ABSTRACT

This article is about the tender nature of patient-doctor relationship, its limitations, details and particularities. Here, we make some considerations about actual behavior of media and internet, each time more and more demanding and tendentious, as well as how to act in a consultation room. We realized that Ethics and an abroad comprehension of our possibilities and limitations are the key for avoiding juridic problems and to obtain a satisfactory patient-doctor relationship.

RESUMO

Este artigo versa sobre a delicada natureza da relação médico-paciente, suas limitações, nuances e particularidades. Nele, tecemos considerações sobre o comportamento atual da mídia e internet, cada vez mais exigentes e tendenciosas, bem como o atuar dentro do consultório médico. Observamos que a ética, e um amplo entendimento de nossas possibilidades e limitações são a chave para se obter boa relação médico-paciente evitando demandas judiciais. .

Positioning the scar of the region brachioplasty upper arm in ex-obese patients

ABSTRACT

INTRODUCTION: Brachioplasty is the surgical treatment of severe ptosis of the arm tissues, which is located on the inside and bottom, extending from the armpit to the elbow when the arm is in abduction, having or not fat deposits. OBJECTIVE: The objective of this work is to show the positioning of the brachioplasty scar in the lower region of the arm, viewed only afterwards, in ex-obese patients, not being visible by patient or whoever is before him or her, during the arm abduction. METHODS: 18 patients were selected and undergone bilateral brachioplasty with lower region positioning of the scar. RESULTS: the satisfaction degree of the patients with the scar positioning in this region was relevant, allowing it to be imperceptible during arm abduction by the patient or whoever is before him or her. CONCLUSIONS: The surgical technique applied to brachioplasty is safe and reproducible, constituting a good alternative to disguise the scar in this region, important factor in ex-obese patients' surgery.

RESUMO

INTRODUÇÃO: A braquioplastia consiste no tratamento cirúrgico da acentuada ptose dos tecidos do braço, que se localiza na região interna e inferior, estendendo-se da axila ao cotovelo, quando o braço está em abdução, contendo ou não depósito de gordura. OBJETIVO O objetivo deste trabalho é mostrar o posicionamento da cicatriz da braquioplastia na região inferior do braço, visto somente posteriormente, em pacientes ex-obesos, não sendo visível pelo paciente ou por quem estiver à sua frente durante a abdução do braço. MÉTODOS: Foram selecionados 18 pacientes, realizando-se a braquioplastia bilateral, com posicionamento da cicatriz na região inferior. RESULTADOS: O grau de satisfação dos pacientes com o posicionamento da cicatriz nesta região foi elevado, permitindo que a mesma fique imperceptível durante a abdução do braço, não sendo vista pelo paciente ou por outra pessoa que se encontre em sua frente. CONCLUSÕES: A técnica cirúrgica apresentada para braquioplastia é segura e reprodutível, constituindo uma boa opção para disfarçar a cicatriz nesta região, fator importante na cirurgia dos pacientes ex-obesos.

Sistematizacion and conduct of plastic surgery service of hospital geral de goiânia in mammoplasty in massive weight loss patients in last ten years

ABSTRACT

BACKGROUND: With the increase of the population obesity indices occurred a growth of the numbers of ex-obese patients and aesthetic/reconstructive plastic surgery in this population. Objectives: To present a systematic approach adopted in patients undergoing breast surgery after massive weight loss at Hospital Geral de Goiânia (HGG) in the last 10 years, and assess the results. METHOD: We conducted a retrospective study of cases of breast surgery after massive weight loss in the Department of Plastic Surgery of HGG, from January 2002 to December 2011. The patients were individually analyzed preoperatively and submitted to the classification of Kaluf to breast dysmorphism after major weight loss. The operative technique varied depending on the classification of the type of breast. RESULTS: The sample totaled 73 female patients, with mean age of 44 years and mean weight loss of 48 kg. In 41 patients (56.16%) was used the technique with inclusion of prostheses and in 32 (43.83 %) chose to not include silicone implants. Complications occurred in eight patients (10.95%), one case of hematoma (1.36%), two cases of suffering partial nipple-areola complex (2.73%), five cases of small suture dehiscence or delayed healing (6.84%) and one case of breast ptosis in the immediate postoperative period (1.36%). CONCLUSION: Due to the wide range of dimorphisms in emaciated patients, in particular changes in their breasts, we note that it is necessary to establish protocols and routines for effective assessment and treatment, with the least possible morbidity, associated with better cosmetic results.

RESUMO

INTRODUÇÃO: Com o aumento dos índices de obesidade populacional ocorreu um crescimento do número de pacientes ex-obesos, e de cirurgias plásticas nesta população. OBJETIVOS: Apresentar a sistematização e conduta adotada nos pacientes submetidos a mamoplastia, após grande perda ponderal, operados no Hospital Geral de Goiânia (HGG) nos últimos 10 anos, bem como avaliar os resultados obtidos. Método: Foi realizado um estudo retrospectivo dos casos de mamoplastia após grande perda ponderal, operados no Serviço de Cirurgia Plástica do HGG, de janeiro de 2002 a dezembro de 2011. As pacientes foram analisadas individualmente no pré-operatório e submetidas à classificação de Kaluf, para dismorfias mamárias após grande emagrecimento. A técnica operatória variou em função da classificação do tipo de mama. RESULTADOS: A casuística totalizou 73 pacientes, do sexo feminino, com faixa etária média de 44 anos, e perda ponderal média de 48 Kg. Em 41 pacientes (56,16%) foi utilizada a técnica com inclusão de próteses, e em 32 (43,83%) optou-se por não incluir os implantes de silicone. As complicações ocorreram em oito pacientes (10,95%), sendo um caso de hematoma (1,36%), dois casos de sofrimento parcial do complexo aréolo-mamilar (2,73%), cinco casos de pequenas deiscências de sutura ou retardo de cicatrização (6,84%) e um caso de ptose de mama no pós-operatório imediato (1,36%). CONCLUSÃO: Devido à grande variação de dismorfismos em pacientes emagrecidos, em especial as alterações mamárias, notamos que se torna necessário o estabelecimento de protocolos e rotinas para avaliações e tratamentos eficazes, com a menor morbimortalidade possível, associados aos melhores resultados estéticos.

ABSTRACT

According to the Ministry of Health are expected in Brazil, in 2012, 52,680 new cases of breast cancer with an estimated risk of 52 cases per 100 000 women. Recently the quality of life (QOL), now considered a parameter increasingly important to evaluate the result of treatment. To assess the QOL of women with breast cancer, Pusic et al. developed a questionnaire called BREAST-Q, related to breast surgery, and divided into three modules: an augmentation, reduction and reconstruction with independent scales to assess the major problems affecting patients undergoing each type of procedure, either in the pre and postoperatively. This questionnaire was developed in English but now has versions in several languages. We present the translation process for the construction of the Portuguese version, which took part in the translation, backward translation and testing to obtain the final version and its approval by the owner (MAPI Trust© _Mapi Research Institute 2002). Translation is the beginning of the process of validation of questionnaires and their use in order to assess the impact of treatment of breast cancer from the perspective of Brazilian women.

RESUMO

Segundo o Ministério da Saúde, esperavam-se para o Brasil, em 2012, 52.680 casos novos de câncer de mama, com risco estimado de 52 casos a cada 100 mil mulheres. Recentemente, a qualidade de vida (QV), passou a ser considerado um parâmetro cada vez mais importante para avaliar o resultado do tratamento. Para avaliar a QV das mulheres com câncer de mama, Pusic et al. desenvolveram um questionário denominado BREAST-Q, relacionado à cirurgia de mama, e dividido em três módulos: aumento, redução e reconstrução com escalas independentes para avaliar os maiores problemas que afetam as pacientes submetidas a cada tipo de procedimento, tanto no pré como no pós-operatório. Este questionário foi desenvolvido na língua inglesa, mas já possui versões em vários idiomas. Apresentamos o processo de tradução para a construção da versão na língua portuguesa, do qual fizeram parte a tradução, tradução reversa e testagem até a obtenção da versão final e sua aprovação pelo proprietário (MAPI Trust© Mapi Research Institute 2002). A tradução é o início do processo de validação dos questionários e sua utilização com o objetivo de avaliar o impacto do tratamento do câncer de mama sob a perspectiva das mulheres brasileiras.

