Fistula after primary palatoplasty: consensus among plastic surgery and Speech-Language Pathology

ABSTRACT

Introduction: The identification of complications of primary palatoplasty may vary among professionals from different areas of health due to the lack of standardization of the fistula classification. This study aimed to verify the consensus among professionals of plastic surgery (PC) and Speech-Language Pathology (SLP), regarding the occurrence of fistula, according to what was reported in the same craniofacial service.
Methods: Analysis of the chart's records of the areas of the PC and SLP of 466 patients with cleft lip and palate was performed about the presence and location of fistula about the presence and location of fistulas, as reported in 466 medical records of patients with a history of unilateral cleft lip and palate. To compare the findings between both areas, a gold standard classification for the occurrence of fistula (GSF) was established by an experienced plastic surgeon.
Results:
The PC area reported that 25% of the 466 patients had a fistula compared to the 37% reported by the SLP, while the GSF indicated fistula in 35% of the cases. The Kappa statistic reveals regular agreement between GSF and PC (r = 0.32) and substantial agreement between GSF and SLP (r = 0.63).
Conclusion: There was a discrepancy between the areas of Speech-Language Pathology and plastic surgery regarding the occurrence and location of the fistula after primary palatoplasty in the same craniofacial center. The data indicates the need to create and implement a standardized fistula classification system. In this way, craniofacial teams can use it effectively, taking advantage of the scientific evidence that emerges from the results of cleft lip and palate treatment.

RESUMO

Introdução: A identificação de complicações da palatoplastia primária pode variar entre profissionais de diferentes áreas da saúde, devido à falta de padronização da classificação de fístula. Este estudo teve o objetivo de verificar o consenso entre profissionais da cirurgia plástica (CP) e da fonoaudiologia (FGA), quanto à ocorrência de fístula, conforme reportado em um mesmo serviço craniofacial.
Métodos: Foi realizada uma análise dos registros das áreas da CP e FGA quanto à presença e localização de fístulas, conforme reportado em 466 prontuários de pacientes com história de fissura transforame incisivo unilateral. Para comparar os achados entre ambas as áreas uma verificação padrão ouro da ocorrência de fístula (POF) foi estabelecida por um cirurgião plástico experiente.
Resultados: A área da CP reportou que 25% dos 466 pacientes apresentaram fístula comparado à 37% reportado pela FGA, enquanto o POF indicou fístula em 35% dos casos. Estatística Kappa revela concordância regular entre POF e CP (r=0,32) e concordância substancial entre POF e FGA (r=0,63).
Conclusão: Observou-se discordância entre as áreas da fonoaudiologia e da cirurgia plástica quanto à ocorrência e localização da fístula após a palatoplastia primária, em um mesmo centro craniofacial. Os dados apontam para a necessidade da criação e da implementação de um sistema de classificação de fístula padronizado, que possa ser utilizado de forma efetiva por equipes craniofaciais favorecendo evidências científicas dos resultados do tratamento da fissura labiopalatina.

Closure of an oronasal fistula with a myomucosal upper lip flap

ABSTRACT

INTRODUCTION: Palatal fistula is the most common complication after palatoplasty, and its presence entails various implications. Its presentation is diverse and repair can be difficult, which is reflected in the existence of a numerous surgical techniques described for its treatment. OBJECTIVE: To report the surgical correction of palatal fistula with a myomucosal upper lip flap, along with a brief review of the literature. CASE REPORT: The patient underwent repair of complete cleft palate. At the immediate postoperative time, an anterior fistula of the hard palate and alveolar process was present and was subjected to a correction with an oronasal myomucosal upper lip flap. The patient progressed satisfactorily, without complications or lesion recurrence after 1 year of follow-up. CONCLUSION: The technique presented is a simple and efficient method for correction of an anterior palatal fistula.

