Strategies for chest reconstruction following extensive resection of locally advanced breast tumors: an 11-case series

ABSTRACT

INTRODUCTION: Breast cancer is a major cause of mortality among women in Brazil. Locally advanced breast tumors are classified as stage III because they compromise the breast tissue in all or almost all of its extensions and regional lymph nodes, without distant metastases. Surgical treatment of these advanced tumors includes modified radical mastectomy, which involves axillary dissection and large skin resection, leading to major defects in the chest cavity that require complex reconstruction. METHOD: A retrospective clinical study including a series of 11 cases diagnosed with locally advanced breast tumors was performed from January 2006 to March 2014. RESULTS: The chest cavity reconstruction methods performed after resection of advanced breast tumors included three fasciocutaneous flaps, two skin grafts, and six reconstructions with muscle flaps. These procedures resulted in effective coverage of extensive chest defects. CONCLUSION: Aggressive surgical treatment is feasible in these tumors; although numerous reconstruction options are available, individual case assessment is necessary to determine the optimal technique to reduce surgical trauma and lower morbidity in order to avoid delays in adjuvant treatment in these patients. The reconstructions were safe, with satisfactory results similar to those reported in the literature.

RESUMO

INTRODUÇÃO: O câncer de mama representa um dos maiores responsáveis pela mortalidade feminina no Brasil. Os tumores de mama localmente avançados são classificados como estádio III, pois comprometem a mama em toda ou quase toda sua extensão e/ou linfonodos regionais sem apresentar metástase à distância. O tratamento cirúrgico desses tumores avançados consiste na realização de mastectomia radical modificada, que inclui esvaziamento axilar e grandes ressecções cutâneas, levando a grandes defeitos na caixa torácica, necessitando de complexas reconstruções. MÉTODO: Foi realizado um estudo clínico retrospectivo, no período de janeiro de 2006 a março de 2014, de uma série de 11 casos diagnosticados com tumores de mama localmente avançados. RESULTADOS: Foram realizados 3 retalhos fasciocutâneos, 2 enxertos de pele e 6 reconstruções com retalhos musculares para a reconstrução da caixa torácica após a resseção de tumores avançados da mama. Observouse uma cobertura eficaz dos extensos defeitos em tórax. CONCLUSÃO: O tratamento cirúrgico agressivo é viável nestes tumores; diferentes técnicas para a reconstrução do tórax são possíveis; cada caso deve ser avaliado individualmente, devendo ser realizado o reparo que possibilite um menor trauma cirúrgico e menor morbidade, de forma a não retardar o tratamento adjuvante nestes pacientes. As reconstruções apresentadas foram consideradas seguras, os resultados obtidos foram satisfatórios e de acordo com a literatura.

Anatomical representation of the latissimus dorsi and its application in plastic surgery

ABSTRACT

INTRODUCTION: The latissimus dorsi muscle (LDM) is an important element in the lateroposterior wall of the thorax. The LDM is valued in plastic surgery for its constant anatomy, and its use is recommended for mammary reconstruction, microsurgery, and upper limb reconstruction among other procedures, which makes it a versatile flap for reconstructive surgery. However, its representation in books and anatomical studies is controversial. Mathes & Nahai described that the position of the LDM is configured in a location where the muscle is found in the lateroposterior thoracic segment but with a more posterior than lateral representation. A great number of anatomists share the same opinion. The objective of this study was to discuss and describe the real position of the LDM through study of its anatomy and comparison with images published in books. METHODS: We studied the anatomical description and representation of the LDM in drawings published in books and articles by classical authors. The drawings were compared with the neurovascular and muscular anatomy of the LDM during its dissection from 47 fresh cadavers in the dorsal decubitus (DD) position. RESULTS: Study of the anatomical description and representation of the LDM in the revised books compared with the dissection of fresh corpses in the DD position revealed that the published authors described the LDM in a more posterior position than that found in the body. CONCLUSION: This study identified the LDM position as being more anterior than that described by drawings in published books. LDM rotation and individualization with the patient in a DD position is a safe approach.

RESUMO

INTRODUÇÃO: O músculo latíssimo do dorso (MLD) é um importante elemento da parede látero-posterior do tórax. A aplicabilidade do MLD em cirurgia plástica é reconhecida por sua anatomia constante, sendo indicado, na reconstrução mamária, na microcirurgia, nas reconstruções de membros superiores, entre outros, tornando-se um retalho versátil em cirurgia reconstrutora. Sua representação em livros e estudos anatômicos apresentam controvérsias. Mathes & Nahai expressam que a posição do MLD é configurada em uma localização onde o músculo encontra-se em segmento torácico látero-posterior, mas com uma representação mais posterior que lateral. Um grande número de anatomistas escreve da mesma forma. O objetivo é o de discutir e descrever a real posição do MLD por meio do estudo da sua anatomia e comparação com imagens publicadas em livros. MÉTODOS: Estudou-se a descrição e representação anatômica do MLD em desenhos publicados em livros e artigos de autores clássicos. Os desenhos foram comparados com o estudo da anatomia vásculo-nervosa e muscular do MLD durante a dissecção, na posição de Decúbito Dorsal (DD), de 47 cadáveres frescos. RESULTADOS: O estudo da descrição e representação anatômica do MLD em livros de autores consultados, comparada com a dissecção em DD de cadáveres frescos, permitiu conhecer que estes autores identificam o MLD em uma posição mais posterior do que a encontrada na anatomia. CONCLUSÃO: Este estudo identifica a posição do MLD com uma representação mais anterior do que os desenhos dos livros estudados. A rotação e individualização do MLD com o paciente em decúbito dorsal é uma abordagem segura.

Breast reconstruction with autologous tissue: 380 consecutive cases

ABSTRACT

INTRODUCTION: Breast reconstruction plays an important role in the treatment of breast cancer. Several options are available for autologous breast reconstruction, the more widespread being the transverse rectus abdominis myocutaneous (TRAM) flap, the latissimus dorsi myocutaneous (LDM) flap, and the local muscle (LM) flap. The objective of this work was to demonstrate the initial experience in breast reconstruction with autologous tissue, with or without implants. METHOD: A retrospective analysis was performed of medical charts of 367 patients who underwent immediate and delayed breast reconstruction with the unipediculated TRAM flap, LD flap, or LM flap. RESULTS: Three hundred eighty breasts were reconstructed. There were 156 TRAM flap procedures, 179 LD flap procedures, and 49 other techniques. The size of the implants ranged between 155 cc and 640 cc. The mean age of the patients was 49.33 years. One hundred ninety-seven patients underwent surgery on the right side and 169 on the left; 14 patients underwent bilateral reconstruction. Reconstruction was immediate in 80% of the patients. There were few moderate (partial dehiscence of the wound requiring suturing) and severe complications (flap liponecrosis, extrusion of the implant after infection, and pulmonary thromboembolism) and some minor complications that did not require surgical correction. CONCLUSIONS: Breast reconstruction with autologous tissue provides the plastic surgeon with a consistent and reliable method of breast reconstruction, with very satisfactory aesthetic results and low morbidity in selected patients.

RESUMO

INTRODUÇÃO: A reconstrução mamária desempenha papel importante no tratamento do câncer de mama. Várias opções estão disponíveis para a reconstrução autóloga, sendo as mais difundidas o retalho do reto abdominal (TRAM), o retalho de grande dorsal e retalho muscular local. O trabalho visa demonstrar a experiência inicial na reconstrução mamária com tecido autólogo, acrescido ou não de implante. MÉTODO: Foi realizada análise retrospectiva de prontuários de 367 pacientes que foram submetidas à reconstrução mamária imediata e tardia com retalho do reto abdominal (TRAM) unipediculado ou retalho de grande dorsal (GD) ou retalho muscular local (RL). RESULTADOS: Foram reconstruídas 380 mamas. Em 156 pacientes, a técnica foi TRAM; em 179, GD; e, em 49 pacientes, outras técnicas foram empregadas. O tamanho dos implantes variou entre 155 cc e 640 cc. A idade média das pacientes foi de 49,33 anos. 197 pacientes foram operadas do lado direito, 169 do lado esquerdo e em 14 pacientes a reconstrução foi bilateral. A reconstrução foi imediata em 80% das pacientes. Houve poucas complicações moderadas (deiscência parcial da sutura, com necessidade de ressutura) e graves (liponecrose do retalho, extrusão do implante após infecção e tromboembolismo pulmonar) e, ainda, algumas complicações menores que não demandaram correção cirúrgica. CONCLUSÕES: A reconstrução mamária com tecido autólogo fornece ao cirurgião plástico um método consistente e confiável de reconstrução mamária, com resultados estéticos muito satisfatórios e com morbidade pequena para pacientes selecionadas.

