Reverse flow sural artery flap

ABSTRACT

Introduction: The reverse flow sural artery flap is an important skin flap that can be used for reconstruction of tissue defects of the distal third of the leg, heel and proximal plantar region. It is a versatile flap with a wide arc of rotation that varies from 90º-180º and has an axial pattern based on reverse flow of superficial sural artery and main venous drainage by the short saphenous vein. Method: Twenty-two patients were operated with ages ranging from 12 to 71 years and 19 cases due to motorcycle accident. Results: Five (22%) flaps had parcial necrosis and one (4.1%) showed total loss. Split thickness skin on donor site was done in 18 (82%) cases.

RESUMO

Introdução: O retalho sural de fluxo reverso é um importante retalho fasciocutâneo, que pode ser usado para a reconstrução de perda de substância do terço distal da perna, calcâneo e região plantar proximal. É um retalho versátil, com amplo arco de rotação, que varia de 90º-180º e apresenta padrão axial baseado no fluxo arterial reverso da artéria sural superficial ramo da artéria fibular e drenagem venosa principal por tributárias surais da veia safena parva. Método: Foram operados 22 pacientes, com idade variando de 12 a 71 anos, sendo 19 casos devido a acidente automobilístico. Resultados: Cinco (22%) retalhos apresentaram necroses parciais e um (4,1%) apresentou necrose total. Enxertia em área doadora foi realizada em 18 (82%) casos.

Surgical treatment of cervical region associated with polytetrafluoroethylene platysmal suspension sutures

ABSTRACT

Introduction: Since the 1970s, great importance has been atributed to superficial musculoaponeurotic system (SMAS) and platysma dissection, considering the proper approach of these structures the key point in order to achieve better results in facial rejuvenation. Difficult necks, as referred as grade IV by McKinney, are those with significant flaccidity of the platysma muscle and skin, pronounced and separated medial bands, large protrusion of the submandibulary glands, low hyoid position, great amount of fat and poorly defined jawline. Method: A group of 89 patients with these characteristics and submitted to surgical treatment was retrospectively studied. The surgical treatment included face-neck lifting, with SMAS-platysma dissection and traction, Feldman's corset platysmaplasty and cross-crossed polytetrafluoroethylene (PTFE) suture on platysmal edges and anchored to the mastoid. It is recommended that the suspension suture of the SMAS-platysma complex is done only after the conclusion of the neck region once the platysma movement towards the midline will cause inferior displacement and tissue accumulation at the mandibular edge. Results: The results achieved show low complication rates (3.2%) and high post-operative satisfaction. Conclusion: The results also point to the efficacy of this surgical routine and the safety of the PTFE suture that secondarily contributes to favorable results in such difficult condition.

RESUMO

Introdução: A partir da década de 70, grande importância tem sido atribuída ao sistema músculo-aponeurótico superficial (SMAS) e ao platisma, sendo o manejo adequado destas estruturas considerado ponto-chave na obtenção de melhores resultados nas cirurgias de rejuvenescimento facial. Consideram- se casos de difícil resolução, atribuídos por McKinney como grau IV, pacientes com significativa flacidez de pele e músculos, bandas platismais pronunciadas e separadas, protrusão das glândulas submandibulares, baixa posição do osso hióide, grande quantidade de gordura e uma linha mandibular pouco definida. Método: Um grupo de 89 pacientes com estas características e submetido a tratamento cirúrgico foi avaliado, retrospectivamente. O tratamento cirúrgico constou de ritidoplastias cérvico-faciais associadas a dissecção e tração do complexo SMAS-platisma, platismoplastia medial segundo Feldman e sutura entrecruzada com fio de politetrafluoroetileno (PTFE), realizada nos bordos platismais e fixa à mastóide. Preconiza- se que a sutura de suspensão do complexo SMAS-platisma seja efetuada somente após a conclusão da região cervical, caso contrário, o avanço do platisma à linha média promoveria tração inferior e novo acúmulo tecidual na margem mandibular. Resultados: Os resultados apurados demonstram baixos índices de complicações relevantes (3,2%), associados a maior satisfação pós-operatória. Conclusão: Conclui-se pela eficácia da rotina cirúrgica adotada e pela segurança da sutura de PTFE, possibilitando resultados favoráveis nesta difícil condição.

