Study of the maxillary advancement and complications in cleft and non-cleft patients underwent to orthognathic surgery

ABSTRACT

Introduction: Cleft lip and palate patients can develop undergrowth of the maxilla due to scars in the process of rehabilitation until adolescence. Objective: Evaluate and compare the horizontal advancement index of the maxilla and the complications index in the cleft and noncleft that underwent orthognathic surgery. Method: Forty six charts of pacientes underwent to surgery between 2006 and 2007 were reviewed and the indexes were obtained. The of patients horizontal advancement mean was obtained from the Ricketts cephalometric measurements of the pre-operative and three months post-operative cephalograms. Results: Cleft patients had a mean horizontal advancement of 4.1 mm and a complication index of 4.7%. The non-cleft patients had a mean horizontal advancement of 4.0 mm and a complication index of 44%. Conclusion: The mean horizontal advancement of the maxilla was similar but the noncleft patients had higher index of complications. The cephalometric method used is effective in the evaluation of both the horizontal and vertical advancement of the maxilla.

RESUMO

Introdução: Os pacientes portadores de fissura lábio-palatina podem desenvolver hipomaxilismo devido a cicatrizes presentes no lábio e no palato durante o processo de reabilitação até a fase da adolescência. Objetivos: Avaliar e comparar o índice de avanço horizontal da maxila e das complicações em cirurgias ortognáticas, às quais foram submetidos os pacientes portadores de fissura lábio-palatina e os pacientes com diagnóstico de hipomaxilismo sem cicatrizes no lábio e no palato. Método: Foram revisados 46 prontuários consecutivos de pacientes submetidos a cirurgia entre 2006 e 2007, sendo obtidos os índices de complicações e avanço da maxila. A análise cefalométrica à Ricketts das telerradiografias foi realizada no pré-operatório e pós-operatório de três meses, nos pacientes fissurados e pacientes não-fissurados. Resultados: Os pacientes fissurados tiveram avanço horizontal médio de 4,1 mm e índice complicação de 4,7%. Os pacientes não-fissurados apresentaram avanço horizontal médio de 4,0 mm e índice de complicações de 44%. Conclusão: O avanço horizontal maxilar médio foi semelhante, mas os pacientes não-fissurados tiveram maior índice de complicações no pós-operatório. Concluiu-se, também, que este método de cefalometria é eficaz para avaliação do avanço maxilar horizontal e vertical.

Romberg's syndrome: a series of cases

ABSTRACT

BACKGROUND: The Romberg's syndrome is a disease that was first described more than a century ago, and during this period had several names. It is characterized by a slow and progressive atrophy of the tissues of a hemiface, can affect all tissues and can also display neurological and ocular manifestations. The aim of this study is to report a series of patients with progressive hemifacial atrophy, addressing treatment options for each individual case. METHODS: Was conducted a retrospective, descriptive and analytic study of the patients treated in our service, Plastic Surgery at the Hospital de Clinicas - Universidade Federal do Paraná, and the Centro de Atendimento Integral ao Fissurado Lábio Palatal (CAIF), at Curitiba, PR, Brazil. RESULTS: Were analyzed 13 patients, 10 (76.9%) female and 3 (23.1%) male. The age of onset of symptoms ranged from 2 to 15 years. The first medical consultation with an specialist occurred at 11.7 years. The left hemiface was most commonly affected. Seven (53.8%) patients underwent surgery. CONCLUSIONS: The Romberg's syndrome is a rare disorder, devastating to facial appearance, and it has been widely studied. Despite great advances in medicine, its etiology definition and the treatment directed to its cause is still only a wish.

RESUMO

INTRODUÇÃO: A síndrome de Romberg é uma moléstia descrita há mais de um século e, nesse período, recebeu várias denominações. É caracterizada por lenta e progressiva atrofia dos tecidos de uma hemiface, podendo acometer todos os tecidos e apresentar, também, manifestações neurológicas e oculares. O objetivo deste estudo é relatar série de casos de pacientes com atrofia hemifacial progressiva, abordando as opções terapêuticas individualizadas para cada caso. MÉTODO: Foi realizado estudo retrospectivo, descritivo e analítico de pacientes atendidos nos ambulatórios de Cirurgia Plástica no Hospital de Clínicas da Universidade Federal do Paraná e no Centro de Atendimento Integral ao Fissurado Lábio Palatal (CAIF), em Curitiba, PR, Brasil. RESULTADOS: Foram analisados 13 pacientes, sendo 10 (76,9%) do sexo feminino e 3 (23,1%) do sexo masculino. A idade de início dos sintomas variou de 2 anos a 15 anos. A primeira consulta ocorreu, em média, aos 11,7 anos. A hemiface mais frequentemente afetada foi a esquerda. Sete (53,8%) pacientes foram submetidos a cirurgia. CONCLUSÕES: A síndrome de Romberg é uma entidade rara, devastadora na aparência facial, que tem sido muito estudada. Apesar dos grandes avanços da medicina, a definição de sua etiologia e um tratamento direcionado a sua causa ainda são apenas um desejo.

Evaluation of maxillofacial trauma at the Hospital Maria Amélia Lins of the Fundação Hospitalar do Estado de Minas Gerais

ABSTRACT

Introduction: The incidence of traumatic injuries located in the face is higher than injuries in other body areas. The Hospital Maria Amélia Lins of the Fundação Hospitalar do Estado de Minas Gerais (FHEMIG) is reference for the attendance of the victims of maxillofacial traumas in Belo Horizonte. Objective: The main goal of the present work is to verify epidemiologic, etiologic, social-economic and therapeutic characteristics of maxillofacial trauma in patients treated in this institution. Method: The data were evaluated through retrospective analysis of patient charts, during the period of 2004 to 2005, including prospective, randomized interview with patients in 2006. Absolute values and percentages were described. The qui-quadrado test was utilized to compare sex and age of the patients with level of significance of 5% (p < 0.05). Results: The facial trauma etiology was in this order traffic accidents, personal violence and falls. It was observed male prevalence of 4 to 1, with ages around 20 to 29 years. In order, their skin color were white, colored and black. The most frequent lesions were nasal fracture, mandibular fracture, zigomatic complex fracture and maxillary fracture. The hospitalar permanence varied of 0 to 14 days. The treatment was similar to the preconized in the literature. Most of the patients had a low social status and a bad oral care. However, it did not affect significantly their treatment. Conclusion: The characteristics of the patients with maxillofacial trauma at the Hospital Maria Amélia Lins - FHEMIG are similar to those described in the literature.