Microsurgical reconstructions in pediatric patients: a series of 12 cases

ABSTRACT

INTRODUCTION: Reconstructive plastic surgery is used to treat defects of different sizes and locations. The advent of microsurgery has enabled the single-stage reconstruction of complex and large defects with lower morbidity rates. Despite being one of the first microsurgery applications, large pediatric microsurgical reconstruction series have been published only recently. METHOD: Between January 2007 and December 2014, 12 consecutive patients underwent microsurgical reconstruction upon the removal of head and neck tumors or complex trauma in the limbs of pediatric patients. The medical records were retrospectively reviewed for injury location and etiology, flap type, and complications. RESULTS: A total of 13 flaps were created for microsurgical reconstruction, including reconstruction upon head and neck tumor removal in five, limb complex post-trauma reconstruction in seven, and a microsurgical revascularization in the arm of one child. CONCLUSION: Microsurgical reconstruction in children is peculiar. Our initial experience and the studies published in the literature to date show that in this particular group of patients, it is possible to achieve rewarding results that render this type of reconstruction feasible and highly successful in this age range.

RESUMO

INTRODUÇÃO: A cirurgia plástica reparadora depara-se com defeitos dos mais variados portes e localizações. O advento da microcirurgia possibilitou a reconstrução de defeitos complexos e extensos em um único estágio e com menor morbidade. Apesar de ter sido uma das primeiras aplicações da microcirurgia, a reconstrução pediátrica microcirúrgica só teve grandes series publicadas recentemente. MÉTODO: No período de janeiro de 2007 a dezembro de 2014 foram operados 12 pacientes consecutivos de reconstruções microcirúrgicas pós-ressecção de neoplasias em cabeça e pescoço e traumas complexos em membros de pacientes pediátricos. Os prontuários foram revisados retrospectivamente quanto à localização e etiologia das lesões, tipos de retalhos e complicações. RESULTADOS: Foram realizados 13 retalhos para a reconstrução microcirúrgica em 11 pacientes e uma revascularização microcirúrgica do braço, sendo cinco (41,7%) casos reconstruções pósressecção de neoplasias de cabeça e pescoço e sete (58,3%) casos pós-trauma complexos de extremidades. A taxa de viabilidade no nosso estudo foi de 84,6% dos retalhos realizados. CONCLUSÃO: A reconstrução microcirúrgica em crianças tem particularidades. Nossa experiência inicial e a literatura mostram resultados gratificantes neste grupo especifico de pacientes, o que torna este tipo de reconstrução nessa faixa etária factível e com uma alta taxa de sucesso.

Burn injuries: self-inflicted patients

ABSTRACT

INTRODUCTION: Self-inflicted burn injuries lead to physical, psychological, and economic consequences not only to the victims but also to their families and the society. The prevalence of this type of accident varies greatly throughout the existing literatures. This study aimed to analyze the epidemiological profile, behavior, and lethality rate in patients with self-inflicted burn injuries who received treatment in a burn unit, as a way to contribute to the effective intervention to minimize the effects of risk factors and behaviors associated with this type of accident. METHOD: We conducted a retrospective study in which we analyzed medical records of patients hospitalized in the burn unit of Asa Norte's Regional Hospital, DF, Brazil, during the period between January 2008 and December 2012. RESULTS: The study sample consisted of 88 patients with self-inflicted burn injuries, of whom 54.5% were female, with a mean age of 33 years and mean burned body surface area of 36%. The mean hospitalization time was 23 days. Open flame was the etiological agent in 97.7% of the burn cases, and alcohol was the accelerating agent in 68.2% of these cases. The lethality rate was 32.9%. Sixty patients had comorbidities, with psychiatric disorders and alcoholism being the most common. CONCLUSION: The patients with self-inflicted burn injuries were generally women, with a mean age of 33 years, who used alcohol as accelerating agent. The mean body surface area affected was 36%. Of the patients, 32.9% had associated psychiatric disorders. The lethality rate was 32.9%. From a social perspective, public measures should be established to detect potential patients in order to administer appropriate therapies.

RESUMO

INTRODUÇÃO: As queimaduras autoinfligidas geram consequências físicas, psicológicas e financeiras não só para a vítima, mas também para familiares e para sociedade. A prevalência desse tipo de acidente tem ampla variedade na literatura. O trabalho tem como objetivo analisar o perfil epidemiológico, a conduta e a taxa de letalidade dos pacientes com queimaduras por autoagressão atendidos em centro de tratamento de queimados, como uma forma de contribuir para intervenção efetiva em fatores e comportamentos de risco para tais acidentes. MÉTODO: Estudo retrospectivo por meio da análise dos prontuários de pacientes internados na Unidade de Queimados do Hospital Regional da Asa Norte, Brasília - DF, no período de janeiro de 2008 a dezembro de 2012. RESULTADOS: Foram incluídos no estudo 88 pacientes autoinfligidos, 54,5% do gênero feminino, idade média de 33 anos, média da superfície corporal queimada de 36%. A média de tempo de internação foi de 23 dias. A chama aberta foi o agente etiológico das queimaduras em 97,7% dos casos e o álcool foi o agente acelerador em 68,2%. A taxa de letalidade foi de 32,9%. Sessenta pacientes apresentavam comorbidades, sendo os distúrbios psiquiátricos e o etilismo os principais. CONCLUSÃO: As vítimas de queimaduras por autoagressão são geralmente mulheres, com idade média de 33 anos, utilizando álcool como agente da queimadura, afetando em média 36% da superfície corporal, com distúrbios psiquiátricos associados em 32,9% e com taxa de letalidade de 32,9%. No âmbito social, deve-se pensar em medidas públicas que detectem os pacientes em potencial para que seja instituída terapêutica adequada.

Use of a transverse submandibular cervical flap in the repair of defects of the middle third of the face

ABSTRACT

INTRODUCTION: The need for large amounts of skin and subcutaneous tissue makes it complex to repair extensive defects of the middle third of the face. Aiming to reduce morbidity and attain good aesthetic-functional outcomes, Ariyan and McGrath and, subsequently, Behan et al. proposed reconstruction techniques that use transverse and submandibular cervical flap, respectively. Such flaps have fair amounts of tissue and are associated with low morbidity in the donor area. The present authors propose and describe a variant of the Behan flap for use in the reconstruction of large defects of the middle third of the face. METHODS: We conducted a retrospective study of 8 cases of reconstruction with transverse submandibular cervical flaps for defects of the middle third of the face, conducted between June 2011 and December 2013. The following parameters were analyzed: possible results, and early and late complications. RESULTS: Eight patients with a mean age of 73.5 years were included. All of the patients presented aesthetic-functional satisfactory results. Among the early complications, vascular congestion occurred in 3 patients in the first week with spontaneous resolution, and salivary fistula occurred after tumor resection in 1 patient. With regard to late complications, scar retraction was observed, manifested by either by ectropion (2 patients) or labial retraction (1 patient). CONCLUSION: Randomized transverse submandibular cervical flaps with axial pedicles may be considered as another option for reconstruction of defects of the middle third of the face.

RESUMO

INTRODUÇÃO: As reparações de defeitos extensos do terço médio da face, ao necessitarem de grandes quantidades de pele e subcutâneo, tornam-se complexas. Buscando reduzir morbidade e associar bom resultado estético-funcional, Ariyan & McGrath e, posteriormente, Behan et al. propuseram reconstruções a partir de retalhos transversos cervicais e cérvico-submandibulares, respectivamente. Tais retalhos dispõem de boas quantidades de tecidos e baixa morbidade da área doadora. Os autores propõem e descrevem uma variante do retalho de Behan para reconstruções de grandes defeitos no terço médio de face. MÉTODOS: Foi realizado um estudo retrospectivo de 8 casos de reconstrução do terço médio da face pelo retalho cérvico-submandibular transverso, no período de junho de 2011 a dezembro de 2013. Os parâmetros analisados foram: resultados possíveis e complicações precoces e tardias. RESULTADOS: Foram operados 8 pacientes, com média de idade de 73,5 anos. Todos os pacientes apresentaram resultados estético-funcionais satisfatórios. Dentre as complicações precoces, 3 pacientes apresentaram congestão vascular na primeira semana com resolução espontânea e 1 apresentou fistula salivar consequente à ressecção tumoral. Em relação às complicações tardias, a retração cicatricial foi a complicação observada, manifestada por ectrópio (2 pacientes) e retração labial (1 paciente). CONCLUSÃO: O retalho cérvico-submandibular transverso randomizado e com pedículos axiais é mais uma opção para reconstruções de defeitos do terço médio da face.

