Silicone breast implant: loops to facilitate its placement and to give better precision to the positioning

ABSTRACT

Background: Transaxillary breast augmentation has the disadvantages of greater difficulties on its implantation and the possibility of displacement and malpositioning (rotation), especially if anatomic ("tear drop") implants used. The polyurethane shell also increase he friction forces to its placement trough an axillary's incision. With the intent to avoid these problems, the authors designed four small Dacron loops on the implants undersurface, so that they could have been pulled through the skin with a 2-0 nylon. Methods: This operation was performed in six patients (ages 21 to 41) and the implants were placed transaxillary in the subfascial plane (under pectoralis major fascia), with fiber optic light source. The implants with Dacron loops had a polyurethane shell, around base and natural profile. The volumes varied from 120 to 205 ml. Two 2-0 nylon stitches were passed throu gh the skin at the medial and lateral aspects of inframammary fold. After implantation, the stitches were pulled out to determinate the adjustment of the implant to its ideal position. A third stitch in the upper lateral loop was exteriorized through the incision and helped the correct placement on the implant as well. Results: No inflammatory reaction or capsular contraction has been detected to the present date. The consistence of all breasts is normal and the axillary scars are all of good quality. The implants with loops can be used also if another incision is chosen (inframammary or periareolar). Conclusions: The authors concluded that the loops allowed an easy and precise transaxillary placement of natural profile breast implants with a polyurethane shell.

RESUMO

Introdução: A via axilar tem a desvantagem de maior dificuldade na colocação dos implantes e possibilidade do implante ficar rodado, mal posicionado, especialmente se for em forma de gota. O revestimento com poliuretano aumenta o atrito, dificultando também sua colocação via axilar. Tentando evitar estes percalços, os autores decidiram colocar quatro pequenas alças de Dacron sob a base dos implantes, de forma a se poder tracioná-los com fios passados pela pele. Método: Foram operadas seis pacientes, com idades variando de 21 a 41 anos. Os implantes mamários foram colocados no plano subfascial, via axilar, com visualização por fibra óptica. Foram passados dois fios de náilon 2-0 transcutâneos, nas extremidades medial e lateral do sulco inframamário. Após a introdução do implante pela incisão axilar, os fios foram tracionados, determinando o deslocamento do implante para a posição ideal. O terceiro fio, fixado na alça súpero-lateral, saía pela incisão e também auxiliou na determinação da adequada posição do implante. Resultados: Nenhuma paciente apresentou reação inflamatória local ou formação de cápsula até a presente data. A consistência em todas as mamas era normal. As cicatrizes axilares apresentaram excelente qualidade. Os implantes com alças podem ser utilizados também por outras vias, como a infra-axilar e transareolar. Conclusão: Os autores concluíram que as alças permitiram alocar os implantes mamários de perfil natural e superfície de poliuretano via axilar com facilidade e precisão.

Animal model of keloids using genetically modified fibroblasts

ABSTRACT

Introduction: There is little understanding of the molecular mechanisms leading to the formation of keloids or hypertrophic scars, nor is there an adequate animal model for their study, as they occur only in humans. A leading theory proposes that upregulation of TGF-B leads to the formation of these scars. Objective: To create an animal model of keloid using genetically modified cells encoding TGF-B. Methods: Normal human dermal fibroblasts were genetically modified to overexpress TGF-B as either the wild type latent molecule or as a mutant active molecule. The transfected fibroblasts and controls were implanted intradermally into athymic mice and fibrotic tissue formation by these cells was analyzed at various time points by histology and immunostaining. Results: After intradermal injection into athymic mice, only those fibroblasts expressing active TGF-B formed "keloid-like" nodules containing collagen and fibronectin. These structures persisted longer microscopically than implants of control fibroblasts or fibroblasts overexpressing latent TGF-B, but eventually regressed. Conclusions: The injection of transfected fibroblasts encoding TGF-B led to formation of keloid-like nodules in athymic mice skin. TGF-B needs to be in the active form to produce the fibrotic nodules.