Changing in the aetiology of facial trauma of patients attended to emergency department in Distrito Federal

ABSTRACT

Background: Trauma remains one of the principal causes of morbidity and mortality in the western world, especially among young adults. The prevalence of traumatic injuries located in the maxillofacial region is higher than injuries in other body areas. The Hospital Regional da Asa Norte is reference to the attendance of the victims of facial traumas in Brasília, Distrito Federal. Objective: This study aims to evaluate the etiology, age, gender and location of the lesions of facial trauma of all patients diagnosed in our hospital. Method: The data were evaluated through retrospective analysis of patient charts from 1 January to 31 December 2004. Results: The sample comprised 711 patients with facial trauma. Blunt assaults were the most common cause followed by motor vehicle crashes. Falls were by far, the predominant cause of injury in children, but with increasing age, assaults become more common. It was observed a male:female ratio of 3:1. The most frequent age group was of the 21-40 years with 52%. Facial fractures were found in 24.9% of facial injuries. The most frequent fracture was nasal. Most of the patients had a low social status and a bad oral care. Conclusion: Aggressions or interpersonal violence were the main cause of facial trauma in our population. The causes of facial trauma were directly related to the age.

RESUMO

Introdução: O trauma permanece como uma das principais causas de morbidade e mortalidade no mundo ocidental, especialmente entre os adultos jovens. A prevalência de lesões traumáticas na face é alta devido à enorme exposição e à pouca proteção da região. Objetivo: Avaliar a etiologia, a idade, o gênero e a localização dos traumas de face dos pacientes atendidos no Hospital Regional da Asa Norte (HRAN), Brasília, Distrito Federal. Método: Trata-se de um estudo retrospectivo, realizado no pronto socorro do HRAN-DF, visando a avaliar o perfil epidemiológico dos pacientes atendidos pela equipe da Unidade de Cirurgia Plástica, vítimas de trauma de face no período de 1 de janeiro a 31 de dezembro de 2004. Resultados: O estudo compreendeu 711 pacientes, destacando- se o sexo masculino (72,8%). Quanto à causa, predominou a agressão física, seguida por acidente com veículos/motos. As quedas foram a causa predominante das lesões em crianças, mas se verificou a participação cada vez maior da agressão física como mecanismo de trauma facial com o aumento da idade. A relação de homem:mulher foi de 3:1. A principal faixa etária foi de 21 a 40 anos, representando 52% dos pacientes. As fraturas foram encontradas em 24,9% das lesões faciais. O nariz foi o local mais acometido nas fraturas de face (76,8%). Conclusão: A violência interpessoal foi a principal causa de trauma de face. A queda da própria altura mostrou-se como importante mecanismo de trauma nos extremos de idade.

Versatility of free lateral arm flap in complex reconstruction

ABSTRACT

Introduction: The free lateral arm flap has been considered reliable and versatile in many clinical series. This paper describes the versatility of the lateral arm flap in complex reconstruction at Hospital of Clinics of Federal University of Bahia. Method: Fifteen microsurgical lateral arm flaps were used in complex reconstructions between February 2003 and June 2007. There were analyzed the following data: surgical times, flap and pedicle dimensions, site and etiology of defects. Results: The flaps were used in head and neck, superior and inferior limb reconstructions (etiology: trauma and cancer). There were no donor or flap complications. The flap dimensions ranged from 7X5cm to 16X8.5cm and the pedicle strength ranged from 6 to 10cm. The mean surgical time was 4.3h. Conclusion: The lateral arm flap was reliable and versatile in head and neck and limb reconstruction. Its dimensions and pedicle are variable to fit this versatility.

