Comparative experimental study between first treatment to hydrochloric acid burns used irrigation with water versus caustic soda

ABSTRACT

Background: The literature preconises that the acids burn must be irrigated with large amounts of water as soon as possible. The base utilization in the therapy of acids chemical burns is controversies, because of the occurancy of an exothermical reaction. A chemical concept says that when an acid has to be diluted, the acid must be added to a water container, and not the opposite, because of the exothermical liberation of energy. Method: 40 rats were utilized divided in two groups of 20 rats. In both groups all rats were exposed to 12 molar hydrochloric acid for 30 seconds. One group had all rats treated with water irrigation for 15 minutes. The other group had all rats treated with caustic soda. Results: The results of the treatments were observed micro and macroscopically, and they are very similar Conclusion: The results being similar, the irrigation with water is the best choice because is cheap and easier use.

RESUMO

Introdução: Na literatura é amplamente preconizada para queimaduras com ácidos, uma lavagem exaustiva com água corrente o mais precoce possível. A utilização de bases no intuito de neutralizar o ácido é polêmica, pois a reação de um ácido com uma base é exotérmica. Também é um conceito em química, que a diluição de um ácido com água causa também uma reação exotérmica. Método: Foram utilizados 40 ratos divididos em dois grupos de 20 ratos. Em ambos os grupos, todos os ratos foram expostos ao ácido clorídrico 12 molar por 30 segundos e, posteriormente, um grupo teve todos os ratos tratados imediatamente com lavagem exaustiva com água corrente por quinze minutos e o outro grupo todos os ratos foram submetidos imediatamente a neutralização do ácido com soda cáustica 1 molar. Resultados: Os resultados foram observados macroscopicamente e microscopicamente e em ambos os tratamentos foram muito semelhantes. Conclusão: Como os resultados de ambas as modalidades de tratamento foram muito semelhantes, lavar com água corrente ainda é a melhor alternativa pela facilidade de manipulação e o baixo custo.

Histopathologic study of the use of topical heparin in scalded rats burns

ABSTRACT

Background: Burn studies and non-burn studies using parenteral heparin uncovered heparin effects which were antiinflammatory, neoangiogenic, epithelializing, collagen-regulating, and, in addition to anticoagulating. Use of heparin in burned patients, relieved pain, limited inflammation, resulted in a better evolution and skin results. This present study intend to evaluate these properties through seriated histopathologic analisys of skin treated with topical heparin only. Method: 20 rats with 2nd degree burn scalded by water at 99ºC in the back. Divided into two groups: 1) treated with topical heparin; and 2) treated with 0.9 saline solution. It was utilized diary one topical throw of heparin spray, that have 0.14ml or 1,400 UI. The rats were sacrificed through 2-3 days, and the burned skin sent to histopathologic study. The cross sections were eveluated by a pathologist that did not know which one was from group 1 ou group 2. Results: Heparin group revealed less acute (neutrophil count) and chronical (lymphocyte count) inflamatory process, more intense angiogenesis and less fibroblastic proliferation (measured the thickness from the dermal-epidermal junction) in comparison to the control group with 0.9 saline solution. Conclusion: The use of topical heparin alone improved the treatment of partial-thickness burns, resulted in less inflammatory process (acute and chronic), more neoangiogenesis, and less thick fibroblastic proliferation compared to similar burns treated with saline.

RESUMO

Introdução: Muitos estudos com uso da heparina parenteral, entre eles em queimados, demonstraram seus efeitos antiinflamatórios, angiogênico, estimulante de epitelização, modulador de colágeno, sem contar o anticoagulante. Nos queimados, aliviou a dor, limitou a inflamação, melhorando a evolução e o resultado da pele pós-queimadura. Este trabalho visa avaliar estas propriedades por meio de análise histopatológica seriada da pele tratada com heparina apenas por via tópica. Método: Foram avaliados 20 ratos submetidos à queimadura de 2º grau por escaldo com água aquecida a 99ºC no dorso. Divididos em dois grupos: 1) 10 tratados com heparina; e 2) 10 tratados com SF 0,9%. Foi utilizado um jato tópico diário de heparina sódica spray, que apresenta 0,14 ml ou 1.400 UI. Os animais foram sacrificados a cada 2-3 dias e enviada peça da área queimada para estudo histopatológico. As lâminas foram avaliadas por um patologista que desconhecia a qual grupo (1 ou 2) pertencia o corte. Resultados: O grupo 1 (com heparina) apresentou menor processo inflamatório agudo (contagem de neutrófilos) e crônico (contagem de linfócitos), angiogênese mais intensa e menor proliferação fibroblástica (medida espessura a partir da transição derme-epiderme) em relação ao grupo controle com SF 0,9%. Conclusão: A heparina sódica, em seu uso tópico isolado, melhorou o tratamento das queimaduras de acometimento parcial da pele, uma vez que diminuiu o processo inflamatório e a proliferação fibroblástica, com melhor resultado final.

