ABSTRACT

The pain after peeling limits the universalization of this aesthetic procedure. Many patients with indication for such treatment deny the use of this therapy, fearing the pain immediately after the peeling. The effective control of the pain could became the a procedure safe, simple, cheap and easily reproductable more diffused among patients and professionals, especially by a topical therapy. The evaluation of the pain after the use of sodium aqueous heparin solution in spray (ALIMAX®), utilized for topical treatment in burns, and its comparison to the distilled water, utilized by the same way, was studied in 12 patients after the medium-depth peeling with trichloroacetic acid (TCA) 30% on face.

RESUMO

A dor pós-peeling é um fator limitante ao seu uso universal. Alguns pacientes com indicação para tal tratamento estético acabam se abstendo desta terapia devido ao temor da dor imediatamente após este procedimento. O que poderia tornar um procedimento seguro, simples, barato e de fácil reprodução, mais difundido entre pacientes e profissionais, seria o controle efetivo desta dor, de preferência se fosse por meio de uma terapia tópica. A avaliação da dor após o uso de heparina sódica tópica em spray (ALIMAX®), utilizada para tratamento tópico em queimaduras, e sua comparação à água destilada, utilizada pela mesma via, foi estudada em 12 pacientes após o peeling médio com ácido tricloroacético (ATA) a 30% em face.

Results of keloid treatment with surgery and imiquimod 5% cream: a prospective study

ABSTRACT

Introduction: Keloid treatment represents a therapeutic challenge, often with disappointing results. More effective adjuvant therapies are necessary to reduce the high rates of recurrence. This research is aimed at assessing the treatment and recurrence of keloid scars after the use of a combined treatment modality comprising the application of imiquimod 5% cream and surgery. Methods: A prospective study that assessed 14 patients with a total of 25 keloids, with a minimum follow-up of 6 months, was carried out. Intralesional excision was performed, and the application of imiquimod 5% cream was initiated on the day of surgery and was continued for two months. Scar assessment was carried out by means of an assessment scale (patient and an observer - Head of the Plastic Surgery Service) and by observing signs of recurrence and scar size during the pre- and postoperative periods. Results: At the 6-month follow-up, there was an improvement of the scar as rated by patients in 72% of the cases and as rated by the observer in 88% of the cases, with a recurrence rate of 8% and significant scar area reduction (p=0.0053). Conclusion: The use of imiquimod 5% cream after keloid resection surgery significantly reduced recurrence in the 6-month postoperative assessment.

RESUMO

Introdução: O tratamento do queloide representa um desafio terapêutico, com resultados muitas vezes frustrantes. Terapias adjuvantes mais eficazes são necessárias para reduzir as altas taxas de recidivas. Essa pesquisa é baseada em uma modalidade conjunta com a aplicação de imiquimode 5% creme associada à cirurgia, tendo como objetivo a avaliação do tratamento e recidiva da cicatriz queloidiana. Método: Estudo prospectivo que avaliou 14 pacientes em um total de 25 queloides, com um seguimento mínimo de 6 meses. Foi realizada excisão intralesional e iniciada a aplicação do imiquimode 5% creme no dia da cirurgia e mantido por dois meses. A avaliação da cicatriz foi analisada por meio de uma Escala de Avaliação (paciente e um observador - regente de serviço de Cirurgia Plástica), sinais de recidiva e também do tamanho da cicatriz no pré e pós-operatório. Resultados: Após o acompanhamento de 6 meses, houve melhora da cicatriz por parte do paciente em 72% e para o observador em 88%, tendo uma taxa de recidiva de 8% e redução da área cicatricial (p=0,0053). Conclusão: O uso do Imiquimode 5% creme no pós-operatório da cirurgia de ressecção do queloide diminuiu significativamente sua recidiva na avaliação de 6 meses.