RESUMO

Introdução: A incidência de lesões traumáticas localizadas na face é elevada comparada a injúrias em outras áreas. O Hospital Maria Amélia Lins da Fundação Hospitalar do Estado de Minas Gerais (FHEMIG) é referência para o atendimento de vítimas de trauma bucomaxilofacial em Belo Horizonte. Objetivo: O principal objetivo desse trabalho foi verificar características epidemiológicas, etiológicas, socioeconômicas e terapêuticas do trauma bucomaxilofacial em pacientes atendidos nesta instituição. Método: Foi realizada análise retrospectiva de prontuários de pacientes, durante o período de 2004 e 2005, além de entrevista prospectiva e randomizada de pacientes atendidos em 2006. Os dados coletados foram descritos em valores absolutos e porcentagens. O teste qui-quadrado foi utilizado para comparar o sexo e a idade dos pacientes, sendo considerado significante um p-valor < 0,05. Resultados: A etiologia do trauma facial foi, nesta ordem, acidentes de trânsito, agressões e quedas. Houve uma prevalência masculina de 4:1, com predomínio de pacientes entre 20 e 29 anos. Com relação à cor da pele, houve prevalência de leucodérmicos, seguidos de feodérmicos e melanodérmicos. Os tipos de lesões mais encontradas foram fraturas de nariz, de mandíbula, de complexo zigomático e de maxila. O tempo de permanência hospitalar variou de 0 a 14 dias. O tratamento utilizado não diferiu do preconizado na literatura. A maioria dos pacientes apresentava baixa condição social e uma saúde bucal ruim, entretanto, isso não afetou de maneira significativa o tratamento. Conclusão: As características dos pacientes do Hospital Maria Amélia Lins com trauma bucomaxilofacial assemelham-se às descritas na literatura.

Epidemiological profile of patients undergoing surgery to treat facial fractures in a university hospital

ABSTRACT

Introduction: Facial trauma presents aesthetic, social and economic relevance. Knowing its epidemiology makes it possible to formulate measures for prevention, education and systematization of care.
Methods: Research through the hospital information system, looking for patients who needed to undergo surgery for face fracture between April 2015 and April 2020. Epidemiological data were then collected.
Results: 141 patients were selected. The average age was 34 years, with most males (85%). The predominant etiology was motor vehicle accidents, and the most prevalent surgical fracture was orbit (67%). The median time between trauma and surgery was 18 days. Sixty patients had injuries associated with facial fractures, especially orthopedic and neurological injuries.
Conclusion: The most common etiology of surgical facial fractures was a traffic accident, predominantly among men. Orbit fractures were the most surgically treated.

RESUMO

Introdução: Trauma facial apresenta relevância estética, social e econômica. Conhecer sua epidemiologia permite formular medidas de prevenção, educação e sistematização de atendimento.
Métodos: Triagem, através do sistema de informação hospitalar, buscando pacientes que necessitaram de cirurgia para fratura de face entre abril de 2015 e abril de 2020. Foram, então, coletados dados epidemiológicos.
Resultados: Foram selecionados 141 pacientes. A média de idade foi 34 anos, com maioria do sexo masculino (85%). A etiologia predominante foi acidente com veículo automotor e a fratura cirúrgica mais prevalente foi a de órbita (67%). A mediana de tempo entre o trauma e a cirurgia foi de 18 dias. Sessenta pacientes apresentaram lesões associadas à fratura de face, com destaque para as ortopédicas e neurológicas.
Conclusão: A etiologia mais comum de fraturas faciais cirúrgicas foi acidente de trânsito, sendo o sexo masculino mais afetado. As fraturas de órbita foram as mais tratadas cirurgicamente.

Epidemiological analysis of facial fractures in a secondary hospital

ABSTRACT

Background: Facial traumas are frequently diagnosed within an emergency hospital. Their causes vary and the most prevalent are interpersonal violence and traffic accidents. Epidemiological data is important to understand the problem and to propose technical, preventive and teaching issues. Purpose: The goal of this study was to show the author's experience with facial trauma in a small hospital and evaluate the first 37 months of this service. Methods: It was an observational, prospective, longitudinal study of the patients from the emergency from March 1, 2004 to April 30, 2007. Data from the 136 patients were registered in a specific protocol. Results: There was a male predominance (76%) and the mean age was 29.67 years. The main causes were falls (27.8%) and interpersonal violence (26.3%). The nose was the most affected location with 50% of the cases, followed by orbitozygomatic complex (24.3%). In the mandible fractures the condile was the most affected location (28.6%). Nasal closed reduction with Asch forceps was the most utilized treatment (40.2%), followed by miniplates and screws for the other fracture locations (31.8%). Complications were present in 5.8% of the cases. Follow up of the patients has shown a satisfaction rate of 85%. Conclusion: Incidence, treatment and complication data obtained are according to the literature. Epidemiological data collected will be useful to take measures to prevent facial trauma.