RESUMO

INTRODUÇÃO: A fístula palatina é a complicação mais frequente após palatoplastias e sua presença traz diversas implicações. Sua apresentação é diversificada e seu reparo pode ser difícil, o que se traduz na existência de uma diversidade de técnicas cirúrgicas descritas. OBJETIVO: Relatar a correção cirúrgica de fístula palatina anterior com retalho miomucoso labial superior, além de fazer uma breve revisão da literatura. RELATO DE CASO: Paciente submetido à correção de fissura palatina completa, apresentando, no pós-operatório mediato, fístula anterior de palato duro e processo alveolar, submetido à correção da fístula palatina oronasal com retalho miomucoso de lábio superior. O paciente evoluiu satisfatoriamente, sem complicações e sem recidiva da lesão após um ano de seguimento. CONCLUSÃO: A técnica em questão mostrou-se simples e eficiente, prestando-se à correção da fístula palatal anterior.

Primary palatoplasty using the von Langenbeck technique: surgical experience and aesthetic results of 278 cases

ABSTRACT

Introduction: Palatoplasty with elevated bilateral mucoperiosteal flaps using the von Langenbeck technique associated with intravelar veloplasty is a common procedure with low rates of oronasal fistula (ONF) and velopharyngeal insufficiency. The objective is to present the author's surgical experience and the incidence of ONF among 278 patients who underwent primary palatoplasty using the von Langenbeck technique associated with intravelar veloplasty.
Methods: This retrospective study analyzed the medical records of 278 patients who underwent primary palatoplasty at the Mário Covas Treatment Center for Craniofacial Malformations of the Guilherme Álvaro Hospital located in the municipality of Santos, São Paulo, Brazil, between May 2010 and May 2018.
Results: A total of 278 primary palatoplasty procedures were performed; of them, 225 (80.9%) were performed in two surgical stages and 53 (19.1%) in one surgical stage. The study population included 182 men (65.5%) and 96 women (34.5%). The prevalence of left and bilateral cleft lip and palate was 26.3% and 27%, respectively, and the prevalence of bilateral cleft palate, and right cleft lip and palate was 37.4% and 7.6%, respectively. Sixty-one patients had ONF (21.94%), the incidence of which decreased progressively throughout the study period.
Conclusion: Primary palatoplasty, using the von Langenbeck technique associated with intravelar veloplasty, is reproducible when performed in one or two surgical stages, and considered safe when the learning curve is reached with a complication rate similar to those in the literature.

RESUMO

Introdução: A palatoplastia com elevação de retalhos mucoperiostais bipediculados pela técnica de Von Langenbeck associada a veloplastia intravelar é técnica mais utilizadas na atualidade apresentando na literatura baixa taxa de fístula oronasal e de insuficiência velofaríngea. O objetivo é apresentar a experiência acumulada do autor e avaliar a incidência de fístula oronasal após 278 casos de palatoplastia primária, pela técnica de Von Langenbeck associada a veloplastia intravelar.
Métodos: Estudo retrospectivo de 278 prontuários de pacientes submetidos à palatoplastia primária no Centro de Tratamento de Malformações Craniofaciais Mário Covas - Hospital Guilherme Álvaro - Santos/SP, entre de maio de 2010 a maio de 2018.
Resultados: 278 procedimentos de palatoplastia primária pela técnica relatada, 225 (80,9%) em duas etapas cirúrgicas e 53 (19,1%) em única etapa. Masculino 182 (65,5%) e feminino 96 (34,5%). Fissuras labiopalatais esquerda e bilaterais (26,3% e 27%, respetivamente). As fissuras palatais completas corresponderam a 37,4% e a fissura labiopalatal direita com 7,6%. 61 pacientes apresentaram fístula oronasal (21,94%) observando-se uma diminuição progressiva da incidência em cada período.
Conclusão: A palatoplastia primária pela técnica de Von Langenbeck associada à veloplastia intravelar é uma técnica reprodutível em uma ou duas etapas cirúrgicas e pode ser considerada segura quando alcançada uma adequada curva de aprendizado apresentando um índice de complicações acorde com a literatura mundial.

