Diced cartilage graft to increase the nasal radix

ABSTRACT

INTRODUCTION: The main complaint of patients visiting plastic surgery clinics for rhinoplasty is the nasal dorsum. Cartilage grafts are routinely used in the nasal radix to reduce inappropriate nasofacial angles or redefine the transition point of the radix and then lengthen or shorten the nose. The aim of this study is to present cartilage nasal radix grafts prepared with diced cartilage and introduced to the radix with the aid of a syringe. METHOD: We evaluated the surgical procedure and the results obtained by analyzing the medical records of 37 patients who underwent this surgery between May 1, 2011 and May 1, 2013. RESULTS: In all cases, we achieved the expected result of successfully increasing the nasal radix. No graft absorption was clinically detected. CONCLUSION: The use of a diced cartilage graft as described in this study effectively increased the nasal radix.

RESUMO

INTRODUÇÃO: A principal queixa dos pacientes que procuram as clínicas de cirurgia plástica para serem submetidos a rinoplastia é o dorso nasal. A utilização de enxerto cartilaginoso em radix nasal é rotineira e suas indicações habituais são para diminuir o ângulo nasofacial inadequado ou para redefinir o ponto de transição do radix, para alongamento ou encurtamento do nariz. Este trabalho tem o intuito de apresentar a enxertia de cartilagem do radix nasal, com cartilagem fragmentada e introduzida até o radix com seringa. MÉTODO: Foi analisado o ato cirúrgico e seu resultado através da revisão de prontuários dos pacientes submetidos ao ato cirúrgico no período de 1/maio/2011 a 1/maio/2013, totalizando 37 casos. RESULTADOS: Em todos os casos, conseguiu-se o resultado planejado de aumento do radix nasal com sucesso. Não foi observada absorção clinicamente detectável do enxerto. CONCLUSÃO: A utilização do enxerto de cartilagem fragmentada realizada da forma descrita neste trabalho mostrou-se eficaz para a elevação do radix nasal.

Nasal reconstruction with costal cartilage

ABSTRACT

Introduction: Among the most autogenous materials used in nasal reconstruction, the septal cartilage is the ideal donor, because often provides material of good quality and easily accessible. But their use is limited in cases of major deformities and even in the simplest cases where the amount of cartilage is limited by factors such as large deviations of the cartilaginous septum or prior septoplasty. Aloplastic implants should be an exception. Cartilage is more resistant to resorption and also have lower rates of extrusion. Objective: To evaluate the safety and durability of the results of rhinoplasty with the use of costal cartilage. Method: Thirty patients were operated with nasal deformities with the following inclusion criteria: patients of both sex who had nasal deformity that was necessary the use of multiple cartilaginous transplants for its correction; patients with age between 18 and 60 years. The exclusion criteria were: patients with impossibility to medical-surgical procedure, with changes in preoperative examinations and high surgical risk. Results: From April 2004 to July 2007, were selected consecutively 30 patients with an indication correction of nasal deformity requiring multiple cartilaginous grafts. There were no anesthetic or cardiorrespiratory complications, deep vein thrombosis, perforations of viscera, infection, or other higher. Conclusion: The technique proved to be safe and durable and excellent results.

RESUMO

Introdução: Entre os materiais autógenos mais utilizados na reconstrução nasal, a cartilagem septal é a fonte doadora ideal, pois geralmente fornece material de boa qualidade e fácil obtenção. Porém seu uso é limitado em casos de grandes deformidades e mesmo nos casos mais simples em que a quantidade de cartilagem é limitada por fatores como grandes desvios do septo cartilaginoso ou septoplastia prévia. Qualquer substância aloplástica para aumentar o nariz deve ser instrumento de exceção. Cartilagens são mais resistentes à reabsorção e também possuem menores índices de extrusão e reabsorção. Objetivo: Avaliar a segurança e a durabilidade dos resultados das rinoplastias com o uso de cartilagem costal. Método: Foram operados 30 pacientes com deformidades nasais que preenchessem os seguintes critérios de inclusão: pacientes de ambos os sexos que apresentassem deformidade nasal necessitando de enxertos cartilaginosos múltiplos para sua correção; pacientes entre 18 e 60 anos de idade. Foram considerados os seguintes critérios de exclusão: pacientes com contra-indicações clínico-cirúrgicas ao procedimento indicado, com alterações nos exames pré-operatórios e com risco cirúrgico elevado. Resultados: No período de abril de 2004 a julho de 2007, foram selecionados consecutivamente 30 pacientes com indicação de correção de deformidade nasal necessitando de múltiplos enxertos cartilaginosos. Não foram encontradas complicações anestésicas ou cardiorrespiratórias, trombose venosa profunda, perfurações de vísceras, infecção ou outras maiores. Conclusão: A técnica provou ser segura, com resultados duradouros e excelentes.