Introdução: Os traumas de face são diagnósticos frequentes no pronto-socorro. Suas causas são variadas, sendo as principais a violência e os acidentes de trânsito. Os dados epidemiológicos são importantes para o conhecimento do problema e para contribuir na realização de medidas preventivas, educacionais e técnicas. Objetivo: O objetivo do trabalho foi relatar a experiência do autor no atendimento aos traumatismos de face em um hospital secundário e avaliar as características epidemiológicas durante os primeiros 37 meses do serviço. Método: Estudo observacional, prospectivo, longitudinal dos pacientes atendidos no Pronto-socorro, no período de 1 de março de 2004 a 30 de abril de 2007. Dados dos 136 pacientes do estudo foram registrados em protocolo específico. Resultados: Houve predomínio do sexo masculino (76%) e a idade média foi de 29,67 anos. As principais causas foram as quedas (27,8%) e a as agressões (26,3%). O nariz foi o local mais acometido (50% dos casos), seguido pelas fraturas do complexo órbito-zigomático (24,3%). Nas fraturas de mandíbula, o côndilo foi o local mais afetado (28,6%). A redução nasal com a pinça de Asch foi tratamento mais utilizado (40,2%), seguido das placas e parafusos nos outros tipos de fraturas (31,8%). O índice de complicações foi de 5,8%. O seguimento pós-operatório demonstrou índice de satisfação dos pacientes de 85%. Conclusão: Os dados de incidência, tratamento e complicações obtidos estão de acordo com a literatura científica. Os dados epidemiológicos obtidos serão utilizados na implementação de medidas para a prevenção do trauma facial.

Facial trauma: analysis of 194 cases

ABSTRACT

Introduction: Facial trauma has become increasingly important in the field of plastic surgery as the number of cases of facial trauma due to traffic accidents and urban violence continue to increase. To assist in this effort, this study examined the characteristics of a sample of facial trauma patients. Methods: Data were collected for 194 cases of facial trauma treated at a referral hospital in Fortaleza (Ceará) between 2005 and 2009 and analyzed using Excel 2003 and Epi Info version 6.04 software. Results: The mean age of the patients was 30.35 years and ranged from 4 to 71 years. A total of 80.4% of all cases involved men, and the male/female ratio was 4.1:1. Cases involving senior citizens and children accounted for 5.7%. The most frequent cause of facial trauma was related to traffic accidents, especially motorcycle accidents, followed by interpersonal violence. Although a majority of patients originated from the countryside, a higher percentage of fractures related to interpersonal violence were observed in patients from the capital city. The facial bone most frequently fractured was the mandible, followed by the nasal and zygomatic bones. The injury most closely associated with facial trauma was cranium encephalic trauma. Conclusions: The incidence of facial fractures can be reduced by implementing educational measures stressing the importance of routinely using seat belts and helmets and decreasing alcohol consumption, as well as the articulation of strategies to cope with hostile situations in order to deter interpersonal violence.

RESUMO

Introdução: O trauma facial tem crescido em importância para a Cirurgia Plástica, especialmente nas últimas quatro décadas, tendo estreita relação com o aumento de acidentes automobilísticos e violência urbana. O presente estudo objetiva traçar um perfil dos pacientes que sofreram esse tipo de trauma. Método: Foram analisados 194 casos de trauma facial atendidos em serviço de referência em Fortaleza (Ceará), entre 2005 e 2009. Os dados obtidos foram entrecruzados e classificados utilizando os programas Excel versão 2003 e Epi Info versão 6.04. Resultados: A média de idade foi de 30,35 anos, variando de 4 a 71 anos. Os homens foram os responsáveis por 80,4%, e proporção homem/mulher foi de 4,1:1. Idosos e crianças corresponderam a 5,7%. Os traumas relacionados a acidentes de trânsito foram prevalentes (60,31%), com destaque para os acidentes com motociclistas, que representam 44,8% do total. Em segundo lugar, ficou a violência interpessoal, com 18,6%. A maioria dos pacientes era oriunda do interior do estado (57,2%), porém os da capital tiveram maior proporção de fraturas relacionadas à violência interpessoal (66,66%). O osso mais fraturado foi a mandíbula (30,49%), seguida pelo osso nasal (22,2%) e pelo zigoma (17,5%). A lesão associada mais frequente foi o TCE (21,1%). Conclusões: A incidência de fraturas faciais pode ser reduzida por medidas educativas, como o uso rotineiro do cinto de segurança e do capacete; pelo menor consumo de álcool e por estratégias para lidar com situações hostis, no intuito de evitar a crescente violência interpessoal.

ABSTRACT

INTRODUCTION: Trauma is a major public health problem in all countries, and injuries involving the facial region are very common. This study aims to assess the epidemiological data of patients who suffered facial trauma with fracture. METHODS: Epidemiological study conducted on 92 patient records. Individuals with facial trauma of any intensity presenting between January 2009 and January 2013 were selected and grouped according to the etiology and location of fractures. Data were presented as absolute values and percentages. RESULTS: There was a higher prevalence of male patients. The most frequent cause of facial trauma was interpersonal violence in most groups, except for those over 45 years old, for whom the predominant causes were falls and car accidents. The frequency of the causes varied according to age: <18 years, 19-25 years old, 26-35 years old, 36-45 years and> 45 years. Seventy-five percent of mandibular fractures were unilateral and 25% bilateral. Surgical fixation with plates was the most common treatment. In our study, fourteen patients had postoperative complications. CONCLUSION: There is a need for systemized care for facial trauma. The variation in the age range found among the studied patients demonstrates that facial trauma includes individuals of any age, although it is more common among young people. We believe that this epidemiological study will enable the improvement of the quality of care for patients with facial trauma.