Ethyl-2-cyanoacrylate (Epiglu®) evaluation in the skin closure: study in rats

ABSTRACT

Background: The Ethyl-2-cyanoacrylate (ECA) is a synthetic tissue adhesive used by some physicians in clinical practice in wound closures. They polymerize rapidly to adhere tightly to proteinaceous surfaces, form a strong and flexible bond. This study was performed to evaluate the efficacy and biocompatibility of this polymer (Epiglu®, of Meyer-Haake company, Germany) in skin wound closures, in rat model. Methods: Ten male Wistar rats were subjected to three incisions which were closed using different treatments. The central incision was treated witch Simple suture of nylon 4.0 (control group). Left incision was treated with sub-dermis suture associated with ECA, and right incision only with ECA. Rats were sacrificed on days 3, 7, 14, 21 and 30 after the intervention. The biocompatibility was evaluated due to direct observation: necrosis, inflammation, dermatitis, infection and dehiscence; and microscopy: inflammatory process, angiogenesis, number of fibroblasts, collagen, giants cells and eosynophils. Results: The wound closured only with ECA presented a higher dehiscence level (p < 0.05). It was not observed another macro or microscopic significant difference comparing to the control group (p > 0.05). Conclusion: The use of the ECA is a good option to the skin closure, with good biocompatibility. Special attention must receive the wounds with some tension, where the association with a sub-dermis suture becomes imperative.

RESUMO

Introdução: O Etil-2-cianoacrilato (ECA) é um adesivo tecidual utilizado por alguns médicos na prática clínica para fechamento de lesões de pele. São substâncias que se autopolimerizam quando em contato com a ferida, formando uma cola forte e flexível. Este trabalho foi realizado visando à avaliação da eficácia e da biocompatibilidade deste polímero (Epiglu®, da empresa Meyer- Haake, Alemanha) na síntese de pele, em modelo experimental. Método: Dez ratos Wistar machos foram submetidos a três incisões longitudinais no dorso. A incisão central foi tratada com sutura de pontos simples de nylon 4.0 (grupo controle), a esquerda com sutura subdérmica de nylon 4.0 associado ao ECA, e a direita apenas o ECA. Os animais foram sacrificados nos dias 3, 7, 14, 21 e 30 pós-operatórios. A biocompatibilidade foi avaliada sob observação direta: necrose, inflamação, dermatite de contato, infecção e deiscência; e à microscopia: reação inflamatória, angiogênese, fibroblastos, colágeno, células gigantes e eosinófilos. Resultados: As sínteses realizadas apenas com o ECA apresentaram maior índice de deiscência (p < 0,05). Todos os outros critérios macro e microscópicos avaliados não apresentaram diferenças estatísticas em relação ao controle (p > 0,05). Conclusão: O uso do ECA é uma boa opção de síntese da pele, com boa biocompatibilidade. Atenção deve ser feita às lesões que apresentem algum grau de tensão, onde a associação com a sutura subdérmica torna-se obrigatória.

Biochemical analysis of the tissues expanders: study

ABSTRACT

Introduction: The tissues expanders represent useful tools to correct scar's sequels. Neumann (1957) was the first to publish a report on expansion of skin. Since then the expanders have been used for repairs. Objectives: The goals of the work are to report and evaluate the permeability of tissues expanders under study "in vitro". Methods: Method 1 - It was analyzed seven different expanders in seven different solutions, where it's been immersed in their respective solutions, filled with air in their totals, found their periods of saturations after weighing, when it was done washed of its inside with bidestilated water (control) and sent to analysis. Method 2 - It was put all the solutions together in a single bottle, immersed two aleatory expanders filled with bidestilated water, and after 48 hours, sent samples to analysis. Another bottle was done with albumin and repeated the same previous methodology. Results: Method 1 - In the analysis of empty expanders, the albumin, chlorine, calcium, phosphorus, magnesium and glucose performed under to quantification limits of the used methods. The sodium and potassium performed near to lower limit. Method 2 - In the analysis after 48 hours, all of ions performed under to quantification limit, except the glucose and sodium in expander texturized were in very low value to used methods. Conclusion: The present work suggests a weak or irrelevant permeability relating to this analysis.