Pyriform plasty associated with open rhinoplasty

ABSTRACT

INTRODUCTION: Mouth or mixed breathing since childhood that is due to allergy factors causing turbinate hypertrophy and adenoids, or traumatic septal deviation at childbirth or first trauma in childhood leads to the impairment of facial development, including ogival palate, excessive growth of the maxillary crest, changes in the dental arch, and small jawbone. Mouth or mixed breathing may be due to the lack of aeration of the paranasal sinuses. A septum with limited space for growing pushes the osteocartilaginous framework cranially, originating from the nasal hump with septal deviations and gibbus. Consequently, this hampers nasal breathing. METHOD: Pyriform plasty by incision in the gingivolabial sulcus allows broad access to the maxillary crest and ogival palate. The maxillary crest is worn out with a drill, and the palate is modeled or may have its dome subperiosteally removed. RESULTS: Open rhinoplasty facilitates the approach and fixation of graft reamers to treat changes in the internal valve and prevents scar reactions at the level of the cage cartilage grafts and structure of the columella to project and prevent a cartilaginous scar retraction at this level. The open approach also enables the symmetrization of nasal structures, which allows the treatment of bent nose at a single time, and correction of nasal valve by using external grafts. Moreover, pyriform plasty is a more comprehensive approach to nasal aesthetic and functional nasal alterations. CONCLUSION: Our preferred techniques are open rhinoplasty, given the ease of graft fixation, and use of reamer grafts to treat changes in the internal valve to prevent future respiratory problems.

RESUMO

INTRODUÇÃO: A respiração bucal ou mista desde a infância, por fatores alérgicos que causam hipertrofia de cornetos e adenoides ou desvio septal traumático no parto ou por traumas na primeira infância, provoca incorreto desenvolvimento da face com o palato ogival, crescimento excessivo da crista maxilar, alterações na arcada dentária e hipomaxilismo, por falta de aeração dos seios paranasais. O septo com limitado espaço para de crescer empurra o arcabouço osteocartilaginoso cranialmente, originando a giba nasal com desvios do septo, comprometendo a respiração nasal. MÉTODO: A piriformeplastia mediante incisão no sulco gengivolabial permite amplo acesso à crista maxilar e ao palato ogival. A crista maxilar é desgastada com broca e o palato é modelado ou pode ter sua cúpula removida subperiostealmente. RESULTADOS: A rinoplastia aberta facilita a abordagem e a fixação dos enxertos alargadores para tratar as alterações da válvula interna e prevenir retrações cicatriciais ao nível da gaiola cartilaginosa, além dos enxertos estruturais na columela para projeção e evitar retração cicatricial neste nível. A abordagem aberta permite também a simetrização das estruturas nasais, que possibilita o tratamento das laterorrinias num tempo único e a correção da válvula nasal externa por meio dos enxertos específicos. A piriformeplastia permite ainda a abordagem mais abrangente das alterações nasais estéticas e funcionais. CONCLUSÃO: A rinoplastia aberta é por nós preferida dada a facilidade de fixação dos enxertos, bem como dos enxertos alargadores para tratamento das alterações da válvula interna com o objetivo de prevenir futuros problemas respiratórios.

Correction of low or saddle nasal dorsum with a composite graft of fragmented conchal cartilage fixed to the perichondrium wrapped in mastoid fascia

ABSTRACT

INTRODUCTION: In many cases, augmentation rhinoplasty is more difficult than reduction rhinoplasty. Solid dorsal grafts performed with costal cartilage have been widely used for dorsal augmentation; however, they are associated with high rates of revision. Thus, many authors began to use cartilage cut into cubes wrapped in fascia. The mastoid fascia, connected to the perichondrium of the auricular conchal cartilage can form a composite graft to augment the nasal dorsum, which is also a treatment option. The objective is to demonstrate the possibility of using fragmented auricular conchal cartilage fixed to its perichondrium and wrapped in mastoid fascia to form a composite graft for augmentation of the nasal dorsum. METHODS: This is a retrospective study of 9 patients who underwent operation between 2012 and 2016 at the Base Hospital of the Faculty of Medicine of São José do Rio Preto, in which the nasal dorsum was augmented with fragmented conchal cartilage fixed to its perichondrium and wrapped in the mastoid fascia. RESULTS: The patients were followed up for up 6 to 48 months. They were questioned about their satisfaction with the nasal procedure and hearing sensitivity, and provided a positive evaluation of the surgeons. CONCLUSION: The conchal cartilage seems a highly valuable alternative graft for nasal dorsum augmentation procedures. The technique of using cartilage wrapped in mastoid fascia seems to be an advantageous alternative when compared with those using cartilage wrapped in other fasciae: it has low morbidity and complications rates and can be a great option for saddle nose treatment.

RESUMO

INTRODUÇÃO: A rinoplastia de aumento em muitos casos torna-se mais difícil que a rinoplastia de redução. Enxertos dorsais sólidos realizados com cartilagem costal têm sido muito utilizados para aumento dorsal, porém estão associados com altos índices de revisão, por isso, muitos autores passaram a utilizar cartilagem em cubos envoltos por fáscia. A fáscia da mastoide, conectada ao pericôndrio da cartilagem conchal auricular, pode formar um enxerto composto para o aumento do dorso nasal, sendo também uma opção de tratamento. O objetivo é demonstrar a possibilidade do uso de cartilagem da concha auricular fragmentada fixa ao seu pericôndrio, e envoltos na fáscia da mastoide, formando um enxerto composto para aumento do dorso nasal. MÉTODOS: Trata-se de um estudo retrospectivo de 9 pacientes operados entre 2012 e 2016 no Hospital de Base da Faculdade de Medicina de São José do Rio Preto, em que foi realizado aumento do dorso nasal com cartilagem conchal fragmentada fixa ao seu pericôndrio e envolto à fáscia da mastoide. RESULTADOS: Os pacientes foram acompanhados de 6 a 48 meses. Foram questionados quanto à satisfação do procedimento nasal e sensibilidade auricular, com avaliação positiva dos pacientes e cirurgiões. CONCLUSÃO: A cartilagem conchal parece ser uma alternativa de grande valia para procedimentos de aumento de dorso nasal. Esta cartilagem envolta com fáscia da mastoide parece ser uma alternativa vantajosa em comparação ao uso de outras fáscias, com baixa morbidade e taxa de complicações, podendo ser uma grande opção para tratamento do nariz em sela.