RESUMO

Introdução: O retalho lateral do braço microcirúrgico tem se mostrado confiável e versátil, em diversas séries de casos clínicos. Este trabalho tem como objetivo relatar a versatilidade do retalho lateral do braço, nas reconstruções complexas em microcirurgia do Serviço de Cirurgia Plástica do Hospital das Clínicas da UFBA. Método: No período de fevereiro de 2003 a junho de 2007, foram realizados 15 procedimentos de microcirurgia reconstrutiva com a utilização do retalho fasciocutâneo lateral do braço. Foram levantados e analisados os seguintes dados: tempos cirúrgicos, dimensões do pedículo e retalho, localização da reconstrução e natureza da doença primária. Resultados: Os retalhos foram utilizados para reconstrução de cabeça e pescoço, membro superior e inferior por defeitos diversos em etiologia (trauma e oncologia). Não houve complicações em relação à reconstrução nem à área doadora. As dimensões do retalho variaram de 7X5cm a 16X8,5cm, o pedículo variou de 6 a 10cm e o tempo cirúrgico médio foi de 4,3h. Conclusão: O retalho lateral do braço mostrou-se eficaz e seguro no tratamento das lesões de extremidades e cabeça e pescoço por trauma ou câncer. Observou-se versatilidade na determinação das dimensões do pedículo e ilha de pele, sem seqüelas significativas para a área doadora.

Use of tissue expanders in head and neck reconstructive surgical procedures

ABSTRACT

Introduction: One of the greatest challenges in the treatment of scars and defects in the head and neck region occurs in reconstruction in this area. Various techniques and different modalities of reconstructive surgical procedures have been used. The intention of this work is to contribute to the consolidation of the use of tissue expanders as one more viable alternative for selected cases. Objective: To present and evaluate the results of the use of tissue expanders in the reconstruction of defects located in the head and/or neck. Method: A descriptive, retrospective and analytical study was made from a serie of cases. The medical records of ten patients, all with defects in the head and/or neck were analyzed for the period from February 1998 to July 2006. Results: Fifteen surgeries were performed on the ten patients selected, with a total of twenty-nine expanders placed. Of these, nine patients had a result considered satisfactory. This procedure was ineffective in one case. Conclusion: The use of tissue expanders presents itself as a good alternative for covering regions in the head and neck for selected patients.

RESUMO

Introdução: Um dos grandes desafios no tratamento dos defeitos e cicatrizes da região da cabeça e pescoço é a sua reconstrução. Várias técnicas e modalidades diferentes de cirurgias reparadoras têm sido utilizadas. Este trabalho pretende contribuir para consolidar o uso de expansores teciduais como mais uma alternativa viável para casos selecionados. Objetivo: Apresentar e avaliar os resultados do uso de expansores teciduais na reconstrução de defeitos localizados na região da cabeça e pescoço. Método: Realizou-se um estudo descritivo, retrospectivo e analítico de série de casos. Analisaram-se os prontuários de 10 pacientes portadores de defeitos na região da cabeça e pescoço, no período compreendido entre fevereiro de 1998 a julho de 2006. Resultados: Nos 10 pacientes selecionados, foram realizadas 15 cirurgias com colocação de 29 expansores ao todo. Destes, 9 pacientes tiveram resultado considerado satisfatório. Em 1 caso, esse procedimento mostrou-se ineficaz. Conclusão: A utilização de expansores teciduais se apresenta como uma alternativa eficaz no planejamento de cobertura de lesões na região da cabeça e pescoço, em casos selecionados.

Treatment of burned patients admitted in a general hospital

ABSTRACT

Objective: The present study analyzes the treatment of burned patients in a general hospital from Salvador, Bahia, Brazil. Method: 150 patients with acute burn injuries were evaluated from January 1999 to May 2006. Until this time, this kind of patients weren't admitted to that hospital. All patients were assessed by the same plastic surgeon, who performed all the surgical procedures. The patients were admitted to the emergency room, and taken either to an isolated apartment, or to the semi-ICU, or to the ICU, depending on the severity of their condition. A covered bandage method was performed in the operating room, and grafting was performed as early as possible. The statistical data were analyzed by the Epi-Info system. Results: We found out that most of the patients were male (72.5%), and there were also a chemical burning rate and an 11.8% electrical burning rate, which were higher than the rates (15.7%) found in medical literature, probably because of our reference and proximity to a petrochemical polo. The mortality rate was 7.8% and the mean length of stay was 12.8 days. Conclusion: It was concluded that, even though there was no burning treatment center at that hospital, the results were satisfactory, according to the burning treatment centers analyzed.