Incidence of oronasal fistulas after palatoplasties

ABSTRACT

The aim of this study was to evaluate the characteristics of a group of patients suffering from congenital cleft palate, its treatment and the incidence of oronasal fistulas. A retrospective analysis of 98 patients operated on by six plastic surgeons from March 2002 to February 2003 is included. Fifty-one male and 47 female patients with a median age of 34 months were submitted to the surgery. The mean follow-up was 10 months. The length of the cleft was described using the classification by Spina and Veau. The variables observed were age, gender, surgical technique, surgeon and complications. For statistic analysis the Chi-square test of association was utilized. The most common approaches were the Bardach and Veau techniques. Complications occurred in 20.4% of the cases with oronasal fistulas occurring in 15.3% of the palatoplasties. This complication rate gave a significant difference when compared with the length of the cleft (p=0.006). No significant difference was seen in respect to the other variations considered. Our rate of fistulas is similar to that reported in the literature and shows that the different techniques utilized are acceptable in the treatment of cleft palate.

RESUMO

O objetivo deste estudo foi avaliar as características de um grupo de pacientes com fissura palatina congênita, seu tratamento e a incidência de fístulas oronasais. Inclui uma análise retrospectiva de 98 pacientes, operados por seis cirurgiões plásticos, no período de março/2002 a fevereiro/2003. Foram 51 pacientes do sexo masculino e 47 do feminino, idade mediana de 34 meses e acompanhamento médio de 10 meses. A extensão da fissura foi descrita pela classificação de Spina e Veau. As variáveis observadas foram: idade, sexo, técnica cirúrgica, cirurgião e complicações. Na análise estatística utilizou-se o teste Qui-quadrado de associação. As técnicas mais utilizadas foram a de Bardach e a de Veau. As complicações incidiram em 20,4% dos pacientes, sendo que a fístula oronasal ocorreu em 15,3% das palatoplastias. Esta taxa de complicação obteve diferença significativa quando relacionada à extensão da fissura (p=0,006); não havendo diferenças quando relacionada a outras variáveis. Nossa incidência de fístula encontra-se equiparada à literatura e mostra que as diversas técnicas utilizadas são válidas no tratamento da fenda palatina.

Lateral thoracic fasciocutaneous flap for reconstruction of axillary defects after resection of hidradenitis suppurativa: a series of 10 cases

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic follicular inflammation that presents varied clinical features, from isolated small nodules to several abscessed lesions, with formation of fistulas and scars. The axillary region is one of the most frequently affected areas. The objective is to evaluate a series of patients with HS in the axillary region who underwent extensive surgical excision and reconstruction with a lateral thoracic fasciocutaneous flap. METHODS: A retrospective analysis of the medical records of patients who underwent treatment between 2010 and 2012 was conducted. RESULTS: Ten patients were operated, of whom two had bilateral involvement, totaling 12 procedures. The mean progression time before the surgical indication was 38 months. The mean defect size after the resection was 10 x 9 cm. The mean size of the flaps was 15 x 10 cm. With regard to early complications, two cases of dehiscence (axillary region) and one case of partial epitheliolysis (distal segment of the flap) were observed. CONCLUSION: We observed that dissection of lateral thoracic fasciocutaneous flaps is not technically difficult and does not present significant functional or aesthetic sequelae in the donor area, making it a reliable and versatile option for reconstruction of larger axillary defects.

RESUMO

INTRODUÇÃO: A hidradenite supurativa (HS) é uma inflamação folicular crônica que apresenta quadro clínico variado, desde pequenos nódulos isolados até diversas lesões abscedadas, com formação de fístulas e cicatrizes. A região axilar é uma das áreas mais frequentemente acometidas. O objetivo é analisar uma série de casos acometidos por HS na região axilar, submetidos à exérese cirúrgica ampliada e reconstrução com retalho fasciocutâneo torácico lateral. MÉTODOS: Análise retrospectiva dos prontuários dos pacientes submetidos a tratamento no período entre 2010 e 2012. RESULTADOS: Dez pacientes foram operados, sendo que em dois havia acometimento bilateral, totalizando 12 procedimentos. Foi observado um tempo médio de evolução antes da indicação cirúrgica de 38 meses. O defeito médio observado após a ressecção foi de 10 x 9 cm. O tamanho médio dos retalhos foi de 15 x 10 cm. Em relação às complicações precoces, foram observados dois casos de deiscência (região axilar) e um caso de epiteliólise parcial (segmento distal do retalho). CONCLUSÃO: Observamos que a dissecção deste retalho não é tecnicamente difícil e que não apresenta sequelas funcionais ou estéticas significativas na área doadora, sendo uma opção confiável e versátil para reconstrução de defeitos axilares de maior porte.