Use of 3D geometrical models to improve efficacy and safety of Laser-assisted liposuction: a prospective study

ABSTRACT

INTRODUCTION: On the basis of the available information, it is difficult to establish the appropriate dose of laser energy with which to obtain predictable results in laser-assisted liposuction. The purpose of this study is to evaluate the safety and efficacy of a 980-nm diode laser by using geometrical formulas. These formulas allow a precise quantification of tissue volume and, as a result, an estimation of the required laser dose. METHODS: This prospective study was conducted to evaluate 39 consecutive patients who underwent 980-nm diode laser-assisted liposuction (LAL) between November 2011 and November 2013. Four geometric patterns were used to determine the volume of fat before laser application. Ultrasonography was used to determine fat depth. Biopsies were conducted to determine immediate laser tissue reaction relative to laser dose. The aesthetic results and complications were recorded. Pearson's correlation coefficient was used to determine the relationship between the applied energy and tissue volume. RESULTS: 163 anatomic regions were treated with 980-nm LAL. The applied energy was relative to the volume in each area (R = 0.8786). 8 to 14 J/cm3 was the dose needed to produce effective hemostasis and minimal lipolysis. The results were very satisfactory in most cases. Biopsy results revealed lipolysis, ballooning, membrane rupture, and collagen band breakage; these changes varied with the radiation dose. The only complications were edema and transient bruising. CONCLUSION: The use of 3D geometrical models improved safety and efficacy of laser-assisted liposuction by permitting accurate determination of the tissue volume.

RESUMO

INTRODUÇÃO: Baseada na informação disponível encontram-se dificuldades em estabelecer a dose apropriada de energia a laser para alcançar resultados previsíveis na lipoaspiração a laser. Este estudo avaliou a segurança e eficácia do laser diodo 980-nm com uso de fórmulas geométricas. Essas fórmulas permitem uma qualificação precisa do volume do tecido, e como consequência, uma estimativa da dose de laser necessária. MÉTODOS: Tratase de estudo prospectivo que avaliou consecutivamente 39 pacientes submetidos a lipoaspiração a laser diodo (LLD) 980-nm entre Novembro de 2011 e Novembro de 2013. Quatro padrões geométricos foram adotados para determinar o volume de gordura antes da aplicação do laser. A ultrassonografia foi utilizada para determinar a profundidade da gordura. Foram realizadas biópsias para estabelecer a reação imediata do tecido ao laser relativo à dose do laser. Os resultados estéticos e as complicações foram documentadas. Utilizou-se o coeficiente de correlação de Pearson para determinar a relação entre a energia aplicada e o volume do tecido. RESULTADOS: Um total de 163 áreas anatômicas foram tratadas com LLD 980-nm. A energia aplicada foi baseada no volume de cada área (R = 0,8786). Foi necessário dose de 8 a 14 J/cm3 para produzir dose de hemostase efetiva, e lipectomia mínima. Os resultados foram muito satisfatórios na maioria dos casos. A biópsia relevou lipectomia, balonamento, ruptura de membrana, e rompimento de banda de colágeno; essas mudanças variaram com a dose de radiação. As complicações foram somente edema e hematoma transitório. CONCLUSÃO: O uso de modelos geométricos tridimensionais melhoraram a segurança e eficácia da lipoaspiração a laser, pois permitiram determinar o volume do tecido.

Demographic and etiologic profiles of patients hospitalized in a reference center for burn treatment in Mato Grosso do Sul

ABSTRACT

INTRODUCTION: The present study addressed the issue of hospitalization in 13 patients, children and adolescents, specifically focusing on burn patients admitted to a center for burn treatment. The objective of this study was to observe and analyze the perception of those involved in the treatment process in order to understand the relationship of the patient with pain and the treatment process with the quality of life in the hospital environment and with their self-image, and to collect information on the importance of prophylactic treatment on thermal trauma. METHODS: To meet the objectives of the research, we used the case study method with a qualitative approach. RESULTS: The results showed that most of the patients were male. The predominant etiologic agent was ethanol. The accidents caused burns of various severities, first- and second-degree burns in 1 patient, second- and third-degree burns in 7 patients, and third-degree burns in 5 patients. The length of hospitalization ranged from 15 to 75 days. The ages of the patients ranged from 1 year 8 months to 18 years. As to education, with the exception of the youngest patient, all attended the school network. Regarding socioeconomic level, most had a family income of up to 2 minimum wages, which was not sufficient. In general, only one person worked per family. CONCLUSIONS: The results demonstrate the need to provide guidance to parents, with educational programs and campaigns.

RESUMO

INTRODUÇÃO: O presente estudo abordou a questão da hospitalização de 13 pacientes, crianças e adolescentes, enfocando especificamente o paciente queimado internado em um Centro de Tratamento de Queimaduras. O objetivo foi o de observar e analisar a percepção dos envolvidos no processo de tratamento, para compreender a relação desse paciente com a dor, com o processo de tratamento, com a qualidade de vida no ambiente hospitalar e com sua autoimagem. Levantar informações que revelem a importância de um trabalho profilático do trauma térmico. MÉTODO: Para atender os objetivos da pesquisa, foi utilizado o método de investigação de Estudo de Caso com abordagem qualitativa. RESULTADOS: Os resultados apontaram uma maioria dos pacientes do sexo masculino. O agente etiológico predominante foi o etanol. Os acidentes causaram queimaduras de gravidades diversas, sendo 1 de 1º e 2º graus, 7 de 2º e 3º graus e 5 de 3º grau. O tempo de internação variou de 15 a 75 dias. As idades dos pacientes variaram entre 1 ano e oito meses a 18 anos. Quanto à escolarização, com exceção do paciente mais novo, todos frequentavam a rede de ensino. Quanto ao nível socioeconômico, a maioria tem uma renda familiar de até dois salários mínimos, que não são suficientes. De modo geral, encontra-se apenas uma pessoa trabalhando, por família. CONCLUSÕES: Os resultados pesquisados demonstram a necessidade de orientação aos pais, com programas educativos e campanhas de prevenção.

Publication involvement of plastic surgery residents in accredited services: A 10-year comparative analysis

ABSTRACT

INTRODUCTION: Conducting research and publishing articles during residency are not mandatory yet. However, these are shown to be necessary during training. This study aimed to evaluate the participation of resident physicians from services accredited by the Brazilian Society of Plastic Surgery in the publication of articles in the Brazilian Journal of Plastic Surgery by analyzing how they have evolved over a period of 10 years. METHODS: We evaluated articles published in the Brazilian Journal of Plastic Surgery between 2003 and 2012, excluding editorial, letter, message, erratum and acknowledgment articles. We also evaluated data regarding sex, demographic region, type of work, number of papers published with the participation of residents, mean number of residents per publication, subject area, and educational institution. RESULTS: We identified 204 articles, of which 53 were published between 2003 and 2007, and 151 were published between 2008 and 2012. The most common subject was body contour, accounting for 95.1% of descriptive studies. Most of the authors were not affiliated with higher institutions (54.4%), and the residents from the southeast region were most active participants. On comparing different periods, we observed an increase in the number of residents as authors and in the participation of residents. The number of articles increased significantly in all regions of the country, except in the south. CONCLUSION: The participation of residents in the publication of scientific articles that were published in the last 5 years in the southeast, northeast, and midwest regions has increased. The training structure for resident physicians should be improved to increase scientific publication in both, quantitative and qualitative aspects.