Practical model for tenorrhaphy training

ABSTRACT

INTRODUCTION: The training of residents in surgical specialties is a challenge, since psychomotor skills are eventually acquired and developed in vivo in the very first attempt. The simulation techniques through practical models allows the resident's prior training, so that motor skills can be enhanced and evaluated in vivo before surgery. In the context of Plastic Surgery, the repair of tendons, especially flexors, presents technical difficulties during the first attempts of the resident. OBJECTIVE: The proposed model is designed for practicing tenorraphies, designed to be easy reproducibility and low cost. METHODS: We used a simulator using fixed mount, PVC pipe, pig tendons of legs and elastic envisioned to mimic the natural tension exists in the flexor tendons. RESULTS: The model was satisfactory in the intention of allowing the simulation of tenorraphies, due to the similarities between human and pig tendons, and conferred by the elastic tension. CONCLUSIONS: The model provided adequate training of motor skills in tendon sutures. This is an interesting option to be used in the early learning curve.

RESUMO

INTRODUÇÃO: O treinamento dos residentes de especialidades cirúrgicas representa um desafio, uma vez que as habilidades psicomotoras são, eventualmente, adquiridas e desenvolvidas in vivo já nas primeiras tentativas. A simulação das técnicas com o uso de modelos práticos permite o treinamento prévio do residente, de forma que as habilidades motoras possam ser incrementadas e avaliadas antes da cirurgia in vivo. No âmbito da Cirurgia Plástica, o reparo de tendões, em especial dos flexores, apresenta dificuldades técnicas durante as primeiras tentativas do residente. OBJETIVO O modelo proposto foi idealizado para a prática de tenorrafias, idealizado para ser de fácil reprodutividade e baixo custo. MÉTODO: Foi utilizado um simulador de tenorrafias utilizando suporte fixo, tubos de PVC, tendões de patas de suínos e elásticos idealizados para imitar a tensão natural existente nos tendões flexores. RESULTADOS: O modelo utilizado mostrou-se satisfatório na intenção de permitir a simulação de tenorrafias, devido às semelhanças entre tendões humanos e suínos, além da tensão conferida pelos elásticos. CONCLUSÕES: O modelo propiciou adequado treinamento de habilidades motoras nas suturas tendinosas. Trata-se de uma opção interessante a ser utilizada no início da curva de aprendizado.

Rhytidectomy sector time - incision intracapilar marginal and drift of the orbicularis blepharoplasty

ABSTRACT

SUMMARY: Introduction The authors refer to developments in the treatment of facial aging. OBJECTIVE: Through an incision marginal temporal intracapillary reduce sagging eyelids and raise the tail of the eyebrow, making economic blepharoplasty. METHOD: We propose performing rhytidectomy segment temporal incision intra capillary marginal curve, facilitating compensation skin, within 0.5 cm of the scalp, it desepidermizando 2 mm. Removing fat conclude the edge of the flap and performing suture tricofítica and camouflaging the best scar, which becomes pre capillary by growth of hair along it. Detachment in the subcutaneous plane to the edge of the lateral half of the orbicularis, traction suture and lateral-superior range with separate points of it. Other procedures for the remainder of the face and neck were performed when necessary. The lateral traction reduces sagging eyelid skin and its excesses, making blepharoplasty small economical resections with skin, and removal of bags for minimal dilatation individualized orbicularis muscle at the site of projection of the same. RESULTS: We obtained good results by reducing the stigma of aging orbital region. It is a simple procedure of low morbidity, leaving scar marginal temporal good quality for small skin traction and easily camuflável by the hair growing along her downward covering it. 485 cases were operated from 1997 to 2012. CONCLUSION: The method is easy to perform, low morbidity, scarring of good quality even in young people, reducing needs detachments of the eyelids.

RESUMO

INTRODUÇÃO: Os autores referem à evolução no tratamento do envelhecimento facial. OBJETIVO Por meio de incisão marginal intracapilar temporal reduzir a flacidez palpebral e elevar a cauda do supercílio, tornar a blefaroplastia econômica. MÉTODO: Propõem realizar a ritidoplastia do segmento temporal com incisão intracapilar marginal curva, facilitando as compensações de pele, 0,5 cm dentro do couro cabeludo, desepidermizando 2 mm dele. Concluem o procedimento, desengordurando o bordo do retalho e realizando sutura tricofítica, camuflando melhor a cicatriz, que se torna pré-capilar, pelo crescimento de cabelos junto dela. Descolamento no plano subcutâneo até o bordo da metade lateral do músculo orbicular, tração e sutura latero-superior em leque com pontos separados do mesmo. Outros procedimentos no restante da face e pescoço quando necessários foram realizados. A tração lateral reduz a flacidez palpebral e seus excessos de pele, tornando a blefaroplastia econômica, com ressecções pequenas de pele, e remoção das bolsas por mínima divulsão individualizada do músculo orbicular, no local da projeção das mesmas. RESULTADOS: Obtiveram bons resultados, reduzindo o estigma do envelhecimento da região orbitaria. Trata-se de um procedimento simples, de morbidade baixa, deixando cicatriz marginal temporal de boa qualidade, pela pequena tração da pele, é facilmente camuflável pelos cabelos que crescem junto dela, em direção inferior, cobrindo-a. Foram operados 485 casos de 1997 a 2012. CONCLUSÃO: O método é de fácil execução, baixa morbidade, cicatrizes de boa qualidade mesmo em jovens, reduzindo necessidades de descolamentos das pálpebras.

ABSTRACT

OBJECTIVE: The aim of the current study is to propose a classification based on neck lamellas for the aesthetic variations of the cervical region, considering its unique anatomy and analysing each structure involved (skin, muscle, fat and submandibular gland), in order to make it easier to choose the most appropriate surgical treatment. METHOD: The senior author, elaborated an observational study of 480 patients operated for neck and face lift between 1998 and 2010, established a classification comparing the skin flaccidity, the amount of fat accumulation, muscle flaccidity and cervical contour alterations, with the best surgical treatment for each group, being grade I (11.7%), grade II (7%), grade III (23%), grade IV (40%) and grade V (18.3%). RESULTS: The development of the current classification made it possible to group the cervical alterations that results from the aging process. Each group has common, easily recognizable alterations and for each one we have proposed a different treatment algorithm, in relation to the technique and the scar positioning (classic neck and facelift, short scar facelift, cervical lift, atypical lift and neck and facelift with anterior temporal scar). CONCLUSION: With the present study it was possible to come to the conclusion that the cervical region is the main determinant for the selection of the best surgical technique and scars placement in the surgery for the rejuvenation of the neck and face.

RESUMO

OBJETIVO: O objetivo do presente trabalho foi propor uma nova classificação das alterações estéticas da região cervical, com base na sua anatomia, a fim de direcionar o tratamento cirúrgico adequado. O intuito foi classificar e sistematizar a abordagem cirúrgica de cada estrutura envolvida (pele, músculo, gordura e glândula submandibular), correlacionando o procedimento cirúrgico mais indicado para o seu tratamento. MÉTODO: O autor sênior, baseado em observação de 480 casos operados, entre os anos de 1998-2010, estabeleceu uma classificação correlacionando a flacidez de pele encontrada, grau de lipodistrofia, alteração muscular e alteração do contorno cervical, com o tratamento empregado, sendo grau I (11,7%), grau II (7%); grau III (23%), grau IV (40 %), grau V (18,3%.). RESULTADOS: O desenvolvimento da presente classificação demonstrou ser possível agrupar as alterações cervicais, decorrentes do processo de envelhecimento humano, em subgrupos (Grau I-V), com alterações comuns, facilmente reconhecíveis, facilitando a proposição do melhor tratamento para a região cervical e propiciando a criação de um algoritmo terapêutico. Esta classificação em subgrupos auxilia o cirurgião na escolha do tipo de abordagem cirúrgica e cicatrizes (Lifting cervico facial clássico, Lifting cérvico facial com cicatriz reduzida, Lifting cervical, Lifting atípico com cicatriz temporal anterior). CONCLUSÃO: Concluímos que a região cervical é a principal determinante na escolha do tipo de abordagem cirúrgica e cicatrizes, no rejuvenescimento cérvico facial.

Assessment of the degree of correction of prominent ears using a standardized treatment algorithm

ABSTRACT

BACKGROUND: The goal of aesthetic otoplasty is the correction of ear deformities by creating harmonious and symmetrical external ears, without visible scars. Otoplasty techniques based on the excision of postauricular skin are associated with high recurrence rates. Modern otoplasty is based on cartilage-cutting and cartilage-sparing techniques, alone or in combination, which lead to lower recurrence rates. OBJECTIVE: We evaluated the efficacy of otoplasty combined with other techniques in the correction of ear deformities, based on a modified version of the "algorithm for otoplasty at the Craniofacial Center at Texas Children's Hospital". METHODS: Forty patients, who underwent otoplasty for prominent ears in our institution between March and September of 2009, were prospectively assessed. The mastoid-helix distance was measured preoperatively (baseline) and at 1, 3, and 6 months postoperatively. RESULTS: The most common deformities were scaphoconchal angle greater than 90º (51.3%, 41/80 ears) and conchal hypertrophy > 2.5 cm (46.3%, 37/80 ears). Recurrence occurred in 2 (5%) patients and partial stenosis of the ear canal in 1 (2.5%). Significant reductions in the mastoid-helix distance were observed at the three time points compared with baseline (P<0.001); a significant reduction was present in the immediate postoperative period (P<0.001), followed by a small increase 3 months postoperatively (P<0.005), which appeared to stabilize between 3-6 months postoperatively (P=0.520). CONCLUSION: Otoplasty performed according to the modified algorithm resulted in a significant reduction of the mastoid-helix distance, which slightly increased in the early postoperative period, and re-mained unchanged 6 months postoperatively.

