ABSTRACT

Introduction: The reverse flow sural artery flap is an important skin flap that can be used for reconstruction of tissue defects of the distal third of the leg, heel and proximal plantar region. It is a versatile flap with a wide arc of rotation that varies from 90º-180º and has an axial pattern based on reverse flow of superficial sural artery and main venous drainage by the short saphenous vein. Method: Twenty-two patients were operated with ages ranging from 12 to 71 years and 19 cases due to motorcycle accident. Results: Five (22%) flaps had parcial necrosis and one (4.1%) showed total loss. Split thickness skin on donor site was done in 18 (82%) cases.

RESUMO

Introdução: O retalho sural de fluxo reverso é um importante retalho fasciocutâneo, que pode ser usado para a reconstrução de perda de substância do terço distal da perna, calcâneo e região plantar proximal. É um retalho versátil, com amplo arco de rotação, que varia de 90º-180º e apresenta padrão axial baseado no fluxo arterial reverso da artéria sural superficial ramo da artéria fibular e drenagem venosa principal por tributárias surais da veia safena parva. Método: Foram operados 22 pacientes, com idade variando de 12 a 71 anos, sendo 19 casos devido a acidente automobilístico. Resultados: Cinco (22%) retalhos apresentaram necroses parciais e um (4,1%) apresentou necrose total. Enxertia em área doadora foi realizada em 18 (82%) casos.

Anatomical basis for the use of the fibularis tertius muscle in myocutaneous flaps

ABSTRACT

BACKGROUND: Myocutaneous flaps have been increasingly used in surgical reconstruction of the lower limbs, requiring the use of muscles that result in less functional and esthetic damage as flaps. This study aimed to evaluate the use of the fibularis tertius muscle (in terms of frequency and anatomy) as flaps in this procedure. METHODS: Sixty-four lower limbs from preserved cadavers were dissected and evaluated based on the following parameters: proximal insertion, distal insertion, syntopy, morphology, and morphometry. RESULTS: The fibularis tertius muscle was detected in 96.9% of the study cases. Most proximal insertions (96.8%) were found at the interosseous membrane, anterior border of the fibula, and anterior intermuscular septum. Most distal insertions (77.4%) were found at the lateral and dorsal sides of the 5th metatarsal. Mean value of muscle belly length was 17.89 cm and width was 1.95 cm. The mean length of the distal tendon with no muscle fibers was 1.2 cm, and the mean width was 0.45 cm. CONCLUSIONS: The fibularis tertius muscle is frequent and has a distinct morphology, making it a viable option for the repair of lower limb (distal segment) defects.

RESUMO

INTRODUÇÃO: A utilização de retalhos miocutâneos é cada dia mais frequente nas cirurgias plásticas reconstrutoras de membros inferiores, tornando-se essencial a utilização de músculos que denotem menor prejuízo tanto funcional como estético. Foram estudados a frequência e os aspectos anatômicos do músculo fibular terceiro, com o intuito de avaliar, sob esses aspectos, a possibilidade de seu uso nesses procedimentos. MÉTODO: Foram dissecados 64 membros inferiores de cadáveres fixados e verificadas as seguintes características: inserção proximal, inserção distal, sintopia, morfologia e morfometria. RESULTADOS: A presença do músculo foi constatada em 96,9% dos casos, sendo analisados os aspectos supracitados. A inserção proximal mais frequente (96,8%) ocorreu na membrana interóssea, na margem anterior da fíbula, e no septo intermuscular anterior. A inserção distal mais comum (77,4%) foi nas faces dorsal e lateral do 5º metatarsiano. O valor médio do comprimento e da largura do ventre muscular foi, respectivamente, de 17,89 cm e 1,95 cm, enquanto a média do comprimento do tendão distal livre de fibras musculares foi de 1,2 cm e a largura média do tendão distal, de 0,45 cm. CONCLUSÕES: O músculo fibular terceiro é frequente, de morfologia distinta, que, sob aspectos morfométricos, se constitui em opção viável para um estudo mais específico de seu uso no reparo de defeitos no segmento distal do membro inferior.

The use of soleus muscle flap in musculocutaneus losses of the middle third of lower leg

ABSTRACT

Introduction: Lower limb damaged areas are open doors to infection, to the loss of plasma, blood, electrolytes and fluids, in addition to being responsible for intense pain. Therefore it is necessary to cover them as soon as possible so that the patient can be stabilized preventing early complications and future sequels. Muscle and musculocutaneous flaps are used to repair these areas. Over the years several treatments with flaps have been suggested for injuries repairment, among them the soleus muscle flap, the focus of this work. Objective: To describe the author's experience using the rotation of the soleus muscle. Methods: Eighteen patients with loss of substance of the medial third of the leg were treated by using the rotation of the soleus muscle, from 2008 to 2010. Results: The results were satisfactory and without major complications. Conclusion: This is an option to treat lesions caused by various etiologies such as ulcers, osteomyelitis, loss of cutaneous substance of third middle leg, and to cover skin lesions with exposure of osteotendinous structures, becoming a great assistance tool to the plastic surgeon.

RESUMO

Introdução: Áreas lesionadas de membros inferiores são portas abertas à infecção, às perdas de plasma, sangue, eletrólitos e líquidos, além de serem responsáveis por dor intensa. Por isso, é necessário cobri-las o mais rapidamente possível, para que o paciente se estabilize precocemente, prevenindo complicações e futuras sequelas. Os retalhos musculares e musculocutâneos são opções de reparo cirúrgico destas áreas. Ao longo dos anos, sugeriram vários tratamentos com retalhos para reparações de lesões, dentre eles o retalho do músculo sóleo, foco deste trabalho. Objetivo: Relatar a experiência do autor empregando a rotação do músculo sóleo. Método: Foram tratados cirurgicamente 18 pacientes com perda de substância no terço medial da perna, utilizando a rotação do músculo sóleo, no período de 2008 a 2010. Resultados: Os resultados obtidos foram satisfatórios e sem complicações maiores. Conclusão: Este procedimento é um recurso para o tratamento de lesões decorrentes de diferentes etiologias, como úlceras, osteomielite, perda de substância cutânea de terço médio da perna e cobertura cutânea em lesões com exposição de estruturas osteotendinosas, tornando-se uma ferramenta de grande auxílio ao cirurgião plástico.

