Liposuction for treatment of axillary hyperhidrosis

ABSTRACT

Introduction: Axillary hyperhidrosis and bromohidrosis are common problems for which many patients seek medical treatment. The removal of axillary sweat glands using liposuction with tumescent anesthesia in an out patient setting is a relatively short and simple procedure with few complications, as seen in this case series. The purpose of is study is to contribute for the spread of this technique. Methods: Twenty five patients were treated with axillary liposuction under tumescent anesthesia. Results: One year after the procedure, three patients had regular results and two required additional liposuction to the same area. Those who had bromohidrosis associated presented good results with significant reduction of the odor. Conclusion: This procedure is efficient and safe when used to treat primary axillary hiperhidrosis and bromohidrosis.

RESUMO

Introdução: A hiperidrose axilar primária e a bromohidrose são problemas comuns que levam vários pacientes a procurar atendimento médico. A remoção das glândulas sudoríparas écrinas e apócrinas da região axilar por meio de lipoaspiração superficial sob anestesia intumescente é um procedimento de pequeno porte, simples e com poucas complicações, o que pode ser observado em nosso estudo. O objetivo deste trabalho é a difusão desta técnica. Método: Vinte e cinco pacientes foram submetidos a lipoaspiração superficial da axila sob anestesia local e sedação. Resultados: Após um ano de acompanhamento pós-operatório, três pacientes apresentaram respostas regulares e dois apresentaram resultados insatisfatórios. Dentre os pacientes analisados, quatorze apresentavam bromohidrose associada e obtiveram respostas satisfatórias com redução significativa ou até mesmo desaparecimento do odor característico. Conclusão: Concluímos que o procedimento é eficaz e seguro para o tratamento de hiperidrose axilar primária e bromohidrose.

Results in surgical rehabilitation for facial palsy

ABSTRACT

Objectives: To describe results in surgical rehabilitation in a series of cases with facial nerve injury, evaluated over a five year period at Hospital de Reabilitação SARAH. Methods: Retrospective study of a series of cases admitted during the years 2001 at 2006 period, with assessment of the surgical rehabilitation using the House Brackmann scale in 29 (10%), 16 (55%) and 13 (45%) female, were submitted to surgical rehabilitation. The following surgical procedures adopted are direct nerve suturing in 2 (7%), end-to-end neurorraphy, facial-hypoglossal in 4 (14%), temporal muscle transposition in 7 (24%), canthoplasty in 12 (41%) and complementary procedures in 4 (14%) cases. The follow up was three years in average. Results: The improvement in all case was at least one degree in HB score. Conclusion: All patients evolved with a functional improvement with a surgical rehabilitation and can be considered a highly relevant for the paralyzed face rehabilitation.

RESUMO

Objetivo: Descrever os resultados da reabilitação cirúrgica de uma série de casos com lesão de nervo facial, avaliados durante um período de cinco anos no Hospital de Reabilitação SARAH-Brasília. Método: Estudo retrospectivo de série de casos admitidos no período de 2001 a 2006, avaliando-se os resultados da reabilitação cirúrgica conforme a escala de House Brackmann (HB), em 29 (10%) pacientes, sendo 16 (55%) homens e 13 (45%) mulheres. Os procedimentos realizados foram microneurorrafia término-terminal em 2 (7%) pacientes, hipoglossofacial em 4 (14%), transposição do músculo temporal em 7 (24%), cantoplastia em 12 (41%) e procedimentos complementares em 4 (14%). O tempo de seguimento pós-operatório foi, em média, de 3 anos. Resultados: Todos os casos evoluíram com a recuperação em no mínimo um grau na escala de HB. Conclusão: Todos os pacientes apresentaram melhora funcional com a reabilitação cirúrgica e esta pode ser considerada um importante componente na reabilitação da face paralisada.

