Bibliometric analysis of articles published in the Brazilian Journal of Plastic Surgery between 2005 and 2012. Part I: quantitative analysis of articles, authors, and geographical distribution

ABSTRACT

INTRODUCTION: Bibliometric indicators have been implemented in several national and international studies. However, there is no specific information about the scientific articles discussing plastic surgery in Brazil. This study therefore aimed to carry out a bibliometric analysis of articles published in the Brazilian Journal of Plastic Surgery (RBCP), the official journal of the Brazilian Society of Plastic Surgery. METHODS: A bibliometric analysis using quantitative indicators of all articles published in the RBCP was performed to characterize the scientific results over a seven-year period (2005-2012) marked by major changes to the journal. All articles selected were analyzed individually, in order to collect data on article and author numbers, and their geographical distribution. Two periods (2005-2008 vs. 2009-2012) were considered for comparative analysis. RESULTS: A total of 603 articles met the inclusion criteria. The average number of articles published per year and the number of authors per article were 75.38 ± 32.12 articles/year and 3.98 ± 2.01 authors/article, respectively. There was a significant increase (p < 0.05) in articles/year in 2009-2012 compared to the earlier time period. No significant variations were detected (p > 0.05) in the number of authors/article (2005-2008 = 2009-2012). Most articles came from Brazilian institutions (98.67%; p < 0.05), and the Southeast region (63.70%; p < 0.05) accounted for most of these publications. CONCLUSION: During the observation period, the number of scientific articles published in the RBCP increased, there was no change in the number of authors per paper, and there was a predominance of articles coming from the Southeast Brazil.

INTRODUÇÃO: Os indicadores bibliométricos têm sido aplicados em diversos estudos nacionais e internacionais. No entanto, não existem informações específicas sobre a produção científica da cirurgia plástica no Brasil. Este estudo, portanto, tem o objetivo de realizar uma análise bibliométrica dos artigos publicados na Revista Brasileira de Cirurgia Plástica (RBCP), periódico oficial da Sociedade Brasileira de Cirurgia Plástica. MÉTODOS: Foi realizada uma análise bibliométrica com indicadores quantitativos de todos os artigos publicados na RBCP, para caracterizar os resultados científicos de um período (2005 a 2012) marcado por grandes mudanças. Todos os artigos incluídos foram analisados individualmente, a fim de se coletarem dados referentes ao número de artigos, número de autores e distribuição geográfica. Dois períodos (2005-2008 versus 2009-2012) foram considerados para análise comparativa. RESULTADOS: Seiscentos e três artigos preencheram os critérios de inclusão. As médias de artigos publicados por ano e de autores por artigo foram 75,38±32,12 artigos/ano e 3,98±2,01 autores/artigo, respectivamente. Houve um aumento significativo (p<0,05) de artigos/ano (2005-2008<2009-2012). Não foram detectadas variações significativas (p>0,05) no número de autores/artigo (2005-2008=2009-2012). A maioria (p<0,05) dos artigos foi proveniente de instituições brasileiras (98,67%), sendo a região sudeste (63,70%) a responsável pela maioria dessas publicações (p<0,05). CONCLUSÃO: Durante o período investigado, cresceu o número de artigos científicos publicados na RBCP, não houve modificação no número de autores por artigo e existiu um predomínio de artigos oriundos da região sudeste do Brasil.

ABSTRACT

INTRODUCTION: Although bibliometry has been used to characterize scientific results in numerous national and international journals, bibliometric data on scientific publications related to plastic surgery are scarce. Therefore, this study analyzed articles published in the Brazilian Journal of Plastic Surgery (BJPS), the official journal of the Brazilian Society of Plastic Surgery (BSPS). METHODS: A quantitative bibliometric analysis of all articles published in the BJPS between 2005 and 2012 was performed. Information regarding presentation of the work at scientific meetings, institution type (BSPS-accredited or not), and article category were extracted from the articles included in this study. Data from two time periods (2005-2008 and 2009-2012) were analyzed and compared. RESULTS: A total of 603 scientific articles fulfilled the insertion criteria. Most articles were from accredited services institutions (58.21%), had not been presented at scientific meetings (84.74%), and were published as original articles (78.77%) in the BJPS (p<0.05 for all comparisons). Comparative analysis revealed a significant reduction in the proportion of review articles and a significant increase in the proportion of scientific articles that had not previously been presented at scientific meetings in 2005-2008 compared to 2009-2012 (p<0.05 for both). CONCLUSION: This survey revealed a prevalence of original articles published by BSPS-accredited institutions that were not presented at scientific meetings.