RESUMO

INTRODUÇÃO: O trauma é um dos principais problemas de saúde pública em todos os países, sendo os que acometem a região facial muito frequentes. O presente trabalho objetiva avaliar dados epidemiológicos de pacientes que sofreram trauma de face com fratura. MÉTODOS: Estudo epidemiológico realizado por meio dos prontuários de 92 pacientes. Foram selecionados indivíduos com trauma facial de qualquer intensidade, no período de janeiro de 2009 a janeiro de 2013, e agrupados de acordo com a etiologia e a localização das fraturas. Os dados coletados foram apresentados em valores absolutos e porcentagens. RESULTADOS: Houve prevalência de pacientes do sexo masculino. A etiologia do trauma de face mais encontrada foi a violência interpessoal, observada na maioria dos grupos, exceto naquele acima de 45 anos, cuja predominância etiológica foi de queda e acidente de automóvel. A incidência das causas variou de acordo com a faixa etária: < 18 anos, de 19 a 25 anos, de 26 a 35 anos, de 36 a 45 anos e > 45 anos. Setenta e cinco por cento das fraturas de mandíbula foram unilaterais e 25%, bilaterais. O tratamento cirúrgico de fixação com placas foi o mais utilizado. No nosso estudo, catorze pacientes apresentaram complicação pós-cirúrgica. CONCLUSÃO: Há necessidade de um atendimento sistematizado para os traumas faciais. A variação na faixa etária encontrada entre os pacientes estudados demonstra que o trauma facial abrange indivíduos em qualquer idade, embora seja maior entre os jovens. Acreditamos que o presente estudo epidemiológico possibilitará a melhora da qualidade no atendimento aos pacientes com trauma facial.

Surgery of depressed closed frontal fractures utilizing the bone itself

ABSTRACT

Closed frontal fractures are generally resolved using prostheses, bone grafts or bone flaps. Acrylic, titanium, vitalium, silastic, acrylic resin, hydroxyapatite, metilmetacrilate prostheses are utilized. But prostheses present the greatest risk of infection and can suffer from extrusion. The iliac bone, ribs, cranial bones and the external structure of the cranial bones are used for bone grafts. Vascularized flaps of the external structure of the cranial bones are also utilized. The author shows repair of a closed frontal fracture with salvage of the depressed frontal bone itself without the necessity of the use of a prosthesis, bone graft or bone flap and with minimum scarring.

RESUMO

As fraturas frontais fechadas são geralmente resolvidas com colocação de próteses, enxertos ou retalhos ósseos. Foram utilizadas próteses de acrílico, titânio, vitalium, silastic, resina acrílica, hidroxiapatita ou metilmetacrilato. As próteses apresentam maior risco de infecção e podem sofrer extrusão. Na enxertia óssea foram utilizados: osso ilíaco, costela, ossos cranianos e a tábua externa dos ossos cranianos. Têm sido também utilizados retalhos vascularizados da tábua externa dos ossos do crânio. O autor mostra a resolução de uma fratura frontal fechada, com recuperação do próprio osso frontal afundado, sem a necessidade do emprego de prótese, enxertia óssea ou retalho e com cicatrizes mínimas.

ABSTRACT

Introduction: Trauma is defined as an injury that leads to changes in an individual's structure due to the energy exchange between tissues and the environment. Because of its location, the maxillofacial skeleton is commonly affected by trauma. Besides, existing studies that seek to address the theme commonly do so in a fragmented way, focused only on a bone structure. Therefore, the present study was proposed as an attempt to bridge this gap in today's literature.
Methods: The search was performed on the platforms PubMed, LILACS, and Cochrane Library using the descriptors: "biomechanical phenomena," "facial injuries" and "fractures, bone," finding 321 articles. The inclusion criteria were: studies published in the last five years, available in full, in English or Portuguese. After using these filters, 50 studies were found, and after analytical reading of the title and available summary, 44 studies were excluded.
Discussion: The mandible is more vulnerable to lateral than frontal impacts; it was shown that in lateral impacts, the most significant stress force was exerted on structures ipsilateral to the impact. It was also demonstrated that dentition's partial or total absence presented greater stress forces on the condyle. In the orbit, there are mainly edge fractures and globe/floor fractures. The first are fractures that tend to be smaller and anteriorly arranged, whereas those on the floor would be the opposite.
Conclusion: In short, several factors can influence the occurrence of facial trauma; among them are the biomechanical phenomena involved.

RESUMO

Introdução: O trauma é definido como um agravo que leva a alterações na estrutura do indivíduo por causa da troca de energia entre os tecidos e o meio. Por causa da sua localização, o esqueleto maxilofacial é comumente acometido por traumas. Além disso, os estudos existentes que buscam abordar a temática comumente a abordam de maneira fragmentada, focada apenas em uma estrutura óssea. Portanto, o presente estudo foi proposto como tentativa de minorar essa lacuna existente na literatura hodierna.
Métodos: A busca foi realizada nas plataformas PubMed, LILACS e Cochrane Library utilizando os descritores: "biomechanical phenomena", "facial injuries" e "fractures, bone", encontrando 321 artigos. Os critérios de inclusão foram: estudos publicados nos últimos 5 anos, disponíveis integralmente, nos idiomas inglês ou português. Após a utilização desses filtros foram encontrados 50 estudos, e após leitura analítica do título e do resumo disponível, foram excluídos 44 estudos.
Discussão: A mandíbula é mais vulnerável aos impactos laterais do que frontais, evidenciou-se que nos impactos laterais a maior força de estresse era exercida em estruturas ipsilaterais ao impacto. Também se demonstrou que a ausência parcial ou total de dentição apresentavam maiores forças de estresse ao côndilo. Na órbita há principalmente fraturas de borda e fraturas de globo/assoalho. A primeira são fraturas que tendem a ser menores e dispostas anteriormente, já as de assoalho, seria o inverso.
Conclusão: Em suma, existem vários fatores que podem influenciar na ocorrência do trauma de face, dentre elas estão os fenômenos biomecânicos envolvidos.

Alterations of the myofunctional orofacial system in the Parry-Romberg syndrome: a critical literature review

ABSTRACT

INTRODUCTION This qualitative literature review analyzed international scientific publications on possible orofacial myofunctional alterations in patients with Parry-Romberg syndrome by using PubMed. METHODS: The survey was conducted in English, between 2002 and 2012, and was limited to human beings of any age. Publications without full access, duplicated by overlapping keywords, literature reviews, letters to the editor, and those not directly related to the research topic were excluded. RESULTS: We identified 719 studies, of which 21 were within the established criteria. Based on the selected studies, patients with Parry-Romberg syndrome may show changes in soft and hard tissues such as atrophy of the sternocleidomastoid, masseter, and pterygoid muscles; atrophy in the cheek region and depression of the nasolabial fold; deviation of the lips and nose; unilateral tongue atrophy; atrophy of the mouth angle; progressive resorption of the maxilla and mandible bone; atrophy of the zygomatic arch and frontal bone, and facial asymmetry; atrophic root development or pathological resorption of permanent tooth numbers; and jaw reduction and delayed eruption of the upper and lower teeth. CONCLUSION: Despite the growing interest in the diagnosis and symptomatic description of individuals with Parry-Romberg syndrome, publications that address functional and interdisciplinary treatments are scarce. Therefore, specific studies aimed at improving the quality of life of these patients are needed.