The quilting suture's efficiency in breast reconstruction with transverse rectus abdominis myocutaneous (TRAM) flap plus prolene mesh

ABSTRACT

Introduction: The TRAM flap in breast reconstruction is the best surgery technique when using autogenous tissue although the seroma's formation at abdominal donor site area still remains a problem to be solved. Methods: In this prospective study, 20 patients with T1 to T2 breast cancer were submitted to immediate breast reconstruction with TRAM flap plus prolene mesh at abdominal donor site area. The authors had evaluated the quilting suture's efficiency to prevent seroma after breast reconstruction and the role of suction drainage when combined with quilting sutures to prevent seroma's formation at donor site area.

RESUMO

Introdução: O retalho miocutâneo transverso do reto abdominal (TRAM) constitui-se a opção cirúrgica de eleição nas reconstruções mamárias com tecidos autógenos, porém o seroma na área doadora do retalho ainda apresenta-se como uma complicação freqüente. Método: Neste estudo prospectivo de 20 pacientes com câncer de mama (T1 e T2) submetidas a reconstrução com TRAM mais tela cirúrgica sintética não-absorvível (polipropileno), os autores avaliaram a eficácia dos pontos de adesão na eliminação do seroma quando empregados no retalho cutâneo abdominal. Conclusão: Concluiu-se ser esse método uma maneira efetiva de eliminar a formação de seroma e excluir a necessidade de dreno de sucção abdominal.

Academic league: is it positive? Experience of Liga Baiana de Cirurgia Plástica

ABSTRACT

Introduction: Plastic Surgery academic league (LBCP) of the medical school of Federal University of Bahia (UFBA) is an academic league with no financial interests. Objective: The aim of this study is evaluate the main positive and negative points of LBCP, member's satisfaction and knowledge after one year. Method: The questioner applied to the members analyzed, by the end of the year, the main LBCP events during May 2006 and July 2007. They were: weekly lectures, activities in plastic surgery clinics at Universitary Hospital, Brazilian Plastic Surgery Society - Regional Bahia meetings, surgical instrumentation of plastic surgeries, events organization and scientific development. Results: The percentage of student interested in acting as plastic surgeon grew from 28.6% to 78.6%. Satisfaction and improvement of knowledge ranged 84% and 78%. Conclusion: Academic leagues are powerful instruments for graduates insertion in scientific and medical activities. They bring real contact and knowledge, demystifying unknown aspects of the specialty.

RESUMO

Introdução: A Liga Baiana de Cirurgia Plástica (LBCP) da Faculdade de Medicina da Bahia da Universidade Federal da Bahia é uma liga acadêmica sem fins lucrativos, fundada em maio de 2006 com a finalidade de aproximar graduandos à especialidade médica de Cirurgia Plástica. Objetivo: O presente estudo pretende avaliar os pontos positivos e negativos, o grau de satisfação e aprendizado dos membros após um ano de implantação da LBCP. Método: Foram analisadas, por meio de questionário aplicado aos membros no final do ano letivo, todas as atividades realizadas pela LBCP, no período de maio de 2006 a julho de 2007. As atividades foram: reuniões semanais com aulas sobre os principais assuntos em cirurgia plástica, atendimento ambulatorial e instrumentação cirúrgica no Hospital Universitário, reuniões da Sociedade Brasileira de Cirurgia Plástica - Regional Bahia, organização de eventos e elaboração de trabalhos científicos. Resultados: A taxa de alunos que se interessavam pela especialidade era 28,6% antes da liga, evoluindo para 78,6%. As taxas de satisfação e aprendizado após 1 ano de LBCP foram de 84% e 78%, respectivamente. Conclusão: As ligas acadêmicas são instrumentos de forte poder para inserção na atividade médica e científica, propiciam melhor contato e conhecimento da especialidade, desmistificando assuntos outrora desconhecidos.