Reverse anterolateral thigh flap: a reconstruction option for the lower limbs

ABSTRACT

Introduction: Cutaneous and soft tissue reconstruction in the region around the knee is often challenging for the plastic surgeon and aimed at providing good knee joint function with acceptable aesthetic appearance. Surgical planning requires considering the simplest technique that leads to wound closure, with minimal morbidity in the donor area. The literature has reported only few cases of the use of the reverse anterolateral (ALT) flap for defects in the popliteal fossa, knee, and leg proximal portion. The objective is to describe a series of cases of patients who underwent reconstruction with a reverse ALT flap for wounds in the popliteal fossa, knee, and upper third of the leg.
Methods: Among four patients, three received popliteal fossa reconstructions and one received knee and leg proximal third reconstructions. All the patients underwent wound/tumor extensive resection, dissection of the reverse ALT flap, and transposition to the defect.
Results: During follow-up for 2 to 18 months, minimal morbidity was observed in the donor area, with a total coverage of the defect, optimal functional result of the affected limb, and good aesthetic result. Only one case had a partial necrosis (15%) of the flap due to hematoma and pedicle compression. In the other patients, no evidence of flap failure, dehiscence, seroma, or infection were found.
Conclusion: The reverse ALT flap was a viable option, presenting good functional and aesthetic results for cutaneous and soft tissue reconstructions in the popliteal fossa, knee, and upper third of the leg.

RESUMO

Introdução: A reconstrução cutânea e de tecidos moles na região ao redor do joelho é frequentemente desafiadora para o cirurgião plástico e tem como objetivo fornecer uma boa função articular do joelho com aparência estética aceitável. O planejamento cirúrgico requer considerar a técnica mais simples que leve ao fechamento da ferida, com mínima morbidade na área doadora. Há poucos relatos do uso do retalho anterolateral (ALT) reverso na literatura, em se tratando de defeitos na fossa poplítea, joelho e porção proximal da perna. O objetivo é descrever uma serie de casos de pacientes submetidos à reconstrução de ferida em fossa poplítea, joelho e terço superior da perna com retalho ALT reverso.
Métodos: Quatro pacientes, dos quais 3 eram reconstruções de fossa poplítea e 1 reconstrução de joelho e terço proximal da perna. Todos submetidos à ressecção ampla da ferida/tumor, dissecção do retalho ALT reverso, e transposição para o defeito.
Resultados: Follow-up de 2 a 18 meses, evidenciando-se morbidade mínima para área doadora, com cobertura total do defeito e ótimo resultado funcional do membro afetado, com bom resultado estético. Apenas em um dos casos evidenciou-se sofrimento e necrose parcial (15%) do retalho devido a hematoma e compressão do pedículo, nos demais não foi evidenciado sofrimento dos retalhos, deiscência, seroma ou infecção.
Conclusão: O retalho ALT da coxa reverso mostrou-se uma opção viável, apresentando bom resultado funcional e estético para a reconstrução cutânea e de tecidos moles na região de fossa poplítea, joelho e terço superior da perna.

Effect of medial hemisoleus muscle flap on wound healing in the medial and distal thirds of the lower leg

ABSTRACT

Introduction: The use of many flaps has been described to repair substance loss in the medial and distal thirds of the lower leg. The main advantage of the muscle medial hemisoleus flap is preserving one half of the innervated medial soleus muscle in situ and keeping the foot's plantar flexion power. Moreover, the flap has a longer arc of rotation. As recently described, new vascular concepts and techniques have increased flap viability. This study aimed to examine the use and advantages of the medial hemisoleus flap with emphasis on an indication and complication analysis.
Methods: Over a 10-year period, nine medial hemisoleus flaps were executed to repair traumatic wounds with tibial exposure in eight patients. We describe the indications and surgical techniques in detail.
Results: Complete wound healing was achieved in all patients. There was a low donor-site complications rate. One patient suffered from partial necrosis of the extremity flap. The mean operative time was 2 h.
Conclusions: The medial hemisoleus flap is useful for repairing substance loss and provides coverage with intermediary thickened tissue, high vascularity, minimal donor-site morbidity, foot plantar flexion power preservation, faster rehabilitation, accessible surgical techniques, and a shorter operative time.

RESUMO

Introdução: Muitos retalhos são descritos para reparar as perdas de substância nos terços médio e distal da perna. A principal vantagem do retalho de músculo hemisóleo medial é a preservação da inervação da metade do músculo sóleo no sítio doador, mantendo a força de flexão plantar. Além disso, este retalho tem um arco de rotação maior quando comparado ao retalho de músculo sóleo convencional. Novos conceitos vasculares e técnicos, descritos recentemente, aumentaram a viabilidade deste retalho. O objetivo deste estudo é relatar a utilização do retalho de hemisóleo medial enfatizando as vantagens, reafirmando as indicações e analisando as complicações.
Métodos: Em dez anos, um total de nove retalhos de hemisóleo medial foram confeccionados para a reparação de feridas traumáticas com exposição da tíbia em oito pacientes. Os critérios de indicação e técnica cirúrgica são descritos detalhadamente.
Resultados: A cicatrização completa da perda de substância foi alcançada em todos os pacientes. Observou-se baixo índice de complicações no leito doador e em um paciente houve necrose parcial da extremidade do retalho. O tempo médio de realização do procedimento foi de duas horas.
Conclusões: Retalhos de hemisóleo medial são muito úteis na reparação destas perdas de substância e permitem a cobertura com tecido de espessura intermediária, rica vascularização, baixo índice de morbidade na área doadora, preservação da função motora plantar, reabilitação mais rápida no pós-operatório, técnica cirúrgica acessível e menor tempo operatório.