RESUMO

INTRODUÇÃO Esta revisão qualitativa da literatura analisou publicações científicas internacionais sobre possíveis alterações miofuncionais orofaciais em pacientes acometidos pela Síndrome de Parry-Romberg, por meio da base de dados PubMed. MÉTODOS: O levantamento realizado limitou-se a seres humanos, de qualquer faixa etária, no idioma inglês, entre os anos 2002 e 2012. As publicações sem acesso completo, repetidas por sobreposição das palavras-chave, revisões de literatura, cartas ao editor e as não relacionadas diretamente ao tema foram excluídas. RESULTADOS: Foram identificados 719 estudos, sendo 21 dentro dos critérios estabelecidos. Com base nos estudos selecionados, pacientes acometidos pela Síndrome de Parry-Romberg podem apresentar alterações dos tecidos mole e duro, tais como atrofia dos músculos esternocleidomastoideo, masseter e pterigoideos; atrofia na região da bochecha e depressão da prega nasolabial; desvio dos lábios e nariz; atrofia unilateral da língua; atrofia do ângulo da boca; reabsorção progressiva do osso da maxila e da mandíbula; atrofia do arco zigomático, do osso frontal e assimetria facial; desenvolvimento atrófico das raízes ou reabsorção patológica dos números de dentes permanentes; redução da mandíbula e erupção atrasada dos dentes superiores e inferiores. CONCLUSÃO: Apesar do crescente interesse pelo diagnóstico e pela descrição sintomatológica de indivíduos com Síndrome de Parry-Romberg, a escassez de publicações que abordem tratamentos funcionais e interdisciplinares é evidente. Verifica-se a necessidade da realização de estudos mais específicos que visem à melhoria da qualidade de vida desses pacientes.

Speech therapy and orthognathic surgery: a literature review

ABSTRACT

Introduction: Dentofacial deformities are associated with changes in dental occlusion, which may cause changes in the myofunctional system according to the type of disproportion. These deformities can cause changes and/or adaptations in the stomatognathic system. The objective of this study was to identify the relationship between speech therapy and orthognathic surgery based on refereed publications.
Method: A bibliographic survey was conducted without considering a specific publication period in the databases, PubMed, SciELO and BVS.
Results: Fifteen articles were critically evaluated with respect to the type of study, objectives, number and sex of participants, methodology, results and conclusions. Based on their aims, the articles were grouped into 1) speech-therapy group (STG); 2) changes group (CG); 3) techniques group (GT) and 4) others group (OG).
Conclusion: There is little information regarding direct speech therapy intervention, and the majority of the studies were directed to the aesthetic and functional changes in the hard and soft tissue in patients who underwent orthognathic surgery. Some studies addressed methods and/or specific examination procedures to verify the changes in the soft and hard tissues of the facial profile of patients who underwent orthognathic surgery, and a study detailed the profile of individuals who underwent treatment for dentofacial deformity and apnea-hypopnea syndrome.

RESUMO

Introdução: As deformidades dentofaciais estão associadas às alterações de oclusões dentárias, que podem causar modificações no sistema miofuncional de acordo com o tipo de desproporção. Essas deformidades, podem provocar alterações e/ou adaptações no sistema estomatognático. O objetivo desse estudo foi identificar, com base na literatura arbitrada, a relação entre a Fonoaudiologia e a Cirurgia Ortognática.
Método: Foi realizado levantamento bibliográfico sem período específico, nas bases de dados PubMed, SciELO e BVS.
Resultados: Foram avaliados 15 artigos de forma crítica, quanto ao tipo de estudo, seus objetivos, número e gênero dos participantes, metodologia, resultados e conclusões. Os artigos foram agrupados com base nos seus objetivos em 1) Grupo Fonoaudiologia (GF); 2) Grupo Mudanças (GM); 3) Grupo Técnicas (GT) e 4) Grupo Outros (GO).
Conclusão: Pouco se encontrou sobre a intervenção fonoaudiológica direta, sendo a maioria dos estudos direcionada às mudanças estéticas e funcionais do tecido duro e mole de pacientes submetidos à cirurgia ortognática. Houve também estudos que abordaram métodos e/ou exames específicos para verificar as mudanças nos tecidos moles e duros do perfil facial dos pacientes submetidos à cirurgia ortognática e estudo que detalhou o perfil dos indivíduos que realizaram tratamento para deformidade dentofacial e síndrome da Apneia e Hipopneia do Sono.

Giant lipoma: surgical treatment

ABSTRACT

Introduction: Giant lipomas (>1000g), although rare, have relevant medical importance, as they can affect any region of the body and are diagnosed in advanced stages, in which there is functional impairment. They can also be confused with malignant soft tissue neoplasms. Furthermore, the natural history of the emergence and evolution of giant lipomas is still not exactly known, which justifies further studies. The current study aims to present the results of the surgical treatment of a case of giant lipoma.
Case Report: Case report that occurred in the plastic surgery service of the Roberto Santos Hospital in Salvador, Bahia, in the year 2016. A 62-year-old male farmer, 1.80 m tall, 74 kg, with a dorsal tumor measuring 35 cm in diameter, evaluated through clinical history and computed tomography examination and submitted to elliptical excision and plastic closure in W. There were no postoperative complications. Histopathology confirmed the diagnosis of giant lipoma (2881g). Post-surgical results were considered excellent by the patient and surgical team.
Conclusion: The giant lipoma was satisfactorily removed without complications, with excellent functional and aesthetic results.