RESUMO

INTRODUÇÃO: O objetivo da otoplastia estética é a correção de deformidades da orelha, criando orelhas externas harmoniosas e simétricas. As técnicas de otoplastia, baseadas na excisão da pele pós-auricular, estão associados com a recorrência elevada. A otoplastia moderna é baseada nas técnicas de ressecção e/ou modelagem da cartilagem, que levam a menores taxas de recorrência. OBJETIVO Foi avaliada a eficácia da otoplastia com técnica combinada na correção de deformidades da orelha, baseado numa versão modificada do "algoritmo para otoplastia", no Centro Craniofacial do Hospital Infantil do Texas". MÉTODO: Quarenta pacientes submetidos à otoplastia por orelhas proeminentes, entre março e setembro de 2009, foram avaliados prospectivamente. A distância mastoide-hélice foi medida no pré-operatório e após 1, 3 e 6 meses após a cirurgia. RESULTADOS: As deformidades mais comuns foram ângulo escafo-conchal maior que 90º (51,3%, 41/80 orelhas) e hipertrofia de concha> 2,5 cm (46,3%, 37/80 orelhas). Reduções significativas na distância mastoide-hélice foram observadas nos três períodos em comparação com os valores basais (P<0,001). Houve uma redução desta medida no período pós-operatório imediato (P<0,001), seguido por um pequeno aumento após três meses da cirurgia (P<0,005) e subsequente tendência de estabilização entre 3-6 meses após a cirurgia (P=0,520). CONCLUSÃO: Otoplastia, realizada de acordo com o algoritmo citado, resultou em redução significativa da distância mastoide-hélice, com ligeira perda desta redução no início do período pós-operatório, e com tendência a manter-se inalterado após 6 meses de pós-operatório.

ABSTRACT

INTRODUCTION: Ear flappy is a congenital deformity most common of external ear, is transmitted by dominant form and occurs in 5% of general population. There are three main ways to do the modern surgery: the shaving, exision and fixation for stitchs of ear gristle. The technique of Mestardé, in 1963, do a new antihelice was supported in permanent sutures for posterior way and the stitchs takes until anterior perichondrium, without takes the skin. Furnas (1968) done suture with unabsorbed thread in conchal gristle, transfixing until anterior perichondrium and sutures in mastoid fascia. The otoplasty can be done with local anesthesia associated or not with sedation or with general anesthesia. The aim of the study is to evaluate the intensity of pain of patients submited to otoplasty with local anesthesia with sedation and without sedation, comparing two techniques. METHOD: There was separated 2 groups: Group 1: 12 patients operates with local anesthesia and sedation in surgery center; group 2: 26 patients operated with local surgery in clinic of small surgeries. The study was done in one year (2011 to 2012 May). It was done a lot of questions with these patients to evaluate the level of pain during the surgery and post-surgery. RESULTS: There was not significant diference between two groups. CONCLUSION: It is possible to do this surgery with less hospital cost and the use the rooms in surgery center for more complex surgeries.

INTRODUÇÃO: Orelha em abano representa a deformidade congênita mais comum da orelha externa, é transmitida de forma autossômica dominante e afeta aproximadamente 5% da população em geral. Existem três vias principais para a realização da otoplastia moderna: a raspagem, excisão e fixação por pontos da cartilagem auricular. A abordagem de Mustardé, em 1963, para a criação de uma nova anti-hélice era baseada em suturas permanentes por acesso posterior e os pontos englobavam até o pericôndrio anterior, sem transfixar a pele. Furnas (1968) realizava sutura com fio inabsorvível posicionado na cartilagem conchal, transfixando até pericôndrio anterior e suturado na fáscia mastoide. A otoplastia pode ser realizada com anestesia local associada ou não à sedação ou com anestesia geral. OBJETIVO O presente estudo tem como objetivo avaliar a intensidade da dor dos pacientes submetidos à otoplastia sob anestesia local com sedação e sem sedação, comparando as duas técnicas anestésicas. MÉTODO: Foram separados dois grupos: Grupo 1 - 12 pacientes operados com anestesia local e sedação no centro cirúrgico; Grupo 2 - 26 pacientes operados com anestesia local e sem sedação no ambulatório de pequenas cirurgias. O estudo foi realizado no período de 1 ano (maio de 2011 a maio de 2012). Foi realizado um questionário com esses pacientes para avaliarmos o nível de dor no intraoperatório e pós-operatório. RESULTADOS: Não houve diferença estatisticamente significativa entre os dois grupos. CONCLUSÃO: É possível realizarmos essa cirurgia com menor custo hospitalar e utilizarmos as salas do centro cirúrgico com anestesista para cirurgias mais complexas.

Un unprecedent auricular reconstruction

ABSTRACT

INTRODUCTION: As auricular reconstruction is a very difficult procedure, patients are unfortunately sometimes found in doctor's surgeries who have previously been operated on with disastrous results that are impossible to correct or improve upon with known methods. Some of these patients accept the result achieved, while others do not and thus seek a better solution. What is to be presented now cannot be found in other publications and may be added to the specialty's arsenal as a new method. This work focuses on a twenty-one-year-old patient born with microtia on the right side who, between the age of six and the day of their consultation, had undergone twenty-five unsuccessful operations. CASUISTRY AND METHOD: Only one case was operated on and the method consisted of the transfer of all the retroauricular skin from the left ear in order to cover the right auricular region and thus create an ear. The fingertips of the 2nd and 3rd fingers of the right hand were used as vectors. RESULT: The author had a satisfactory result within the desired expectations. CONCLUSION: It is possible to use fingertips as a vector for the transfer of auricular flaps for complicated cases of auricular or facial reconstruction.

INTRODUÇÃO: Por ser a reconstrução auricular um procedimento muito difícil, infelizmente encontram-se nos consultórios alguns pacientes já operados e com resultados desastrosos, impossíveis de serem corrigidos ou melhorados com os métodos já conhecidos. Alguns pacientes se conformam com o resultado conseguido, mas outros não e procuram uma melhor solução. O que vai se apresentar agora não consta em outras publicações e pode ser incluído no arsenal da especialidade como método novo. Este trabalho trata de um paciente, 21 anos, que nasceu com microtia do lado direito, que desde os 6 anos, até o dia da consulta, já tinha sido submetido a vinte e cinco operações, sem sucesso. CASUÍSTICA E MÉTODO: Um caso, que foi operado, cujo método consistiu em transferir toda a pele retroauricular da orelha esquerda, para cobrir a região auricular direita, fazendo assim uma orelha. Usaram-se como vetores as polpas dos dedos 2º e 3º da mão direita. RESULTADO: O autor ficou satisfeito com o resultado alcançado. CONCLUSÃO: É possível usar as polpas digitais como vetor para transferir retalhos auriculares para casos complicados de reconstrução auricular ou facial.

Morphometric analysis of the upper lip and the action of ageing

ABSTRACT

BACKGROUND: A pleasant smile depends on harmonious relationships between the teeth, gums, and lips. The present study measured upper lip changes related to ageing using a morphometric analysis. METHODS: Forty-six Caucasian women at least 15 years of age were selected and divided into four groups: 15 - 30 years old, 31 - 45 years old, 46 - 60 years old, and > 60 years old. Frontal photographs with closed lips and parted lips were taken. The following measurements were performed: upper lip height, upper vermilion height in parted and closed lips positions, and exposure of the central upper incisors in the relaxed position. RESULTS: Upper lip height increased with age. The average upper lip height in the closed lips position was 13.75 mm in the youngest age-range (15 - 30 years old). The upper lip height gradually increased with age until reaching an average of 19.24 mm was observed in the > 60 years old group. The opposite result was observed in upper vermilion height, which decreased with increasing age. The average vermilion height in the parted lips position was 7.09 mm in the youngest age-range (15-30 years old) and decreased to 4.58 mm in the oldest group (> 60 years old). Teeth exposure decreased with age. The average upper teeth exposure was 3.55 mm in the youngest group and decreased to 0.40 mm in the oldest group. CONCLUSION: The upper lip height increases, while the upper vermilion height and exposure of the upper teeth decrease with age.

