Pharyngoesophageal Reconstruction: Using Microsurgical Jejunum and Pectoralis Major Flaps

ABSTRACT

Pharyngoesophageal reconstruction is one of the major challenges of neck surgery. Many treatment options have been described. All of them involve procedures of greater or lesser technical complexity and high rates of complications and mortality. The present study describes the techniques performed at the Service of Plastic Surgery of the PUC-RS São Lucas Hospital in 10 reconstructions: myocutaneous pectoralis major flap and microsurgical jejunal flap. Indications, complications, advantages and disadvantages of each method are discussed.

RESUMO

A reconstrução faringoesofágica é um dos maiores desafios cirúrgicos da região cervical. Existem diversas opções terapêuticas descritas. Todas envolvem procedimentos de maior ou menor grau de complexidade técnica e altas taxas de complicações e de mortalidade. Este artigo descreve as técnicas utilizadas no Serviço de Cirurgia Plástica do Hospital São Lucas da PUC-RS em 10 reconstruções: o retalho miocutâneo peitoral maior e o retalho microcirúrgico de jejuno. São discutidas as indicações, complicações, vantagens e desvantagens de cada método.

Single Digit Replantation in Outpatient Surgery: Experience of 120 Cases

ABSTRACT

We performed 120 single digit replantations on an outpatient surgery basis between 1994 and 2001. The levels of amputation were: proximal phalanx in 50 patients, proximal interphalanx in 21, distal interphalanx in 42 and metacarpophalanx in 7 patients. Trauma mechanism included: avulsion (31.7%) and guillotine (68.3%). The mean normothermal ischemia time was 8h (ranging between 6 -15h). Patients stayed in the hospital a maximum of 8 h after the surgery. All of them were told about the possibility of an arterial or venous thrombosis and instructed on how to identify it. Patients who believed any circulatory abnormality was occurrinq, telephoned the surgeon and were immediately assessed. Reassessment was performed in 20 patients, and circulatory deficits were confirmed in 15 patients. There was replantation loss in 12 of the latter patients. The replantation success rate was 90%, and the major determining factor was trauma mechanism and not the postoperative regimen. We suggest that single digit replantation be performed whe never there is an indication and that the availability of hospital beds should not be a decisive factor in the choice of the method of treatment of amputations and single digit devascularizations.

RESUMO

Realizamos 120 reimplantes unidigitais em nível ambulatorial entre 1994 e 2001. Os níveis de amputação foram: falange proximal em 50 pacientes, interfalangeana proximal em 21, interfalangeana distal em 42 e metacarpofalangeana em 7 pacientes. O mecanismo de trauma incluiu: avulsão (31,7%) e guilhotina (68,3%). O tempo médio de isquemia normotérmica foi 8 h (extremos 6-15h). Os pacientes permaneciam em ambiente hospitalar no máximo 8 h após o término da cirurgia. Todos os pacientes foram orientados sobre as possibilidades de trombose arterial ou venosa e de que forma isso poderia ser identificado. Os pacientes que julgassem haver qualquer anormalidade circulatória ou que estivessem em dúvida telefonavam ao cirurgião que os avaliava imediatamente. A reavaliação foi realizada em 20 pacientes, mas o déficit circulatório comprovou-se em 15 pacientes. Destes, houve perda do reimplante em 12 pacientes. A taxa de sucesso dos reimplantes foi de 90%, e o mecanismo do trauma foi o principal fator determinante e não o regime pós-operatório. Sugerimos que o reimplante unidigital seja realizado sempre que houver indicação e que a existência de leitos hospitalares disponíveis não seja o fator decisivo na escolha do método de tratamento das amputações e desvascularizações unidigitais.