RESUMO

Introdução: Existe pouco entendimento a respeito dos mecanismos moleculares que levam à formação dos quelóides e cicatrizes hipertróficas, assim como não existe um modelo animal adequado para seu estudo, visto que só ocorrem em humanos. A teoria mais aceita propõe que um aumento na regulação do TGF-B leva à formação dessas cicatrizes. Objetivo: Criar um modelo animal de quelóide utilizando células geneticamente modificadas codificando o TGF-B. Método: Fibroblastos dérmicos humanos foram geneticamente modificados para aumentarem a expressão de TGF-B na forma selvagem ou latente ou na forma mutante ou ativa. Os fibroblastos transfectados foram transplantados intradermicamente em camundongos atímicos, e a formação de tecido fibroso analisada em diferentes intervalos de tempo por meio de histologia e imunohistoquímica. Resultados: Após a injeção intradérmica nos camundongos atímicos, apenas os fibroblastos expressando a forma ativa do TGF-B formaram nódulos "quelóide-like", contendo colágeno e fibronectina. Estas estruturas persistiram microscopicamente por mais tempo que os implantes dos fibroblastos controle e os expressando a forma latente do TGF-B. Conclusões: A injeção de fibroblastos transfectados codificando o TGF-B levou à formação de nódulos semelhantes ao quelóide na pele de camundongos atímicos. O TGF-B precisa estar em sua forma ativa para produzir os nódulos fibróticos.

Body contourning surgery after massive weight loss

ABSTRACT

Background: The great skin excess resultant after the weight loss of patients submitted to a surgical treatment for morbid obesity is unpleasant and most of the time debilitating. The treatment of these sequelae often needs several surgical interventions. Method: The study presents a series of 11 patients submitted to 20 operations to the correction of skin excess secondary to gastroplasty from December 2004 to December 2006. There were eight female and three male. The average age was 39.6. The mean of weight loss after bariatric surgery was 65 kg. Regarding the operations, there were ten abdominoplasties, eight being anchor and three conventional, with three patients requiring hernioplasty associated. The average of abdominal skin flap and fat was 3.7kg. Still, three mamoplasties were done with silicone prostheses, three brachial lifts, one thoracic lift and two crural lift. Results: Four minor complications have happened after the procedures. Conclusion: The body contour surgery in patients with major weight loss, who underwent gastroplasty, has provided an improvement in life quality, reintegrating them into society, into their jobs and restoring their self esteem.

RESUMO

Introdução: O grande excesso de pele após o emagrecimento dos pacientes submetidos à gastroplastia redutora é desagradável e, às vezes, debilitante. A correção destas seqüelas, freqüentemente, necessita várias intervenções cirúrgicas. Método: Este trabalho apresenta uma série de onze pacientes, totalizando vinte operações para correção de seqüelas cutâneas secundárias à gastroplastia, operados no período de dezembro de 2004 a dezembro de 2006. Foram oito mulheres e três homens, com média de idade de 39,6 anos e média de perda de peso após gastroplastia de 65 kg. Com relação às operações, foram dez abdominoplastias, sendo oito em "âncora" e três clássicas, com três pacientes necessitando herniorrafia incisional associada. A média do peso do tecido abdominal retirado foi de 3,7 kg. Foram ainda realizadas três mastopexias com prótese, três lifting de braços, um lifting de tórax e dois lifting de coxas. Resultados: Ocorreram quatro pequenas complicações após os procedimentos. Conclusão: A cirurgia do contorno corporal nos pacientes com grande perda de peso que realizaram gastroplastia redutora permitiu melhora da qualidade de vida, reintegrando-os ao convívio social e laboral e restabelecendo sua auto-estima.

Construction of an in vitro equivalent skin

ABSTRACT

Background: Human skin is a tegument composed by two different layers: a dermis and epidermis. In situations like trauma and burns, partial or complete loss of this tegument may occur. In 1979, Bell et al. have demonstrated that human fibroblast cultivated over a type I mouse collagen lattice could cause contract that lattice. Methods: In this study, we construct an artificial skin equivalent composite using keratinocytes, cultured over a collagen lattice populated by fibroblasts. This cocultured system was investigated for 3 weeks, and saw in the light microscopic level in the days 4, 7, 14 and 21 days. Results: This study showed the stratification and differentiation of the epithelium in contact with the air is much better than without. Maybe, this system can be use like a skin-substitute.