RESUMO

Objetivo: O presente estudo visa analisar o tratamento dos pacientes queimados, em um hospital geral terciário, predominantemente privado de Salvador, Bahia. Método: Foram analisadas 150 vítimas de queimaduras internadas, de janeiro de 1999 a maio de 2006, onde, até então, não eram admitidos pacientes queimados. Todos foram acompanhados por um único cirurgião plástico, que realizou todos os procedimentos cirúrgicos. Os pacientes foram admitidos pela unidade de emergência e, quando preenchiam aos critérios de internação, eram encaminhados a apartamentos, semi-intensiva ou UTI, conforme a gravidade do caso. Utilizou-se o método fechado na realização dos curativos em centro cirúrgico e as enxertias foram feitas o mais precocemente possível. Os dados estatísticos foram analisados pelo sistema Epi-Info. Resultados: Foi identificada uma clientela predominantemente do sexo masculino (72,5%) e um porcentual de queimaduras químicas (15,7%) e elétricas (11,8%) maior que o encontrado na literatura, pelo fato de sermos referência de um grande Pólo Petroquímico. O tempo médio de internamento foi de 12,8 dias e a mortalidade de 7,8%. Conclusão: Concluímos que, embora não disponhamos de um centro de tratamento de queimados, nossos resultados foram equivalentes.

Use of latissimus dorsi myocutaneous flap in serious electric burnings

ABSTRACT

Introduction: Electrical injuries generally represent serious lesions with deforming sequels for the patient. Depending on the extension and localization of the injury, several techniques of reconstruction may be used, ranging from simple skin grafts to more complex repairs with the use of free flaps. Objective: The objective of this work is to demonstrate the use of the latissimus dorsi myocutaneous flap in the reconstruction of injuries caused by serious electrical burns. Method: Seven patients have been treated, victims of high voltage, injuries with lesions in several regions of the body. Results: In all cases, results were satisfactory, with good return of the patients to normal activities.

RESUMO

Introdução: Queimaduras elétricas geralmente representam lesões graves com seqüelas deformantes para o paciente. Dependendo da extensão e localização da lesão, podem ser utilizadas diversas técnicas de reconstrução, variando desde simples enxertias até reparações complexas com o uso de retalhos livres. Objetivo: O objetivo deste trabalho é demonstrar a utilização do retalho miocutâneo do grande dorsal na reconstrução de lesões causadas por queimaduras elétricas graves. Método: Foram tratados sete pacientes, vítimas de lesões de alta voltagem, com lesões em diversas regiões do corpo. Resultados: Em todos os casos, foram obtidos resultados satisfatórios, com bom retorno dos pacientes às suas atividades.

Müller muscle-conjunctiva resection for treatment of blepharoptosis

ABSTRACT

Mild blepharoptosis is a deformity characterized by malposition of the upper eyelid 4mm under scleral limb. Can be caused by malfunction of the Müller muscle, and it is diagnosticated by instilling several drops of 10% phenylephrine. This is a review of the studies related to the methods applied in mild blepharoptosis, as well as discussion of its indications and technique of Müller muscleconjunctiva resection. The authors present the experience of the Divisão de Cirurgia Plástica HC-FMUSP with this procedure during the period between 1995 and 2006.

RESUMO

A ptose palpebral suave constitui deformidade caracterizada por deslocamento da pálpebra superior até 4mm abaixo do limbo escleral. Pode ser decorrente de disfunção do músculo de Müller, diagnosticada pelo teste do colírio de fenilefrina a 10%. Neste trabalho, realizou-se revisão bibliográfica orientada sobre os métodos corretivos da ptose palpebral suave, objetivando apontar o tratamento de escolha para correção dessa deformidade. Mostrou-se também a experiência da Divisão de Cirurgia Plástica do HC da FMUSP com a utilização deste procedimento durante o período de 1995 a 2006.

Surgical rehabilitation for facial paralysis

ABSTRACT

Facial palsy is the most common cranial neuropathy. It is expressed clinically as a dysfunction of the facial expression. For many years, the surgical reanimation of facial movements and above all of the facial expression has been a challenge for plastic and microsurgeons. In this article, the authors present a review of the literature on surgical rehabilitation of facial paralysis. Many procedures have been used and still today the results are limited. Despite the need of a multidisciplinary team, the plastic surgeon has prominent role in facial surgical rehabilitation.