RESUMO

Introdução: Lipomas gigantes (>1000g), embora raros, têm importância médica relevante, pois podem acometer qualquer região do corpo e ser diagnosticados em fases avançadas, nas quais há comprometimento funcional. Eles também podem ser confundidos com neoplasias malignas de tecidos moles. Ademais, a história natural de surgimento e evolução de lipomas gigantes ainda não é exatamente conhecida, o que justifica a realização de estudos adicionais. O objetivo do corrente estudo é apresentar os resultados do tratamento cirúrgico de um caso de lipoma gigante.
Relato de Caso: Relato de caso ocorrido no serviço de cirurgia plástica do Hospital Roberto Santos em Salvador, Bahia, no ano de 2016. Homem de 62 anos, lavrador, 1,80m de altura, 74kg, com tumoração dorsal de 35cm de diâmetro, avaliado por meio de história clínica e exame de tomografia computadorizada e submetido a excisão elíptica e fechamento em plástica em W. Não houve complicações pós-operatórias. A histopatologia confirmou o diagnóstico de lipoma gigante (2881g). Os resultados pós-cirúrgicos foram considerados excelentes pelo paciente e equipe cirúrgica.
Conclusão: O lipoma gigante foi removido satisfatoriamente sem complicações, com excelentes resultados funcionais e estéticos.

ABSTRACT

Haemangioma of the nasal bone are very rare pathology, as the bone haemangioma. They are locate, in the most of cases, on the vertebral spine and skull. The nasal bone haemangioma are pathology more rare. On the literature reviewed, there were related 32 cases, all of them with just bone involved, and obscure origin.

RESUMO

Hemangiomas nasais são afecções muito raras. Hemangiomas ósseos, da mesma forma, se localizam preferencialmente na coluna vertebral e no crânio. Os hemangiomas em ossos do nariz, por sua vez, são afecções ainda mais raras. Na literatura pesquisada, são descritos 32 casos. Todos estes, com comprometimento ósseo apenas, e de etiologia ainda obscura. Os autores descrevem o caso, em paciente de 49 anos, com história de tumoração nasal há 10 anos, cujo tratamento realizado foi ressecção cirúrgica.

Calcinosis in an area of late burn: a case report and management discussion

ABSTRACT

Introduction: Cutaneous calcinosis is a rare disease characterized by the precipitation of calcium crystals in the skin tissue. It can be localized, generalized, dystrophic, metastatic, iatrogenic, or idiopathic.
Case Report: Female patient, 66 years old, victim of second and third-degree burns by fire in the lower abdomen and thighs at 8 years old, reaching 25% of the body surface. After 58 years, she was diagnosed with dystrophic calcinosis in the burn scar, which was confirmed through biopsy and histopathological analysis. She underwent surgical excision associated with rotation of the upper abdomen dermal-fat flap and total skin graft for scar correction.
Conclusion: Although the best therapeutic choice is still unclear, treating complications leading to functional disability is essential to reduce morbidity and increase the patient's quality of life.

RESUMO

Introdução: Calcinose cutânea é uma doença rara caracterizada por precipitações de cristais de cálcio no tecido cutâneo. Pode ser localizada ou generalizada, distrófica, metastática, iatrogênica ou idiopática.
Relato do Caso: Paciente feminina, 66 anos, vítima de queimaduras de segundo e terceiro graus por fogo em abdome inferior e coxas aos 8 anos de idade atingindo 25% de superfície corpórea. Após 58 anos, recebeu o diagnóstico de calcinose distrófica na cicatriz da queimadura, contemplado através de biópsia e análise histopatológica. Submetida a exérese cirúrgica associada a rotação de retalho dermogorduroso de abdome superior e enxertia de pele total para correção de cicatrizes.
Conclusão: Embora a melhor escolha terapêutica ainda não seja clara, o tratamento de complicações que podem culminar em incapacidade funcional é fundamental para reduzir a morbidade e aumentar a qualidade de vida do paciente.

ABSTRACT

Dermatofibrosarcoma protuberans (DFSP) is a soft tissue neoplasm with remarkable local aggressiveness, high levels of recurrence (> 40%) and low metastasizing potential. Occurs predominantly in trunk and proximal extremities, being uncommon in scalp, where represents less than 5% of all cases. Usually presents average size of 5 cm at the time of surgical excision. The objective of this study is to report a patient with giant tumor of scalp (20x16x14 cm), emphasizing its peculiarities and therapeutic options.

RESUMO

O dermatofibrossarcoma protuberans (DFSP) é uma neoplasia de tecidos moles de notável agressividade local, altos índices de recorrência (> 40%) e baixo potencial metastatizante. Ocorre principalmente em tronco e porção proximal de extremidades, sendo incomum em couro cabeludo, onde perfaz menos de 5% do total de casos. Habitualmente apresenta dimensões médias de 5 cm à ocasião da ressecção cirúrgica. O objetivo deste trabalho é relatar caso de paciente com tumoração gigante em couro cabeludo (20x16x14 cm), enfatizando suas peculiaridades e discutindo possibilidades terapêuticas.

Laser lipolysis with a 980-nm diode laser: experience with 400 cases

ABSTRACT

INTRODUCTION: Liposuction has undergone several improvements since its first description, including changes in the cannulas, variation in the concentration of the infiltrating solution, and the use of different devices and technologies. The use of laser technology devices for lipolysis and stimulation of skin retraction has contributed to the procedure. This article presents the authors' experience with laser lipolysis in 400 patients, within a 5-year period, and discusses the principles of the technology and its effect on tissues. METHODS: This is a study performed between July 2007 and July 2012 and included 400 patients who underwent laser lipolysis. All procedures were performed following the original protocol - infiltration of cold saline, passage of the cannula with an optic fiber for conducting the energy needed for laser lipolysis, skin retraction, and finally, conventional liposuction. RESULTS: Hospitalization type ranged from outpatient to overnight surgery. Approximately 45% (180 of 400) of patients had minimal bruising, with involvement of 2% or more of the affected body surface. Hematoma, seroma, and dehiscence occurred in a total of 9% (36 of 400) of patients. We did not find any case of thermal burn of the skin. CONCLUSIONS: Laser lipolysis performed according to the described technique was safe and reproducible.