RESUMO

Introdução: Dentre as diversas pesquisas realizadas na área de bioengenharia de tecidos, a criação in vitro de tecidos vivos tornase uma das mais promissoras formas de atuação científica. Neste caminho, cada vez mais têm sido descritos novos substitutos cutâneos para o tratamento de grandes lesões da pele. Bell et al. descreveram um modelo de matriz colágena tipo I contrátil após a adição de fibroblastos. Método: O presente estudo propõe a elaboração in vitro de um equivalente dermo-epidérmico composto por uma "derme" de colágeno povoada por fibroblastos, acompanhado de um epitélio de queratinócitos cultivados em sua superfície anterior no papel de "epiderme". Além disso, estuda as alterações histológicas encontradas nos dias de cultura 4, 7, 14 e 21, avaliando a influência da interface ar-líquido na estratificação do epitélio. Resultados: Os resultados mostraram que houve a criação de um substituto dermo-epidérmico in vitro, com a formação de um epitélio com boa estratificação, sendo que o contato com o ar aumenta esta estratificação.

Abdominoplasty after bariatric surgery: laboratorial aspect

ABSTRACT

Background: The increase in frequency of reductive gastroplasty surgery and the increasing aesthetics demand by society, made plastic surgery an important stage in morbid obesity treatment. Beside cutaneous problems and comorbities, these patients present metabolic and nutritional disturbances, that could interfere with trans and postoperative treatment. Methods: Forty-one patients who were submitted to abdominoplasty after reductive gastroplasty were studied by the Fobi and Capella technique, during the period of January of 2004 to July of 2006, with attention to pre and postoperative laboratorial aspects and pre, trans and postoperative care.

RESUMO

Introdução: O aumento da freqüência da gastroplastia redutora e a crescente exigência estética da sociedade tornaram a cirurgia plástica uma importante etapa no tratamento da obesidade mórbida. Estes pacientes apresentam, além das alterações cutâneas e comorbidades, distúrbios metabólicos e nutricionais que podem interferir no per e pós-operatório. Método: Foram estudados 41 pacientes submetidos à abdominoplastia pós-gastroplastia redutora pela técnica de Fobi e Capella, no período de janeiro de 2004 a julho de 2006, sendo avaliado o perfil laboratorial pré e pós-operatório e cuidados pré, per e pós-operatórios.

Immediate volumetric breast reconstruction with permanent expanders

ABSTRACT

The utilization of permanent expanders in immediate breast reconstructions allows volumetric repair in a single bow, with a well shaped reconstructed breast, no further scars and little morbidity. It's another option in the surgical arsenal for total breast reconstruction, replacing the two steps surgery with conventional expanders and late exchange for gel prosthesis, with satisfactory aesthetic results. Methods: The authors describe their experience after eighteen procedures utilizing permanent expanders, with a variable follow-up time, highlighting the sampling characteristics, technical details, results and complications.

RESUMO

Introdução: O uso de expansores permanentes em reconstruções mamárias imediatas possibilita a reparação volumétrica em um único estágio, com bom formato da mama reconstruída, ausência de cicatrizes adicionais e pequena morbidade. Trata-se de mais uma opção no arsenal cirúrgico para a reconstrução total da mama, substituindo a cirurgia em dois tempos, quando utilizados expansores convencionais com posterior troca por próteses de silicone gel, com resultado estético satisfatório. Método: Os autores descrevem sua experiência após 18 reconstruções imediatas de mama utilizando expansores permanentes, com período de acompanhamento variável, chamando atenção para as características da amostra, detalhes técnicos do procedimento, resultados obtidos e complicações ocorridas.

Ischemia and reperfusion of tissues

ABSTRACT

The viability of tissues after a surgical aggression is a great challenge for surgeons. The ischemia leads to the blockage of blood flow in peripherical tissues, which is known as noreflow phenomenon. This vascular event is progressive, leading to formation of thrombus in microcirculation, platelet aggregation and cell and tissue swelling. The progression of this phenomenon leads to irreversibility of the injuries and necrosis of tissue. The present paper reviewed the main factors responsible for this phenomenon, its physiopathology and therapeutic possibilities.