Evaluation of the use of polimetilmetacrilato for facial lipodystrophya correction related to the antiretroviral therapy in HIV positive patients

ABSTRACT

Introduction: The lipodystrophy observed in positive HIV patients, resultant of the use of antiretrovirals leads to the reduction or loss of self-esteem, social exclusion and difficulties in the familiar and social relationship. These psychological riots contribute for the difficulty of adhesion or even the abandonment to the treatment. In 2004, a portray of "Health Department" included, in the table of the "Only Service of Health", eight procedures for the treatment of positive HIV patients who present lipodistrophy, in between them, the face fulfilling with polimetilmetacrilato (PMMA). Method: The present study observed the results of the procedure for 12 months in 46 patients. The average of age of the group was 35 years of old with predominance of the masculine sex in 56% of the cases. The submitted regions to the treatment had been the nasojugal (100%), the temporal (83%) and the parotid (23%). The injected average volume in the face of each patient was of 12.5 ml (varying between 4 and 30 ml). For 69% it did not have necessity of complementary procedures. However, 27% of the patients had been submitted through 2 fulfillings and 4% to 3 fulfillings, being most frequent the nasojugal region (86%). Complementaries fulfillings were done, at least, 3 months after the last procedure. Results: The time of treatment with antiretrovirals did not show an evident relation with the injected volume. The fulfilling procedure had no problem itself in the great majority of the time, wether in intra or postoperative , for the medical team or to the patients. Conclusion: The achieved levels of satisfaction confirm the expectations of the Health Department into the rescue of the self-esteem and the social reinclusion of these patients.

RESUMO

Introdução: A lipodistrofia observada nos pacientes HIV positivos, resultante do uso de anti-retrovirais, leva à diminuição ou perda da auto-estima, exclusão social e dificuldades no relacionamento familiar e social. Estes distúrbios psicológicos contribuem para a dificuldade de adesão ou até o abandono ao tratamento. Em 2004, uma Portaria do Ministério da Saúde incluiu, na tabela do Serviço Único de Saúde, oito procedimentos para o tratamento de pacientes HIV positivos que apresentam lipodistrofias, entre eles o preenchimento facial com polimetilmetacrilato (PMMA). Método: O presente estudo observou os resultados do procedimento por 12 meses em 46 pacientes. A média de idade do grupo foi de 35 anos, com predominância do sexo masculino em 56% dos casos. As regiões submetidas a tratamento foram a nasojugal (100%), a temporal (83%) e a parotídea (23%). O volume médio injetado na face de cada paciente foi de 12,5 ml (variando entre 4 e 30 ml). Para 69% não houve necessidade de procedimentos complementares. Porém, 27% dos pacientes realizaram 2 preenchimentos e 4% foram submetidos a 3 preenchimentos, sendo a região nasojugal a mais freqüente (86%). Os preenchimentos complementares eram feitos, no mínimo, 3 meses após o último procedimento. Resultados: O tempo de tratamento com anti-retrovirais não mostrou uma relação evidente com o volume infundido. O procedimento de preenchimento transcorreu, na grande maioria das vezes, sem nenhuma intercorrência intra e pós-operatória, tanto para a equipe médica quanto para os pacientes. Conclusão: Os níveis de satisfação obtidos confirmam as expectativas do Ministério da Saúde quanto ao resgate da auto-estima e a reinclusão social destes pacientes.

Histopathologic study of the use of topical heparin in scalded rats burns

ABSTRACT

Background: Burn studies and non-burn studies using parenteral heparin uncovered heparin effects which were antiinflammatory, neoangiogenic, epithelializing, collagen-regulating, and, in addition to anticoagulating. Use of heparin in burned patients, relieved pain, limited inflammation, resulted in a better evolution and skin results. This present study intend to evaluate these properties through seriated histopathologic analisys of skin treated with topical heparin only. Method: 20 rats with 2nd degree burn scalded by water at 99ºC in the back. Divided into two groups: 1) treated with topical heparin; and 2) treated with 0.9 saline solution. It was utilized diary one topical throw of heparin spray, that have 0.14ml or 1,400 UI. The rats were sacrificed through 2-3 days, and the burned skin sent to histopathologic study. The cross sections were eveluated by a pathologist that did not know which one was from group 1 ou group 2. Results: Heparin group revealed less acute (neutrophil count) and chronical (lymphocyte count) inflamatory process, more intense angiogenesis and less fibroblastic proliferation (measured the thickness from the dermal-epidermal junction) in comparison to the control group with 0.9 saline solution. Conclusion: The use of topical heparin alone improved the treatment of partial-thickness burns, resulted in less inflammatory process (acute and chronic), more neoangiogenesis, and less thick fibroblastic proliferation compared to similar burns treated with saline.