Coverage of the lower third of the limb and foot injuries using reverse sural fasciocutaneous flap described by Carriquiry

ABSTRACT

Introduction: Skin grafts are not effective to cover lesions in the distal third of the lower limbs that expose the bones, joints, tendons, and blood vessels due to a limited vascular bed and poor granulation of the wounds. These lesions can only be corrected with microsurgical transfer or muscle, myocutaneous, or fasciocutaneous flaps.
Methods: The lower border of the flap was marked 5 cm above the malleolus. The upper border was marked after providing sufficient length for complete coverage of the lesion. The incision was performed at the marked upper border, and the skin and subcutaneous tissue were elevated together with muscle fascia. The sural nerve was preserved in its original bed. The flap was lifted to the marked lower border (the pedicle). At this point, the flap was transposed at a sufficient angle to cover the lesion.
Results: Eight cases of surgery were conducted using the flap described above. All cases had exposed bones and tendons in the distal region of the limb, back of the foot, or both, in which the reverse sural fasciocutaneous flap with the technique proposed by Carriquiry was used. The cases showed satisfactory esthetic and functional results.
Conclusion: The used flap can correct lesions of the lower third of the limbs and foot. It is relatively easy to make, with good vascular supply, and there is no functional loss of the donor area.

RESUMO

Introdução: Lesões no terço distal dos membros inferiores, com exposição de ossos, articulações, tendões e vasos sanguíneos, não são passíveis do uso de enxertos de pele. Isto ocorre porque o leito vascular é exíguo e pela pobre granulação das feridas, podendo apenas ser corrigidas com retalhos musculares, miocutâneos, fasciocutâneos ou transferência microcirúrgica.
Métodos: O retalho em seu limite inferior é demarcado a partir de 5cm acima dos maléolos. Superiormente, é marcado num comprimento suficiente para cobertura total da lesão. Realizada incisão em demarcação prévia, e elevados pele e tecido subcutâneo juntamente com a fáscia muscular. O nervo sural é preservado em seu leito original. A elevação do retalho se dá até o ponto inferior marcado (o pedículo). Neste ponto, o retalho é transposto numa angulação suficiente para alcançar a lesão.
Resultados: Oito casos foram operados utilizando o retalho descrito. Todos apresentavam exposição de ossos e tendões em região distal da perna, dorso do pé ou ambos, nos quais foram utilizados o retalho fasciocutâneo reverso da perna com a técnica proposta por Carriquiry. Os casos apresentaram resultados estético e funcional satisfatórios.
Conclusão: O retalho utilizado se presta à correção de lesões do terço inferior da perna e do pé. É relativamente fácil de ser confeccionado, com bom suprimento vascular, e não há perda funcional do leito doador.

Versatility of the medial gastrocnemius muscle flap in reconstructing lower limb soft tissue lesions

ABSTRACT

INTRODUCTION: Covering complex lower limb wounds is a major challenge. The medial gastrocnemius muscle flap is usually indicated for reconstruction of loss of substance of the knee and proximal third of the leg. The objective of this study was to evaluate the results using gastrocnemius muscle flaps in lower limb wounds. METHODS: A total of 28 patients with loss of substance in the lower limbs were treated with medial gastrocnemius muscle flaps. Data were obtained from clinical records of patients who underwent surgery from 2003 to 2012. RESULTS: In 28 patients who underwent surgery, the age range was 4 to 57 years, with a mean of 30.6 years. The etiology of the lesions was a motorcycle accident in 16 cases (57.1%), being run over in 8 cases (28.6%), car crash in 2 cases (7.2%), meningococcemia in 1 case (3.6%), and snake bite in 1 case (3.6%). After an average follow-up of 15 months (range: 6 to 26 months), all patients had stable coverage. The mean number of preoperative debridements was 2.4 (range: 1 to 8). In 22 patients, the flaps were made without the release of the muscle origin and in 6 patients the muscle flaps were separated from their origin in the femur. CONCLUSIONS: Transfer of a gastrocnemius muscle flap is a simple and safe procedure in the treatment of lower limb injuries. The application of the medial gastrocnemius muscle flap was demonstrated in different situations, with satisfactory results and easy reproducibility.

RESUMO

INTRODUÇÃO: A cobertura de feridas complexas de membros inferiores é um grande desafio. O retalho muscular do gastrocnêmio medial é comumente indicado para reconstrução de perdas de substância do joelho e terço proximal da perna. O objetivo desse trabalho é avaliar os resultados da utilização do retalho muscular do gastrocnêmio nos casos de ferida em membros inferiores. MÉTODOS: Um total de 28 pacientes com perda de substância de membros inferiores foi tratado com retalho muscular do gastrocnêmio medial. Os dados foram obtidos em registros clínicos dos pacientes operados no período de 2003 a 2012. RESULTADOS: Vinte e oito pacientes foram operados, com idades variando de 4 a 57 anos, e média de 30,6 anos. A etiologia das lesões foi acidente por motocicleta em 16 casos (57,1%), atropelamentos em oito casos (28,6%), acidente automobilístico em dois casos (7,2%), meningococcemia em um caso (3,6%) e acidente ofídico em um caso (3,6%). Depois de um seguimento médio de 15 meses (variação de 6 a 26 meses), todos pacientes mostraram cobertura estável. O número médio de desbridamentos pré-operatórios foi 2,4 (variação de 1 a 8). Em 22 pacientes, os retalhos foram confeccionados sem a liberação da origem muscular e em seis pacientes os retalhos musculares foram separados de sua origem no fêmur. CONCLUSÕES: A transferência do retalho muscular do gastrocnêmio é um procedimento simples e seguro no tratamento de lesões de membros inferiores. Foi demonstrada a aplicação do retalho muscular do gastrocnêmio medial em diferentes situações, com resultados satisfatórios e fácil reprodução.