Fistulas and dehiscences in primary palatoplasty: an institutional experience

ABSTRACT

Objective: The study endpoint was to analyze the occurrence of fistulas and dehiscences after primary palatoplasty performed in patients of the Instituto de Cirurgia Plástica Santa Cruz (ICPSC). Methods: Fifty-five patients underwent to this procedure between 1998 and 2005 were retrospectively observed. Results: The Von Langenbeck technique was performed in 22 (43.1%) cases, being the average age of 48 months, followed for the technique of Bardach in 15 (29.5%) cases, with average age of 24 months, Furlow technique in 11 (21.6%) cases, with average age of 34 months. The Von Langenbeck technique was the most used in the incomplete palate clefts (59.2%). Bardach technique was the most used in the complete labiopalatal cleft (54.1%). There were observed fistulas in the technique of Von Langenbeck, Furlow and Bardach, respectively, in 22.7%, 18.2% and 39.9%. The incidence of dehiscences in the techniques of Von Langenbeck, Furlow and Bardack was 13.5%, 9.1%, and 13.3% respectively. Conclusion: Thus, it had a correlation of the incidence of fistulas with the used technique having been observed bigger incidence in the Bardach technique, precocious age at time of time of surgery and complete labiopalatal cleft.

Objetivo: O objetivo deste trabalho foi analisar a ocorrência de fístulas e deiscências pós-palatoplastia primária em pacientes operados no Instituto de Cirurgia Plástica Santa Cruz (ICPSC). Método: Foram avaliados, de forma retrospectiva, 55 pacientes operados entre 1998 e 2005. Resultados: A técnica de Von Langenbeck foi realizada em 22 (43,1%) casos, com a idade média de 48 meses, seguida pela técnica de Bardach em 15 (29,5%) casos, com idade média de 24 meses, Furlow em 11 (21,6%) casos, com idade média de 34 meses. A técnica de Von Langenbeck foi a mais empregada nas fissuras pós-forame (59,2%). A técnica Bardach foi a mais utilizada nas fissuras transforame completa (54,1%). Observou-se a presença de fístulas nas técnicas de Von Langenbeck, Furlow e Bardach, respectivamente, em 22,7%, 18,2% e 39,9%. A incidência de deiscência nas técnicas Von Langenbeck, Furlow e Bardach, a incidência foi de 13,5%, 9,1% e 13,3%, respectivamente. Conclusão: Houve maior incidência de fístulas na técnica Bardach, em pacientes operados em idade mais precoce e fissuras transforame completas.

Correction of palatine fistulas with a musculo-mucosal buccinator flap: results of 6 cases after 27 years

ABSTRACT

Introduction: The buccal musculo-mucosal patch, described in 1989, can be used to correct palatine fistulas and fissures with stretching of the soft palate, or to cover bloody areas after tumor resection.
Methods: This is an analysis of the 27-year postoperative results for 6 patients who underwent operation at Base Hospital and Santa Casa de São José do Rio Preto between 1984 and 1989, and reassessed in 2016, when a myo-buccinator mucosa was used for cleft palate correction.
Results: Of the 36 operated cases, 6 were reevaluated after 27 years, of which 5 had primary correction and 1 had a secondary correction (fistula after cleft palate closure). All the cases had satisfactory results in terms of maxillary growth, correction of the palatine fistula, and speech function.
Conclusion: Although not statistically significant, the present study demonstrated that the buccal musculo-mucosal flap is an adequate procedure for correction and stretching of the palate, with normal or near-normal maxillary growth and practically normal speech even without adequate phono-audiological treatment.