RESUMO

INTRODUÇÃO: O desenvolvimento de pesquisas e a publicação de artigos durante o período de residência médica ainda não é obrigatório, porém tem sido evidenciada a sua necessidade durante a formação. Este estudo tem como objetivo fazer uma avaliação com relação à participação de médicos residentes dos serviços reconhecidos pela SBCP na publicação de artigos na Revista Brasileira de Cirurgia Plástica, comparando a evolução destes em um período de 10 anos. MÉTODOS: Foram avaliados artigos publicados na Revista Brasileira de Cirurgia Plástica no período de 2003-2012, excluindo artigos relacionados a editoriais, carta, mensagem, erratas e agradecimentos. Foram avaliados dados como: sexo, região demográfica, tipo de trabalho, número de trabalhos publicados com participação de residentes, média de residentes por publicação, área temática, instituição de ensino. RESULTADOS: Foram identificados 204 artigos, sendo 53 no período de 2003-2007 e 151 artigos de 2008-2012. O tema mais comum foi o contorno corporal, com 95,1% em estudos descritivos. A maioria dos autores não tinha vínculo com instituição superior (54,4%), sendo a região demográfica mais participante a Sudeste. Quando comparados os períodos, houve aumento de residentes como autores, na participação dos residentes e o número de artigos cresceu significativamente em todas as regiões do país, exceto na região Sul. CONCLUSÃO: Houve aumento na participação de residentes da produção de artigos científicos publicados nos últimos 5 anos nas regiões Sudeste, Nordeste e Centro-Oeste. Existe necessidade de aperfeiçoamento na estrutura de formação do médico residente para melhorar a produção científica, seja nos aspectos quantitativos ou qualitativos.

Closed platysmotomy: a 7-year review

ABSTRACT

INTRODUCTION: The approach of the cervical region has remained challenging over the years. The closed platysmotomy technique, proposed by Daher, is a minimally invasive technique that offers a viable alternative to the approach of the cervical region, with low complication rates and lasting results over the years. The objective is to evaluate recurrent cases of platysmal bands over the years, including 7-year results. METHODS: Photographic assessment was performed by a plastic surgeon blinded to the patient characteristics and the number of recurrences 7 years after surgery in all the female patients who underwent operation in our institution. The analysis was performed based on photographic documentation, and the results were classified as excellent, satisfactory, and unsatisfactory. The assessment did not involve patient participation. RESULTS: The recurrence rates 7 years after surgery were similar to those found in traditional techniques. Of the cases, 57% were classified as excellent; 29%, as satisfactory; and 14%, as unsatisfactory. CONCLUSION: The closed platysmotomy technique is a valid alternative, with lasting results and the advantage of being a procedure with comorbidities fewer than those associated with the existing techniques, displaying similar recurrence rates over the years with other more invasive techniques.

RESUMO

INTRODUÇÃO: A abordagem da região cervical permanece desafiadora ao longo dos tempos. A platismotomia fechada, proposta por Daher, é uma técnica minimamente invasiva, que oferece uma alternativa viável para abordagem da região cervical, com pequenas taxas de complicações e resultados duradouros ao longo dos anos. Avaliar as recorrências das bandas platismais ao longo dos anos, avaliado os resultados após 7 anos. MÉTODOS: Avaliação fotográfica por um Cirurgião Plástico isento, que não conhecia as pacientes, quanto ao número de recidivas em 7 anos de cirurgia, em pacientes operados em nosso serviço, todas do sexo feminino. A análise foi feita por meio de documentação fotográfica, sendo os resultados classificados em excelentes, satisfatórios e insatisfatórios. Não houve participação das pacientes na avaliação. RESULTADOS: Os índices de recidiva após 7 anos de cirurgia encontrados são semelhantes aos encontrados em técnicas já consagradas. 57% dos casos considerados excelentes, 29% satisfatórios e 14% insatisfatórios. CONCLUSÃO: A técnica da platismotomia fechada é uma alternativa válida, com resultados duradouros, e com a vantagem de ser um procedimento com menos comorbidades quando comparado com as técnicas existentes, mantendo-se os índices de recidiva ao longo dos anos semelhantes com outras técnicas mais invasivas.

ABSTRACT

INTRODUCTION: Previous studies have shown that the public has misconceptions about the work of plastic surgeons in hand surgery. However, no specific Brazilian data on this issue are available. The objective of this study was to evaluate the public perceptions about the role of plastic surgeons in the field of hand surgery in Brazil. METHODS: Members of the Brazilian public chose one or two specialists whom they believed to be experts in eight scenarios related to hand surgery. The patterns of the responses were distributed into "plastic surgeons" or "non-plastic surgeons." RESULTS: Non-plastic surgeons were significantly (p < 0.05 for all comparisons) more recognized as experts than plastic surgeons in all scenarios related to hand surgery. CONCLUSION: The knowledge and perceptions of the Brazilian public about the work performed by plastic surgeons in the field of hand surgery are limited.

INTRODUÇÃO: Estudos prévios têm revelado que o público tem conhecimentos equivocados sobre a atuação dos cirurgiões plásticos em cirurgia da mão. No entanto, não existem dados específicos na literatura científica brasileira. O objetivo deste estudo foi avaliar as percepções do público sobre o papel dos cirurgiões plásticos no campo de cirurgia da mão no Brasil. MÉTODOS: Membros do público brasileiro escolheram um ou dois especialistas que eles acreditassem serem experts para oito cenários relacionados à cirurgia de mão. Os padrões de respostas foram distribuídos como "cirurgiões plásticos" ou "não cirurgiões plásticos". RESULTADOS: "Não cirurgiões plásticos" foram significativamente (p < 0,05 para todas as comparações) mais reconhecidos como experts que "cirurgiões plásticos" em todos os cenários relacionados à cirurgia de mão. CONCLUSÃO: Os conhecimentos e as percepções do público brasileiro sobre o trabalho realizado por cirurgiões plásticos no campo cirurgia da mão são limitados.

Round versus anatomical breast implants: algorithm for choosing the appropriate form

ABSTRACT

INTRODUCTION: Several surgeons have their own formulas or protocols to select the volume and shape of breast implants. To determine the shape, we measured the distances between the upper edge of the breast and the papilla (A), and between the papilla and the inframammary fold (B). Based on these measurements, we propose an algorithm to select round or anatomical implants. METHODS: Preoperative assessment was performed with the patients in the orthostatic position. The following distances were considered: 1) from the sternal notch to the papilla, to assess the need for supra-areolar skin excision; 2) breast base, to assess the volume of the implant; 3) distances A and B, to evaluate the shape of the implant. This algorithm was applied to 59 patients undergoing augmentation mammoplasty. RESULTS: We used round implants in 27 patients; nine had a distance A = B, and 18 had B > A. We utilized anatomical implants in 32 patients. The volume of round implants ranged from 195 to 425 cc, whereas that of anatomical implants ranged from 185 and 315 cc. Regarding postoperative measurements of the patients who used round implants, 26 (96.3%) maintained the desired ratio with B > A or A = B. Among the patients with anatomical implants, 25 (78.1 %) showed proportional changes from A > B to A = B or B > A. CONCLUSIONS: When the distance A is equal to or smaller than the distance B, we recommend round implants. When B < A, we recommend anatomical implants.

RESUMO

INTRODUÇÃO: Vários cirurgiões têm suas próprias fórmulas ou protocolos para selecionar os volumes e formato de implantes mamários. Para determinar a escolha do formato, medimos as distâncias entre a borda superior da mama e a papila (A) e entre a papila e sulco submamário (B). Baseados nestas medidas, propomos um algoritmo para selecionar próteses redondas ou anatômicas. MÉTODOS: As avaliações pré-operatórias foram realizadas com a paciente em posição ortostática considerando-se as medidas: 1) da fúrcula esternal à papila, para avaliar a necessidade de retirada de pele supra-areolar; 2) da base da mama, para avaliar o volume do implante; 3) das distâncias A e B, para avaliar a forma do implante. Este algoritmo foi aplicado a 59 pacientes submetidas à mamoplastia de aumento. RESULTADOS: Utilizamos implantes redondos em 27 pacientes; nove tinham distância a = b, e 18 B > A. Empregamos implantes anatômicos em 32 pacientes. Os volumes dos implantes redondos variaram entre 195 cc e 425 cc, enquanto os implantes anatômicos ficaram entre 185 cc e 315 cc. Com relação às medidas pós-operatórias das pacientes que utilizaram implantes redondos, 26 (96,3%) mantiveram a proporção desejada com B > A ou A = B. Entre as pacientes com implantes anatômicos, as medidas de 25 delas (78,1%) mostraram alteração das proporções, de A > B para A = B ou B > A. CONCLUSÕES: Quando a distância A é igual ou menor que a distância B, recomendamos implantes redondos. Quando B < A, recomendamos implantes anatômicos.