INTRODUÇÃO: Embora a bibliometria tenha sido aplicada na caracterização dos resultados científicos de inúmeros periódicos nacionais e internacionais, dados bibliométricos sobre a produção científica da cirurgia plástica brasileira são escassos. Assim, o objetivo deste estudo é analisar os artigos publicados na Revista Brasileira de Cirurgia Plástica (RBCP), periódico oficial da Sociedade Brasileira de Cirurgia Plástica (SBCP). MÉTODOS: Trata-se de uma análise bibliométrica, quantitativa, de todos os artigos publicados entre 2005 e 2012 na RBCP. Informações referentes à apresentação em eventos científicos, aos tipos de instituição (credenciadas ou não pela SBCP) e a categoria dos artigos foram extraídas dos artigos incluídos. Dados de dois períodos (2005-2008 versus 2009-2012) foram analisados, comparativamente. RESULTADOS: Um total de 603 artigos científicos preencheram os critérios de inclusão. A imensa maioria (p<0,05 para todas as comparações) desses artigos foi proveniente de Serviços Credenciados pela SBCP (58,21%), não foi apresentada em eventos científicos (84,74%) e foi publicada na seção artigo original (78,77%) da RBCP. A análise comparativa entre os períodos revelou uma redução significativa (p<0,05) na proporção de artigos de revisão (2005-2008>2009-2012) e um aumento significativo (p<0,05) na proporção de artigos científicos que não foram apresentados previamente em eventos científicos (2005-2008<2009-2012). CONCLUSÃO: Esta investigação revelou um predomínio de artigos publicados na seção artigos originais, provenientes de Serviços Credenciados pela SBCP, e que não foram apresentados em eventos científicos.

Use of Bernard-Webster flap for lower lip reconstruction after excision of squamous cell carcinoma: analysis of functional results

ABSTRACT

INTRODUCTION Defects in the lower lips after the excision of squamous cell carcinoma (SCC) are usually repaired using several surgical techniques. However, the functional reconstruction of full-thickness defects in the lower lip remains a challenge. Therefore, the objectives of this study were to describe surgical strategies for the reconstruction of full-thickness defects of the lower lip after surgical excision of the SCC and evaluate the functional results. METHODS: This was a retrospective study of all patients with full-thickness defects in the lower lip after excisions of SCC that were greater than one third of the lip that were repaired using the Bernard-Webster flap between 2011 and 2014. The functional results (sphincteric, motor and sensory function) were evaluated according to previously used criteria. RESULTS: Six lip defects were reconstructed without complications using Bernard-Webster flaps. During the postoperative period, the patients reported liquid incontinence (16.67%), intermittent salivary incontinence (16.67%), and difficulty fully opening the mouth (33.33%). The tests revealed sensory deficits in the labiomental areas (100%) and lower lip (33.33%). There was full recovery of all functional changes an average of 3 months after surgery. The observed late functional results were considered satisfactory. CONCLUSIONS: The Bernard-Webster flap was an excellent alternative to repairing the defects that affect more than one third of the lower lip since it allowed the use of similar local tissues, a single surgery could be performed, and the result was functionally efficient.

RESUMO

INTRODUÇÃO Defeitos dos lábios inferiores após exéreses de carcinoma espinocelular (CEC) vêm sendo reparados com inúmeras técnicas cirúrgicas. No entanto, a reconstrução funcional de defeitos de espessura total do lábio inferior continua a ser um desafio. O objetivo deste estudo, portanto, foi descrever as estratégias cirúrgicas para a reconstrução de defeitos de espessura total do lábio inferior, subsequentes a exéreses cirúrgicas de CEC, bem como avaliar resultados funcionais. MÉTODO: Trata-se de um estudo retrospectivo de todos os pacientes com defeitos de espessura total de lábio inferior após exérese de CEC maiores que um terço do lábio, reconstruídos com retalhos de Bernard-Webster, no período de 2011 a 2014. Os resultados funcionais (funções esfincteriana, motora e sensorial) foram avaliados de acordo com critérios previamente utilizados. RESULTADOS: Seis defeitos labiais foram reconstruídos com retalhos de Bernard-Webster, sem complicações. No período pós-operatório, os pacientes relataram incontinência para líquidos (16,67%), incontinência salivar intermitente (16,67%) e dificuldade para abrir a boca completamente (33,33%). Os testes revelaram déficits sensoriais nas regiões mentoniana (100%) e de lábio inferior (33,33%). Houve recuperação completa de todas as alterações funcionais, em média, três meses após as intervenções cirúrgicas. Os resultados funcionais tardios foram considerados satisfatórios. CCONCLUSÕES: O retalho de Bernard-Webster mostrou-se uma excelente alternativa para reparar defeitos que comprometem mais de um terço do lábio inferior, pois permitiu a utilização de tecidos vizinhos semelhantes, pôde ser realizado em tempo único e foi funcionalmente eficaz.

