.

RESUMO

INTRODUÇÃO Defeitos dos lábios inferiores após exéreses de carcinoma espinocelular (CEC) vêm sendo reparados com inúmeras técnicas cirúrgicas. No entanto, a reconstrução funcional de defeitos de espessura total do lábio inferior continua a ser um desafio. O objetivo deste estudo, portanto, foi descrever as estratégias cirúrgicas para a reconstrução de defeitos de espessura total do lábio inferior, subsequentes a exéreses cirúrgicas de CEC, bem como avaliar resultados funcionais. MÉTODO: Trata-se de um estudo retrospectivo de todos os pacientes com defeitos de espessura total de lábio inferior após exérese de CEC maiores que um terço do lábio, reconstruídos com retalhos de Bernard-Webster, no período de 2011 a 2014. Os resultados funcionais (funções esfincteriana, motora e sensorial) foram avaliados de acordo com critérios previamente utilizados. RESULTADOS: Seis defeitos labiais foram reconstruídos com retalhos de Bernard-Webster, sem complicações. No período pós-operatório, os pacientes relataram incontinência para líquidos (16,67%), incontinência salivar intermitente (16,67%) e dificuldade para abrir a boca completamente (33,33%). Os testes revelaram déficits sensoriais nas regiões mentoniana (100%) e de lábio inferior (33,33%). Houve recuperação completa de todas as alterações funcionais, em média, três meses após as intervenções cirúrgicas. Os resultados funcionais tardios foram considerados satisfatórios. CCONCLUSÕES: O retalho de Bernard-Webster mostrou-se uma excelente alternativa para reparar defeitos que comprometem mais de um terço do lábio inferior, pois permitiu a utilização de tecidos vizinhos semelhantes, pôde ser realizado em tempo único e foi funcionalmente eficaz.

Assessment of nasal anthropometric parameters after primary cleft lip repair using the Mohler technique

ABSTRACT

BACKGROUND: Efforts to obtain the best possible results in the nasal region of cleft patients are an ongoing process; it is therefore essential to correctly diagnose any changes in the nasal anatomy of cleft palate patients that occur over time and after surgical intervention. The aims of the present study were: (1) to evaluate the percentage of nose asymmetry in patients with either complete or incomplete unilateral cleft lip and palate who underwent primary cleft lip repair with the Mohler technique, both in the immediate postoperative period (T1) and after 1 year (T2); and (2) to compare the percentage of nasal asymmetry in both postoperative periods using the same indexes in children without cleft lip and palate. METHODS: During the period from March 2007 to December 2010, 27 patients with unilateral cleft lip and palate were submitted to primary cleft lip repair. Measurements of area, height, height at ¼ of the medial width, and height of the base of the nose were obtained from two-dimensional photos by using Adobe Photoshop CS5 Extended®. Statistical comparison of the values obtained for nasal asymmetry in the immediate postoperative period and 1 year after surgery was performed with ANOVA. RESULTS: Percentile indexes of nasal asymmetry for cleft patients showed statistical differences in all variables when compared to controls, with the exception being the base of the nose. CONCLUSIONS: The results we obtained showed an improvement in nasal symmetry, although there was a tendency to relapse from the result obtained in T1. Patients with complete cleft were more susceptible to relapse with regard to the position of the nose.

RESUMO

INTRODUÇÃO: Os esforços para obtenção de melhores resultados da região nasal de pacientes fissurados são contínuos, portanto é necessário o correto diagnóstico das alterações da anatomia nasal sofrida com o decorrer do tempo após uma intervenção cirúrgica. Os objetivos do presente estudo foram: 1) avaliar o porcentual de assimetria do nariz de pacientes portadores de fissura labiopalatina unilateral completa e incompleta submetidos a queiloplastia primária com a técnica de Mohler, no pós-operatório imediato (T1) e após um ano da cirurgia (T2); e 2) comparar o porcentual de assimetria nasal em ambos os períodos pós-operatórios com os mesmos índices obtidos em crianças não-portadoras de fissura labiopalatina. MÉTODO: No período de março de 2007 a dezembro de 2010, 27 pacientes portadores de fissura labiopalatina unilateral foram submetidos a queiloplastia primária. Foram obtidas medidas de área, altura, largura, altura em ¼ medial da largura e altura da base narinar identificados na fotografia em duas dimensões por meio do programa Adobe Photoshop CS5 Extended®. Foi utilizado o teste de ANOVA para comparação estatística entre os valores da assimetria nasal no pós-operatório imediato e um ano após a cirurgia. RESULTADOS: Foram obtidos índices porcentuais de assimetria nasal entre os pacientes fissurados e o grupo controle, demonstrando diferença estatística em todas as variáveis quando comparado ao grupo controle, exceto quanto à altura da base narinar. CONCLUSÕES: Os resultados obtidos demonstraram melhora da simetria nasal, porém tendência a recidiva do resultado obtido em T1. Os pacientes com fissura completa são mais suscetíveis à recidiva em relação à posição do nariz.