Use of the myocutaneous latissimus dorsi flap with fat extension in breast reconstruction: an option for filling the upper pole

ABSTRACT

INTRODUCTION: The sequelae of mastectomy presents in numerous ways, requiring the experience and creativity of the surgeon to achieve the best result in the reconstruction of a new breast. One of the difficulties in achieving this objective is the challenge of adequately filling the upper pole of the breast. The objective of this work was to present an alternative therapeutic option for correction of the upper pole of the neobreast. METHODS: In our surgical technique, a latissimus dorsi muscle flap with fat extension is used for filling the upper pole of the neobreast during its reconstruction. The described technique was used in 8 patients during late breast reconstruction. The patients' ages ranged from 39 and 70 years. The size of the fat extension ranged from 4.0 × 10.0 cm to 7.0 × 13.0 cm. The fat component of the flap was evaluated after 3 months by using magnetic resonance imaging (MRI). RESULTS: Breast implants that varied in volume between 270 and 435 mL were used. The follow-up period after surgery ranged from 3 months to 1 year. Two patients had epidermolysis (28%) at the junction of the cutaneous flap and the receiver area. No graft loss or other complications occurred. Our results demonstrate clinically or visually that correction of the depression in the upper pole of the neobreast was satisfactorily achieved. The viability of the fat flap, as observed on MRI, was adequate. CONCLUSION: The proposed technique is a suitable alternative method for filling the depression in the upper pole of the neobreast in most cases of breast reconstruction.

RESUMO

INTRODUÇÃO: As sequelas de mastectomia se apresentam de formas variadas exigindo, do cirurgião, experiência e criatividade para obter o melhor resultado na reconstrução de uma nova mama. Dentre as dificuldades para que este objetivo seja alcançado, está o desafio de um melhor preenchimento do polo superior da mama. O objetivo deste trabalho é apresentar uma opção terapêutica alternativa para correção do polo superior dentre as técnicas já existentes com este propósito. MÉTODO: A técnica cirúrgica utilizou o retalho de músculo grande dorsal com extensão gordurosa para preenchimento do polo superior da neomama durante sua reconstrução. A técnica descrita foi utilizada em 8 pacientes durante a reconstrução mamária tardia, com idades variando entre 39 e 70 anos. O tamanho desta extensão gordurosa variou entre 4,0 × 10,0 e 7,0 × 13,0 cm. O componente gorduroso do retalho foi avaliado após 3 meses através de ressonância magnética. RESULTADOS: Foram usados implantes mamários que variavam entre 270 e 435 ml. O acompanhamento pós operatório variou entre 3 meses a 1 ano. Ocorreram 2 casos de epidermólise (28%) na junção do retalho cutâneo com a área receptora. Não houve perda ou sofrimento do retalho. Os resultados demonstram clinicamente ou visualmente que a correção da depressão do polo superior da neomama foi alcançada adequadamente, bem como a viabilidade do retalho gorduroso, observada nos exames de imagem (ressonância magnética). CONCLUSÃO: A técnica proposta é uma alternativa adequada para o tratamento da maioria dos casos de reconstrução mamária em que se busca o preenchimento da depressão existente no polo superior da mama.

Application of fat grafting in craniofacial deformities: an INTO-RJ experience

ABSTRACT

INTRODUCTION: The fat graft is currently widely used in aesthetic plastic and reconstructive surgery as a natural filler. In cranio-maxillofacial surgery, fat grafting can be very beneficial for patients with various congenital and acquired deformities. METHODS: We included patients who had undergone fat grafting for reconstruction and correction of defects in the face during 2012 and 2013 by using a standard technique. Our assessment was both clinical and subjective, taking into account the patient's opinion. In patients with late enophthalmos secondary to trauma sequelae, we conducted exophthalmometry with an exophthalmometer (Hertel) and preoperative and postoperative CT to objectively evaluate our results. RESULTS: Twenty-two patients were treated. Most were female (77%). The volume of fat grafted varied from 1 ml to 37 ml, with a mean of 15 ml per session. The number of sessions ranged from 1 to 4. Fat grafting was used as a single treatment in only 30% of cases. We performed retrobulbar fat grafting ranging from 6 to 10 ml in volume; in one patient, the grafting was carried out over two sessions. There was a gain of 3 to 7 mm in projection of the eyeball. In tomographic evaluation, an increased eye projection of between 2.2 mm and 4 mm was found. However, the clinical outcome was poor. CONCLUSION: Fat grafting is a simple, inexpensive and reproducible procedure that should be part of the plastic and craniofacial surgeons' inventory. It may be an alternative in difficult cases of late enophthalmos.

RESUMO

INTRODUÇÃO: O enxerto de gordura, atualmente, é usado amplamente na cirurgia plástica estética e reparadora como um preenchimento natural. Na cirurgia crânio-maxilofacial há uma diversidade de pacientes com deformidades congênitas e adquiridas que podem ser muito beneficiados com a lipoenxertia. MÉTODOS: Os pacientes foram submetidos à lipoenxertia para reconstrução e correção de defeitos da face no INTO em 2012 e 2013 utilizando uma técnica padronizada. Nossa avaliação foi clínica e subjetiva, levando em conta a opinião do paciente. Especificamente nos pacientes com enoftalmo tardio secundário à sequela de trauma, realizamos exoftalmometria com o exoftalmômetro de Hertel e tomografia no pré e no pós-operatório para avaliar objetivamente nossos resultados. RESULTADOS: Vinte e dois pacientes receberam tratamento. A maioria foi do gênero feminino (77%). O volume do gordura aplicado variou de 1 ml até 37 ml, com média de 15 ml por sessão. A quantidade de sessões variou de 1 a 4. A lipoenxertia foi usada como tratamento único em apenas 30% dos casos. Fizemos lipoenxertia retrobulbar variando de 6 a 10 ml o volume enxertado, sendo que em um paciente realizamos duas sessões. Houve um ganho de 3 a 7 mm de projeção do globo ocular. Na avaliação tomográfica constatamos também aumento da projeção ocular de 4 mm e 2,2 mm. O resultado clínico foi pobre. CONCLUSÃO: A lipoenxertia é um procedimento simples, barato e reprodutível que deve fazer parte do armamentário do cirurgião plástico e do cirurgião craniofacial. Pode ser uma alternativa nos difíceis casos de enoftalmo tardio.

Surgical correction of prominent ears: association of the Furnas and Mustardé techniques

ABSTRACT

INTRODUCTION: Prominent ears, popularly known as bat ears, are the most common deformity of the head and neck, occurring in approximately 5% of the general population. This study aims to demonstrate that, with the use of simple surgical techniques, it is possible to correct the deformity and achieve optimal patient satisfaction. METHOD: A total of 60 patients of both sexes, aged between 11 and 40 years, underwent bilateral otoplasty between February 2009 and December 2010. RESULTS: In all cases, bilateral otoplasty was performed. There were no cases of hematomas, surgical site infection, or hypertrophic scars. In 5 cases (8.3%) visible scars or foreign body granulomas were found in the posterior surface of the ear. Chondritis occurred in one patient. Bilateral residual deformity occurred in 3 cases at 1 year post-surgery. Total recurrence was observed in 1 case (1.7%). After one year of surgery, 56 patients (93.3%) considered the result as good and were satisfied, 3 patients underwent reintervention for residual deformity, and total recurrence occurred in 1 case, which the patient did not wish to correct. CONCLUSION: The present study demonstrates that concurrent use of the Mustardé and Furnas otoplasty techniques results in a high degree of satisfaction and a low rate of complications. The procedure can easily be performed at the outpatient level and at a low cost.

INTRODUÇÃO: Orelha proeminente, popularmente conhecida como orelha em abano, é uma afecção muito frequente em nosso meio. Constitui a deformidade mais comum da cabeça e pescoço, prevalecendo em aproximadamente 5% da população em geral. O trabalho tem como objetivo demonstrar que, com a junção de técnicas cirúrgicas simples, é possível corrigir as deformidades e obter um ótimo índice de satisfação dos pacientes. MÉTODO: Um total de 60 pacientes de ambos os sexos, entre 11 e 40 anos foram submetidos à otoplastia bilateral entre fevereiro de 2009 e dezembro de 2010. RESULTADOS: Em todos os casos se realizou otoplastia bilateral. Não houve casos de hematomas, infecção de sítio cirúrgico ou cicatriz hipertrófica. Cicatrizes visíveis ou granulomas de corpo estranho na face posterior da orelha foram constatados em 5 casos (8,3%). Houve 1 caso de condrite. O índice de deformidade residual com 1 ano de pós-operatório ocorreu em 3 casos, sendo todos bilaterais. Recidiva total foi presenciada em 1 caso (1,7%). Após um ano da cirurgia, 56 pacientes (93,3%) consideraram o resultado como bom e estavam satisfeitos, 3 pacientes sofreram reintervenção por deformidade residual e em 1 caso ocorreu a recidiva total, em que o mesmo não desejou correção. CONCLUSÃO: O presente estudo vem demonstrar que a associação das técnicas de Mustardè com a de Furnas traz alto grau de satisfação, baixo índice de complicações, podendo ser realizada com tranquilidade em nível ambulatorial e com baixo custo.