RESUMO

Introdução: Muitos estudos com uso da heparina parenteral, entre eles em queimados, demonstraram seus efeitos antiinflamatórios, angiogênico, estimulante de epitelização, modulador de colágeno, sem contar o anticoagulante. Nos queimados, aliviou a dor, limitou a inflamação, melhorando a evolução e o resultado da pele pós-queimadura. Este trabalho visa avaliar estas propriedades por meio de análise histopatológica seriada da pele tratada com heparina apenas por via tópica. Método: Foram avaliados 20 ratos submetidos à queimadura de 2º grau por escaldo com água aquecida a 99ºC no dorso. Divididos em dois grupos: 1) 10 tratados com heparina; e 2) 10 tratados com SF 0,9%. Foi utilizado um jato tópico diário de heparina sódica spray, que apresenta 0,14 ml ou 1.400 UI. Os animais foram sacrificados a cada 2-3 dias e enviada peça da área queimada para estudo histopatológico. As lâminas foram avaliadas por um patologista que desconhecia a qual grupo (1 ou 2) pertencia o corte. Resultados: O grupo 1 (com heparina) apresentou menor processo inflamatório agudo (contagem de neutrófilos) e crônico (contagem de linfócitos), angiogênese mais intensa e menor proliferação fibroblástica (medida espessura a partir da transição derme-epiderme) em relação ao grupo controle com SF 0,9%. Conclusão: A heparina sódica, em seu uso tópico isolado, melhorou o tratamento das queimaduras de acometimento parcial da pele, uma vez que diminuiu o processo inflamatório e a proliferação fibroblástica, com melhor resultado final.

Ethyl-2-cyanoacrylate (Epiglu®) evaluation in the skin closure: study in rats

ABSTRACT

Background: The Ethyl-2-cyanoacrylate (ECA) is a synthetic tissue adhesive used by some physicians in clinical practice in wound closures. They polymerize rapidly to adhere tightly to proteinaceous surfaces, form a strong and flexible bond. This study was performed to evaluate the efficacy and biocompatibility of this polymer (Epiglu®, of Meyer-Haake company, Germany) in skin wound closures, in rat model. Methods: Ten male Wistar rats were subjected to three incisions which were closed using different treatments. The central incision was treated witch Simple suture of nylon 4.0 (control group). Left incision was treated with sub-dermis suture associated with ECA, and right incision only with ECA. Rats were sacrificed on days 3, 7, 14, 21 and 30 after the intervention. The biocompatibility was evaluated due to direct observation: necrosis, inflammation, dermatitis, infection and dehiscence; and microscopy: inflammatory process, angiogenesis, number of fibroblasts, collagen, giants cells and eosynophils. Results: The wound closured only with ECA presented a higher dehiscence level (p < 0.05). It was not observed another macro or microscopic significant difference comparing to the control group (p > 0.05). Conclusion: The use of the ECA is a good option to the skin closure, with good biocompatibility. Special attention must receive the wounds with some tension, where the association with a sub-dermis suture becomes imperative.

RESUMO

Introdução: O Etil-2-cianoacrilato (ECA) é um adesivo tecidual utilizado por alguns médicos na prática clínica para fechamento de lesões de pele. São substâncias que se autopolimerizam quando em contato com a ferida, formando uma cola forte e flexível. Este trabalho foi realizado visando à avaliação da eficácia e da biocompatibilidade deste polímero (Epiglu®, da empresa Meyer- Haake, Alemanha) na síntese de pele, em modelo experimental. Método: Dez ratos Wistar machos foram submetidos a três incisões longitudinais no dorso. A incisão central foi tratada com sutura de pontos simples de nylon 4.0 (grupo controle), a esquerda com sutura subdérmica de nylon 4.0 associado ao ECA, e a direita apenas o ECA. Os animais foram sacrificados nos dias 3, 7, 14, 21 e 30 pós-operatórios. A biocompatibilidade foi avaliada sob observação direta: necrose, inflamação, dermatite de contato, infecção e deiscência; e à microscopia: reação inflamatória, angiogênese, fibroblastos, colágeno, células gigantes e eosinófilos. Resultados: As sínteses realizadas apenas com o ECA apresentaram maior índice de deiscência (p < 0,05). Todos os outros critérios macro e microscópicos avaliados não apresentaram diferenças estatísticas em relação ao controle (p > 0,05). Conclusão: O uso do ECA é uma boa opção de síntese da pele, com boa biocompatibilidade. Atenção deve ser feita às lesões que apresentem algum grau de tensão, onde a associação com a sutura subdérmica torna-se obrigatória.