Reconstruction of Fingertip Pulp Loss

ABSTRACT

The loss of volar fingertip pulp is frequent in hand trauma. Eighty-six homodigital flaps (22 Tranquilli; 20 Hueston; 40 Single Pedicle Island and 4 Kuttler) were performed from January 1994 to July 1998) to repair volar fingertip substance loss, except for the thumb. The mean age was 28 years (ranging from 18 -58) and the average post-operative follow-up period was 12 months. We analyzed advancements, sensitivity and cold intolerance for each flap used: Tranquilli (0.4 cm / 8 mm / 6); Kuttler (0.3 cm / 10 mm / 3); Hueston (1.0 cm / 9mm / 5); Single Pedicle Island (1.5 cm / 8 mm/ 6). The single pedicle island flap was demonstrated to be clearly better in all aspects analyzed. We consider the utilization of homodigital flaps the first choice alternative for reconstructing fingertips.

RESUMO

As perdas de substâncias digitais distais volares apresentam-se com freqüência nos traumas da mão. De janeiro de 1994 a julho de 1998, 86 retalhos homodigitais (22 Tranquilli; 20 Hueston; 40 Ilha Unipediculado e 4 Kuttler) foram realizados para reparar perdas de substância distais volares, excluindo o polegar. A idade média foi de 28 anos (extremos 18 - 58) e o tempo de seguimento médio pós-operatório foi de 12 meses. Analisamos o avançamento obtido, a sensibilidade e a intolerância ao frio de cada retalho utilizado. Tranquilli (0,4 cm/ 8 mm/ 6); Kuttler (0,3 cm/ 10 mm/ 3); Hueston (1,0 cm/ 9mm/ 5); Ilha Unipediculado (1,5 cm/ 8 mm/ 6). O retalho em ilha unipediculado mostrou-se nitidamente superior em todos os aspectos analisados. Consideramos que a utilização de retalhos homodigitais constitui a alternativa de primeira escolha na reconstrução da extremidade digital.

The Open Palm Technique in Severe Dupuytren's Contracture

ABSTRACT

This paper aims to study the authors' experience with the open palm technique originally described by McCash for the treatment of severe Dupuytren's contractures. All patients presented hand ulnar border (ring and little fingers) impairment and belonged to stages III to IV of the classification proposed by Tubiana. In a period of 6 years, 35 patients were submitted to regional fasciectomy. On average, 18 days were necessary (extremes of 15 and 25) to complete scar healing. We obtained an average articular residual deficit of 25° to the ring finger and 30° to the little finger at the proximal interphalangeal articulation level.

RESUMO

Este estudo tem como objetivo demonstrar a experiência dos autores com a técnica da palma aberta descrita originalmente por McCash para o tratamento das contraturas graves de Dupuytren. A totalidade dos pacientes apresentava acometimento da borda ulnar da mão (anular e mínimo) e pertenciam aos estágios III e IV da classificação proposta por Tubiana. Em um período de 6 anos, 35 pacientes foram submetidos à fasciectomia regional. Foram necessários 18 dias, em média (extremos 15 e 25), para completar a cicatrização cutânea. Obteve-se um déficit residual articular médio de 25º ao dedo anular e 30º ao dedo mínimo, sempre ao nível da articulação interfalangiana proximal.

Incidence of Hematoma and Seroma in Abdominoplasty with and without the Use of Drains

ABSTRACT

Based on the experience with 69 patients submitted to abdominoplasty, the authors compare the use of capillary tubes, suction drains and not using drains in the postoperative period as to the effictiveness in preventing hematomas and seromas under the abdominal flap. They also study the association of these fluid collections with known risk factors. They describe the indications and surgical technique employed in their cases and compare results obtained with data available in literature.

RESUMO

Os autores, baseados na experiência de 69 pacientes submetidos a abdominoplastia, comparam a utilização de dreno de capilaridade, de sucção e a não utilização de drenos no pós-operatório quanto à eficácia na prevenção da formação de hematomas e seromas sob o retalho abdominal, bem como estudam a associação dessas coleções com fatores de risco conhecidos. Descrevem as indicações e técnica cirúrgica empregadas em sua casuística e comparam seus resultados com dados presentes na literatura.