INTRODUÇÃO: Um sorriso considerado agradável envolve relações harmônicas entre os dentes, gengivas e lábios. O presente estudo mediu as alterações no lábio superior, relacionadas ao envelhecimento, utilizando análise morfométrica. MÉTODO: Foram selecionadas 46 mulheres caucasianas com idade mínima de 15 anos e divididas em quatro grupos: 15-30 anos, 31-45 anos, 46-60 anos e > 60 anos. Foram tiradas fotografias frontais com a boca fechada e na posição de repouso. As seguintes medidas foram realizadas: altura do lábio superior, altura do vermelhão do lábio superior nas posições repouso e fechada, e exposição dos dentes incisivos superiores centrais na posição repouso. RESULTADOS: A altura do lábio superior aumentou com a idade. A média da altura do lábio superior com a boca fechada foi 13,75 mm na faixa etária mais nova (15-30 anos). A altura do lábio superior gradualmente aumenta com a idade até atingir uma média de 19,24 mm, observada no grupo > 60 anos. O resultado oposto foi observado na altura do vermelhão superior, que diminuiu com o aumento da idade. A altura média do vermelhão com a boca em repouso foi 7,09 mm na faixa etária mais nova (15-30 anos) e diminuiu para 4,58 mm no grupo mais velho (> 60 anos). A exposição dos dentes diminuiu com a idade. A exposição média dos dentes superiores foi de 3,55 mm no grupo mais jovem e diminuiu para 0,40 mm no grupo mais velho. CONCLUSÃO: A altura do lábio superior aumenta, enquanto a altura do vermelhão superior e a exposição dos dentes superiores diminuem com a idade.

Use of blades of 0.7 to 1mm in diameter and coronal incisions in hair transplantation: Surgical approach to improve density and natural results

ABSTRACT

INTRODUCTION: The hair transplant evolved greatly in the past two decades primarily by the use of microscopes to make follicular units. The use of Micro blades is another step to reduce the surgical trauma and increase the naturalness of the result. Coronal incisions help to maintain the correct direction of hair placed. The goal of this work is to show our experience with the use of blades 0.7 to 1mm in diameter, with coronal incisions for hair transplantation. METHOD: We evaluated 131 consecutive surgeries from June 2011 to June 2012, in which all patients operated on by the same surgeon with the use of microblades 0.7 to 1mm wide using coronal incisions. RESULTS:The postoperative results were considered satisfactory by the patients and the surgical team. CONCLUSION:The use of small blades with coronal incisions helps to give more density with little surgical trauma. The results were considered natural with good cosmetic density.

RESUMO

INTRODUÇÃO: O transplante capilar evoluiu muito nas últimas duas décadas, principalmente com o uso de microscópios para lapidar unidades foliculares. A utilização de microlâminas é mais um avanço para diminuir o trauma cirúrgico e aumentar a naturalidade do resultado. As incisões coronais ajudam a manter a direção correta dos cabelos colocados. O objetivo deste trabalho é mostrar nossa experiência com o uso de lâminas de 0,7 a 1 mm de diâmetro, com incisões coronais para o transplante capilar. MÉTODO: Foram avaliadas 131 cirurgias consecutivas de junho de 2011 a junho de 2012, em que todos os pacientes foram operados pelo mesmo cirurgião, com o uso de microlâminas de 0,7 a 1 mm de largura utilizando a técnica coronal para a incisão. RESULTADOS: Os resultados pós-operatórios foram considerados satisfatórios pelos pacientes e pela equipe cirúrgica. CONCLUSÃO: O uso de lâminas pequenas nas incisões coronais promove maior densidade com pouco trauma cirúrgico. Os resultados foram considerados naturais e com boa densidade cosmética.

ABSTRACT

INTRODUCTION: The augmentation mammoplasty commonly provokes in patients an inquiry about the possibility of breast falling. The plastic surgery usually answers yes, but don't find studies justifying this. In this research we focus to understand the position variation of papillary-areolar complex after breast implants observing principally the "ptosis tendency" and "bottoming out" phenomenon. METHODS: Medical records selection between patients operated in Instituto Ivo Pitanguy - Infirmary 38 of Santa Casa de Misericórdia do Rio de Janeiro since 2009 January until 2010 December based on inclusion and exclusion criterions using for this research medical records revision, photographer register and literature revision. RESULTS: Among the 20 breasts (10 patients) analyzed we observed "bottoming out" in 8 breasts (5 patients) 4 moderated and 4 mild. Occur "ptosis tendency" in 10 breasts (5 patients) 4 mild and 6 moderated. Only 1 breast didn't suffered APC variation. CONCLUSION: The "ptosis tendency" and "bottoming out" phenomenon are a reality that has to be largely studied to better elucidation for patients.

RESUMO

INTRODUÇÃO: A mastoplastia de aumento, muito comumente, gera na paciente indagações acerca da possibilidade de queda das mamas. A resposta do cirurgião plástico é geralmente afirmativa, porém, este não encontra respaldo na literatura. Neste trabalho, objetivamos justamente entender a variação de posição do Complexo aréolo-papilar pós-inclusão de implantes mamários observando, principalmente, os fenômenos de "tendência à ptose" e "bottoming out". MÉTODOS: Seleção de prontuários dentre as pacientes operadas no Instituto Ivo Pitanguy - Enfermaria 38 da Santa Casa de Misericórdia do Rio de Janeiro de jan/2009 a dez/2010, com base em critérios de inclusão e exclusão, utilizando-se para esta pesquisa de revisão dos prontuários, registro fotográfico e revisão da literatura. RESULTADOS: : Dentre as 20 mamas ( 10 pacientes ) analisadas observamos "bottoming out" em 8 mamas ( 5 pacientes ) das quais 4 de graus moderados e 4 leves. Houve "tendência à ptose" em 10 mamas (5 pacientes) sendo 4 leves e 6 moderadas. . CONCLUSÃO: Os fenômenos "bottoming out" e tendência à ptose" são uma realidade que precisa ser amplamente estudada para melhor esclarecimento das pacientes.

Treatment of contractures in retroglandular mammary augmentation using retropectoral implants and capsular flaps

ABSTRACT

INTRODUÇÃO: Capsular contracture is one of the most common causes of surgical revision in the mammary augmentation procedure. This phenomenon can happen with any kind of implant, or layer of the pocket. The literature shows that there is an increased prevalence in the implant in the retro mammary position, and for this reason the majority of patients whom operate for treating contracture are in this situation. In these patients we observe a significant thinning of the gland and the subcutaneous tissue, then, we create a rectropectoralis pocket to insert the new implant making use of a capsular flap, to involve it completely. This allows larger implants house with great security, nice cosmetic results and high satisfaction.

RESUMO

INTRODUÇÃO: Este trabalho propõe, nos casos de contratura capsular em mamoplastias de aumento retroglandulares, a inclusão de novos implantes no plano retropeitoral, empregando retalhos de cápsula para sua fixação. MÉTODO: Foram avaliadas, retrospectivamente, 27 pacientes portadoras de contratura capsular tratadas desta forma entre maio de 2007 e junho de 2012. RESULTADOS: A técnica utilizada permitiu abrigar implantes maiores com bom aspecto de contorno mamário, assegurados pela cobertura muscular, estabilizada pelo retalho capsular. CONCLUSÕES: A técnica apresentada é um método seguro, reprodutível, tecnicamente simples, de resultados satisfatórios e baixa prevalência de intercorrências.

Neoadjuvante chemotherapy and immediate breast reconstruction: a good option?

ABSTRACT

BACKGROUND: The combination of neoadjuvant chemotherapy and immediate breast reconstruction is a field under discussion. There are doubts whether the association of events could result in an increase of postoperative complications in patients undergoing the surgical procedure. METHODS: Retrospective study of patients who underwent immediate breast reconstruction from January 2010 to January 2012. Patients submitted to neoadjuvant chemotherapy were compared with those who had not receive any form of chemotherapy and those who received adjuvant chemotherapy. RESULTS: Fifty-four patients were selected, totalizing 71 immediate breast reconstructions. Thirteen patients received neoadjuvant chemotherapy (15 reconstructions) twenty-three patients did not receive chemotherapy (33 reconstructions) and eighteen received adjuvant chemotherapy (23 reconstructions). Minor complications occurred in 33.3% of breast reconstruction in the group undergoing neoadjuvant therapy. There were 6.7% major complications in the same group. There was no statistical differences in the number of minor and major complications between groups. CONCLUSION: In the series analyzed, the use of neoadjuvant chemotherapy did not increase the number of postoperative complications in patients undergoing immediate breast reconstruction.