Increased nasal tip projection with "pseudo-flaps" using excess lateral cephalic alar cartilage

ABSTRACT

Introduction: An increased nasal tip projection is sometimes necessary to achieve an appropriate proportion between nasal tip and dorsum. Numerous techniques and tactics have been described for this purpose using cartilaginous grafts obtained from the nasal septum, auricular concha, and costal cartilage. When this increased projection must be discrete, the use of excess lateral alar cartilage in the form of "pseudo-flaps" is proposed.
Methods: In primary open rhinoplasty, excess alar cartilage, which is generally removed, was used to produce "pseudo-flaps". The cartilages were folded over themselves in the form of a "French soldier's suspender" over the domes of the alar cartilage and supported by interdomal soft tissue padding raised over the domus. It was kept detached, and relocated to the nasal tip and was contained by "pseudoflaps" of the alar cartilages sutured there or covering the columella's structural graft. Thirty-six patients underwent surgery using this technique.
Results: Thirty-five had good results and one had a nasal tip abscess, caused by endonasal exposure to a non-absorbable suture, which was removed. A second intervention was then performed using a new auricular graft, but the result was still unsatisfactory. The "pseudoflaps" method is relatively simple for those performing nasal surgery.
Conclusion: The nasal tip can be projected discretely using the excess of alar cartilage "pedicled" in the domus.

RESUMO

Introdução: O aumento da projeção da ponta nasal às vezes se torna necessário para a obtenção de uma boa proporção entre ela e o dorso. Inúmeras técnicas e táticas são descritas com essa finalidade utilizando enxertos cartilaginosos obtidos do septo nasal, concha auricular e cartilagem costal. Quando esse aumento deve ser discreto é proposto o uso dos excedentes de cartilagens alares laterais em forma de "pseudo-retalhos".
Métodos: Em rinoplastias abertas primárias os excedentes das cartilagens alares, geralmente removidas, são utilizados como "pseudo-retalhos", dobrados sobre si mesmos, em forma de "suspensório de soldado francês", sobre o domus das cartilagens alares, tendo como acolchoamento de apoio os tecidos moles delas próprias, e o tecido mole interdomal, geralmente desprezado, que é liberado, e elevado para sobre os domus. Ele é mantido, descolado e deslocado para a ponta nasal, e fica contido pelos "pseudo-retalhos" das cartilagens alares ali suturados ou cobrindo o extremo do enxerto estrutural da columela. Foram operados com essa tática 36 pacientes.
Resultados: 35 com bons resultados e um apresentou um abcesso de ponta nasal, provocado pela exposição endonasal de um fio de sutura não absorvível, que foi removido. Houve necessidade de uma segunda intervenção, utilizando novo enxerto auricular, ainda com resultado insatisfatório. O método é relativamente simples para quem opera narizes.
Conclusão: A ponta nasal pode ser discretamente mais projetada utilizando os excessos de cartilagens alares, "pediculadas" no domus.

Preoperative and postoperative assessment of external nasal valve in rhinoplasty

ABSTRACT

INTRODUCTION: The external nasal valve is located on the rim of the nostrils and is composed of soft tissues and cartilage. Any imbalance between these structures always leads to external nasal valve insufficiency, which may be partial or total, depending on the degree of alteration. The external nasal valve was evaluated before and after rhinoplasty to assess the efficiency of the technique used. METHODS: This is a retrospective study that included 34 patients operated at private hospitals. The collapse of the nasal alae during deep inspiration and the vector of the lower lateral cartilage were evaluated. To provide more structure to the nasal alae, the lateral crural strut graft and/or alar contour graft were used. RESULTS: The inappropriate vector of the lower lateral cartilage is related to the external nasal valve insufficiency (p = 0.006), which was corrected with grafts providing nasal alae remodeling in most of the cases (p = 0.006). A significant difference was observed in the frequency of using grafts in primary (66%) and secondary rhinoplasty (80%). CONCLUSION: The inappropriate vector of the lower lateral cartilages usually resulted in an unstructured nasal alae, presenting external nasal valve insufficiency. Structuring the nasal alae with a lateral crural strut graft and/or alar contour graft was proven effective to correct external nasal valve insufficiency in 90% of the cases and to provide better aesthetic proportions and nasal contour. The frequency of grafts used in secondary rhinoplasty (80%) was higher than that in primary (66%), which indicates the need for grafts in a more complex surgery.