ABSTRACT

INTRODUCTION: Bibliometric methods have been used to analyze study designs and levels of evidence of articles published in various periodicals. Such information does not exist in the field of Brazilian plastic surgery. This study aimed to evaluate the study designs and levels of evidence of articles published in the Brazilian Journal of Plastic Surgery ("Revista Brasileira de Cirurgia Plástica," RBCP), the only Brazilian journal devoted exclusively to plastic surgery. METHOD: Study designs and levels of evidence of articles published in the RBCP between 2005 and 2012 were bibliometrically and quantitatively analyzed. The articles published in two periods (2005-2008 and 2009-2012) were compared, in order to characterize the evolution of the scientific production of the RBCP. RESULTS: During the period analyzed, 603 scientific articles met the inclusion criteria. The overall analysis revealed a significant predominance of retrospective studies and articles with level of evidence III (p < 0.05). The comparison between the periods showed a significant increase (p < 0.05) in the proportion of case series and scientific articles with level of evidence IV (2005-2008 < 2009-2012), and a significant reduction (p < 0.05) in the proportion of articles with level of evidence V (2005-2008 > 2009-2012). CONCLUSION: This study showed that a predominance of retrospective studies and articles with level of evidence III published in the RBCP between 2005 and 2012.

INTRODUÇÃO: Os desenhos dos estudos e os níveis de evidência dos artigos publicados em diversos periódicos vêm sendo analisados bibliometricamente. Como não existem tais informações no campo da cirurgia plástica brasileira, o presente estudo tem o objetivo de avaliar os desenhos dos estudos e os níveis de evidência dos artigos publicados na Revista Brasileira de Cirurgia Plástica (RBCP), o único periódico brasileiro destinado exclusivamente à cirurgia plástica. MÉTODO: Foi realizada uma análise bibliométrica, quantitativa dos desenhos dos estudos, e níveis de evidência dos artigos publicados na RBCP, entre 2005 a 2012. Dois períodos (2005-2008 versus 2009-2012) foram comparados para caracterizar a evolução da produção científica da RBCP. RESULTADOS: No período analisado, 603 artigos científicos preencheram os critérios de inclusão. A análise global revelou um predomínio significativo (p<0,05) de estudos retrospectivos e artigos com nível de evidência III. A comparação entre os períodos revelou aumentos significativos (p<0,05) nas proporções de séries de casos e artigos científicos com nível de evidência IV (2005-2008<2009-2012), e uma redução significativa (p<0,05) na proporção de artigos com nível de evidência V (2005-2008>2009-2012). CONCLUSÃO: Este estudo demonstrou que houve um predomínio de estudos retrospectivos e artigos com nível de evidência III, publicados na RBCP entre 2005 e 2012.

Evaluation of neurosensory disturbance in patients with craniofacial syndrome subjected to horizontal chin advancement

ABSTRACT

BACKGROUND: Although neurosensory disturbances after genioplasty have been evaluated in different studies, standardization for testing and grading of neurosensory injuries is lacking. For this reason, the incidence of neurosensory disturbance varies from 0% to 100%, depending on the definition of nerve damage, the sensitivity of the diagnostic test method, and the follow-up period. Therefore, the aim of this study was to perform an objective evaluation of the permanent neurosensory disturbances in patients who underwent horizontal chin advancement. METHODS: A retrospective study of all patients who underwent horizontal chin advancement at the Hospital SOBRAPAR between 2009 and 2010 was conducted. The objective neurosensory assessment of the lower lip and chin was performed using 2 neurological tests, namely the Semmes-Weinstein pressure and thermal sensitivity tests. Permanent neurosensory disturbance was defined as abnormal clinical test results obtained at least 12 months after surgery. RESULTS: Thirteen patients (8 with craniofacial syndrome) were evaluated. The prevalence of the patients who showed normal results for sensitivity to pressure/touch and thermal sensitivity (warm and cold; P < 0.05) was significantly high. The analysis of the data of the patients with craniofacial syndrome revealed that most of the patients had normal pressure sensitivity test results (P < 0.003). Results from the thermal sensitivity tests showed no significant difference between these patients (P = 0.317). No significant differences were observed between the anatomical regions with abnormal sensitivity test results (P > 0.05). CONCLUSIONS: Tactile sensitivities of the lower lip and chin to pressure and temperature were preserved in most of the patients 12 months after horizontal chin advancement.