RESUMO

Introdução: Os expansores teciduais representam instrumentos úteis para correção de seqüelas cicatriciais. Neumann (1957) foi o primeiro a publicar relato sobre expansão de pele. Desde então, os expansores vêm sendo utilizados para reparações. Objetivos: Os objetivos do trabalho são apresentar e avaliar a permeabilidade dos expansores teciduais em estudo "in vitro". Método: Método 1 - Foram analisados 7 expansores aleatórios em 7 soluções distintas, onde foram mergulhados nas respectivas soluções, preenchidos com ar em seus volumes totais, encontrados seus períodos de saturação após pesagens, realizado lavado de seu interior com água bidestilada (controle) e enviados para análise. Método 2: Foram unidas tais soluções em único frasco, mergulhados 2 expansores aleatórios preenchidos com água bidestilada, aguardados 48 horas e analisadas amostras. Outro frasco foi feito com solução de albumina e repetida a mesma metodologia anterior. Resultados: Método 1 - Na análise dos expansores vazios, a albumina, cloreto, cálcio, fósforo, magnésio e glicose apresentaram-se abaixo do limite de quantificação dos métodos utilizados. O sódio e potássio apresentaram valores próximos ao limite inferior. Método 2 - Na análise após 48 horas, todos os íons apresentaram-se abaixo do limite de quantificação, exceto a glicose e o sódio no expansor texturizado em baixíssimo valor pelo método utilizado. Conclusões: O presente estudo sugere permeabilidade débil ou irrelevante nas análises referidas.

Burns immediate dressing with water, oil of melaleuca and emulsificants

ABSTRACT

Introduction: Burnshield® (United Medical, Brazil) is an option of immediate dressing for burns aimed at hydrating and cooling the wound. Purpose: Investigate, histopathologically, the efficacy of Burnshield® as initial treatment for thermal burns. Method: Blind controlled experimental study, with 36 Wistar rats divided into 3 groups of 12 rats: (A) treated with Burnshield®, (B) treated with running water for 15 minutes and (C) not treated. Half of the animals of each group were sacrificed 30 minutes after the burn and the other half, within 24 of evolution. The assessment criterion was the extension of lesion found in each animal. Results: Group C showed the deepest burns compared to the others, within 30 minutes and 24 hours. There was no difference between groups A and B within 30 minutes; however, group A showed better and significant results within 24 hours of evolution. Conclusion: Burnshield® showed efficacy comparable to running water at the initial treatment of burns, with better results after 24 hours of evolution. Its main advantage is its practicality, once it can be used while the patient is carried up to the center of specialized treatment.

RESUMO

Introdução: O Burnshield® (United Medical, Brasil) é uma opção de curativo imediato para queimaduras que visa à hidratação e ao resfriamento da ferida, sendo objetivo deste trabalho avaliar sua eficácia. Método: Estudo experimental controlado cego, com 36 ratos Wistar divididos em 3 grupos de 12: (A) tratados com Burnshield®, (B) tratados com água corrente por 15 minutos e (C) não tratados. Metade dos animais de cada grupo foi sacrificada com 30 minutos após queimadura, e a outra metade com 24 horas de evolução. O critério de avaliação foi a maior profundidade de lesão encontrada em cada animal. Resultados: O grupo C apresentou as queimaduras mais profundas em relação aos demais, com 30 minutos e 24 horas. Não houve diferença entre os grupos A e B com 30 minutos, porém houve melhor resultado significativo no grupo A nas 24 horas de evolução. Conclusão: O Burnshield® mostrou eficácia comparável à água corrente no tratamento inicial de queimaduras, com melhores resultados depois de 24 horas de evolução. Apresenta como maior vantagem a praticidade, podendo ser usado enquanto o paciente é deslocado até o centro de tratamento especializado.