Skin coverage of the anterior region of the knee with an anterolateral reverse flow thigh flap based on the perforating artery of the transverse branch of the lateral femoral circumflex artery: a case report

ABSTRACT

The patella is an uncommon place for the appearance of bone tumors, benign or malignant, and pseudotumoral lesions. In the literature, there is a small amount of work on this topic. This case report is about a patient with a metastatic tumor in the patella and reconstruction of the deformity with an anterolateral reverse flow thigh flap. Skin and soft tissue reconstruction in the region around the knee are often challenging for the plastic surgeon and aim to cover the exposed bone, preserving the joint function of the knee and providing a good esthetic result. There is an option for the use of free flaps, where there is a need for adequate material and trained staff. However, the vascular anastomosis region at the back of the knee makes the procedure even more complex. The anterolateral reverse thigh fasciocutaneous flap, first described in 1990 by Zhang et al., appears to be an effective and reliable option for reconstructing defects around the knee. It has a long vascular pedicle length and the availability of soft parts. There are some anatomical variations in which the cutaneous perforator does not originate from the descending branch. This report presents a patient with a skin perforator originating from the transverse branch of the lateral femoral circumflex artery, with an acceptable aesthetic result for skin coverage in the anterior region of the knee.

RESUMO

A patela é um local infrequente para o aparecimento de tumores ósseos, benignos ou malignos, assim como de lesões pseudotumorais. Na literatura existe uma quantidade pequena de trabalhos sobre esse tema. Esse relato de caso trata-se de uma paciente com tumor metastático em patela e reconstrução da deformidade com retalho anterolateral da coxa de fluxo reverso. A reconstrução cutânea e de tecidos moles na região ao redor do joelho é frequentemente desafiadora para o cirurgião plástico e tem como objetivo cobrir a parte óssea exposta, preservando a função articular do joelho e conferir um bom resultado estético. Existe a opção pela utilização de retalhos livres, onde há necessidade de material adequado e equipe treinada. Contudo, a região de anastomose vascular na parte posterior do joelho torna o procedimento ainda mais complexo. O retalho fasciocutâneo anterolateral da coxa reverso, descrito pela primeira vez em 1990, por Zhang et al., parece ser uma opção efetiva e confiável na reconstrução de defeitos ao redor do joelho, haja visto que possui um longo comprimento do pedículo vascular e disponibilidade de partes moles. Existem algumas variações anatômicas em que a perfurante cutânea não se origina do ramo descendente. Esse relato traz uma paciente com perfurante cutânea oriunda do ramo transverso da artéria circunflexa femoral lateral com resultado estético aceitável para cobertura cutânea em região anterior do joelho.

Reconstruction of complex lesions of the lower limbs with reverse fasciocutaneous flaps

ABSTRACT

INTRODUCTION: Complex lesions on the lower limbs have always been challenging because of their difficult resolution. Microsurgical free flaps have been an excellent surgical option; however, they have limited use because there are an insufficient number of clinicians trained in such structured services. Fasciocutaneous flaps, specifically reverse flow, have been shown to be an excellent option in treating lower limb injuries as an alternative to complex microsurgical free flaps. METHODS: From January 2012 to June 2015, nine patients underwent reconstruction of the lower limbs with reverse fasciocutaneous flaps. All had extensive lesions on these limbs. RESULTS: In eight patients, the results were satisfactory, and there were no major complications. In one patient, there was total necrosis of the flap. CONCLUSION: Because of the relative technical simplicity of performing such a technique, fasciocutaneous flaps are a viable option for the reconstruction of complex wounds of the lower limbs and are an additional therapeutic option for patients with these lesions.

RESUMO

INTRODUÇÃO: Lesões extensas em membros inferiores sempre foram desafiadoras devido a sua difícil resolução. Retalhos livres microcirúrgicos se mostraram como excelente opção cirúrgica, todavia, por carecer de equipe capacitada e serviços estruturados, têm seu uso restrito. Os retalhos fasciocutâneos, especificamente os de fluxo reverso, têm se mostrado excelente opção na abordagem de lesões de membros inferiores, numa alternativa aos complexos retalhos livres microcirúrgicos. MÉTODOS: De janeiro de 2012 a junho de 2015, nove pacientes foram submetidos à reconstrução de membros inferiores com retalhos reversos fasciocutâneos. Todos eram portadores de lesões extensas nos referidos membros. RESULTADOS: Em oito pacientes o resultado foi satisfatório, sem complicações maiores. Em um paciente houve necrose total do retalho. CONCLUSÃO: Pela sua relativa simplicidade técnica, os retalhos fasciocutâneos de fluxo reverso vêm se consolidando como opção bastante viável na reconstrução de feridas complexas dos membros inferiores, sendo mais uma opção terapêutica para os pacientes portadores destas lesões.

Complex reconstruction of the upper and lower limbs by using reverse-flow flaps: a series of 32 cases

ABSTRACT

INTRODUCTION: Reconstruction of the soft tissues after loss of substance in the distal third of the upper and lower limbs, in particular when associated with the exposure of noble structures (bone, tendon, nerves, or blood vessels), remains a challenge in plastic surgery. Fasciocutaneous reverse flow flaps are an efficient surgical option for covering small and medium lesions in this location. METHOD: A retrospective clinical study was performed on a series of 32 cases of complex trauma of the upper and lower limbs treated between January 2013 and December 2014. RESULTS: A total of 24 reverse-flow flaps were performed in the lower limbs and eight in the upper limbs, resulting in efficient coverage of the defects. CONCLUSION: Reverse-flow flaps are reliable and present rotation arcs that allow coverage of a variety of lesions in the distal third of the upper and lower limbs.

RESUMO

INTRODUÇÃO: A reconstrução de partes moles após perdas de substância do terço distal dos membros superiores e inferiores, principalmente quando associada à exposição de estruturas nobres (osso, tendão, nervos ou vasos sanguíneos), continua a desafiar a cirurgia plástica. Os retalhos fasciocutâneos de fluxo reverso são uma eficiente opção cirúrgica na cobertura de lesões pequenas e médias nesta localização. MÉTODO: Foi realizado um estudo clínico retrospectivo, no período de janeiro de 2013 a dezembro de 2014, de uma série de 32 casos de traumas complexos em membros superiores e inferiores. RESULTADOS: Foram realizados 24 retalhos de fluxo reverso nos membros inferiores e 8 nos membros superiores, observando-se uma cobertura eficaz dos defeitos. CONCLUSÃO: Os retalhos de fluxo reverso são confiáveis e apresentam arcos de rotação que permitem a cobertura de lesões diversas no terço distal de membros superiores e inferiores.

