Comparative study of pinch power return after chronic carpal tunnel release associated with opponensplasty

ABSTRACT

Thumb opponency is a complex and important hand's movement, realized by the thenar muscle wich is innervated by the median nerve, providing it of sensibility, strength and the ability to do the pinch movement. The goal of this work is to compare the pinch strength and the degrees of opponency before and after the surgery in 10 patients with chronical carpal tunnel's syndrome, hypotrophic thenar muscle and weakness of pinch's movement, wich were selected in our hand's surgery ambulatory center and submitted to an opponensplasty Thumb surgery using the Camitz technique. All the patients had a faster and satisfactory evolution with an amazing comeback to their quotidian activities.

RESUMO

A oposição do polegar é um importante e complexo movimento da mão, realizado por meio da musculatura tênar, que recebe inervação do nervo mediano, permitindo a realização do movimento de pinça, força e sensibilidade. O objetivo deste trabalho é comparar a força de pinça e grau de oponência do polegar, no pré e pós-operatório em 10 pacientes com síndrome do túnel do carpo crônica, hipotrofia da musculatura tênar e diminuição da força de pinça, que foram selecionados no nosso ambulatório de cirurgia da mão, e submetidos a cirurgia de oponentoplastia pela técnica de Camitz. Todos os pacientes evoluíram satisfatoriamente com rápido retorno as suas atividades cotidianas.

Umbilical Restoration in Abdominoplasty: A Simple Rectangular Technique

ABSTRACT

The authors present a simple technique for umbilicus restoration in abdominal dermolipectomy. The procedure consists of a rectangular incision in the skin of the abdomen, slightly curved at the superior and inferior poles, after freeing the umbilicus with a circular incision in the abdominal tissue. The technique gives the umbilicus a natural, circular shape, obtaining a satisfactory result. Different drawings and pictures are shown to explain the skin procedure.

RESUMO

Os autores apresentam uma técnica simples para a restauração do umbigo na dermolipectomia abdominal. Este procedimento está representado por uma incisão cutânea retangular, ligeiramente curvilínea nos pólos superior e inferior, feita na pele do abdome, após uma incisão circular no tecido abdominal liberando o umbigo. Esta técnica deixa o umbigo com uma forma natural, circular, apresentando um resultado satisfatório. Diferentes desenhos e figuras propõem-se a explicar o procedimento cutâneo.

Glomus tumor: an easy or a difficult diagnosis?

ABSTRACT

Introduction: The glomus tumor is rare subungueal pathology that consists of a neuromyoarterial. Glomus bodies sensible to temperature oscillations, that regulate arteriolar flow, located at the arteriolar-venula anastomosis. The main symptom of this pathology is an excruciating pain leading to functional limitation of the Lyme. Objective: The purpose this study is to highlight its clinical and histopathological diagnosis, as well as the use of microcopy on its surgical treatment. Results: The treatment resulted in a great reduction of recurrence, with functional recovery of the limb and improvement of apaticutp's physical, social and psycosocial health. Conclusion: The surgical resection of the tumor was efficient and the results were very satisfactory, with no significant post operative complications and no recurrence.

RESUMO

Introdução: O tumor glômico é uma lesão neoplásica benigna, geralmente única, que se forma no glomo terminal. Esta estrutura é uma anastomose arteriovenosa subcutânea distribuída por toda a superfície corporal, em particular nos leitos ungueais, polpas digitais, palmas das mãos e plantas dos pés. Apesar da sua raridade, recebe atenção especial por seus sintomas caracterizados por dor paroxística, sensibilidade à pressão do local, ao frio e por sua localização típica na ponta do dedo. O diagnóstico inicial é raramente feito, levando os pacientes a apresentarem os sintomas por meses e até anos, sem o diagnóstico e tratamento adequados. Objetivo: O objetivo desse trabalho é evidenciar o método diagnóstico clínico e histopatológico do tumor glômico, e a utilização da microscopia em seu tratamento cirúrgico. Resultados: Observou-se grande diminuição nos casos de recidiva, com retorno funcional do órgão acometido e melhora da saúde física, funcional e psicossocial do paciente. Conclusão: A exérese do tumor mostrou-se eficaz e os resultados foram plenamente satisfatórios, não havendo complicações pós-operatórias significativas, nem recidivas até o momento.