INTRODUÇÃO: A combinação de quimioterapia neoadjuvante e reconstrução mamária imediata é um campo em discussão. Há dúvidas se a associação dos eventos poderia resultar em um aumento de complicações pós-operatórias nas pacientes submetidas ao procedimento cirúrgico. MÉTODOS: Estudo retrospectivo de pacientes submetidas à reconstrução mamária imediata entre janeiro de 2010 e janeiro de 2012. As pacientes submetidas à quimioterapia neoadjuvante foram comparadas com as que não receberam qualquer forma de quimioterapia e com as que receberam quimioterapia adjuvante. RESULTADOS: Cinquenta e quatro pacientes foram selecionadas, totalizando 71 reconstruções mamárias imediatas. Treze pacientes receberam quimioterapia neoadjuvante (15 reconstruções), vinte e três pacientes não receberam quimioterapia (33 reconstruções) e dezoito receberam quimioterapia adjuvante (23 reconstruções). Complicações menores ocorreram em 33,3% das reconstruções mamárias no grupo submetido à neoadjuvância. Houve 6,7% de complicações maiores no mesmo grupo. Não houve diferenças estatísticas no número de complicações menores e maiores entre os grupos. CONCLUSÃO: Na série analisada, o emprego da quimioterapia neoadjuvante não resultou em aumento do número de complicações pós-operatórias nas pacientes submetidas à reconstruções mamárias imediatas.

Breast reconstruction: an analysis of the new trends and major complications

ABSTRACT

INTRODUCTION: Because of its high incidence, breast cancer becomes a major concern, justified by psychological and social impacts. The purpose of the study is to analyze the current trends for breast reconstruction and evaluate its major complications. METHOD: A retrospective study was performed in a consecutive series of breast reconstructions in the period February 2011 to February 2012, through analysis of medical records. RESULTS: A total of 127 breast reconstructions were performed. The immediate reconstructions represent 73% of the total. The following techniques were used: permanent silicone implant, 54% (n = 69), Becker adjustable implant, 14% (n = 17), transverse rectus abdominis myocutaneous flap (TRAM), 13% (n=17), latissimus dorsi flap and implant 13% (n=16) and single expander 6% of cases (n=8). Major complications occurred in 16.5% (n = 21) and most (63% n=12) had a history of radiation therapy, neoadjuvant chemotherapy and / or smoking history. The complications resulted from hematoma reoperation (0.72% n=1), extensive necrosis of the flap requiring debridement and / or positioning of the implant to recover the lost volume (4% n=5), infections (3 % n=4), extrusion and removal of the implant (7% n=9) and thromboembolic events (1.5% n=2). CONCLUSION: The choice of technique for breast reconstruction must be individualized and based on the characteristics and desires of each patient, the familiarity of the technique by the surgical team, the hospital resources available and costs generated by the techniques to optimize the results, reduce costs and complication rates.

RESUMO

INTRODUÇÃO: Em razão da sua elevada incidência, o câncer de mama torna-se uma das grandes preocupações, sobretudo pelos impactos psicológicos e sociais. Os objetivos do presente trabalho são analisar as tendências atuais para a reconstrução mamária e avaliar suas complicações maiores. MÉTODO: O estudo retrospectivo foi realizado numa série consecutiva de reconstruções mamárias, no período de fevereiro de 2011 a fevereiro de 2012, pela análise de prontuários médicos. RESULTADOS: Um total de 127 reconstruções mamárias foram realizadas. As reconstruções imediatas representam 73% do total. As seguintes técnicas foram utilizadas: implante de silicone permanente, 54% (n=69), implante ajustável de Becker, 14% (n=17), retalho transverso do músculo reto abdominal (TRAM), 13% (n=17), retalho do músculo latíssimo do dorso com implante, 13% (n=16), e expansor simples, 6% dos casos (n=8). Complicações maiores ocorreram em 16,5 % (n=21) sendo que na maioria (63% n=12) possuíam passado de radioterapia, QT neoadjuvante e/ou história de tabagismo. As complicações foram decorrentes de hematoma com reintervenção / reinternação (0,72% n=1), necrose extensa do retalho com necessidade de desbridamentos e/ou posicionamento de implante para recuperação do volume perdido (4% n=5), infecções (3% n=4), extrusão e retirada do implante (7% n=9), e eventos tromboembólicos (1,5% n=2). CONCLUSÃO: A escolha da técnica para a reconstrução mamária deve ser individualizada e baseada nas características e anseios de cada paciente, na familiaridade da técnica pela equipe cirúrgica, nos recursos hospitalares disponíveis e nos custos gerados pelas técnicas, a fim de otimizar os resultados, reduzir custos e os índices de complicações.

Lifting of the mons pubis after massive weigth loss

ABSTRACT

INTRODUCTION: This article describes the surgical planning, conducts pre and post operatively, results in the Lifting Pubic ex-obese patients submitted to gastroplasty or not, that feature large sagging skin and excess fat from the pubis. METHOD: Two hundred and eighteen patients underwent surgery from May 2005 to August 2012. The pubic lifting was held in conjunction with the Anchor Adbominoplasty. The pubis was raised through points positioned 2-3 cm above its original position in the aponeurosis of the muscles rectus abdominis. Traction lines were directed from the center to up and the sides of the pubis and abdominal flaps were pulled to the midline of the abdomen No significant complication was observed, which could compromise the final result. RESULTS: The elevation of the pubis, the correct shape and resection of excess adipocutâneos were achieved in all cases, bringing satisfaction to patients. Conclusion: Considering the pubic deformity after massive weight loss, and the results obtained, the procedures adopted proved to be safe, easily reproducible and stable over the long term.

RESUMO

INTRODUÇÃO: O presente artigo descreve o planejamento cirúrgico, as condutas de pré e pós-operatório, e os resultados do lifting púbico em pacientes ex-obesos, submetidos ou não à Gastroplastia, que apresentavam grande flacidez cutânea e excesso adiposo do púbis. MÉTODO: Duzentos e dezoito pacientes foram submetidos à cirurgia no período de maio de 2005 a agosto de 2012. O Lifting pubiano foi realizado conjuntamente com a Abdominoplastia em âncora. O púbis foi elevado por meio de pontos posicionados 2 a 3 cm acima de sua posição original, na aponeurose dos retos abdominais. Vetores de tração foram direcionados do centro do púbis para cima e para os flancos e os retalhos abdominais foram tracionados para a linha média do abdome. Nenhuma complicação significativa foi observada que pudesse comprometer o resultado final. RESULTADOS: A elevação do púbis, a correção do formato e a ressecção, dos excessos adipocutâneos, foram alcançadas em todos os casos, trazendo satisfação aos pacientes. CONCLUSÃO: Considerando-se a deformidade pubiana após grandes perdas ponderais e os resultados obtidos, os procedimentos adotados mostraram-se seguros, facilmente reproduzíveis e estáveis a longo prazo.

ABSTRACT

INTRODUCTION: The author recalls the classification proposed by Bozola and Psillakis in 1988, and "aesthetic pathologies" abdominal divided into 5 groups identical to initial treatment in this segment body as excess skin, subcutaneous fat trunk, diastasis of the rectus muscles, external obliques, and deployment vertical position of the navel. Proposed model abdomen aesthetically beautiful like "lira" as he had done with elevations and depressions, highlights and shadows, creating relief. Define beauty and possibility of its reproduction. OBJECTIVE: To assess the practicality of their diagnosis and treatments of the proposed classification and evolution of the same in the 27 subsequent years with different tactics from those described initially, keeping unchanged the classification. METHOD: Describes the five treatment groups and percentages of each of the last 200 operated cases. 614 cases were operated from 2001 to 2011, with possible outcomes to obtain. RESULTS: The classification was effective in 99% of the operated cases. CONCLUSION: The classification proposed by Bozola and Psillakis remains unchanged, the treatments included new tactics, now more extensive and intense liposuction (vibroliposuction) plications of the rectus and obliques through tunnels hipsters in areas without major perforating vessels that supply skin and subcutaneous tissue of the abdomen, it refixando aponeurotic wall, reducing seroma and hematoma. And reducing traction and skin resection, with better quality of scars, and results.