RESUMO

INTRODUÇÃO: A válvula nasal externa está localizada no rebordo das narinas e é composta por tecidos moles e cartilagem. Caso haja algum desequilíbrio entre estas estruturas, invariavelmente ocorrerá insuficiência desta válvula nasal externa, podendo ser parcial ou total, dependendo do grau das alterações. Avaliou-se a válvula nasal externa no pré e pós-operatório em rinoplastia verificando a eficiência da técnica utilizada. MÉTODOS: Estudo retrospectivo de 34 pacientes operados em hospitais particulares. Foram avaliados o colabamento da asa nasal durante a inspiração profunda e o vetor das cartilagens laterais inferiores. Para estruturação da asa nasal, utilizou-se o enxerto de suporte da cruz lateral e/ou enxerto de contorno alar. RESULTADOS: O vetor não adequado da cartilagem lateral inferior está relacionado com a insuficiência da válvula nasal externa (p = 0,006) e a estruturação da asa nasal, mediante o uso de enxertos, corrigiu esta afecção na maioria dos casos (p = 0,006). Houve diferença na frequência de utilização de enxertos na rinoplastia primária (66%) e secundária (80%). CONCLUSÃO: O vetor inadequado das cartilagens laterais inferiores geralmente resultou em uma asa nasal desestruturada, com insuficiência da válvula nasal externa. A estruturação da asa nasal com enxerto de suporte da cruz lateral e/ou enxerto de contorno alar se mostrou eficaz na correção da insuficiência da válvula nasal externa em 90% dos casos, além de conferir proporções e contornos mais belos ao nariz. A frequência de utilização dos enxertos na rinoplastia secundária (80%) foi maior do que na primária (66%) e nos mostrou a necessidade dos enxertos numa cirurgia mais complexa.

Efficacy of structured and conventional techniques on aesthetic and corrective rhinoplasty

ABSTRACT

INTRODUCTION: Rhinoplasty, like other plastic surgery subspecialties, has greatly evolved in recent decades, as both a surgical technique and a diagnosis of the alterations that need to be performed. The objective of the current study was to evaluate the efficacy of the structured and conventional surgical techniques on nasal aesthetic and corrective approaches. METHODS: A total of 49 patients, who underwent surgical operation by the author of this work, were retrospectively studied. The patient generally underwent one of the two surgical techniques. The efficacy of each technique was evaluated by means of the critique provided by the patients and the surgeon regarding the results, and the ability to reach the pre-operative goals. RESULTS: Both techniques were efficient in attaining the final objective of the surgery. Patient and surgeon satisfaction were similar in both cases, and there were no major complications in any of the groups. CONCLUSION: The present work showed that both the structured and the conventional techniques were efficient in the treatment of nasal alterations. The most important factor for selecting the best approach in each case was the correct pre-operative diagnosis of the required alterations.

RESUMO

INTRODUÇÃO: A rinoplastia, como outras subespecialidades da Cirurgia Plástica, apresentou evolução muito grande nas últimas décadas, tanto em termos de técnica operatória quanto em diagnóstico das alterações a serem tratadas. O objetivo deste trabalho é avaliar a eficácia das técnicas Estruturada e Convencional na abordagem estética e reparadora do nariz. MÉTODOS: Foram estudados de forma retrospectiva 49 pacientes operados pelo autor do trabalho, que foram submetidos a uma das duas técnicas operatórias. A eficácia das técnicas foi avaliada a partir da crítica dos pacientes e do cirurgião com relação aos resultados e à capacidade de se chegar ao objetivo do pré-operatório. RESULTADOS: Ambas as técnicas se mostraram eficazes para alcançar o objetivo final da cirurgia. A satisfação dos pacientes e do cirurgião foram semelhantes nos dois casos, e não houve complicações maiores em nenhum dos grupos. CONCLUSÃO: O trabalho mostrou que tanto a técnica Estruturada quanto a Convencional são eficazes para o tratamento das alterações nasais, sendo o mais importante o correto diagnóstico pré-operatório das alterações para a escolha da melhor abordagem em cada caso.

Perception of nasal reduction by radix and tip augmentation rhinoplasty

ABSTRACT

INTRODUCTION: A straight bridge has always been the aesthetic ideal. Simple hump removal, the classical and most commonly applied method, can have aesthetic and functional consequences. However, great resistance to augmentation procedures persists because most patients request reduction and the benefits of improving nasal balance are counterintuitive. An augmented nose can look smaller, a particular benefit in patients with thick, inelastic skin or a large lower nose. On the other hand, decreased size perception after raising of the radix and tip is very common but has not been measured to date. METHODS: This study created graphic and real interventions to achieve a straight bridge through radix and tip raising and analyzed how patients and independent observers perceive these changes. A sample of 42 sequential rhinoplasty patients was analyzed, including nine cases of primary surgery and dorsal convexity. RESULTS: There was a 6.5% mean augmentation after graphic computing intervention but a perception of size reduction (p = 0.004). There was a 1% mean augmentation after rhinoplasty and an overall size reduction perception. CONCLUSION: Correction of the nasal dorsum, making a straight bridge through slightly increasing radix and tip, creates the perception of a decreased nose size.