RESUMO

A viabilidade dos tecidos após uma agressão cirúrgica é um grande desafio para os cirurgiões. A isquemia leva à obstrução do fluxo sanguíneo para os tecidos periféricos, o que é conhecido pelo fenômeno de não reperfusão. Esta obstrução tem caráter progressivo, com a formação de trombos na microcirculação, agregação plaquetária, edema celular e tecidual. A progressão deste fenômeno leva à irreversibilidade das lesões e morte do tecido. O presente trabalho faz uma revisão dos principais fatores responsáveis por este fenômeno, sua fisiopatologia e possibilidades de tratamento.

Torsion of the cecum in the early abdominoplasty postoperative

ABSTRACT

Background: Abdominoplasty was first referred to in 1880. Its refinement and the realization of the growing numbers of this surgery, also brought on complications. Case report: The authors report the case of a 38 years old female patient who underwent abdominoplasty due to a flaccid and globose abdomen. The surgery proceeded without complications. The patient returned on the 21st postoperative day, complaining of cessation of elimination of gases and feces, vomiting and abdominal distention. The laboratory examinations confirmed leucocytosis and the abdominal X-ray displayed complete obstruction of the small bowel. The patient was submitted to an exploratory laparotomy through the same abdominoplasty incision, which showed a marked distension of the small bowel, ready to rupture due to torsion of the cecum. The torsion was undone and the cecum fixed in the right parietocolic goteira with nonabsorbable sutures, followed by closure of the abdominal wall. The patient's progress was good, and she was discharged on the second postoperative day. Conclusion: Based on the case and a review of the literature, the authors conclude that: 1. Early walking is very important for preventing postoperative complications; 2. The surgeon should not reject others complications not restricted to the abdominoplasty, and act promptly; 3. The classical abdominoplasty incision is adequate for an abdominal approach and should be chosen as the initial option for an eventual complication.

RESUMO

Introdução: A primeira referência sobre abdominoplastia foi feita em 1880; com o aprimoramento e a realização de um número cada vez maior deste tipo de operação, vieram também as complicações. Relato do caso: Paciente do sexo feminino, 38 anos, submetida a abdominoplastia devido a abdome flácido e globoso. A cirurgia transcorreu sem intercorrências. No 21º pós-operatório, a paciente retornou referindo parada de eliminação de gases e fezes, vômitos e distensão abdominal. Aos exames laboratoriais, evidenciou-se leucocitose, e à radiografia de abdome, obstrução intestinal completa de intestino delgado. Foi realizada laparotomia exploradora pela mesma incisão da abdominoplastia, que revelou distensão acentuada do intestino delgado em iminência de ruptura, causada por uma torção do ceco. Foi desfeita a torção e fixado o ceco, na goteira parietocólica direita, com pontos de fio inabsorvível, com posterior fechamento da parede abdominal. A paciente apresentou boa evolução, tendo recebido alta hospitalar no segundo dia de pós-operatório. Conclusão: Diante do caso e da revisão da literatura, os autores concluíram: 1. A deambulação precoce é muito importante para prevenção de complicações no pós-operatório; 2. O cirurgião não deve descartar outras complicações independentes da abdominoplastia, como a descrita, e intervir rapidamente; 3. A incisão clássica da abdominoplastia é suficiente para a abordagem do abdome e deve ser escolhida como opção inicial na eventualidade de uma complicação.

Bilateral carpal tunnel syndrome secondary to gouty: case report

ABSTRACT

Introduction: Despite being a rare condition, the tophaceous gout is recognized as one possible cause of carpal tunnel syndrome in male subjects. Objective: Report the evolution and treatment of a bilateral carpal tunnel syndrome, associated with tenosynovitis and gout. Case report: Male patient with gout, a significant bilateral compression of the median nerve in the carpal tunnel, and tenosynovitis of the left middle and right index fingers. The patient was submitted to a carpal release, tenosynovectomy and primary reconstruction with tendon graft, and evolved with recovery of hand function, relief of gout and nerve symptoms. Conclusion: The authors recommend early diagnosis of the described combinations and clinical treatment as first choice. Surgery intervention is prescribed for cases with complications.