Evaluation of neurosensory disturbance in patients with craniofacial syndrome subjected to horizontal chin advancement

ABSTRACT

BACKGROUND: Although neurosensory disturbances after genioplasty have been evaluated in different studies, standardization for testing and grading of neurosensory injuries is lacking. For this reason, the incidence of neurosensory disturbance varies from 0% to 100%, depending on the definition of nerve damage, the sensitivity of the diagnostic test method, and the follow-up period. Therefore, the aim of this study was to perform an objective evaluation of the permanent neurosensory disturbances in patients who underwent horizontal chin advancement. METHODS: A retrospective study of all patients who underwent horizontal chin advancement at the Hospital SOBRAPAR between 2009 and 2010 was conducted. The objective neurosensory assessment of the lower lip and chin was performed using 2 neurological tests, namely the Semmes-Weinstein pressure and thermal sensitivity tests. Permanent neurosensory disturbance was defined as abnormal clinical test results obtained at least 12 months after surgery. RESULTS: Thirteen patients (8 with craniofacial syndrome) were evaluated. The prevalence of the patients who showed normal results for sensitivity to pressure/touch and thermal sensitivity (warm and cold; P < 0.05) was significantly high. The analysis of the data of the patients with craniofacial syndrome revealed that most of the patients had normal pressure sensitivity test results (P < 0.003). Results from the thermal sensitivity tests showed no significant difference between these patients (P = 0.317). No significant differences were observed between the anatomical regions with abnormal sensitivity test results (P > 0.05). CONCLUSIONS: Tactile sensitivities of the lower lip and chin to pressure and temperature were preserved in most of the patients 12 months after horizontal chin advancement.

RESUMO

INTRODUÇÃO: Embora os distúrbios neurossensoriais após genioplastias tenham sido avaliados em diferentes estudos, não existe uma padronização de como testar e classificar tais alterações. Por essa razão, a incidência de distúrbios neurossensoriais varia de 0 a 100%, dependendo da definição da lesão dos nervos, da sensibilidade do método diagnóstico e do período de seguimento. Portanto, o propósito deste estudo foi avaliar objetivamente o déficit neurossensorial permanente em pacientes submetidos a avanço horizontal do mento. MÉTODO: Foi realizado estudo retrospectivo de todos os pacientes submetidos a avanço horizontal do mento no Hospital SOBRAPAR, no período de 2009 a 2010. A avaliação neurossensorial objetiva do lábio inferior e do mento foi realizada com dois testes neurológicos (teste dos limiares de pressão de Semmes-Weinstein e teste de sensibilidade térmica). O déficit neurossensorial permanente foi definido como testes clínicos anormais com no mínimo 12 meses de pós-operatório. RESULTADOS: Foram avaliados 13 pacientes, sendo 8 deles portadores de síndromes craniofaciais. Houve predomínio de pacientes com os testes de sensibilidade tátil à pressão e térmica (quente e frio) normal (P < 0,05). A análise dos pacientes sindrômicos revelou que a maioria teve o teste de sensibilidade tátil à pressão normal (P < 0,003), não existindo diferenças no teste de sensibilidade térmica (P = 0,317). Não foram identificadas diferenças entre as regiões anatômicas com testes de sensibilidade anormais (P > 0,05). CONCLUSÕES: A maioria dos pacientes apresenta sensibilidade tátil (pressão e temperatura) do lábio inferior e mento preservada 12 meses após terem sido submetidos a avanço horizontal do mento.