RESUMO

INTRODUÇÃO: Embora os distúrbios neurossensoriais após genioplastias tenham sido avaliados em diferentes estudos, não existe uma padronização de como testar e classificar tais alterações. Por essa razão, a incidência de distúrbios neurossensoriais varia de 0 a 100%, dependendo da definição da lesão dos nervos, da sensibilidade do método diagnóstico e do período de seguimento. Portanto, o propósito deste estudo foi avaliar objetivamente o déficit neurossensorial permanente em pacientes submetidos a avanço horizontal do mento. MÉTODO: Foi realizado estudo retrospectivo de todos os pacientes submetidos a avanço horizontal do mento no Hospital SOBRAPAR, no período de 2009 a 2010. A avaliação neurossensorial objetiva do lábio inferior e do mento foi realizada com dois testes neurológicos (teste dos limiares de pressão de Semmes-Weinstein e teste de sensibilidade térmica). O déficit neurossensorial permanente foi definido como testes clínicos anormais com no mínimo 12 meses de pós-operatório. RESULTADOS: Foram avaliados 13 pacientes, sendo 8 deles portadores de síndromes craniofaciais. Houve predomínio de pacientes com os testes de sensibilidade tátil à pressão e térmica (quente e frio) normal (P < 0,05). A análise dos pacientes sindrômicos revelou que a maioria teve o teste de sensibilidade tátil à pressão normal (P < 0,003), não existindo diferenças no teste de sensibilidade térmica (P = 0,317). Não foram identificadas diferenças entre as regiões anatômicas com testes de sensibilidade anormais (P > 0,05). CONCLUSÕES: A maioria dos pacientes apresenta sensibilidade tátil (pressão e temperatura) do lábio inferior e mento preservada 12 meses após terem sido submetidos a avanço horizontal do mento.

Assessment of donor area pain in patients with cleft lip and palate undergoing alveolar bone defects repairs using iliac crest autogenous grafting: a prospective randomized comparison of two bone extractors

ABSTRACT

INTRODUCTION: Autogenous bone grafting is the standard treatment for alveolar bone defects. However, morbidity in the donor area after the bone graft has been obtained continues to be a significant problem in cleft patients. This prospective randomized study compared donor area pain associated with the use of 2 bone extractors in patients with cleft lip and palate, who underwent treatment of alveolar bone defects using a bone graft obtained from the iliac crest. METHOD: Thirty-six patients with cleft lip and palate underwent alveolar bone defect repair using a graft from the iliac crest, harvested with either a SOBRAPAR bone extractor (group A) or UCLA bone extractor (group B). Donor area pain was evaluated in the postoperative period with the aid of a unidimensional numerical pain scale (0, "no pain"; 10, "worst pain imaginable"). RESULTS: Comparison of the mean donor area pain score did not reveal any significant differences (p >0.05 for all comparisons) between the groups A and B, at any of the postoperative times evaluated. A significantly higher number of patients in group B reported no pain in the donor area, compared with group A (p <0.05). CONCLUSIONS: This study showed that a significantly greater number of patients in group B reported "no pain", compared with patients in group A; with regard to patients who reported any level of pain greater than zero, there were no between-group differences.