RESUMO

INTRODUÇÃO: O autor recorda a classificação proposta por Bozola & Psillakis em 1988, e as "patologias estéticas" abdominais divididas em cinco grupos idênticas à inicial, no tratamento desse segmento corporal, conforme excedente de pele, gorduras subcutâneas do tronco, diastasis dos músculos retos, oblíquos externos, e posição de implantação vertical do umbigo. Propõe modelo de abdome esteticamente bonito semelhante à "lira", como havia feito, com saliências e depressões, brilhos e sombras, criando o relevo. Define beleza e possibilidade de sua reprodução. OBJETIVO Analisar a praticidade de diagnósticos e respectivos tratamentos da classificação proposta, e evolução dos mesmos nos 27 anos subsequentes, com táticas diferentes das descritas inicialmente, mantendo inalterada a classificação. MÉTODO: Descreve o tratamento nos cinco grupos e porcentagens de cada um nos últimos 200 casos operados. Seiscentos e catorze casos foram operados de 2001 a 2011, com os resultados possíveis de se obter. RESULTADOS: A classificação foi efetiva em 99% dos casos operados. CONCLUSÃO: A classificação proposta por Bozola & Psillakis permanece inalterada, os tratamentos incluíram novas táticas, atualmente com mais extensa e intensa lipoaspiração (vibrolipoaspiração), plicaturas dos músculos retos e oblíquos através de túneis descolados em áreas sem vasos perfurantes importantes que irrigam pele e subcutâneo do abdome, refixando-o a parede aponeurótica, reduzindo seromas e hematomas. E redução da tração e ressecção de pele, com melhor qualidade das cicatrizes, e dos resultados.

Evaluation of topic linoleic acid application on random-pattern skin flaps in rats

ABSTRACT

BACKGROUND: Concern about the viability of skin flaps, especially those of random pattern, has been the subject of several studies in plastic surgery. The limiting factor of these flaps is the unpredictability of its microvasculature, which could lead to partial or complete necrosis of the flap. This study tested the role of topical use of linoleic acid on the survival of random skin flaps. This is a polyunsaturated fatty acid, which in wound healing studies showed to act by mediating the inflammatory response, and decreasing the production of nitric oxide, accelerating the repair process. OBJECTIVE: The aim of this project was to evaluate morphometric and histologically the improvement on skin flaps survival in rats treated with topical linoleic acid. METHOD: 24 male Wistar rats randomly divided into 3 groups, the control group or Glycerin (GG, n = 8), Dersani® group (GD, n = 8) and Linoleic Acid group (GL, n = 8). Under general anesthesia, a cranial base modified McFarlane random skin flap was elevated in dorsal region of the rats. RESULTS: The mean percentage of necrosis area among the groups was: GG - 44.49%, GD - GL and 44.72% - 44.20%. It showed no statistically significant differences (p> 0.05). Histological analysis has also not found differences in the degree of necrosis or inflammation. CONCLUSION: In this study, topical application of LA showed no statistically significant differences (P>0.05) on improving the survival area of random skin flap in rats.

RESUMO

INTRODUÇÃO: A preocupação com a viabilidade de retalhos cutâneos, especialmente os randomizados, tem sido motivo de diversas pesquisas na cirurgia plástica. O fator limitante para a utilização desses retalhos é a imprevisibilidade de sua vascularização distal, o que poderá resultar em necrose parcial ou completa do retalho. Neste estudo, foi testado o uso tópico do ácido linoleico na melhoria da sobrevivência dos retalhos de pele. Este é um ácido graxo poli-insaturado, que em estudos sobre cicatrização de feridas mostrou atuar na mediação da resposta inflamatória e na diminuição da produção de óxido nítrico, acelerando o processo de reparo. OBJETIVO O objetivo deste trabalho foi avaliar morfométrica e histologicamente o aumento da viabilidade de retalhos cutâneos randomizados em ratos, tratados com uso tópico do ácido linoleico. MÉTODO: Vinte e quatro ratos Wistar machos divididos aleatoriamente em três grupos: grupo Controle ou Glicerina (GG, n = 8), grupo Dersani® (GD, n = 8) e grupo Ácido Linoleico (GL, n = 8). Sob anestesia geral, foi elevado um retalho randômico modificado de McFarlane na região dorsal dos ratos. RESULTADOS: A média da porcentagem da área de necrose dos retalhos foi: GG - 44,49%, GD - 44,72% e GL - 44,20%. Estas não apresentaram diferenças estatísticas significativas (P>0,05). A análise histológica não evidenciou diferenças significativas no grau de necrose ou processo inflamatório. CONCLUSÃO: Neste estudo, a aplicação tópica do AL não apresentou diferenças estatísticas significativas em aumentar a área de viabilidade do retalho cutâneo randomizado em ratos.

ABSTRACT

BACKGROUND: The Hyperbaric Oxygen Therapy-HOT, is a non-invasive therapeutic method in which patient breaths 100% oxygen (pure oxygen) through masks, while remain in a pressurized chamber to a pressure higher than atmospheric. Its act as an accelerator of recovery process, by increasing the oxygen saturation in the body that allows to speed up cicatrization and engagement to infection. The aim of this study was to determine the epidemiological profile of hyperbaric oxygen therapy patient of Mato Grosso do Sul state. OBJECTIVE: To demonstrate the Epidemiological Profile of patients treated with the aid of hyperbaric oxygen in the State of Mato Grosso do Sul May 2007 to October 2012. METHODS: We conducted a restrospective study of HOT patients admitted to Santa Casa de Campo Grande Hospital from May 2007 to October 2012. RESULTS: Among 600 patients who underwent HOT, there was a predominance for males (71% - 425 patients); Plastic surgery is the medical speciality which most request HOT associated with conventional treatment for their patients (71% of the total patients), and most of these are due to burn injury (82%). Most of the patients were aged between 31 and 60 years old. (the range with highest prevalence were 31 to 60 years). The mean number of hyperbaric sessions among the burn patients was between 6 and 10 sessions (66.19% of burn patients). CONCLUSIONS: The majority of our service to patients requiring hyperbaric oxygen therapy was male because of moderate to severe burns, and the Plastic Surgery specialty that most used this tool as adjuvant treatment.

INTRODUÇÃO: A oxigenoterapia hiperbárica - OHB é uma modalidade terapêutica não invasiva em que o paciente respira oxigênio a 100% (oxigênio puro), com o uso de máscaras, enquanto permanece em uma câmara pressurizada a uma pressão superior à atmosférica. Seu papel é atuar como um acelerador do processo de recuperação, por meio do aumento da saturação de oxigênio no organismo, que permite a aceleração nas cicatrizações e no combate a diversas infecções. OBJETIVO Demostrar o perfil epidemiológico dos pacientes tratados com auxílio da oxigenioterapia hiperbárica no Estado de Mato Grosso do Sul, de maio 2007 a outubro 2012. MÉTODO: Realizou-se estudo retrospectivo dos pacientes admitidos na Santa Casa de Campo Grande, que utilizaram oxigenioterapia hiperbárica, de maio de 2007 a outubro de 2012. RESULTADOS: Dos 600 pacientes que se submeteram à OHB, predominou o sexo masculino (71% - 425 pacientes); A cirurgia plástica foi a especialidade que mais solicitou OHB associada ao tratamento convencional de seus pacientes (71% do total de pacientes), sendo destes a maioria devido à lesão por queimadura (82%). O intervalo de idade de maior prevalência foi dos 31 aos 60 anos. A média do número de sessões de hiperbárica, dentre os pacientes com queimadura, foi entre 6 e 10 (66,19% do total de queimados). CONCLUSÕES: A maioria dos pacientes que necessitavam de oxigenioterapia hiperbárica era do sexo masculino devido a queimaduras moderadas a graves, sendo a Cirurgia Plástica a especialidade que mais utilizou esta ferramenta como tratamento adjuvante.

Scrotal reconstruction using a superomedial thigh flap after Fournier's syndrome

ABSTRACT

INTRODUCTION: Necrotizing fasciitis is a severe infection that consumes the perineal-scrotal skin quickly. Overcome the infection, it is necessary to cover the compromised structures. We present a series of patients suffering from necrotizing fasciitis who underwent scrotal reconstruction using the superior medial thigh flap. METHODS: Seven male patients underwent unilateral or bilateral scrotal reconstruction with superior medial thigh flap. RESULTS: The patients showed satisfactory results as regards coverage testicular and appearance of the donor area. CONCLUSION: The superior medial thigh flap is presented as an excellent choice for scrotal reconstruction by testicular produce good coverage, is easy to perform and improve local aesthetics.

