Tactic to avoid complications in augmentation mastoplasty via submammary groove

ABSTRACT

The techniques for augmentation mammoplasty presented over the course of time numerous adaptations that ensure refinements and better results. In addition to the technique already described by Graf, in 1998, we report here a tactic of fascial flap closure in the inframammary groove. In order to avoid complications such as the dreaded operative wound dehiscence and early mammary ptosis, the closure strategy described guarantees a natural form of silicone implant protection. The technique of fascial flap in the region of the submammary sulcus, already a routine in our service, is shown to be effective in implant protection.

RESUMO

As técnicas para mamoplastia de aumento apresentaram ao longo do tempo inúmeras adaptações que garantiram refinamentos e melhores resultados. Como adição à técnica já descrita por Graf, em 1998, relatamos aqui uma tática de fechamento de retalho fascial em sulco inframamário. A fim de evitar complicações, como a temida deiscência de ferida operatória e ptose mamária precoce, a estratégia de fechamento descrito garante uma forma natural de proteção ao implante de silicone. De rápida e fácil execução, a técnica do retalho fascial em região de sulco submamário, já rotina em nosso serviço, mostra ser eficaz na proteção do implante.

Evaluation of the periorbital senile deformities and review of the literature on current proposals for the treatment of these alterations

ABSTRACT

Introduction: Aging of the orbital region results from a combination of a dynamic process that involves bone resorption and soft tissue changes. Objective: Evaluate periorbital senile deformities.
Method: Digital photographs were analyzed using ImageJ software to compare elderly patients with young patients. The measurements of eyebrow curvature, height-width ratio of the palpebral cleft, eyelid-malar distance and relative position between the lateral and the medial corners of the eye were evaluated.
Results: Only the lateral corner of the eye presented a statistically significant decline in the older age group in relation to the younger.
Conclusions: In order to achieve the most natural and harmonious rejuvenation of the orbital region, senile deformities must be identified objectively and corrected according to the origin of each one.

RESUMO

Introdução: O envelhecimento da região orbitária resulta de uma combinação de um processo dinâmico que envolve reabsorção óssea e alterações de partes moles. Objetivo: Avaliar as deformidades senis periorbitárias.
Método: Foram analisadas fotografias digitais utilizando-se o software ImageJ para comparar pacientes com idade avançada com pacientes jovens. Avaliaram-se as medidas da curvatura da sobrancelha, relação altura-largura da fenda palpebral, distância pálpebro-malar e posição relativa entre os cantos lateral e medial do olho.
Resultados: Apenas o canto lateral do olho apresentou declínio estatisticamente significativo no grupo e idade mais avançada em relação aos jovens.
Conclusão: Com o intuito de alcançar o rejuvenescimento mais natural e harmonioso da região orbitária, as deformidades senis devem ser identificadas de forma objetiva e corrigidas de acordo com a origem de cada uma delas.

Dermolipectomies after bariatric procedures in the Public Health System: a long way to go

ABSTRACT

Introduction: Obesity is one of the main health problems faced by the population, and its incidence has gradually increased in recent decades. Amid the global obesity epidemic, bariatric procedures have increased significantly and, despite the growing number of post-bariatric procedures, these are not equivalent to the number of bariatric surgeries.
Methods: Data were collected from the public health registry (DATASUS) between 2008 and 2019 to analyze the selected parameters, with an assessment of the main post-bariatric dermolipectomy techniques, their distribution in the national territory, their length of stay, their mortality, costs for the Public System, the comparison between post-bariatric dermolipectomies and the distribution of bariatric procedures in the national territory. Furthermore, dermolipectomies and the distribution of plastic surgeons in Brazil were compared.
Results: An increase of 164% was evidenced in the number of postbariatric dermolipectomies during the studied period. Post-bariatric abdominal dermolipectomy was the most performed procedure, accounting for 65% of the procedures, followed by brachial (14.8%), crural (14.7%) and circumferential (4.7%) dermolipectomy. There was an inequality in the distribution of post-bariatric procedures among Brazilian macro-regions, with the Southeast Region having the highest percentage (49.8%) of dermolipectomies.
Conclusions: Despite the progressive increase in post-bariatric dermolipectomies, they did not follow the number of bariatric procedures in the national territory. Therefore, there is a need for a parallel growth between both so that there is complementation in treating these patients. Then, there might be an improvement in the distribution of dermolipectomies in the national territory, allowing more patients to benefit.

Introdução: A obesidade é um dos principais problemas de saúde enfrentados pela população e sua incidência cresce gradativamente nas últimas décadas. Em meio à epidemia global de obesidade, os procedimentos bariátricos aumentaram expressivamente e, apesar do crescente número dos procedimentos pós-bariátricos, esses não se equivalem ao número de cirurgias bariátricas.
Métodos: Foram coletados dados do registro de saúde pública (DATASUS) entre 2008 e 2019 para análise dos parâmetros selecionados, com avaliação das principais técnicas de dermolipectomia pós-bariátrica, sua distribuição em território nacional, seu tempo de internação, sua mortalidade, os custos para o Sistema Público, a comparação entre as dermolipectomias pós-bariátricas e a distribuição dos procedimentos bariátricos no território nacional. Além disso, comparou-se as dermolipectomias e a distribuição de cirurgiões plásticos no Brasil.
Resultados: Um aumento de 164% foi evidenciado no número de dermolipectomias pós-bariátricas durante o período estudado. A dermolipectomia abdominal pós-bariátrica foi o procedimento mais realizado, sendo responsável por 65% dos procedimentos, seguido da dermolipectomia braquial (14,8%), crural (14,7%) e circunferencial (4,7%). Observou-se uma desigualdade na distribuição dos procedimentos pós-bariátricos entre as macrorregiões brasileiras, sendo a Região Sudeste com o maior número percentual (49,8%) de dermolipectomias.
Conclusões: Apesar do aumento progressivo do número de dermolipectomias pós-bariátricas, elas não acompanharam o número de procedimentos bariátricos em território nacional. Por isso, há necessidade de um crescimento paralelo entre ambas, para que haja uma complementação no tratamento desses pacientes. Sendo assim, poderá existir melhora na distribuição das dermolipectomias no território nacional, fazendo com que mais pacientes possam ser beneficiados.