Comparative study of tissue biopsy and surface swab culture for the diagnosis of infection in burn patients at HPS-Porto Alegre

ABSTRACT

Introduction: Burn wound infection is the most common type of complication in burn patients. The gold standard for its diagnosis is a biopsy, but the most utilized method in the burn centers is the swab. Objective: The main goal of this study is to compare the results obtained through biopsy and swab when diagnosing burn wound infection amongst patients admitted in the Burn Unit of HPS - Porto Alegre. Also, it is an objective to determine the rate of burn wound infection and to identify the different types of germs found in these cultures. Method: A prospective clinical study was performed from June to October 2007. Throughout that period, swabs and biopsies were taken in 25 patients during the first, and/or second, and/or third week of admission in the facility. Results: A total of 74 exams were performed (37 swabs and 37 biopsies). The sensibility of the swab was 100%, and its specificity was 77.4%. 20% of the patients presented burn wound infection and the most common germ found in the cultures (26.6%) was S. aureus. Conclusion: When the swab presents a negative result, we have an excellent exam to exclude the possibility of burn wound infection.

RESUMO

Introdução: A infecção de pele é a complicação mais comum no paciente queimado. O padrão-ouro para o seu diagnóstico é a biópsia de pele, porém o método mais utilizado nos centros de queimados é o "swab". Objetivo: Comparar os resultados obtidos pela cultura entre a biópsia e o "swab" da escara, para o diagnóstico de infecção de pele nos pacientes internados na Unidade de Tratamento de Queimados do HPS de Porto Alegre; além disso, determinou-se a taxa de infecção das escaras e identificaram-se os tipos de germes encontrados nas culturas. Método: Realizou-se um estudo clínico prospectivo entre junho e outubro de 2007, em que foi realizada a coleta de "swab" e biópsia de pele em 25 pacientes, entre a primeira e a terceira semanas de internação. Resultados: Foram coletados 74 exames (37 "swabs" e 37 biópsias). A sensibilidade do "swab" foi de 100% e sua especificidade de 77,4%. Vinte por cento dos pacientes apresentaram infecção de pele, sendo o germe mais encontrado nas culturas (26,6%) o S. aureus. Conclusão: O "swab", quando for negativo, é um excelente exame para excluir a hipótese de infecção de pele.

Nipple-areola complex position and scar extension following-up after reduction mammaplasty with inferior pedicle technique

ABSTRACT

Introduction: The objective of the present work is to propose a long-term evaluation model of the breasts after reduction mammaplasty. Method: Forty patients who underwent reduction mammaplasty using an inferior pedicle technique were evaluated. Supraesternal notch-to-nipple (SNN) and nipple-to-inframammary fold (NIF) distances in addition to horizontal scar (HS) and vertical scar (VS) lengths were measured. Paired T-test was utilized for statistic analyses, with significant values for p<0.05. Results: SNN increased slowly after the sixth postoperative month. However, NIF was longer since the first postoperative month and it rose quickly. Although VS augmented steadily after the first postoperative month, HS followed in random. Conclusion: In inferior pedicle flap techniques the vertical part of the scar should be the smallest possible in order to prevent further stretching.

RESUMO

Introdução: O objetivo deste trabalho é propor um modelo de avaliação da evolução das mamas após mamoplastia redutora. Método: Foram avaliadas 40 pacientes submetidas a mamoplastia redutora com utilização do retalho areolado de pedículo inferior. Foram medidas as distâncias da fúrcula esternal ao mamilo (FM), do mamilo ao sulco submamário (MS), além da extensão da cicatrizes horizontal (CH) e vertical (CV). Para a análise estatística foi utilizado o teste t pareado e foram considerados significativos valores correspondentes para p<0,05. Resultados: FM aumentou paulatinamente a partir do 6o mês, já MS foi maior a partir do 30o dia e ascendeu de maneira gradual. CH evoluiu ao acaso e CV aumentou de forma regular a partir do 30o dia. Conclusão: Em técnicas que utilizam o pedículo de base inferior, o componente vertical da cicatriz deve ser o menor possível, visto que esse se alonga muito no decorrer do tempo.