RESUMO

Introdução: O retalho miomucoso de músculo bucinador, descrito em 1989, pode ser utilizado para corrigir fístulas palatinas, fissuras com alongamento do palato mole ou cobrir áreas cruentas após ressecções de tumores.
Métodos: Trata-se da análise do resultado após 27 anos de 6 casos de pacientes operados no Hospital de Base e na Santa Casa de São José do Rio Preto, no período de 1984 a 1989, e reavaliados em 2016, nos quais foram realizados retalhos miomucosos de bucinador para correção de fissura palatina.
Resultados: Dos 36 casos operados, 6 foram reavaliados após 27 anos, dos quais 5 trataram-se de correção primária e 1 de correção secundária (fístula após fechamento de fissura palatina). Todos os casos obtiveram resultados satisfatórios no crescimento maxilar, na correção da fistula palatina e na função da fala.
Conclusão: Apesar de estatisticamente não significativo, o presente estudo demonstrou que o retalho miomucoso de músculo bucinador para correção e alongamento do palato é um procedimento adequado, com resultados de crescimento maxilar normal ou próximo disso e fala praticamente normal, mesmo sem adequado tratamento fonoaudiológico.

Brosco-Dutka classification system for palate fistulas

ABSTRACT

Introduction: The occurrence of post-palatoplasty oronasal fistula (ONF) is undesirable, challenging and difficult to classify complications. The objective is to present a classification protocol for palate fistula based on the fistula's morphological, embryological criteria and symptomatology.
Methods: The elaboration of the classification involved the following steps: definition of ONF; definition of anatomical references; establishment of embryological and morphological criteria; inclusion of symptomatology.
Discussion: The established protocol includes strategies for identifying anatomical references of complex visualization such as foramen (FI) and the transition area between the hard and soft palate. From the point of view of embryology, the fistula can be classified as PREFI (located in the region before the FI), POSFI (located in the region after the FI) and PREPO (which affects both the region before and after the FI). The morphological criterion establishes as areas: region-1: prealveolar and/or the alveolar arch; region-2: hard palate before FI; region-3: hard palate after FI; region-4: transition between hard and soft palate; and region-5: soft palate. Symptom identification includes hypernasality, ear infections and nasal reflux, in addition to asymptomatic fistulas. Obtaining adequate intraoral photographs facilitates the protocol's applicability, and the positioning for the photographic image requires the visualization of the palatal face of the upper incisor teeth.
Conclusion: The Brosco-Dutka protocol for the classification of palate fistula was developed for use by the craniofacial team during a face-to-face consultation or photographic image analysis. The proposal presents illustrations to guide the proper use of the criteria.

RESUMO

Introdução: A ocorrência de fístula oronasal (FON) pós-palatoplastia é uma complicação indesejável, desafiadora e de difícil classificação. O objetivo é apresentar um protocolo de classificação de fístula de palato baseado em critérios morfológicos, embriológicos e sintomatologia da fístula.
Métodos: A elaboração da classificação envolveu as seguintes etapas: definição de FON; definição de referências anatômicas; estabelecimento de critérios embriológicos e morfológicos; inclusão da sintomatologia.
Discussão: O protocolo estabelecido inclui estratégias para a identificação de referências anatômicas de complexa visualização como forame incisivo (FI) e a área de transição entre o palato duro e mole. Do ponto de vista da embriologia, a fístula pode ser classificada como PREFI (localizada em região anterior ao FI), POSFI (localizada em região posterior ao FI) e PREPO (que acomete tanto a região anterior quanto posterior ao FI). O critério morfológico estabelece como áreas: região-1: pré-alveolar e/ou do arco alveolar; região-2: palato duro anterior ao FI; região-3: palato duro posterior ao FI; região-4: transição entre palato duro e mole; e região-5: palato mole. A identificação de sintomas inclui: hipernasalidade, otites e refluxo nasal, além das fístulas assintomáticas. A obtenção de fotografias intraorais adequadas facilita a aplicabilidade do protocolo, sendo que o posicionamento para imagem fotográfica requer a visualização da face palatina dos dentes incisivos superiores.
Conclusão: O protocolo Brosco-Dutka de classificação de fístula de palato, foi elaborado para uso pela equipe craniofacial em consulta presencial ou durante análise de imagens fotográficas. A proposta apresenta ilustrações para nortear o uso adequado dos critérios.