A profile of patients with lower limb trauma treated by the Hospital Metropolitano de Urgência e Emergência reconstructive surgery team

ABSTRACT

BACKGROUND: In the area of medical emergency services, the incidence of trauma to the lower limbs is growing, and excessive damage may result in incapacity and physical disabilities. The aim of this study was to evaluate the epidemiological profile of patients with lower limb trauma who were treated by the reconstructive surgery team of Hospital Metropolitano de Urgência e Emergência. METHODS: This prospective cohort study included patients with lower limb trauma who were under the care of the reconstructive surgery team. RESULTS: The study mainly included young male road accident victims (primarily motorcycle) with dark complexions who were single and had not completed their elementary education. Ninety percent of the subjects had a paid occupation with a per capita mean monthly income of R$290.76 [122.76 USD]. Sixty-five percent of these subjects were alcohol consumers and 35% were smokers. The majority of the patients came from interior areas of Pará. The most commonly sustained injuries were contused lacerated wounds of the leg with exposure of the underlying structures. The median length of time between the trauma occurrence and surgery was 32.5 days. The principal reasons for postponing surgery included delays in approval for patient assessment by the plastic surgery team and waiting for the wound condition to improve. Reconstruction with flaps was used in 65% of cases with or without grafts, with the most common procedure using reverse-flow sural flaps. A cutaneous graft alone was used in 35% of the patients. CONCLUSIONS: Young, low-income, motorcycle-riding males with poor education levels, dark complexions, and alcohol consuming and smoking habits are the most involved in accidents that cause injuries to the lower limbs and require surgical reconstruction.

RESUMO

INTRODUÇÃO: Nos serviços de urgências médicas, é crescente a incidência de lesões traumáticas de membros inferiores com perda de substância, que podem ocasionar deficiências e incapacitação física. O objetivo deste estudo foi avaliar o perfil epidemiológico das vítimas de trauma em membro inferior, atendidas pela equipe de cirurgia reparadora do Hospital Metropolitano de Urgência e Emergência. MÉTODO: Foi realizado um estudo de coorte prospectivo com pacientes que sofreram trauma de membro inferior e que se encontravam sob os cuidados da equipe de cirurgia reparadora. RESULTADOS: A amostra foi majoritariamente composta por homens jovens, vítimas de acidente de trânsito (principalmente por motocicletas), pardos, solteiros e com ensino fundamental incompleto. Noventa por cento dos pesquisados exerciam alguma atividade remunerada e apresentavam renda per capita mensal média de R$ 290,76. Sessenta e cinco por cento dos pesquisados eram etilistas, dos quais 35% eram tabagistas. A maioria dos pacientes era proveniente do interior do Pará. As lesões mais comuns eram as lacerocontusas, localizadas na perna e com exposição de estruturas nobres. O tempo decorrido entre o trauma e a cirurgia apresentou mediana de 32,5 dias. Os principais motivos de retardar a cirurgia foram atrasos na solicitação para que a equipe da cirurgia plástica avaliasse o paciente e espera para melhorar as condições da ferida. Retalhos foram utilizados em 65% dos casos, acompanhados ou não de enxertos, sendo o mais frequente o sural de fluxo reverso. Em 35% dos pacientes, realizou-se enxerto cutâneo isoladamente. CONCLUSÕES: Homens jovens, de baixa renda, com pouca escolaridade, pardos, etilistas, tabagistas e usuários de motocicletas são os mais envolvidos em acidentes que ocasionam lesões em membros inferiores que necessitam de reparo cirúrgico.

Use of a flap based on the superior lateral genicular artery to cover lower limb injuries: case reports

ABSTRACT

The knee is one of the main joints in the human body, and due to its conformation and functionality, it is easily susceptible to injuries. Surgical reconstruction of injuries around the knee and proximal region of the leg represents a major challenge due to the exposure of osteoarticular, tendinomuscular and neurovascular structures and has been performed using various muscle and musculocutaneous flaps. Other flaps are also described for treating these lesions, although less frequently in the literature, such as the cutaneous flap based on the pedicle of the superior lateral genicular artery. Next, It presents a report of five cases of patients who were victims of motorcycle accidents in which the lesion was covered around the knee and proximal region of the leg using a skin flap based on the pedicle of the superior lateral genicular artery, in which good coverage of lesions, high flap survival rate, good clinical and aesthetic results. The use of the superior lateral genicular artery presents aesthetic advantages since the color and texture of the flap are similar to those of the knee region and provide a better quality appearance, in addition to not causing joint mobility problems.

RESUMO

O joelho é uma das principais articulações do corpo humano e, em função de sua conformação e funcionalidade, é facilmente suscetível a lesões. A reconstrução cirúrgica de lesões ao redor do joelho e região proximal da perna representa um grande desafio devido à exposição de estruturas osteoarticulares, tendinomusculares e neurovasculares e tem sido realizada com uso de diversos retalhos musculares e musculocutâneos. Outros retalhos também são descritos para tratamento dessas lesões, embora em menor frequência na literatura, como o retalho cutâneo baseado no pedículo da artéria genicular lateral superior. A seguir, é apresentado o relato de cinco casos de pacientes vítimas de acidentes motociclísticos nos quais realizou-se a cobertura da lesão ao redor do joelho e região proximal da perna com o uso do retalho cutâneo baseado no pedículo da artéria genicular lateral superior, nos quais foram obtidos uma boa cobertura das lesões, alta taxa de sobrevida do retalho, bons resultados clínicos e estéticos. O uso do artéria genicular lateral superior apresenta vantagens estéticas, visto que a cor e a textura do retalho são semelhantes às da região do joelho e conferem uma aparência de melhor qualidade, além de não causarem problemas de mobilidade articular.