INTRODUÇÃO: O dorso reto tem sido sempre um ideal na rinoplastia estética. A simples remoção da giba tem sido o método clássico e mais utilizado, mas pode ter consequências estéticas e funcionais adversas. Ainda existe grande resistência a procedimentos de aumento, porque a maioria dos pacientes solicitam redução e porque os benefícios de melhoria do equilíbrio nasal pelo aumento não são intuitivos. Um nariz aumentado pode parecer menor, o que é um benefício em particular em pacientes com pele espessa ou com o aspecto de terço inferior grande. Por outro lado, a percepção de redução com o aumento do radix e da ponta é muito comum, embora nunca tenha sido medida. MÉTODOS: Esse estudo cria intervenções gráficas e reais para criar um dorso reto por meio do aumento do radix e da ponta e analisa como os pacientes e observadores independentes percebem as modificações. Analisou-se uma amostra de 42 casos consecutivos de rinoplastia. Desses, foram incluídos os que tinham dorso convexo e eram cirurgias primárias, restando 9 casos. RESULTADOS: Houve aumento médio de 6,5% no tamanho do nariz na após a modificação gráfica, enquanto houve percepção de redução do nariz (p = 0,004). Houve aumento médio de 1% após a rinoplastia, enquanto houve percepção de redução. CONCLUSÃO: A retificação do dorso nasal pelo aumento do radix e da ponta causa percepção de redução do nariz.

Parenthesis-shaped deformity of the alar cartilages: a retrospective analysis of 49 cases of treatment by caudal rotation of the lateral crura

INTRODUCTION: Alar cartilage cephalic malposition was first described as a parenthesis-shaped deformity by Sheen. The aim of this study was to analyze the results of treatment by caudal rotation of the lateral crus for this deformity. METHOD: A retrospective study was conducted with 49 cases, operated on between July 2011 and January 2013 at the 2nd Infirmary of the Santa Casa da Misericórdia in Rio de Janeiro and at a private clinic. RESULTS: Most of the patients (76% of cases) had a severe form of this deformity. All others presented with a moderate form (24% of cases). Fifty seven percent of the patients (57%) had a "boxy tip" and an initial alar pinching, 61% showed alar retraction and 8%, external valve collapse. The "boxy tip" was corrected in 100% of cases; initial alar retraction, 97%; alar pinching, 96%; and external valve collapse, 100%. The 3 cases of unilateral relapse were corrected by the same technique during a second rhinoplasty. CONCLUSIONS: This study shows that caudal rotation of the lateral crus can treat the deformity described by Sheen with a small number of relapses and without serious complications.

RESUMO

INTRODUÇÃO: Sheen descreveu pela primeira vez uma má posição cefálica das cartilagens alares como a «deformidade em forma de parênteses». O objetivo deste trabalho foi analisar os resultados do tratamento desta deformidade pela rotação caudal da cruz lateral. MÉTODO: Foi realizado estudo retrospectivo, com 49 casos, operados entre Julho 2011 e Janeiro 2013 na 2ª Enfermaria da Santa Casa da Misericórdia do Rio de Janeiro e em uma clínica particular. RESULTADOS: A maioria dos pacientes (76% dos casos) tinha a forma severa desta deformidade, os outros apresentavam uma forma moderada (24% dos casos). Cinquenta e sete por cento dos pacientes tinham um "boxy tip" e um piçamento alar inicial, 61% apresentavam uma retração alar e 8% um colapso da válvula externa. O «boxy tip" foi corrigido em 100% dos casos, a retração alar inicial foi corrigida em 97% dos casos, o pinçamento alar em 96% e o colapso da válvula externa foi tratado em 100% dos casos. Os 3 casos de recidiva unilateral foram corrigidos pela mesma técnica durante uma segunda rinoplastia. CONCLUSÕES: Este trabalho mostra que a rotação caudal da cruz lateral permite tratar a deformidade descrita por Sheen com poucas recidivas e sem complicações graves.

ABSTRACT

Introduction: Nasal reconstruction is the oldest plastic surgery technique. The nasal anatomy is complex and requires an association of techniques for the restoration of function and adequate nasal esthetics. Pereira et al. described a technique that allows total nasal reconstruction of the alar cartilage through the use of an auricular cartilage graft, with minimal deformity secondary to the donor site. The objective of the present study is to present a modification, by Collares et al., of the technique described above, which allows the reconstruction of another anatomical region of the nose without increasing morbidity, and its insertion into the total nasal reconstruction protocol of Hospital de Clínicas of Porto Alegre.
Methods: A retrospective study was conducted. We evaluated technique modification in 10 patients who underwent total nasal reconstructions.
Results: After examining the 10 patients who were treated with the modified total nasal reconstruction protocol at the Hospital de Clínicas of Porto Alegre, we observed an improvement in the nose shape and internal nasal valve with preservation of function, without sequelae secondary to auricular graft removal.
Conclusion: In this case series, the modification of the Max Pereira technique resulted in adequate aesthetic-functional treatment when implemented in the total nasal reconstruction protocol of the Hospital de Clínicas of Porto Alegre, without increasing the morbidity in the donor area.

RESUMO

Introdução: A reconstrução nasal é a mais antiga das cirurgias plásticas. A anatomia nasal é complexa e necessita de uma associação de técnicas para a restauração da função e estética nasal adequada. Pereira et al. descreveram uma técnica que possibilita a reconstrução nasal total da cartilagem alar, com o uso de um enxerto da cartilagem auricular, com mínima deformidade auricular secundária à retirada do enxerto. O objetivo deste trabalho é apresentar uma modificação da técnica acima descrita, que possibilita reconstruir mais uma região anatômica do nariz, sem aumentar a morbidade, realizada por Collares et al., e a sua inserção no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre.
Métodos: Foi realizado um estudo retrospectivo. Avaliou-se a inserção da modificação da técnica em 10 pacientes que realizaram reconstrução nasal total.
Resultados: Após a análise dos 10 casos, utilizando a modificação da técnica inserida no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre, encontramos uma melhoria da forma do nariz, a válvula nasal interna com preservação da função e sem sequelas secundárias à retirada do enxerto auricular.
Conclusão: Nesta série de casos, a modificação da técnica de Max Pereira resultou em tratamento estético-funcional adequado quando implementada no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre, sem aumentar a morbidade na área doadora.