Introduction: Data mining techniques expand access to important information for the decision-making process during health care. The objective the study proposes using data mining techniques to identify variables (surgical treatment protocols, patient characteristics, post-surgical complications) associated with fistulas after primary palatoplasty in patients with unilateral transforamen incisor cleft (UTIC).
Method: A data set of 222 patients with UTIC without syndromes, operated by four surgeons with Furlow's or von Langenbeck's primary palatoplasty techniques, was analyzed for this study. Two models for detecting the outcome of surgery were induced using data mining techniques (Decision Tree and Apriori).
Results: Five rules were selected from a decision tree pointing to some variables as predictors of fistulas associated with primary palatoplasty: infection, cough, hypernasality, and surgeon. Analysis of the model indicates that it correctly classifies 95.9% of occurrences between the absence and presence of fistulas. The second model indicates that the absence of post-surgical complications (infection and fever) and normal speech results (absent hypernasality, without suggestive of velopharyngeal dysfunction) are related to the absence of fistulas. Regarding surgical procedures, the Furlow technique and the Vomer flap were more frequent in patients with fistulas.
Conclusion: Data mining techniques, as applied in the present study, pointed to infection and cough, hypernasality, and surgeon and surgical techniques as predictors of fistulas related to primary palatoplasty.

RESUMO

Introdução: As técnicas de mineração de dados ampliam o acesso a informações importantes para o processo de tomada de decisão durante os cuidados com a saúde. O objetivo do estudo propõe a utilização de técnicas de mineração de dados para identificar variáveis (protocolos de tratamento cirúrgico, características do paciente, intercorrências pós-cirúrgicas) associadas à ocorrência de fístulas após palatoplastia primária em pacientes com fissura transforame incisivo unilateral (FTIU).
Método: Um conjunto de dados de 222 pacientes com FTIU sem síndromes, operados por quatro cirurgiões com as técnicas de palatoplastia primária de Furlow ou von Langenbeck, foi analisado para este estudo. Dois modelos para detecção do resultado da cirurgia foram induzidos usando técnicas de mineração de dados (Árvore de Decisão e Apriori).
Resultados: Cinco regras foram selecionadas de uma árvore de decisão apontando para algumas variáveis como preditivas de fístulas associadas à palatoplastia primária: infecção, tosse, hipernasalidade, cirurgião. A análise do modelo indica que ele classifica corretamente 95,9% das ocorrências entre ausência e presença de fístulas. O segundo modelo indica que a ausência de intercorrências pós-cirúrgicas (infecção e febre) e resultado de fala normal (hipernasalidade ausente, sem sugestivo de disfunção velofaríngea) estão relacionados à ausência de fístulas. Em relação aos procedimentos cirúrgicos, o uso da técnica de Furlow e retalho de Vomer foram mais frequentes nos pacientes com fístulas.
Conclusão: Técnicas de mineração de dados, conforme aplicadas no presente estudo, apontaram para infecção e tosse, presença de hipernasalidade, cirurgião e técnica cirúrgica como preditores de fístulas relacionadas à palatoplastia primária.

Single layer closure of hard palate cleft and oronasal fistula

ABSTRACT

INTRODUCTION: Traditionally, cleft of the hard palate is repaired in two layers, with a nasal mucosal layer and an oral mucoperiosteal layer. The aim of this study was to evaluate the results of one layer closure of hard palate cleft compared to the traditional two layers closure. METHODS: The charts of 101 consecutive cases of repair of hard palate cleft performed by the authors from 1981 to 2012 at a tertiary care clinic/hospital in central Wisconsin were reviewed. The cases utilized the single layer closure and were followed in the Cleft Palate Clinic on a yearly basis. Cases included unilateral and bilateral cleft lip and palate as well as isolated cleft palate. RESULTS: All cases healed satisfactorily except for two cases that later required small fistulae repair. CONCLUSION: Single layer closure of the hard palate cleft is as effective as traditional two-layer closure, with minimal complications.