Orbital relapse after hypertelorbitism correction

ABSTRACT

Introduction: When Tessier introduced his concepts of both an intra and extracranial approach to treat severe craniofacial malformations, like hypertelorbitism, the modern field of craniofacial surgery was ushered in. Objective: The aims of this study were the following: 1. To compare preoperative and postoperative interorbital distance in patients with hypertelorbitism who underwent hyperteleorbitism correction. 2. To evaluate the orbital relapse rate, 6 months after hyperteleorbitism correction. Methods: Eleven patients underwent hypertelorbitism correction. The interorbital distance was measured at frontal cephalograms preoperatively, and with a caliper intraoperatively and at 6 months after surgery using the frontal cephalograms. Orbital relapse was measured by subtracting the follow-up measurement from the intraoperative measurement. Wilcoxon test was used to compare preoperative interorbital distance and postoperative interorbital distance. A value of p <0.05 was considered to be statistically significant. Results: Statistically significant differences between preoperative interobital distance (31.14 mm) and postoperative interorbital distance was shown in this study (18.73 mm) (p < 0.001). The average orbital relapse rate was 1.64 mm. Conclusion: Hypertelorbitism correction allows the orbital mobilization trough an intracranial approach, and leads to a satisfactory aesthetic and functional outcomes. The average orbital relapse rate was 1.64 mm, corresponding to 12% of bone interorbital reduction.

RESUMO

Introdução: Paul Tessier propôs, na década de 60, a correção das deformidades craniofaciais por abordagem intracraniana. Objetivo: Os objetivos do presente trabalho foram: 1. Comparar a distância interorbital interna (DIO) do período pré e pós-operatório de pacientes submetidos à correção de hiperteleorbitismo; 2. Aferir o índice de recidiva da posição orbital após 6 meses da cirurgia para a correção do hiperteleorbitismo. Método: Onze pacientes foram submetidos à correção do hiperteleorbitismo. A aferição da DIO foi realizada no período pré-operatório por meio da telerradiografia cefalométrica de frente. A aferição da DIO pós-operatória foi realizada no intra-operatório com paquímetro e no seguimento pós-operatório por meio da telerradiografia cefalométrica, 6 meses após a correção do hiperteleorbitismo. O teste de Wilcoxon foi utilizado para a comparação entre DIO pré e pós-operatória. O nível de significância adotado no presente estudo foi de p<0,05. Resultados: Houve diferença estatisticamente significante entre as DIO pré-operatória (média de 32,14 mm) e pós-operatória (média de 18,73 mm) (p<0,001). O índice de recidiva médio da posição orbital foi de 1,64 mm. Conclusão: A correção do hiperteleorbitismo é uma cirurgia intracraniana que possibilita a mobilização das órbitas medialmente, proporcionando resultados estéticos e funcionais satisfatórios. O índice médio de recidiva da órbita (1,64 mm) representou 12% da extensão da redução óssea média (13,41 mm).

Anthropometric study of craniofacial asymmetry in unilateral coronal craniosynostosis

ABSTRACT

Introduction: Unilateral coronal craniosynostosis (UCC) is the unilateral premature ossification of the coronal suture, leading to a 3-dimensional deformity that may affect the skull and orbits. Objective: The purpose of this study was to measure and compare the craniofacial asymmetry of patients with UCC in the preoperative and postoperative periods to assess the effectiveness of surgical UCC correction. Methods: Nine patients (4 girls and 5 boys) underwent surgical correction of UCC between January 2007 and December 2010. Their craniofacial dimensions were measured with a goniometer to calculate the craniofacial asymmetry index (CAI) in the preoperative and postoperative periods, and the CAI averages of the periods were compared. The CAI was calculated as the ratio of the craniofacial diagonal measurements of the unaffected side to those of the affected side. Results: The mean patient age was 2 years 1 month. The average surgical time was 2 h 46 min. The average volume of transfused blood was 280 ml. The preoperative and postoperative CAI averages were 1.045 and 1.009, respectively (p=0.0109), indicating a strong tendency for symmetry after the surgery. Conclusion: Surgical UCC correction is effective for achieving craniofacial symmetry. Overcorrection of the bony structures is required to obtain bone symmetry postoperatively.