Reverse-flow sural flap: 10 years of clinical experience and modifications

ABSTRACT

BACKGROUND: The reverse flow sural flap has been featured as a relevant option for reconstruction of the distal region of the legs and feet. However, some common mistakes are made while surgeons are learning how to employ this flap. The aim of this study was to analyze use of the flap by surgeons in with 10 years of experience by comparing the first 5 years to the last 5 years in order to identify technical alterations that contribute to better results. This study also aimed at alerting the surgeons in the learning phase regarding the most important details that should be addressed when using this technique. METHODS: We studied the results obtained for 61 reverse flow sural flaps used from January 2002 to December 2011. The patients were divided into 2 groups: group 1, patients that underwent surgery from January 2002 to December 2006 and group 2, patients that underwent surgery from January 2007 to December 2011. In group 2, we made changes to the fasciocutaneous flaps that included dissection of a fasciosubcutaneous margin exceeding the cutaneous portion of the flap and preservation of a lane of skin over the pedicle. RESULTS: In group 1, the incidences of partial necrosis and total necrosis were 18.5% and 3.7%, respectively, whereas group 2 had an 8.8% incidence of partial necrosis and no cases of total necrosis. CONCLUSIONS: The reverse flow sural flap is a good option for the treatment of the distal region of the legs and feet. Experience with this technique and the additional cautions described herein may contribute to even better results.

INTRODUÇÃO: O retalho sural de fluxo reverso tem se destacado como importante opção para reconstrução da região distal de pernas e pés. Alguns erros, porém, são comuns na fase de aprendizagem de emprego desse retalho. O objetivo deste estudo é analisar 10 anos de experiência com o emprego do retalho, comparando os primeiros 5 anos aos 5 anos subsequentes, buscando identificar se as alterações técnicas introduzidas contribuíram para obtenção de melhores resultados. Este estudo busca, ainda, alertar para os principais cuidados que os cirurgiões que estão em fase de aprendizagem da técnica devem tomar. MÉTODO: Foram estudados os resultados obtidos em 61 retalhos surais de fluxo reverso realizados no período de janeiro de 2002 a dezembro de 2011. Os pacientes foram alocados em dois grupos: grupo 1, constituído por pacientes operados de janeiro de 2002 a dezembro de 2006; e grupo 2, pacientes operados de janeiro de 2007 a dezembro de 2011. No grupo 2, foram realizadas alterações nos retalhos fasciocutâneos, como dissecção de uma borda fasciossubcutânea excedendo a porção cutânea do retalho e preservação de uma faixa de pele sobre o pedículo. RESULTADOS: No grupo 1, a incidência de necrose parcial foi de 18,5% e de necrose total, de 3,7%; no grupo 2, houve 8,8% de necrose parcial e nenhum caso de necrose total. CONCLUSÕES: O retalho sural de fluxo reverso é uma boa opção para o tratamento da região distal de pernas e pé. A experiência com a técnica e os cuidados implementados sugerem obtenção de melhores resultados.

ABSTRACT

Introduction: Loss of substance in the lower third of the leg usually requires complex surgical procedures for tissue reconstruction. Among the existing options for tissue reconstruction, reverse sural flap of the fasciosubcutaneous pedicle or reverse adipofascial flap, described in the literature as a viable option, has been little studied compared with other techniques. The objectives of this study were to present the results of surgical treatment of a complex injury due to Achilles tendon rupture using the reverse sural flap of the fasciosubcutaneous pedicle concomitantly with total skin grafting and to discuss the advantages of the procedure as well as other surgical alternatives. Case report: A.D., a 28-year-old male patient, presented with a complex injury in the lower third of the leg because of a poor outcome of surgical repair of Achilles tendon rupture that had occurred during an automobile accident more than 30 days previously. The patient underwent injury reconstruction with the reverse sural flap of the fasciosubcutaneous pedicle. The postoperative result was favorable, without complications. Conclusion: The surgical technique described here is satisfactory for treating lesions in the lower third of the leg due to Achilles tendon rupture.

RESUMO

Introdução: As perdas de substância no terço inferior da perna costumam exigir elaboradas estratégias cirúrgicas para sua reconstrução. Dentre as opções existentes, o uso do retalho sural reverso de pedículo fasciossubcutâneo ou adipofascial reverso, citado na literatura como interessante alternativa, é pouco relatado, se comparado às outras técnicas mais utilizadas. O objetivo deste estudo é apresentar o resultado do tratamento de uma lesão exposta do tendão de Aquiles, realizado com retalho sural reverso de pedículo fasciossubcutâneo, associado à enxertia de pele total no mesmo tempo cirúrgico, discutindo as alternativas técnicas e as vantagens do procedimento.
Relato de caso: A.D., 28 anos, portador de lesão complexa em terço distal da perna, decorrente de evolução desfavorável de procedimento ortopédico de reconstrução do tendão de Aquiles rompido em acidente automobilístico, ocorrido há mais de 30 dias, foi submetido à reconstrução da ferida por meio do retalho sural reverso de pedículo fasciossubcutâneo. A evolução pós-operatória foi favorável, não havendo complicações ou intercorrências.
Conclusão: A técnica relatada apresentou uma boa opção com resultado satisfatório para a cobertura de lesão de terço inferior da perna com exposição do tendão de Aquiles.