Burns immediate dressing with water, oil of melaleuca and emulsificants

ABSTRACT

Introduction: Burnshield® (United Medical, Brazil) is an option of immediate dressing for burns aimed at hydrating and cooling the wound. Purpose: Investigate, histopathologically, the efficacy of Burnshield® as initial treatment for thermal burns. Method: Blind controlled experimental study, with 36 Wistar rats divided into 3 groups of 12 rats: (A) treated with Burnshield®, (B) treated with running water for 15 minutes and (C) not treated. Half of the animals of each group were sacrificed 30 minutes after the burn and the other half, within 24 of evolution. The assessment criterion was the extension of lesion found in each animal. Results: Group C showed the deepest burns compared to the others, within 30 minutes and 24 hours. There was no difference between groups A and B within 30 minutes; however, group A showed better and significant results within 24 hours of evolution. Conclusion: Burnshield® showed efficacy comparable to running water at the initial treatment of burns, with better results after 24 hours of evolution. Its main advantage is its practicality, once it can be used while the patient is carried up to the center of specialized treatment.

RESUMO

Introdução: O Burnshield® (United Medical, Brasil) é uma opção de curativo imediato para queimaduras que visa à hidratação e ao resfriamento da ferida, sendo objetivo deste trabalho avaliar sua eficácia. Método: Estudo experimental controlado cego, com 36 ratos Wistar divididos em 3 grupos de 12: (A) tratados com Burnshield®, (B) tratados com água corrente por 15 minutos e (C) não tratados. Metade dos animais de cada grupo foi sacrificada com 30 minutos após queimadura, e a outra metade com 24 horas de evolução. O critério de avaliação foi a maior profundidade de lesão encontrada em cada animal. Resultados: O grupo C apresentou as queimaduras mais profundas em relação aos demais, com 30 minutos e 24 horas. Não houve diferença entre os grupos A e B com 30 minutos, porém houve melhor resultado significativo no grupo A nas 24 horas de evolução. Conclusão: O Burnshield® mostrou eficácia comparável à água corrente no tratamento inicial de queimaduras, com melhores resultados depois de 24 horas de evolução. Apresenta como maior vantagem a praticidade, podendo ser usado enquanto o paciente é deslocado até o centro de tratamento especializado.

Post-traumatic reconstructions orbitals with utilization of the osteofascials flaps of the temporoparietal fascia and bone graft

ABSTRACT

Purpose: The purpose of the present paper is to present the reconstructive method of posttraumatic orbital abnormalities with osteofascial flaps and bone grafts as an excelent reconstructive method and one more therapeutic option for selected patients. Cases report: The present paper reported the authors experience with the application of the temporoparietal osteofascial flaps or autogenous skull bone graft in the reconstruction of the traumatic orbital abnormalities in three consecutive cases. Conclusions: It was found favorable results with low complications rates. The supremacy of temporoparietal osteofascial flaps and autogenous skull graft in facial reconstructions of selected pacients is confirmed.

RESUMO

Objetivo: O objetivo do presente trabalho é apresentar os métodos reconstrutivos de anormalidades pós-traumáticas orbitárias, como enxerto ósseo autólogo e retalho osteofascial de fáscia temporoparietal, como alguns dos bons métodos reconstrutivos e como opção terapêutica para esses pacientes. Relato de casos: No presente artigo, relatouse a experiência do uso dos retalhos osteofasciais de fáscia temporoparietal e enxerto ósseo autólogo de calvário na reconstrução das anormalidades orbitárias pós-traumáticas em três casos consecutivos. Conclusão: Com a obtenção de resultados favoráveis e baixa taxa de complicações no presente trabalho, confirmamos os dados da literatura quanto à supremacia na utilização dos retalhos osteofasciais de fáscia temporoparietal e enxerto ósseo nas reconstruções faciais em casos selecionados.