Anatomy of lower lateral nasal cartilage in African descendants: a cadaveric study

Introduction: Aesthetic deformity of the nose and impaired nasal functioning may be related to structural variations in the lower lateral cartilage (LLC) and these variations can be either congenital or acquired. Few studies have investigated LLC morphology, and even fewer have investigated LLC morphology among African descendants. To meet this research gap, this study conducted an in-depth examination of LLC anatomy and its variation among a sample of African descendants. Methods: The noses of 16 fresh cadavers were dissected through exorhinoplasty to collect data regarding the shape, length, and width of the lateral and middle/medial crus, variations in these parameters in terms of the right and left sides and gender, and distance between the domus and the caudal septum for analysis using BioEstat 5.0 software. Results: Convex shape of the lateral crus was found to be predominant among the samples. Only 12.5% of the samples were found to have a unilateral concave shape and 6.25% to have a bilateral concave shape. The feet of the medial crus were found to be united in 81.25% of the samples and separated in 18.75%. The distance between the angle of the caudal septum and the domus was found to range from 4 to 10 mm (mean 6.86 ± 1.75 mm) among the samples. Conclusion: The parameters of LLC morphology among African descendants measured in this study were found to be similar to those previously described in the literature.

Introdução: São poucos os estudos na literatura que descrevem a morfologia das cartilagens laterais inferiores, sendo ainda mais raros em populações afrodescendentes. O objetivo desse estudo foi descrever a anatomia das cartilagens laterais inferiores e suas variações em afrodescendentes. Método: Foram estudados 16 cadáveres frescos. Os narizes foram dissecados através de exorinoplastia, com identificação e liberação das conexões das cartilagens laterais inferiores. As cruras laterais e cruras médias/mediais foram analisadas quanto a forma, comprimento e largura, sendo identificadas quanto ao lado e sexo. A distância entre o dômus e o septo caudal foi avaliada. Foi utilizada uma régua milimetrada para as mensurações. As medidas encontradas foram registradas e analisadas com auxílio do software BioEstat 5.0. Resultados: A forma convexa das cruras laterais foi predominante; enquanto a forma côncava ocorreu unilateralmente em dois casos e bilateralmente em um. Os "pés" da crura mediais estavam unidos em 81,25% da amostra e separados em 18,75% dos casos. A distância entre o ângulo do septo caudal e o dômus foi de 4 a 10 mm (média = 6,86 ± 1,75 mm). Conclusão: As cartilagens laterais inferiores dos afrodescendentes apresentaram dimensões semelhantes aos descritos na literatura.

ABSTRACT

Introduction: The nasal tip is a frequent object of study by surgeons, since it is a common reason for pre- and postoperative complaints. Thus, the tip should be thoroughly evaluated to determine the best technique to be used. The objective is to demonstrate an original alar cartilage flap technique for projection of the nasal tip, performed alone or in combination with other procedures for the same purpose.
Method: A retrospective cross-sectional study of 22 patients submitted to open rhinoplasty with use of the alar flap to project the nasal tip.
Results: A total of 22 open rhinoplasties were performed on primary bulbous, fatty, and under-projected tips. Among the patients in the study, 90% were black and 2% were white; 72.7% reported a high degree of satisfaction with the operation, 18.1% considered the result very good, and 9.2% considered the result acceptable. There were no complications/reoperations.
Conclusion: The alar cartilage flap for projection of the nasal tip is simple to perform and can be considered an excellent supplemental method for use in treatment of the black nose.

Ideal positioning of the lower lateral cartilages associated with the optimization of the external nasal valve

Introduction: The Lower Lateral Cartilages (LLC) positioning is directly related to good nasal functionality. When these cartilages have cephalic malpositioning, the lateral wall of the external nasal valve is not adequately supported, which can lead to valvular insufficiency. The objective is to define the ideal anatomical positioning of the LLC associated with optimizing the external nasal valve.
Methods: Review narrative literature in the following databases: SciELO, LILACS and Medline. The descriptors used were: "nasal cartilages,"; "nasal obstruction," and "rhinoplasty," being selected 15 essential articles for the understanding of the subject. Literature review: Positioning the lateral crura of the LLC forms the contour of the nasal tip and provides stability to the lateral wall of the external nasal valve. Constantian defined the ideal positioning of the lateral crura of the LLC at the margin of the nasal alae should be 45° or less. For Toriumi, the angle is measured from the lateral crura of the LLC concerning the midsagittal plane, and the appropriate value is approximately 45°. For Silva, the positioning of the LLC is measured by the angle of divergence between the LLCs, and its appropriate value is approximately 90°.
Conclusion: The external nasal valve works better when the LLCs are well-positioned, namely: the angle formed between the lateral edge of the LLCs and the alar margin is close to 45° or less; the angle formed between the LLC and the midsagittal plane is close to 45°; the divergence angle formed between the LLC is close to 90°.