INTRODUÇÃO: A técnica de lipoaspiração recebeu várias contribuições desde sua primeira descrição, como modificações nas cânulas, variação na concentração da solução de infiltração e uso de aparelhos com tecnologias variadas. A utilização de aparelhos com tecnologia laser vem contribuir com o procedimento por meio da lipólise e com o estímulo de retração cutânea. Neste artigo é apresentada a experiência dos autores com a laserlipólise em 400 pacientes, no intervalo de 5 anos, sendo discutidos aspectos dos princípios da tecnologia e sua ação sobre os tecidos. MÉTODO: Estudo realizado entre julho de 2007 e julho de 2012, que incluiu 400 pacientes submetidos a procedimento de laserlipólise. Os procedimentos foram realizados seguindo protocolo original, com infiltração de soro gelado, passagem da cânula com fibra óptica para a condução da energia laser visando à laserlipólise, retração cutânea e, por último, lipoaspiração convencional. RESULTADOS: O período de internação variou de cirurgia em regime ambulatorial a pernoite. Cerca de 45% (180/400 pacientes) dos pacientes evoluíram com equimoses mínimas, com acometimento de 2% ou mais da superfície corporal comprometida. Os casos de hematoma, seroma e deiscência totalizaram 9% (36/400 pacientes). Em nenhum caso foi constatada queimadura por lesão térmica na pele. CONCLUSÕES: O procedimento de laserlipólise realizado com a técnica descrita demonstrou segurança e reprodutibilidade.

Changes in the orofacial myofunctional system after treatment for facial trauma: a critical review of the literature

ABSTRACT

INTRODUCTION: This qualitative literature review aims to highlight international scientific publications selected from the PubMed database that describe the changes in the function of the orofacial myofunctional system after facial trauma and the associated treatment outcomes. METHODS: Studies published in English between 2005 and 2011 and including individuals of all age groups were included in this review. Publications that were not open access, studies appearing more than once because of overlapping keywords, case studies, literature reviews, letters to the editor, and studies that were not directly related to the subject were excluded. RESULTS: A total of 831 studies were identified, 14 of which fulfilled the established criteria. Assessment of jaw function was the most frequent evaluation performed in the included studies, followed by the analysis of treatments. The incidence of condylar fractures was higher than that of mandibular angle fractures. The majority of cases were managed by surgery combined with conservative treatment, followed by surgery alone and conservative treatment alone. Adult men exhibited a higher incidence of facial trauma. Few studies included children or control groups. Further assessment of jaw and clinical functions before and after surgery revealed the following findings. The bite force and occlusal contact area improved after treatment, whereas mandibular asymmetry persisted even after surgery. The maximum mouth opening returned to normal after treatment, although the range of mouth opening was lower in patients with facial trauma than in controls. Persistent mobility in the mandibular teeth and pain were observed even after treatment. Myofunctional therapy resulted in an overall improvement in jaw function. CONCLUSIONS: Although the number of studies on facial trauma is increasing, few studies address the use and benefits of orofacial myofunctional therapy in this field. Further studies on orofacial myofunctional therapy combined with surgery and/or conservative treatment for facial trauma are necessary.

INTRODUÇÃO: Esta revisão qualitativa da literatura levantou publicações científicas internacionais sobre a funcionalidade do sistema miofuncional orofacial nos traumas faciais, por meio da base de dados PubMed. MÉTODO: O levantamento realizado limitou-se a seres humanos, de qualquer faixa etária, no idioma inglês, entre os anos de 2005 e 2011. As publicações sem acesso completo, repetidas por sobreposição das palavras chave, estudos de caso, revisões de literatura, cartas ao editor e as não relacionadas diretamente ao tema foram excluídas. RESULTADOS: Foram identificados 831 estudos, sendo 14 dentro dos critérios estabelecidos. Notou-se que a avaliação mais frequente foi a da função mandibular e depois análise de tratamentos; ocorreu mais fratura no côndilo que ângulo mandibular; utilizou-se mais tratamento cirúrgico juntamente com o conservador, seguido pelo somente cirúrgico e finalmente somente conservador; a maior incidência de traumas faciais foi em adultos do sexo masculino; poucas pesquisas foram realizadas com crianças e grupo-controle; utilizaram-se mais avaliações da função mandibular e clínicas, na maioria pré e pós-cirurgia; a força de mordida e a área oclusal apresentaram melhora pós-tratamento, no entanto a assimetria mandibular permaneceu; os valores de abertura máxima da boca atingiram a normalidade, porém inferiores ao grupo-controle; houve persistência de alterações na mobilidade mandibular e dor, mesmo após o tratamento; e a terapia miofuncional melhorou o quadro de alterações. CONCLUSÃO: É necessário mais publicações sobre o tratamento fonoaudiológico baseado na abordagem miofuncional orofacial nos traumas faciais.

Epidermólise bolhosa em recém-nascidos no Hospital de Base Ary Pinheiro: relato de três casos

RESUMO

Introdução: Epidermólise bolhosa é uma desordem genética que promove o desenvolvimento de bolhas ao trauma mecânico e ao atrito. As alterações fisiopatológicas são devido às anormalidades estruturais e bioquímicas no colágeno.
Objetivo: Descrever três casos de epidermólise bolhosa e o tratamento realizado pelo Serviço de Cirurgia Plástica do Hospital de Base.
Método: Entre 2017 e 2018 foram identificados três casos de recém-nascidos. Os diagnósticos foram feitos entre o 8º e 12º dia de vida, quando os sintomas foram relatados, sendo iniciado o tratamento após o diagnóstico com o exame dermatológico.
Resultados: foram realizadas medidas de restrição aos movimentos para evitar trauma e atrito, curativos com silicone, uso de corticoide tópico e vitamina E.
Discussão: O diagnóstico em recém-nascidos depende principalmente da manifestação dos sintomas nos primeiros dias de vida e do exame dermatológico.
Conclusão: Observou-se a necessidade de métodos diagnósticos precoces e a utilização de curativos que previnam e tratem as lesões existentes.