Mastoplasty by crossed flaps technique: reassessment of technique

ABSTRACT

Introduction: The definition and relation between content and continent are fundamental for the treatment of mammary ptosis. The crossed flaps technique described by Sperli, in 1972, for the treatment of mammary ptosis aims to reach the balance between content and continent and to provide harmonic mammary cone. Method: Sixty patients with ages between 24 to 58 years, with mammary ptosis varying from I, II and III grades, without hypertrophy, were treated by the crossed flaps technique. In case breast reduction was necessary, recommendations of Bozola according his classification for breast reduction were applied. Results: The complications were basically the same as noted in other techniques for breast reduction and mammary ptosis. Conclusion: The technique remolded the mammary content, projection of mammary cone and restored the balance between mammary content and continent. The Bozola's volume reduction principles simplified and improved the application of this technique.

RESUMO

Introdução: A definição e a relação entre conteúdo e continente mamários são fundamentais para o tratamento das ptoses mamárias. A técnica dos retalhos cruzados descrita por Sperli, em 1972, para o tratamento das ptoses mamárias tem como objetivo restabelecer o equilíbrio entre conteúdo e continente mamários e fornecer cones mamários harmônicos. Método: Sessenta pacientes com idades variando entre 24 a 58 anos, com ptoses mamárias graus I, II e III sem hipertrofia ou com pequenas a médias hipertrofias, foram tratadas com a técnica dos retalhos cruzados. Quando houve necessidade de redução de volume foram aplicados os princípios da classificação de Bozola. Resultados: As complicações foram as mesmas observadas em outras técnicas de redução de volume e ptose mamárias. Conclusão: A técnica remodelou o conteúdo mamário, por meio de discreta redução da largura da base mamária, projeção do cone mamário e restauração do equilíbrio entre o conteúdo e o continente mamários. Os princípios de redução de volume de Bozola facilitaram e aprimoraram a aplicação da técnica.

Ethyl-2-cyanoacrylate (Epiglu®) evaluation in the skin closure: study in rats

ABSTRACT

Background: The Ethyl-2-cyanoacrylate (ECA) is a synthetic tissue adhesive used by some physicians in clinical practice in wound closures. They polymerize rapidly to adhere tightly to proteinaceous surfaces, form a strong and flexible bond. This study was performed to evaluate the efficacy and biocompatibility of this polymer (Epiglu®, of Meyer-Haake company, Germany) in skin wound closures, in rat model. Methods: Ten male Wistar rats were subjected to three incisions which were closed using different treatments. The central incision was treated witch Simple suture of nylon 4.0 (control group). Left incision was treated with sub-dermis suture associated with ECA, and right incision only with ECA. Rats were sacrificed on days 3, 7, 14, 21 and 30 after the intervention. The biocompatibility was evaluated due to direct observation: necrosis, inflammation, dermatitis, infection and dehiscence; and microscopy: inflammatory process, angiogenesis, number of fibroblasts, collagen, giants cells and eosynophils. Results: The wound closured only with ECA presented a higher dehiscence level (p < 0.05). It was not observed another macro or microscopic significant difference comparing to the control group (p > 0.05). Conclusion: The use of the ECA is a good option to the skin closure, with good biocompatibility. Special attention must receive the wounds with some tension, where the association with a sub-dermis suture becomes imperative.