Lipoaspiração abdominal: Recomendações técnicas para maior segurança

ABSTRACT

Liposuction, described in 1979, was the largest recent plastic surgery finding, and the most common surgical procedure in this specialty. Safety criteria of this procedure have been previously described, however, we conducted a literature review and a specific technique description to increase the safety of liposuction in abdominal region, in order to inform specialists and medical class about the main technical safety maneuvers during the procedure in this area.

RESUMO

A Lipoaspiração, descrita em 1979, foi a maior descoberta recente da cirurgia plástica, sendo o procedimento cirúrgico mais realizado nesta especialidade. Os critérios de segurança deste procedimento em geral já foram anteriormente descritos, contudo realizamos uma revisão da literatura e descrição de técnica específica para aumento da segurança da lipoaspiração da região abdominal, com o objetivo de informar os especialistas e a classe médica sobre as principais manobras técnicas de segurança durante o procedimento nesta área.

Osteogenic distraction for late treatment of plagiocephaly

ABSTRACT

Introduction: Plagiocephaly refers to an asymmetrical coronal plane of the skull. The surgical approach should be performed early. Objective: To propose a new approach to osteogenic distraction as a technique to correct plagiocephaly.
Method: Between February 2005 and February 2019, three patients were treated with the osteogenic distraction technique.
Results: The mean age of the procedure was 6.7 years. The mean distraction time was 120 days. Discussion: The bone distractor for repositioning the skull cap promotes the simultaneous movement of the middle facial third and frontal region, in monoblock. This process is based on the formation of new bone tissue between two segments through controlled traction. Its use is more recommended in children with at least 6 months of life, due to bone malleability.
Conclusions: The technique of osteogenic distraction in a patient with more advanced age has shown to be a safe technique with good functional esthetic results.

RESUMO

Introdução: A plagiocefalia refere-se a um plano coronal assimétrico do crânio. A abordagem cirúrgica, deve ser realizada precocemente. Objetivos: Propor uma nova abordagem da distração osteogênica como técnica para correção da plagiocefalia.
Método: Entre fevereiro de 2005 e fevereiro de 2019, foram tratados três pacientes com a técnica de distração osteogênica.
Resultados: A idade média da realização do procedimento foi de 6,7 anos. O tempo médio de distração foi de 120 dias. Discussão: O distrator ósseo para reposicionamento da calota craniana promove o movimento simultâneo do terço médio facial e região frontal, em monobloco. Esse processo baseia-se na formação de tecido ósseo novo entre dois segmentos por meio de tração controlada. Seu uso é mais recomendado em crianças com pelo menos 6 meses de vida, pela maleabilidade óssea.
Conclusão: A técnica de distracão osteogênica em paciente com idade mais avançada, mostrou-se uma técnica segura e com bons resultados estéticos funcionais.

Treatment of nasoethmoidal fractures with fixation with Kirschner wire

ABSTRACT

Introduction: Considering lesions of high incidence, nasal fractures, recurrently taken as minor, can bring about significant functional and aesthetic damage.
Method: Between October 2012 and October 2018, 11 patients were attended at the Hospital do Trabalhador, in Curitiba-PR, undergoing fixation of a nasal septum fracture with Kirschner wire.
Results: Of the total of 11 patients, eight male and three female, with a mean age of 24.4 years, 63% of them were victims of facial aggression. The interval between trauma and treatment ranged from 13 to 21 days. In 72% only one Kirschner wire was used for fixation and, finally, the material removal time was, on average, 39.9 days.
Conclusions: This treatment option is characterized as a simple and rapid surgical strategy, with low cost and morbidity and a satisfactory postoperative result.

Introdução: Consideradas lesões de elevada incidência, as fraturas nasais, recorrentemente consideradas de menor importância, podem trazer prejuízos relevantes do ponto de vista funcional e estético.
Método: Entre outubro de 2012 e outubro 2018 foram atendidos 11 pacientes no Hospital do Trabalhador, em Curitiba-PR, submetidos a fixação de fratura de septo nasal com fio de Kirschner.
Resultados: Do total de 11 pacientes, oito masculinos e três femininos, a idade média permanecendo em 24,4 anos, 63% deles foram vítimas de agressão em face. O intervalo entre o trauma e o tratamento variou entre 13 a 21 dias. Em 72% foram usados apenas um fio de Kirschner para fixação e, por fim, o tempo para retirada do material foi, em média, 39,9 dias.
Conclusão: Essa opção de tratamento se caracteriza como uma estratégia cirúrgica de execução simples e rápida, com baixo custo e morbidade e resultado satisfatório no pós-operatório.