RESUMO

Introdução: A craniossinostose coronal unilateral (CCU) é a ossificação prematura da sutura coronal unilateralmente e provoca uma deformidade em 3 dimensões, que pode afetar o crânio e as órbitas. O objetivo do presente trabalho foi aferir e comparar a assimetria do crânio e face dos pacientes portadores de CCU no período pré e pós-operatório. Método: Nove pacientes foram submetidos à correção da craniossinostose coronal unilateral, entre janeiro de 2007 a dezembro de 2010. Quatro pacientes foram do sexo feminino e cinco do sexo masculino. Foram aferidas medidas da região craniofacial para quantificar o índice de assimetria craniofacial no período pré-operatório e compará-lo com o período pósoperatório. O índice de assimetria craniofacial (IAC) foi determinado pela diferença entre as medidas craniofaciais diagonais obtidas com o goniômetro. Resultados: A idade média dos pacientes submetidos à correção de CCU foi de 2 anos e 1 mês. O tempo médio de cirurgia foi de 2 horas e 46 minutos. O volume médio de sangue transfundido foi de 280 ml. A média das diferenças das medidas diagonais obtidas com o goniômetro no período pré-operatório (IAC) foi de 1,045 e do pós-operatório de 1,009 (p=0,0109), indicando forte tendência à simetria craniofacial após o ato cirúrgico. Conclusão: O tratamento proposto para as CCU foi eficiente na obtenção de simetria craniofacial. Foi necessária a sobrecorreção das estruturas ósseas para obtenção de simetria óssea no período pós-operatório.

Assessment of donor area pain in patients with cleft lip and palate undergoing alveolar bone defects repairs using iliac crest autogenous grafting: a prospective randomized comparison of two bone extractors

ABSTRACT

INTRODUCTION: Autogenous bone grafting is the standard treatment for alveolar bone defects. However, morbidity in the donor area after the bone graft has been obtained continues to be a significant problem in cleft patients. This prospective randomized study compared donor area pain associated with the use of 2 bone extractors in patients with cleft lip and palate, who underwent treatment of alveolar bone defects using a bone graft obtained from the iliac crest. METHOD: Thirty-six patients with cleft lip and palate underwent alveolar bone defect repair using a graft from the iliac crest, harvested with either a SOBRAPAR bone extractor (group A) or UCLA bone extractor (group B). Donor area pain was evaluated in the postoperative period with the aid of a unidimensional numerical pain scale (0, "no pain"; 10, "worst pain imaginable"). RESULTS: Comparison of the mean donor area pain score did not reveal any significant differences (p >0.05 for all comparisons) between the groups A and B, at any of the postoperative times evaluated. A significantly higher number of patients in group B reported no pain in the donor area, compared with group A (p <0.05). CONCLUSIONS: This study showed that a significantly greater number of patients in group B reported "no pain", compared with patients in group A; with regard to patients who reported any level of pain greater than zero, there were no between-group differences.

RESUMO

INTRODUÇÃO: Enxerto ósseo autógeno é o padrão no tratamento da falha óssea alveolar. Como a morbidade na área doadora após a obtenção de enxerto ósseo continua sendo um problema relevante em pacientes fissurados, este estudo avaliou a dor na área doadora de pacientes fissurados submetidos ao tratamento de falhas ósseas alveolares com a transferência de enxerto ósseo obtido da crista ilíaca, por meio de um estudo prospectivo randomizado, comparando dois extratores ósseos. MÉTODO: Trinta e seis pacientes com fissura labiopalatina, submetidos ao reparo da falha óssea alveolar com enxerto obtido da crista ilíaca com auxílio do extrator ósseo SOBRAPAR (grupo A) ou extrator ósseo UCLA (grupo B), foram incluídos. A dor na área doadora foi avaliada no período pós-operatório com auxílio da escala numérica unidimensional de dor (0- "sem dor"; 10- "pior dor que se pode imaginar"). RESULTADOS: As médias das mensurações da dor na área doadora não revelaram diferenças significativas (p>0,05 para todas as comparações) nas comparações realizadas entre os grupos A e B, em nenhum dos momentos pós-operatórios avaliados. Houve um maior número (p<0,05) de pacientes do grupo B que não reportaram dor na área doadora, quando comparado ao grupo A. CONCLUSÕES: Este estudo apresentou um maior número de pacientes do grupo B "sem dor", quando comparado aos pacientes do grupo A, não existindo diferenças entre aqueles que reportaram quaisquer notas diferentes de zero.