RESUMO

INTRODUÇÃO: Enxerto ósseo autógeno é o padrão no tratamento da falha óssea alveolar. Como a morbidade na área doadora após a obtenção de enxerto ósseo continua sendo um problema relevante em pacientes fissurados, este estudo avaliou a dor na área doadora de pacientes fissurados submetidos ao tratamento de falhas ósseas alveolares com a transferência de enxerto ósseo obtido da crista ilíaca, por meio de um estudo prospectivo randomizado, comparando dois extratores ósseos. MÉTODO: Trinta e seis pacientes com fissura labiopalatina, submetidos ao reparo da falha óssea alveolar com enxerto obtido da crista ilíaca com auxílio do extrator ósseo SOBRAPAR (grupo A) ou extrator ósseo UCLA (grupo B), foram incluídos. A dor na área doadora foi avaliada no período pós-operatório com auxílio da escala numérica unidimensional de dor (0- "sem dor"; 10- "pior dor que se pode imaginar"). RESULTADOS: As médias das mensurações da dor na área doadora não revelaram diferenças significativas (p>0,05 para todas as comparações) nas comparações realizadas entre os grupos A e B, em nenhum dos momentos pós-operatórios avaliados. Houve um maior número (p<0,05) de pacientes do grupo B que não reportaram dor na área doadora, quando comparado ao grupo A. CONCLUSÕES: Este estudo apresentou um maior número de pacientes do grupo B "sem dor", quando comparado aos pacientes do grupo A, não existindo diferenças entre aqueles que reportaram quaisquer notas diferentes de zero.

ABSTRACT

INTRODUCTION: The Parry-Romberg Syndrome (PRS) is characterized by progressive hemifacial atrophy that often leads to severe esthetic and functional difficulties. Although there are systems for grading disease severity, none have proven ideal in optimizing the therapeutic approach to these patients. This study aimed to establish the surgical strategies for the treatment of PRS based on a new system for severity grading of the disease. METHODS: This retrospective study included PRS patients undergoing surgery between 2005 and 2011. The surgical strategies were adapted for each patient according to a clinical severity grading system based on disease progression: type I, affecting the epidermis, dermis, and subcutaneous tissue; type II, type I + muscle involvement; and type III, Types I+ II + bone involvement. The sample included four patients (28.57%) with PRS type I, six patients (42.85%) with PRS type II, and four patients (28.57%) with PRS type III. RESULTS: Forty-seven procedures were performed. Free-fat grafts were used in all patients. Dermal fat grafts were used in all type II patients and one type III patient (25%). Bone grafts with temporoparietal fascia flaps were performed for the treatment of all type III patients. One type III patient (25%) underwent orthognathic surgery. All patients were improved in their overall facial appearance and there were no procedure-related complications. CONCLUSION: Our proposed system for grading PRS severity can facilitate the choice of therapeutic approaches and with a combination of surgical techniques based on the severity of the disease partially satisfactory outcomes can be attained.

RESUMO

INTRODUÇÃO: Síndrome de Parry-Romberg (SPR) é caracterizada pela atrofia hemifacial progressiva que, muitas vezes, resulta em graves distúrbios estéticos e funcionais. Embora existam escalas de gravidade, nenhuma delas é completamente ideal para auxiliar na abordagem terapêutica destes pacientes. O objetivo deste estudo foi delinear as estratégias cirúrgicas para o tratamento da SPR baseado em um novo sistema de classificação de gravidade da doença. MÉTODO: Trata-se de uma análise retrospectiva dos pacientes com SPR operados em 2005-2011. As abordagens cirúrgicas foram individualizadas de acordo com a escala de gravidade clínica baseada na evolução da doença: tipos I (envolvimento da epiderme, derme e tecido subcutâneo); II (tipo I + envolvimento muscular); e III (tipo I + II + envolvimento ósseo). Quatro (28,57%) pacientes com SPR tipo I, 6 (42,85%) tipo II e 4 (28,57%) tipo III foram incluídos. RESULTADO: Um total de 47 procedimentos foi realizado. Gordura livre foi enxertada em todos os pacientes. Todos os pacientes do tipo II e 1 (25%) do tipo III foram submetidos a enxertos dermogordurosos. Enxertos ósseos com retalhos de fáscia têmporo-parietal foram aplicados no tratamento de todos os pacientes do tipo III. Um (25%) paciente do tipo III foi submetido à cirurgia ortognática. Houve melhora global na aparência facial em todos os pacientes, sem complicações relacionadas aos procedimentos. CONCLUSÃO: O sistema de classificação de gravidade proposto para a SPR pode facilitar a decisão terapêutica e resultados parcialmente satisfatórios podem ser alcançados com a combinação de técnicas cirúrgicas de acordo com a gravidade da doença.