RESUMO

INTRODUÇÃO: Fasceíte necrotizante é uma grave infecção que consome a pele períneo-escrotal rapidamente. Superada a infecção, faz-se necessária a cobertura das estruturas comprometidas. Apresentamos uma série de pacientes acometidos por fasceíte necrotizante que foram submetidos à reconstrução escrotal com a utilização do retalho súpero-medial da coxa. MÉTODO: Sete pacientes do sexo masculino foram submetidos à reconstrução escrotal, uni ou bilateralmente, com o retalho súpero-medial da coxa. RESULTADOS: Os pacientes apresentaram resultados satisfatórios quanto à cobertura testicular e ao aspecto da área doadora. CONCLUSÃO: O retalho súpero-medial da coxa se apresentou como excelente opção para reconstrução escrotal por produzir boa cobertura testicular, ser de fácil execução e melhorar a estética local.

A surgical technique for nail mimicry

ABSTRACT

BACKGROUND: Nails play important roles in helping individuals pick up small objects and protecting the fingertips. However, since nails are located at the distal portion of the fingers, they are susceptible to injuries that can require their amputation. Here we present our technique for nail mimicry that aimed to provide a better aesthetic result after amputation. METHODS: A total of 14 surgical procedures were performed in 10 patients over 5 years. RESULTS: The goal of the surgery was achieved in all cases and no complications were observed. The surgery was repeated in three cases due to unsatisfactory results. CONCLUSIONS: Our technique is easy to perform. Patients were highly satisfied with the results since they no longer had the stigma of an amputated finger with a stump, which improved their quality of life.

RESUMO

INTRODUÇÃO: As unhas cumprem importante papel de auxílio na pinça e proteção das extremidades digitais. Por seu posicionamento distal nos dedos, estão sujeitas a traumas que podem levar a sua amputação. Apresentamos nossa técnica de mimetização ungueal com o objetivo de melhora estética do dedo tratado. MÉTODO: 14 procedimentos cirúrgicos foram realizados em 5 anos, beneficiando 10 pacientes. RESULTADO: em todos os casos o objetivo da cirurgia foi atingido, sem nenhuma complicação observada. Em 3 casos a cirurgia foi repetida, devido ao resultado inicial ser insatisfatório. CONCLUSÃO: é uma técnica de fácil execução e que traz ao paciente um grau de satisfação elevado por retirar o estigma de um coto de amputação com melhor aparência para um convívio social.

RESUMO

INTRODUÇÃO: Avaliar a intensidade e osníveis de sintomas de depressão nos pacientes com feridas crônicas. MÉTODO: Estudo exploratório e analítico. Participaram do estudo 25 pacientes com úlceras venosas, 30 com úlceras por pressão e 18 diabéticos com pé ulcerado. Para mensurar a intensidade de sintomas e o nível de depressão foi utilizado o Inventário de Avaliação de Depressão de Beck. RESULTADOS: Dentre os pacientes com úlcera venosa, sete (28%) apresentaram depressão leve a moderada e três (12%) depressão moderada a grave. Dezesseis (53,3%) pacientes com úlceras por pressão manifestaram depressão leve a moderada, e cinco (16,7%), depressão de moderada a grave. Dez (55,6%) pacientes diabéticos com pé ulcerado apresentaram depressão leve a moderada e, quatro (22,20%)sem depressão ou depressão mínima. CONCLUSÃO: Os autores concluíram que os pacientes do estudo apresentaram entre depressão leve a moderada e grave.

Association of sociodemographic effects and injury to feelings of powerlessness and hope in individuals with venous ulcers

ABSTRACT

OBJECTIVE: This study's objective was to evaluate feelings of powerlessness and hope in patients with venous ulcers and identify sociodemographic characteristics in the studied population. METHODS: A clinical, descriptive, analytical and prospective study. 40 patients with venous ulcers participated. We used 3 instruments: a questionnaire that assessed demographic and clinical data, an Instrument for Measuring feelings of impotence and Herth Hope Index. RESULTS: At Feelings of Impotence Scale patients scored a mean of 34.3 and at Herth Hope Index 27.50, revealing that these individuals feel powerless and hopeless that the wound can heal. Individuals aged between 50 and 59 years had an average of 39.00 at Feelings of Powerlessness Scale (P 0.120). At Herth Hope Index the average was 14.20 (P=0.508). Male, in both scales, had mean changes, with average of 36.00 at Feelings of Powerlessness Scale (P=0.068) and 26.70 as median at Herth Hope Index (P=0.332). Regarding the time of the injury, the average score was 39.00 at Feelings of Impotence Scale (p = 0.251) and 27.10 in Herth Hope Index, showing changes in patients over 1 to 2 years with injury. Patients who presented odor scored a high mean (36.10 /P=0.155) at Feelings of Impotence Scale and low mean at Herth Hope Scale (26.80). CONCLUSION: Results have revealed that the subjects with venous ulcers in this study feel helpless and hopeless about the possibility of wound healing.

OBJETIVO: Este estudo teve como objetivo avaliar o sentimento de impotência e esperança em pacientes com úlcera venosa e identificar características sociodemográficas nesta população. MÉTODO: Estudo clínico, descritivo, analítico, prospectivo. Participaram 40 pacientes com úlcera venosa. Foram utilizados três instrumentos: um questionário que avaliou dados demográficos e clínicos, o Instrumento de Medida dos Sentimentos de Impotência e a Escala de Esperança de Herth. RESULTADOS: Na Escala de Sentimentos de Impotência, os pacientes pontuaram escore médio de 34,3 e na Escala de Esperança de Herth 27,50, revelando sentimentos de impotência e pouca esperança de cicatrização. Os indivíduos entre 50 e 59 anos apresentaram a média total de 39,00, na Escala de Sentimentos de Impotência (p=0,120). Já na Escala de Esperança de Herth, a média foi de 14,20 (p=0,508). O gênero masculino, em ambas as escalas, apresentou média alterada, sendo 36,00 a média da Escala de Sentimentos de Impotência (p=0,068) e 26,70 a mediana na Escala de Esperança de Herth (p=0,332). Quanto ao tempo da lesão, o escore médio foi de 39,00 na Escala de Sentimentos de Impotência (p=0,251) e de 27,10 na Escala de Esperança de Herth, mostrando alteração em pacientes com mais de 1 até 2 anos com a lesão. Os pacientes que apresentaram odor pontuaram o escore médio alto (36,10/p=0,155) na Escala de Sentimentos de Impotência e escore médio baixo na Escala de Esperança de Herth (26,80). CONCLUSÃO: Os resultados permitem afirmar que os indivíduos com úlcera venosa avaliados sentem-se impotentes e sem esperança quanto à cicatrização da lesão.

Model of informed consent in plastic surgery with evaluation and attestation of the transferred information

ABSTRACT

BACKGROUND: Informed consent is an indispensable tool in the doctor-patient relationship. Aims broadly and unrestrictedly clarify the patient about the procedure, which he/she will be submitted, including all stages of treatment and possible complications. As for the professional who will perform the procedure it's importance goes beyond the precise information to the patient, and also relevance in the legal scope for any grievances that may arise from the committed act. However for the purpose of consent is reached, one should do it in a way in which the physician has the conviction that his informations and explanations about all stages of treatment were in fact understood by the patient. Method: In order to confirm the transfer of information for the informed consent in an objective manner the authors have developed a method using a test administered to the patient after the clarification on the procedure. RESULTS: We observed a greater satisfaction by patients regarding explanations because they felt more secure about the treatment's knowledge to be carried out, taking the chance of further clarification if necessary CONCLUSIONS: This model of consent present itself as evidence that the information about the procedure was conveyed unequivocally, ensuring tranquility and safety for the patient and the professional.

RESUMO

INTRODUÇÃO: O consentimento informado é uma ferramenta indispensável na relação médico-paciente. Tem como objetivo o esclarecimento amplo e irrestrito pelo paciente a respeito do procedimento ao qual será submetido, incluindo todas as etapas do tratamento, bem como as possíveis complicações. Já para o profissional que executará o procedimento, sua importância vai além da informação precisa ao paciente, tendo também relevância no escopo jurídico por eventuais insatisfações que possam advir do ato praticado. Contudo, para que o objetivo do consentimento seja atingido, há de se fazê-lo de uma forma na qual o médico tenha convicção de que suas informações e explicações sobre todas as etapas do tratamento foram de fato compreendidas pelo paciente. MÉTODO: Com o objetivo de confirmar a transferência de informação do consentimento informado de forma objetiva, os autores desenvolveram um método que utiliza um teste aplicado ao paciente após os esclarecimentos sobre o procedimento. RESULTADOS: Foi observada satisfação maior pelos pacientes, pois, estes se sentiram mais seguros quanto ao entendimento das explicações realizadas pelo médico, tendo a chance de novos esclarecimentos caso necessário, eliminando a alegação de desconhecimento do ato a ser praticado, inclusive provando de forma objetiva, na possibilidade de demanda judicial, a transferência inequívoca de informação. CONCLUSÃO: Este modelo de consentimento é uma opção mais didática no esclarecimento de dúvidas pelos pacientes, além de eliminar a dúvida do profissional em relação à compreensão que o paciente teve de suas explicações sobre o procedimento a ser realizado.