ABSTRACT

Introduction: The Dupuytren's disease is characterized by the progressive contracture in flexion of the digits, which could carry the hand to a full functional incapacity. The purpose of this study is precognize an efficient operative technique less traumatic for treating the disease. Methods: Fifty-five patients underwent to surgery, in different levels of the disease, were analyzed. Results: In all of the cases were obtained satisfactory results, returning the patients to their activities.

RESUMO

Introdução: A doença de Dupuytren é caracterizada pela contratura em flexão progressiva dos dígitos, o que pode levar à incapacidade funcional da mão. O objetivo deste trabalho é preconizar uma técnica operatória eficaz e menos traumática para conduzir a doença. Método: Foram tratados cirurgicamente 55 pacientes em vários estágios da doença. Resultados: Em todos os casos, foram obtidos resultados satisfatórios, com bom retorno dos pacientes às suas atividades.

ABSTRACT

Introduction: De Quervain's stenosing tenosynovitis it is characterized by being the inflammation of the hem of the long and extending abductor short of the thumb, in the 1st number compartment of the fist, more frequently attacking the women in the age group between 30 and 50 years. That pathology is associated mainly to secondary chronic trauma and overload of the daily activities of the hands and fist, could also be caused by other factors, but in many cases no there is a very defined cause. Results:All the patients had a faster and satisfactory evolution with an amazing comeback to their quotidian activities.

RESUMO

Introdução: A tenossinovite estenosante De Quervain caracteriza-se por ser a inflamação da bainha do abdutor longo e extensor curto do polegar, no primeiro compartimento dorsal do punho, acometendo mais frequentemente as mulheres na faixa etária entre 30 e 50 anos. Essa doença está associada principalmente a trauma crônico secundário e sobrecarga das atividades diárias das mãos e punho, podendo também ser causada por outros fatores, mas em muitos casos não há uma causa bem definida. Objetivo: O objetivo deste trabalho é apresentar nossa casuística de tenossinovite de De Quervain no Ambulatório de Cirurgia da Mão, os diferentes tratamentos e uma nova proposta cirúrgica utilizando um retalho fáscio-gorduroso de vizinhança. Resultados: Com a técnica proposta foram obtidos resultados satisfatórios, diminuindo as recidivas, com boa recuperação funcional e rápido retorno dos pacientes às atividades cotidianas.

Flexion contracture in burned hand: treatment evaluation through open cutaneous incision and second intention scare

ABSTRACT

Objectives: Evaluate the evolution of patients with contracture burn hand submitted to surgical treatment across cutaneous open and second intention scare. Methods: In a period of ten months, the authors evaluated five patients with hand contracture because of burn. Three of them were male and two female, with middle ages 10 years. All the patients were submitted to open lesions using transverse incision with second intention scare. They use immobilization and physiotherapy. Results: All five operated cases show satisfactory functional evolution.

RESUMO

Objetivos: Avaliar a evolução dos pacientes com contratura na mão queimada submetidos ao tratamento cirúrgico por meio da abertura cutânea e cicatrização por segunda intenção. Métodos: Em um período de dez meses, os autores avaliaram cinco pacientes com contraturas na mão em decorrência de queimaduras. Três eram do sexo masculino e dois do sexo feminino, com média de idade de 10 anos. Todos os pacientes foram submetidos à abertura das lesões através de incisões transversas e permitida a cicatrização por segunda intenção. Também fizeram uso de imobilização e fisioterapia. Resultados: Os cinco casos avaliados apresentaram evolução funcional considerada satisfatória pelos médicos, fisioterapeutas e pelos próprios pacientes.

ABSTRACT

Syndactyly is a finger congenital disease of the hand and foot, characterized by a separated defect of parts. Their treatment is essential surgery. The objective of this study is showing the experience with the Marumo's modified technique in twelve operated hands. In all cases we have good results, with return of aesthetic and functionality of hand.

RESUMO

Sindactilia é um defeito congênito dos dedos das mãos e pés, caracterizado por um defeito de separação das partes. Seu tratamento é essencialmente cirúrgico. O objetivo deste trabalho é relatar a experiência da equipe com a técnica de Marumo modificada, em doze mãos operadas. Em todos os casos, obtivemos bons resultados, com retorno da funcionalidade e estética das mãos.