ABSTRACT

INTRODUCTION: Previous studies have shown that the public has misconceptions about the work of plastic surgeons in hand surgery. However, no specific Brazilian data on this issue are available. The objective of this study was to evaluate the public perceptions about the role of plastic surgeons in the field of hand surgery in Brazil. METHODS: Members of the Brazilian public chose one or two specialists whom they believed to be experts in eight scenarios related to hand surgery. The patterns of the responses were distributed into "plastic surgeons" or "non-plastic surgeons." RESULTS: Non-plastic surgeons were significantly (p < 0.05 for all comparisons) more recognized as experts than plastic surgeons in all scenarios related to hand surgery. CONCLUSION: The knowledge and perceptions of the Brazilian public about the work performed by plastic surgeons in the field of hand surgery are limited.

INTRODUÇÃO: Estudos prévios têm revelado que o público tem conhecimentos equivocados sobre a atuação dos cirurgiões plásticos em cirurgia da mão. No entanto, não existem dados específicos na literatura científica brasileira. O objetivo deste estudo foi avaliar as percepções do público sobre o papel dos cirurgiões plásticos no campo de cirurgia da mão no Brasil. MÉTODOS: Membros do público brasileiro escolheram um ou dois especialistas que eles acreditassem serem experts para oito cenários relacionados à cirurgia de mão. Os padrões de respostas foram distribuídos como "cirurgiões plásticos" ou "não cirurgiões plásticos". RESULTADOS: "Não cirurgiões plásticos" foram significativamente (p < 0,05 para todas as comparações) mais reconhecidos como experts que "cirurgiões plásticos" em todos os cenários relacionados à cirurgia de mão. CONCLUSÃO: Os conhecimentos e as percepções do público brasileiro sobre o trabalho realizado por cirurgiões plásticos no campo cirurgia da mão são limitados.

ABSTRACT

INTRODUCTION: To assess the possible determinants that lead public to choose plastic surgeons as hand surgery specialists. METHODS: General public members (n = 701) were asked to choose one or two specialists that they perceived to be an expert in 11 hand surgery-related scenarios. Bivariate and multivariate analyses were applied to assess the possible determinants (socio-demographic data, source of reported information, and previous plastic surgery contact) of public choice of plastic surgeons as experts in the hand surgery-related scenarios. RESULTS: A significantly (all p < 0.05) poor understanding of the role of plastic surgeons was seen in infectious hand injury, hand tumor, hand fracture, hand tendon injury, carpal tunnel syndrome, rheumatoid arthritis deformity, and dupuytren contracture. Age was a significant (all p < 0.05) determinant of plastic surgeon as a response pattern. CONCLUSION: Participants' age was a determinant of public choose plastic surgeons as experts in hand surgery area.

INTRODUÇÃO: O objetivo deste estudo foi avaliar os possíveis determinantes para o público escolher os cirurgiões plásticos como especialistas em cirurgia de mão. MÉTODOS: Membros do público (n = 701) escolheram um ou dois especialistas que eles acreditassem serem experts para 11 cenários relacionados à cirurgia de mão. Análises bivariadas e multivariadas foram aplicadas para avaliar os possíveis determinantes (dados sociodemográficos, fontes de informações e contato prévio com a cirurgia plástica) para o público escolher os cirurgiões plásticos como especialistas em cirurgia de mão. RESULTADOS: Houve uma compreensão limitada (p < 0,05 para todas as comparações) sobre o papel dos cirurgiões plásticos em infecção da mão, tumor da mão, fratura da mão, lesão tendinosa da mão, síndrome do túnel do carpo, artrite reumatoide e contratura de Dupuytren. Apenas a idade foi um (p < 0,05 para todas as comparações) determinante significativo de cirurgião plástico como um padrão de resposta. CONCLUSÃO: A idade foi um fator determinante da escolha pública de cirurgiões plásticos como especialistas em arena de cirurgia da mão.

Formal training in scientific research increases the participation of plastic surgery residents in peer-reviewed articles

ABSTRACT

Introduction: The objectives of this study were as follows: (1) to outline a scientific research skills training program, (2) to evaluate the profile of participation of plastic surgery residents in articles, and (3) to analyze the impact of the implementation of the training program on quantitative bibliometric indexes.
Methods: This was a bibliometric analysis of the participation of plastic surgery residents of a single institution in articles published in peer-reviewed journals between 2006 and 2014. The data collected were the number of authors, position of residents among authors, article titles, indexing databases and impact factor of the journals, study design, and levels of evidence. Two periods (January 2006 to January 2010 [A] and February 2010 to February 2014 [B]) were created to study the evolutionary profile of the impact of the implementation of the training program outlined in this study.
Results: A significant predominance (p < 0.05) was observed among articles published in national journals in the Portuguese language and in the SciELO and LILACS databases, and articles without residents as corresponding author, without impact factor, without assumptions, and with a level of evidence III (retrospective studies). The inter-period comparative analysis revealed a significant increase (p < 0.05) in the numbers of published articles and residents with publications at the end of their residency, in the involvement of one or more residents, and in the articles published in English (period A < period B).
Conclusion: The implementation of a scientific research skills training program led to an increase in research activity of (peer-reviewed articles) during the residency.

