Late Evaluation of Results of Periareolar Mammaplasties

ABSTRACT

The authors studied twenty-five patients submitted to periareolar mammaplasty with a lower glandular fiap and with more than 10 postoperative years. Data studied included type and amount of breast parenchyma resection; amount of skin resection; age at the time of surgery; patient's assessment of symmetry, final volume, shape/ptosis, shape of the areolar-nipple complex (ANC) and of scars; complications and level of patient satisfaction.

RESUMO

São estudadas 25 pacientes submetidas a mamoplastia periareolar com retalho glandular inferior, todas com mais de 10 anos de pós-operatório. Foram levantados dados como tipo e quantidade de ressecção do parênquima mamário; quantidade de pele ressecada; idade na época da cirurgia; avaliação do paciente referente à simetria, volume final, forma/ptose, forma do complexo aréolo-mamilar (CAM) e das cicatrizes; complicações e graus de satisfação das pacientes.

The Effects of Pentoxifylline and Buflomedil Hydrochloride in Random Flaps in Rats under the Influence of Nicotine

ABSTRACT

Many reports in the literature show the harmful effects of nicotine on microcirculation, leading to increased risk of skin flap ischemia. In an attempt to reverse these effects, an experimental trial was carried out to test two drugs: pentoxifylline, whose action changes blood viscosity, and buflomedil, which is a vasoactive substance that acts on microcirculation. After a 6-week treatment with nicotine, 40 rats were subdivided into 4 groups and operated on to prepare a random dorsal skin flap. Group A was not treated with any of the drugs. Groups B and C were treated for 7 postoperative days with pentoxifylline and buflomedil, respectively. Group D, the control group, received only saline solution. VVhen compared, there was a significant difference between groups A and D (p< 0.05). We concluded that these drugs showed some effectiveness in the reversal of the effects caused by nicotine, but it would be necessary to have larger samples to achieve statistically significant results.

RESUMO

São abundantes na literatura trabalhos que demonstraram os efeitos nocivos da nicotina na microcirculação, resultando em maior risco de isquemia de retalhos cutâneos. Na tentativa de reverter esses efeitos, foi desenvolvido um estudo experimental no qual foram testadas duas drogas: a pentoxifilina, que atua alterando a viscosidade sangüínea, e o buflomedil, que é uma substância vasoativa com ação na microcirculação. Após tratamento com nicotina durante 6 semanas, 40 ratos foram subdivididos em 4 grupos e operados para confecção de um retalho cutâneo dorsal randomizado. O grupo A não recebeu tratamento com as drogas. Os grupos B e C foram tratados no pós-operatório durante 7 dias com pentoxifilina e buflomedil, respectivamente. O grupo D, controle, recebeu apenas soro. Quando comparados, houve uma diferença significativa entre os grupos A e D (p< 0,05), mostrando uma participação efetiva da nicotina na ocorrência de necrose. Os grupos tratados com as drogas mostraram uma resposta clínica importante na melhora da sobrevida dos retalhos, entretanto esses valores não foram estatisticamente significantes (p>0,05). Concluímos que essas drogas mostram alguma efetividade na reversão dos efeitos causados pela nicotina, mas se faz necessário ampliar as amostras para obtermos resultados com significado estatístico.

Single Digit Replantation in Outpatient Surgery: Experience of 120 Cases

ABSTRACT

We performed 120 single digit replantations on an outpatient surgery basis between 1994 and 2001. The levels of amputation were: proximal phalanx in 50 patients, proximal interphalanx in 21, distal interphalanx in 42 and metacarpophalanx in 7 patients. Trauma mechanism included: avulsion (31.7%) and guillotine (68.3%). The mean normothermal ischemia time was 8h (ranging between 6 -15h). Patients stayed in the hospital a maximum of 8 h after the surgery. All of them were told about the possibility of an arterial or venous thrombosis and instructed on how to identify it. Patients who believed any circulatory abnormality was occurrinq, telephoned the surgeon and were immediately assessed. Reassessment was performed in 20 patients, and circulatory deficits were confirmed in 15 patients. There was replantation loss in 12 of the latter patients. The replantation success rate was 90%, and the major determining factor was trauma mechanism and not the postoperative regimen. We suggest that single digit replantation be performed whe never there is an indication and that the availability of hospital beds should not be a decisive factor in the choice of the method of treatment of amputations and single digit devascularizations.

RESUMO

Realizamos 120 reimplantes unidigitais em nível ambulatorial entre 1994 e 2001. Os níveis de amputação foram: falange proximal em 50 pacientes, interfalangeana proximal em 21, interfalangeana distal em 42 e metacarpofalangeana em 7 pacientes. O mecanismo de trauma incluiu: avulsão (31,7%) e guilhotina (68,3%). O tempo médio de isquemia normotérmica foi 8 h (extremos 6-15h). Os pacientes permaneciam em ambiente hospitalar no máximo 8 h após o término da cirurgia. Todos os pacientes foram orientados sobre as possibilidades de trombose arterial ou venosa e de que forma isso poderia ser identificado. Os pacientes que julgassem haver qualquer anormalidade circulatória ou que estivessem em dúvida telefonavam ao cirurgião que os avaliava imediatamente. A reavaliação foi realizada em 20 pacientes, mas o déficit circulatório comprovou-se em 15 pacientes. Destes, houve perda do reimplante em 12 pacientes. A taxa de sucesso dos reimplantes foi de 90%, e o mecanismo do trauma foi o principal fator determinante e não o regime pós-operatório. Sugerimos que o reimplante unidigital seja realizado sempre que houver indicação e que a existência de leitos hospitalares disponíveis não seja o fator decisivo na escolha do método de tratamento das amputações e desvascularizações unidigitais.

Multifunctional Abdominoplasty

ABSTRACT

The present study approaches the description of a procedure that combines mixed incisions(1-4) and associated, oblique, side and upper tractions that are meant to treat patients with large abdominal aprons that underwent massive weight loss after clinical methods or by means of vertical bariatric surgery(5). The technique has been employed since September 2002 and was used in 15 patients. Weight losses ranged from 50 to 145 kg. In vertical bariatric surgery, the correction by plastic surgery was conducted within a year and a half to two postoperative years. In cases of weight loss by clinical means, this time went down to a year, on average. In regard to patients' gendet; the procedure was carried out in 7 men and 8 women. In regard to the causes of weight loss, 13 cases were after vertical bariatric surgery and 2 after clinical methods. In this procedure we approached the abdominal apron, peiformed anterior and posterior trunk modeling, and elevated the pubis and, in some cases, the thighs as well.

RESUMO

O presente artigo aborda a descrição de um procedimento que combina incisões mistas(1-4) e trações associadas, oblíquas, laterais e superiores, que se destinam a tratar pacientes que sofreram grandes perdas ponderais com grandes aventais, pós-métodos clínicos ou por cirurgia bariátrica vertical(5). A técnica vem sendo empregada desde setembro de 2002 e foi efetuada em 15 pacientes e as perdas ponderais variaram entre 50 e 145 kg. Nos casos de cirurgia bariátrica vertical, a correção pela cirurgia plástica foi efetuada em um ano e meio a dois anos do pós-operatório. Já nos casos de perda ponderal por meios clínicos, esse tempo foi, em média, de um ano. Quanto ao sexo dos pacientes, o procedimento foi feito em 7 homens e 8 mulheres. E, quanto às causas de perdas ponderais, 13 casos foram após cirurgia bariátrica vertical e 2 pós-métodos clínicos. Neste procedimento tratamos do avental, realizamos as modelagens anterior e posterior do tronco, além da elevação do púbis e, em alguns casos, das coxas.