Facial trauma undergoing surgery: A 10-year epidemiological analysis in the interior of Brazil

ABSTRACT

Introduction: Facial trauma represents significant incapacitation for the victim, as well as a challenge for healthcare teams due to its complexity and involvement of important structures. Analyzing its epidemiology allows us to coordinate public health measures to improve care and prevention.
Method: Observational, descriptive, longitudinal study with a retrospective approach based on the medical records of patients who suffered facial trauma treated by the surgical clinic between 2010 and 2019.
Results: Among in individuals aged 20 to 29 years, which corresponds to 31.76% of total cases. It was also more common in males, corresponding to 78.45% of total cases. Car accidents were the most common cause, described in 22.31% of medical records, and the main fracture, present in 85.83% of cases, was of the bones of the nose.
Conclusion: Victims of oral and maxillofacial trauma treated at the Hospital de Clínicas da Universidade Federal do Triângulo Mineiro are predominantly men in their third decade of life, involved in automobile accidents, with injuries to the bones of the nose that were treated electively.

RESUMO

Introdução: O trauma de face representa significativa incapacitação para a vítima, além de um desafio para as equipes de saúde devido a sua complexidade e envolvimento de estruturas nobres. Analisar a sua epidemiologia permite coordenar medidas em saúde pública para melhorar o atendimento e a prevenção.
Método: Estudo observacional, descritivo, longitudinal, com abordagem retrospectiva a partir dos prontuários dos pacientes vítimas de trauma de face atendidos pela clínica cirúrgica no período entre 2010 e 2019.
Resultados: Dentre os 529 prontuários incluídos no estudo e analisados, 71,08% tratava-se de cirurgias eletivas e o restante, 28,92%, de cirurgias de urgência. O trauma foi mais frequente em indivíduos de 20 a 29 anos, o que corresponde a 31,76% do total de casos. Também foi mais frequente em indivíduos do sexo masculino, correspondendo a 78,45% do total de casos. Acidentes automobilísticos foram a causa mais comum, descrita em 22,31% dos prontuários, e a principal fratura, presente em 85,83% dos casos, foi dos ossos próprios do nariz.
Conclusão: As vítimas de traumatismo bucomaxilofacial atendidas no Hospital de Clínicas da Universidade Federal do Triângulo Mineiro são predominantemente homens na terceira década de vida, envolvidos em acidentes automobilísticos, com lesões em ossos do nariz que foram abordadas de forma eletiva.

Jaw fracture: analysis of 50 surgical cases in a teaching hospital

ABSTRACT

Introduction: The mandible is the largest and strongest of the bones in the face. Due to its topography, it is vulnerable to trauma. Data analysis on mandible fractures is fundamental for treatment and public health policies. This study aims to conduct an epidemiological survey of surgically treated mandibular fractures.
Method: Screening through the hospital information system, seeking patients undergoing surgery for jaw fracture performed at a teaching hospital by the plastic surgery team in Campinas-SP from April 2015 to April 2020. Data were then collected through analysis of medical records.
Results: 50 patients were included, 90% male. The average age was 30.7 years. The predominant etiology was an automobile accident, and the most fractured region in the mandible was the parasymphysis. The median time between trauma and surgery was 19 days. Eleven (22%) patients had some comorbidity. Fourteen patients (28%) were admitted to the Intensive Care Unit (ICU), and 42% underwent surgery with another specialty besides Plastic Surgery. Ten (20%) patients had some complication of the surgery, the most common being surgical wound dehiscence.
Conclusion: There was a predominance among young men and traffic accidents as etiology. Fractures were preferably located in the parasymphysis region and were treated using rigid internal fixation. The high rates of ICU admission, associated injuries, and surgical procedures carried out by other specialties demonstrate the severity of the patients assisted in the service.

RESUMO

Introdução: A mandíbula é o maior e mais forte dos ossos da face. Em razão de sua topografia, apresenta vulnerabilidade nos traumas. A análise de dados sobre as fraturas de mandíbula se mostram fundamentais para auxiliar no tratamento e em políticas de saúde pública. O objetivo desse estudo é realizar um levantamento epidemiológico de fraturas mandibulares tratadas cirurgicamente.
Método: Triagem através do sistema de informação hospitalar, buscando pacientes submetidos a cirurgia para fratura de mandíbula realizadas em um hospital escola pela equipe de cirurgia plástica, em Campinas-SP, de abril de 2015 a abril de 2020. Foram, então, coletados dados por meio da análise de prontuários.
Resultados: Foram incluídos 50 pacientes, sendo 90% do sexo masculino. A média de idade foi 30,7 anos. A etiologia predominante foi acidente automotivo e a região mais fraturada na mandíbula foi a parassínfise. A mediana de tempo entre o trauma e cirurgia foi de 19 dias. Onze (22%) pacientes apresentavam alguma comorbidade. Quatorze pacientes (28%) foram internados em Unidade de Terapia Intensiva (UTI) e 42% operaram com outra especialidade além da Cirurgia Plástica. Dez (20%) pacientes apresentaram alguma complicação da cirurgia, sendo a mais comum a deiscência de ferida operatória.
Conclusão: Houve predominância entre homens jovens e de acidentes de trânsito como etiologia. As fraturas foram localizadas preferencialmente na região da parassínfise e foram tratadas por meio de fixação interna rígida. Os elevados índices de internação em UTI, lesões associadas e realizações de procedimentos cirúrgicos por outras especialidades evidenciam a gravidade dos pacientes assistidos no serviço.