RESUMO

Introdução: Os objetivos deste estudo foram: (1) delinear um programa de treinamento em habilidades de pesquisa científica, (2) avaliar o perfil da participação dos residentes de Cirurgia Plástica em artigos, e (3) analisar o impacto da implementação do programa de treinamento sobre índices bibliométricos quantitativos.
Métodos: Trata-se de uma análise bibliométrica da participação de residentes de Cirurgia Plástica de uma única instituição em artigos publicados em periódicos revisados por pares entre 2006 e 2014. Dados coletados: número de autores, posição dos residentes entre os autores, títulos, bases de indexação e fator de impacto dos periódicos, desenhos dos estudos e níveis de evidência. Dois períodos (janeiro/2006-janeiro/2010 [A] e fevereiro/2010-fevereiro/2014 [B]) foram criados para estudar o perfil evolutivo do impacto da implementação do programa de treinamento delineado neste estudo.
Resultados: Houve predomínio significativo (p < 0,05) de artigos publicados em periódicos nacionais, em língua portuguesa, nas bases de dados SciELO e LILACS, artigos sem residentes como autor correspondente, sem fator de impacto, sem hipóteses e com nível de evidência III (estudos retrospectivos). A análise comparativa interperíodos revelou um aumento significativo (p < 0,05) de artigos publicados, de residentes com publicações ao término da residência, da participação de um ou mais residentes e de artigos publicados em inglês (período A < período B).
Conclusão: A implementação do programa de treinamento em habilidades de pesquisa científica determinou um aumento da atividade de pesquisa (artigos revisados por pares) durante a residência.

ABSTRACT

BACKGROUND: To date, there is no consensus regarding the best surgical approach (conservative or radical) for craniofacial fibrous dysplasia. This study presented the experience of a single institution in the surgical treatment of craniofacial fibrous dysplasia. METHOD: This was a retrospective analysis of patients with craniofacial fibrous dysplasia who underwent surgery between 1997 and 2012. Surgical treatment was individualized according to patient age, the involved anatomical site (zones I-IV), aesthetic and/or functional impairment, and the preferences of the patients and surgical team. The surgical results were classified on the basis of the Whitaker system. RESULTS: Ten, 1, 1, and 1 patients with zone I, zone II, zone I/II, and zone I/IV involvement, respectively, were included in the study. In total, conservative surgeries and 9 radical surgeries were performed for the treatment of primary bone tumors. There was 1 surgical complication, and 6 recurrences were identified during the postoperative follow-up period. The global average of surgical outcomes, according to the Whitaker scale, was 1.69 ± 0.94. CONCLUSIONS: According to the experience and surgical results presented in this study, the surgical approach for craniofacial fibrous dysplasia should be individualized.

INTRODUÇÃO: Até o momento, não existe consenso sobre qual a melhor abordagem cirúrgica (conservadora ou radical) da displasia fibrosa craniofacial. O objetivo deste estudo foi apresentar a experiência de uma única instituição no tratamento cirúrgico da displasia fibrosa craniofacial. MÉTODO: Trata-se de uma análise retrospectiva dos pacientes com displasia fibrosa craniofacial, operados entre 1997 e 2012. O tratamento cirúrgico foi individualizado de acordo com idade, sítio anatômico envolvido (zonas I-IV), comprometimento estético e/ou funcional e preferências dos pacientes e da equipe cirúrgica. Os resultados cirúrgicos foram classificados com base no sistema de Whitaker. RESULTADOS: Dez pacientes com acometimento da zona I, um da zona II, um das zonas I e III, e um das zonas I e IV foram incluídos. Nove cirurgias conservadoras e nove cirurgias radicais foram realizadas para o tratamento de tumores ósseos primários. Houve uma complicação cirúrgica. Seis recidivas foram identificadas durante o seguimento pós-operatório. A média global dos resultados cirúrgicos, de acordo com a escala de Whitaker, foi de 1,69 ± 0,94. CONCLUSÕES: De acordo com a experiência e resultados cirúrgicos apresentados neste estudo, a abordagem cirúrgica da displasia fibrosa craniofacial deve ser individualizada.