RESUMO

Introdução: O tofo gotoso é uma rara, porém reconhecida, causa de síndrome de compressão do túnel do carpo no sexo masculino. Objetivo: Descrever a evolução e o tratamento de um caso de disfunção das mãos devido à associação de gota, tenossinovite dos flexores e síndrome do túnel do carpo bilateral. Relato do caso: Paciente do sexo masculino, portador de gota tofácea. Apresentava grave compressão do nervo mediano no canal do carpo bilateral e associação de tenossinovite estenosante do terceiro quirodáctilo à esquerda e do segundo à direita. Realizou-se a retinaculotomia dos flexores, tenossinovectomia e a reconstrução tendínea com enxerto de tendão em tempo único. O paciente evoluiu com a recuperação da função das mãos, controle dos sintomas de gota e da compressão do nervo mediano. Conclusão: Os autores recomendam o diagnóstico precoce desta associação e o tratamento clínico como primeira escolha, indicando-se a cirurgia para as complicações.

Body dysmorphophobia or Quasimodo Complex

ABSTRACT

Objective: To present a psychiatric pathology (dysmorphophobia), an underrecognized disorder when out of psychiatric field, quite linked to plastic surgery, to which the author gives the name of Quasimodo Complex (QC). Some relevant aspects are presented to allow plastic surgeons to recognize and to have right behavior with these patients. Methods: Using biothanatology knowledges and the review of currently available literature, an up-to-date definition for this pathology and diagnostic methods are searched so they can be well used by plastic surgeons. Appropriate behaviors are pointed so plastic surgeons can use it to help these patients with QC, avoiding upsetting them. Results: The QC receives this name because of the Hunchback of Notre Dame, character of Victor Hugo whose image caused repulsion to any one who looked at him. Being of difficult recognition and with an incidence around 15% among patients presenting for plastic surgery, it may cause medical and legal problems. When surgically treated, despite the result it has around 91% of insatisfaction. So, these patients must not have surgical procedures, without previous psychiatric support. Conclusions: Plastic surgeons must know quite well this pathology and the ways to identify it. If there is any suspicion of QC, patient must be first referred to a psychiatrist, never for surgical procedure, even if there are some evident physical deformities.

RESUMO

Objetivo: Demonstrar uma doença psiquiátrica (dismorfofobia) pouco conhecida fora do campo da psiquiatria, à qual o autor dá o nome de Complexo de Quasímodo (CQ) e que está muito ligada à cirurgia plástica. Demonstrar alguns de seus aspectos mais importantes, para que o cirurgião plástico possa reconhecê-lo e tomar atitude correta frente a um paciente portador desta doença. Método: Utilizando conhecimentos da biotanatologia e pela revisão da bibliografia mais recente, busca-se uma definição atualizada desta doença, assim como métodos propedêuticos que possam ser utilizados por cirurgiões plásticos, para o seu reconhecimento. Estabelece condutas para que esses especialistas possam lidar com pacientes portadores de CQ, sem causar-lhes danos, ajudando-os de maneira apropriada. Resultados: O CQ recebe este nome tendo em vista o personagem de Victor Hugo, conhecido como Corcunda de Notre Dame, cujo aspecto causava repugnância em quantos o viam. Sendo de difícil reconhecimento e tendo uma incidência que pode chegar a 15% dos pacientes que procuram a cirurgia plástica, é causador de graves problemas médicos e jurídicos para o especialista. Quando operados, independentemente do resultado, apresentam taxas de insatisfação que chegam a 91%, o que contra-indica seu tratamento cirúrgico isolado. Conclusões: Os cirurgiões plásticos necessitam conhecer bem esta doença e os métodos para diagnosticá-la. Na suspeição de CQ, devem encaminhar o paciente para consulta e possível tratamento psiquiátrico, nunca indicando uma cirurgia para eles, sem esta avaliação, mesmo existindo defeitos físicos evidentes.