Radical undermining of the soft palate muscles in secondary cleft patients

ABSTRACT

Objective: To demonstrate the efficacy of the muscular set back in secondary cases. Method: 16 soft palate re-repair were performed with muscular set back. To evaluate the results of the surgeries through the voice, nasal endoscopies were done pre and post operative (3, 9 and 15 months). The results were evaluated by two speech pathologists specialized in cleft lip and palate patients. And the hypernasality was graded as equilibrium, mild, moderate and severe. The Kappa index was used to evaluate the agreement between the two observers and the equality of two proportion test gave the statistics significance, as p < 0.05. Results: Six patients were males. The mean age was 17.93 years. There was a good concordance between the two evaluators. The least was 76.6% according to the Kappa index. With the retropositioning of the muscles` bundle there was an improvement in the velopharingeal insufficiency (p < 0.05) mainly in the mild and moderate cases. Conclusion: There was a great improvement in velopharingeal incompetence after radical muscle dissection, showing that this procedure is necessary and must be part of secondary palatoplasty protocols. And also must be used during primary soft palate repair.

RESUMO

Objetivo: Demonstrar a eficácia do reposicionamento do músculo elevador do véu palatino em pacientes portadores de fissura lábio-palatina anteriormente submetidos a palatoplastia por outras técnicas. Método: Foram realizadas, 16 cirurgias de repalatoplastia posterior com dissecção radical da musculatura do véu palatino. A nasofibroscopia foi o instrumento de mensuração da voz no período pré e pós-operatório, 3, 9 e 15 meses após a cirurgia. Duas fonoaudiólogas com experiência no tratamento do fissurado participaram na avaliação da voz nos períodos pré e pós-operatórios, sendo a hipernasalidade classificada em equilíbrio oronasal, hipernasalidade leve, moderada e importante. O índice de Kappa foi utilizado para avaliar o grau de concordância entre os observadores. O teste de igualdade proporcional foi utilizado com o objetivo de comparar as diferenças de voz no pré e pós-operatórios de 3, 9 e 15 meses. O valor de p < 0,05 foi adotado para a significância estatística. Resultados: Seis pacientes eram do sexo masculino, com idade média de 17,93 anos. Houve uma boa concordância entre as avaliadoras, sendo a menor de 76,6% (índice de Kappa). Com o retroposicionamento da musculatura houve melhora no índice da hipernasalidade (p < 0,05), principalmente na hipernasalidade leve e moderada. Conclusão: Houve grande melhora do quadro da insuficiência velofaríngea após a dissecção radical, o que evidencia que tal procedimento é necessário e deve ser incluído no algoritmo dos protocolos em casos secundários e preconiza-se que seja usado, também, em casos primários.

ABSTRACT

Introduction: The reconstruction to correct microtias and acquired ear deformities is a defying surgery due to clinical variations and many forms of treatment. Rogers and Tanzer described the clinical classification that is the most utilized in the literature and Firmin described a surgical classification to assist the plastic surgeon in the treatment. Objective: Demonstrate that there is no strict indications between the clinical types and types of skin incision and to present the best way to use Firmin's surgical classification. Methods: 12 patients with congenital (microtias) or acquired (burn sequela) ear deformities were evaluated. The patients were categorized clinically and surgically by the senior author. All patients underwent reconstructive surgery and were followed up for one year. It was also evaluated the complication index. Results: The clinical classification was: 3 patients as small anomaly, 4 as lobular type, 3 as conchal type and 2 patients with total defect / burn sequel. The surgical classification of skin incision was: 3 patients as type II, 2 as type IIIa and 4 as type IIIb. For the framework was: 5 patients type I and 4 patients type III. The complication index was low. Conclusion: The Firmin's surgical classification for ear reconstruction demonstrated to be an excellent tool to guide the plastic surgeon in the therapeutic treatment. The Firmin's types of incision utilized in the corrective surgery have a relationship with the size and location of the auricular remanent and are independent of the clinical classification.