RESUMO

A paralisia do nervo facial é a neuropatia craniana mais comum. Clinicamente se observa a disfunção da expressão facial. Por muitos anos, a restauração cirúrgica dos movimentos faciais, principalmente da expressão facial, tem sido um grande desafio para os cirurgiões plásticos e microcirurgiões. Neste artigo, os autores apresentam uma revisão da literatura sobre a reabilitação cirúrgica da paralisia facial. Apesar da necessidade de uma equipe multidisciplinar, o cirurgião plástico tem papel fundamental na reabilitação cirúrgica facial. Vários procedimentos já foram empregados, e ainda hoje os resultados obtidos na recuperação da expressão facial têm se mostrado limitados.

ABSTRACT

The authors praise an efficient treatment for the solution of mammary malformation (Poland's Syndrome), with the situs inversus totalis. The technique employed was the one used in a plastic surgeon's routine, without need of special hospital facilities or any longer time of internment. The use of the expansor of mammary tissue and the placing of a permanent prosthesis were the solution, which we found suitable for correcting the deformities above mentioned.

RESUMO

Os autores preconizam um tratamento eficaz para a solução das má formações mamárias (Síndrome de Poland), na presença de situs inversus totalis. A técnica utilizada é empregada na rotina do cirurgião plástico, não exigindo hospital de grande porte, nem tempo de internação prolongado. O uso de expansor de tecido mamário e a colocação de prótese definitiva foram as soluções que achamos adequadas para a correção da deformidade.

Chest wall reconstruction with methylmethacrylate: case report

ABSTRACT

During the past 25 years, numerous authors have made remarkable contributions to the reconstruction of the thorax. The methylmethacrylate is an inexpensive and easy to apply option for reconstruction of chest wall skeletal support after extensive resections, and provides satisfactory and durable cosmetic and functional results, associated with stable results and prevention of uncoordinated chest wall motion. This report describes a chest wall reconstruction with molded methylmethacrylate after extensive resection of an anterolateral chest wall tumor, with excellent cosmetic and functional outcome.

RESUMO

Nos últimos 25 anos, numerosos autores descreveram técnicas de reconstrução da parede torácica. O metilmetacrilato é uma opção atrativa para reconstruções do suporte esquelético da parede torácica após ressecções extensas, por apresentar baixo custo e proporcionar bons e duradouros resultados estéticos e funcionais, além de estabilidade à reconstrução, evitando a movimentação descoordenada do tórax durante a respiração. O presente relato descreve a reconstrução da parede torácica com metilmetacrilato moldado, após exérese tumoral extensa ântero-lateral, com excelente resultado estético e funcional.

Debridement and surgical reconstruction of trunk after trauma

ABSTRACT

Introduction: Gangrene was first description by Meleney, in 1924. This disease has terrible evolution, which can be obstructed with recent diagnostic and debridement. Objective: Systematize the treatment of a child victim of trauma with infection and gangrene. Case report: A patient victim of a cyclist accident, showing colostomy and external pelvis establish, 5 days after surgery was submitted to effective debridement of inguinal region, gluteus maximus muscle and back trunk, extensive amputation and surgical reconstruction in second time. Conclusion: The authors recommend early diagnosis and quickly treatment to patient has any chance of survive.

RESUMO

Introdução: A primeira descrição de infecção com gangrena dos tecidos moles foi realizada por Meleney, em 1924. Observou-se que esta doença é de caráter fulminante, com chance de sobrevida quando prontamente diagnosticadas e desbridadas. Objetivo: Sistematizar o tratamento de um menino vítima de trauma com infecção e gangrena. Relato de caso: Menor traumatizado em acidente ciclístico. Apresentava colostomia em alça e fixação externa de bacia, evoluindo com gangrena dos tecidos moles. Foi submetido a desbridamento efetivo de região inguinal, glúteos e dorso, com extensa amputação dos tecidos moles e músculos e reconstrução em 2º tempo. Conclusão: Os autores recomendam que o diagnóstico e o tratamento sejam imediatos para existir uma chance de sobreviver à doença.