RESUMO

Introdução: O Etil-2-cianoacrilato (ECA) é um adesivo tecidual utilizado por alguns médicos na prática clínica para fechamento de lesões de pele. São substâncias que se autopolimerizam quando em contato com a ferida, formando uma cola forte e flexível. Este trabalho foi realizado visando à avaliação da eficácia e da biocompatibilidade deste polímero (Epiglu®, da empresa Meyer- Haake, Alemanha) na síntese de pele, em modelo experimental. Método: Dez ratos Wistar machos foram submetidos a três incisões longitudinais no dorso. A incisão central foi tratada com sutura de pontos simples de nylon 4.0 (grupo controle), a esquerda com sutura subdérmica de nylon 4.0 associado ao ECA, e a direita apenas o ECA. Os animais foram sacrificados nos dias 3, 7, 14, 21 e 30 pós-operatórios. A biocompatibilidade foi avaliada sob observação direta: necrose, inflamação, dermatite de contato, infecção e deiscência; e à microscopia: reação inflamatória, angiogênese, fibroblastos, colágeno, células gigantes e eosinófilos. Resultados: As sínteses realizadas apenas com o ECA apresentaram maior índice de deiscência (p < 0,05). Todos os outros critérios macro e microscópicos avaliados não apresentaram diferenças estatísticas em relação ao controle (p > 0,05). Conclusão: O uso do ECA é uma boa opção de síntese da pele, com boa biocompatibilidade. Atenção deve ser feita às lesões que apresentem algum grau de tensão, onde a associação com a sutura subdérmica torna-se obrigatória.

Incisional hernias treatment on multifunctional abdominoplasties

ABSTRACT

Introduction: Abdominal wall hernias frequently occur after open bariatric surgical procedures, which may be imperceptible during physical examination or very remarkable as big eventrations with loss of domain. Methods: In this article, the authors report their experience with the treatment of these hernias during the multifunctional abdominoplasty, thru the isolation and reduction of the herniary sac, primary synthesis of the defect and plicature of the aponeurosis, without using aloplastic materials. The authors made a retrospective study with 44 patients, over five and a half year's period, where the effectiveness and safety of the technique were analized.

RESUMO

Introdução: Os pacientes que são submetidos a cirurgias bariátricas freqüentemente evoluem com hérnias de parede abdominal, sejam estas imperceptíveis ao exame físico ou mesmo grandes eventrações com perda de domicílio. Método: Neste artigo, os autores relatam suas experiências com o tratamento destas hérnias durante a abdominoplastia multifuncional, por meio do isolamento e redução do saco herniário e síntese primária da falha aponeurótica com posterior plicatura desta, sem o uso de materiais aloplásticos. Foi realizado um estudo retrospectivo abrangendo um total de 44 pacientes, num período de cinco anos e meio, onde a eficácia e segurança da técnica foram analisadas.

Composed gluteal-crural lift

ABSTRACT

Introduction: The authors propose a new technique of buttocks and thighs lift in massive weigh loss patients, overlapping the volume of the buttocks, elevating and projecting them. Methods: The trocantheric area and the internal aspect of the thighs, when necessary, are liposuctioned. They remove the skin excess through an incision that initiates in the intergluteal furrow and outlines the posterior and anterior iliac crest. It goes down for the pleat of the groins until the internal face of the thigh and, in "L", it goes down near the knees. The skin resections are always done above the fascia superficialis. The surgery begins in ventral decubitus for the suspension of the buttocks and it is completed in dorsal decubitus with the suspension of the thighs.

RESUMO

Introdução: Os autores propõem uma nova técnica de suspensão de nádegas e coxas em pacientes pós-grandes emagrecimentos, sobrepondo o volume das nádegas, elevando-as e projetando-as. Método: A região trocantérica e a face interna das coxas, quando necessário, são lipoaspiradas. Retira-se o excesso de pele por incisão que parte do sulco interglúteo, contorna a crista ilíaca posterior e anterior, desce pela prega inguinal até a face interna da coxa e, em "L", desce até acima dos joelhos. As ressecções de pele são feitas sempre acima da fascia superficialis. Inicia-se a cirurgia em decúbito ventral para a suspensão das nádegas e conclui-se em decúbito dorsal com a suspensão das coxas.