Treatment of bone exposure of the lower limb using negative-pressure wound therapy in the acute phase followed by free flaps in the subacute phase

ABSTRACT

BACKGROUND: Fractures of the distal third of the leg, ankle, and foot frequently present with exposed bone, requiring specialized teams to accomplish skin coverage. These teams are often not readily available, which can prevent the standard treatment of extensive debridement, fracture fixation, and use of flaps, which is termed the "fix and flap" method. METHODS: The author proposes a 2-stage treatment method for such cases: negative pressure is imposed during debridement and bone exposure by the orthopedic team, followed by the elective preparation of free flaps for definitive coverage. Five patients were treated with a total of 6 free flaps, including 1 latissimus dorsi muscle and 5 of anterolateral thigh flaps. There was 1 total flap loss. Thus, the success rate was 83.34%. RESULTS: The patients had good outcomes with limb salvage, preserved function, and no osteomyelitis. CONCLUSIONS: Negative-pressure wound therapy is an option for the emergency treatment of open fractures of the lower limb, allowing the survival of elective free flaps without resulting in impairment as a final outcome.

RESUMO

INTRODUÇÃO: Traumas do terço distal da perna, do tornozelo e do pé cursam frequentemente com exposição óssea, demandando equipes especializadas para realizar a cobertura cutânea definitiva. Muitas vezes, essas equipes não estão prontamente disponíveis, o que pode impedir o tratamento padrão, que consiste em amplo desbridamento, fixação das fraturas e utilização de retalhos (fix and flap). MÉTODO: Neste trabalho, o autor propõe tratamento em duas etapas, sendo a primeira a instituição de terapia por pressão negativa quando do desbridamento e exposição óssea pela equipe da ortopedia, seguida pela realização de retalhos livres para cobertura definitiva, de forma eletiva. Foram tratados 5 pacientes, com realização de 6 retalhos livres, sendo 1 do músculo grande dorsal e outros 5 da face ântero-lateral da coxa. Houve perda total de um retalho, com índice de sucesso de 83,34%. RESULTADOS: Os pacientes apresentaram boa evolução, com salvamento do membro, função preservada, e sem osteomielite. CONCLUSÕES: A terapia por pressão negativa é uma opção no tratamento de urgência das exposições ósseas do membro inferior, permitindo a realização de retalhos livres de forma eletiva, sem prejuízo no resultado final para o paciente.

Reverse-flow supramalleolar flap: clinical applications

ABSTRACT

The distal regions of the lower limbs, including the lower third of the legs and feet, are very prone to soft tissue loss due to trauma and trophic lesions. The anterior part of the tibia with its slightly thick, vulnerable cover and large number of tendons, which cannot remain exposed, requires fast and complex skin coverage. The blood-flow direction, lack of muscle bellies for flaps, and inelastic skin for viable randomized flaps complicate this situation. In the past, intermediate-transfer flaps and cross-legged flaps were used for skin coverage, with drawbacks: they require more than one surgery, cause great discomfort to the patient, and are not always effective. Microsurgery is an option that greatly helps to cover lesions at this site; however, it requires a team with specialized training and equipment. Donor areas that are larger, may cause functional impairment, and use larger vascular trunks. Fasciocutaneous flaps with reverse flow may help to cover small and medium lesions at this location, with minor aesthetic and functional sequelae, and less surgical time. They are an effective surgical option because of their ease, safety, and versatility. The aim of this paper is to demonstrate the use of reverse-flow supramalleolar flaps in 4 different situations and discuss their advantages and applications.

RESUMO

As regiões distais do membro inferior, incluindo o terço inferior da perna e os pés, são áreas muito propensas a perda de partes moles por trauma e lesões tróficas. A posição anterior da tíbia, com sua cobertura pouco espessa e vulnerável, além da grande quantidade de tendões, que não podem permanecer expostos, exigem uma cobertura cutânea rápida e, ao mesmo tempo, complexa. A situação em relação à direção do fluxo sanguíneo, a escassez de ventres musculares para retalhos e pele pouco elástica para retalhos randomizados viáveis complicam esta situação. No passado, retalhos com transferência intermediária foram utilizados; assim como os de perna cruzada, "cross leg", ainda são, com desvantagens importantes: dependem de mais de um tempo cirúrgico, geram muito desconforto ao paciente e nem sempre são eficazes. A microcirurgia tornou-se uma opção que contribui muito para cobertura de lesões neste aspecto, porém, depende de equipe com treinamento e material especializado. As áreas doadoras são maiores, podem causar relativo déficit funcional e utilizam troncos vasculares maiores. Os retalhos fasciocutâneos de fluxo reverso podem contribuir para a cobertura de lesões pequenas e médias nesta localização, com sequelas funcionais e estéticas menores; e menor tempo cirúrgico. Constitui eficiente opção cirúrgica por sua facilidade, segurança e versatilidade. Neste estudo, temos o objetivo demonstrar o retalho supramaleolar de fluxo reverso utilizado em 4 casos, com aplicações em situações diferentes, com resultados satisfatórios de fácil execução e reprodução.

Reconstruction of the dorsal foot with a retrograde flow lateral supramalleolar flap in a 4-year-old child

ABSTRACT

INTRODUCTION: Injuries in the distal third of the leg may expose noble areas that should be covered. The lateral supramalleolar flap is one of a series of flaps described in the last decade; it is an elevated fasciocutaneous flap in the lateral aspect of the lower leg, and is employed as a distally based pedicled flap. The purpose of this case report is to present the surgical results and the effectiveness of the lateral supramalleolar flap in extensive lower limb injuries with significant loss of substance in a 4-year-old child. CASE REPORT: Patient A. K. E. S., a girl, was admitted 13 years previously. She had been in an accident, and had extensive injuries on the dorsum of the left foot, with exposure of noble areas approximately 6 cm in diameter. After debridement, lesion coverage was performed on the fifth day by using a fasciocutaneous lateral supramalleolar flap. The patient was discharged for outpatient monitoring and progressed without complication. She only returned to the hospital 13 years later, overweight and complaining of swelling on the dorsal aspect of the foot. Lipectomy of the distal third of the flap was performed, with uncomplicated progress. A second procedure for the proximal third was scheduled six months later. DISCUSSION: Injuries in the distal third of the leg may expose noble areas. Lateral supramalleolar, sural, and pedicled perforating flaps have similar applications, but show individual technical peculiarities. The lateral supramalleolar flap has no need for microsurgery, is rapidly performed, and simulates the receiving area while considering several local characteristics. CONCLUSION: This flap is easily implemented and reproducible, and can be used in children younger than 5 years of age. It is an alternative for injuries in the lower third of the leg, the ankle, and the foot.