RESUMO

Introdução: A reconstrução para corrigir as deformidades auriculares, congênitas ou adquiridas, é uma cirurgia desafiadora devido à grande variabilidade clínica e dos tipos de tratamento. Firmin descreveu uma classificação cirúrgica com a finalidade de auxiliar o cirurgião plástico na realização do tratamento. Objetivo: Demonstrar que não existe uma regra única entre os tipos clínicos e os possíveis tipos de incisão na pele e apresentar a melhor maneira de utilizar a classificação cirúrgica de Firmin. Método: Foram avaliados 12 pacientes, todos portadores de deformidades congênitas ou adquiridas. Os pacientes foram classificados clínica e cirurgicamente pela autora sênior. Foram excluídos do estudo os pacientes submetidos à reconstrução de orelha sem a necessidade de cartilagem costal, utilizando cartilagem conchal da orelha contralateral. Todos os pacientes foram submetidos à reconstrução de orelha e acompanhados por um ano. Foi avaliado também o índice de complicações. Resultados: As classificações cirúrgicas de incisão na pele foram: 3 pacientes tipo II, 2 pacientes tipo IIIa e 4 pacientes tipo IIIb. Os tipos de maquete foram: 5 pacientes tipo I e 4 pacientes tipo III. Não houve mudanças das indicações cirúrgicas. O índice de complicações foi considerado baixo. Conclusão: A classificação cirúrgica de Firmin para reconstrução auricular demonstrou ser uma excelente ferramenta para direcionar o cirurgião plástico no planejamento terapêutico. O tipo de incisão, proposto por Firmin, a ser utilizado na correção cirúrgica tem relação com o tamanho e a localidade do remanescente auricular ou com sua ausência, e é independente da classificação clínica.

ABSTRACT

INTRODUCTION: The Parry-Romberg Syndrome (PRS) is characterized by progressive hemifacial atrophy that often leads to severe esthetic and functional difficulties. Although there are systems for grading disease severity, none have proven ideal in optimizing the therapeutic approach to these patients. This study aimed to establish the surgical strategies for the treatment of PRS based on a new system for severity grading of the disease. METHODS: This retrospective study included PRS patients undergoing surgery between 2005 and 2011. The surgical strategies were adapted for each patient according to a clinical severity grading system based on disease progression: type I, affecting the epidermis, dermis, and subcutaneous tissue; type II, type I + muscle involvement; and type III, Types I+ II + bone involvement. The sample included four patients (28.57%) with PRS type I, six patients (42.85%) with PRS type II, and four patients (28.57%) with PRS type III. RESULTS: Forty-seven procedures were performed. Free-fat grafts were used in all patients. Dermal fat grafts were used in all type II patients and one type III patient (25%). Bone grafts with temporoparietal fascia flaps were performed for the treatment of all type III patients. One type III patient (25%) underwent orthognathic surgery. All patients were improved in their overall facial appearance and there were no procedure-related complications. CONCLUSION: Our proposed system for grading PRS severity can facilitate the choice of therapeutic approaches and with a combination of surgical techniques based on the severity of the disease partially satisfactory outcomes can be attained.

RESUMO

INTRODUÇÃO: Síndrome de Parry-Romberg (SPR) é caracterizada pela atrofia hemifacial progressiva que, muitas vezes, resulta em graves distúrbios estéticos e funcionais. Embora existam escalas de gravidade, nenhuma delas é completamente ideal para auxiliar na abordagem terapêutica destes pacientes. O objetivo deste estudo foi delinear as estratégias cirúrgicas para o tratamento da SPR baseado em um novo sistema de classificação de gravidade da doença. MÉTODO: Trata-se de uma análise retrospectiva dos pacientes com SPR operados em 2005-2011. As abordagens cirúrgicas foram individualizadas de acordo com a escala de gravidade clínica baseada na evolução da doença: tipos I (envolvimento da epiderme, derme e tecido subcutâneo); II (tipo I + envolvimento muscular); e III (tipo I + II + envolvimento ósseo). Quatro (28,57%) pacientes com SPR tipo I, 6 (42,85%) tipo II e 4 (28,57%) tipo III foram incluídos. RESULTADO: Um total de 47 procedimentos foi realizado. Gordura livre foi enxertada em todos os pacientes. Todos os pacientes do tipo II e 1 (25%) do tipo III foram submetidos a enxertos dermogordurosos. Enxertos ósseos com retalhos de fáscia têmporo-parietal foram aplicados no tratamento de todos os pacientes do tipo III. Um (25%) paciente do tipo III foi submetido à cirurgia ortognática. Houve melhora global na aparência facial em todos os pacientes, sem complicações relacionadas aos procedimentos. CONCLUSÃO: O sistema de classificação de gravidade proposto para a SPR pode facilitar a decisão terapêutica e resultados parcialmente satisfatórios podem ser alcançados com a combinação de técnicas cirúrgicas de acordo com a gravidade da doença.