Introdução: O posicionamento das Cartilagens Laterais Inferiores (CLI) está diretamente relacionado à boa funcionalidade nasal. Quando essas cartilagens apresentam um mau posicionamento cefálico, a parede lateral da válvula nasal externa fica sem suporte adequado, podendo levar à insuficiência valvular. O objetivo é definir qual o posicionamento anatômico ideal das CLI associado à otimização da válvula nasal externa.
Métodos: Revisão de literatura narrativa nas seguintes bases de dados: SciELO, LILACS e Medline. Os descritores utilizados foram: "cartilagens nasais"; "obstrução nasal" e "rinoplastia", sendo selecionados 15 artigos essenciais para o entendimento do assunto. Revisão de literatura: O posicionamento do ramo lateral das CLI forma o contorno da ponta nasal e dá estabilidade à parede lateral da válvula nasal externa. Constantian definiu que o posicionamento ideal do ramo lateral das CLI à margem da asa nasal deve ser 45° ou menos. Para Toriumi, o ângulo é mensurado a partir do ramo lateral das CLI em relação ao plano sagital mediano e o valor adequado é de aproximadamente 45°. Para Silva, o posicionamento das CLI é mensurado pelo ângulo de divergência entre as CLI e tem como valor apropriado aproximadamente 90°.
Conclusão: A válvula nasal externa apresenta melhor funcionamento quando as CLI estão bem posicionadas, a saber: o ângulo formado entre a borda lateral das CLI e a margem alar é próximo de 45° ou menos; o ângulo formado entre as CLI e o plano sagital mediano é próximo de 45°; o ângulo de divergência formado entre as CLI é próximo a 90°.

Single block costal cartilage graft in rhinoplasty

ABSTRACT

Introduction: It is often necessary to use inclusion materials in rhinoplasty for nose restructuring. The costal cartilage graft is one of the inclusion material options, and its use is indicated when septal cartilage is not available or is not sufficient to provide necessary remodeling of the nose. Methods: Four patients who, for diverse reasons, had saddle nose or poor projection of the nasal dorsum underwent rhinoplasty. All received a costal cartilage graft carved in an "L" shape. The long branch of the "L" was used to reconstruct the dorsum and the short branch was used to support the new dorsum and the columella, repositioning the nasal tip. Results: All patients had a good postoperative evolution and had no significant complications. There was a clear reconstruction of the dorsum and harmonization of the nasal tip. The results were as expected and were long-lasting. Conclusions: The use of a costal cartilage graft carved in a single block in rhinoplasty provides enhancement of the nasal dorsum and tip projection, promoting appropriate nasal remodeling.

Introdução: Na rinoplastia frequentemente é necessário o uso de materiais de inclusão para que se obtenha a reestruturação nasal. O enxerto de cartilagem costal é uma das opções e tem seu uso indicado quando a cartilagem septal não está disponível ou não é suficiente para promover o necessário remodelamento nasal. Método: Quatro pacientes que, por motivos diversos, apresentavam nariz em sela ou dorso nasal pouco projetado foram submetidos a rinoplastia. Todos receberam enxerto de cartilagem costal esculpido em formato de "L". O ramo longo do "L" teve como objetivo reconstituir o dorso e o ramo curto, dar suporte ao novo dorso e à columela, reposicionando a ponta nasal. Resultados: Todos os pacientes evoluíram bem no pós-operatório e não apresentaram complicação significativa. Houve nítida reconstituição do dorso e harmonização da ponta do nariz. Os resultados mostraram-se previsíveis e duradouros. Conclusões: O emprego de enxerto de cartilagem costal esculpido em monobloco na rinoplastia proporciona ganho de projeção do dorso e da ponta nasais, promovendo adequado remodelamento nasal.

Anatomia artística e opções técnicas na rinosseptoplastia

RESUMO

INTRODUÇÃO: A prática da rinosseptoplastia envolve uma percepção adequada e objetiva da "deformidade", uma adequada avaliação funcional do nariz, e uma idealização da beleza, baseada na anatomia artística, e no contexto das expectativas de cada paciente. MÉTODOS: O autor apresenta os mais frequentes aspectos da anatomia artística, relevantes para a rinoplastia, e as técnicas, táticas, e atitudes cirúrgicas que têm possibilitado a convergência, entre a referida anatomia e a obtenção de resultados estética e funcionalmente adequados na sua prática. RESULTADOS: Os narizes com alterações mínimas na ponta foram submetidos à rinoplastia convencional, enquanto os que necessitaram maiores intervenções foram submetidos a rinoplastia aberta, ambos com resultados igualmente satisfatórios. CONCLUSÃO: A anatomia artística tem sido uma importante contribuição ao sucesso das rinosseptoplastias, tanto na abordagem fechada como na aberta.

