Study of composite prefabricated flap of abdominal skin and gastromental vessels in arc on rabbits

ABSTRACT

The authors have carried out a prefabricated pattern of abdominal flap using gastromental vessels. Eighteen rabbits were divided in three equal groups. In group one, a piece of square abdominal cutaneous tissue was excised and sutured back (control group). In group two, gastromental vessels were dissecated and sutured into the superficial muscular fascia. After 15 days, a 10x10 cm2 square abdominal flap was released pedicled only by the loop of the gastromental vessels. In group three, the group two procedure was repeated, but with an interval of 60 days between the first and second surgeries. There was total necrosis in whole control group, partial necrosis on average of 54.33 cm2, in group two and lack of necrosis, in group three. The authors have concluded it's possible to prefabricate flaps of considerable size pedicled by the gastromental vessels in arch.

RESUMO

Os autores realizaram um modelo de retalho abdominal pré-fabricado, utilizando os vasos gastromentais. Dezoito coelhos foram divididos em três grupos iguais. No grupo um, controle, um fragmento de tecido cutâneo quadrangular abdominal foi excisado e suturado de volta. No grupo dois, vasos gastromentais foram dissecados e suturados na fáscia muscular superficial. Após 15 dias, um retalho quadrangular abdominal de 10x10 cm foi elevado pediculado apenas no arco das artérias gastromentais. No grupo três, foi repetido o procedimento do grupo dois, entretanto, com um intervalo de 60 dias entre a primeira e segunda cirurgia. Houve necrose total em todos do grupo controle, necrose parcial em média de 54,33 cm2 de área do retalho, no grupo dois e ausência de necrose, no grupo três. Os autores demonstraram a viabilidade de pré-fabricar retalhos de dimensões consideráveis, baseados no pedículo dos vasos gastromentais em arco.

Surgical standardization of abdominoplasty after bariatric surgery

ABSTRACT

Introduction: The authors propose changes in the post-bariatric abdominoplasty technique to address some imperfections (tension on the T-junction, poor placement of the horizontal scar and unaesthetic umbilical scar) seen with the classic standardization (e.g. en-bloc skin resection and maintenance of the umbilicus). Methods: Assessment was realized on 67 patients who had previously been submitted to bariatric surgery and undergone modified vertical abdominoplasty with staged skin resection (first the vertical component and then the horizontal one), neo-onphaloplasty and dermolipectomy of the pubic area. Results: The new standardization led to reduction on the previous described downsides of the technique (better scar positioning and less tension). Conclusions: The new systematization provided an easier pre-operatory marking and improved aesthetic results.

RESUMO

Introdução: Os autores propõem alterações na técnica empregada nas dermolipectomias abdominais após cirurgia bariátrica, com o intuito de corrigir imperfeições nos resultados obtidos com a padronização clássica (i.e. ressecção de pele em bloco com manutenção da cicatriz umbilical). Método: Sessenta e sete pacientes ex-obesos foram submetidos à dermolipectomia abdominal em âncora com ressecção parcelada de pele (primeiro o fuso vertical e após o excesso horizontal), confecção de neoumbigo e dermolipectomia pubiana associada. Resultados: A nova padronização adotada permitiu reduzir as insatisfações previamente observadas, com melhor posicionamento da cicatriz horizontal e menor tensão na intersecção das cicatrizes. Conclusão: A nova sistematização facilitou a marcação préoperatória e permitiu refinar alguns aspectos que se mostravam insatisfatórios na experiência com estes casos.

Triangular flap technique for medial thighplasty after massive weight loss in women

ABSTRACT

Background: The increasing number of patients who undergo bariatric surgery implies on a greater demand of reparative procedures for improvement of body contour. Methods: We describe a variation technique medial "T" thighplasty with addition of a triangular skin flap from the upper base, breaking the horizontal incision that runs the inguinal crease. This flap is positioned at the point of greatest traction craniocaudal corresponding to the vaginal opening and the tendon of the adductor muscle. The technique was employed in 18 patients. Results: The mean age of patients was 47 years, 22% had comorbidities (hypertension, diabetes mellitus and varicose disease). All had undergone bariatric surgery at a mean period of postoperative 63 months, mean BMI before bariatric surgery 48 and before thighplasty of 29 kg/m2. Seven (39%) patients underwent another procedure at the same intervention. The incidence of delayed healing was 25%, dehiscence of 8% and 11% of hypertrophic scars. None of them has traction side of the vulva or migration flow scars along the mean follow up of 10 months. Conclusions: The use of the triangular flap for medial thighplasty after major weight loss was shown to be a viable technical change that minimized the pull side of the vulva and the inferior migration of scars.

RESUMO

Introdução: Com o aumento do número de pacientes submetidos à cirurgia bariátrica, cresce também a realização de procedimentos reparadores visando à melhoria do contorno corporal. Método: Descrevemos uma variação técnica de cruroplastia medial em "T" com adição de um retalho cutâneo triangular, de base superior, interrompendo a incisão horizontal que corre na prega inguinal. Esse retalho fica posicionado no ponto de maior tração crânio-caudal correspondente ao intróito vaginal e ao tendão do músculo adutor. A técnica foi empregada em 18 pacientes. Resultados: A média de idade das pacientes foi de 47 anos; 22% apresentavam comorbidades (hipertensão, diabete mellitus, varizes). Todas haviam sido submetidas à cirurgia bariátrica num período médio de pós-operatório de 63 meses, sendo o IMC médio antes da cirurgia bariátrica de 48 e antes da cruroplastia de 29 kg/m2. Sete (39%) pacientes foram submetidas a outro procedimento simultaneamente à cruroplastia. A incidência de cicatrização retardada foi de 25%, deiscências de 8% e de cicatrizes hipertróficas de 11%. Nenhuma delas apresentou tração lateral da vulva ou migração caudal de cicatrizes ao longo do seguimento médio de 10 meses. Conclusões: A utilização do retalho triangular mostrou ser uma variação técnica viável que evitou tração lateral da vulva e minimizou a migração inferior de cicatrizes.