INTRODUÇÃO: Tradicionamente, a fissura palatina é corrigida em duas camadas - uma camada mucosa nasal e camada mucoperiosteal oral. Este estudo avaliou os resultados do fechamento em camada única de fissura palatina comparado ao fechamento tradicional em camada dupla. MÉTODOS: Tratase de revisão de pronturários de 101 casos de correção de fissura palatina realizados entre 1981 e 2012 em uma clínica assistencial/hospital terciário localizado no centro de Wisconsin. Os casos utilizaram fechamento em camada única e foram acompanhados em Clínica de Lábio Leporino por 12 meses. Foram incluidas fissura labial e palatina também como fissura palatina isolada. RESULTADOS: Todos os casos apresentaram cicatrização satisfatória exceto dois casos que necessitaram de correção posterior de pequena fistula. CONCLUSÃO: O fechamento em camada única de fissura palatina é tão efetivo quanto o fechamento tradicional em camada dupla, além disso apresenta mínimas complicações.

Use of the microsurgical anterolateral thigh flap to correct esophagocutaneous fistula

ABSTRACT

INTRODUCTION: The anterolateral thigh (ALT) flap has become an important tool in the microsurgical reconstruction of cutaneous and subcutaneous defects. Since the ALT flap can be up to 35 cm long, it can be used in both, esophageal reconstruction and cervical skin defects. CASE REPORT: Patient C. S. M., a 57-year-old woman, presented with squamous cell carcinoma of the larynx and underwent radiotherapy and a total laryngectomy followed by reconstruction by primary closure. However, she developed a postoperative complication due to the formation of a cervical esophagocutaneous fistula that required four surgical procedures to ensure permanent closure. The latter procedure consisted of the creation of a microsurgical ALT flap to correct the esophagocutaneous fistula, the subject of this report. CONCLUSIONS: The microsurgical ALT flap has extensive applications in various fields based on the following characteristics: reliable vascularization, long and broad vascular pedicle (at least 8 cm long), an extensive and easily delimited flap region, the possibility of reducing the primary flap thickness to 3-5 mm without compromising its vascularization, the possibility of a simultaneously dual approach because of the distance between the donor and recipient site, and the possibility of primary closure without the need for skin grafting. The case report presented in this study emphasizes the possibility of the use of a microsurgical ALT sandwich flap to correct a high output esophagocutaneous fistula.

RESUMO

INTRODUÇÃO: O retalho anterolateral da coxa (RALC) vem se tornando uma ferramenta importante na reconstrução microcirúrgica de defeitos cutâneos e subcutâneos. Tendo em vista que o RALC pode se estender por até 35 cm, ele pode ser utilizado tanto na reconstrução esofágica como em defeitos cutâneos cervicais. RELATO DO CASO: Paciente C.S.M., sexo feminino, 57 anos, apresentando um carcinoma espinocelular de laringe foi submetida à radioterapia e tratamento cirúrgico de laringectomia total seguido de reconstrução por fechamento primário. Porém, a paciente evoluiu com uma complicação pósoperatória devido à formação de uma fístula esôfago-cutânea cervical, sendo necessária a realização de 4 procedimentos cirúrgicos para fechamento definitivo da fístula. O último procedimento consistiu na realização de retalho microcirúrgico da região anterolateral da coxa para correção da fístula esôfago-cutânea, objeto desse relato. CONCLUSÕES: O retalho microcirúrgico anterolateral da coxa possui extensa aplicação em diversos campos, baseada nas seguintes características: vascularização confiável, pedículo vascular longo e largo de no mínimo 8 cm, região do retalho extensa e de fácil delimitação, possibilidade de diminuir a espessura primária do retalho em 3 a 5 mm sem risco de comprometer sua vascularização, possibilidade de abordagem dupla simultânea devido à distância entre sítio doador e receptor, possibilidade de fechamento primário sem a necessidade de enxerto de pele. O relato do caso apresentado neste estudo justifica-se com o intuito de ressaltar a possibilidade da utilização microcirúrgica do RALC em sanduíche na correção de fístula esôfago-cutânea de alto débito.

ABSTRACT

The iatrogenic injuries of the parotid gland after ritidectomia are uncommon and have incidence around 2%. In the present article, a case is reported, followed by the solution found by the authors for the resolution of fistula.

RESUMO

As lesões iatrogênicas da glândula parótida pós-ritidectomia são incomuns e têm incidência aproximada de 2%. No presente artigo, é relatado um caso com a solução encontrada pelos autores para a resolução da fístula.