INTRODUCTION: Previous studies have revealed that residents have misconceptions about plastic surgeons as hand surgery specialists. Until now, however, there have been no specific data on this trend in Brazilian scientific literature. Therefore, the aim of this study was to evaluate Brazilian residents' knowledge and perceptions about the role of plastic surgeons in the field of hand surgery. METHOD: Brazilian residents chose one or two specialists they believed to be experts for eight scenarios related to hand surgery. The responses were categorized as "plastic surgeons", "without plastic surgeons", "only plastic surgeons", "plastic surgeons combined with other specialists", or "plastic surgeons absent". RESULT: Responses without plastic surgeons and with plastic surgeons absent were chosen more often than all other responses in all scenarios (p<0.05 for all comparisons). Plastic surgeons were less often recognized as experts compared to orthopedic (six scenarios), general (one scenario), and neurosurgeons (one scenario) (p<0.05 for all comparisons). CONCLUSION: Brazilian residents have limited knowledge and perceptions about hand surgery performed by plastic surgeons.

RESULTADO: "Sem cirurgiões plásticos" e "cirurgiões plásticos ausentes" foram mais (p<0,05, para todas as comparações) escolhidos que todos os outros padrões de respostas, em todos os cenários. Cirurgiões plásticos foram menos (p<0,05, para todas as comparações) reconhecidos que cirurgiões ortopédicos (seis cenários), cirurgiões gerais (um cenário) e neurocirurgiões (um cenário). CONCLUSÃO: Os conhecimentos e as percepções dos residentes brasileiros sobre o trabalho realizado por cirurgiões plásticos no campo cirurgia da mão são limitados.

Formal training in two-dimensional standardized photographic documentation during residency in plastic surgery

ABSTRACT

Introduction: Plastic surgery requires diverse skills, knowledge, and competencies, and its scope is broad and challenging. Thus, all plastic surgeons go through extensive education/training, which begins during residency and continues throughout their lives. This study aims to outline a formal training program in 2-dimensional standardized photographic documentation ("photodocumentation") to complement surgical and scientific training during residency in Plastic Surgery.
Methods: The components (strategies, topics covered, skills acquisition, and feedback processes) of training in photodocumentation during residency in Plastic Surgery were detailed.
Results: Topics in the photodocumentation training program included resident activities, interactions among residents, professional photographers, and surgeons/preceptors, studio settings, cameras, lenses, principles (exposure, aperture, speed, ISO, and depth of field), lighting, preparation and positioning of patients and the camera, photoshoot conditions, photo views, photo database, image formats, computerized photogrammetry, and ethical and medical-legal issues.
Conclusion: Training in photodocumentation complements the arsenal of skills needed by residents in Plastic Surgery for continuing education, in order to better prepare them for future careers.

RESUMO

Introdução: A Cirurgia Plástica exige inúmeras habilidades, conhecimentos e competências, e seu escopo de atuação é amplo e desafiador. Dessa forma, todos os cirurgiões plásticos passam por uma extensa educação/formação, que se inicia durante a residência e continua ao longo da vida. O objetivo deste estudo foi delinear um treinamento formal em documentação fotográfica bidimensional padronizada para complementar o treinamento cirúrgico e científico durante a residência em Cirurgia Plástica.
Métodos: Os componentes (as estratégias, os tópicos abordados e os processos de aquisição de habilidades e feedback) do treinamento em documentação fotográfica durante a residência em Cirurgia Plástica foram detalhados.
Resultados: As atividades dos residentes, as interações entre os residentes o fotógrafo profissional e os cirurgiões/preceptores e os tópicos abordados (estúdio fotográfico, câmeras fotográficas, objetivas, princípios fotográficos [exposição, abertura, velocidade, ISO e profundidade de campo], iluminação, preparo e posicionamento dos pacientes e da câmera fotográfica, situações fotográficas, vistas fotográficas, banco fotográfico, formatos de imagens, fotogrametria computadorizada e questões éticas e médico-legais) do programa de treinamento em documentação fotográfica foram delineados.
Conclusão: O treinamento em documentação fotográfica complementa o arsenal de habilidades necessários aos residentes de Cirurgia Plástica em formação, com o intuito de melhor prepará-los para as carreiras futuras.