ABSTRACT

INTRODUCTION: An important sign of facial aging is the fall of the eyebrow tail. Frontotemporal minilifting aims to reposition the brow and soften the expression marks to restore youthful appearance. To describe an ambulatory surgical technique capable of eyebrow tail lift with minimal scarring and wound camouflage in the capillary line that reduces the facial signs of aging. METHOD: The study included 10 consecutive patients who attended the Plastic Surgery outpatient clinic of the UNIFESP and submitted to minilifting after receiving the diagnosis of eyebrow fall. RESULTS: Preliminary results did not show the occurrence of major complications. All patients were women. One patient presented with small suture line dehiscence 15 days after surgery. A total of 90% of the patients scored their body image index as extremely satisfied or satisfied. The average eyebrow tail lift was 5 mm. The average preoperative eyebrow fall was 13.3 mm, with a mean elevation and maintenance of 18.2 mm in 12 months. CONCLUSION: The technique is easily reproducible, provides outcomes comparable to those of other procedures published in the literature, and presents the following advantages: is low cost; can be performed in an outpatient clinic; provides a rejuvenating periocular effect since it also reduces wrinkles, expression marks, and sagging of the lateral region of the orbital and glabellar fissure; and achieves significant long-lasting results.

RESUMO

INTRODUÇÃO: Um sinal importante do envelhecimento facial é a queda da cauda da sobrancelha. O minilifting frontotemporal visa reposicionar a sobrancelha e amenizar as marcas de expressão, restaurando uma aparência mais jovial. Descrever uma técnica cirúrgica, ambulatorial, capaz de elevar a cauda da sobrancelha, com cicatriz mínima e camuflada na linha capilar, melhorando o aspecto de envelhecimento facial. MÉTODO: Foram arregimentados dez pacientes sequenciais do ambulatório de cirurgia plástica da UNIFESP, que após diagnóstico de queda de supercílio foram submetidos ao minilifting. RESULTADOS: Como resultados preliminares, não houvera nenhuma complicação importante, todos pacientes eram mulheres, um caso apresentara pequena deiscência em linha de sutura após 15 dias de cirurgia. 90% apresentaram índice de imagem corporal em extremamente satisfeitos ou satisfeitos. A média de elevação da cauda do supercílio foi de 5 mm. A média de queda de supercílio pré-operatória foi de 13,3 mm, tendo sua elevação média e manutenção em 12 meses de 18,2 mm. CONCLUSÃO: A técnica sugerida pelos autores é de fácil reprodutibilidade, produz resultados comparáveis às demais técnicas da literatura com as vantagens de apresentar baixo custo, ser realizada ambulatorialmente e ter um efeito rejuvenescedor periocular, pois atenua também as rugas, marcas de expressão e flacidez da região lateral da fissura orbitária e glabelar com significativo tempo de manutenção de resultados.

Mammoplasty with bipedicled flap from the pectoral muscle: 30 years of experience

ABSTRACT

INTRODUCTION: Several breast augmentation techniques have been developed to avoid late ptosis. However, these techniques result in persistent pendulum and changed shape due to loss of filling in the upper pole. The combination of breast augmentation with a thoracic flap involving a bipedicled flap from the greater pectoral muscle has been used with good results that are maintained in late postoperative periods. METHOD: More than 4,000 procedures were performed using the author's technique. The marking of traditional mammoplasty and construction of the inferior pedicle flap were performed as described by Ribeiro. A pectoral muscle strap was divulsed and the thoracic flap passed through and attached to the chest wall. The muscle and glandular flaps were wrapped by the upper pole tissue and sutured to the lateral breast pillars. RESULTS: Use of the thoracic flap technique fixed by a pectoral muscle strap results in minimum pendulum, which provides better long-term aesthetic results. With this technique, the breast tissue is divided and repositioned in the desired location while maintaining the breast shape rather than depending on the dermal closure to provide the final shape. CONCLUSION: This technique provides long-term preservation of the upper breast pole and reduced scar tension with consequent aspect improvement; it also eliminates post-operative pendulum movement (ptosis) without significantly increasing surgical time.

RESUMO

INTRODUÇÃO: Existem inúmeras técnicas de mamoplastia com objetivo de evitar a ptose tardia. Observamos em todas estas técnicas que a forma da mama após a sua báscula permaneceu com perda de preenchimento do seu polo superior. A associação da mamoplastia com o uso do retalho de base torácica associado ao retalho bipediculado de músculo peitoral maior tem sido utilizada devido ao bom resultado que é mantido num pós-operatório tardio. MÉTODO: Foram feitos mais de 4000 procedimentos utilizando a técnica do autor. Foi realizada a marcação de mamoplastia tradicional e confecção de retalho de pedículo inferior conforme descrito por Ribeiro. Divulsiona-se uma faixa do músculo peitoral e realiza-se a passagem completa do retalho torácico através da alça muscular com posterior fixação do retalho à parede torácica. Os retalhos muscular e glandular são envolvidos pelo tecido do polo superior fazendo se a sutura dos pilares laterais. RESULTADOS: Com a utilização da técnica de um retalho torácico, fixado por uma alça de músculo peitoral, obtivemos uma báscula mínima que forneceu um melhor resultado estético a longo prazo. Com esta técnica, o tecido mamário é dividido e reposicionado para o local desejado, mantendo a forma da mama e não dependendo do fechamento dérmico para se obter o contorno final. CONCLUSÃO: Observamos nos casos operados a manutenção do polo superior da mama mesmo tardiamente, redução da tensão sobre a cicatrização com consequente melhora do aspecto, ausência do movimento de báscula (ptose) pós-operatória, sem aumento significativo no tempo cirúrgico.

Risk factors of burn injury and prevention methods in the elderly

ABSTRACT

INTRODUCTION: As the population ages, the elderly become more susceptible to burn injuries. Studies that assess the epidemiological data and risk factors of burn injuries in the elderly are valuable tools to evaluate the strategies developed with the aim of preventing these injuries. OBJECTIVE: We aimed to describe the risk factors of burn injury and prevention methods in the elderly. In addition, we aimed to determine the correlation between the risk factors and the epidemiological data on the incidence and mortality from studies published in the literature worldwide. METHODS: In the literature review, we consulted 21 articles published between January 2000 and December 2012 in 4 continents from the following databases: Lilacs, Medline, PubMed, Cochrane, and ScienceDirect. RESULTS: We selected 21 articles from 4 continents. The research period ranged from 2 to 28 years. The number of elderly patients ranged from 45 to 308. Their mean age ranged from 64.5 to 85 years. The predominant accident site was the place of residence (68% to 98%), with the kitchen being the most prevalent location (17% to 65%), followed by the bathroom (9% to 31%). The main causes were direct fire or flammable liquids (29% to 73%) and scalding (17% to 66%). Injury by inhalation was found in 12% to 41% of the elderly, with mortality ranging from 6% to 65%. CONCLUSION: We conclude that the place of residence is the most important and prevalent location of burn injuries in the elderly. Therefore, preventive measures against burn injuries in this type of environment should be intensified.

INTRODUÇÃO: Com o envelhecimento populacional, os idosos estão sujeitos a aumento dos traumas por queimadura. Estudos que realizam mensuração de dados epidemiológicos e fatores de risco de queimadura em idosos são um recurso importante para avaliar resultados de tratamento e desenvolvimento de estratégias que visem à prevenção. OBJETIVO: Descrever os fatores de risco e os métodos de prevenção de queimadura em idosos, correlacionando-os aos dados epidemiológicos de incidência e mortalidade em estudos publicados na literatura mundial. MÉTODO: Na revisão da literatura realizada, foram consultados 21 artigos de 4 continentes nas bases de dados: Lilacs, Medline, PubMed, Cochrane e ScienceDirect, de janeiro de 2000 a dezembro de 2012. RESULTADOS: Foram selecionados 21 artigos de 4 continentes. O período variou de 2 anos a 28 anos. O total de pacientes idosos variou de 45 a 308. A idade média foi de 64,5 a 85 anos. O gênero foi descrito em 18 trabalhos, sendo 1132 homens e 1076 mulheres. O local predominante do acidente foi a residência (68 a 98%), com a cozinha sendo a localidade prevalente (17 a 65%), seguindo o banheiro em 9 a 31%. Os agentes principais foram o fogo direto ou líquidos inflamáveis (29 a 73%) e a escaldadura (17 a 66%). Lesão por inalação foi encontrada em 12 a 41% dos idosos e a mortalidade variou de 6 a 65%. CONCLUSÃO: Conclui-se que a residência é o local de maior importância e prevalência da queimadura em idosos e devem ser intensificadas as medidas de prevenção deste acidente neste ambiente.