The bundle of twigs cartilage graft for augmentation of the nasal dorsum

ABSTRACT

INTRODUCTION: Autologous grafts play an important role in rhinoplasty, they provide support with improved functional and aesthetic results. Fragmented cartilaginous grafts are an alternative that help avoid the perceptible deformities that occur with sculpted grafts. Grafts in the form of unbound strips have also shown good results. This study demonstrated the effectiveness of a new technique, which involves filling of the nasal dorsum using cartilaginous strips bound in the form of a bundle of twigs. METHOD: A total of 28 open structure rhinoplasties were performed from January to June 2015 at the Plastic Surgery Clinic of the Felício Rocho Hospital. V-shaped infracolumellar incisions were made by dissection of the deep planes. The nasal septum was the main donor area of the cartilage grafts. The grafts were prepared with a #11 scalpel blade into thin strips with an average thickness of 1 mm. Using simple 5-0 catgut wire, the grafts were bound into a self-contained bundle. RESULTS: Thirteen patients were female and 15 male, with a mean age of 34.8 years. The "bundle of twigs" technique displayed good postoperative results, with maintenance of the filling and elevation of the nasal dorsum. Follow-up assessments confirmed the augmentation of the root and dorsum in 100% of the cases, without local complications or need for surgical revision. CONCLUSION: The bundle of twigs technique was efficient and easily performed. It offers an attractive alternative for treatment of the nasal dorsum and root in aesthetic, functional, and traumatic cases.

INTRODUÇÃO: Os enxertos autólogos conquistaram importante papel na rinoplastia gerando sustentação com melhor resultado funcional e estético. Os enxertos cartilaginosos fragmentados surgiram como alternativa para evitar deformidades perceptíveis que ocorrem com enxertos esculpidos. O uso de enxertos em forma de filetes não fixados entre si demonstrou bons resultados. Este estudo objetiva divulgar nova técnica de preenchimento de dorso nasal utilizando filetes cartilaginosos amarrados em forma de "feixe de gravetos". MÉTODO: Foram realizadas 28 rinoplastias abertas estruturadas de janeiro a junho de 2015 na Clínica de Cirurgia Plástica do Hospital Felício Rocho. Realizou-se incisão infracolumelar em V e dissecção em planos profundos. A principal área doadora de enxerto cartilaginoso foi o septo nasal. Os enxertos foram preparados com lâmina 11, em longas e finos filetes com espessura média de 1 mm. Com fio catgut simples 5-0, os enxertos foram agrupados em um feixe autossustentável. RESULTADOS: Treze pacientes eram do gênero feminino e 15 do masculino. A idade média foi 34,8 anos. A técnica do feixe de gravetos demonstrou bons resultados no per e pósoperatório com manutenção do preenchimento e elevação do dorso nasal. O aumento da raiz e do dorso foi obtido em 100% dos casos, sem complicação local ou necessidade de revisão cirúrgica no seguimento atual. CONCLUSÃO: O enxerto de cartilagem em feixe de gravetos mostrou-se eficiente e de fácil execução. Oferece boa forma no tratamento do dorso e raiz nasal em casos estéticos, funcionais e traumáticos.

RESUMO

Rinoplastia é uma das cirurgias estéticas mais realizadas no Brasil, mas apresenta riscos reais de complicações. Ressecções do dorso nasal mal-sucedidas podem gerar problemas funcionais ou estéticos. Realizamos uma revisão de literatura acerca das técnicas essenciais para uma ressecção segura do dorso nasal. Dentre as mais publicadas, ressecções compostas ou por componentes foram as duas grandes vertentes, cada uma apoiada por nomes de peso da rinoplastia mundial. A rotina padronizada em cinco passos pareceu ser ideal dentro de um ambiente de ensino devido à ressecção óssea gradual realizada por raspas e pela reprodutibilidade do método de separação dos componentes do dorso nasal com uma abordagem individualizada de cada estrutura, sendo efetiva e segura ao mesmo tempo.

Nasal morphology - harmony and proportion applied to rhinoplasty

ABSTRACT

The study of nasal morphology is of fundamental importance for a better understanding and planning of rhinoplasty. The objective of this study is to analyze the basic shape of the nose seen as an isolated structure, and especially in equilibrium with the patient's face. The existing patterns to do so are quite subjective, since the concept of beauty depends on several factors. The classic proportion measurements assist the surgeon with the conception and aesthetic view. When combined with an understanding of the anatomy and surgical technique they determine the final outcome of rhinoplasty. Thus, the concept of "optimal" nose must be applied on an individual basis, respecting classical anthropometry patterns, gender, ethnic-racial, cultural and psychosocial factors. Morphology is not limited only to the static aspect of the nose, but is directly related to personal dynamics, mimicry and personality of the patient.

RESUMO

O estudo da morfologia nasal é de fundamental importância para um melhor entendimento e planificação das rinoplastias. O objetivo deste trabalho é analisar a forma básica do nariz visto como estrutura isolada, e principalmente em equilíbrio com a face do paciente. Os padrões determinados são bastante subjetivos, pois o conceito de beleza depende de vários fatores. As medidas clássicas de proporção auxiliam na concepção e visão estética do cirurgião. E quando associadas ao conhecimento da anatomia e técnica cirúrgica irão determinar o resultado final da rinoplastia. Logo, o conceito do nariz "ideal" deve ser aplicado para cada um, respeitando os padrões clássicos da antropometria, o sexo, grupo étnico-racial, fatores culturais e psicossociais. A morfologia não se restringe apenas ao aspecto estático do nariz, mas relaciona-se diretamente com a dinâmica pessoal, a mímica e a personalidade.

RESUMO

A rinoplastia continua a ser um dos procedimentos cirúrgicos estéticos mais comumente realizados. O contorno de um nariz visualmente atrativo é criado a partir de linhas, sombras e luzes que recobrem o dorso, ponta e base nasal. O controle da anatomia de superfície nasal requer conhecimento amplo das estruturas subjacentes e de técnicas cirúrgicas capazes de modificá-las a favor da estética nasal.