Formal training in scientific research increases the participation of plastic surgery residents in peer-reviewed articles

ABSTRACT

Introduction: The objectives of this study were as follows: (1) to outline a scientific research skills training program, (2) to evaluate the profile of participation of plastic surgery residents in articles, and (3) to analyze the impact of the implementation of the training program on quantitative bibliometric indexes.
Methods: This was a bibliometric analysis of the participation of plastic surgery residents of a single institution in articles published in peer-reviewed journals between 2006 and 2014. The data collected were the number of authors, position of residents among authors, article titles, indexing databases and impact factor of the journals, study design, and levels of evidence. Two periods (January 2006 to January 2010 [A] and February 2010 to February 2014 [B]) were created to study the evolutionary profile of the impact of the implementation of the training program outlined in this study.
Results: A significant predominance (p < 0.05) was observed among articles published in national journals in the Portuguese language and in the SciELO and LILACS databases, and articles without residents as corresponding author, without impact factor, without assumptions, and with a level of evidence III (retrospective studies). The inter-period comparative analysis revealed a significant increase (p < 0.05) in the numbers of published articles and residents with publications at the end of their residency, in the involvement of one or more residents, and in the articles published in English (period A < period B).
Conclusion: The implementation of a scientific research skills training program led to an increase in research activity of (peer-reviewed articles) during the residency.

RESUMO

Introdução: Os objetivos deste estudo foram: (1) delinear um programa de treinamento em habilidades de pesquisa científica, (2) avaliar o perfil da participação dos residentes de Cirurgia Plástica em artigos, e (3) analisar o impacto da implementação do programa de treinamento sobre índices bibliométricos quantitativos.
Métodos: Trata-se de uma análise bibliométrica da participação de residentes de Cirurgia Plástica de uma única instituição em artigos publicados em periódicos revisados por pares entre 2006 e 2014. Dados coletados: número de autores, posição dos residentes entre os autores, títulos, bases de indexação e fator de impacto dos periódicos, desenhos dos estudos e níveis de evidência. Dois períodos (janeiro/2006-janeiro/2010 [A] e fevereiro/2010-fevereiro/2014 [B]) foram criados para estudar o perfil evolutivo do impacto da implementação do programa de treinamento delineado neste estudo.
Resultados: Houve predomínio significativo (p < 0,05) de artigos publicados em periódicos nacionais, em língua portuguesa, nas bases de dados SciELO e LILACS, artigos sem residentes como autor correspondente, sem fator de impacto, sem hipóteses e com nível de evidência III (estudos retrospectivos). A análise comparativa interperíodos revelou um aumento significativo (p < 0,05) de artigos publicados, de residentes com publicações ao término da residência, da participação de um ou mais residentes e de artigos publicados em inglês (período A < período B).
Conclusão: A implementação do programa de treinamento em habilidades de pesquisa científica determinou um aumento da atividade de pesquisa (artigos revisados por pares) durante a residência.

Radical versus conservative surgery for craniofacial fibrous dysplasia: stratification of surgical strategies

ABSTRACT

BACKGROUND: To date, there is no consensus regarding the best surgical approach (conservative or radical) for craniofacial fibrous dysplasia. This study presented the experience of a single institution in the surgical treatment of craniofacial fibrous dysplasia. METHOD: This was a retrospective analysis of patients with craniofacial fibrous dysplasia who underwent surgery between 1997 and 2012. Surgical treatment was individualized according to patient age, the involved anatomical site (zones I-IV), aesthetic and/or functional impairment, and the preferences of the patients and surgical team. The surgical results were classified on the basis of the Whitaker system. RESULTS: Ten, 1, 1, and 1 patients with zone I, zone II, zone I/II, and zone I/IV involvement, respectively, were included in the study. In total, conservative surgeries and 9 radical surgeries were performed for the treatment of primary bone tumors. There was 1 surgical complication, and 6 recurrences were identified during the postoperative follow-up period. The global average of surgical outcomes, according to the Whitaker scale, was 1.69 ± 0.94. CONCLUSIONS: According to the experience and surgical results presented in this study, the surgical approach for craniofacial fibrous dysplasia should be individualized.