Late seroma in breast reconstructions and mammoplasty with silicone implants: a case report and literature review

ABSTRACT

The occurrence of seroma as a late complication of silicone breast implant is of great interest, given the aesthetic implications such as asymmetry and the possible association with infections or even malignancies. This complication is believed to be exclusive of textured prostheses. The present authors reviewed the literature by conducting a search of data in publications available in Medline by using the search term "late breast seroma" in order to clarify the pathological features of seroma. The etiology is unknown in most cases. The definitive treatment of choice is surgery, and most authors recommend bacteriological and cytological evaluations for seroma, preferably guided by ultrasonography. To provide patients with the best treatment, the treatment should be individualized according to clinical presentation, anticipating the possibility of recurrence and final sequelae.

RESUMO

A ocorrência de seroma como complicação tardia por implante de silicone mamário é de grande interesse, dadas as implicações estéticas, como assimetria, e a possível associação com infecções ou até mesmo malignidades. Acredita-se que esta complicação seja exclusiva de próteses texturizadas. Os autores fazem síntese da literatura a partir de pesquisa de dados em publicações disponíveis em MEDLINE com o termo "late breast seroma" em busca de maior esclarecimento da patologia. A etiologia é desconhecida na maioria dos casos. Nota-se que o tratamento definitivo de escolha é cirúrgico, sendo que grande parte dos autores recomenda a avaliação bacteriológica e citológica do seroma, preferencialmente guiado por ultrassonografia. O tratamento deve ser individualizado, de acordo com a clínica apresentada, antecipando a possibilidade de recorrência do evento e a sequela final, oferecendo, assim, o melhor tratamento à paciente.

ABSTRACT

INTRODUCTION: The anterolateral thigh (ALT) flap has become an important tool in the microsurgical reconstruction of cutaneous and subcutaneous defects. Since the ALT flap can be up to 35 cm long, it can be used in both, esophageal reconstruction and cervical skin defects. CASE REPORT: Patient C. S. M., a 57-year-old woman, presented with squamous cell carcinoma of the larynx and underwent radiotherapy and a total laryngectomy followed by reconstruction by primary closure. However, she developed a postoperative complication due to the formation of a cervical esophagocutaneous fistula that required four surgical procedures to ensure permanent closure. The latter procedure consisted of the creation of a microsurgical ALT flap to correct the esophagocutaneous fistula, the subject of this report. CONCLUSIONS: The microsurgical ALT flap has extensive applications in various fields based on the following characteristics: reliable vascularization, long and broad vascular pedicle (at least 8 cm long), an extensive and easily delimited flap region, the possibility of reducing the primary flap thickness to 3-5 mm without compromising its vascularization, the possibility of a simultaneously dual approach because of the distance between the donor and recipient site, and the possibility of primary closure without the need for skin grafting. The case report presented in this study emphasizes the possibility of the use of a microsurgical ALT sandwich flap to correct a high output esophagocutaneous fistula.

RESUMO

INTRODUÇÃO: O retalho anterolateral da coxa (RALC) vem se tornando uma ferramenta importante na reconstrução microcirúrgica de defeitos cutâneos e subcutâneos. Tendo em vista que o RALC pode se estender por até 35 cm, ele pode ser utilizado tanto na reconstrução esofágica como em defeitos cutâneos cervicais. RELATO DO CASO: Paciente C.S.M., sexo feminino, 57 anos, apresentando um carcinoma espinocelular de laringe foi submetida à radioterapia e tratamento cirúrgico de laringectomia total seguido de reconstrução por fechamento primário. Porém, a paciente evoluiu com uma complicação pósoperatória devido à formação de uma fístula esôfago-cutânea cervical, sendo necessária a realização de 4 procedimentos cirúrgicos para fechamento definitivo da fístula. O último procedimento consistiu na realização de retalho microcirúrgico da região anterolateral da coxa para correção da fístula esôfago-cutânea, objeto desse relato. CONCLUSÕES: O retalho microcirúrgico anterolateral da coxa possui extensa aplicação em diversos campos, baseada nas seguintes características: vascularização confiável, pedículo vascular longo e largo de no mínimo 8 cm, região do retalho extensa e de fácil delimitação, possibilidade de diminuir a espessura primária do retalho em 3 a 5 mm sem risco de comprometer sua vascularização, possibilidade de abordagem dupla simultânea devido à distância entre sítio doador e receptor, possibilidade de fechamento primário sem a necessidade de enxerto de pele. O relato do caso apresentado neste estudo justifica-se com o intuito de ressaltar a possibilidade da utilização microcirúrgica do RALC em sanduíche na correção de fístula esôfago-cutânea de alto débito.

Soft tissue chondroma case report and review of the literature

ABSTRACT

The present study presents a case report of extraskeletal chondroma or soft tissue chondroma, a rare and benign lesion, diagnosed by clinical findings, imaging studies and histopathological exam. The 24-year-old patient presented at the Orthopedics Section of Unimed Betim Hospital with a slow painless growth on the left thumb about 1 year ago. During the physical examination, a nodular lesion was noted; it had a hard consistency, was mobile and painless, and didn't cause functional impairment for the finger. The patient was referred to the imaging unit of the hospital, where an X-ray was performed; later on, ultrasound and magnetic resonance imaging of the left thumb was done. After the imaging studies, the medical team chose excisional biopsy of the lesion. The histopathological examination confirmed the diagnosis of extra-skeletal chondroma or soft-tissue chondroma. Soft tissue chondroma is usually not considered for a soft-tissue tumor of the hand. Excision is easy and must be complete if recurrences are to be avoided. Histopathological evaluation is required for the final diagnosis.

RESUMO

O presente estudo tem o objetivo de apresentar um relato de caso sobre condroma extra-esquelético ou de partes moles, uma lesão rara e benigna diagnosticada através da clínica, radiologia e exame histopatológico. O paciente (24 anos) compareceu ao Setor de Ortopedia do Hospital Unimed Betim com queixa de aumento de volume no polegar esquerdo, indolor há aproximadamente um ano. Durante o exame físico verificou-se presença de uma lesão nodular, de consistência dura, móvel, indolor que não acarretava em alteração funcional do dedo. O paciente foi encaminhado para setor de radiologia do hospital onde foi realizado RX e posteriormente ultrasonografia e ressonância magnética do polegar esquerdo. Diante dos achados dos estudos de imagem a equipe optou pela biópsia excisional da lesão. O exame histopatológico confirmou o diagnóstico de condroma extra-esquelético. Os elementos definidores do condroma de partes moles são: 1) curso clinico benigno e sintomatologia frusta; 2) ausência de conexão entre o tumor e estruturas adjacentes como periosteo, capsula articular ou osso; 3) crescimento lento; 4) ausência de prevalência por sexo; 5) características radiológicas e histológicas do tumor. O diagnóstico raramente é aventado quando confrontamos tumores de partes moles das mãos. A excisão é simples e deve ser completa para se evitar recorrências. A avaliação histopatológica deve ser realizada para confirmação diagnóstica.

ABSTRACT

The greater omentum, a large fold of the peritoneum, can be used as a free flap in numerous procedures due to the extensive network of its broad vascular pedicle and its versatility when manipulated. In this case report, we present a patient with a complex degloving injury, in which an omental flap was the best therapeutic option for reconstruction of the hand. The greater omental flap is a viable option for complex lesions of the hand, and provides good coverage of the lesion and good functional and aesthetic results.

O grande omento, uma larga prega de peritônio, pode ser usado como retalho livre em numerosos procedimentos devido à vasta e calibrosa rede de seu pedículo vascular e à sua versatilidade para ser manipulado. Neste relato de caso, nós apresentamos um paciente vítima de complexa lesão de desenluvamento, cujo retalho de grande omento se provou como a melhor opção para a reconstrução da mão dentre as outras opções terapêuticas. O retalho de grande omento é uma opção viável para lesões complexas da mão, oferecendo boa cobertura da lesão e bom resultado funcional e estético.