Ligamental mammaplasty

ABSTRACT

Introduction: The purpose of this paper is to present a technique for the surgical treatment of the changes in breast positioning, either in the anteroposterior, cephalocaudal or mediolateral directions. Methods: This surgical technique mainly consists of strengthening the ligaments, including the breast capsule (Giraldés's ligament) and its attachments (Cooper's ligaments), which are responsible for supporting and suspending the breast. For that purpose, it is necessary to resect the excess areolar, periareolar and infraerolar skin, both on epidermic and partially dermic levels, as observed before surgery. After the proper repositioning of the ligaments, we then proceed to the resection of the remaining measured excess of skin in these areas. Conclusion: This technique is less traumatic to the mammary gland and the fat tissue, and produces good aesthetic results with no functional or sensibility changes. It can be easily and quickly performed, even in ambulatory surgery, using local anesthesia and sedation.

RESUMO

Introdução: Este trabalho apresenta a possibilidade de tratamento cirúrgico das alterações de posicionamento mamário nos sentidos ântero-posterior, crânio-caudal e látero-medial. Método: A técnica utilizada baseia-se na correção das estruturas ligamentares que são a base da fixação, sustentação e estática de todo o conjunto mamário, ou seja, a cápsula mamária (ligamento de Giraldés) e seus prolongamentos (ligamentos de Cooper). Para o tratamento das estruturas ligamentares e aréolas é realizado primariamente a ressecção do excesso de pele areolar, peri-areolar e infra-areolar, nas porções epidérmicas e parcialmente dérmicas, baseado no excedente de pele preexistente nestas regiões, associado ao excedente provocado após o reposicionamento adequado das estruturas ligamentares. Conclusão: Esta técnica é pouco traumática aos tecidos mamários glandulares e gordurosos, não acarreta alterações funcionais e/ou sensitivas, tem baixa complexidade, facilidade e rapidez na execução, podendo ser realizada ambulatorialmente, com anestesia local associada à sedação, aliada a resultados estéticos satisfatórios.

ABSTRACT

Introduction: The combined surgery of mastoplasty and prosthesis include is far known as a challenge, due to the high potential risk of post operative complications, and also a high number of surgical retouches. Methods: In this paper, a new approach named the mirror D technique is suggested, using also skin resection and prosthesis inclusion to the treatment of hypoplasic breasts, both pre surgical treated or not, in any ptosis degree, and once the patients complain was the ptosis instead of the volume lost. The described procedure uses a proper marking, that guide the block resection including both parenquima and skin used to give symmetry, the pedicle chose is the medial one. Silicone-textured-high-profile-breast-implants are placed always in equal volume under the muscle, and the technique results in a final vertical scar. Conclusion: The technique above suggested promotes an easy surgical perform, a decrease of complications, increasing the patients satisfaction with an aesthetical long lasting results.

RESUMO

Introdução: A inclusão de implante mamário combinada com pexia é uma cirurgia desafiadora, não somente pela ausência de procedimento padrão, mas por se mostrar como uma cirurgia com elevado potencial de complicações, entre elas alto índice de revisão pós-cirúrgica. Método: Neste trabalho, é descrita uma junção de novas táticas, que leva o nome de técnica em D espelhado, que corresponde a tratamento de mastopexia e inclusão de implantes de silicone em mamas hipoplásicas associadas à ptose moderada a grave, em pacientes com queixa de ptose e não do tamanho, pacientes estes com tolerabilidade para somente leves aumentos. O procedimento descrito faz uso de marcação própria, que determina ressecção em bloco de pele e parênquima subjacente para simetrização, retalho de pedículo medial, implantes de silicone texturizado perfil super alto, volumes iguais bilateralmente, são posicionados em plano submuscular, resultando em uma cicatriz final vertical. Conclusão: A técnica aqui descrita, além de fácil execução, proporciona alto grau de satisfação, diminuição dos índices de complicações e resultados estéticos duradouros.