RESUMO

INTRODUÇÃO: Até o momento, não existe consenso sobre qual a melhor abordagem cirúrgica (conservadora ou radical) da displasia fibrosa craniofacial. O objetivo deste estudo foi apresentar a experiência de uma única instituição no tratamento cirúrgico da displasia fibrosa craniofacial. MÉTODO: Trata-se de uma análise retrospectiva dos pacientes com displasia fibrosa craniofacial, operados entre 1997 e 2012. O tratamento cirúrgico foi individualizado de acordo com idade, sítio anatômico envolvido (zonas I-IV), comprometimento estético e/ou funcional e preferências dos pacientes e da equipe cirúrgica. Os resultados cirúrgicos foram classificados com base no sistema de Whitaker. RESULTADOS: Dez pacientes com acometimento da zona I, um da zona II, um das zonas I e III, e um das zonas I e IV foram incluídos. Nove cirurgias conservadoras e nove cirurgias radicais foram realizadas para o tratamento de tumores ósseos primários. Houve uma complicação cirúrgica. Seis recidivas foram identificadas durante o seguimento pós-operatório. A média global dos resultados cirúrgicos, de acordo com a escala de Whitaker, foi de 1,69 ± 0,94. CONCLUSÕES: De acordo com a experiência e resultados cirúrgicos apresentados neste estudo, a abordagem cirúrgica da displasia fibrosa craniofacial deve ser individualizada.

Bilateral buccinator myomucosal flap for the treatment of velopharyngeal insufficiency: preliminary results

ABSTRACT

BACKGROUND: Velopharyngeal insufficiency in patients with cleft lip and palate is challenging for plastic surgeons. One of the treatment options is palatoplasty using buccinator muscle myomucosal flaps to extend the nasal and oral mucosa. This prospective study presents the preliminary results of the use of buccinator muscle myomucosal flaps for the treatment of velopharyngeal insufficiency in patients with cleft lip and palate. METHODS: This study was performed between January 2010 and July 2012 at the Craniofacial Institute of the Hospital of SOBRAPAR. All patients with cleft lip and palate who had undergone nasofibroscopy and had severe velopharyngeal insufficiency with scar tissue accumulation in the oral cavity as observed by oroscopy, previous palatoplasty with total dissection of the soft palate, or were referred by other services were submitted to a secondary or in some cases, tertiary palatoplasty using a bilateral buccinator myomucosal flap to extend the soft palate. RESULTS: Among 20 patients undergoing treatment, 4 (20%) were excluded from the analysis, 1 went from a very large to a large circular gap (5%), 6 went from a large circular gap to no gap (5%), 2 went from a medium to a small coronal gap (10%), and 3 exhibited no postoperative improvement (15%). CONCLUSIONS: Although longer postoperative follow-up is required, the results indicate THAT buccinator muscle flaps are reliable, reproducible, and lead to good final results.

RESUMO

INTRODUÇÃO: A insuficiência velofaríngea em pacientes com fissura labiopalatal é um desafio para o cirurgião plástico e pode ser resolvida, entre outras maneiras, pela palatoplastia, com a utilização de retalhos miomucosos do músculo bucinador para alongar as mucosas nasal e oral. Este estudo prospectivo teve por objetivo fazer a avaliação preliminar do uso de retalho miomucoso do músculo bucinador bilateral para o tratamento da insuficiência velofaríngea em pacientes com fissura labiopalatal. MÉTODO: Entre janeiro de 2010 e julho de 2012, todos os pacientes do Hospital SOBRAPAR - Crânio e Face com fissura labiopalatal submetidos a nasofibroscopia e que apresentavam insuficiência velofaríngea grave, grande quantidade de tecido cicatricial na oroscopia, que já haviam sido submetidos a repalatoplastias com dissecção radical da musculatura do véu e/ou encaminhados de outros serviços foram submetidos a palatoplastia secundária, por vezes terciária, com retalho miomucoso bucinador bilateral para alongamento do véu palatino. RESULTADOS: Dos 20 pacientes submetidos à técnica proposta, 4 pacientes foram excluídos do estudo (20%), 1 paciente passou de um gap circular muito grande para grande (5%), 6 passaram de circular grande para médio (30%), 2 de circular grande para pequeno (10%), 1 de circular grande para puntiforme (5%), 1 de circular grande para ausente (5%), 2 de coronal médio para coronal pequeno (10%), e 3 mantiveram o gap circular grande no pós-operatório (15%). CONCLUSÕES: Embora o presente estudo necessite de maior tempo de seguimento pós-operatório, os retalhos do músculo bucinador são confiáveis e de fácil reprodução, obtendo-se bons resultados.