RESUMO

INTRODUÇÃO: Lesões em terço distal da perna podem deixar expostas áreas nobres, que devem ser recobertas. O retalho supramaleolar lateral faz parte de uma série de retalhos descritos na última década, é fasciocutâneo elevado no aspecto lateral da perna inferior, sendo empregado como um retalho pediculado com base distal. O objetivo do presente relato de caso é apresentar os resultados cirúrgicos e a eficácia do retalho supramaleolar lateral em lesões extensas no membro inferior com perdas de substâncias significativas em criança de 4 anos. RELATO DE CASO: Paciente A.K.E.S., sexo feminino, foi internada há 13 anos, vítima de atropelamento, com lesão extensa em dorso de pé esquerdo com exposição de áreas nobres de aproximadamente seis centímetros de diâmetro. Após desbridamento, no quinto dia foi realizada cobertura da lesão com retalho fasciocutâneo supramaleolar lateral. A paciente teve alta para acompanhamento ambulatorial, evoluindo sem intercorrência. Ela retornou ao serviço apenas 13 anos após com sobrepeso e reclamando do aumento de volume no dorso do pé. Foi realizada lipectomia do terço distal do retalho, evoluindo sem intercorrência. Um segundo procedimento do terço proximal foi programado para seis meses após. DISCUSSÃO: Lesões no terço distal da perna podem expor áreas nobres. Os retalhos supramaleolar lateral, sural e de perfurantes pediculados possuem aplicações semelhantes, mas apresentam peculiaridades técnicas individuais. Como vantagens no retalho supramaleolar lateral: não há necessidade de microanastomoses, é rápido para ser executado e simula a área receptora em várias características locais. CONCLUSÃO: É um retalho de maior facilidade de execução e reprodutível, podendo ser realizado em crianças menores de 5 anos. Tem sua aplicação como alternativa para lesões em terços inferiores da perna, tornozelo e pé.

ABSTRACT

Severe trauma to the lower limbs, with major skin loss and the compromise of vital structures, is often difficult to treat. A combination of several factors such as ability and effort made to distinguish lesion severity - that is, to differentiate recoverable traumas from those that will inevitably progress to amputation - plays an important role. Therefore, indices were developed to facilitate the stratification of injury severity and prediction of the need for amputation, such as the Mangled Extremity Severity Score (MESS). In this article, we report a case of severe trauma to the lower limb, where reconstruction was performed based on the MESS index with multiple muscle flaps, and a good functional outcome was achieved.

RESUMO

Traumatismos graves de membros inferiores, com grandes perdas cutâneas e comprometimento de estruturas vitais, são casos de difícil resolução. A combinação de vários fatores, como esforço realizado e capacidade de distinguir a gravidade das lesões, ou seja, diferenciar as lesões recuperáveis ou viáveis daquelas que inevitavelmente irão evoluir para amputação, tem importante papel. Para tanto, surgiram índices que buscam estratificar a gravidade da lesão e predizer a necessidade de amputação, como o índice MESS (do inglês, Mangled Extremity Severity Score). Neste artigo, é reportado um caso de trauma grave de membro inferior, com reconstrução baseada no índice MESS, com emprego de múltiplos retalhos musculares e boa evolução funcional.

Reversed hemisoleus flap for wound coverage in the distal third of the leg

ABSTRACT

Reconstruction of the distal third of the leg represents a challenge for many plastic surgeons. Although microsurgical flaps have been the method of choice for this reconstruction, many hospitals do not have equipment or microsurgical staff trained for this type of procedure. Moreover, the patient's clinical condition does not allow a more complex surgery in some cases. This study reports two cases of reconstruction of the distal third of the leg with reverse flap of the soleus muscle, based on the perforators of the posterior tibial artery, as an alternative to microsurgical flaps. The reverse soleus flap had an excellent outcome with short surgical duration, easy implementation, excellent resolution, and low morbidity of the donor area.

RESUMO

A reconstrução do terço distal da perna representa um desafio para muitos cirurgiões plásticos. Usualmente, os retalhos microcirúrgicos têm sido considerados de escolha para essas reconstruções; no entanto, muitos hospitais ainda não dispõem de equipamentos ou equipe microcirúrgica treinada nesse tipo de procedimento, além dos casos em que as condições clínicas do paciente não permitem uma cirurgia de maior porte. O presente trabalho propõe o relato de dois casos de reconstrução do terço distal da perna com retalho reverso do músculo solear, baseado nas perfurantes da artéria tibial posterior, como alternativa aos retalhos microcirúrgicos. O retalho solear reverso apresentou excelente resultado, com pequeno tempo cirúrgico, fácil realização, excelente resolubilidade e pouca morbidade da área doadora.

Uso do retalho superomedial para reconstrução perineal

RESUMO

Introdução: A gangrena de Fournier é uma doença infecciosa de início insidioso, rapidamente progressiva e agressiva. Existem várias técnicas possíveis, como enxertos cutâneos, retalhos miofasciocutâneos e fasciocutâneos, além do fechamento primário da lesão, se viável. O retalho fasciocutâneo superomedial da coxa é uma opção muito utilizada, devido ao fato de apresentar bons resultados estéticos e funcionais, além de simples execução.
Relato de caso: Paciente masculino, 62 anos, encaminhado após desbridamento por gangrena de Fournier. Foi realizado cirurgia para reconstrução de períneo, sendo usado retalho superomedial de coxa bilateral.
Discussão: O retalho fasciocutâneo superomedial da coxa foi descrito por Hirshowitz, et al., como sendo um retalho provavelmente arterial, localizado na curvatura medial da coxa. Possui boa espessura, facilidade de rotação, excelente aspecto estético e complicações como necrose são muito raras, devido à rica vascularização que o torna muito seguro, inclusive em pacientes diabéticos e portadores de vasculopatias.