ABSTRACT

The umbilicus is an essential feature of the beauty of the abdomen. Scars around it, caused by relocation during abdominoplasties, may harm the final results of a well performed surgery. After a brief historic review, the author presents a fast and simple method of neoumbilicoplasty that avoids visible scars and yields goods results, and therefore encouraging its routine utilization, in substitution of the reimplantation of the natural umbilicus, nowadays widely used by plastic surgeons.

RESUMO

O umbigo é um componente essencial à beleza do abdome e as cicatrizes periumbilicais decorrentes da sua transposição nas abdominoplastias podem prejudicar o resultado final de um abdome bem operado. Após uma breve revisão histórica, o autor demonstra técnica de neo-onfaloplastia simples, rápida, que evita cicatrizes aparentes e produz bons resultados, estimulando a sua utilização rotineira, em substituição à transposição umbilical, amplamente divulgada entre os cirurgiões plásticos.

Reconstruction of the nostril floor in the surgery of unilateral primary cleft lip

ABSTRACT

Introduction: The surgical treatment of the unilateral complete cleft lip consists of an association of techniques for reconstruction of the lip and nose. Some techniques of reconstruction of the nostril floor may be associated to these with the intention of obtaining a better symmetry in the repairing of these structures. Objective: The objective of this work is to demonstrate the reconstruction of the nostril floor in the surgery of unilateral primary cleft lip. Method: One hundred e thirty-three pacients with unilateral complete cleft lip were treated with a combination of techniques of Millard II and McComb and the confection of extended flaps for improvement of the nostril floor. Results: The final results had been considered satisfactory in the great majority of the patients, having as main complication the permanence of the asymmetry of the nasal openings. Conclusion: The reconstruction of the nostril floor is an important time in the surgery of unilateral primary cleft lip.

RESUMO

Introdução: O tratamento cirúrgico da fissura labial completa unilateral consiste em uma associação de técnicas para reconstrução do lábio e do nariz. Podem ser associadas a estas, técnicas de reconstrução do assoalho nasal com o intuito de se conseguir uma melhor simetria na reparação destas estruturas. Objetivo: O objetivo deste trabalho é demonstrar a reconstrução do assoalho nasal nas rinoqueiloplastias primárias unilaterais. Método: Foram tratados 133 pacientes portadores de fissura labial completa unilateral, utilizandose uma combinação de técnicas de Millard II e McComb e a confecção de retalhos estendidos para melhora do assoalho nasal. Resultados: Os resultados obtidos foram considerados satisfatórios na grande maioria dos pacientes, tendo como principal complicação a permanência da assimetria das aberturas nasais. Conclusão: A reconstrução do assoalho nasal é um tempo importante da rinoqueiloplastia primária unilateral, visando-se à obtenção de melhores resultados.

ABSTRACT

Introduction: Graves ophthalmopathy it is an autoimmune disease. Surgical treatment for expansion and decompression of the orbits are indicated in 5% of the patients. A literature revision has been done, related to the physiopathology, surgical treatment indication, critical review of the existing techniques and description of an iatrogenic surgical lesion and how it was corrected. Case report: Patient with an iatrogenic severe deformity, after surgery to correct Graves ophthalmopathy disease. She had bilateral exophthalmia and proptose and the overall thickness of the superior eyelids were open, with chronic exposition of the cornea. It was treated surgically with orbital walls osteotomy and orbital fat resection and synthesis of the eyelids tissues in three different planes in a way to avoid coincident plane suture. It was possible to obtain complete reestablishment of the continuity of the eyelid and regression of the exophthalmia. Conclusion: Literature revision showed that Graves Ophthalmopathy should be treated clinically in 95% of the cases. Surgical treatment is indicated only for severe cases and in the resting healing phase of the disease. Improper surgical indication of the present case, at the first operation, should be of a teaching experience to all of us. Surgical expansion of three walls of the orbits associated to orbital lipectomy was important to reduce the pressure over the eyelids and correct the iatrogenia.