Hidradenitis suppurativa (acne inversa): review of the literature and case report on the surgical treatment of a presternal lesion

ABSTRACT

Hidradenitis suppurativa is a chronic debilitating and stigmatizing disease that is difficult to treat. The disease presents several clinical characteristics, which may occur alone or simultaneously in various locations, generally symmetrical and distributed in the "milk line". It affects the following areas of the skin where intertriginous apocrine glands are numerous, in the descending order: axilla, anogenital region, areolas, and inframammary crease. Its insidious progression begins with formation of subcutaneous nodules that rupture and/or coalesce, forming extremely painful abscesses in the deep dermis. The lesions often drain foul purulent exudate, with significant damage to quality of life. As the disease progresses, formation of fistulas, comedones, fibrosis, dermal contractures, and hardening of the skin occur. The highest chances of cure are lie in early diagnosis and individualized treatment, which covers pharmacological, behavioral, and surgical measures. Surgical treatment has been considered a more effective curative measure. The decision between the different modalities will depend on the stage, presentation, and local commitment and include incision and drainage of abscesses, deroofing, marsupialization, electrosurgery, Nd:YAG laser, CO2 laser, and extensive surgical excision. The reconstruction options include healing by second intention, immediate or delayed full-thickness skin graft, primary closure, and flaps. The reported case of presternal injuries presented clinical and histological characteristics compatible with hidradenitis suppurativa; this location has been rarely reported in the literature. The postoperative results of complete resection of the lesion with primary closure indicated resolution over a long follow-up period. More randomized clinical trials are needed to determine the best management strategy for hidradenitis suppurativa.

A Hidradenite Supurativa é uma doença crônica debilitante, estigmatizante e de difícil tratamento. A doença apresenta várias características clínicas, podendo ocorrer isolada ou simultaneamente em diversas localizações, geralmente simétricas, distribuídas na "linha do leite". Afeta a pele onde há maior quantidade de glândulas apócrinas intertriginosas, em ordem decrescente: axilas, região ano-genital, aréolas e sulco inframamário. Seu curso insidioso inicia com nódulos subcutâneos que se rompem e/ou coalescem, formando abscessos na derme profunda, extremamente doloridos. As lesões frequentemente drenam exudato purulento fétido, com importante prejuízo à qualidade de vida. Com a progressão da doença, ocorre formação de fistulas, comedões, fibrose, contraturas dérmicas e endurecimento da pele. Suas maiores chances de cura estão no diagnóstico precoce e tratamento individualizado, que abrange medidas farmacológicas, comportamentais e cirúrgicas. O tratamento cirúrgico tem sido considerado a medida curativa mais efetiva. A decisão entre as diversas modalidades vai depender do estágio, apresentação e comprometimento local e incluem incisão e drenagem dos abscessos, deroofing, marsupialização, eletrocirurgia, laser Nd:YAG, laser de CO2 e excisão cirúrgica extensa. As opções de reconstrução incluem cicatrização por segunda intenção, enxerto de pele total imediato ou tardio, fechamento primário e retalhos. O caso relatado de lesões préesternais apresentava características clínicas e histológicas compatíveis com HS, sendo esta uma localização incomum na Literatura. O resultado pós-operatório da ressecção de toda a lesão com fechamento primário mostrou-se resolutivo após longo tempo de seguimento. Mais ensaios clínicos randomizados são necessários para estipular o melhor manejo na HS.

ABSTRACT

In more-distal amputations of the fingertips, microsurgical replantation is not feasible. For these cases, composite graft provides the best functional and aesthetic results. However, its reintegration is uncertain. Several techniques have been proposed to improve bulky graft survival by basically reducing its volume, regardless of whether a skin flap is connected. Other techniques create an additional contact surface for plasmatic diffusion, the so-called subcutaneous pocket, without reducing the composite graft volume and yielding high success rates. This article presents a case of amputation of the distal tip of the fifth digit (Ishikawa zone I) of a 2-year-old child. Because of the impossibility of microsurgical replantation, a composite graft was used to reintegrate the amputated stump, without debridement, by creating a new contact surface for plasmatic diffusion at the distal end of the grafted stump, on the dermal plane in the hypothenar region, thereby increasing the contact area and decreasing the radial distance for the plasmatic diffusion of the composite graft. Twelve days later, the additional contact was separated and both surfaces presented bleeding. Full reintegration occurred with minimal scarring of the finger and hypothenar region. A brief literature review was conducted, discussing surgical concepts and factors that influence composite graft survival. The most appropriate and best vascularized anatomic plane for additional contact with the graft in the recipient area needs to be determined.

RESUMO

Nas amputações mais distais da ponta dos dedos, o reimplante microcirúrgico pode não ser praticável. Nestes casos, o enxerto composto oferece os melhores resultados funcionais e estéticos, sendo, porém, incerta a sua reintegração. Várias técnicas foram aventadas para melhorar a sobrevida do enxerto volumoso, basicamente diminuindo o seu volume, associando ou não um retalho cutâneo. Outras técnicas criam uma superfície adicional de contato para difusão plasmática, o "bolso subcutâneo", sem diminuir o volume do enxerto composto, com altas taxas de sucesso. O presente artigo apresenta um caso de amputação da ponta distal do dedo mínimo (zona I de Ishikawa) numa criança de 2 anos de idade. Impossível de reimplante microcirúrgico, a reintegração foi feita com enxerto composto do coto amputado, sem desbridamento, e com a criação de uma nova superfície de contato para difusão plasmática, na extremidade distal do coto enxertado, no plano dérmico da região hipotênar, aumentando assim a área de contato e diminuindo a distância radial da difusão plasmática no enxerto composto. Doze dias após, esse contato adicional foi separado e ambas as superfícies apresentaram sangramento. A reintegração foi total, com mínimas cicatrizes no dedo e na região hipotênar. Uma breve revisão bibliográfica foi feita e discutidos os conceitos cirúrgicos, assim como os fatores que influenciam na sobrevida do enxerto composto. Na área receptora, o plano anatômico mais adequado e melhor vascularizado, para o contato adicional com o enxerto, necessita ser determinado.

Cultural adaptation of rhytidectomy outcome evaluation questionnaire: facial outcome evaluation

ABSTRACT

INTRODUCTION: Quality of life questionnaires have been shown to be useful to confer greater objectivity to the evaluation of treatment outcomes. In turn, the internationalization of these instruments allows the comparison of different populations, although it requires a specific methodology to avoid misinterpretations due to translation errors or cultural differences. The Facial Outcome Evaluation (FOE) questionnaire, translated to English, is an instrument that is easy to apply and is based on objective questions; therefore, it is highly relevant for this purpose. The questionnaire has already been tested in relation to its reliability, validity, and responsiveness. OBJECTIVES: To translate and culturally adapt to the Brazilian Portuguese version of the FOE questionnaire. METHODS: The questionnaire was translated and culturally adapted to Portuguese, according to the methodology proposed by Beaton et al., based on the following 5 phases: phase 1, translation by two native Portuguese speakers; phase 2, generation of a synthetic version; phase 3, reverse translation by two native English speakers; phase 4, review by an evaluation committee; and phase 5, application to a population of 20 people. RESULTS: The questionnaire was translated and successfully adapted, without comprehension problems in the final population.

RESUMO

INTRODUÇÃO: O emprego de questionários de qualidade de vida (QV) tem se mostrado muito útil no sentido de dar maior objetividade à avaliação de resultados de tratamentos. A internacionalização desses instrumentos, por sua vez, permite a comparação interpopulacional, mas requer uma metodologia específica, a fim de não causar distorções devido a falhas na tradução ou a diferenças culturais. O questionário FOE (Facial Outcome Evaluation), de língua inglesa, é uma ferramenta de simples aplicação, com perguntas objetivas com boa aplicação para esse fim. O questionário já foi testado em relação à sua confiabilidade, validade e capacidade de resposta. OBJETIVOS: Traduzir e adaptar culturalmente para o português brasileiro o questionário Facial Outcome Evaluation. MÉTODOS: Realizada tradução e adaptação cultural para a língua portuguesa, segundo a metodologia proposta por Beaton et al., na qual existem 5 estágios. Estágio 1 - tradução por meio de dois tradutores nativos de língua portuguesa. Estágio 2 - confecção de versão de síntese. Estágio 3 - tradução reversa por dois tradutores nativos de língua inglesa. Estágio 4 - revisão por um comitê avaliador. Estágio 5 - aplicação a uma população de 20 pessoas. RESULTADOS: O questionário foi traduzido e adaptado com sucesso, sem problemas de compreensão para a população final.