ABSTRACT

OBJECTIVE: To evaluate the quality of the records of healing performed by nursing records. METHOD: Descriptive study with retrospective documentary analysis. This study was conducted in adult Intensive Care Unit of the Hospital of Samuel Libanius. RESULTS: 65 (65%) of annotations not present in this type of tissue lesion, 85 (85%) had no type of exudate, 100 (100%) indicated no measurement of the wound and in 80 (80%), the appearance of margin and center of the lesion were not recorded. 100 (100%) of the notes were with abbreviation, 59 (59%) had no clarity of notes and 80 (80%) had grammatical errors and language of 80 (80%) contained incorrect errata and technical terminologies. 75 (75%) of nursing prescriptions were not checked and in 54 (54%) of the notes the letter was not legible. CONCLUSION: In this study the authors concluded that the quality of the ratings of nursing care are limited and inadequate.

RESUMO

OBJETIVO: Avaliar a qualidade dos registros dos curativos realizados pela enfermagem em prontuários. MÉTODO: Estudo descritivo, com análise documental retrospectiva. Este estudo foi realizado na Unidade de Terapia Intensiva adulto do Hospital das Clínicas Samuel Libânio. RESULTADOS: Sessenta e cinco (65%) das anotações não descreviam qual o tipo de tecido presente na lesão, 85 (85%) não apresentavam tipo de exsudato, 100 (100%) não indicavam mensuração da ferida, e em 80 (80%) o aspecto da margem e centro da lesão não foram registrados. Todas as (100%) anotações estavam com abreviatura, 59 (59%) das anotações não apresentavam clareza, 80 (80%) apresentavam erros gramaticais da linguagem e 80 (80%) continham terminologias técnicas erratas e incorretas. Setenta e cinco (75%) das prescrições de enfermagem não estavam checadas, e em 54 (54%) das anotações a letra não estava legível. CONCLUSÃO: Por meio deste estudo, os autores concluíram que a qualidade das anotações da assistência de enfermagem são limitadas e inadequadas.

Rhytidoplasty in an 11-year-old patient as adjuvant treatment for post inflammatory elastolysis and cutis laxa - challenges in the treatment

ABSTRACT

Cutis laxa is a rare morbidity, is caracterized as a connective tissue disorder manifested primarily by sagging cut and may affect other organs. The authors report a case of cutis laxa in a patient of 11 years old with the development of a senile fascia with pronounced ritidose perioral, periocular and nasolabial sulcus. Rhytidectomy was performed as an auxiliary treatment. The patient showed a good evolution of the postoperative scars with good quality and remarkable improvement in sagging of the midface and later recurrence of the same part in the follow-up evaluation two years. Patient and family were very pleased with the outcome. The surgical team, however, realized the limitation of the procedure and the need for additional treatment with auxiliaries and the like as peeling.

RESUMO

Cútis laxa é uma morbidade rara, caracterizada por ser uma desordem do tecido conjuntivo que se manifesta principalmente por flacidez cutânea, podendo acometer variavelmente outros órgãos. Os autores relatam um caso de cútis laxa em paciente de 11 anos que desenvolveu face senil com pronunciada ritidose perioral e periocular, bem como acentuação dos sulcos nasogenianos. Foi realizada ritidoplastia como tratamento cirúrgico. Apresentou boa evolução pós-operatória, com cicatrizes de boa qualidade e melhora notável na flacidez do terço médio da face e, posteriormente, recorrência parcial da mesma no acompanhamento tardio de dois anos. Paciente e familiares ficaram muito satisfeitos com o resultado. A equipe cirúrgica, entretanto, percebeu limitação do resultado cirúrgico e necessidade de complementação com tratamentos auxiliares como peelings e similares.

ABSTRACT

Multiple symmetric lipomatosis is an unusual disease of unknown etiology. It is characterized by symmerical accumulation of fat tissue deposits in neck, face, trunk and proximal limbs. It is more common in male adult subjects with a history of alcohol intake. Paliative surgery is the main treatment and it can be performed by conventional open excision or liposuction. Recurrence is common. The objetive of this article is to report two cases of multiple symmetric lipomatosis treated by liposuction in Regional Hospital of Asa Norte, in Brasilia - Federal District.

RESUMO

A lipomatose simétrica múltipla é uma doença incomum, de etiologia desconhecida, caracterizada pelo acúmulo simétrico de depósitos de tecido adiposo em face, pescoço, tronco e região proximal dos membros. É mais frequente em indivíduos adultos do sexo masculino, geralmente com história de etilismo. O tratamento é cirúrgico, com ressecção paliativa por via aberta ou por lipoaspiração. A recidiva é comum. O presente artigo tem por objetivo relatar dois casos de lipomatose simétrica múltipla, tratados por lipoaspiração no Hospital Regional da Asa Norte, em Brasília-DF.

Rupture of a silicone breast implant after firearm injury

ABSTRACT

The rupture of silicone implants for injury due gunshot is rare with only two cases reported, and only one implant was done with gel high-cohesive. A male patient, 25 years transsexual with breast implants silicone, was the victim of gunshot wound. The bullet penetrated the posterior chest and get off at the anterior-medial chest which transfixed the prosthesis. During digital exploration of the wound, still at the surgical emergency, it was found the rupture of the implant, taking place however only the simple suture of the skin. It was performed ultrasonography, computed tomography and magnetic resonance imaging exam, the results showed no perforation of the prosthesis and silicone leakage. Faced with the assertion of the general surgeon that had perforation of the implant, it was indicated the immediate removal of the prosthesis, but the patient refused to be operated by evading the hospital. After two months he had hyperemia and the silicone liked out of the wound. Then he looked for a particular service to exchange the prosthesis. We can conclude that despite of the high specificity of current imaging studies, the artificial high-density silicone may be presented without modification to examinations and their removal is the treatment of choice.

RESUMO

A ruptura do implante de silicone devido à ferimento por arma de fogo é um evento raro, com apenas dois casos descritos na literatura, sendo que somente em um deles o implante era feito com gel de alta coesão. Paciente masculino, 25 anos, transexual, com prótese mamária de silicone, vítima de ferimento por arma de fogo, sendo que o projétil entrou na região dorsal e saiu através da região antero-medial direita do tórax, transfixando a prótese. Durante a exploração digital do ferimento da mama, ainda no centro cirúrgico da emergência, foi constatada a ruptura do implante, realizando-se, no entanto, somente a sutura simples da pele. Realizadas ultrassonografia, tomografia computadorizada e ressonância magnética; nenhum exame demonstrou perfuração da prótese e extravasamento do silicone. Diante da afirmação do cirurgião geral que havia perfuração do implante, foi indicada ao paciente a remoção imediata da prótese, no entanto, o paciente recusou ser operado, deixando o hospital. Após dois meses, evoluiu com hiperemia e saída de silicone pela ferida da mama, quando procurou serviço particular para troca da prótese. Podemos concluir que, apesar da alta especificidade dos atuais exames de imagem, as próteses de alta densidade de silicone podem apresentar-se sem alteração aos exames e sua remoção é o tratamento de escolha.

Delayed reconstruction of the vulvar and lower abdominal regions as a complementary treatment for postsurgical sequelae of bladder exstrophy correction

ABSTRACT

Patients with malformations of the exstrophy-epispadias complex, including bladder exstrophy, may present for correction of deformities and sequelae in abdominal area, after primary treatment of urogenital malformations, performed early in life and in multiple stages. The secondary correction with aesthetic and functional goals is usually performed after growth and definition of hairy and fat distribution as well as after completion of urological treatment. Psychological aspects should also be considered. We report three female patients, with a history of bladder exstrophy correction in the neonatal period, presenting multiple deformities in the abdominal and vulvar areas, treated at our institution.

RESUMO

Os portadores de malformações do complexo extrofia-epispádia, incluindo a extrofia vesical, podem se apresentar para correções de deformidades e sequelas abdominais após o tratamento primário das malformações urogenitais, realizado nos primeiros anos de vida e em múltiplos estágios. A correção secundária, com objetivos estéticos e funcionais, é normalmente realizada após o crescimento e definição da distribuição pilosa e adiposa, bem como após a finalização do tratamento urológico. Os aspectos psicológicos também devem ser considerados. Relatamos uma série de três casos de pacientes do sexo feminino, com antecedente de correção de extrofia vesical no período neonatal, apresentando múltiplas deformidades na região abdominal e vulvar, submetidas à reconstrução em nosso serviço.