Introdução: A oftalmopatia de Graves é doença auto-imune. A cirurgia para expandir e descomprimir a órbita está reservada a apenas 5% dos casos. Foi realizada uma revisão da literatura referente a fisiopatologia da doença, indicação do tratamento cirúrgico, técnicas existentes, bem como descrição de uma paciente com lesão iatrogênica cirúrgica. Relato do caso: Paciente com grave deformidade iatrogênica pós-cirurgia, para corrigir a oftalmopatia de Graves. Apresentava, exoftalmia, proptose bilateral e abertura iatrogênica ao nível do sulco das pálpebras superiores, exposição crônica da córnea. Foi tratada cirurgicamente com: osteotomia das paredes das órbitas e ressecção de gordura periocular e síntese dos tecidos palpebrais em três diferentes planos e dispostas de forma não coincidentes. Obtidos completo restabelecimento da integridade palpebral e regressão do exoftalmo. Conclusão: Revisão da literatura mostra que a oftalmopatia de Graves deve ser tratada em 95% dos casos clinicamente. O tratamento cirúrgico está reservado para os casos graves e na fase quiescente cicatricial da doença. A indicação cirúrgica inadequada do presente caso na primeira cirurgia deve servir a todos nós de aprendizado. A expansão de três paredes orbitárias associada à lipectomia foi importante para aliviar a pressão sobre as pálpebras e corrigir a iatrogenia.

ABSTRACT

Introduction: Despite the fact that lipomas are very common mesenchymal tumors, the subfascial variant is quite rare. It is subdivided into intermuscular and intramuscular. The intermuscular type is less common. Giant lipomas are also rarely found and are characterized by measuring at least 10 centimeters in one dimension or weigh at least 1000 grams. Case report: In this article, we describe the case of a patient presenting a giant intermuscular lipoma located in the thigh.

Introdução: Contrastando com o fato de que os lipomas são tumores mesenquimais muito comuns, a variante subfascial é bastante rara. Esta é subdividida em intermuscular e intramuscular. O tipo intermuscular é o menos comum. Os lipomas gigantes também são raramente encontrados e são caracterizados por medir, no mínimo, 10 centímetros em uma das dimensões ou pesar ao menos 1000 gramas. Relato do caso: No presente artigo, relatamos o caso de uma paciente apresentando um lipoma intermuscular gigante localizado na coxa.

Post-traumatic reconstructions orbitals with utilization of the osteofascials flaps of the temporoparietal fascia and bone graft

ABSTRACT

Purpose: The purpose of the present paper is to present the reconstructive method of posttraumatic orbital abnormalities with osteofascial flaps and bone grafts as an excelent reconstructive method and one more therapeutic option for selected patients. Cases report: The present paper reported the authors experience with the application of the temporoparietal osteofascial flaps or autogenous skull bone graft in the reconstruction of the traumatic orbital abnormalities in three consecutive cases. Conclusions: It was found favorable results with low complications rates. The supremacy of temporoparietal osteofascial flaps and autogenous skull graft in facial reconstructions of selected pacients is confirmed.

RESUMO

Objetivo: O objetivo do presente trabalho é apresentar os métodos reconstrutivos de anormalidades pós-traumáticas orbitárias, como enxerto ósseo autólogo e retalho osteofascial de fáscia temporoparietal, como alguns dos bons métodos reconstrutivos e como opção terapêutica para esses pacientes. Relato de casos: No presente artigo, relatouse a experiência do uso dos retalhos osteofasciais de fáscia temporoparietal e enxerto ósseo autólogo de calvário na reconstrução das anormalidades orbitárias pós-traumáticas em três casos consecutivos. Conclusão: Com a obtenção de resultados favoráveis e baixa taxa de complicações no presente trabalho, confirmamos os dados da literatura quanto à supremacia na utilização dos retalhos osteofasciais de fáscia temporoparietal e enxerto ósseo nas reconstruções faciais em casos selecionados.
